  • Hypothermic treatment for neonatal asphyxia in low-resource settings using phase-changing material : An easy to use and low-cost method

    • Aim: To evaluate whether phase-changing material can be used for therapeutic hypothermia of asphyxiated newborns in low-resource settings.Methods: Prospective interventional study of asphyxiated term infants fulfilling criteria for hypothermia treatment at Vietnam National Children's Hospital from September 2014 to September 2016. Hypothermia was induced within 6 hours after birth and maintained for 72 hours by a phase-changing material mattress with melting point of 32 degrees C. Rectal temperature was continuously measured, and deviations from target temperature range 33.5-34.5 degrees C were recorded.Results: In total 52 infants (mean gestational age 39.3 1.1 weeks) included and cooled, the median temperature at initiation of cooling was 35.3 (IQR 34.5-35.9)degrees C. The median time to reach target temperature was 2.5 (IQR 2-3) hours. The mean temperature during the cooling phase was 33.95 +/- 0.2 degrees C. Throughout the cooling phase, the target temperature range (33.5-34.5 degrees C) was maintained more than 80% of the time. Rate of rewarming was 0.5 +/- 0.14 degrees C/hour.Conclusion: Phase-changing material can be used as an effective cooling method. Though not a servo-controlled system, it is easy to induce hypothermia, maintain target temperature and rewarm infants in a slow and controlled manner without need for frequent changes and minimum risk of skin injury.

  • Capacity of Vitiver grass in treatment of a mixture of labaratory and domestic wastewaters

    •  In this study, laboratory wastewater containing organic matters, heavy metals and aromatic compounds, was treated by vetiver grass (Vetiveria zizanioides) as a phytoremediation method to remove the above three groups of pollutants. Sewage effluent, as a source of nutrient supply for plant growth, was firstly fed to two wetland systems: mini horizontal subsurface flow (HSSF) and floating raft (FR) wetlands. Next, laboratory wastewater was added gradually to mix with sewage. Nominal hydraulic retention time in both wetlands are 12 hours. Pollutants removal efficiencies were monitored. Microbial community change corresponding with each stages of sewage only and mixture with laboratory wastewater was also examined. The examined microbial community includes Nitrogen-fixing (N-fixing) bacteria, Phosphate-solubilizing (P-solubilizing) microorganism, Pseudomonas sp., and Zoogloea sp. In HSSF wetland, base materials (gravel and sand), algae, and vetiver root were in turn investigated for pollutant removal efficiencies. The results reveal that even with the presences of heavy metals and aromatic compounds, vetiver presented reasonable removal efficiencies of about 62%, 68.6%, and 58.3% for BOD, TN, and TP removal, respectively. Base materials showed almost no effect on pollutant removal. Algae was slightly responsible for approximate 6.3%, 16.6%, and 19.7% of BOD, TN, and TP removals, respectively. On the other hand vetiver roots, in term of heavy metals, had an impressive removal efficiencies of 99.2, 95.8, 96.2, and 96.7% of Cr+6 (in K2Cr2O7), Mn (MnSO4), Fe (FeSO4), and Cu (CuSO4), respectively. For aromatic compounds, the wetland is responsible for 96.8 and almost 100% of correspondingly phenol and benzene removal efficiencies. For microbial aspect, N-fixing microorganisms (e.g. Azospirillum sp., Azotobacter sp.) and Phosphate-solubilizing bacteria (Bacillus sp.) increased gradually in population during domestic wastewater feeding stage. When laboratory wastewater was added, N-fixing and P-solubilizing bacteria were quantitatively decreased slightly while population of Pseudomonas sp. increased. Besides, Zoogloea sp. was also found increasing through out the experiment and keeping a stable growth even during laboratory wastewater adding. In FR wetland, both algae and vetiver root were also investigated for BOD and aromatic compounds and heavy metals. The outcomes show similar tendencies in treatment and microbial behaviours as in HSSF wetland. Vetiver grass, mainly responsible for organic matters and nutrients removal, presented slightly lower removal efficiencies than those in HSSF wetland. The average values of removal efficiencies are 59%, 63.5%, and 53.0% for BOD, TN, and TP removal, respectively. Algae, also, took minor responsibility for approximate 3.3%, 9.1%, and 8.9% of BOD, TN, and TP removals, respectively. Heavy metals of Cr+6 (in K2Cr2O7), Mn (MnSO4), Fe (FeSO4), and Cu (CuSO4) were found removing less than in HSSF wetland with average removal efficiencies values of 92.4, 85.1, 91.8, and 91.5%, respectively, by vetiver root. Algae show almost no effect on heavy metals and aromatic removals. The vetiver root likewise plays important role in phenol and benzene removals with values of 91.5 and 96% in efficiency, respectively. N-fixing and P-solubilizing microorganisms, Pseudomonas sp., and Zoogloea sp. presented similar responses tendencies to different living condition when domestic and laboratory wastewaters, in turn, were fed.

  • The globalizability of temporal discounting

    • Economic inequality is associated with preferences for smaller, immediate gains over larger, delayed ones. Such temporal discounting may feed into rising global inequality, yet it is unclear whether it is a function of choice preferences or norms, or rather the absence of sufficient resources for immediate needs. It is also not clear whether these reflect true differences in choice patterns between income groups. We tested temporal discounting and five intertemporal choice anomalies using local currencies and value standards in 61 countries (N = 13,629). Across a diverse sample, we found consistent, robust rates of choice anomalies. Lower-income groups were not significantly different, but economic inequality and broader financial circumstances were clearly correlated with population choice patterns. Ruggeri et al. find in a study of 61 countries that temporal discounting patterns are globally generalizable. Worse financial environments, greater inequality and high inflation are associated with extreme or inconsistent long-term decisions.

  • Therapeutic hypothermia after perinatal asphyxia in Vietnam : medium-term outcomes at 18 months - a prospective cohort study

    • Aim To determine neurodevelopmental outcome at 18 months after therapeutic hypothermia for hypoxic-ischaemic encephalopathy (HIE) infants in Vietnam, a low-middle-income country. Method Prospective cohort study investigating outcomes at 18 months in severely asphyxiated outborn infants who underwent therapeutic hypothermia for HIE in Hanoi, Vietnam, during the time period 2016-2019. Survivors were examined at discharge and at 6 and 18 months by a neonatologist, a neurologist and a rehabilitation physician, who were blinded to the infants' clinical severity during hospitalisation using two assessment tools: the Ages and Stages Questionnaire (ASQ) and the Hammersmith Infant Neurological Examination (HINE), to detect impairments and promote early interventions for those who require it. Results In total, 130 neonates, 85 (65%) with moderate and 45 (35%) with severe HIE, underwent therapeutic hypothermia treatment using phase change material. Forty-three infants (33%) died during hospitalisation and in infancy. Among the 87 survivors, 69 (79%) completed follow-up until 18 months. Nineteen children developed cerebral palsy (8 diplegia, 3 hemiplegia, 8 dyskinetic), and 11 had delayed neurodevelopment. At each time point, infants with a normal or delayed neurodevelopment had significantly higher ASQ and HINE scores (p<0.05) than those with cerebral palsy. Conclusion The rates of mortality and adverse neurodevelopment rate were high and comparable to recently published data from other low-middle-income settings. The ASQ and HINE were useful tools for screening and evaluation of neurodevelopment and neurological function.

  • Complete sequence of a 93.4 kb contig from chromosome 3 of Trypanosoma cruzi containing a strand switch region

    • We have initiated large-scale sequencing of the third smallest chromosome of the CL Brener strain of Trypanosoma cruzi and we report here the complete sequence of a contig consisting of three cosmids. This contig covers 93.4 kb and has been found to contain 20-30 novel genes and several repeat elements, including a novel chromosome 3-specific 400-bp repeat sequence. The intergenic sequences were found to be rich in di- and trinucleotide repeats of varying lengths and also contained several known T. cruzi repeat elements. The sequence contains 29 open reading frames (ORFs) longer than 700 bp, the longest being 5157 bp, and a large number of shorter ORFs. Of the long ORFs, seven show homology to known genes in parasites and other organisms, whereas four ORFs were confirmed by sequencing of cDNA clones. Two shorter ORFs were confirmed by a database homology and a cDNA clone, respectively, and one RNA gene was identified. The identified genes include two copies of the gene for alanine-aminotransferase as well as genes for glucose-6-phosphate isomerase, protein kinases and phosphatases, and an ATP synthase subunit. An interesting feature of the sequence was that the genes appear to be organized in two long clusters containing multiple genes on the same strand. The two clusters are transcribed in opposite directions and they are separated by an approximately 20-kb long, relatively GC-rich sequence, that contains two large repetitive elements as well as a pseudogene for cruzipain and a gene for U2snRNA. It is likely that this strand switch region contains one or more regulatory and promoter regions. The reported sequence provides the first insight into the genome organization of T. cruzi and shows the potential of this approach for rapid identification of novel genes.   • MET Expression and Cancer Stem Cell Networks Impact Outcome in High-Grade Serous Ovarian Cancer

    • Overexpression of the receptor tyrosine kinase MET has been linked to poor survival in several cancer types, and MET has been suggested to interact with stem cell networks. In vitro studies have further suggested a possible benefit of a combined treatment using PARP and MET inhibitors. We used a tissue microarray (TMA) with 130 samples of advanced-stage high-grade serous fallopian tube/ovarian cancer (HGSC) to investigate the prognostic value of MET protein expression alone and in combination with the stem cell factor SOX2. The possible synergistic effects of a PARP and MET inhibitor treatment were evaluated in two cell lines with BRCA1 or BRCA2 deficiency and in their BRCA1/2-proficient counterparts. Patients with tumors positive for MET had worse overall survival (log-rank test, p = 0.015) compared to patients with MET-negative tumors. The prognostic role of MET was even more prominent in the subgroup of patients with SOX2-negative tumors (p = 0.0081). No synergistic effects of the combined treatment with PARP and MET inhibitors were found in the cell lines examined. We conclude that MET expression could be used as a marker for OS in HGSC and that stemness should be taken into consideration when evaluating the mechanisms of this effect.
