  • Sorting and Transforming Program Repair Ingredients via Deep Learning Code Similarities

    • In the field of automated program repair, the redundancy assumption claims large programs contain the seeds of their own repair. However, most redundancy-based program repair techniques do not reason about the repair ingredients-The code that is reused to craft a patch. We aim to reason about the repair ingredients by using code similarities to prioritize and transform statements in a codebase for patch generation. Our approach, DeepRepair, relies on deep learning to reason about code similarities. Code fragments at well-defined levels of granularity in a codebase can be sorted according to their similarity to suspicious elements (i.e., code elements that contain suspicious statements) and statements can be transformed by mapping out-of-scope identifiers to similar identifiers in scope. We examined these new search strategies for patch generation with respect to effectiveness from the viewpoint of a software maintainer. Our comparative experiments were executed on six open-source Java projects including 374 buggy program revisions and consisted of 19,949 trials spanning 2,616 days of computation time. Deep-Repair's search strategy using code similarities generally found compilable ingredients faster than the baseline, jGenProg, but this improvement neither yielded test-Adequate patches in fewer attempts (on average) nor found significantly more patches (on average) than the baseline. Although the patch counts were not statistically different, there were notable differences between the nature of DeepRepair patches and jGenProg patches. The results show that our learning-based approach finds patches that cannot be found by existing redundancy-based repair techniques.

    • This paper presents a novel end-to-end approach to program repair based on sequence-to-sequence learning. We devise, implement, and evaluate a technique, called SEQUENCER, for fixing bugs based on sequence-to-sequence learning on source code. This approach uses the copy mechanism to overcome the unlimited vocabulary problem that occurs with big code. Our system is data-driven; we train it on 35,578 samples, carefully curated from commits to open-source repositories. We evaluate SEQUENCER on 4,711 independent real bug fixes, as well on the Defects4J benchmark used in program repair research. SEQUENCER is able to perfectly predict the fixed line for 950/4,711 testing samples, and find correct patches for 14 bugs in Defects4J benchmark. SEQUENCER captures a wide range of repair operators without any domain-specific top-down design.

  • Neck Surgery for Non-Well Differentiated Thyroid Malignancies: Variations in Strategy According to Histopathology

    • Lymph node metastases in non-well differentiated thyroid cancer (non-WDTC) are common, both in the central compartment (levels VI and VII) and in the lateral neck (Levels II to V). Nodal metastases negatively affect prognosis and should be treated to maximize locoregional control while minimizing morbidity. In non-WDTC, the rate of nodal involvement is variable and depends on the histology of the tumor. For medullary thyroid carcinomas, poorly differentiated thyroid carcinomas, and anaplastic thyroid carcinomas, the high frequency of lymph node metastases makes central compartment dissection generally necessary. In mucoepidermoid carcinomas, malignant peripheral nerve sheath tumors, sarcomas, and malignant thyroid teratomas or thyroblastomas, central compartment dissection is less often necessary, as clinical lymphnode involvement is less common. We aim to summarize the medical literature and the opinions of several experts from different parts of the world on the current philosophy for managing the neck in less common types of thyroid cancer.

    • Constitutional 11q deletion is a chromosome imbalance possibly found in MCA/MR patients analyzed for chromosomal anomalies. Its role in determining the phenotype depends on extension and position of deleted region. Loss of heterozygosity of 11q (region 11q23) is also associated with neuroblastoma, the most frequent extra cranial cancer in children. It represents one of the most frequent cytogenetic abnormalities observed in the tumor of patients with high-risk disease even if germline deletion of 11q in neuroblastoma is rare. Hereby, we describe a 18 months old girl presenting with trigonocephaly and dysmorphic facial features, including hypotelorism, broad depressed nasal bridge, micrognathia, synophrys, epicanthal folds, and with a stage 4 neuroblastoma without MYCN amplification, carrying a germline 11q deletion (11q14.1-q22.3), outside from Jacobsen syndrome and from neuroblastoma 11q critical regions. The role of 11q deletion in determining the clinical phenotype and its association with neuroblastoma development in the patient are discussed. (C) 2013 Elsevier Masson SAS. All rights reserved.

  • Management of Recurrent Well-Differentiated Thyroid Carcinoma in the Neck: A Comprehensive Review

    • Surgery has been historically the preferred primary treatment for patients with well-differentiated thyroid carcinoma and for selected locoregional recurrences. Adjuvant therapy with radioactive iodine is typically recommended for patients with an intermediate to high risk of recurrence. Despite these treatments, locally advanced disease and locoregional relapses are not infrequent. These patients have a prolonged overall survival that may result in long periods of active disease and the possibility of requiring subsequent treatments. Recently, many new options have emerged as salvage therapies. This review offers a comprehensive discussion and considerations regarding surgery, active surveillance, radioactive iodine therapy, ultrasonography-guided percutaneous ablation, external beam radiotherapy, and systemic therapy for well-differentiated thyroid cancer based on relevant publications and current reference guidelines. We feel that the surgical member of the thyroid cancer management team is empowered by being aware and facile with all management options.

    • Thermal and chemical ablation are minimally invasive procedures that avoid removal of the thyroid gland and target symptomatic nodules directly. Internationally, Radiofrequency ablation (RFA) is among one of the most widely used thermal ablative techniques, and is gaining traction in North America. Surgery remains the standard of care for most thyroid cancer, and in the right clinical setting, Active Surveillance (AS) can be a reasonable option for low risk disease. Minimally invasive techniques have emerged as an alternative option for patients deemed high risk for surgery, or for those patients who wish to receive a more active treatment approach compared to AS. Herein, we review the literature on the safety and efficacy of RFA for treating benign non-functioning thyroid nodules, autonomously functioning thyroid nodules, primary small low risk thyroid cancer (namely papillary thyroid cancer) as well as recurrent thyroid cancer.
