  • Rationale and Study Design for an Individualized Perioperative Open Lung Ventilatory Strategy in Patients on One-Lung Ventilation (iPROVE-OLV)

    • Objective: The aim of this clinical trial is to examine whether it is possible to reduce postoperative complications using an individualized perioperative ventilatory strategy versus using a standard lung-protective ventilation strategy in patients scheduled for thoracic surgery requiring one-lung ventilation. Design: International, multicenter, prospective, randomized controlled clinical trial. Setting: A network of university hospitals. Participants: The study comprises 1,380 patients scheduled for thoracic surgery. Interventions: The individualized group will receive intraoperative recruitment maneuvers followed by individualized positive end-expiratory pressure (open lung approach) during the intraoperative period plus postoperative ventilatory support with high-flow nasal cannula, whereas the control group will be managed with conventional lung-protective ventilation. Measurements and Main Results: Individual and total number of postoperative complications, including atelectasis, pneumothorax, pleural effusion, pneumonia, acute lung injury; unplanned readmission and reintubation; length of stay and death in the critical care unit and in the hospital will be analyzed for both groups. The authors hypothesize that the intraoperative application of an open lung approach followed by an individual indication of high-flow nasal cannula in the postoperative period will reduce pulmonary complications and length of hospital stay in high-risk surgical patients.     • The presence of SARS-CoV-2 RNA in wastewater was first reported in March 2020. Over the subsequent months, the potential for wastewater surveillance to contribute to COVID-19 mitigation programmes has been the focus of intense national and international research activities, gaining the attention of policy makers and the public. As a new application of an established methodology, focused collaboration between public health practitioners and wastewater researchers is essential to developing a common understanding on how, when and where the outputs of this non-invasive community-level approach can deliver actionable outcomes for public health authorities. Within this context, the NORMAN SCORE “SARS-CoV-2 in sewage” database provides a platform for rapid, open access data sharing, validated by the uploading of 276 data sets from nine countries to-date. Through offering direct access to underpinning meta-data sets (and describing its use in data interpretation), the NORMAN SCORE database is a resource for the development of recommendations on minimum data requirements for wastewater pathogen surveillance. It is also a tool to engage public health practitioners in discussions on use of the approach, providing an opportunity to build mutual understanding of the demand and supply for data and facilitate the translation of this promising research application into public health practice.

  • Historical reconstruction of Small-scale gold mining activities in tropical wetland sediments in Bajo Cauca-Antioquia, Colombia

    • Global mining activities in Latin America have increased exponentially over the last decade. The present study aims to assess the historical impact of Artisanal and Small-scale Gold Mining activities (ASGM) in the Department of Antioquia, Colombia, a region characterized by increased mining development over the past century. Historical trends of heavy metals (i.e., Ag, Cr, Cu, Hg, Ni, Pb, and Zn) were reconstructed for the past century in a tropical wetland near the mining district. Results indicate that local mining operations did not have a significant influence in the area until the mid-20th century when metal concentrations began to increase and exceeded background values. The significant increase in both sediment accumulation rates and total organic carbon (TOC) content during the 1920s reflects the deforestation of the area due to the diversification of the economy (e.g. coffee cultivation, mining or animal husbandry). Both concentrations and accumulation rates of metals increased exponentially after the 1980s as a consequence of the reactivation of alluvial gold exploitation, reaching values that exceeded up to 2-5 times the background levels. The historical metal trends in sediments from Las Palmas wetland reflected the historical socio-economic development in Antioquia and can be used as a good proxy for evaluating anthropogenic impacts in this region.

  • Health and safety at work : achievements, shortcomings, and policy options

    • This working paper presents a comparative study on the health and safety conditions workers face in two sectors (construction and PHS/domestic work) and concerning the extent to which workers in these sectors can effectively exercise their social rights concerning OSH. Also, it examines how the main EU acquis on occupational health and safety at work has been transposed and applied and what its impact has been in terms of available power resources for workers in the two sectors. It explains the way the respective Directives foster effective social rights for workers through legislation and collective agreements. The study includes seven EU member states, namely, Denmark, France, the Netherlands, Spain, Germany, Poland, and Ireland. We combine legal analysis with document analysis, semi-structured interviews, and descriptive statistics from national sources.According to our research, in general terms, the national transposing legislation of the OSH is in conformity with the studied EU OSH Directives. Our research also shows that, in many of the country cases, compliance with the OSH Directives minimum requirements is better in large establishments than in SMEs. However, there are some cases where the national legislation goes beyond the requirements of the EU framework directive. For example, in certain EU Member States domestic workers are included in the definition of ‘worker’ when transposing the Framework Directive, setting a broader personal scope of application than the Directive. But still, the workers in the two sectors face many health and safety problems and lack sufficient power resources to deal with them. Based on our analysis we present a series of policy recommendations that can help to improve this situation and strengthen the power resources of workers in the two sectors.

  • Free Space Microwave Sensing of Carbon Fiber Composites with Ferromagnetic Microwire Inclusions

    • We provide new experimental results on studies of composites with glass-coated ferromagnetic microwires aligned with the requirements of carbon composites. This work focuses on the free space microwave measurements of composites made from carbon fibers and ferromagnetic microwires inclusion focusing on the electromagnetic properties. We prepared several glass-coated microwires and selected Co-based microwires with optimum soft magnetic properties and high magnetoimpedance effect for composite fabrication. We observed that by using a low frequency modulating AC magnetic field parallel oriented to the ferromagnetic microwires allows us to distinguish the microwave signals originated from the ferromagnetic microwires inclusions from the response generated by the carbon fibers. The location of carbon fibers near magnetic microwires has a critical effect on the response signals (parameters S amplitude) obtained from such composites.
