    • In this paper we present new results for prompt fission γ-ray spectral characteristics from the thermal neutron induced fission of Pu∗240. The measured spectra were unfolded by using the detectors' response functions, simulated with geant4. We obtained in average per fission a γ-ray multiplicity Mγ=(7.35±0.12), a mean photon energy ϵγ=(0.85±0.02) MeV, and an average total energy released in fission Ēγ,tot=(6.27±0.11) MeV. Our results are in good agreement with historical data measured in the 1970s by Verbinski et al. and results from recent calculations in the framework of Monte Carlo Hauser-Feshbach models. Our measured average total energy is slightly smaller than the one deduced previously and present in evaluated data. From this we conclude that the Pu239(nth,f) reaction may be ruled out as possible source of γ heating underestimation, when compared with benchmark calculations based on existing nuclear data.

  • The fission experimental programme at the CERN n_TOF facility : status and perspectives

    • Neutron-induced fission reactions play a crucial role in a variety of fields of fundamental and applied nuclear science. In basic nuclear physics they provide important information on properties of nuclear matter, while in nuclear technology they are at the basis of present and future reactor designs. Finally, there is a renewed interest in fission reactions in nuclear astrophysics due to the multi-messenger observation of neutron star mergers and the important role played by fission recycling in r-process nucleosynthesis. Although studied for several decades, many fundamental questions still remain on fission reactions, while modern applications and the development of more reliable nuclear models require high-accuracy and consistent experimental data on fission cross sections and other fission observables. To address these needs, an extensive fission research programme has been carried out at the n_TOF neutron time-of-flight facility at CERN during the last 18 years, taking advantage of the high energy resolution, high luminosity and wide energy range of the neutron beam, as well as of the detection and data acquisition systems designed for this purpose. While long-lived isotopes are studied on the 185 m long flight-path, the recent construction of a second experimental area at a distance of about 19 m has opened the way to challenging measurements of short-lived actinides. This article provides an overview of the n_TOF experimental programme on neutron-induced fission reactions along with the main characteristics of the facility, the various detection systems and data analysis techniques used. The most important results on several major and minor actinides obtained so far and the future perspectives of fission measurements at n_TOF are presented and discussed.

    • In this paper we present results from the first high-precision prompt-gamma-ray spectral measurements from the reaction Pu-241(n(th), f). Apart from one recent experiment, no data are reported in the literature for this fissioning system, which motivated a new dedicated experiment. We have measured prompt-fission gamma rays with three cerium-doped LaBr3 (two 5.08 cm x 5.08 cm and one 7.62 cm x 7.62 cm) and one CeBr3 (5.08 cm x 5.08 cm) scintillation detectors, which all exhibit excellent timing and good energy resolution. The average gamma-ray multiplicity was determined to be (nu) over bar (gamma) = (8.21 +/- 0.09) per fission, the average energy to be epsilon(gamma) = (0.78 +/- 0.01) MeV, and the total energy to be E-gamma,E-tot = (6.41 +/- 0.06) MeV as the weighted average from all detectors. Since the results from all detectors are in excellent agreement, and the total released gamma energy is modestly higher than the one in the present evaluated nuclear data files, we suspect that the underestimation of the prompt-gamma heating in nuclear reactors is due to fast-neutron-induced fission on U-238 or rather from fission induced by gamma rays from neutron capture in the construction material.

  • Falling through the cracks: Increased vulnerability and limited social assistance for TB patients and their households during COVID-19 in Cape Town, South Africa

    • Amid the COVID-19 crisis, Tuberculosis (TB) patients in South Africa, as elsewhere, faced increased vulnerability due to the consequences of the COVID-19 response such as loss of income, challenges to access diagnostic testing, healthcare services and TB medication. To mitigate the socio-economic impact of the pandemic, especially among the most vulnerable, the South African government expanded social assistance programmes by creating the Social Relief of Distress grant (SRDG), the first grant for unemployed adults in South Africa. Our study investigated how TB patients experienced the COVID-19 pandemic and the ensuing socio-economic fallout, how this affected their health and that of their household, income and coping mechanisms, and access to social assistance. We interviewed 15 TB patients at a health facility in Cape Town and analysed data thematically. To situate our findings, we adapted the United Nations’ conceptual framework on determinants of vulnerability and resilience during or following a shock such as climate shocks or pandemics. We found increased vulnerability among TB patients due to a high exposure and sensitivity to the COVID-19 shock but diminished coping capacity. The loss of income in many households resulted not only in increased food insecurity but also a decreased ability to support others. For the most vulnerable, the loss of social support meant resorting to begging and going hungry, severely affecting their ability to continue treatment. In addition, most participants in the study and especially the most vulnerable, fell through the cracks of the most extensive social assistance programme in Africa as few participants were accessing the special COVID-19 SRDG. Targeted social protection for TB patients with a heightened vulnerability and low coping capacity is urgently needed. TB patients with a heightened vulnerability and low coping capacity should be prioritized for urgent assistance.

  • Provider perspectives of the introduction and implementation of care for drug-resistant tuberculosis patients in district-level facilities in South Africa: a qualitative study

    • Drug-resistant tuberculosis (DR-TB) is a growing concern in many low-income and middle-income countries. Facing rising numbers of DR-TB patients, South Africa (SA) introduced a decentralised model of care for DR-TB in 2011. We aimed to document the introduction and implementation of the new models of care for patients with DR-TB in four provinces (Northern Cape, KwaZulu-Natal, Eastern Cape and Gauteng) in 2015 using mixed methods, including interviews, register reviews and clinical audits. This paper reports on the qualitative component of the study.DesignThis is a qualitative interview study.SettingData were collected in 22 decentralised DR-TB sites, primary healthcare facilities and district hospitals and one provincial central DR-TB hospital.Participants58 healthcare workers (HCWs), facility staff and provincial and district TB coordinators were included in qualitative interviews.ResultsHCWs felt that the introduction of DR-TB care in their facility came with little warning or engagement, creating fear and anxiety. They expressed a need for support from the district and province to guide them through the changes but this support was often lacking. In addition, many respondents expressed feeling isolated and not supported by other healthcare providers which they feel impacts on the quality of the care they provide.ConclusionIntroduction of a new service such as DR-TB care can be difficult and does not always result in the intended outcomes. Improved engagement with front-line providers and addressing the fear and anxiety that may be raised by changes in daily practices should be addressed to ensure successful implementation and prevent negative consequences that can hamper quality of care for patients. Attention should be paid to how the decentralised DR-TB unit can be supported by district management and other healthcare providers.
