  • Tacrolimus Once Daily (ADVAGRAF) Versus Twice Daily (PROGRAF) in De Novo Renal Transplantation : A Randomized Phase III Study

    • This multicenter, 1:1-randomized, parallel-group, noninferiority study compared the efficacy and safety of twice-daily tacrolimus (Tacrolimus BID; Prograf) and once-daily tacrolimus prolonged release (Tacrolimus QD; Advagraf), combined with steroids and low-dose mycophenolate mofetil without antibody induction, in 667 de novo kidney transplant recipients. A double-blind, double-dummy 24-week period was followed by an open extension of up to 12 months posttransplant. Biopsy-proven acute rejection rate at 24 weeks (primary endpoint, per-protocol analysis) was 15.8% for Tacrolimus BID versus 20.4% for Tacrolimus QD (p = 0.182; treatment difference 4.5%, 95% confidence interval-1.8%, 10.9%, just outside the prespecified 10% noninferiority margin). Kaplan-Meier 12-month patient and graft survival rates were 97.5% and 92.8% for Tacrolimus BID and 96.9% and 91.5% for QD. Both treatment groups showed equally well-maintained renal function at 12 months (mean creatinine clearance approximately 67 mL/min) and similar adverse event profiles. Overall results obtained with either Tacrolimus QD or BID, without antibody induction, were good, supporting use of the once-daily formulation as an effective alternative to the established twice-daily formulation.

  • Performance of timber board models for prediction of local bending stiffness and strength-with application on douglas fir sawn timber

    • Efficient utilization of structural timber requires accurate methods for machine strength grad-ing. One of the most accurate methods presented this far is based on data of local fiber orientation on board surfaces, obtained from laser scanning. In this paper, two potential improvements of this method are exam-ined. The first one consists of replacing a model based on simple integration over cross sections of boards for calculation of local bending stiffness by a 3D solid finite element (FE) model from which local bending stiffness is derived. The second improvement concerns replacement of a simple model for the fiber orienta-tion in the interior of board by a more advanced one taking location of pith and growth direction of knots into account. Application of the alternative models on a sample of more than 200 Douglas fir boards, size 40 mm x 100 mm x 3000 mm, cut from large logs, show that each of the evaluated model improvements contributes to improved grading accuracy. When local bending stiffness is calculated utilizing the herein suggested FE model in combination with the improved model of fiber orientation in the interior of boards, a coefficient of determination to bending strength as high as 0.76 is obtained. For comparison, a coefficient of determination of 0.71 is obtain using the simpler original models.

    • Several reports have recently raised the possible significance of the presence of autoantibodies against the beta 1-adrenoceptor in patients with idiopathic dilated cardiomyopathy. An investigation was thus initiated to study the immune response against this receptor at the T-cell and the B-cell level. Using membranes of E coli transfected with the human beta 1-adrenoceptor gene as immunogen, T-helper cells of the immunized mice were stimulated with synthetic peptides derived from the receptor and predicted to be immunogenic to assess the T-cell immunodominant regions of the receptor. Three peptides derived from the second transmembrane region, from the second extracellular loop and from the C-terminal domain were shown to be stimulatory. Synthetic peptides, derived from two domains of the receptor which could be potential targets for autoantibodies, yielded an antibody response after immunization with the free peptides. The peptide derived from the N-terminal region yielded antibodies which recognized the receptor in immunoblot and by immunoprecipitation but they had no functional effect on the receptor. The peptide derived from the second extracellular loop yielded antibodies which recognized the receptor in immunoblot and by immunoprecipitation of the free receptor and which had a pharmacological effect on the receptor. The second extracellular loop thus contains T- and B-cell epitopes which could be involved in the autoimmune process.
