  • The Anorexia Nervosa Genetics Initiative: Study description and sample characteristics of the Australian and New Zealand arm

    • Anorexia nervosa is a severe psychiatric disorder with high mortality rates. While its aetiology is poorly understood, there is evidence of a significant genetic component. The Anorexia Nervosa Genetics Initiative is an international collaboration which aims to understand the genetic basis of the disorder. This paper describes the recruitment and characteristics of the Australasian Anorexia Nervosa Genetics Initiative sample, the largest sample of individuals with anorexia nervosa ever assembled across Australia and New Zealand.Methods:Participants completed an online questionnaire based on the Structured Clinical Interview Diagnostic and Statistical Manual of Mental Disorders (4th ed.; DSM-IV) eating disorders section. Participants who met specified case criteria for lifetime anorexia nervosa were requested to provide a DNA sample for genetic analysis.Results:Overall, the study recruited 3414 Australians and 543 New Zealanders meeting the lifetime anorexia nervosa case criteria by using a variety of conventional and social media recruitment methods. At the time of questionnaire completion, 28% had a body mass index ⩽ 18.5 kg/m2. Fasting and exercise were the most commonly employed methods of weight control, and were associated with the youngest reported ages of onset. At the time of the study, 32% of participants meeting lifetime anorexia nervosa case criteria were under the care of a medical practitioner; those with current body mass index < 18.5 kg/m2were more likely to be currently receiving medical care (56%) than those with current body mass index ⩾ 18.5 kg/m2(23%). Professional treatment for eating disorders was most likely to have been received from general practitioners (45% of study participants), dietitians (42%) and outpatient programmes (42%).Conclusions:This study was effective in assembling the largest community sample of people with lifetime anorexia nervosa in Australia and New Zealand to date. The proportion of people with anorexia nervosa currently receiving medical care, and the most common sources of treatment accessed, indicates the importance of training for general practitioners and dietitians in treating anorexia nervosa.

  • ACA CI observations of the starburst galaxy NGC 253

    • Context. Carbon monoxide (CO) is widely used as a tracer of the molecular gas in almost all types of environments. However, several shortcomings of CO complicate usaging it as H-2 tracer, such as its optical depth effects, the dependence of its abundance on metallicity, or its susceptibility to dissociation in highly irradiated regions. Neutral carbon emission has been proposed to overcome some of these shortcomings and hence to help revealing the limits of CO as a measure of the molecular gas. Aims. We aim to study the general characteristics of the spatially and spectrally resolved carbon line emission in a variety of extragalactic sources and evaluate its potential as complementary H-2 tracer to CO. Methods. We used the Atacama Compact Array to map the [CI](P-3(1)-P-3(0)) line emission in the nearby starburst galaxy NGC 253 at unprecedented angular resolution (similar to 3 ''). This is the first well-resolved interferometric [CI] map of an extragalactic source. Results. We have detected the [CI] line emission at high significance levels along the central disk of NGC 253 and its edges where expanding shells have previously been found in CO. Globally, the distribution of the [CI] line emission strongly resembles that of CO, confirming the results of previous Galactic surveys that [CI] traces the same molecular gas as CO. However, we also identify a significant increase of [CI] line emission with respect to CO in (some of) the outflow or shocked regions of NGC 253, namely the bipolar outflow emerging from the nucleus. A first-order estimate of the [CI] column densities indicates abundances of [CI] that are very similar to the abundance of CO in NGC 253. Interestingly, we find that the [CI] line is marginally optically thick within the disk. Conclusions. The enhancement of the [CI]/CO line ratios (similar to 0.4-0.6) with respect to Galactic values (

  • Can incontinence be cured? A systematic review of cure rates

    • Background: Incontinence constitutes a major health problem affecting millions of people worldwide. The present study aims to assess cure rates from treating urinary (UI) or fecal incontinence (FI) and the number of people who may remain dependent on containment strategies. Methods: Medline, Embase, PsycINFO, Cochrane Central Register of Controlled Trials (CENTRAL), CINAHL, and PEDro were searched from January 2005 to June 2015. Supplementary searches included conference abstracts and trials registers (2013-2015). Included studies had patients >= 18 years with UI or FI, reported treatment cure or success rates, had >= 50 patients treated with any intervention recognized in international guideline algorithms, a followup >= 3 months, and were published from 2005 onwards. Title and abstract screening, full paper screening, data extraction and risk-of-bias assessment were performed independently by two reviewers. Disagreements were resolved through discussion or referral to a third reviewer where necessary. A narrative summary of included studies is presented. Results: Most evidence was found for UI: Surgical interventions for stress UI showed a median cure rate of 82.3% (interquartile range (IQR), 72-89.5%); people with urgency UI were mostly treated using medications ( median cure rate for antimuscarinics = 49%; IQR, 35.6-58%). Pelvic floor muscle training and bulking agents showed lower cure rates for UI. Sacral neuromodulation for FI had a median cure rate of 38.6% (IQR, 35.6-40.6%). Conclusions: Many individuals were not cured and hence may continue to rely on containment. No studies were found assessing success of containment strategies. There was a lack of data in the disabled and in those with neurological diseases, in the elderly and those with cognitive impairment. Surgical interventions were effective for stress UI. Other interventions for UI and FI showed lower cure rates. Many individuals are likely to be reliant on containment strategies.

  • A Total Molecular Gas Mass Census in z~ 2-3 Star-forming Galaxies: Low-J CO Excitation Probes of Galaxies' Evolutionary States

    • We present CO(1-0) observations obtained at the Karl G. Jansky Very Large Array for 14 z similar to 2 galaxies with existing CO(3-2) measurements, including 11 galaxies that. contain active galactic nuclei (AGNs) and three submillimeter galaxies (SMGs). We combine this sample with an additional 15 z similar to 2 galaxies from the literature that have both CO(1-0) and CO(3-2) measurements in order to evaluate differences in CO excitation between SMGs and AGN host galaxies, to. measure the effects of CO excitation on the derived molecular gas properties of these populations, and to look for correlations between the molecular gas excitation and other physical parameters. With our expanded sample of CO(3-2)/CO(1-0) line ratio measurements, we do not find a statistically significant difference in the mean line ratio between SMGs and AGN host galaxies as can be found in the literature; we instead find r(3,1) = 1.03. 0.50 for AGN host galaxies and r(3,1) = 0.78. 0.27 for SMGs (or r(3,1) = 0.90 +/- 0.40 for both populations combined). We also do not measure a statistically significant difference between the distributions of the line ratios for these populations at the p = 0.05 level, although this result is less robust. We find no excitation dependence on the index or offset of the integrated Schmidt-Kennicutt relation for the two CO lines, and we. obtain indices consistent with N = 1 for the various subpopulations. However, including low-z "normal" galaxies increases our best-fit Schmidt-Kennicutt index to N similar to 1.2. While we do not reproduce correlations between the CO line width and luminosity, we do reproduce correlations between CO excitation and star-formation efficiency.

    • We present the results of ALMA spectroscopic follow-up of a z =6.766 Lyα emitting galaxy behind the cluster RX J1347.1-1145. We report the detection of [C ii] 158 μm line fully consistent with the Lyα redshift and with the peak of the optical emission. Given the magnification of μ =5.0 ±0.3, the intrinsic (corrected for lensing) luminosity of the [C ii] line is L [C ii] = 1.4+0.2-0.3 × 107 L⊙, roughly ∼5 times fainter than other detections of z ∼ 7 galaxies. The result indicates that low L [C ii] in z ∼ 7 galaxies compared to the local counterparts might be caused by their low metallicities and/or feedback. The small velocity offset (δv = 20+140-40 Km s-1) between the Lyα and [C ii] line is unusual, and may be indicative of ionizing photons escaping.

    • We present new ALMA observations of the [O III] 88 mu m line and high angular resolution observations of the [C II] 158 mu m line in a normal star forming galaxy at z = 7.1. Previous [C II] observations of this galaxy had detected [C II] emission consistent with the Ly alpha redshift but spatially slightly off set relative to the optical (UV-rest frame) emission. The new [C II] observations reveal that the [C II] emission is partly clumpy and partly diffuse on scales larger than about 1 kpc. [O III] emission is also detected at high significance, off set relative to the optical counterpart in the same direction as the [C II] clumps, but mostly not overlapping with the bulk of the [C II] emission. The off set between different emission components (optical/UV and different far-IR tracers) is similar to that which is observed in much more powerful starbursts at high redshift. We show that the [O III] emitting clump cannot be explained in terms of diffuse gas excited by the UV radiation emitted by the optical galaxy, but it requires excitation by in-situ (slightly dust obscured) star formation, at a rate of about 7 M circle dot yr(-1). Within 20 kpc from the optical galaxy the ALMA data reveal two additional [O III] emitting systems, which must be star forming companions. We discuss that the complex properties revealed by ALMA in the z similar to 7.1 galaxy are consistent with expectations by recent models and cosmological simulations, in which differential dust extinction, differential excitation and different metal enrichment levels, associated with different subsystems assembling a galaxy, are responsible for the various appearance of the system when observed with distinct tracers.

    • Introduction Obstetric anal sphincter injuries (OASI) are severe complications that can cause considerable short- and long-term morbidity. Austria, Canada, Norway, and Sweden have similar socio-economic characteristics, and all four countries have access to national birth registers. In this study, we hypothesized that the incidence of OASI should be very similar for different obstetric scenarios in these four countries. Therefore, the aim was to compare the incidence of OASI in these four countries in primiparous women, with spontaneous or instrumental delivery (vacuum or forceps), and in women with a first vaginal birth after cesarean section (VBAC). Material and methods Aggregated data on 1 933 930 vaginally delivered primiparous women and women with VBAC were retrieved from the birth registers gathered in Austria, Canada, Norway, and Sweden. The annual rate of OASI (ICD-10 codes O70.2-O70.3) was presented as the percentage of women with a spontaneous delivery, vacuum or forceps delivery, and a VBAC during the period 2004-2016. Results The incidence of OASI varied considerably between countries and over time. Canada and Sweden had the highest rates, and Austria and Norway the lowest. In Norway, the rate of OASI decreased consistently for all types of deliveries after introducing a perineal protection program in 2004 (p < 0.001). During vacuum delivery, the incidence of OASI varied between countries from 4.1% to 15.5% across the study period. In Canada and Norway, the rate of OASI after a forceps delivery was similar in 2004 at similar to 20% and with differing trajectories to 24.3% (beta 0.49) and 6.2% (beta -1.15) (trend, all p < 0.001) in 2016. Conclusions This comparative register study suggests that there may be considerable potential for lowering the incidence of OASI. The perineal protection program implemented by Norway has been successful. Each country should critically, without prejudice, analyze their current clinical practices and rate of OASI and consider the best preventive strategy.

  • Factors associated with the responsive behaviours of older adults living in long-term care homes towards staff: a systematic review protocol

    • In the last decade, increasing research interest has been expressed in responsive behaviours of older adults living in long-term care (LTC) homes, including nursing homes and assisted living facilities. Responsive behaviours are not only a sign of underlying unmet needs, but when directed against (towards) paid staff can lead to decreased quality of work life, and may contribute to lower quality of care. In this systematic review, we aim to synthesise empirically based quantitative and qualitative evidence on factors and stakeholder (eg, staff and family members) experiences of factors associated with the responsive behaviours of people living in LTC directed towards staff.Methods and analysisThis study will be a systematic review of published and ‘grey’ literature. Twelve bibliographical databases will be searched, and for each database, we will use appropriate subject headings and keywords that cover two concepts: LTC and responsive behaviour. No publication date or language filter will be used. The title and abstract of each extracted record will be screened, followed by screening of full text of included papers. Then data extraction and quality assessments will be undertaken. Each stage will be completed independently by pairs of authors. For quantitative studies, meta-analysis will be conducted if pooling is possible; otherwise, a critical narrative analysis will be conducted. For qualitative studies, thematic analysis will be conducted. Factors will then be organised at the individual, interpersonal, institutional and larger societal levels. Sensitivity analysis will be conducted to explore the influence of risk of bias and publication bias on the results. Subgroup analysis will be conducted for people who live with dementia and those who do not.Ethics and disseminationEthics approval is not required for this systematic review. The results of this study will be disseminated via peer-reviewed publication and presentation at professional conferences.

  • STAR FORMATION AND GAS KINEMATICS OF QUASAR HOST GALAXIES AT z similar to 6: NEW INSIGHTS FROM ALMA

    • We present Atacama Large Millimeter/submillimeter Array (ALMA) observations of the [C Pi] 158 mu m fine structure line and dust continuum emission from the host galaxies of five redshift 6 quasars. We also report complementary observations of 250 GHz dust continuum and CO (6-5) line emission from the z = 6.00 quasar SDSS J231038.88+185519.7 using the IRAM facilities. The ALMA observations were carried out in the extended array at 0.'' 7 resolution. We have detected the line and dust continuum in all five objects. The derived [C Pi] line luminosities are 1.6 x 10(9) to 8.7 x 10(9) L-circle dot and the [C Pi]-to-FIR luminosity ratios are 2.9-5.1 x 10(-4), which is comparable to the values found in other high-redshift quasar-starburst systems and local ultra-luminous infrared galaxies. The sources are marginally resolved and the intrinsic source sizes (major axis FWHM) are constrained to be 0.'' 3-0.'' 6 (i.e., 1.7-3.5 kpc) for the [C Pi] line emission and 0.'' 2-0.'' 4 (i.e., 1.2-2.3 kpc) for the continuum. These measurements indicate that there is vigorous star formation over the central few kpc in the quasar host galaxies. The ALMA observations also constrain the dynamical properties of the star-forming gas in the nuclear region. The intensity-weighted velocity maps of three sources show clear velocity gradients. Such velocity gradients are consistent with a rotating, gravitationally bound gas component, although they are not uniquely interpreted as such. Under the simplifying assumption of rotation, the implied dynamical masses within the [C Pi]-emitting regions are of order 10(10)-10(11) M-circle dot. Given these estimates, the mass ratios between the supermassive black holes and the spheroidal bulge are an order of magnitude higher than the mean value found in local spheroidal galaxies, which is in agreement with results from previous CO observations of high redshift quasars.
