    • The Swedish salmonella control programme has been very successful in reducing the number of salmonella infections in both humans and animals. However, the costs for the control have increased and it has thus been questioned if the control measures could be relaxed and, if so, what effect this would have on human and animal health. The aim of the present study is to evaluate the expected effects on human health of a relaxation of the Swedish control i.e. a substitution of the present programme with a programme similar to the ones present in Denmark or the Netherlands. Data from the year 2010 was used to illustrate this. It was assumed that the domestic exposure to salmonella would then become the same in Sweden as it was in Denmark or the Netherlands in that year. As official statistics on the number of reported salmonella cases are not comparable across European countries, data from five different sources were used to try to obtain comparable estimates of the domestic salmonella exposure in the three countries. The study shows that the number of reported domestic human salmonella cases in Sweden in 2010 would increase by approximately 900 to 2 400 cases in the Danish scenarios and 6 400 to 8 400 in the Dutch scenarios. Although uncertainty exists, it was concluded that the number of reported domestic salmonella cases would increase substantially in Sweden in case of a relaxation of the current control programme.

  • First identification of Echinococcus multilocularis in rodent intermediate hosts in Sweden

    • Echinococcus multilocularis is a zoonotic tapeworm with a sylvatic lifecycle and an expanding range in Europe. Monitoring efforts following its first identification in 2011 in Sweden have focused on the parasite's definitive host, the red fox (Vulpes vulpes). However, identifying rodent intermediate hosts is important to recognize opportunities for parasite transmission. During 2013-2015, livers from a total of 1566 rodents from four regions in Sweden were examined for E.multilocularis metacestode lesions. Species identity of suspect parasite lesions was confirmed by PCR and sequencing. E.multilocularis positive lesions >6mm in diameter were also examined histologically. One Microtus agrestis out of 187 (0.5%, 95%CI: 0-2.9%), 8/439 (1.8%, 95%CI: 0.8-3.6%) Arvicola amphibius, 0/655 (0%, 95%CI: 0-0.6%) Myodes glareolus, and 0/285 (0%, 95%CI: 0-1.3%) Apodemus spp. contained E.multilocularis metacestode lesions. Presence of protoscoleces was confirmed in the infected M.agrestis and in three of eight infected A.amphibius. Six of the nine positive rodents were captured from the same field. This is the first report of E.multilocularis in intermediate hosts in Sweden. The cluster of positive rodents in one field shows that local parasite prevalence can be high in Sweden despite overall low national prevalence in foxes (<0.1%). The presence of protoscoleces in infected M.agrestis and A.amphibius indicate these species can serve as competent intermediate hosts in Sweden. However, their relative importance for E.multilocularis transmission in the Swedish environment is not yet possible to assess. In contrast, the negative findings in all M.glareolus and Apodemus spp. suggest that these species are of no importance.

  • Support for targeted sampling of red fox (Vulpes vulpes) feces in Sweden: a method to improve the probability of finding Echinococcus multilocularis

    • Background: Localized concentrations of Echinococcus multilocularis eggs from feces of infected red fox (Vulpes vulpes) can create areas of higher transmission risk for rodent hosts and possibly also for humans; therefore, identification of these areas is important. However, in a low prevalence environment, such as Sweden, these areas could be easily overlooked. As part of a project investigating the role of different rodents in the epidemiology of E. multilocularis in Sweden, fox feces were collected seasonally from rodent trapping sites in two regions with known parasite status and in two regions with unknown parasite status, 2013-2015. The aim was to evaluate background contamination in rodent trapping sites from parasite eggs in these regions. To maximize the likelihood of finding fox feces positive for the parasite, fecal collection was focused in habitats with the assumed presence of suitable rodent intermediate hosts (i.e. targeted sampling). Parasite eggs were isolated from feces through sieving-flotation, and parasite species were then confirmed using PCR and sequencing.Results: Most samples were collected in the late winter/early spring and in open fields where both Arvicola amphibius and Microtus agrestis were captured. Fox feces positive for E. multilocularis (41/714) were found within 1-3 field collection sites within each of the four regions. The overall proportion of positive samples was low (<= 5.4%) in three regions, but was significantly higher in one region (22.5%, P < 0.001). There was not a significant difference between seasons or years. Compared to previous national screenings, our sampling strategy identified multiple E. multilocularis positive feces in all four regions, including the two regions with previously unknown parasite status.Conclusions: These results further suggest that the distribution of E. multilocularis is highly aggregated in the environment and provide support for further development of a targeted sampling strategy. Our results show that it was possible to identify new areas of high contamination in low endemic environments. After further elaboration, such a strategy may be particularly useful for countries designing surveillance to document freedom from disease.

  • Water quality modelling: microbial risks associated with manure on pasture and arable land

    • While agricultural activities, such as the application of manure on arable land and animal grazing on pastures, provide economic and environmental benefits, they may also pose microbial risks to water sources. The aim of this paper was to study the microbial fate and transport in an agricultural catchment and recipient water source through further development of the hydrological model HYPE. Hydrological modelling was combined with hydrodynamic modelling to simulate the fate and transport of Salmonella spp., verotoxin-producing Escherichia coli O157:H7 and Cryptosporidium parvum in an agricultural catchment of a drinking water source, Lake Vombsjön, in Sweden. This approach was useful to study the influence of different processes on the pathogen fate and transport, and to interpret the relative changes in the simulated concentrations. Sensitivity analysis indicated that the largest uncertainties in the model were associated with the estimation of pathogen loads, parameterisation of the pathogen processes, and simulation of partitioning between surface runoff and infiltration. The proposed modelling approach is valuable for assessing the relative effect of different risk-reducing interventions.

    • Background: The Swedish salmonella control programme covers the entire production chain, from feed to food. All salmonella serotypes are notifiable. On average, less than 20 cases of salmonella in food-producing animals are reported every year. In some situations, the cases would be expected to cluster geographically. The aim of this study was to illustrate the geographic distribution of the salmonella cases detected in pigs, cattle and sheep. Methods: Data on all herds with pigs, cattle and sheep found to be infected with salmonella during the time period from 1993 to 2010 were obtained from the Swedish Board of Agriculture. Using the ArcGIS software, various maps were produced of infected herds, stratified on animal species as well as salmonella serotype. Based on ocular inspection of all maps, some were collapsed and some used separately. Data were also examined for temporal trends. Results: No geographical clustering was observed for ovine or porcine cases. Cattle herds infected with Salmonella Dublin were mainly located in the southeast region and cattle herds infected with Salmonella Typhimurium in the most southern part of the country. Some seasonal variation was seen in cattle, but available data was not sufficient for further analyses. Conclusions: Analyses of data on salmonella infected herds revealed some spatial and temporal patterns for salmonella in cattle. However, despite using 18 years' of data, the number of infected herds was too low for any useful statistical analyses.

  • Economic effects of introducing alternative Salmonella control strategies in Sweden

    • The objective of the study was to analyse the economic effects of introducing alternative Salmonella control strategies in Sweden. Current control strategies in Denmark and the Netherlands were used as benchmarks. The true number of human Salmonella cases was estimated by reconstructing the reporting pyramids for the various scenarios. Costs were calculated for expected changes in human morbidity (Salmonella and two of its sequelae), for differences in the control programmes and for changes in cattle morbidity. The net effects (benefits minus costs) were negative in all scenarios (€ -5 to -105 million), implying that it would not be cost-effective to introduce alternative control strategies in Sweden. This result was mainly due to an expected increase in the incidence of Salmonella in humans (6035-57108 reported and unreported new cases/year), with expected additional costs of € 5-55 million. Other increased costs were due to expected higher incidences of sequelae (€ 3-49 million) and a higher cattle morbidity (€ 4-8 million). Benefits in terms of lower control costs amounted to € 4-7 million.

    • The effect of random slaughter and meat inspection as a tool to detect or eradicate tuberculosis in large, extensive deer herds in Sweden was evaluated. A computer spreadsheet model based on the Reed-Frost method was developed. Numbers of new infections and of infected deer slaughtered as well as probability of detecting tuberculosis or slaughtering all infected deer in a herd, were simulated. The model predicted that, given a 20% annual slaughter and that disease was introduced with one infected deer, the infection would be detected or eliminated in most herds (90%) after 15 years.

    • Previous investigations suggest that the prevalence of Mycobacterium avium subsp. paratuberculosis (MAP) in Swedish cattle is low and all recent cases have been linked to imported animals. The aim of this study was to evaluate the surveillance system for MAP infection in Swedish cattle and to estimate the probability that the Swedish cattle population is free from this infection. Calculations of surveillance sensitivities and probability of freedom were made using stochastic scenario-tree modelling, which allows inclusion of information from several different sources, of complex surveillance data including results from non-representative sampling, as well as of documentations of differences in risk of being infected. The surveillance components included in the model were: (1) clinical surveillance, (2) fallen stock investigations, (3) the national surveillance programme (mainly beef herds), (4) a survey involving dairy herds and (5) a risk-based survey targeting herds with imported cattle. Previous or current presence of imported animals and participation in the on-going control programme was specified for each tested herd, in order to adjust for differences in risk. Calculations were made for each year from the start of 2005 to the end of 2008, and this formed the basis for a final estimate covering the whole study period and predictions of future probabilities of freedom from MAP. Results show that when applying a design prevalence of one animal in 0.1% of the herds, the probability of freedom at the end of 2008 was 0.63. At the design prevalence of one animal in 0.5% of herds, the estimated probability is >95% and it is demonstrated that the prevalence of MAP in Swedish cattle is below this level or absent. In order to increase the annual surveillance sensitivity in the future and thereby improve the probability of freedom, new surveillance activities or an intensification of current ones are needed.     • Data and experiences in Sweden show that it is possible to combine high productivity in animal production with the restricted use of antibiotics. The major key factors that explain Sweden’s success in preventing AMR are: Swedish veterinary practitioners were aware of the risk of AMR as early as the 1950s, and the need for prudent use of antibiotics was already being discussed in the 1960s. Early establishment of health services and health controls to prevent, control and, when possible, eradicate endemic diseases reduced the need for antibiotics. Access to data on antibiotic sales and AMR made it possible to focus on areas of concern. State veterinary leadership provided legal structures and strategies for cooperation between stakeholders and facilitated the establishment of coordinated animal health services that are industry-led, but supported by the State.
