  • Self-assessed competence and need for further training among registered nurses in somatic hospital wards in Sweden : a cross-sectional survey

    • Background Professional competence and continuous professional development is essential for ensuring high quality and safe nursing care, and it might be important for motivating nurses to stay in the profession. Thus, there is a need to identify the developmental process of nursing competency. Assessment of competence and need for further training helps to identify areas for quality improvement, and to design interventions in order to facilitate continuous competence development in different work contexts. The current study aimed to 1) describe registered nurses' self-assessment of clinical competence as well as the need for further training, and 2) explore possible differences between registered nurses with varying lengths of professional experience as a nurse (<= 0,5 year, > 0,5-5 years, and >= 6 years). Methods A cross-sectional survey design was applied, using the Professional Nurse Self-Assessment Scale of clinical core competencies II. Registered nurses (n = 266) working in medical and surgical contexts in hospitals in Sweden responded (response rate 51%). Independent student t-test and analysis of variance were carried out. Results Registered nurses assessed their competence highest in statements related to cooperation with other health professionals; taking full responsibility for own activities; and acting ethically when caring for patients. They assessed their need for further training most for statements related to assessing patients' health needs by telephone; giving health promotion advice and recommendations to patients by telephone; as well as improving a creative learning environment for staff at the workplace. For self-assessed competence and need for further training, differences between the groups for 35 and 46 items respectively, out of 50 were statistically significant. Conclusions Although the registered nurses assessed their competence high for important competence components expected of professionals such as cooperation with other healthcare professionals, it is problematic that knowledge of interactions and side-effects of different types of medication were reported as having the highest need of training. Longitudinal follow up of newly graduated nurses regarding their continuous development of competence as well as further training is needed.

  • The Professional Nurse Self-Assessment Scale II - Translation and cultural adaptation for Nordic countries

    • BACKGROUND: It is important to map the clinical competence of newly graduated nurses in Nordic countries. The use of a common Nordic instrument could provide insights into nurses' levels of self-assessed clinical competence and perceptions of their need for professional development.AIM: To translate and culturally adapt the original Norwegian version of the Professional Nurse Self-Assessment Scale II (PROFFNurse SAS II) into (1) Danish, (2) Finnish and (3) Icelandic versions.METHOD: The PROFFNurse SAS II was translated and cross-culturally adapted. This translation was inspired by the process used in the Guidelines for Cross-Cultural Adaptation.RESULT: The translation and cultural adaptation processes employed the required steps and provided specific details. In addition, practical issues encountered during the translation process while translating and adapting instruments that may influence future translations were revealed. This study found that having a professional bilingual/bicultural agency translator was partly problematic in the process of translation and found that it is important to adjust the translations to each country's specific words used in nursing.CONCLUSION: Translating the PROFFNurse SAS II instrument into all Nordic languages enables us to use the instrument from a Nordic perspective and across various countries. This is important when comparing self-awareness and reflecting on nurses' clinical competencies. Professional development is central to valuing and developing clinical competence and allowing for the discovery of gaps in clinical competence.

  • A qualitative document analysis of national guidelines in Nordic nursing education using the European Federation of Nurses Associations Competency Framework

    • Initial harmonization has been found within nursing education in the European Union member states, but with a need for the establishment of further, well-defined standards. The aim of this study was to describe and analyze the main content of nursing education expressed in national guidelines in the Nordic countries, seen as comparisons between the countries and to the European Federation of Nurses Associations (EFN) Competency Framework. A qualitative deductive research method and content analysis were applied to analyze 20 documents, using the EFN Competency Framework as a theoretical framework and analysis matrix. The study was performed in line with the Standards for Reporting Qualitative Research (SRQR) checklist. Variations in structure between the included countries was seen and evident uniformity was lacking. There were differences, e.g. the number of European Credit Transfer and Accumulation System (ECTS) credits needed for a degree that needs to be further explored in relation to the quality of education and the competence of newly qualified nurses. There is a question of whether the EU Framework corresponds to the need for nursing competencies for today and the future. Homogenization of guidelines and structures might facilitate further development and deeper collaboration between the Nordic countries, thereby leading to enhanced patient safety and care quality.

  • An international study of Advanced Practice Nursing Students’ clinical competence by means of the PROFFNurse SAS : a professional nurse self-assessment scale of clinical core competencies

    • Background: Various educational programs in advanced practice nursing have been developed throughout the world during the last decades to raise registered nurses’ clinical competence, thereby improving nurses’ competence for independent nursing roles. The “Professional Nurse Self-Assessment Scale” instrument has been developed to determine the core competences of advanced practice nurses (APNs), in accordance with the ICN’s definition of advanced practice nursing. A central question is whether such educational programs develop the required core competencies.Aim: To describe APN students’ perception of competence and perception of need for further training/learning by means of the PROFFNurse SAS instrument.Methods: Approximately 50 APN students from Norway, Sweden, Finland, Iceland, the Netherlands and Great Britain will be asked to participate and answer the PROFFNurse SAS questionnaire (through Questback) from October 2013 to April 2014.  Data will be collected during the last study year of the students’ study program. IBM SPSS Statistics 20 for Windows will be used for statistical analyses.Results: Descriptive statistics of the students’ self-assessment of clinical competence and need for more training/learning will be presented during the conference. Differences between countries and between NP and CNS programs will be analyzed.Conclusions: The results of the study will provide an overview of APN students’ self-assessed clinical competence and possible gaps in the APN education programs will be identified.Intended learning outcomes:Understand core APN competencies through the PROFFNurse SAS instrument;Discern whether differences in NP and CNS educational programs exist;Recognize the need for assessing education programs from students’ perspectives.

  • Bachelor of nursing students' attitudes toward teamwork in healthcare : The impact of implementing a teamSTEPPS® team training program — A longitudinal, quasi-experimental study

    • Background: Teamwork skills are essential to the quality of care and patient safety; nevertheless, team training is limited in Bachelor of Nursing degree programs in Norway. Objectives: The objective of this study was to explore the impact of implementing a TeamSTEPPS® team training intervention on Bachelor of Nursing students' attitudes toward teamwork in health care. Design: A longitudinal quasi-experimental design with pre- and posttests was used. Settings: One intervention group and one control group were recruited from two campuses at a Norwegian university offering a Bachelor of Nursing degree. Participants: Subjects were recruited from a population of 423 students. Methods: For 26 months, the intervention group was exposed to the TeamSTEPPS® team training program with various learning activities to enhance teamwork skills. The intervention group and the control group responded to the Norwegian version of the TeamSTEPPS® Teamwork Attitude Questionnaire (T-TAQ) before the intervention (T0), after ten months (T1), and after 24 months (T2). The students participated in survey T0 and T1 was defined as Sample 1 and students participated in survey T0 and T2 was defined as Sample 2 The data were analyzed with parametric and nonparametric statistics. Results: At T0 there was a significant difference between the intervention and control group. The intervention group showed a significant positive change in the Total T-TAQ score from T0 to T1 and from T0 to T2. The change in mean score differed significantly between the intervention and control group in favor of the intervention group. Conclusions: This study showed that a team training program improved Bachelor of Nursing students' attitudes toward teamwork. Therefore, we recommend that the TeamSTEPPS® team training program be implemented in Bachelor of Nursing programs to facilitate a culture of teamwork.

    • Aims: To describe nursing students' experiences of a TeamSTEPPS® longitudinal team training program and the application of teamwork skills in clinical practice. Design: A descriptive qualitative design. Methods: Overall, 22 nursing students participated in six online focus group interviews after attending a TeamSTEPPS® team training program from their first semester. The data were audio-recorded, transcribed and analysed using inductive content analysis and reported following the COREQ guidelines. The focus group interviews took place in the students' fifth's semester. Results: The main category “Learning teamwork is not an event; it's a journey” emerged from 3 generic categories and 12 subcategories. The participants reported that grasping the relevance of team training and the use of teamwork skills takes time. Utilizing these skills improved their awareness of being a team member and facilitated learning. Conclusion: Team training raised the participants' awareness of teamwork as an essential component of being a professional nurse. Additionally, understanding the complexity of teamwork takes time.

  • Reliability and structural validity of the Norwegian version of the TeamSTEPPS Teamwork Attitudes Questionnaire : A cross-sectional study among Bachelor of Nursing students

    • Aim: To test the reliability and structural validity of the Norwegian version of the TeamSTEPPS(R) Teamwork Attitudes Questionnaire (T-TAQ) among Bachelor of Nursing students. Design: Cross-sectional study. Methods: Bachelor of Nursing students (N = 1,624) at three campuses in different regions of Norway were invited to complete the survey. The data were analysed with descriptive statistics, Cronbach's alpha and confirmatory factor analysis (CFA). Three models were tested. Model 3 was a post hoc modification with a correlation between four negatively worded items. The data was collected in September 2018 and May-June 2019. Results: A total of 509 students were included in the study. Cronbach's alpha ranged from 0.44-0.70 for the dimensions and was 0.79 for the total questionnaire. The fit indexes of model 3 were as follows: RMSEA = 0.043, chi-square = 724.3 (p < .000), normed chi-square = 1.862, TLI = 0.812 and CFI = 0.832. The questionnaire shows some potential to display attitudes towards teamwork in health care among Bachelor of Nursing students. Low Cronbach's alpha in the dimensions might indicate that the questionnaire should be considered used as a unidimensional questionnaire.

    • In Europe, there are an increasing number of persons suffering from depression, which also affectsmany relatives. The burden and health when being the relative of an inpatient suffering fromsevere depression has been less examined. The aim of the study was to describe burden, healthand sense of coherence among relatives of inpatients with severe depression. Furthermore, theaim was to investigate relatives’ burden in relation to their health and sense of coherence. Across-sectional design was performed, with a questionnaire consisting of background questionsand three instruments; Burden Assessment Scale, General Health Questionnaire and Sense of Coherencescale. The participants consisted of 68 relatives recruited from a sample of inpatients diagnosedwith depression in the psychiatric specialist health services in one hospital trust in Norway.The Regional Committee for Medical and Health Research Ethics, Norway South East, gaveapproval to the study. The relatives reported burden to a various degree, with some reporting asignificantly greater burden, poorer health and a weaker sense of coherence than others. With regardto subjective burden eight out of ten relatives reported “Worry about future”, and almost sixout of ten were “Upset by change in patient”. Regarding objective burden, more than half the relativesreported having “Less time for friends” and “Reduced leisure time”. In conclusion the relativeswith a high level of burden reported more mental distress, poorer health and weaker senseof coherence than those with lower level of burden. Further research should focus on identificationof factors predicting burden and health of relatives of inpatients with severe depression.
