  • Clinical Course Content As A Dynamic Variable In Supervision Of Medical Students

    • BackgroundDuring clinical supervision, medical students are expected to gain experience through clinical work, with the support of their clinical supervisor. What each supervisor chooses to emphasize and considers important will have a decisive impact on students’ understanding of what is content necessary to master in order to gain clinical skills. Therefore, in this study, the focus of attention is on what supervisors choose to emphasise during clinical supervision of fourth year medical students during a surgical course. MethodAn ethnographic approach was used, including a selective intermittent time mode, where observation and informal interviews were conducted. Twelve supervisors and nine medical students at a teaching hospital in Sweden participated. Field notes were made during observation; these were transcribed and analysed qualitatively. ResultsThe analysis resulted in six topic areas describing what was emphasized during supervision. The topic areas were: 1) Identifying, collecting and combining information, 2) Problem-solving and decision-making, 3) Handling treatment of disease, 4) Practical skills and illustration of technical equipment, 5) Communicating with patients, and 6) Handling organisational demands.ConclusionsThe findings of this study show the existence of several areas that are focused on in supervision. In authentic clinical situations, these topic areas were intertwined and overlapped and often appeared simultaneously. The clinical situations were adjusted neither to the students’ clinical experience nor to the needs of the students. Consequently, the students may find it difficult to determine what to learn and what to achieve during supervision. They may also find it difficult to understand the situations in the same way as their supervisors, because students focus on handling situations with a more theoretical and declarative approach to a larger extent than do their supervisors. The students therefore need supervisor support to develop and integrate theoretical knowledge. One conclusion that can be made is that supervisor awareness of students’ understanding is of crucial significance for the effective supervision. Regarding the nature of the content chosen in supervised situations, research in other settings and specialties would be required to map and to determine a more general theory of what is focused during medical supervision.

    • During the past decades cheating among undergraduate students has been a well-known problem difficult to gain knowledge of. European research in this field of research is scarce. The aim of this paper is to present a study, investigating the frequency of cheating, the cheating methods used and the students’ motives for cheating or not cheating in a Swedish-Finnish university context. Comparisons with other higher education contexts were possible since an anonymous questionnaire, worked out and used by Newstead, Franklyn-Stokes and Armstead (1995), was translated into Swedish and used in the study. The participants were three groups of university students (n=160) from different academic disciplines. The findings implicate that cheating among undergraduates is common and mainly is a problem of ethic character. The paper also discusses consequences of student cheating for the university staff, legislators, and society. Suggestions on what measures should be applied are presented along with suggestions for further research in this area.

  • The oldest old and their children : how strong are the relationships?

    • Eighty-five-year-old parents (N = 99) and their middle-aged children (N = 58) participated in a study on solidarity in intergenerational relationships in Sweden. The research model utilized evaluated six dimensions of family solidarity: family structures and associational, affectional, consensual, functional and normative solidarity. The finding of close family bonds in most of these areas may be attributed to the fact that the extensive Swedish public home-help system facilitated relationships on more equal terms. While the children were relieved from much responsibility for their parents' needs, most parents did not experience feelings of dependency on their children. It was in fact evident that even very old parents can be a source for help to their children. Children were, however, the main providers for emotional support and this role could not be substituted by formal caregivers.

    • A qualitative method was used to study the life experiences and the adjustment to very old age of a non-institutionalized population of 129 85-year-olds living in Gothenburg, Sweden. In-depth interviews were analyzed and categorized and seven patterns of adjustment emerged: Self-Realizing, Mature Aging, Adapting, Dependent, Resignedly Accepting, Despairing and Withdrawing. Additionally, a coping strategy—ritualization of time and occupations—was revealed in the interviews. These results reflect the heterogeneity among this age group and indicate that the subjective reality should be seriously considered both in further research and in the care of the elderly.

    • The aim of the present study was to investigate how dependence was experienced by 129 non-institutionalized 85-year-old persons, who previously had been categorized into seven different patterns of adjustment. The participants' subjective experiences of dependence were expressed in in-depth interviews and additionally, mobility, self-maintenance and need of help in daily living was conventionally assessed. When the results from these two methods were compared, it was found that subjectively experienced dependence did not generally correspond with the scorings on dependence. The in-depth interviews revealed that subjects with poorer adjustment could be most affected by minor impairments, while physical impairments were of a subordinate importance to those who were better adjusted. For appropriate caring interventions, it is therefore suggested that conventionally assessed dependence needs to be supplemented with documented subjective experiences of dependence.
