    • Analysis of transaction costs in multiple land use situations is helpful in policymaking and land use management, especially in natural resource management situations where interdependence prevails. By using reindeer herding–forestry land use management as an example, the aim of this study is to analyze transaction costs among stakeholders in a comanagement situation. The results demonstrate that a key variable driving transaction costs is the presence of a “land use plan for reindeer husbandry,” which is an interesting paradox as reindeer herders pursue the development of these land use plans even though this drives their transaction costs.(JEL D7, Q23)

    • With complex common pool resources, it is important to balance the multitude of interests in order to generate a sustainable management regime. This is not the case in the northern parts of Sweden, where forest resources are used for different extractive purposes by forest companies and the reindeer herding industry. In many respects, the present situation represents a classic collective-action problem with a number of reasons why no cooperative behaviour might be expected. This article illuminates the relationship between the two industries in an historical, ecological and institutional perspective in order to explain the limited scope of coordinated action between the two actors. It also, through the use of collaborative learning techniques and scenario methods, explores the possibilities for the two industries to consider each other's needs and to identify strategies for co-existence and co-management. The testing of a broad range of scenarios among a selected group of stakeholders leads to the identification of possibilities for improving the management of the forest and lichen resource by changing institutional arrangements and improving coordination between the stakeholders.

    • The forestry and reindeer herding sectors utilize the same land in northern Sweden, and adversely affect each other's productivity. The common pool resource character of this situation has made it difficult to find ways to resolve conflicts that could threaten the two sectors' continued co-existence. A consultation procedure that was introduced to reduce conflicts does not appear to be effective, since conflicts between the two actors still occur. One reason for this failure might be found in the power distribution between forestry and reindeer herding. Earlier research has shown that a co-management system in which the allocation of power between the stakeholders is uneven is difficult to maintain in the long term. However, it is unclear just how uneven the power distribution is between the two actors in this case, and the consequences the disparity might have for the viability and stability of the management system. Focusing on the power relations within the consultation procedures, this paper explores the potential of the present institutional system to take the different interests of the stakeholders into account and to use the consultation procedures as tools for co-managing the forest resources in northern Sweden.

    • This paper describes the development and implementation of a method to measure the transaction costs in situations of multiple land-use, where the actions of one actor have negative effects on the other and vice versa (i.e., where the two actors' usage patterns are interdependent). In situations where more than one agent is using a limited resource, transaction costs arise, which may affect the governance of that resource. For example, in northern Sweden the forestry and reindeer husbandry sectors incur transaction costs during consultations over land-use management. The analysis presented here indicates that the transaction costs incurred by the forestry industry are mainly affected by the number of consultations held, and that the industry's costs could be reduced if more of the reindeer-herding communities drew up formal land-use plans (RBP). The availability of these RBP affected the behaviour of the forestry companies' agents. For example, they were more likely to draw up detailed plans prior to consultations when a RBP was available. The method used to measure and model transaction costs may be useful in other situations involving multiple interdependent actors competing for the use of a common resource, especially in cases involving co-management.

    • Interdependence, that is, when the choice of one actor influences that of another, has relevance for the success of an institutional arrangement. Transaction cost theory can help to explain responses to governance solutions and problems associated with governance and policy of such arrangements. This paper focuses on the parallel use of the forest resources by the forestry industry and indigenous groups practising reindeer husbandry in Northern Sweden as an example of such interdependence. A theoretical model of governance of interdependence and the effect of transaction costs is developed in the paper to meet the needs of being able to evaluate the effect of interdependence in a multiple-use situation. The model is then tested on the empirical example of forestry and reindeer husbandry, using scenario and focus-group methods, to discuss the effect of transaction costs in the present hybrid institution system. This institution is intended to address the interdependence between the actors and manage conflicts, but currently fails to reduce conflict. Next, the model is used to evaluate possible changes; the results of the analysis show that modifying the institutional arrangement can bring about improvements that would have some redistributive effects in terms of transaction costs, lessening the impact of interdependence.

  • Assessing Radiographic Response to 223Ra with an Automated Bone Scan Index in Metastatic Castration-Resistant Prostate Cancer Patients

    • For effective clinical management of patients being treated with 223Ra, there is a need for radiographic response biomarkers to minimize disease progression and to stratify patients for subsequent treatment options. The objective of this study was to evaluate an automated bone scan index (aBSI) as a quantitative assessment of bone scans for radiographic response in patients with metastatic castration-resistant prostate cancer (mCRPC). Methods: In a multicenter retrospective study, bone scans from patients with mCRPC treated with monthly injections of 223Ra were collected from 7 hospitals in Sweden. Patients with available bone scans before treatment with 223Ra and at treatment discontinuation were eligible for the study. The aBSI was generated at baseline and at treatment discontinuation. The Spearman rank correlation was used to correlate aBSI with the baseline covariates: alkaline phosphatase (ALP) and prostate-specific antigen (PSA). The Cox proportional-hazards model and Kaplan-Meier curve were used to evaluate the association of covariates at baseline and their change at treatment discontinuation with overall survival (OS). The concordance index (C-index) was used to evaluate the discriminating strength of covariates in predicting OS. Results: Bone scan images at baseline were available from 156 patients, and 67 patients had both a baseline and a treatment discontinuation bone scan (median, 5 doses; interquartile range, 3-6 doses). Baseline aBSI (median, 4.5; interquartile range, 2.4-6.5) was moderately correlated with ALP (r = 0.60, P < 0.0001) and with PSA (r = 0.38, P = 0.003). Among baseline covariates, aBSI (P = 0.01) and ALP (P = 0.001) were significantly associated with OS, whereas PSA values were not (P = 0.059). After treatment discontinuation, 36% (24/67), 80% (54/67), and 13% (9/67) of patients demonstrated a decline in aBSI, ALP, and PSA, respectively. As a continuous variable, the relative change in aBSI after treatment, compared with baseline, was significantly associated with OS (P < 0.0001), with a C-index of 0.67. Median OS in patients with both aBSI and ALP decline (median, 134 wk) was significantly longer than in patients with ALP decline only (median, 77 wk; P = 0.029). Conclusion: Both aBSI at baseline and its change at treatment discontinuation were significant parameters associated with OS. The study warrants prospective validation of aBSI as a quantitative imaging response biomarker to predict OS in patients with mCRPC treated with 223Ra.

  • Cabazitaxel followed by androgen deprivation therapy (ADT) significantly improves time to progression in patients with newly diagnosed metastatic hormone sensitive prostate cancer (mHSPC) : A randomized, open label, phase III, multicenter trial

    • Background: Patients with newly diagnosed mHSPC have a poor prognosis with a 3-year overall survival (OS) rate of 50%. Recently, combination of docetaxel (75mg/m2 every 6 weeks for 6 cycles) with ADT has become a new standard for such patients, based on results of 2 large phase 3 trials showing a significant OS benefit. In these trials, docetaxel was initiated within 3 months after ADT start. Timing of ADT and chemotherapy (CT) is controversial. In breast cancer, endocrine therapy is always started after CT, the rational being that ADT will turn clones of tumor cells in to a stage of dormancy where CT is less effective.Methods: This phase 3 trial randomized newly diagnosed mHSPC patients to receive cabazitaxel (CABA), 25 mg/m2 every 3 weeks for 10 cycles, followed by ADT (immediately after last CABA cycle) versus ADT alone. Primary end-point was OS. Secondary end-point was progression free survival. The study planned to include 400 patients but was closed prematurely due to low inclusion rate. A total 31 patients with newly diagnosed mHSPC were included and here we present the results.Results: Median follow up was 31 month. Of the CABA treated patients, 66.8% got six cycles or more and 46.7% completed all 10 courses. Median OS was 32,5 months with CABA followed by ADT and 29,5 months with ADT alone (HR 1.43, 95% CI 0.38-5.38). Median progression free survival was significantly longer in CABA treated patients (29 vs 12 months, HR 3.96 (95% CI 1.49-10.49). Main grade ≥ 3 toxicities were neutropenia (66%).Conclusions: In conclusion, results from this prematurely terminated trial suggest that CABA followed by ADT is effective in newly diagnosed mHSPC and shows a manageable toxicity. These results have to be validated in larger randomized trials.
