  • Hormonal Contraceptive Use and Risk of Depression Among Young Women With Attention-Deficit/Hyperactivity Disorder

    • Objective: Women with attention-deficit/hyperactivity disorder (ADHD) have an increased risk of becoming teenage mothers. Adverse effects of hormonal contraception (HC), including depression, may affect adherence to user-dependent contraception and increase the risk for unplanned pregnancies and teenage births in women with ADHD. The current study analyzed whether girls and young women with ADHD are at increased risk for depression during HC use compared with women without ADHD.Method: A linkage of Swedish national registers covering 29,767 girls and young women with ADHD aged 15 to 24 years and 763,146 without ADHD provided measures of ADHD and depression diagnoses (International Classification of Diseases [ICD] code) and prescription of stimulant medication, HC, and antidepressant medication (Anatomical Therapeutic Chemical [ATC] code). Cox regression models applying an interaction term (ADHD diagnosis x HC use) evaluated the excess risk of HC-induced depression in women with ADHD.Results: Women with ADHD had a 3-fold higher risk of developing depression, irrespective of HC use (adjusted hazard ratio [aHR] = 3.69, 95% CI = 3.60-3.78). Oral combined HC users with ADHD had a 5 times higher risk of depression compared with women without ADHD who were not using oral combined HC (aHR = 5.19, 95% CI = 4.94-5.47), anda 6 times higher risk in comparison with women without ADHD who were on oral combined HC (aHR = 6.10 (95% CI = 5.79-6.43). The corresponding risk of depression in women with ADHD who used a progestogen-only pill (aHR = 5.00, 95% CI = 4.56-5.49). The risk of developing depression when using non-oral HC was similarly moderately increased in both groups.Conclusion: Girls and young women with ADHD have an increased risk of developing depression when using oral HC compared with their un-affected peers. Information on risks with HCs as well as potential benefits with long-acting reversible contraceptives needs to be an integrated part of the shared decision making and contraception counseling for young women with ADHD.

  • Anemia and iron deficiency in inflammatory bowel disease: an open, prospective, observational study on diagnosis, treatment with ferric carboxymaltose and quality of life

    • Objective. Iron deficiency and anemia are being increasingly recognized as a complication of inflammatory bowel disease (IBD). The aim of this study was to observe, in a non-interventional way, how Swedish gastroenterologists adhere to guidelines in IBD outpatients treated with intravenous ferric carboxymaltose (FCM), and the result of treatment. Material and methods. Altogether 394 IBD patients (Crohns disease (CD) 60%, ulcerative colitis (UC) 40%) from 14 centers were included. Group A (n = 216) was observed from November 2008 and group B (n = 178) from March 2010. Time of observation ranged from 12 to 29 months. Results. S-Ferritin (mmol/l) and transferrin saturation (T-Sat; %) were recorded at baseline in 62% and 50% in group A. Median values for Hb, ferritin and T-Sat at baseline were 111 g/l, 10 mu mol/l and 10%, respectively, and 134 g/l, 121 mmol/l and 20% after iron treatment (p andlt; 0.001 for all three parameters). Similar results were found in group B. Approximately three-quarters of all patients had only one iron infusion during the study period. Median time to reinfusion was 6 (1-25) months. Only previously described infusion reactions occurred in 27 (7%) patients. Conclusions. Adherence to European guidelines was rather poor and needs to be improved. The effect on iron parameters of intravenous FCM was significant, and resulted in a ferritin level that indicates an effect on the iron stores. The effect was mostly sustained for a year since only one-quarter of the patients were given repeated iron infusions. No unforeseen safety concerns emerged during the observation period.

  • Choosing small sets of policy-relevant scenarios by combining vulnerability and diversity approaches

    • Computer simulation models can generate large numbers of scenarios, far more than can be effectively utilized in most decision support applications. How can one best select a small number of scenarios to consider? One approach calls for choosing scenarios that illuminate vulnerabilities of proposed policies. Another calls for choosing scenarios that span a diverse range of futures. This paper joins these two approaches for the first time, proposing an optimization-based method for choosing a small number of relevant scenarios that combine both vulnerability and diversity. The paper applies the method to a real case involving climate resilient infrastructure for three African river basins (Volta, Orange and Zambezi). Introducing selection criteria in a stepwise manner helps examine how different criteria influence the choice of scenarios. The results suggest that combining vulnerability- and diversity-based criteria can provide a systematic and transparent method for scenario selection.

    • Co-evolutionary scenarios are used for creative prototyping with the purpose of assessing potential implications of future autonomous robot systems on civil protection. The methodology is based on a co-evolutionary scenario approach and the development of different evolutionary paths. Opportunities, threats and ethical aspects in connection with the introduction of robotics in the domestic security and safety sector are identified using an iterative participatory workshop methodology. Three creative prototypes of robotic systems are described: "RoboMall", "RoboButler" and "SnakeSquad". The debate in society that might follow the introduction of these three robot systems and society's response to the experienced ethical problems and opportunities are discussed in the context of two scenarios of different future societies.

    • Handling uncertainties is a major challenge in climate change adaptation. A variety of robust decision support approaches that aim for better management of uncertainty have recently been emerging and are used in environmental planning. The present study examined to what extent existing processes of planning for future sea-level rise in Sweden utilised similar approaches. Three core principles of robust decision support approaches were identified and used as a tool for analyzing five cases of planning for future sea-level rise in companies and authorities at different levels in society. The results show that planning processes typically do not embrace uncertainties, do not use a bottom-up approach and do not specifically aim for robustness, which points to a discrepancy between current planning paradigms and the core principles of robust decision support approaches.

  • Insights from Testing a Modified Dynamic Adaptive Policy Pathways Approach for Spatial Planning at the Municipal Level

    • The Dynamic Adaptive Policy Pathways (DAPP) approach has successfully been used to manage uncertainties in large infrastructure projects. However, the viability of the DAPP approach for spatial planning in smaller municipal settings is not clear. This paper examines opportunities and constraints of using adaptive pathways approaches to help small municipalities plan for future sea-level rise. The methodology was based on developing a simplified DAPP-approach, which was tested in a multiple experimental case study of spatial planning projects in three municipalities in Sweden. The results show that the approach promoted vulnerability-based thinking among the end-users and generated new ideas on how to manage the uncertain long-term impacts of future sea-level rise. However, the increased understanding of uncertainties was used to justify static, rather than adaptive, solutions. This somewhat surprising outcome can be explained by perceived legal constraints, lack of experience of adaptive pathways, and unwillingness to prescribe actions that could prove difficult to enforce in the future. More research is needed to further understand at what planning phases dynamic policy pathway approaches work best and how current barriers in legislation, practices, mind-set, organization, and resources can be overcome.
