  • A catalog of genetic loci associated with kidney function from analyses of a million individuals

    • Chronic kidney disease (CKD) is responsible for a public health burden with multi-systemic complications. Through transancestry meta-analysis of genome-wide association studies of estimated glomerular filtration rate (eGFR) and independent replication (n = 1,046,070), we identified 264 associated loci (166 new). Of these,147 were likely to be relevant for kidney function on the basis of associations with the alternative kidney function marker blood urea nitrogen (n = 416,178). Pathway and enrichment analyses, including mouse models with renal phenotypes, support the kidney as the main target organ. A genetic risk score for lower eGFR was associated with clinically diagnosed CKD in 452,264 independent individuals. Colocalization analyses of associations with eGFR among 783,978 European-ancestry individuals and gene expression across 46 human tissues, including tubulo-interstitial and glomerular kidney compartments, identified 17 genes differentially expressed in kidney. Fine-mapping highlighted missense driver variants in 11 genes and kidney-specific regulatory variants. These results provide a comprehensive priority list of molecular targets for translational research.

  • A Hierarchically Coordinated Operation and Control Scheme for DC Microgrid Clusters Under Uncertainty

    • In the existing works of microgrid clusters, operation and real-time control are normally designed separately in a hierarchical architecture, with the real-time control in the primary and secondary levels, and operation in the tertiary level. This article proposes a hierarchically coordinated control scheme for DC MG clusters under uncertainty. In each MG, the tertiary level controller optimizes the operating cost in the MG by taking into account the real-time uncertainties of renewable generations and loads deviated from the forecasting data; and the primary controller responds to the real-time power fluctuations through an optimised droop curve. The hierarchically coordinated optimization problem is formulated to optimize the power set points and droop curve coefficients simultaneously under uncertainties using an adjustable robust optimization model. For the MG cluster, the energy sharing of each MG in the cluster is optimized to minimize the total operating cost and the transmission loss. The overall optimization problem is solved in a distributed manner by alternating direction method of multipliers (ADMM) where each MG entity only exchanges boundary information (i.e. the power exchange of MG entity with the MG cluster), thus information privacy and plug-and-play feature of each MG are guaranteed. The proposed approach optimally coordinates the operation and real-time control layers of a DC MG cluster with uncertainties; it achieves decentralized power sharing at the real-time control layer and distributed optimization at the operation layer, featuring high scalability, reliability and economy. Case studies of a DC MG cluster are conducted in Matlab/Simulink in order to demonstrate the effectiveness of the proposed approach.

  • Self-Supervised Video Representation Learning by Video Incoherence Detection

    • This article introduces a novel self-supervised method that leverages incoherence detection for video representation learning. It stems from the observation that the visual system of human beings can easily identify video incoherence based on their comprehensive understanding of videos. Specifically, we construct the incoherent clip by multiple subclips hierarchically sampled from the same raw video with various lengths of incoherence. The network is trained to learn the high-level representation by predicting the location and length of incoherence given the incoherent clip as input. Additionally, we introduce intravideo contrastive learning to maximize the mutual information between incoherent clips from the same raw video. We evaluate our proposed method through extensive experiments on action recognition and video retrieval using various backbone networks. Experiments show that our proposed method achieves remarkable performance across different backbone networks and different datasets compared to previous coherence-based methods.

  • Identification of RNA Expression Profiles in Thyroid Cancer to Construct a Competing Endogenous RNA (ceRNA) Network of mRNAs, Long Noncoding RNAs (IncRNAs), and microRNAs (miRNAs)

    • Background: The aims of this study were to use RNA expression profile bioinformatics data from cases of thyroid cancer from the Cancer Genome Atlas (TCGA), the Kyoto Encyclopedia of Genes and Genomes (KEGG), and the Gene Ontology (GO) databases to construct a competing endogenous RNA (ceRNA) network of mRNAs, long noncoding RNAs (IncRNAs), and microRNAs (miRNAs).Material/Methods: TCGA provided RNA profiles from 515 thyroid cancer tissues and 56 normal thyroid tissues. The DESeq R pack- age analyzed high-throughput sequencing data on differentially expressed RNAs. GO and KEGG pathway analysis used the DAVID 6.8 and the ClusterProfile R package. Kaplan-Meier survival statistics and Cox regression analysis were performed. The thyroid cancer ceRNA network was constructed based on the miRDB, miRTar-Base, and TargetScan databases.Results: There were 1,098 mRNAs associated with thyroid cancer; 101 mRNAs were associated with overall survival (OS). Multivariate analysis developed a risk scoring system that identified seven signature mRNAs, with a discriminative value of 0.88, determined by receiver operating characteristic (ROC) curve analysis. A ceRNA network included 13 mRNAs, 31 IncRNAs, and seven miRNAs. Four out of the 31 IncRNAs and all miRNAs were down-regulated, and the remaining RNAs were upregulated. Two IncRNAs (MIR1281A2HG and OPCML-IT1) and one miRNA (miR-184) were significantly associated with OS in patients with thyroid cancer.Conclusions: Differential RNA expression profiling in thyroid cancer was used to construct a ceRNA network of mRNAs, IncRNAs, and miRNAs that showed potential in evaluating prognosis.

  • Gene-educational attainment interactions in a multi-ancestry genome-wide meta-analysis identify novel blood pressure loci

    • Educational attainment is widely used as a surrogate for socioeconomic status (SES). Low SES is a risk factor for hypertension and high blood pressure (BP). To identify novel BP loci, we performed multi-ancestry meta-analyses accounting for gene-educational attainment interactions using two variables, “Some College” (yes/no) and “Graduated College” (yes/no). Interactions were evaluated using both a 1 degree of freedom (DF) interaction term and a 2DF joint test of genetic and interaction effects. Analyses were performed for systolic BP, diastolic BP, mean arterial pressure, and pulse pressure. We pursued genome-wide interrogation in Stage 1 studies (N = 117 438) and follow-up on promising variants in Stage 2 studies (N = 293 787) in five ancestry groups. Through combined meta-analyses of Stages 1 and 2, we identified 84 known and 18 novel BP loci at genome-wide significance level (P < 5 × 10-8). Two novel loci were identified based on the 1DF test of interaction with educational attainment, while the remaining 16 loci were identified through the 2DF joint test of genetic and interaction effects. Ten novel loci were identified in individuals of African ancestry. Several novel loci show strong biological plausibility since they involve physiologic systems implicated in BP regulation. They include genes involved in the central nervous system-adrenal signaling axis (ZDHHC17, CADPS, PIK3C2G), vascular structure and function (GNB3, CDON), and renal function (HAS2 and HAS2-AS1, SLIT3). Collectively, these findings suggest a role of educational attainment or SES in further dissection of the genetic architecture of BP.

  • Multiancestry Genome-Wide Association Study of Lipid Levels Incorporating Gene-Alcohol Interactions

    • A person's lipid profile is influenced by genetic variants and alcohol consumption, but the contribution of interactions between these exposures has not been studied. We therefore incorporated gene-alcohol interactions into a multiancestry genome-wide association study of levels of high-density lipoprotein cholesterol, low-density lipoprotein cholesterol, and triglycerides. We included 45 studies in stage 1 (genome-wide discovery) and 66 studies in stage 2 (focused follow-up), for a total of 394,584 individuals from 5 ancestry groups. Analyses covered the period July 2014-November 2017. Genetic main effects and interaction effects were jointly assessed by means of a 2-degrees-of-freedom (df) test, and a 1-df test was used to assess the interaction effects alone. Variants at 495 loci were at least suggestively associated (P < 1 x 10(-6)) with lipid levels in stage 1 and were evaluated in stage 2, followed by combined analyses of stage 1 and stage 2. In the combined analysis of stages 1 and 2, a total of 147 independent loci were associated with lipid levels at P < 5 x 10(-8) using 2-df tests, of which 18 were novel. No genome-wide-significant associations were found testing the interaction effect alone. The novel loci included several genes (proprotein convertase subtilisin/kexin type 5 (PCSK5), vascular endothelial growth factor B (VEGFB), and apolipoprotein B mRNA editing enzyme, catalytic polypeptide 1 (APOBEC1) complementation factor (A1CF)) that have a putative role in lipid metabolism on the basis of existing evidence from cellular and experimental models.

    • Background: The influencing factors of kinesiophobia (fear of movement) in patients with atrial fibrillation(AF)during the post-operative “Blanking Period” are not known. The aims were to investigate the status of kinesiophobia in patients with AF during the post-operative “Blanking Period”, then further describe the occurrence and analyze the influencing factors of patients' kinesiophobia by the Fear-Avoidance Model. Materials and methods: In total,400 patients diagnosed with atrial fibrillation, during the post-operative “Blanking Period” from the selected hospital were included in this study. The Tampa Scale for Kinesiophobia Heart (TSK-SV Heart), the Self-efficacy for Exercise (SEE) scale, and the Geriatric Locomotive Function Scale (GLFS) were used to assess kinesiophobia, exercise self-efficacy, and physical function. The study adopted a cross-sectional design. Results: The score of kinesiophobia during the “Blanking Period” after operation in patients with atrial fibrillation was (44.06 ± 10.77), and the rate of high kinesiophobia was 71.61%.Logistic regression results showed that age, education, household monthly income, resting heart rate, EHRA symptom classification, exercise self-efficacy, and physical function influenced the kinesiophobia of patients during the post-operative “Blanking Period”(p<0.05, p<0.01). Conclusions: Kinesiophobia is common in patients with atrial fibrillation during the postoperative “Blanking Period”, and the fear of movement is related to age, education, household monthly income, resting heart rate, EHRA symptom classification, exercise self-efficacy, and physical function. Clinical and nursing staff should pay close attention to the psychological problems in the post-operation “Blanking Period” of exercise rehabilitation in patients with atrial fibrillation, make timely interventions to reduce patients' fear of movement, and improve patients' compliance with exercise rehabilitation.

  • Association between long-term static postures exposure and musculoskeletal disorders among university employees : A viewpoint of inflammatory pathways

    • BackgroundMusculoskeletal disorders (MSDs) are critical occupational and social problems. With the improvement of production mechanization and automation, and the widespread application of computers, more occupations are exposed to static postures and load. This study explored the role of inflammation in the association between static postures exposure and MSDs. MethodsThis study adopted a prospective nested case-control design in which 66 lower back MSDs cases and 66 healthy controls were selected from a cohort study of university employees. The personal information, postural load, musculoskeletal symptoms, pressure pain thresholds (PPTs), and inflammatory cytokines were collected. Logistic and linear regressions were used to investigate the association among postural load, inflammatory cytokines, and lower back MSDs. Mediation analysis was used to calculate the mediation effect. ResultsThe results of logistic and linear regressions showed that postural load and inflammatory cytokines were positively associated with lower back MSDs (P < 0.05), and postural load was positively associated with inflammatory cytokines (P < 0.05). Further, mediation analysis showed that the mediation effect of postural load on the lower back MSDs through TNF-alpha was 0.073 (95%CI: 0.025-0.128), and the mediation effect of posture load on the lower back MSDs through IL-6 was 0.098 (95%CI: 0.041-0.179), respectively. ConclusionStatic postures were associated with the occurrence of MSDs through inflammatory cytokines, and low-level inflammation may be a critical early event in the generation of MSDs. This study may help bridge the gap of potential mechanisms linking static postures to increased risks of MSDs, and provide new evidence for targeted protection against the global increasing MSDs.

  • Derived patterns of musculoskeletal symptoms and their relationships with ergonomic factors among electronic assembly workers : A latent class analysis

    • Introduction: Multi-site musculoskeletal symptoms (MSS) are considered to be more common and have more serious consequences than single-site MSS. This study aimed to determine whether derived patterns of MSS may be identified in electronic assembly workers and if extracted MSS classes are associated with personal and work-related factors. Method: A cross-sectional questionnaire study was performed with 700 participating electronic assembly workers. The questionnaire included individual factors, psychosocial and physical exposures, and MSS. The derived patterns of MSS and their relationships with ergonomic factors were analyzed using latent class analysis (LCA) and multinomial logistic regression models (MLRM). Results: The 1-year prevalence of MSS affecting only one body site or two or more body sites was 14.9% and 32.7%, respectively. The results of LCA showed three distinct classes of MSS patterns, which were labelled 'MSS in most sites' (5.0%), 'MSS in neck and shoulder' (27.0%), and 'MSS in one or no site' (68.0%). The results of MLRM showed that the 'MSS in neck and shoulder' was associated with job tenure (OR 5.579, 95% CI 2.488-12.511), excessive dynamic and static loads (OR 3.868, 95% CI 1.702- 8.793 and OR 5.270, 95% CI 2.020-13.747, respectively); while the 'MSS in most sites' was associated with high job demands (OR 4.528, 95% CI 1.647-12.445) and excessive dynamic loads (OR 111.554, 95% CI 4.996-2490.793). Conclusions: The results showed unique patterns of MSS among electronic assembly workers that were associated with personal and work-related factors. Practical applications: The findings highlight that the high prevalence of multi-site MSS in this group should be a focus. It also provides further evidence that LCA considering the number and location of anatomical sites involving MSS can be used to determine distinct classes of MSS patterns, which is of great significance for the epidemiological study and management of MSS in the future.

  • Study on the Associations of Individual and Work-Related Factors with Low Back Pain among Manufacturing Workers Based on Logistic Regression and Structural Equation Model

    • Work-related musculoskeletal injuries are one of the major occupational health issues of the workers, especially low back pain (LBP). The aim of this study was to survey the prevalence of LBP among manufacturing workers and to identify associations of individual and work-related factors with LBP. A cross-sectional questionnaire study was performed with 1173 participating manufacturing workers. The questionnaire included individual factors, psychosocial and physical exposures, and musculoskeletal discomfort. It was analyzed by logistic regression and structural equation modeling (SEM). The 1-year prevalence of LBP among Chinese manufacturing workers was 33.6%. Logistic regression analysis showed that job tenure, awkward postures, vibration and job demand were positively-while social support and job control were negatively associated with LBP (p < 0.05). The SEM results indicated that, as shown in other studies, job types, job tenure, postural load, high job demand, low job control and vibration were directly associated with LBP, but also that job types, high job demand, low social support and vibration may have indirect effects on LBP-mediated by postural load.
