  • Active gas features in three HSC-SSP CAMIRA clusters revealed by high angular resolution analysis of MUSTANG-2 SZE and XXL X-ray observations

    • We present results from simultaneous modelling of high angular resolution GBT/MUSTANG-2 90 GHz Sunyaev-Zel'dovich effect (SZE) measurements and XMM-XXL X-ray images of three rich galaxy clusters selected from the HSC-SSP Survey. The combination of high angular resolution SZE and X-ray imaging enables a spatially resolved multicomponent analysis, which is crucial to understand complex distributions of cluster gas properties. The targeted clusters have similar optical richnesses and redshifts, but exhibit different dynamical states in their member galaxy distributions: a single-peaked cluster, a double-peaked cluster, and a cluster belonging to a supercluster. A large-scale residual pattern in both regular Compton-parameter y and X-ray surface brightness distributions is found in the single-peaked cluster, indicating a sloshing mode. The double-peaked cluster shows an X-ray remnant cool core between two SZE peaks associated with galaxy concentrations. The temperatures of the two peaks reach similar to 20-30 keV in contrast to the cool core component of similar to 2 keV, indicating a violent merger. The main SZE signal for the supercluster is elongated along a direction perpendicular to the major axis of the X-ray core, suggesting a minor merger before core passage. The S-X and y distributions are thus perturbed at some level, regardless of the optical properties. We find that the integrated Compton y parameter and the temperature for the major merger are boosted from those expected by the weak-lensing mass and those for the other two clusters show no significant deviations, which is consistent with predictions of numerical simulations.

  • Composition and mixing state of individual aerosol particles from northeast Greenland and Svalbard in the Arctic during spring 2018

    • The Arctic region is warming about four times faster than the rest of the globe, and thus it is important to understand the processes driving climate change in this region. Aerosols are a significant component of the Arctic climate system as they form ice crystals and liquid droplets that control the dynamics of clouds and also directly interact with solar radiation, depending on the compositions and mixing states of individual particles. Here, we report on the characteristics of submicron-sized aerosol particles using transmission electron microscopy obtained at two high Arctic sites, northeast Greenland (Villum Research Station) and Svalbard (Zeppelin Observatory), during spring 2018. The results showed that a dominant compound in the submicron-sized spring aerosols was sulfate, followed by sea salt particles. Both model simulations and observations at the Zeppelin Observatory showed that sea salt particles became more prevalent when low-pressure systems passed by the station. Model simulations indicate that both sampling sites were affected by diffused and diluted long-range transport of anthropogenic aerosols from lower latitudes with negligible influences of biomass burning emissions during the observation period. Overall, the composition of measured aerosol particles from the two Arctic sites was generally similar and showed no apparent variation except for the sea salt fractions. This study shows a general picture of high Arctic aerosol particles influenced by marine sources and diffused long-range transport of anthropogenic sources during the Arctic spring period. These results will contribute to a better knowledge of the aerosol composition and mixing state during the Arctic spring, which helps to understand the contributions of aerosols to the Arctic climate.

  • Combined Chylothorax and Chylous Ascites Complicating Liver Transplantation: A Report of a Case and Review of the Literature

    • Chyle leaks may occur as a result of surgical intervention. Chyloperitoneum, or chylous ascites after liver transplantation, is rare and the development of chylothorax after abdominal surgery is even more rare. With increasingly aggressive surgical resections, particularly in the retroperitoneum, the incidence of chyle leaks is expected to increase in the future. Here we present a unique case of a combined chylothorax and chyloperitoneum following liver transplantation successfully managed conservatively. Risk factors for chylous ascites include para-aortic manipulation, extensive retroperitoneal dissection, use of a Ligasure device, and early enteral feeding as well as early enteral feeding. The clinical presentation is typically insidious and may include painless abdominal distension. Diagnosis can be made by noting characteristic milky white drainage which on laboratory examination has a total fluid triglyceride level >110 mg/dl, an ascites/serum triglyceride ratio of >1 and a leukocyte count in fluid >1000/uL with a lymphocyte predominance. Chyle leaks may lead to significant morbidity and mortality. Numerous management options exist, with conservative nonoperative measurements leading to the most consistent and successful outcomes. This includes a step-up approach beginning with dietary modifications to a low-fat or medium chain triglyceride diet followed by nil per os with addition of total parenteral nutrition and somatostatin analogues such as octreotide. Rarely do patients require more invasive treatment. Early recognition and appropriate management are imperative to mitigate this complication.

    • Absolute lymphocyte count (ALC) is considered a surrogate marker for nutritional status and immunocompetence. We investigated the association between ALC and post-liver transplant outcomes in patients who received a deceased donor liver transplant (DDLT). Patients were categorized by ALC at liver transplant: low (<500/mu L), mid (500-1000/mu L), and high ALC (>1000/mu L). Our main analysis used retrospective data (2013-2018) for DDLT recipients from Henry Ford Hospital (United States); the results were further validated using data from the Toronto General Hospital (Canada). Among 449 DDLT recipients, the low ALC group demonstrated higher 180-day mortality than mid and high ALC groups (83.1% vs 95.8% and 97.4%, respectively; low vs mid: P =.001; low vs high: P <.001). A larger proportion of patients with low ALC died of sepsis compared with the combined mid/high groups (9.1% vs 0.8%; P <.001). In multivariable analysis, pretransplant ALC was associated with 180-day mortality (hazard ratio, 0.20; P =.004). Patients with low ALC had higher rates of bacteremia (22.7% vs 8.1%; P <.001) and cytomegaloviremia (15.2% vs 6.8%; P =.03) than patients with mid/high ALC. Low ALC pretransplant through postoperative day 30 was associated with 180-day mortality among patients who received rabbit antithymocyte globulin induction (P =.001). Pretransplant lymphopenia is associated with short-term mortality and a higher incidence of posttransplant infections in DDLT patients.
