  • Combination Therapy with Oral Treprostinil for Pulmonary Arterial Hypertension. A Double-Blind Placebo-controlled Clinical Trial.

    • Rationale: Oral treprostinil improves exercise capacity in patients with pulmonary arterial hypertension (PAH), but the effect on clinical outcomes was unknown.Objectives: To evaluate the effect of oral treprostinil compared with placebo on time to first adjudicated clinical worsening event in participants with PAH who recently began approved oral monotherapy.Methods: In this event-driven, double-blind study, we randomly allocated 690 participants (1:1 ratio) with PAH to receive placebo or oral treprostinil extended-release tablets three times daily. Eligible participants were using approved oral monotherapy for over 30 days before randomization and had a 6-minute-walk distance 150 m or greater. The primary endpoint was the time to first adjudicated clinical worsening event: death; hospitalization due to worsening PAH; initiation of inhaled or parenteral prostacyclin therapy; disease progression; or unsatisfactory long-term clinical response.Measurements and Main Results: Clinical worsening occurred in 26% of the oral treprostinil group compared with 36% of placebo participants (hazard ratio, 0.74; 95% confidence interval, 0.56-0.97; P = 0.028). Key measures of disease status, including functional class, Borg dyspnea score, and N-terminal pro-brain natriuretic peptide, all favored oral treprostinil treatment at Week 24 and beyond. A noninvasive risk stratification analysis demonstrated that oral treprostinil-assigned participants had a substantially higher mortality risk at baseline but achieved a lower risk profile from Study Weeks 12-60. The most common adverse events in the oral treprostinil group were headache, diarrhea, flushing, nausea, and vomiting.Conclusions: In participants with PAH, addition of oral treprostinil to approved oral monotherapy reduced the risk of clinical worsening.Clinical trial registered with www.clinicaltrials.gov (NCT01560624).

  • Ambient Electrosynthesis of Ammonia : Electrode Porosity and Composition Engineering

    • Ammonia, a key precursor for fertilizer production, convenient hydrogen carrier, and emerging clean fuel, plays a pivotal role in sustaining life on Earth. Currently, the main route for NH3 synthesis is by the heterogeneous catalytic Haber-Bosch process (N-2+ 3H(2) -> 2NH(3)), which proceeds under extreme conditions of temperature and pressure with a very large carbon footprint. Herein we report that a pristine nitrogen-doped nanoporous graphitic carbon membrane (NCM) can electrochemically convert N-2 into NH3 in an acidic aqueous solution under ambient conditions. The Faradaic efficiency and rate of production of NH3 on the NCM electrode reach 5.2% and 0.08 gm(-2) h(-1), respectively. Functionalization of the NCM with Au nanoparticles dramatically enhances these performance metrics to 22% and 0.36 gm(-2) h(-1), respectively. As this system offers the potential to be scaled to industrial levels it is highly likely that it might displace the century-old Haber-Bosch process.

  • Unveiling the energetic and structural properties of Pu doped zircon through electrochemical equilibrium diagram from DFT plus U calculations

    • Zircon (ZrSiO4) mineral is a sustainable and promising material to store of radioactive waste that has received extensive attention by material, geochemical and environmental scientists. Although the incorporation of actinide elements in zircon lattices has been experimentally studied, bare fundamental work are carried out to systemically assess the structural and chemical stabilities of Pu doped zircon. The primary aim to unveil the Pu immobilization mechanism and assess the stability of PuxZr1-xSiO4 is carried out by calculating the formation energies, electron and hole affinities, and electronic levels of Pu doped zircon based on density functional theory. Our results reveal under mu = mu(O-poor) condition Pu-Si(4+), Pu-Zr(1+) and Pu-Zr(0) are respectively energetically favorable to form with increasing the electronic chemical potential. Besides, Pu-Zr(4+) is energetically favorable in an n-type environment under all these three conditions (i.e., mu = mu(O-poor), mu = mu(Pu/Zr), mu = mu(Pu/Si)). In addition, Pu doping will induce local structural distortion. Intriguingly however, self-repairing the symmetry of [ZrO8] polyhedra is first observed via the structural distortion in Pu-Zr(4+) configuration, which in turn could enhance the structural stability of PuxZr1-xSiO4. Ab initio molecular dynamic simulations demonstrate the configurations with negative formation energies are thermal stable at 500 K. The charge density difference and charge transfer are investigated to describe the chemical bonding nature. It is demonstrated Pu(5f)-O(2p) hybridization is more profound for interstitial Pu. Moreover, the bonding character of surrounding Zr atoms along [010] direction is almost identical to the pristine one, while it is distinctly changed towards [100] and [001] directions, showing remarkable anisotropy of PuxZr1-xSiO4. Oppositely, the ionicity in Pu-O bond is mainly featured when Zr or Si sites are substituted by Pu atoms which becomes stronger with increasing the hole doping process. (C) 2020 Elsevier B.V.   • Familial co-aggregation of schizophrenia and eating disorders in Sweden and Denmark

    • Eating disorders and schizophrenia are both moderately to highly heritable and share significant genetic risk despite distinct diagnostic criteria. Large-scale family studies on the co-aggregation of these disorders are lacking. Thus, we aimed to estimate the co-occurrence and familial co-aggregation of these disorders within the entire Swedish and Danish population. The proband cohort consisted of individuals born in Sweden (1977-2003) and Denmark (1984-2006) and still residing in their respective country at age six (NSweden = 2,535,191, NDenmark = 1,382,367). Probands were linked to their biological parents, siblings, grandparents, uncles/aunts, and cousins. Diagnoses for anorexia nervosa (AN) and other eating disorders (OED: bulimia nervosa, binge-eating disorder, and eating disorder not otherwise specified) for probands and schizophrenia diagnoses for both probands and relatives were obtained. The likelihood of having schizophrenia in those with AN or OED and their relatives was compared with individuals without eating disorder diagnoses and their relatives. Probands with AN or OED were more likely to have schizophrenia than probands without these disorders. All relatives of probands with AN or OED (except parents and uncles/aunts of probands with AN) were at increased risk of schizophrenia. In general, the magnitude of odds ratios attenuated with decreasing genetic relatedness. These results suggest familial liability contributes to the association between eating disorders and schizophrenia. Clinicians should be mindful of this comorbid and co-aggregation pattern as it may influence case conceptualization and treatment decisions.

  • Ultra-wideband Full-metal Planar Array Antenna with a Combination of Ridge Gap Waveguide and E-plane Groove Gap Waveguide

    • An ultra-wideband (UWB) full-metal planar array antenna based on a combination of ridge gap waveguide (RGW) and E-plane groove gap waveguide (E-GGW) is presented for millimeter-wave (mmWave) applications. First, the radiation element of a double-step double-ridged slot structure is proposed and designed. With removing the vertical walls of the cavity in the H-plane, the impedance matching characteristics presents a UWB bandwidth of 53%. Second, a UWB double-layer feeding network is achieved by combining RGWs and double-side-pin E-GGWs for the radiation elements. Finally, an 8 × 8 full-metal planar array antenna with the presented elements and network is designed, prototyped and measured. The measured results agree well with the simulated results. The proposed antenna provides an impedance matching bandwidth (|S11| < -10 dB) of 46.8% from 18.8 to 30.3 GHz. The maximum gain is 27.7 dBi at 27 GHz and a stable gain of 26 ± 1.7 dBi is obtained over the operation band.
