    • Since emergency vehicles (EMVs) in urban transit systems play a crucial role in responding to time-critical events, the quick response of EMVs is essential for improving the success rate of rescue operations and minimizing property loss. Booming connected vehicle (CV) technology provides a new perspective to further enhance the effectiveness of EMV priority. Based on this CV technology, we propose a cooperative multiple EMV priority model in which the speed, acceleration, and lane changing actions of both the EMVs and surrounding ordinary vehicles (OVs) are set as decision variables. This proposed model is rigorously formulated in integer linear programming to maximize the EMV traffic efficiency and find a trade-off between the interference with normal traffic flows and the smoothness of the EMV driving trajectories. Two customized algorithms are developed to reduce the number of decision variables and constraints to obtain the better feasible solution in an acceptable computational time. A numerical experiment based on real-world data is proposed to further verify the utility and effectiveness of the aforementioned mathematical model. The customized algorithms achieve near-exact solutions with significantly faster computation compared to the benchmark solver. The robustness of the proposed model is tested with different parameter settings in the sensitivity analysis.

  • New Cancers after Autotransplantations for Multiple Myeloma

    • We describe baseline incidence and risk factors for new cancers in 4161 persons receiving autotransplants for multiple myeloma in the United States from 1990 to 2010. Observed incidence of invasive new cancers was compared with expected incidence relative to the US population. The cohort represented 13,387 person-years at-risk. In total, 163 new cancers were observed, for a crude incidence rate of 1.2 new cancers per 100 person-years and cumulative incidences of 2.6% (95% confidence interval [CI], 2.09 to 3.17), 4.2% (95% CI, 3.49 to 5.00), and 6.1% (95% CI, 5.08 to 7.24) at 3, 5, and 7 years, respectively. The incidence of new cancers in the autotransplantation cohort was similar to age-, race-, and gender-adjusted comparison subjects with an observed/expected (O/E) ratio of 1.00 (99% CI, .81 to 1.22). However, acute myeloid leukemia and melanoma were observed at higher than expected rates with O/E ratios of 5.19 (99% CI, 1.67 to 12.04; P = .0004), and 3.58 (99% CI, 1.82 to 6.29; P < .0001), respectively. Obesity, older age, and male gender were associated with increased risks of new cancers in multivariate analyses. This large data set provides a baseline for comparison and defines the histologic type specific risk for new cancers in patients with MM receiving postautotransplantation therapies, such as maintenance.

    • Allogeneic hematopoietic cell transplantation has an established role in the treatment of adults with acute lymphoblastic leukemia whose survival when recipients of grafts from adult unrelated donors approaches that of recipients of grafts from sibling donors. Our aim was to determine the role of mismatched unrelated cord blood grafts in transplantation for 802 adults with acute lymphoblastic leukemia in first or second complete remission. Using Cox regression we compared outcomes after 116 mismatched single or double cord blood transplants, 546 peripheral blood progenitor cell transplants and 140 bone marrow transplants. The characteristics of the recipients and their diseases were similar except cord blood recipients were younger, more likely to be non-Caucasians and more likely to have a low white blood cell count at diagnosis. There were differences in donor-recipient human leukocyte antigen-match depending on the source of the graft. Most adult donor transplants were matched at the allele-level considering human leukocyte antigens-A, -B, -C and -DRB1. In contrast, most cord blood transplants were mismatched and considered antigen-level matching; 57% were mismatched at two loci and 29% at one locus whereas only 29% of adult donor transplants were mismatched at one locus and none at two loci. There were no differences in the 3-year probabilities of survival between recipients of cord blood (44%), matched adult donor (44%) and mismatched adult donor (43%) transplants. Cord blood transplants engrafted slower and were associated with less grade 2-4 acute but similar chronic graft-versus-host disease, relapse, and transplant-related mortality. The survival of cord blood graft recipients was similar to that of recipients of matched or mismatched unrelated adult donor grafts and so cord blood should be considered a valid alternative source of stem cells for adults with acute lymphoblastic leukemia in the absence of a matched unrelated adult donor.

    • Nuclear magnetic resonance (NMR) spectroscopy serves as an indispensable tool in chemistry and biology but often suffers from long experimental time. We present a proof-of-concept of application of deep learning and neural network for high-quality, reliable, and very fast NMR spectra reconstruction from limited experimental data. We show that the neural network training can be achieved using solely synthetic NMR signal, which lifts the prohibiting demand for large volume of realistic training data usually required in the deep learning approach.

  • Older Patients with Myeloma Derive Similar Benefit from Autologous Transplantation

    • Autologous hematopoietic cell transplantation (AHCT) for plasma cell myeloma is performed less often in people >70 years old than in people <= 70 years old. We analyzed 11,430 AHCT recipients for plasma cell myeloma prospectively reported to the Center for International Blood and Marrow Transplant Research between 2008 and 2011, representing the majority of US AHCT activity during this period. Survival (OS) was compared in 3 cohorts: ages 18 to 59 years (n = 5818), 60 to 69 years (n = 4666), and >70 years (n = 946). Median OS was not reached for any cohort. In multivariate analysis, increasing age was associated with mortality (P = .0006). Myeloma-specific mortality was similar among cohorts at 12%, indicating an age-related effect on nonmyeloma mortality. Analyses were performed in a representative subgroup comparing relapse rate, progression-free survival (PFS), and nonrelapse mortality (NRM). One-year NRM was 0% for age >70 years and 2% for other ages (P = not significant). The three-year relapse rate was 56% in age 18 to 59 years, 61% in age 60 to 69 years, and 63% age >70 (P = not significant). Three-year PFS was similar at 42% in age 18 to 59 years, 38% in age 60 to 69 years, and 33% in age >70 years (P = not significant). Postrelapse survival was significantly worse for the older cohort (P = .03). Older subjects selected for AHCT derived similar antimyeloma benefit without worse NRM, relapse rate, or PFS.

  • Revealing the determinants of the intermodal transfer ratio between metro and bus systems considering spatial variations

    • Buses and metros are two main public transit modes, and these modes are crucial components of sustainable transportation systems. Promoting reciprocal integration between bus and metro systems requires a deep understanding of the effects of multiple factors on transfers among integrated public transportation transfer modes, i.e., metro-to-bus and bus-to-metro. This study aims to reveal the determinants of the transfer ratio between bus and metro systems and quantify the associated impacts. The transfer ratio between buses and metros is identified based on large-scale transaction data from automated fare collection systems. Meanwhile, various influencing factors, including weather, socioeconomic, the intensity of business activities, and built environment factors, are obtained from multivariate sources. A multivariate regression model is used to investigate the associations between the transfer ratio and multiple factors. The results show that the transfer ratio of the two modes significantly increases under high temperature, strong wind, rainfall, and low visibility. The morning peak hours attract a transfer ratio of up to 57.95%, and the average hourly transfer volume is 0.94 to 1.38 times higher at this time than in other periods. The intensity of business activities has the most significant impact on the transfer ratio, which is approximately 1.5 to 15 times that of the other independent variables. Moreover, an adaptative geographically weighted regression is utilized to investigate the spatial divergences of the influences of critical factors on the transfer ratio. The results indicate that the impact of a factor presents spatial heterogeneity and even shows opposite effects (in terms of positive and negative) on the transfer ratio in different urban contexts. For example, among the related socioeconomic variables, the impact of the housing price on the downtown transfer ratio is larger than that in the suburbs. Crowd density positively influences the transfer ratio at most stations in the northern region, whereas it shows negative results in the southern region. These findings provide valuable insights for public transportation management and promote the effective integration of bus and metro systems to provide enhanced transfer services.
