    • Digital anamorphosis is used to define a distorted image of health and care that may be viewed correctly using digital tools and strategies. MASK digital anamorphosis represents the process used by MASK to develop the digital transformation of health and care in rhinitis. It strengthens the ARIA change management strategy in the prevention and management of airway disease. The MASK strategy is based on validated digital tools. Using the MASK digital tool and the CARAT online enhanced clinical framework, solutions for practical steps of digital enhancement of care are proposed.

    • Soil organisms, including earthworms, are a key component of terrestrial ecosystems. However, little is known about their diversity, their distribution, and the threats affecting them. We compiled a global dataset of sampled earthworm communities from 6928 sites in 57 countries as a basis for predicting patterns in earthworm diversity, abundance, and biomass. We found that local species richness and abundance typically peaked at higher latitudes, displaying patterns opposite to those observed in aboveground organisms. However, high species dissimilarity across tropical locations may cause diversity across the entirety of the tropics to be higher than elsewhere. Climate variables were found to be more important in shaping earthworm communities than soil properties or habitat cover. These findings suggest that climate change may have serious implications for earthworm communities and for the functions they provide.

    • Empirical studies have rarely investigated the association between improvements on dynamic risk and protective factors for violence during forensic psychiatric treatment and reduced recidivism after discharge. The present study aimed to evaluate the effects of treatment progress in risk and protective factors on violent recidivism. For a sample of 108 discharged forensic psychiatric patients pre- and posttreatment assessments of risk (HCR-20) and protective factors (SAPROF) were compared. Changes were related to violent recidivism at different follow-up times after discharge. Improvements on risk and protective factors during treatment showed good predictive validity for abstention from violence for short-(1 year) as well as long-term (11 years) follow-up. This study demonstrates the sensitivity of the HCR-20 and the SAPROF to change and shows improvements on dynamic risk and protective factors are associated with lower violent recidivism long after treatment.

  • Development and validation of combined symptom-medication scores for allergic rhinitis*

    • Background: Validated combined symptom-medication scores (CSMSs) are needed to investigate the effects of allergic rhinitis treatments. This study aimed to use real-life data from the MASK-air® app to generate and validate hypothesis- and data-driven CSMSs.Methods: We used MASK-air® data to assess the concurrent validity, test-retest reliability and responsiveness of one hypothesis-driven CSMS (modified CSMS: mCSMS), one mixed hypothesis- and data-driven score (mixed score), and several data-driven CSMSs. The latter were generated with MASK-air® data following cluster analysis and regression models or factor analysis. These CSMSs were compared with scales measuring (i) the impact of rhinitis on work productivity (visual analogue scale [VAS] of work of MASK-air®, and Work Productivity and Activity Impairment: Allergy Specific [WPAI-AS]), (ii) quality-of-life (EQ-5D VAS) and (iii) control of allergic diseases (Control of Allergic Rhinitis and Asthma Test [CARAT]).Results: We assessed 317,176 days of MASK-air® use from 17,780 users aged 16-90 years, in 25 countries. The mCSMS and the factor analyses-based CSMSs displayed poorer validity and responsiveness compared to the remaining CSMSs. The latter displayed moderate-to-strong correlations with the tested comparators, high test-retest reliability and moderate-to-large responsiveness. Among data-driven CSMSs, a better performance was observed for cluster analyses-based CSMSs. High accuracy (capacity of discriminating different levels of rhinitis control) was observed for the latter (AUC-ROC = 0.904) and for the mixed CSMS (AUC-ROC = 0.820).Conclusion: The mixed CSMS and the cluster-based CSMSs presented medium-high validity, reliability and accuracy, rendering them as candidates for primary endpoints in future rhinitis trials.

    • Background: Mobile health can be used to generate innovative insights into optimizing treatment to improve allergic rhinitis (AR) control. Objectives: A cross-sectional real-world observational study was undertaken in 22 countries to complement a pilot study and provide novel information on medication use, disease control, and work productivity in the everyday life of patients with AR. Methods: A mobile phone app (Allergy Diary, which is freely available on Google Play and Apple stores) was used to collect the data of daily visual analogue scale (VAS) scores for (1) overall allergic symptoms; (2) nasal, ocular, and asthma symptoms; (3) work; and (4) medication use by using a treatment scroll list including all allergy medications (prescribed and over-the-counter) customized for 22 countries. The 4 most common intranasal medications containing intranasal corticosteroids and 8 oral H-1-antihistamines were studied. Results: Nine thousand one hundred twenty-two users filled in 112,054 days of VASs in 2016 and 2017. Assessment of days was informative. Control of days with rhinitis differed between no (best control), single (good control for intranasal corticosteroid-treated days), or multiple (worst control) treatments. Users with the worst control increased the range of treatments being used. The same trend was found for asthma, eye symptoms, and work productivity. Differences between oral H-1-antihistamines were found. Conclusions: This study confirms the usefulness of the Allergy Diary in accessing and assessing behavior in patients with AR. This observational study using a very simple assessment tool (VAS) on a mobile phone had the potential to answer questions previously thought infeasible.

  • Beyond Blame : Debunking the myths underlying the foreign aid debate

    • During the transition process in Central and Eastern Europe (CEE), the post-socialist countries received much support from Western high developed countries in order to improve the effectiveness of their public administrationand to adapt it to the demands given by the EU. Similar as it was with foreign aid for Latin American, African and Asian countries before, this support was, to say the least, not always effective. The question this paper addresses is how to understand such failures. To answer that question we reviewed the literature on the subject of foreign aid. In that literature an increasing radicalism about foreign aid, based on dubious assumptions, is observed. These assumptions are that the quantity of aid as such makes a difference, that nobody in the aid business cares, that optimizing the input, process and output in organizational and managerial terms will almost automatically improve effectiveness, and that there exists a coherent aid industry.This paper explicates the assumptions, and treats them as hypotheses. The conclusion, based on research into the aid given to CEE-countries during their transition process, is that the hypotheses have to be refuted. The assumptions underlying the debate about foreign aid are one-sided and false

    • © 2020 Sage Publications. This paper proposes a framework to analyze the varying understanding of democratic developments. Based on the theory of frame-analysis, it distinguishes six brackets within which democratic developments can be interpreted. This framework is applied to identify the nature of international rankings of domestic democracy and to compare the framing of developments in Polish democracy in reports on democratic developments from international organizations. The conclusion is that sense-making of democratic developments in general varies enormously, and that this is also visible in the international rankings of democracy. Democratic developments in Poland are also assessed in different ways, resulting in varying claims about the nature of the developments in Poland.

    • Often a distinction is made between interpersonal and institutional trust, as the former is defined in terms of encapsulated interests, that is the idea that somebody will take your interests into account. Scholars have argued that this cannot be applied to institutions and that generalized institutional trust is therefore not a meaningful concept. This article disputes this reasoning by distinguishing this kind of trust in the governance of capital cities from such trust in non-capital cities. It argues that it can be doubted especially for the local administration in capital cities that they predominantly have the interests of their residents in mind when making decisions. The resulting hypothesis that residents of capital cities have less trust in their local administration than residents of non-capital cities is tested and confirmed through a secondary analysis of Urban Audit data. The analysis shows a significant effect in the predicted direction, which remains strong when controlling for the satisfaction with public issues, the respective region, and poverty of the respondent. The conclusion is that citizens in municipalities do know whether or not local institutions have their interests in mind when making decisions, which makes institutional trust equally meaningful a concept as interpersonal trust.

    • This paper proposes a framework to analyze the varying understanding of democratic developments. Based on the theory of frame-analysis, it distinguishes six brackets within which democratic developments can be interpreted, namely on the side of the electorate, the limitations of suffrage and capabilities of the electorate; taking the election process, fairness of elections and the restraints elections pose on government; and regarding elected officials, their promotion of liberal democracy and of social democracy. This framework is applied to understand the nature of international rankings of local democracy and to compare the varying framing of developments in Polish democracy in reports from international organizations and in newspaper articles comparing those in Sweden and Poland. The conclusion is that democratic developments do indeed vary enormously and that this explains the difference in assessments of, among others, the democratic developments in Poland.
