    • Background: Early detection of coronary atherosclerosis using coronary computed tomography angiography (CCTA), in addition to coronary artery calcification (CAC) scoring, may help inform prevention strategies. We used CCTA to determine the prevalence, severity, and characteristics of coronary atherosclerosis and its association with CAC scores in a general population.Methods: We recruited 30 154 randomly invited individuals age 50 to 64 years to SCAPIS (the Swedish Cardiopulmonary Bioimage Study). The study includes individuals without known coronary heart disease (ie, no previous myocardial infarctions or cardiac procedures) and with high-quality results from CCTA and CAC imaging performed using dedicated dual-source CT scanners. Noncontrast images were scored for CAC. CCTA images were visually read and scored for coronary atherosclerosis per segment (defined as no atherosclerosis, 1% to 49% stenosis, or ≥50% stenosis). External validity of prevalence estimates was evaluated using inverse probability for participation weighting and Swedish register data.Results: In total, 25 182 individuals without known coronary heart disease were included (50.6% women). Any CCTA-detected atherosclerosis was found in 42.1%; any significant stenosis (≥50%) in 5.2%; left main, proximal left anterior descending artery, or 3-vessel disease in 1.9%; and any noncalcified plaques in 8.3% of this population. Onset of atherosclerosis was delayed on average by 10 years in women. Atherosclerosis was more prevalent in older individuals and predominantly found in the proximal left anterior descending artery. Prevalence of CCTA-detected atherosclerosis increased with increasing CAC scores. Among those with a CAC score >400, all had atherosclerosis and 45.7% had significant stenosis. In those with 0 CAC, 5.5% had atherosclerosis and 0.4% had significant stenosis. In participants with 0 CAC and intermediate 10-year risk of atherosclerotic cardiovascular disease according to the pooled cohort equation, 9.2% had CCTA-verified atherosclerosis. Prevalence estimates had excellent external validity and changed marginally when adjusted to the age-matched Swedish background population.Conclusions: Using CCTA in a large, random sample of the general population without established disease, we showed that silent coronary atherosclerosis is common in this population. High CAC scores convey a significant probability of substantial stenosis, and 0 CAC does not exclude atherosclerosis, particularly in those at higher baseline risk.

    • Cardiopulmonary diseases are major causes of death worldwide, but currently recommended strategies for diagnosis and prevention may be outdated because of recent changes in risk factor patterns. The Swedish CArdioPulmonarybioImage Study (SCAPIS) combines the use of new imaging technologies, advances in large-scale 'omics' and epidemiological analyses to extensively characterize a Swedish cohort of 30 000 men and women aged between 50 and 64 years. The information obtained will be used to improve risk prediction of cardiopulmonary diseases and optimize the ability to study disease mechanisms. A comprehensive pilot study in 1111 individuals, which was completed in 2012, demonstrated the feasibility and financial and ethical consequences of SCAPIS. Recruitment to the national, multicentre study has recently started.

  • Assessment and condition monitoring of a concrete railway bridge in Kiruna, Sweden

    • A two-span railway concrete trough bridge over Luossajokk in Kiruna in northern Sweden has been studied. The owner wanted to increase the axle loads from 250 to 300 kN in order to reduce freight costs for iron ore. Examples are given of methods used and results obtained from the assessment where bending, shear and fatigue were studied. Material properties, loads and load carrying capacity were evaluated using deterministic and probabilistic methods. It was shown that the bridge could carry the higher loads with a safety index β > 4.7 for reasonable assumptions of the load distributions. A measurement system was installed to check the actual level of critical strains and the worst positions of the train. Results are also given from a condition monitoring program 2001-2006, launched to periodically check the development of strains with time.

  • The utility of histology in assessing the cause of death in medico-legal autopsies in selected trauma deaths : suspension-, immersion-, fire-, and traffic-related

    • The goal of a medico-legal autopsy is primarily to determine the cause and manner of death. To this end, the pathologist often uses auxiliary analyses, including histology. However, the utility of routine histology in all medico-legal autopsies is unknown. Earlier studies on the utility of routine histology have shown inconsistent effects, with some studies recommending it and others rejecting it. To study the degree to which histology informs on the underlying cause of death, we sent autopsy reports from suspension-, immersion-, fire-, and traffic-related deaths to senior board-certified forensic pathologists and had them assess the cause of death, first without knowledge of the histological findings and then with knowledge thereof. Fifty cases were identified in each of four subgroups: fire-, immersion-, suspension-, and traffic-related deaths. The autopsy reports were anonymized, and the histological findings and conclusions were removed. Two board-certified forensic pathologists independently reviewed the reports in each subgroup and assessed the manner and underlying cause of death (including their certainty of this assessment on a five-level scale) with and without access to histological findings. The probability of changing the underlying cause of death posthistology was low in all study groups. There was a slight increase in the degree of certainty posthistology in cases where the underlying cause of death was not changed, but only when the antehistology certainty was low. Our results suggest that histology does not meaningfully inform on the underlying cause of death in suspension-, immersion-, fire-, and traffic-related deaths except when antehistology certainty is low.

    • Two main railway concrete arch bridges have been investigated in northern Sweden: the Vindel Bridge over the Vindel River with a span of 110 m and the Långforsen Bridge over the Kalix River with a span of 90 m. The bridges were constructed in the 1950s with similar designs. In order to investigate the possibility to increase the axle load from 225 kN to 250kN, two 3-D Finite Element models with shell elements were developed and two field tests were carried out with passing trains and ambient vibrations. The measurement frequencies and mode shapes were studied with the stochastic subspace identification method in the time domain. Based on a comparison of the predictions of the FE analyses and the results of the field tests, the FE models were updated. It was found that it was possible to make good predictions of the actual dynamic response with reasonable assumptions of material properties and joint stiffness in the structure.

    • The function of integral membrane proteins is critically dependent on their naturally surrounding lipid membrane. Detergent-solubilized and purified membrane proteins are therefore often reconstituted into cell-membrane mimics and analyzed for their function with single-molecule microscopy. Expansion of this approach toward a broad range of pharmaceutically interesting drug targets and biomarkers however remains hampered by the fact that these proteins have low expression levels, and that detergent solubilization and reconstitution often cause protein conformational changes and loss of membrane-specific cofactors, which may impair protein function. To overcome this limitation, we here demonstrate how antibody-modified nanoparticles can be used to achieve affinity purification and enrichment of selected integral membrane proteins directly from cell membrane preparations. Nanoparticles were first bound to the ectodomain of β-secretase 1 (BACE1) contained in cell-derived membrane vesicles. In a subsequent step, these were merged into a continuous supported membrane in a microfluidic channel. Through the extended nanoparticle tag, a weak (∼fN) hydrodynamic force could be applied, inducing directed in-membrane movement of targeted BACE1 exclusively. This enabled selective thousand-fold enrichment of the targeted membrane protein while preserving a natural lipid environment. In addition, nanoparticle-targeting also enabled simultaneous tracking analysis of each individual manipulated protein, revealing how their mobility changed when moved from one lipid environment to another. We therefore believe this approach will be particularly useful for separation in-line with single-molecule analysis, eventually opening up for membrane-protein sorting devices analogous to fluorescence-activated cell sorting.

    • Members of the cell death-inducing DFF45-like effector (CIDE) gene family have been shown to regulate lipid metabolism. In this article, we report that the third member of the human CIDE family, CIDEC, is down-regulated in response to a reduced caloric intake. The down-regulation was demonstrated by microarray and real-time polymerase chain reaction analysis of subcutaneous adipose tissue in 2 independent studies on obese patients undergoing treatment with a very low calorie diet. By analysis of CIDEC expression in 65 human tissues, we conclude that human CIDEC is predominantly expressed in subcutaneous adipocytes. Together, these observations led us to investigate the effect of decreased CIDEC expression in cultured 3T3-L1 adipocytes. Small interfering RNA-mediated knockdown of CIDEC resulted in an increased basal release of nonesterified fatty acids, decreased responsiveness to adrenergic stimulation of lipolysis, and increased oxidation of endogenous fatty acids. Thus, we suggest that CIDEC is a regulator of adipocyte lipid metabolism and may be important for the adipocyte to adapt to changes in energy availability.

    • The anti-tumour effect of the 131I-labelled antiprostate monoclonal antibody (MAb) E4 was studied in an experimental model with 41 nude mice, subcutaneously xenografted with a human prostate cancer cell line (DU-145). The mice were divided into four study groups, i.e. one receiving single and another repeated injections of the radiolabelled MAb. A third group was injected with non-labelled MAb, and the fourth served as an untreated control group. The tumour volumes increased similarly in all groups during the 27-day observation period. The tumour tissue was morphologically disintegrated in the group that received repeated radioimmunotherapy (RIT). The tumours from this group contained large fluid-filled cystic parts and demonstrated pronounced cellular and subcellular polymorphism in the remaining viable tumour tissue. The untreated control tumours and single therapy tumours remained solid. The proportion of the total tumour volume that consisted of viable tumour cells, as determined by morphometric techniques, was significantly lower in the 131I-E4-treated groups. The use of 131I-labelled E4 MAb has thus demonstrated a promising therapeutic potential.

    • An important aspect of precision medicine is to probe the stability in molecular profiles among healthy individuals over time. Here, we sample a longitudinal wellness cohort with 100 healthy individuals and analyze blood molecular profiles including proteomics, transcriptomics, lipidomics, metabolomics, autoantibodies andimmune cell profiling, complementedwith gut microbiota composition and routine clinical chemistry. Overall, our results show high variation between individuals across different molecular readouts, while the intra-individual baseline variation is low. The analyses show that each individual has a unique and stable plasma protein profile throughout the study period and that many individuals also show distinct profiles with regards to the other omics datasets, with strong underlying connections between the blood proteome and the clinical chemistry parameters. In conclusion, the results support an individual-based definition of health and show that comprehensive omics profiling in a longitudinal manner is a path forward for precision medicine.

    • In this paper we propose a new definition of prime ends for domains in metric spaces under rather general assumptions. We compare our prime ends to those of Caratheodory and Nakki. Modulus ends and prime ends, defined by means of the p-modulus of curve families, are also discussed and related to the prime ends. We provide characterizations of singleton prime ends and relate them to the notion of accessibility of boundary points, and introduce a topology on the prime end boundary. We also study relations between the prime end boundary and the Mazurkiewicz boundary. Generalizing the notion of John domains, we introduce almost John domains, and we investigate prime ends in the settings of John domains, almost John domains and domains which are finitely connected at the boundary.

    • We study various boundary and inner regularity questions for p(.)-(super)harmonic functions in Euclidean domains. In particular, we prove the Kellogg property and introduce a classification of boundary points for p(.)-harmonic functions into three disjoint classes: regular, semiregular and strongly irregular points. Regular and especially semiregular points are characterized in many ways. The discussion is illustrated by examples. Along the way, we present a removability result for bounded p(.)-harmonic functions and give some new characterizations of W-0(1,p(.)) spaces. We also show that p(.)-superharmonic functions are lower semicontinuously regularized, and characterize them in terms of lower semicontinuously regularized supersolutions.

    • The Virus-X-Viral Metagenomics for Innovation Value-project was a scientific expedition to explore and exploit uncharted territory of genetic diversity in extreme natural environments such as geothermal hot springs and deep-sea ocean ecosystems. Specifically, the project was set to analyse and exploit viral metagenomes with the ultimate goal of developing new gene products with high innovation value for applications in biotechnology, pharmaceutical, medical, and the life science sectors. Viral gene pool analysis is also essential to obtain fundamental insight into ecosystem dynamics and to investigate how viruses influence the evolution of microbes and multicellular organisms. The Virus-X Consortium, established in 2016, included experts from eight European countries. The unique approach based on high throughput bioinformatics technologies combined with structural and functional studies resulted in the development of a biodiscovery pipeline of significant capacity and scale. The activities within the Virus-X consortium cover the entire range from bioprospecting and methods development in bioinformatics to protein production and characterisation, with the final goal of translating our results into new products for the bioeconomy. The significant impact the consortium made in all of these areas was possible due to the successful cooperation between expert teams that worked together to solve a complex scientific problem using state-of-the-art technologies as well as developing novel tools to explore the virosphere, widely considered as the last great frontier of life.

    • Advancement in the understanding of biomolecular interactions has benefited greatly from the development of surface-sensitive bioanalytical sensors. To further increase their broad impact, significant efforts are presently being made to enable label-free and specific biomolecule detection with high sensitivity, allowing for quantitative interpretation and general applicability at low cost. In this work, we have addressed this challenge by developing a waveguide chip consisting of a flat silica core embedded in a symmetric organic cladding with a refractive index matching that of water. This is shown to reduce stray light (background) scattering and thereby allow for label-free detection of faint objects, such as individual sub-20 rim gold nanoparticles as well as sub-100 nm lipid vesicles. Measurements and theoretical analysis revealed that light-scattering signals originating from single surface-bound lipid vesicles enable characterization of their sizes without employing fluorescent lipids as labels. The concept is also demonstrated for label-free measurements of protein binding to and enzymatic (phospholipase A2) digestion of individual lipid vesicles, enabling an analysis of the influence on the measured kinetics of the dye-labeling of lipids required in previous assays. Further, diffraction-limited imaging of cells (platelets) binding to a silica surface showed that distinct subcellular features could be visualized and temporally resolved during attachment, activation, and spreading. Taken together, these results underscore the versatility and general applicability of the method, which due to its simplicity and compatibility with conventional microscopy setups may reach a widespread in life science and beyond.

    • A waveguide structure for evanescent wave microscopy and/or spectroscopy, comprising an optically transparent core layer, a lower dielectric cladding layer and an upper dielectric cladding layer arranged on opposite sides of the core layer. The core layer has a refractive index higher than the refractive indices of the cladding layers. The upper cladding layer is made of an organic material. A sample well is arranged on an upper surface of the core layer formed by a cavity in the upper cladding layer, the sample well being adapted to contain a sample medium with one or more sample objects. The core layer is made of a first dielectric inorganic material, and the upper cladding layer has a refractive index which closely matches the refractive index of the sample medium. A method for manufacturing such waveguide structure, and a measurement system comprising the waveguide structure are also disclosed.

    • The techniques and procedures for intensity-modulated radiation therapy (IMRT) are reviewed in the context of the information process central to treatment planning and delivery of IMRT. A presentation is given of the evolution of the information based radiotherapy workflow and dose delivery techniques, as well as the volume and planning concepts for relating the dose information to image based patient representations. The formulation of the dose shaping process as an optimization problem is described. The different steps in the calculation flow for determination of machine parameters for dose delivery are described starting from the formulation of optimization objectives over dose calculation to optimization procedures. Finally, the main elements of the quality assurance procedure necessary for implementing IMRT clinically are reviewed.

    • The variational capacity cap(p) in Euclidean spaces is known to enjoy the density dichotomy at large scales, namely that for every E subset of R-n, infx is an element of R(n)cap(p)(E boolean AND B(x, r), B(x, 2r))/cap(p)(B(x, r), B(x, 2r)) is either zero or tends to 1 as r -amp;gt; infinity. We prove that this property still holds in unbounded complete geodesic metric spaces equipped with a doubling measure supporting a p-Poincare inequality, but that it can fail in nongeodesic metric spaces and also for the Sobolev capacity in R-n. It turns out that the shape of balls impacts the validity of the density dichotomy. Even in more general metric spaces, we construct families of sets, such as John domains, for which the density dichotomy holds. Our arguments include an exact formula for the variational capacity of superlevel sets for capacitary potentials and a quantitative approximation from inside of the variational capacity.

Analyserna görs på uppdrag av Jordbruksverket, som underlag för bl.a. utvärderingen av miljökvalitetsmålet Ett rikt odlingslandskap. Särskilda rutiner har tagits fram för att aggregera olika variabler för att välja ut de småbiotoper som uppfyller de krav som Jordbruksverket har ställt upp, ur den befintliga databasen. Urvalet av småbiotoper är anpassat för att överensstämma med det urval av objekt som ingår i det s.k. KULT-stödet (miljöersättning till lantbrukare för skötsel av värdefulla natur- och kulturmiljöer) inom Jordbruksverkets Landsbygdsprogram. Arbetet har utförts vid institutionen för skoglig resurshushållning, Sveriges lantbruks-universitet, Umeå. Resultaten baseras på data från flygbildsinventeringen inom det nationella miljöövervakningsprogrammet NILS (Nationell Inventering av Landskapet i Sverige) vilket följer tillstånd och förändringar i det svenska landskapet och hur dessa påverkar förutsättningarna för den biologiska mångfalden. NILS finansieras av Naturvårdsverket, och ingår där i programområde Landskap. Ett viktigt syfte med programmet är att följa upp de nationella miljökvalitetsmålen för olika naturtyper och fungera som underlag för att se om genomförda policybeslut och miljövårdsåtgärder leder till önskade förbättringar

    • City–Region Food Systems (CRFS) is a cutting-edge concept and an emerging field of research. As a new analytical lens, it offers an integrated and multi-dimensional perspective on food’s origins, how it is grown and the path it follows to our plates and beyond. Building on this concept, this presentation reflects a prospective research project which seeks to explore opportunities for innovative and sustainable food systems in the Gothenburg region of Sweden by focusing on how rural and urban regions, food production and market can be integrated to promote regional food security. The project intends to: 1) develop scenarios with stakeholders for local food production in the region; 2) analyze the consequences of the scenarios on landscape change and biodiversity; 3) explore socio-economic consequences for producers and local communities; and 4) evaluate the sustainability and feasibility of scenarios with stakeholders. Five municipalities in Western Sweden (Gothenburg, Kungälv, Lerum, Alingsås and Essunga) will serve as study areas for the project, selected to reflect different kinds of potential for local food production in terms of dissimilar environmental conditions, prerequisites for farming and economic histories. The project responds to expressed interests and knowledge needs in the region and will be developed and implemented in direct cooperation with local and regional actors such as Västarvet, the Västra Götaland Region, the municipalities and various producer organizations. In sum, there are premises suggesting that recent urban food strategies and plans with sustainability ambitions are embracing several Sustainable Development Goals in the environmental, social, economic, and equity dimensions. This, in turn, is a characteristic of the Transition Movements pathway, in which the utility of food strategies in the work with sustainability transitions seems inevitable. The results are therefore likely to be transferable to other regions.

    • We develop a framework for studying variational problems in Banach spaces with respect to gradient relations, which encompasses many of the notions of generalized gradients that appear in the literature. We stress the fact that our approach is not dependent on function spaces and therefore applies equally well to functions on metric spaces as to operator algebras. In particular, we consider analogues of Dirichlet and obstacle problems, as well as first eigenvalue problems, and formulate conditions for the existence of solutions and their uniqueness. Moreover, we investigate to what extent a lattice structure may be introduced on ( ordered) Banach spaces via a norm-minimizing variational problem.
