    • The Cellular Internet of Things (CIoT), a new paradigm, paves the way for a large-scale deployment of IoT devices. CIoT promises enhanced coverage and massive deployment of low-cost IoT devices with an expected battery life of up to 10 years. However, such a long battery life can only be achieved provided the CIoT device is configured with energy efficiency in mind. This paper conducts a comprehensive survey on energy-saving solutions in 3GPP-based CIoT networks. In comparison to current studies, the contribution of this paper is the classification and an extensive analysis of existing energy-saving solutions for CIoT, e.g., the configuration of particular parameter values and software modifications of transport- or radio-layer protocols, while also stressing key parameters impacting the energy consumption such as the frequency of data reporting, discontinuous reception cycles (DRX), and Radio Resource Control (RRC) timers. In addition, we discuss shortcomings, limitations, and possible opportunities which can be investigated in the future to reduce the energy consumption of CIoT devices.

  • On the Energy-efficient Use of Discontinuous Reception and Release Assistance in NB-IoT

    • Cellular Internet of Things (CIoT) is a Low-Power Wide-Area Network (LPWAN) technology. It aims for cheap, lowcomplexity IoT devices that enable large-scale deployments and wide-area coverage. Moreover, to make large-scale deployments of CIoT devices in remote and hard-to-access locations possible, a long device battery life is one of the main objectives of these devices. To this end, 3GPP has defined several energysaving mechanisms for CIoT technologies, not least for the Narrow-Band Internet of Things (NB-IoT) technology, one of the major CIoT technologies. Examples of mechanisms defined include CONNECTED-mode DRX (cDRX), Release Assistance Indicator (RAI), and Power Saving Mode (PSM). This paper considers the impact of the essential energy-saving mechanisms on minimizing the energy consumption of NB-IoT devices, especially the cDRX and RAI mechanisms. The paper uses a purpose-built NB-IoT simulator that has been tested in terms of its built-in energy-saving mechanisms and validated with realworld NB-IoT measurements. The simulated results show that it is possible to save 70%-90% in energy consumption by enabling the cDRX and RAI. In fact, the results suggest that a battery life of 10 years is only achievable provided the cDRX, RAI, and PSM energy-saving mechanisms are correctly configured and used

M&A Release B leverages and extends M&A Release A, which has been documented in the previous Deliverable D3.5 [1]. In particular, we present new features and enhancements introduced in this new Release compared to the Release A. We also report some examples of usage of the M&A framework, in order to showcase its integrated in the 5GENESIS Reference Architecture.

    • The successful rollout of fifth-generation (5G) networks requires a full understanding of the behavior of the propagation channel, taking into account the signal formats and the frequencies standardized by the Third Generation Partnership Project (3GPP). In the past, channel characterization for 5G has been addressed mainly based on the measurements performed on dedicated links in experimental setups. This paper presents a state-of-the-art contribution to the characterization of the outdoor-to-indoor radio channel in the 3.5 GHz band, based on experimental data for commercial, deployed 5G networks, collected during a large scale measurement campaign carried out in the city of Rome, Italy. The analysis presented in this work focuses on downlink, outdoor-to-indoor propagation for two operators adopting two different beamforming strategies, single wide-beam and multiple synchronization signal blocks (SSB) based beamforming; it is indeed the first contribution studying the impact of beamforming strategy in real 5G networks. The time and power-related channel characteristics, i.e., mean excess delay and Root Mean Square (RMS) delay spread, path loss, and K-factor are studied for the two operators in multiple measurement locations. The analysis of time and power-related parameters is supported and extended by a correlation analysis between each pair of parameters. The results show that beamforming strategy has a marked impact on propagation. A single wide-beam transmission leads, in fact, to lower RMS delay spread and lower mean excess delay compared to a multiple SSB-based transmission strategy. In addition, the single wide-beam transmission system is characterized by a smaller path loss and a higher K-factor, suggesting that the adoption of a multiple SSB-based transmission strategy may have a negative impact on downlink performance.

    • Understanding radio propagation characteristics and developing channel models is fundamental to building and operating wireless communication systems. Among others uses, channel characterization and modeling can be used for coverage and performance analysis and prediction. Within this context, this paper describes a comprehensive dataset of channel measurements performed to analyze outdoor-to-indoor propagation characteristics in the mid-band spectrum identified for the operation of 5th Generation (5G) cellular systems. Previous efforts to analyze outdoor-to-indoor propagation characteristics in this band were made by using measurements collected on dedicated, mostly single-link setups. Hence, measurements performed on deployed and operational 5G networks still lack in the literature. To fill this gap, this paper presents a dataset of measurements performed over commercial 5G networks. In particular, the dataset includes measurements of channel power delay profiles from two 5G networks in Band n78, i.e., 3.3–3.8 GHz. Such measurements were collected at multiple locations in a large office building in the city of Rome, Italy by using the Rohde & Schwarz (R&S) TSMA6 network scanner during several weeks in 2020 and 2021. A primary goal of the dataset is to provide an opportunity for researchers to investigate a large set of 5G channel measurements, aiming at analyzing the corresponding propagation characteristics toward the definition and refinement of empirical channel propagation models.

    • Introduction: Atrial fibrillation is an independent risk factor of thromboembolism. Women with atrial fibrillation are at a higher overall risk for stroke compared to men with atrial fibrillation. The aim of this study was to evaluate for sex differences in patients with acute stroke and atrial fibrillation, regarding risk factors, treatments received and outcomes. Methods: Data were analyzed from the ‘‘Recurrence and Cerebral Bleeding in Patients with Acute Ischemic Stroke and Atrial Fibrillation’’ (RAF-study), a prospective, multicenter, international study including only patients with acute stroke and atrial fibrillation. Patients were followed up for 90 days. Disability was measured by the modified Rankin Scale (0–2 favorable outcome, 3–6 unfavorable outcome). Results: Of the 1029 patients enrolled, 561 were women (54.5%) (p<0.001) and younger (p<0.001) compared to men. In patients with known atrial fibrillation, women were less likely to receive oral anticoagulants before index stroke (p¼0.026) and were less likely to receive anticoagulants after stroke (71.3% versus 78.4%, p¼0.01). There was no observed sex difference regarding the time of starting anticoagulant therapy between the two groups (6.411.7 days for men versus 6.512.4 days for women, p¼0.902). Men presented with more severe strokes at onset (mean NIHSS 9.26.9 versus 8.17.5, p<0.001). Within 90 days, 46 (8.2%) recurrent ischemic events (stroke/TIA/systemic embolism) and 19 (3.4%) symptomatic cerebral bleedings were found in women compared to 30 (6.4%) and 18 (3.8%) in men (p¼0.28 and p¼0.74). At 90 days, 57.7% of women were disabled or deceased, compared to 41.1% of the men (p<0.001). Multivariate analysis did not confirm this significance. Conclusions: Women with atrial fibrillation were less likely to receive oral anticoagulants prior to and after stroke compared to men with atrial fibrillation, and when stroke occurred, regardless of the fact that in our study women were younger and with less severe stroke, outcomes did not differ between the sexes.

    • Fifth Generation (5G) networks have been operational worldwide for a couple of years. To reveal how the 5G system evolution (e.g., changes in network conditions, deployment, and configurations) affects user performance, empirical long-term analyses are required. This paper presents preliminary insights from our ongoing large-scale measurement study of the commercial 5G non-standalone (NSA) networks deployed in Rome, Italy. An initial comparison between the measurements in 2020-2021 vs. 2023 shows a decrease in throughput and latency performance, calling for deeper analyses toward understanding the root causes and deriving proper optimization solutions.

    • The lack of publicly available large scale measurements has hindered the derivation of empirical path loss (PL) models for Narrowband Internet of Things (NB-IoT). Therefore, simulation-based investigations currently rely on models conceived for other cellular technologies, which are characterized, however, by different available bandwidth, carrier frequency, and infrastructure deployment, among others. In this paper, we take advantage of data from a large scale measurement campaign in the city of Oslo, Norway, to provide the first empirical characterization of NB-IoT PL in an urban scenario. For the PL average term, we characterize Alpha-Beta-Gamma (ABG) and Close-In (CI) models. By analyzing multiple NBIoT cells, we propose a statistical PL characterization, i.e., the model parameters are not set to a single, constant value across cells, but are randomly extracted from well-known distributions. Similarly, we define the PL shadowing distribution, correlation over distance, and inter-site correlation. Finally, we give initial insights on outdoor-to-indoor propagation, using measurements up to deep indoor scenarios. The proposed models improve PL estimation accuracy compared to the ones currently adopted in NB-IoT investigations, enabling more realistic simulations of urban scenarios similar to the sites covered by our measurements.

    • In the context of massive Machine Type Communications (mMTC), the Narrowband Internet of Things (NB-IoT) technology is envisioned to efficiently and reliably deal with massive device connectivity. Hence, it relies on a tailored Random Access (RA) procedure, for which theoretical and empirical analyses are needed for a better understanding and further improvements. This paper presents the first data-driven analysis of NB-IoT RA, exploiting a large scale measurement campaign. We show how the RA procedure and performance are affected by network deployment, radio coverage, and operators’ configurations, thus complementing simulation-based investigations, mostly focused on massive connectivity aspects. Comparison with the performance requirements reveals the need for procedure enhancements. Hence, we propose a Machine Learning (ML) approach, and show that RA outcomes are predictable with good accuracy by observing radio conditions. We embed the outcome prediction in a RA enhanced scheme, and show that optimized configurations enable a power consumption reduction of at least 50%. We also make our dataset available for further exploration, toward the discovery of new insights and research perspectives.

    • User-centric radio access technology (RAT) selection is a key communication paradigm, given the increased number of available RATs and increased cognitive capabilities at the user end. When considered against traditional network-centric approaches, user-centric RAT selection results in reduced network-side management load, and leads to lower operational costs for RATs, as well as improved quality of service (QoS) and quality of experience (QoE) for users. The complex between-users interactions involved in RAT selection require, however, specific analyses, toward developing reliable and efficient schemes. Two theoretical frameworks are most often applied to user-centric RAT selection analysis, i.e., game theory (GT) and multi-agent learning (MAL). As a consequence, several GT models and MAL algorithms have been recently proposed to solve the problem at hand. A comprehensive discussion of such models and algorithms is, however, currently missing. Moreover, novel issues introduced by next-generation communication systems also need to be addressed. This paper proposes to fill the above gaps by providing a unified reference for both ongoing research and future research directions in the field. In particular, the review addresses the most common GT and MAL models and algorithms, and scenario settings adopted in user-centric RAT selection in terms of utility function and network topology. Regarding GT, the review focuses on non-cooperative models, because of their widespread use in RAT selection; as for MAL, a large number of algorithms are described, ranging from game-theoretic to reinforcement learning (RL) schemes, and also including most recent approaches, such as deep RL (DRL) and multi-armed bandit (MAB). Models and algorithms are analyzed by comparatively reviewing relevant literature. Finally, open challenges are discussed, in light of ongoing research and standardization activities.

Increasingly, IoT services and applications require location information to be paired with data collected by devices; NB-IoT still lacks, however, reliable positioning methods. Time-based techniques inherited from Long Term Evolution (LTE) are not yet widely available in existing networks, and are expected to perform poorly on NB-IoT signals due to their narrow bandwidth. This investigation proposes a set of strategies for NB-IoT positioning, based on fingerprinting, that use coverage and radio information from multiple cells. The proposed strategies are evaluated on a large-scale dataset that includes experimental data from two NB-IoT operators. Results show that the proposed strategies, using a combination of coverage and radio information from multiple cells, outperform current state-of-the-art approaches based on single cell finger-printing, with a minimum average positioning error of about 20 meters, consistent across different network scenarios, vs. about 70 meters for current state-of-the-art.

Increasingly, IoT services and applications require location information to be paired with data collected by devices; NB-IoT still lacks, however, reliable positioning methods. Time-based techniques inherited from long-term evolution (LTE) are not yet widely available in existing networks and are expected to perform poorly on NB-IoT signals due to their narrow bandwidth. This investigation proposes a set of strategies for NB-IoT positioning based on fingerprinting that use coverage and radio information from multiple cells. The proposed strategies were evaluated on two large-scale datasets made available under an open-source license that include experimental data from multiple NB-IoT operators in two large cities: Oslo, Norway, and Rome, Italy. Results showed that the proposed strategies, using a combination of coverage and radio information from multiple cells, outperform current state-of-the-art approaches based on single cell fingerprinting, with a minimum average positioning error of about 20 m when using data for a single operator that was consistent across the two datasets vs. about 70 m for the current state-of-the-art approaches. The combination of data from multiple operators and data smoothing further improved positioning accuracy, leading to a minimum average positioning error below 15 m in both urban environments.

  • A Modular Experimentation Methodology for 5G Deployments : The 5GENESIS Approach

    • The high heterogeneity of 5G use cases requires the extension of the traditional per-component testing procedures provided by certification organizations, in order to devise and incorporate methodologies that cover the testing requirements from vertical applications and services. In this paper, we introduce an experimentation methodology that is defined in the context of the 5GENESIS project, which aims at enabling both the testing of network components and validation of E2E KPIs. The most important contributions of this methodology are its modularity and flexibility, as well as the open-source software that was developed for its application, which enable lightweight adoption of the methodology in any 5G testbed. We also demonstrate how the methodology can be used, by executing and analyzing different experiments in a 5G Non-Standalone (NSA) deployment at the University of Malaga. The key findings of the paper are an initial 5G performance assessment and KPI analysis and the detection of under-performance issues at the application level. Those findings highlight the need for reliable testing and validation procedures towards a fair benchmarking of generic 5G services and applications.

    • This deliverable presents the second cycle of trials and experimentation activities executed over 5GENESIS facilities. The document is the continuation of deliverable D6.1, in the sense that it captures tests carried out over the evolved infrastructures hosting 5GENESIS facilities following the methodology defined in D6.1. In this document 8 main KPIs and 4 application specific validation trials achieved, under 123 experiments that performed in total. The tests focus more on i) the evolved 5G infrastructure deployments that includes radio and core elements in non-standalone (NSA) deployment configurations based on commercial and open implementations, and ii) the use of Open 5GENESIS Suite for the execution of the tests.

    • BACKGROUND: The Global Burden of Disease, Injuries, and Risk Factor study 2013 (GBD 2013) is the first of a series of annual updates of the GBD. Risk factor quantification, particularly of modifiable risk factors, can help to identify emerging threats to population health and opportunities for prevention. The GBD 2013 provides a timely opportunity to update the comparative risk assessment with new data for exposure, relative risks, and evidence on the appropriate counterfactual risk distribution.METHODS: Attributable deaths, years of life lost, years lived with disability, and disability-adjusted life-years (DALYs) have been estimated for 79 risks or clusters of risks using the GBD 2010 methods. Risk-outcome pairs meeting explicit evidence criteria were assessed for 188 countries for the period 1990-2013 by age and sex using three inputs: risk exposure, relative risks, and the theoretical minimum risk exposure level (TMREL). Risks are organised into a hierarchy with blocks of behavioural, environmental and occupational, and metabolic risks at the first level of the hierarchy. The next level in the hierarchy includes nine clusters of related risks and two individual risks, with more detail provided at levels 3 and 4 of the hierarchy. Compared with GBD 2010, six new risk factors have been added: handwashing practices, occupational exposure to trichloroethylene, childhood wasting, childhood stunting, unsafe sex, and low glomerular filtration rate. For most risks, data for exposure were synthesised with a Bayesian meta-regression method, DisMod-MR 2.0, or spatial-temporal Gaussian process regression. Relative risks were based on meta-regressions of published cohort and intervention studies. Attributable burden for clusters of risks and all risks combined took into account evidence on the mediation of some risks such as high body-mass index (BMI) through other risks such as high systolic blood pressure and high cholesterol.FINDINGS: All risks combined account for 57·2% (95% uncertainty interval [UI] 55·8-58·5) of deaths and 41·6% (40·1-43·0) of DALYs. Risks quantified account for 87·9% (86·5-89·3) of cardiovascular disease DALYs, ranging to a low of 0% for neonatal disorders and neglected tropical diseases and malaria. In terms of global DALYs in 2013, six risks or clusters of risks each caused more than 5% of DALYs: dietary risks accounting for 11·3 million deaths and 241·4 million DALYs, high systolic blood pressure for 10·4 million deaths and 208·1 million DALYs, child and maternal malnutrition for 1·7 million deaths and 176·9 million DALYs, tobacco smoke for 6·1 million deaths and 143·5 million DALYs, air pollution for 5·5 million deaths and 141·5 million DALYs, and high BMI for 4·4 million deaths and 134·0 million DALYs. Risk factor patterns vary across regions and countries and with time. In sub-Saharan Africa, the leading risk factors are child and maternal malnutrition, unsafe sex, and unsafe water, sanitation, and handwashing. In women, in nearly all countries in the Americas, north Africa, and the Middle East, and in many other high-income countries, high BMI is the leading risk factor, with high systolic blood pressure as the leading risk in most of Central and Eastern Europe and south and east Asia. For men, high systolic blood pressure or tobacco use are the leading risks in nearly all high-income countries, in north Africa and the Middle East, Europe, and Asia. For men and women, unsafe sex is the leading risk in a corridor from Kenya to South Africa.INTERPRETATION: Behavioural, environmental and occupational, and metabolic risks can explain half of global mortality and more than one-third of global DALYs providing many opportunities for prevention. Of the larger risks, the attributable burden of high BMI has increased in the past 23 years. In view of the prominence of behavioural risk factors, behavioural and social science research on interventions for these risks should be strengthened.   • Multi-Timescale Evaluation of Starlink Throughput

    • Although Starlink has been rolled-out for several years, there is still a lack of knowledge regarding system details and performance characteristics. To address this, we perform a network layer measurement campaign utilizing precise times-Tamping to analyze throughput variations at multiple time scales, and infer system timing details. On larger timescales we quantify the diurnal variations where the throughput varies with the time of day. On the medium timescales we establish the likely frequency allocation and beam switching period to be 15 seconds. The associated connectivity disturbances contribute to severe link underutilization for single long-lived TCP flows, which typically reach only 46% of the estimated link capacity. On the sub-millisecond timescale our network layer measurements corroborate recent physical layer investigations of the Starlink frame timing, which is confirmed to be 1.33 ms. © 2023 Owner/Author(s).

    • Mobile networks are highly complex systems. Therefore, it is crucial to examine them from an empirical perspective to better understand how network features affect performance, so to suggest additional improvements. To this aim, this paper presents a large-scale dataset of measurements collected over fourth generation (4G) and fifth generation (5G) operational networks, providing Long Term Evolution (LTE), Narrowband Internet of Things (NB-IoT), and 5G New Radio (NR) connectivity. We collected our dataset during seven weeks in Rome, Italy, by performing several tests on the infrastructures of two major mobile network operators (MNOs). The open-sourced dataset has enabled multi-faceted analyses of network deployment, coverage, and end-user performance, and can be further used for designing and testing artificial intelligence (AI) and machine learning (ML) solutions for network optimization.

    • Narrowband Internet of Things (NB-IoT) is gaining momentum as a promising technology for massive machine type communication. Given that its deployment is rapidly progressing worldwide, measurement campaigns and performance analyses are needed to better understand the system and move toward its enhancement. With this aim, this article presents a large-scale measurement campaign and empirical analysis of NB-IoT on operational networks, and discloses valuable insights in terms of deployment strategies and radio coverage performance. The reported results also serve as examples showing the potential usage of the collected dataset, which we make open source along with a lightweight data visualization platform.

We leverage the collected dataset, which covers two Mobile Network Operators (MNOs) in Rome, Italy, to first, study network deployment and radio coverage aspects, and second, explore the performance of two use cases related to enhanced Mobile Broadband (eMBB) and Ultra-Reliable Low Latency Communication (URLLC). Results show that 5G NSA can provide higher downlink throughput and slightly lower latency compared to Fourth Generation (4G) systems. However, performance is influenced by several factors, including propagation conditions, system configurations, and handover events, ultimately highlighting the need for further system optimization.

In this paper, we present results and insights from our large-scale measurement study on commercial 5G Non Standalone (NSA) deployments in a European country. We leverage the collected dataset, which covers two MNOs in Rome, Italy, to study network deployment and radio coverage aspects, and explore the performance of two use cases related to enhanced Mobile Broadband (eMBB) and Ultra-Reliable Low Latency Communication (URLLC). We further leverage a machine learning (ML)-based approach to model the Dual Connectivity (DC) feature enabled by 5G NSA. Our data-driven analysis shows that 5G NSA can provide higher downlink throughput and slightly lower latency compared to 4G. However, performance is influenced by several factors, including propagation conditions, system configurations, and handovers, ultimately highlighting the need for further system optimization. Moreover, by casting the DC modeling problem into a classification problem, we compare four supervised ML algorithms and show that a high model accuracy (up to 99%) can be achieved, in particular, when several radio coverage indicators from both access networks are used as input. Finally, we conduct analyses towards aiding the explainability of the ML models.

    • Fifth Generation (5G) cellular networks are gaining popularity and are soon expected to become the norm in the global telecommunications industry. In this paper, we follow an experimental approach to study key aspects of Handover (HO) events in commercial 5G Non Standalone (NSA) deployments. We collect our dataset by conducting a large-scale measurement campaign in the city of Rome, Italy. With our analysis, we aim to first, empirically evaluate the current HO strategies followed in 5G NSA, and second, to show the impact of HOs on end-user application performance. Our results show an increased number of intra-RAT HOs, i.e., HOs between cells with the same Radio Access Technology (RAT), in situation where signal quality conditions between multiple candidate cells is on par. In addition, we show that inter-RAT HOs, i.e., HOs between cells with different RATs, can be detrimental to the application performance.

    • Motivated by recent theoretical challenges for 5G, this study aims to position relevant results in the literature on codedomain non-orthogonal multiple access (NOMA) from an information-theoretic perspective, given that most of the recent intuition of NOMA relies on another domain, that is, the power domain. Theoretical derivations for several code-domain NOMA schemes are reported and interpreted, adopting a unified framework that focuses on the analysis of the NOMA spreading matrix, in terms of load, sparsity, and regularity features. The comparative analysis shows that it is beneficial to adopt extreme low-dense code-domain NOMA in the large system limit, where the number of resource elements and number of users grow unboundedly while their ratio, called load, is kept constant. Particularly, when optimum receivers are used, the adoption of a regular low-dense spreading matrix is beneficial to the system achievable rates, which are higher than those obtained with either irregular low-dense or dense formats, for any value of load. For linear receivers, which are more favourable in practice due to lower complexity, the regular low-dense NOMA still has better performance in the underloaded regime (load < 1), while the irregular counterpart outperforms all the other schemes in the overloaded scenario (load > 1).

    • Aims: Recent trial data demonstrate beneficial effects of active rhythm management in patients with atrial fibrillation (AF) and support the concept that a low arrhythmia burden is associated with a low risk of AF-related complications. The aim of this document is to summarize the key outcomes of the 9th AFNET/EHRA Consensus Conference of the Atrial Fibrillation NETwork (AFNET) and the European Heart Rhythm Association (EHRA).Methods and results: Eighty-three international experts met in Munster for 2 days in September 2023. Key findings are as follows: (i) Active rhythm management should be part of the default initial treatment for all suitable patients with AF. (ii) Patients with device-detected AF have a low burden of AF and a low risk of stroke. Anticoagulation prevents some strokes and also increases major but non-lethal bleeding. (iii) More research is needed to improve stroke risk prediction in patients with AF, especially in those with a low AF burden. Biomolecules, genetics, and imaging can support this. (iv) The presence of AF should trigger systematic workup and comprehensive treatment of concomitant cardiovascular conditions. (v) Machine learning algorithms have been used to improve detection or likely development of AF. Cooperation between clinicians and data scientists is needed to leverage the potential of data science applications for patients with AF.Conclusions: Patients with AF and a low arrhythmia burden have a lower risk of stroke and other cardiovascular events than those with a high arrhythmia burden. Combining active rhythm control, anticoagulation, rate control, and therapy of concomitant cardiovascular conditions can improve the lives of patients with AF. Graphical Abstract
