    • We present bright [C ii] 158 mu m line detections from a strongly magnified and multiply imaged (mu similar to 20-160) sub-L* (MUV=-19.75-0.44+0.55) Lyman-break galaxy (LBG) at z = 6.0719 +/- 0.0004, drawn from the ALMA Lensing Cluster Survey (ALCS). Emission lines are identified at 268.7 GHz at >= 8 sigma exactly at the positions of two multiple images of the LBG, behind the massive galaxy cluster RXCJ0600-2007. Our lens models, updated with the latest spectroscopy from VLT/MUSE, indicate that a sub region of the LBG crosses the caustic, and is lensed into a long (similar to 6 '') arc with a local magnification of mu similar to 160, for which the [C ii] line is also significantly detected. The source plane reconstruction resolves the interstellar medium (ISM) structure, showing that the [C ii] line is co-spatial with the rest-frame UV continuum at a scale of similar to 300 pc. The [C ii] line properties suggest that the LBG is a rotation-dominated system, whose velocity gradient explains a slight difference in redshifts between the whole LBG and its sub-region. The star formation rate (SFR)-L-[CII] relations, for whole and sub-regions of the LBG, are consistent with those of local galaxies. We evaluate the lower limit of the faint-end of the [C ii] luminosity function at z = 6, finding it to be consistent with predictions from semi-analytical models and from the local SFR-L-[CII] relation with a SFR function at z = 6. These results imply that the local SFR-L-[CII] relation is universal for a wide range of scales, including the spatially resolved ISM, the whole region of the galaxy, and the cosmic scale, even in the epoch of reionization.

    • Context. The properties of galaxies at redshift z>6 hold the key to our understanding of the early stages of galaxy evolution and can potentially identify the sources of the ultraviolet radiation that give rise to the epoch of reionisation. The far-infrared cooling line of [C II] at 158 mu m is known to be bright and correlate with the star formation rate (SFR) of low-redshift galaxies, and hence is also suggested to be an important tracer of star formation and interstellar medium properties for very high-redshift galaxies. Aims. With the aim to study the interstellar medium properties of gravitationally lensed galaxies at z>6, we search for [C II] and thermal dust emission in a sample of 52 > 6 galaxies observed by the ALMA Lensing Cluster Survey. Methods. We perform our analysis using LINESTACKER, stacking both [C II] and continuum emission. The target sample is selected from multiple catalogues, and the sample galaxies have spectroscopic redshift or low-uncertainty photometric redshifts (sigma(z)<0.02) in nine galaxy clusters. Source properties of the target galaxies are either extracted from the literature or computed using spectral energy distribution fitting. Both weighted-average and median stacking are used, on both the full sample and three sub-samples. Results. Our analyses find no detection of either [C II] or continuum. An upper limit on L-[CII] is derived, implying that [C II] remains marginally consistent for low-SFR z>6 galaxies but likely is under-luminous compared to the local L-[CII]-SFR relationship. We discuss potential biases and possible physical effects that may be the cause of the non-detection. Further, the upper limit on the dust continuum implies that less than half of the star formation is obscured.

  • A Delphi Consensus on Patient Reported Outcomes for Registries and Trials Including Patients with Intermittent Claudication: Recommendations and Reporting Standard.

    • This study aimed to develop a core set of patient reported outcome quality indicators (QIs) for the treatment of patients with intermittent claudication (IC), that allow a broad international implementation across different vascular registries and within trials.A rigorous modified two stage Delphi technique was used to promote consensus building on patient reported outcome QIs among an expert panel consisting of international vascular specialists, patient representatives, and registry members of the VASCUNET and the International Consortium of Vascular Registries. Potential QIs identified through an extensive literature search or additionally proposed by the panel were validated by the experts in a preliminary survey and included for evaluation. Consensus was reached if ≥ 80% of participants agreed that an item was both clinically relevant and practical.Participation rates in two Delphi rounds were 66% (31 participants of 47 invited) and 90% (54 of 60), respectively. Initially, 145 patient reported outcome QIs were documented. Following the two Delphi rounds, 18 quality indicators remained, all of which reached consensus regarding clinical relevance. The VascuQoL questionnaire (VascuQoL-6), currently the most common patient reported outcome measurement (PROM) used within vascular registries, includes a total of six items. Five of these six items also matched with high rated indicators identified in the Delphi study. Consequently, the panel recommends the use of the VascuQoL-6 survey as a preferred core PROM QI set as well as an optional extension of 12 additional patient reported QIs that were also identified in this study.The current recommendation based on the Delphi consensus building approach, strengthens the international harmonisation of registry data collection in relation to patient reported outcome quality. Continuous and standardised quality assurance will ensure that registry data may be used for future quality benchmarking studies and, ultimately, positively impact the overall quality of care provided to patients with peripheral arterial occlusive disease.

    • Objective: To develop a minimum core data set for evaluation of acute limb ischaemia (ALI) revascularisation treatment and outcomes that would enable collaboration among international registries. Methods: A modified Delphi approach was used to achieve consensus among international multidisciplinary vascular specialists and registry members of the International Consortium of Vascular Registries (ICVR). Variables identified in the literature or suggested by the expert panel, and variables, including definitions, currently used in 15 countries in the ICVR, were assessed to define both a minimum core and an optimum data set to register ALI treatment. Clinical relevance and practicability were both assessed, and consensus was defined as >= 80% agreement among participants. Results: Of 40 invited experts, 37 completed a preliminary survey and 31 completed the two subsequent Delphi rounds via internet exchange and face to face discussions. In total, 117 different items were generated from the various registry data forms, an extensive review of the literature, and additional suggestions from the experts, for potential inclusion in the data set. Ultimately, 35 items were recommended for inclusion in the minimum core data set, including 23 core items important for all registries, and an additional 12 more specific items for registries capable of capturing more detail. These 35 items supplement previous data elements recommended for registering chronic peripheral arterial occlusive disease treatment. Conclusion: A modified Delphi study allowed 37 international vascular registry experts to achieve a consensus recommendation for a minimum core and an optimum data set for registries covering patients who undergo ALI revascularisation. Continued global harmonisation of registry infrastructure and definition of items allows international comparisons and global quality improvement. Furthermore, it can help to define and monitor standards of care and enable international research collaboration.

    • We present a kinematic analysis of the dense gas in the central 200 parsecs of thenearby galaxy NGC1097, based on Cycle 0 observations with the Atacama LargeMillimeter/sub-millimeter Array (ALMA). We use the HCN(4-3) line to trace the densest interstellar molecular gas (nH2 ~ 10^8 cm-3), and quantify its kinematics by means of Fourier decomposition. We find a striking similarity between the ALMA kinematic data and the analytic spiral in ow model that we have previously constructed based onionized gas velocity fields on larger scales. We are able to follow dense gas streamingdown to 40 pc distance from the supermassive black hole in this Seyfert 1 galaxy. In order to fulll marginal stability, we deduce that the dense gas is conned to a very thin disc, with 6.0+2.2-2.7 10^6 Msun dynamical mass inside a radius of 40 pc. Finally, we derive a dense gas in ow rate of 0.09Msun yr-1 at 40 pc radius. Combined with previous valuesfrom the H and CO gas, we calculate a combined molecular and ionized gas in ow rateof 0.2Msun yr-1 at 40 pc distance from the central supermassive black hole of NGC1097.

    • We present an unprecedented measurement of the disk stability and local instability scales in the luminous infrared Seyfert 1 host, NGC 7469, based on ALMA observations of dense gas tracers and with a synthesized beam of 165 x 132 pc. While we confirm that non-circular motions are not significant in redistributing the dense interstellar gas in this galaxy, we find compelling evidence that the dense gas is a suitable tracer for studying the origin of its intensely high-mass star-forming ringlike structure. Our derived disk stability parameter Q accounts for a thick disk structure, and its value falls below unity at the radii in which intense star formation is found. Furthermore, we derive the characteristic instability scale lambda(c) and find a striking agreement between our measured scale of similar to 180 pc and the typical sizes of individual complexes of young and massive star clusters seen in high-resolution images.

  • A variable active galactic nucleus at z = 2.06 triply-imaged by the galaxy cluster MACS J0035.4−2015

    • We report the discovery of a triply imaged active galactic nucleus (AGN), lensed by the galaxy cluster MACS J0035.4−2015 (z d = 0.352). The object is detected in Hubble Space Telescope imaging taken for the RELICS program. It appears to have a quasi-stellar nucleus consistent with a point-source, with a de-magnified radius of re ≲ 100 pc. The object is spectroscopically confirmed to be an AGN at z spec = 2.063 ± 0.005 showing broad rest-frame UV emission lines, and detected in both X-ray observations with Chandra and in ALCS ALMA band 6 (1.2 mm) imaging. It has a relatively faint rest-frame UV luminosity for a quasar-like object, MUV, 1450 = −19.7 ± 0.2. The object adds to just a few quasars or other X-ray sources known to be multiply lensed by a galaxy cluster. Some diffuse emission from the host galaxy is faintly seen around the nucleus, and there is a faint object nearby sharing the same multiple-imaging symmetry and geometric redshift, possibly an interacting galaxy or a star-forming knot in the host. We present an accompanying lens model, calculate the magnifications and time delays, and infer the physical properties of the source. We find the rest-frame UV continuum and emission lines to be dominated by the AGN, and the optical emission to be dominated by the host galaxy of modest stellar mass M✶ ≃ 109.2 M⊙. We also observe some variation in the AGN emission with time, which may suggest that the AGN used to be more active. This object adds a low-redshift counterpart to several relatively faint AGN recently uncovered at high redshifts with HST and JWST.

  • Thoracic endovascular aortic repair practice in 13 countries : A report from VASCUNET and the International Consortium of Vascular Registries

    • OBJECTIVE: To assess practice patterns and short-term outcome after TEVAR, based on an international vascular registry collaboration.SUMMARY BACKGROUND DATA: Thoracic endovascular aortic repair (TEVAR) has become the primary surgical treatment modality for descending aortic pathologies, and has expanded to new patient cohorts, including the elderly.METHODS: Data on thoracic aortic aneurysms (TAA), type B aortic dissections (TBAD), and traumatic aortic injuries (TAI) treated with TEVAR from 2012 to 2016 were retrieved from registries and centers in 13 countries.RESULTS: 9518 TEVAR for TAA (n = 4436), TBAD (n = 3976) and TAI (n = 1106) were included. The distribution of TEVAR procedures per pathology varied, with TAA repair constituting from 40% of TEVARs in the US to 72% in the UK (p < 0.001).Mean intact TAA (iTAA) diameter varied from 59 (US) to 69 mm (Nancy, France) (p < 0.001), 25.3% of patients having a diameter of < 60 mm. Perioperative mortality after iTAA repair was 4.9%; combined mortality, stroke, paraplegia and RRT outcome was 12.8%. 18.6% of iTAA patients were ≥80 years old. Mortality was higher in this group (7.2%) than in patients < 80 (3.8%) (p < 0.001). After rTAA repair, perioperative mortality was 26.8%.Mortality was 9.7% after acute (within 14 days from onset of dissection) and 3.0% after chronic TBAD repair (p < 0.001). Mortality after TAI was 7.8%, and depended on injury severity (grade IV (free rupture) 20.9%).CONCLUSIONS:: This registry collaboration provides a unique platform to evaluate cross-border patterns of use and outcomes of TEVAR. A common core dataset is proposed, to achieve harmonization of registry-based quality outcome measures for TEVAR.

  • ALMA OBSERVATIONS OF THE SUBMILLIMETER DENSE MOLECULAR GAS TRACERS IN THE LUMINOUS TYPE-1 ACTIVE NUCLEUS OF NGC 7469

    • We present Atacama Large Millimeter/submillimeter Array (ALMA) Cycle 1 observations of the central kiloparsec region of the luminous type. 1 Seyfert galaxy NGC 7469 with unprecedented high resolution (0.'' 5x0.'' 4 = 165 x 132 pc) at submillimeter wavelengths. Utilizing the wide. bandwidth of ALMA, we simultaneously obtained HCN(4-3), HCO+(4-3), CS(7-6), and partially CO(3-2) line maps, as well as the 860 mu m continuum. The region consists of the central similar to 1 '' component and the surrounding starburst ring with a radius of similar to 1.'' 5-2.'' 5. Several structures connect these components. Except for CO(3-2), these dense gas tracers are significantly concentrated toward the central similar to 1 '', suggesting their suitability to probe the nuclear regions of galaxies. Their spatial distribution resembles well those of centimeter and mid-infrared continuum emissions, but it is anticorrelated with the optical one, indicating the existence of dust-obscured star formation. The integrated intensity ratios of HCN(4-3)/HCO+(4-3) and HCN(4-3)/CS(7-6) are higher at the active galactic nucleus (AGN) position than at the starburst ring, which is consistent with our previous findings (submillimeter-HCN enhancement). However, the HCN(4-3)/HCO+(4-3) ratio at the AGN position of NGC 7469 (1.11 +/- 0.06) is almost half of the corresponding value of the low-luminosity type. 1 Seyfert galaxy NGC 1097 (2.0 +/- 0.2), despite the more than two orders of magnitude higher X-ray luminosity of NGC 7469. But the ratio is comparable to that of the close vicinity of the AGN of NGC 1068 (similar to 1.5). Based on these results, we speculate that some heating mechanisms other than X-ray (e.g., mechanical heating due to an AGN jet) can contribute significantly for shaping the chemical composition in NGC 1097.

    • We present the first 100 pc scale view of the dense molecular gas in the central similar to 1.3 kpc of the type-1 Seyfert NGC 1097, traced by HCN (J = 4-3) and HCO+ (J = 4-3) lines afforded with ALMA band 7. This galaxy shows significant HCN enhancement with respect to HCO+ and CO in the low-J transitions, which seems to be a common characteristic in AGN environments. Using the ALMA data, we consider the characteristics of the dense gas around this AGN, and search for the mechanism of HCN enhancement. We find a high HCN (J = 4-3) to HCO+ (J = 4-3) line ratio in the nucleus. The upper limit of the brightness temperature ratio of HCN (nu(2) = 1(1f), J = 4-3) to HCN (J = 4-3) is 0.08, which indicates that IR pumping does not significantly affect the pure rotational population in this nucleus. We also find a higher HCN (J = 4-3) to CS (J = 7-6) line ratio in NGC 1097 than in starburst galaxies, which is more than 12.7 on the brightness temperature scale. Combined with similar observations from other galaxies, we tentatively suggest that this ratio appears to be higher in AGN-host galaxies than in pure starburst ones, similar to the widely used HCN to HCO+ ratio. LTE and non-LTE modeling of the observed HCN and HCO+ lines using J = 4-3 and 1-0 data from ALMA, and J = 3-2 data from SMA, reveals a high HCN to HCO+ abundance ratio (5 <= [HCN]/[HCO+] <= 20: non-LTE analysis) in the nucleus, and that the high-J lines (J = 4-3 and 3-2) are emitted from dense (10(4.5) cm(-3) <= n(H2) <= 10(6) cm(-3)), hot (70 K <= T-kin <= 550 K) regions. Finally we propose that high-temperature chemistry is more plausible to explain the observed enhanced HCN emission in NGC 1097 than pure gas-phase PDR/XDR chemistry.

  • SUBMILLIMETER-HCN DIAGRAM FOR ENERGY DIAGNOSTICS IN THE CENTERS OF GALAXIES

    • Compiling data from literature and the Atacama Large Millimeter/submillimeter Array archive, we show enhanced HCN(4-3)/HCO+(4-3) and/or HCN(4-3)/CS(7-6) integrated intensity ratios in circumnuclear molecular gas around active galactic nuclei (AGNs) compared to those in starburst (SB) galaxies (submillimeter HCN. enhancement). The number of sample galaxies is significantly increased from our previous work. We expect that this feature could potentially be an extinction-free energy diagnostic tool of nuclear regions of galaxies. Non-LTE radiative transfer modelings of the above molecular emission lines involving both collisional and radiative excitation, as well as a photon trapping effect, were conducted to investigate the cause of the high line ratios in AGNs. As a result, we found that enhanced abundance ratios of HCN to HCO+ and HCN to CS in AGNs as compared to SB galaxies by a factor of a few to even greater than or similar to 10 are a plausible explanation for the submillimeter HCN. enhancement. However, a counterargument of a systematically higher gas density in AGNs than in SB galaxies can also be a plausible scenario. Although we cannot fully distinguish. these two scenarios at this moment owing to an insufficient amount of multi-transition, multi-species data, the former scenario is indicative of abnormal chemical composition in AGNs. Regarding the actual mechanism to realize the composition, we suggest that it is difficult with conventional gas-phase X-ray-dominated region ionization models to reproduce the observed high line ratios. We might have to take into account other mechanisms such as neutral-neutral reactions that are efficiently activated in high-temperature environments and/or mechanically heated regions to further understand the high line ratios in AGNs.

  • Metal-enriched Neutral Gas Reservoir around a Strongly Lensed Low-mass Galaxy at z = 4 Identified by JWST/NIRISS and VLT/MUSE

    • Direct observations of low-mass, low-metallicity galaxies at z ≳ 4 provide an indispensable opportunity for detailed inspection of the ionization radiation, gas flow, and metal enrichment in sources similar to those that reionized the universe. Combining the James Webb Space Telescope (JWST), Very Large Telescope/MUSE, and Atacama Large Millimeter/submillimeter Array, we present detailed observations of a strongly lensed, low-mass (≈107.6 M ⊙) galaxy at z = 3.98 (also see Vanzella et al.). We identify strong narrow nebular emission, including C iv λ λ1548, 1550, He ii λ1640, O iii] λ λ1661, 1666, [Ne iii] λ3868, [O ii] λ3727, and the Balmer series of hydrogen from this galaxy, indicating a metal-poor H ii region (≲0.12 Z ⊙) powered by massive stars. Further, we detect a metal-enriched damped Lyα system (DLA) associated with the galaxy with the H i column density of N H I ≈ 1021.8 cm−2. The metallicity of the associated DLA may reach the supersolar metallicity (≳Z ⊙). Moreover, thanks to JWST and gravitational lensing, we present the resolved UV slope (β) map at the spatial resolution of ≈100 pc at z = 4, with steep UV slopes reaching β ≈ −2.5 around three star-forming clumps. Combining with low-redshift analogs, our observations suggest that low-mass, low-metallicity galaxies, which dominate reionization, could be surrounded by a high covering fraction of the metal-enriched, neutral-gaseous clouds. This implies that the metal enrichment of low-mass galaxies is highly efficient, and further supports that in low-mass galaxies, only a small fraction of ionizing radiation can escape through the interstellar or circumgalactic channels with low-column-density neutral gas.

    • ABSTRACT: BACKGROUND: We evaluated the effect of infliximab on markers of inflammation in patients with juvenile idiopathic arthritis (JIA). METHODS: In this randomized, placebo-controlled substudy, 122 patients with JIA received infliximab 3 mg/kg + methotrexate (MTX)(n = 60) or placebo + MTX (n = 62) at weeks 0, 2, and 6. At week 14, patients receiving placebo + MTX crossed over to infliximab 6 mg/kg + MTX; patients receiving infliximab 3 mg/kg + MTX continued treatment through week 44. Sera and plasma from eligible patients receiving infliximab 3 mg/kg + MTX (n = 34) and receiving placebo→infliximab 6 mg/kg +MTX (n = 38) were collected at weeks 0, 2, 14, 16, 28, and 52 and analyzed for inflammatory markers (IL-6, IL-12p40, ICAM-1, MMP-3, VEGF, TNF-α, and CRP). RESULTS: At week 2, decreases from baseline in IL-6, ICAM-1, MMP-3, TNF-α, and CRP were greater with infliximab versus placebo treatment, and with the exception of CRP, these differences were generally maintained through week 14. The decreases from baseline to week 52 in IL-6, ICAM-1, VEGF, MMP-3, and CRP and increases in IL-12p40 levels were larger in patients receiving placebo→infliximab 6 mg/kg +MTX versus infliximab 3 mg/kg + MTX treatment. Patients receiving infliximab 3 mg/kg+MTX who achieved an American College of Rheumatology Pediatric 30 (ACR-Pedi-30) response had significantly larger decreases from baseline in ICAM-1 (p = 0.0105) and MMP-3 (p = 0.0253) at week 2 and in ICAM-1 (p = 0.0304), MMP-3 (p = 0.0091), and CRP (p = 0.0011) at week 14 versus ACR-Pedi-30 nonresponders. CONCLUSION: Infliximab + MTX attenuated several inflammatory markers in patients with JIA; larger decreases in ICAM-1, MMP-3, and CRP levels were observed in ACR-Pedi-30 responders versus nonresponders.
