  • Determinants of morbidity and mortality following emergency abdominal surgery in children in low-income and middle-income countries

    • Background: Child health is a key priority on the global health agenda, yet the provision of essential and emergency surgery in children is patchy in resource-poor regions. This study was aimed to determine the mortality risk for emergency abdominal paediatric surgery in low-income countries globally.Methods: Multicentre, international, prospective, cohort study. Self-selected surgical units performing emergency abdominal surgery submitted prespecified data for consecutive children aged <16 years during a 2-week period between July and December 2014. The United Nation's Human Development Index (HDI) was used to stratify countries. The main outcome measure was 30-day postoperative mortality, analysed by multilevel logistic regression.Results: This study included 1409 patients from 253 centres in 43 countries; 282 children were under 2 years of age. Among them, 265 (18.8%) were from low-HDI, 450 (31.9%) from middle-HDI and 694 (49.3%) from high-HDI countries. The most common operations performed were appendectomy, small bowel resection, pyloromyotomy and correction of intussusception. After adjustment for patient and hospital risk factors, child mortality at 30 days was significantly higher in low-HDI (adjusted OR 7.14 (95% CI 2.52 to 20.23), p<0.001) and middle-HDI (4.42 (1.44 to 13.56), p=0.009) countries compared with high-HDI countries, translating to 40 excess deaths per 1000 procedures performed.Conclusions: Adjusted mortality in children following emergency abdominal surgery may be as high as 7 times greater in low-HDI and middle-HDI countries compared with high-HDI countries. Effective provision of emergency essential surgery should be a key priority for global child health agendas.

    • An efficient, straight-forward and eco-friendly synthetic strategy for the assembly of novel bisoxazolones via a four-component, sequential reaction of dialdehydes, glycine, benzoyl chloride and acetic anhydride, using ultrasound radiation, is described. Additionally, a diverse group of new bisimidazoles has been synthesized in good yields by the sonication of diamines and (Z)-4-arylidene-2-phenyloxazol-5(4H)-ones. These approaches have resulted in a number of successful routes for the facile synthesis of bis-oxazolone and bis-imidazole frameworks within minutes of irradiation. Excellent outcomes using these environmentally-friendly parameters make these synthetic schemes ideal, sustainable, green-chemistry procedures and provide simple access towards the preparation of bisheterocycles.     • In an effective and straightforward conversion, bis-semicarbazones and bis-thiosemicarbazones are transformed into a diversity of novel substituted bis-2-amino-1,3,4-oxadiazoles and bis-2-amino-1,3,4-thiadiazoles, respectively under ultrasonic irradiation. Bis-tetrazoles are obtained from the dialdehydes by sequential reaction with hydroxylamine hydrochloride, phosphorus pentoxide and sodium azide without isolation of the intermediates oximes and nitriles. All the reactions proceed cleanly and smoothly under mild conditions, with short reaction times and broad functional groups possibility. No side reactions were observed. [GRAPHICS]

    • Hierarchical porous zeolitic imidazolate frameworks nanoparticles (ZIF-8 NPs) were synthesized at room temperature via a template-free approach under dynamic conditions (stirring) using water as a solvent. The ZIF-8 NPs were evaluated as adsorbents for rare earth elements (La3+, Sm3+ and Dy3+). Adsorption equilibrium was reached after 7h and high adsorption capacities were obtained for dysprosium and samarium (430.4 and 281.1 mg g(-1), respectively) and moderate adsorption capacity for lanthanum (28.8 mg g(-1)) at a pH of 7.0. The high adsorption capacitiese, as well as the high stability of ZIF-8 NPs, make the hierarchical ZIF-8 materials as an efficient adsorbent for the recovery of La3+, Sm3+ and Dy3+ from aqueous solution.

    • The problem of water scarcity can be noticed clearly in the lined canals which provide the irrigation networks. Using porous structures like gabion weirs contributes as a part solution to this problem. In the current study, a laboratory flume was used to calculate the water depths upstream and downstream of the stepped gabion weir that is to be put inside it at a certain distance, and this flume comes with dimensions of 10 m long by 0.30 m wide and 0.50 m height. While the tested hydraulic model of the weir was built with dimensions of 0.30 m width by 0.40 m maximum height, and five lengths with different total distance of 0.88, 0.96, 1.08, 1.12, and 1.20 m respectively. The used gravel samples to fill the gabions were of monosize query gravel with diameters ranging between 0.0095-0.0140, 0.0140-0.0190, 0.0190-0.0250, 0.0250-0.0375, and 0.0375-0.0500 m in a respective way. While the values of discharge, measured during the experiments were in the range of 0.0007-0.0150 m3/s, and a total of 175 trial tests. This study achieved that the detention depth value decreases by increasing the diameter of the gravel sample used, but there is no effect of the gravel sample on the value of release depth, the different illustrated formulas for the detention and release depths maybe can be used usefully for design and scheduling actions in the field where it gave a reasonable matching between the measured and the calculated values of the studied depths, and finally, the errors percentage in an average value for both detention and release tested values were 5.278% and -0.265% respectively

    • Metal organic frameworks (MOFs) are porous crystalline materials that can be designed to act as selective adsorbents. Due to their high porosity they can possess very high adsorption capacities. However, overcoming the brittleness of these crystalline materials is a challenge for many industrial applications. In order to make use of MOFs for large-scale liquid phase separation processes they can be immobilized on solid supports. For this purpose, nanocellulose can be considered as a promising supporting material due to its high flexibility and biocompatibility. In this study a novel flexible nanocellulose MOF composite material was synthesised in aqueous media by a novel and straightforward in situ one-pot green method. The material consisted of MOF particles of the type MIL-100(Fe) (from Material Institute de Lavoisier, containing Fe(III) 1,3,5-benzenetricarboxylate) immobilized onto bacterial cellulose (BC) nanofibers. The novel nanocomposite material was applied to efficiently separate arsenic and Rhodamine B from aqueous solution, achieving adsorption capacities of 4.81, and 2.77 mg g‒1, respectively. The adsorption process could be well modelled by the nonlinear pseudo-second-order fitting.

    • The final repository for short-lived, low and intermediate level radioactive waste in Sweden is built to act as a passive repository. Already within a few years after closure water will penetrate the repository and conditions of high alkalinity (pH 10.5―13.5) and low temperature (< 7 °C) will prevail. The mobility of radionuclides in the repository is dependent on the radionuclides distribution between solid and liquid phases. In the present work the incorporation of strontium (II) and europium (III) in α-calcium isosaccharinate (ISA) under alkaline conditions (pH ~10) at 5 °C and 50 °C have been studied. The results show that strontium and europium are incorporated into α-Ca(ISA)2 when crystallized both at 5 °C and 50 °C. Europium is incorporated to a greater extent than strontium. The highest incorporation of europium and strontium at 5 °C rendered the phase compositions Ca0.986Eu0.014(ISA)2 (2.4% of Eu(ISA)3 by mass) and Ca0.98Sr0.02(ISA)2 (2.2% of Sr(ISA)2 by mass). XPS spectra show that both trivalent and divalent Eu coexist in the Eu incorporated samples. Strontium ions were found to retard the elongated growth of the Ca(ISA)2crystals. The incorporation of Sr2+ and Eu3+ into the solid phase of Ca(ISA)2 is expected to contribute to a decreased mobility of these ions in the repository.

  • Homogeneous and Heterogeneous Cluster-Based Catalysts for Asymmetric Reactions

    • Synthesis and characterization of new transition metal carbonyl clusters has been undertaken, and evaluation of the new clusters to act as catalysts/catalysts precursor in asymmetric reactions has been carried out. The catalytic hydrogenation of tiglic acid under relatively mild conditions, using diastereomeric clusters of the general formula [(µ-H)2Ru3(µ3-S)(CO)7(µ-1,2-L)] (L= chiral diphosphine of the ferrocene-based Walphos and Josiphos families) as catalysts, reveal different catalytic behavior in terms of conversion and enantioselectivity. The observed reversal in enantioselectivity when the chirality of the cluster framework is changed strongly supports catalysis by intact Ru3 clusters. A proposed catalytic cycle generated by CO loss from the parent catalyst has been investigated. The ability of carbonyl clusters based on rhenium, ruthenium and osmium derivatised with chiral phosphines and phosphiranes to catalyse the asymmetric hydrogenation of tiglic acid shows high to excellent conversion rates with low to moderate enantioselectivity. A high conversion rate (up to 99%) and an enantioselectivity of 19% have been achieved with the cluster [(µ-H)4Ru4(CO)11(ArPH2)] {where ArP = (R)-(2'-methoxy-[1,1'-binaphthalen]-2-ylphosphine)} acting as catalyst/catalyst precursor; this is, to the best of our knowledge, the first example of an asymmetric hydrogenation induced by a chiral primary phosphine. A chiral solid catalyst derived from the immobilization of [H4Ru4(CO)10(µ-1,2-W001)] (W001 = chiral diphosphine ligand of the Walphos family) onto functionalized MCM-41 has been shown to exhibit excellent conversion rates (> 99%) and as high enantiomeric excesses (≈ 80%) as that of its homogeneous counterpart. The good catalytic properties are coupled with good recyclability of the mesoporous catalyst.

    • There is a high interest to develop suitable solid phase extractants for uranium separation from aqueous solutions in order to reduce cost and enhance the efficiency. This paper describes solid phase extraction of uranium(VI) from aqueous phosphoric acid solution using MCM-41 based D2HEPA-TOPO organophosphorous extractants. The mixture of D2HEPA (di-2-ethyl-hexylphosphoric acid) and TOPO (tri-n-octylphosphine oxide) was impregnated into the pores of MCM-41 and the synthesized sorbent was fully characterized. The influences of different factors such as synergistic mixture ratio, phosphoric acid concentration, mixing time and temperature were investigated. The results showed that 90% of uranium(VI) extraction can be achieved within 5 min, using D2HEPATOPO@MCM-41 (mass ratio 2: 1 w/w) from 1 M phosphoric acid containing 64 ppm of uranium at room temperature. High adsorption capacity of uranium(VI) have been achieved at the mentioned conditions. The rate constant for the chemical adsorption of uranium(VI) was 0.988 g mg. 1 min. 1 calculated by the pseudo-second order rate equation. The obtained thermodynamics parameters showed that uranium(VI) adsorption from H-3 PO4 is an exothermic and spontaneous process.

  • Incidence and factors associated with burnout in radiologists : A systematic review

    • Rationale and objectives: Burnout among physicians has a prevalence rate exceeding 50%. The radiology department is not immune to the burnout epidemic. Understanding and addressing burnout among radiologists has been a subject of recent interest. Thus, our study aims to systematically review studies reporting the prevalence of burnout in physicians in the radiology department while providing an overview of the factors associated with burnout among radiologists. Materials and methods: The search was conducted from inception until November 13th, 2022, in PubMed, Embase, Education Resources Information Center, PsycINFO, and psycArticles. Studies reporting the prevalence of burnout or any subdimensions among radiology physicians, including residents, fellows, consultants, and attendings, were included. Data on study characteristics and estimates of burnout syndrome or any of its subdimensions were collected and summarized. Results: After screening 6379 studies, 23 studies from seven countries were eligible. The number of participants ranged from 26 to 460 (median, 162; interquartile range, 91–264). In all, 18 studies (78.3%) employed a form of the Maslach Burnout Inventory. In comparison, four studies (17.4%) used the Stanford Professional Fulfillment Index, and one study (4.3%) used a single-item measure derived from the Zero Burnout Program survey. Overall burnout prevalence estimates were reported by 14 studies (60.9%) and varied from 33% to 88%. High burnout prevalence estimates were reported by only five studies (21.7%) and ranged from 5% to 62%. Emotional exhaustion and depersonalization prevalence estimates were reported by 16 studies (69.6%) and ranged from 11%−100% and 4%−97%, respectively. Furthermore, 15 studies (65.2%) reported low personal accomplishment prevalence, ranging from 14.7% to 84%. There were at least seven definitions for overall burnout and high burnout among the included studies, and there was high heterogeneity among the cutoff scores used for the burnout subdimensions. Conclusion: Burnout in radiology is increasing globally, with prevalence estimates reaching 88% and 62% for overall and high burnout, respectively. A myriad of factors has been identified as contributing to the increased prevalence. Our data demonstrated significant variability in burnout prevalence estimates among radiologists and major disparities in burnout criteria, instrument tools, and study quality.
