  • Bladder dysfunction: an integral part of the ectopic ureterocele complex.

    • We evaluate whether bladder dysfunction is common in patients with ectopic ureterocele and, if so, whether it is an integral part of the ectopic ureterocele complex or a result of surgery.From 1986 to 1995, 34 patients with a mean age of 10 months were treated for large or medium ectopic ureteroceles at our institution and 32 participated in postoperative followup. Bladder function was investigated by a careful history and repeat uroflowmetry, and residual urine estimation was assessed by ultrasound and cystometry.Of the 32 patients 19 had infrequent voiding and 3 had incontinence. Cystometric bladder capacity was increased to greater than 150% of the normal value for age in 15 of 27 patients (55%). Uroflowmetry revealed greater than 5 ml. residual urine in 15 patients (56%). Postoperatively no radiological signs of bladder neck obstruction were found. Increased bladder capacity and residual urine did not correlate with ureterocele size or location, or surgical procedure. There was no progression of bladder dysfunction with age.Children with ectopic ureterocele are at high risk for a high capacity bladder with incomplete emptying. This bladder dysfunction associated with ectopic ureterocele does not seem to be the result of surgery but an integral part of the disorder.

  • The relationship between measures of physical performance in preoperative oesophageal cancer patients

    • Background and aim: Preoperative testing of physical performance is common for patients undergoing an esophagectomy, but little evidence exists suggesting which measures are most suitable. This study compared outcome measures currently used at participating hospitals (spirometry, cycle ergometer test) against other validated tests–Maximal Inspiratory Pressure (MIP), Maximal Expiratory Pressure (MEP), Timed Up and Go (TUG), 6 Minute Walk Test (6MWT), heel raise test, shoulder abduction test, Timed Stands Test (TST) and handgrip strength. Methods: 109 patients were recruited from three university hospitals in Sweden. Participants were tested using a protocol testing respiratory and peripheral muscle strength, lung function, endurance and fitness. Results: There were strong correlations between the cycle ergometer and grip strength (r = 0.619), 6MWT (r = 0.572), MIP (r = 0.619) and MEP (r = 0.529). A strong correlation was also seen between spirometry and grip strength (r = 0.508–0.574). A moderate correlation was found between spirometry, MIP (r = 0.405–0.470) and MEP (r = 0.366–0.452), the cycle ergometer, TUG (r = −0.321) and TST (r = −0.308) and between 6MWT and PEF (r = −0.333). Conclusion: The strong correlations between different measures suggest that in cases where the standard tests are inadvisable or impractical, simple tests like grip strength and 6MWT could be used instead. © 2019, © 2019 Informa UK Limited, trading as Taylor & Francis Group.

  • Dose-Response Characteristics of the Alcohol Biomarker Phosphatidylethanol (PEth)-A Study of Outpatients in Treatment for Reduced Drinking

    • AimMeasurement of whole-blood phosphatidylethanol (PEth) offers high sensitivity and specificity as alcohol biomarker. A remaining issue of importance for the routine application is to better establish the relationship between PEth concentration and amount and duration of drinking.MethodsThe study included 36 subjects (32–83 years) voluntarily attending outpatient treatment for reduced drinking. At ~ 3- to 4-week intervals, they provided a diary on their daily alcohol intake and gave blood samples for measurement of PEth and carbohydrate-deficient transferrin (CDT). Whole-blood PEth 16:0/18:1 was measured by liquid chromatography-tandem mass spectrometry and serum CDT (%disialotransferrin) by high-performance liquid chromatography.ResultsAt start, the self-reported past 2-week alcohol intake ranged 0–1260 (median 330) g ethanol, the PEth 16:0/18:1 concentration ranged 0.05–1.20 (median 0.23) μmol/L, and the CDT value ranged 0.7–13.0% (median 1.5%). At the final sampling after 5–20 (median 12) weeks, neither reported alcohol intake nor PEth and CDT levels differed significantly from the starting values. The PEth concentration showed best association with past 2-week drinking, followed by for intake in the next last week. The changes in PEth concentration vs past 2-week alcohol intake between two successive tests revealed that an increased ethanol intake by ~ 20 g/day elevated the PEth concentration by on average ~ 0.10 μmol/L, and vice versa for decreased drinking.ConclusionsThe PEth concentration correlated well with past weeks alcohol intake, albeit with a large inter-individual scatter. This indicates that it is possible to make only approximate estimates of drinking based on a single PEth value, implying risk for misclassification between moderate and heavy drinking.

    • CD14 is a predictor of inflammation and associated with atherosclerosis. We analyzed 118 carotid plaques from patients with symptomatic carotid artery stenosis for expression of the macrophage markers CD14, CD68 and the angiotensin II type 1 receptor (AT(1)-R). CD14 staining was significantly increased in thrombotic carotid plaques. AT1-R staining was found in macrophage-rich areas, and AT1-R mRNA was detected in plaque macrophages isolated with anti-CD14 immunobeads. In patients treated with an angiotensin receptor blocker, expression of CD14 and CD68 in carotid plaque and serum levels of inflammatory markers were lower than in untreated patients. In vitro, expression of CD14 in human monocyte-derived macrophages was increased by exposure to lipopolysaccharide and decreased by exposure to an angiotensin receptor blocker. Thus, inhibition of the innate immune responsive lipopolysaccharide receptor CD14 in macrophages, rather than AT(1)-R inhibition, may help explain the anti-inflammatory effects of angiotensin receptor blockade.   • Injectable bone cements for vertebroplasty studied in sheep vertebrae with electron microscopy

    • Vertebral compression fractures were simulated by making a hole into sheep vertebrae and by injecting a stabilizing material. The injectable bio-ceramic Xeraspine™ was evaluated together with a commercially available PMMA (Vertebroplastic™) as the reference material. The Vertebrae were harvested after 7 days and prepared for microscopy. The samples were deposited with gold on the surface and thereafter subjected to SEM and EDX analysis. It was found that the Xeraspine-bone interface was composed of a mixture of elements. The Vertebroplastic implant was embedded in a carbon containing tissue, likely a soft tissue capsule. The Xeraspine sample was subjected to high resolution analysis in the TEM combined with EDX measurements. The TEM sample was prepared with a novel technique for preparation of the tissue-material interface (FIB). In the TEM analysis it was found that the interface region consists of ZrO2 together with a mixture possibly consisting of katoite and apatite formed during setting and/or originating from the boneapatite.

    • Background Occupational health care (OHC) was part of the Swedish Risk Drinking Project, a government-supported continuing professional education (CPE) endeavour for health professionals, 2004-10. Aims To investigate OHC professionals perceived knowledge concerning patient counselling on alcohol, perceived efficiency in achieving patient changes, use of questionnaires and biomarkers, amount and content of their alcohol-related CPE, their reasons for participating in this CPE and improvements experienced in addressing alcohol issues in the last 3 years. Methods Baseline 2005 and follow-up 2008 data were collected by means of anonymous questionnaires to all physicians and nurses in OHC. Results Response rates ranged from 53 to 80%. The share of nurses and physicians who had taken part in greater than= 3 days training on risk drinking issues rose from 10 to 59% and from 7 to 59%, respectively. The OHC professionals perceived knowledge and efficiency concerning lifestyle counselling improved from 2005 to 2008, considerably more so for alcohol than the other lifestyle issues. The most competent nurses and physicians were more likely to have participated in training lasting greater than= 3 days. The most competent nurses and physicians had become better at initiating conversations about alcohol and at informing, providing advice and discussing alcohol with their clients. Conclusions There were major improvements in the OHC professionals perceived knowledge concerning alcohol counselling and efficiency in influencing patients alcohol habits after participation in a comprehensive CPE project. Overall, the nurses achieved more improvements than the physicians.

    • We report analytical expressions for the dimethylsulfoxide (DMSO)-Li+ and DMSO-I- potential energy hypersurfaces. Quantum chemically calculated energies were fitted to polynomial functions of different number of parameters. These calculations are augmented by calculations of the electrostatic potential in the vicinity of DMSO. Global geometry optimizations and calculations of ion-induced frequency shifts have been performed. We find that at its global energy minima, Li+ in the DMSO-Li+ complex is bound to the oxygen atom with an energy of about 2.6 eV while I- in the vicinity is bound between the two methyl groups in a shallow energy minimum with about 0.6 eV.

    • We report results of a molecular dynamics simulation on a system consisting of 343 DMSO molecules, one Li+ cation and one I- anion in the NVE ensemble at 310 K, employing our recently developed all-atom potential functions for the intermolecular interactions derived from quantum chemical calculations. The structure of the solvation shell around the cation and the anion are investigated in terms of radial and angular distribution functions. The first solvation shell around lithium ion consists of 4 DMSO molecules; 8 DMSO molecules coordinate to iodide ion. DMSO coordinates to both ions in a well-defined way. The results are in good agreement with other molecular dynamics simulations of alkali halides in DMSO.

    • Liquid DMSO has been investigated by means of X-ray and neutron diffraction, quantum-chemical calculations and molecular dynamics (MD) simulations. A new analytical all-atom pair potential for DMSO-DMSO interactions was developed from quantum-chemical calculations and employed in the simulations. The MD-derived total structure function agrees favourably with those obtained from the X-ray and neutron diffraction experiments. An analysis of the local structure shows a rather unstructured liquid with a slight preference for antiparallel ordering of the S-O dipoles.

    • Objective: (1) To determine the prevalence of musculoskeletal complaints (MSCs) in individuals with upper limb absence in The Netherlands, (2) to assess the health status of individuals with upper limb absence in general and in relation to the presence of MSCs, and (3) to explore the predictors of development of MSCs and MSC-related disability in this population.Design: Cross-sectional study: national survey.Setting: Twelve rehabilitation centers and orthopedic workshops.Partiscipants: Individuals (n=263; mean age, 50.7±16.7y; 60% men) ≥18 years old, with transverse upper limb reduction deficiency (42%) or amputation (58%) at or proximal to the carpal level (response, 45%) and 108 individuals without upper limb reduction deficiency or amputation (n=108; mean age, 50.6±15.7y; 65% men) (N=371).Interventions: Not applicable.Main outcome measures: Point and year prevalence of MSCs, MSC-related disability (Pain Disability Index), and general health perception and mental health (RAND-36 subscales).Results: Point and year prevalence of MSCs were almost twice as high in individuals with upper limb absence (57% and 65%, respectively) compared with individuals without upper limb absence (27% and 34%, respectively) and were most often located in the nonaffected limb and upper back/neck. MSCs were associated with decreased general health perception and mental health and higher perceived upper extremity work demands. Prosthesis use was not related to presence of MSCs. Clinically relevant predictors of MSCs were middle age, being divorced/widowed, and lower mental health. Individuals with upper limb absence experienced more MSC-related disability than individuals without upper limb absence. Higher age, more pain, lower general and mental health, and not using a prosthesis were related to higher disability.Conclusions: Presence of MSCs is a frequent problem in individuals with upper limb absence and is associated with decreased general and mental health. Mental health and physical work demands should be taken into account when assessing such a patient. Clinicians should note that MSC-related disability increases with age.
