The development of safe and efficacious gene vectors has limited greatly the potential for therapeutic treatments based on messenger RNA (mRNA). Lipid nanoparticles (LNPs) formed by an ionizable cationic lipid (here DLin-MC3-DMA), helper lipids (distearoylphos-phatidylcholine, DSPC, and cholesterol), and a poly(ethylene glycol) (PEG) lipid have been identified as very promising delivery ve ctors of short interfering RNA (siRNA) in different clinical phases; however, delivery of high-molecular weight RNA has been proven much more demanding. Herein we elucidate the structure of hEPO modified mRNA-containing LNPs of different sizes and show how structural differences affect transfection of human adipocytes and hepatocytes, two clinically relevant cell types. Employing small-angle scattering, we demonstrate that LNPs have a disordered inverse hexagonal internal structure with a characteristic distance around 6 nm in presence of mRNA, whereas LNPs containing no mRNA do not display this structure. Furthermore, using contrast variation small-angle neutron scattering, we show that one of the lipid components, DSPC, is localized mainly at the surface of mRNA-containing LNPs. By varying LNP size and surface composition we demonstrate that both size and structure have significant influence on intracellular protein production. As an example, in both human adipocytes and hepatocytes, protein expression levels for 130 nm LNPs can differ as much as 50-fold depending on their surface characteristics, likely due to a difference in the ability of LNP fusion with the early endosome membrane. We consider these discoveries to be fundamental and opening up new possibilities for rational design of synthetic nanoscopic vehicles for mRNA delivery.

    • En majoritet av dagens byggnadsuppgifter gäller att hantera det redan byggda. När vi står inför situationen att restaurera en befintlig byggnad är det viktigt att förstå olika tidsperioders stilideal liksom byggnadsteknik och material. Först då kan vi göra en väl avvägd analys, som tar tillvara och utvecklar de kvaliteter som byggnaderna själva besitter. Därför är utbildningen upplagd som ett växelspel mellan föreläsningar, seminarier, exkursioner och en för året vald studieuppgift.Slott och borgar har varit läsårets tema. Vi har valt att arbeta med Västerås och Örebro slott - två ganska bortglömda Vasaslott som är väl värda att lyfta fram. Särskilt har vi studerat de senaste 300 årens förändringar, som inte tidigare ägnats lika stora forskarmöda som medelitden och Vasatiden. I dessa två exempel finns en provkarta på estetiska, praktiska och tekniska ingrepp från Carl Hårlemans tid och fram till idag.Studierna har således omfattat både gestaltning, funktion och byggnadsteknik. Avsikten är att visa på kvaliteter i de omvandlingar och restaureringar som skett, men också att peka på problem och analysera olika möjligheter inför framtiden. Arbetet har skett i samarbete med Statens fastighetsverk och är tänkt att utgöra ett underlag till vårdprogram och framtida restaureringsinsatser.

  • Streptococcus pyogenes Infection and the Human Proteome with a Special Focus on the Immunoglobulin G-cleaving Enzyme IdeS

    • Infectious diseases are characterized by a complex interplay between host and pathogen, but how these interactions impact the host proteome is unclear. Here we applied a combined mass spectrometry-based proteomics strategy to investigate how the human proteome is transiently modified by the pathogen Streptococcus pyogenes, with a particular focus on bacterial cleavage of IgG in vivo. In invasive diseases, S. pyogenes evokes a massive host response in blood, whereas superficial diseases are characterized by a local leakage of several blood plasma proteins at the site of infection including IgG. S. pyogenes produces IdeS, a protease cleaving IgG in the lower hinge region and we find highly effective IdeS-cleavage of IgG in samples from local IgG poor microenvironments. The results show that IdeS contributes to the adaptation of S. pyogenes to its normal ecological niches. Additionally, the work identifies novel clinical opportunities for in vivo pathogen detection.

    • A quarter millenium of morphological Crystal Models – the makers, the materials, the collections – a current project on retrieving their history.Crystal models (CM) and crystal model collections (CMC) made of paper, wood, clay, metal, glass, etc. have been manufactured, developed, and used as an aid in the fields of mineralogy and crystallography from the 1760’s to 1920’s (Figure 1). CM have for example been used to display ideal morphology of different mineral species or relationship between crystal forms; visualize different crystallographic and physical concepts, e. g. symmetry, crystallographic and optical axes, twinning, etc. Earlier geometrical models, such as platonic and archimedian bodies, known since Antiquity and perhaps originally inspired by natural crystals, were often present in the art and the curiosity cabinets of the Rennaisance era. In the early 20th c. the progress in crystal structure determination shifted the focus in crystallography and crystal modelling from outer morphology and symmetry to inner structural relations. Still, morphological CM were produced ​​and used throughout the century.Apart from a few attempts to describe special CMC, drawing on the collections in the BMNH in London and Teylers Museum in Haarlem, very little has been written about CMC (Tandy 1998; Touret 2004).CMC could and should be complimentary research objects of early crystallography. In their time some CMC conveyed new concepts, many times under direct supervision of the inventor, and by studying them new aspects, blind alleys or pitfalls in the history of crystallography may come to light. The locations of CMC from different schools, may reflect scientific connections and influences, etc. But without any precise knowledge about the different sets and makers no such research can be made, they will remain just CM. The two main institutional uses of CMC – exhibit and education – can also pose potential threats to them, those of being worn in crystallography class or being dispersed both in education and exhibits. Banalized descriptions, e.g. “antique models” or “Krantz models”, is typical if such CM are marketed, ultimately then often losing their institutional provenance.  The aim of this study is to provide the mineralogical community with a deeper knowledge of the old CMC: their makers and scientists, when, where, and what of the sets were made, and how many models they contained. And not the least, where we can see typical sets of the different CMC.How can this work be done? By a synergetic approach from three directions: (i) by studying literature, both primary books and catalogs listing/treating CMC, and different kinds of secondary literature, (ii) by Internet searches, especially utilizing the facility of searching words in OCR scanned texts, e.g. Google Books, Gallica, an effective tool for finding information which else would be almost impossible to retrieve; (iii) by surveying important collections and their archives, some of which are digitized.In this presentation I will give a few examples of what I have accomplished so far by this method.Tandy, P. 1998.     • In 1823 A. von Weissenbach listed in the catalog of the Äußere-Kennzeichen-Sammlung (External characteristics collection) of the Werner-Museum among other items several early crystal model collections. In 2012 we were able to recover and identify these collections as well as some other model sets pertaining to the early history of crystallography. Additional documentation over some of these sets was found in Werner’s correspondence and his other scientific estate. This is a remarkably rich source for studies of material culture and scientific change and exchange, crystal model collections are usually highly depleted and information on them sparing to nonexistent.That such early model sets often are exquisitely made and examples of skilful craftsmanship can be seen in the exhibits in the Krüger-Haus in Freiberg. However, in our paper we want to describe and highlight the contemporary crystallographic thinking of each part of this important suite of crystal model sets representing the toughts of Linnaeus, Werner, Romé de l’Isle, Haüy, etc. from the late 18th to early 19th centuries, provisionally divided into three groups: (1) The oldest model sets pertain to Werner’s early years. These comprise a small wooden collection made after the plates of the Swedish naturalist C. Linnaeus’ Systema Naturae 1768, a collection of models hammered and carved in lead purported to have been made by Werner himself, and 55 wooden models that must be one of the earliest sets produced in more than one copy.  (2) Model sets representing Werner’s ‘developed’ crystallographic thinking. This was first presented in 31 paragraphs in Von den äußerlichen Kennzeichen der Foßilien 1774. We will discuss how it evolved, our sources being the model sets made in Freiberg by C. I. Löscher, and contemporary texts. (3) Model sets arriving from abroad. To these count four collections in brass, wood, and porcelain made by pioneering French model makers, and one set of gypsum models made by F. X. M. Zippe.These collections are in Freiberg either because Werner had chosen them or that they were sent to him due to his reputation. Werner’s use of, response and comments to the different model sets will suggest to us his attitude to the developing new science of crystallography for over 40 years.

    • The purpose of this study was to develop multimodality SPECT/MRI contrast agents for sentinel lymph node (SLN) mapping in vivo. METHODS: Nanoparticles with a solid iron oxide core and a polyethylene glycol coating were labeled with (99m)Tc. The labeling efficiency was determined with instant thin-layer chromatography and magnetic separation. The stability of the radiolabeled superparamagnetic iron oxide nanoparticles (SPIONs) was verified in both sterile water and human serum at room temperature 6 and 24 h after labeling. Five Wistar rats were injected subcutaneously in the right hind paw with (99m)Tc-SPIONs (25-50 MBq, ∼0.2 mg of Fe) and sacrificed 4 h after injection. Two animals were imaged with SPECT/MRI. All 5 rats were dissected; the lymph nodes, liver, kidneys, spleen, and hind paw containing the injection site were removed and weighed; and activity in the samples was measured. The microdistribution within the lymph nodes was studied with digital autoradiography. RESULTS: The efficiency of labeling of the SPIONs was 99% 6 h after labeling in both water and human serum. The labeling yield was 98% in water and 97% in human serum 24 h after labeling. The SLN could be identified in vivo with SPECT/MRI. The accumulation of (99m)Tc-SPIONs (as the percentage injected dose/g [%ID/g]) in the SLN was 100 %ID/g, whereas in the liver and spleen it was less than 2 %ID/g. Digital autoradiography images revealed a nonhomogeneous distribution of (99m)Tc-SPIONs within the lymph nodes; nanoparticles were found in the cortical, subcapsular, and medullary sinuses. CONCLUSION: This study revealed the feasibility of labeling SPIONs with (99m)Tc. The accumulation of (99m)Tc-SPIONs in lymph nodes after subcutaneous injection in animals, verified by SPECT/MRI, is encouraging for applications in breast cancer and malignant melanoma.

    • In anti-glomerular basement membrane (anti-GBM) disease, IgG class autoantibodies induce rapidly progressive glomerulonephritis. Regrettably, many patients are diagnosed at a late stage when even intensive conventional treatment fails to restore renal function The endopeptidase IdeS (Immunoglobulin G degrading enzyme of Streptococcus pyogenes) (imliflidase) rapidly cleaves all human IgG subclasses into F(ab')(2) and Fc fragments. We received permission to treat three patients with refractory anti-GBM nephritis without pulmonary involvement on a compassionate basis. All patients were dialysis-dependent for days or weeks when treated, and all had high levels of circulating anti-GBM despite plasma exchange. A single dose of IdeS led to complete clearance of circulating anti-GBM antibodies in all three patients. After about a week, all rebounded but the rebounds were easily managed by plasma exchange in two of three cases. Renal histology demonstrated severe crescentic glomerulonephritis with acute but mainly chronic changes. Staining for the Fc fragment was negative in all while Fab was positive in two patients. Unfortunately, none of the patients regained independent renal function. Thus, treatment with IdeS led to rapid clearance of circulating and kidney bound antiGBM antibodies. The clinical utility, dosing and usage to preserve renal function remain to be determined.

Åsikterna som förs fram här är upphovsmannens egna. Fysisk aktivitet som prevention och behandling har varit en naturlig del av hälso- och sjuk­vården sedan början av 2000-talet. År 2011 breddade Socialstyrelsen detta med riktlinjer för sjukdoms­förebyggande metoder där tobaksbruk, riskbruk av alkohol, ohälsosamma matvanor och otillräcklig fysisk aktivitet inbegreps. Personer med psykisk ohälsa har här prioriterats av flera skäl, då man löper en stor risk för kroppslig ohälsa och förkortat liv.Nu uppdaterar Social­styrelsen riktlinjer om behandling av depression och ångest och uppmärksammar samsjuklighet som hjärt-kärlsjukdom och diabetes, men nämner inte levnadsvanor som en nödvändig del i omhändertagandet, vilket är olyckligt.Betydelsen av råd om fysisk aktivitet som en del av behandlingen av patienter med depression, framkommer tydligt i andra länder såsom Storbritannien med expertmyndigheten Nice. Här ses fysisk aktivitet som en viktig bas, oberoende av övrig behandling.“Här ses fysisk aktivitet som en viktig bas, oberoende av övrig behandling.”Riktlinjer från 2010 gav stöd för fysisk aktivitet som behandling av mild till måttlig depression. I de nya riktlinjerna som publicerades i december 2016, är detta i stort sett borta.Vi menar att det är kontra­produktivt att Social­styrelsen inom ramen för ett riktlinjearbete inte ser till övrigt arbete inom myndigheten.Socialstyrelsens föreslagna riktlinjer krockar med vad vi har redovisat i den helt nyligen upp­daterade versionen av Fysisk aktivitet i sjukdomsprevention och sjukdomsbehandling, Fyss, avseende behandling av depression. I vårt arbete har ett stort antal vetenskapliga artiklar och meta-analyser granskats. I Social­styrelsens underlag redovisas endast sex vetenskapliga referenser.Vi är inte heller ensamma om att rikta kritik mot de nya rikt­linjerna. Skarp kritik framfördes också av tre sakkunniga inom psykoterapiområdet som själva varit en del av riktlinjearbetet. Där beskriver man en icke-transparent arbetsorganisation som i mycket hög grad har präglats av partiskhet till förmån för vissa insatser. Personer med kunskap om annat än KBT och läke­medels­behand­ling var i extrem minoritet, vilket påverkade omröstnings­förfarandet. Vi saknar insikt om hur denna process gått till, men blir oroade över den kritik som framförts.Sammanfattningsvis är vi kritiska till att Socialstyrelsens olika riktlinjer tillåts krocka med varandra. Socialstyrelsen bör ta ett större grepp och även integrera råd om levnadsvanor i de föreslagna nya riktlinjerna, gärna i samråd med andra myndigheter. Vi ifrågasätter också prioriteringsgruppens slutsats av evidensen för fysisk aktivitet som behandlingsalternativ för mild till måttlig depression.Vi har framfört dessa syn­punkter direkt till Social­styrelsen, i ett detaljerat remissvar. It is unknown if treat-ment with an endopeptidase that cleaves circulating and kidney bound IgG can alter the prognosis.& nbsp;Methods An investigator-driven phase 2a one-arm study (EudraCT 2016-004082-39) was performed in 17 hospitals in five European countries. A single dose of 0.25 mg/kg of imlifidase was given to 15 adults with circulating anti-GBM antibodies and an eGFR < 15 ml/min per 1.73m(2). All patients received standard treatment with cyclophosphamide and corticosteroids, but plasma exchange only if autoantibodies rebounded. The primary outcomes were safety and dialysis independency at 6 months.& nbsp;Results At inclusion, ten patients were dialysis dependent and the other five had eGFR levels between 7 and 14 ml/min per 1.73m(2). The median age was 61 years (range 19-77), six were women, and six were also positive for anti-neutrophil cytoplasmic antibodies. Then 6 hours after imlifidase infusion, all patients had anti-GBM antibodies levels below the reference range of a prespecified assay. At 6 months 67% (ten out of 15) were dialysis independent. This is significantly higher compared with 18% (nine out of 50) in a historical control cohort (P < 0.001, Fisher's exact test). Eight serious adverse events (including one death) were reported, none assessed as probably or possibly related to the study drug.& nbsp;Conclusions In this pilot study, the use of imlifidase was associated with a better outcome compared with earlier publications, without major safety issues, but the findings need to be confirmed in a randomized controlled trial.

    • Mesenchymal stem cells (MSCs) are multipotent cells that have the capacity to differentiate into various different cell lineages and can generate bone, cartilage and adipose tissue. MSCs are presently characterized using a broad range of different cell-surface markers that are not exclusive to MSCs and not sensitive to culture conditions or differentiation capacity. We show that the integrin subunits alpha10 and alpha11 of the collagen binding integrins alpha10beta1 and alpha11beta1 are expressed by human MSCs in monolayer cultures. We also demonstrate that the expression of alpha10 increases, while alpha1 and alpha11 decrease, during aggregate culture of MSCs in chondrogenic medium. Alpha10beta1 is expressed by chondrocytes in cartilage, whereas alpha11beta1 integrins are predominantly expressed by subsets of the fibroblastic lineage. In extensive monolayer cultures of MSCs, alpha10 expression is down-regulated. We show that this down-regulation is reversed by fibroblast growth factor-2 (FGF-2) treatment. Addition of FGF-2 to MSCs not only results in increased alpha10 expression, but also in decreased alpha11 expression. FGF-2 treatment of MSCs has been shown to keep the cells more multipotent and also induces cell proliferation and Sox-9 up-regulation. We demonstrate improved chondrogenecity as well as increased collagen-dependant migratory potential of FGF-2-treated MSCs having a high alpha10 expression. We also demonstrate expression of alpha10 and alpha11 integrin subunits in the endosteum and periosteum of mice, but very low or not detectable expression levels in freshly aspired human or mouse BM. We show that MSCs with high chondrogenic differentiation potential are highly alpha10 positive and propose alpha10 as a potential marker to predict the differentiation state of MSCs.
