  • Rollator related pedestrian single accidents and collision events in Sweden

    • Rollators (four-wheel walkers in USA; zimmer frames in UK) are commonly used as mobility aids for the elderly and people with a variety of disabilities. Pedestrian rollator users are a sub-group of Vulnerable Road Users (VRUs), although this group is rarely recognised in traffic safety contexts. The aim of this study is to extract and analyse rollator related pedestrian accident and injury data in Sweden. The results will provide valuable insight into the risks and obstacles rollator users are exposed to in the traffic environment and may in the long term contribute to improving the mobility of this group. The current study is based on data from 2,020 accidents involving 2,305 persons extracted from the Swedish Traffic Accident Data Acquisition (STRADA) database. For consistency reasons, a subset of data (N = 745) was analysed in order to investigate the development of accidents over a period of 10 years. Thereafter, each accident in the whole data set was registered as either single or collision. The results show that the number of rollator accidents in Sweden increased by approximately 80% during 2007–2016. Females dominate the injury statistics of single accidents, collisions, as well as fatal outcome, which may be due to exposure and/or differences in physical characteristics. Single accidents are much more common than collisions (n=1,668 and n=352, respectively) and the injury consequences are at least as serious. Data from the present study have revealed that the frequency of minor injuries (ISS 1–3) is 4.3 times, moderate (ISS 4–8) 6.0 times and serious injuries (ISS 9–15) 8.9 times higher in single accidents than in collisions. Fatal injuries are, however, more common in collision events (33 in comparison to 8). The vast majority of single accidents (99%) was due to falls. Many of the single accidents (29%) were caused by ground level differences (typically a curb) or due to surface conditions (19%). Collisions involved cars, trucks or buses in 91% of cases, occurring predominantly in car parks (31%) and on (zebra) crossings (30%). In 54% of cases the vehicles reversed into the rollator. Abbreviated Injury Scale (AIS) 3+ injuries were dominated by head (36%) and torso (33%) injuries in collision events, and hip fractures (71%) in single accidents. The present study shows that further research into rollator user related accidents, both single accidents and collision events, is required. In order to introduce appropriate measures, future work should follow up on accident and injury developments and further improve the quality of mobility aid related accident data in general. Improved stability and design, proper training programmes, effective maintenance services, development of a supporting infrastructure would contribute to increased safety for rollator users.

  • Obstacles and risks in the traffic environment for users of powered wheelchairs in Sweden

    • Objective: According to the European Union, fatal road accidents involving Vulnerable Road Users (VRUs) are equal in proportion to fatal car road accidents (46%). VRUs include individuals with mobility challenges such as the elderly and Powered Wheelchair (PWC) users. The aim of this interdisciplinary qualitative study was to identify obstacles and risks for PWC users by exploring their behaviour and experiences in traffic environments.Methods: Videos and in-depth interviews with 13 PWC users aged 20–66 were analysed for this study. The interviews and videos, which include real-life outdoor observations, originate from a qualitative study exploring experiences of PWC use on a daily basis in Sweden. Underlying causal factors to identified risks and obstacles were identified, based on human, vehicle (PWC) and environmental factors in accordance with the Haddon Matrix.Results: The results show significant potential for improvement within all three perspectives of the Haddon Matrix used in the analysis. Participants faced and dealt with various obstacles and risks in order to reach their destination. For example, this includes uneven surfaces, differences in ground levels, steep slopes, as well as interactions with other road users and the influence of weather conditions, resulting in PWC users constantly accommodating and coping with the shortcomings of the vehicle and the environment.Conclusions: There are still major challenges with regard to preventing obstacles and risks in the traffic environment for PWC users. To discern PWC users in traffic accident and injury data bases, a start would be to register type of aid used for persons involved in an accident. Furthermore, to emphasise PWC users’ role as VRUs, it may also be advantageous to describe them as drivers rather than users when navigating the traffic environment. Given the limited sample, further research covering more data from a broader perspective would be beneficial. By incorporating emerging knowledge of PWC users’ prerequisites and needs, and including them in research and traffic planning, the society will grow safer and more inclusive, and become better prepared for meeting future demands on accessibility from an aging population.

  • Olyckor och skador hos rollatoranvändare i trafikmiljön

    • Bakgrund: Rollatorn uppfanns 1978 i Västerås av Aina Wifalk. Idag är Sverige det rullator-tätaste landet i världen och det förskrivs ca 60 000 rullatorer per år. SCB beräknade 2007 att 240 000 personer använde rollator, bland dem ingick var tredje person över 80 år. 2014 fanns 499 000 personer i åldern 80+ i Sverige, år 2030 beräknas antalet ha ökat till 826 000 och 2060 så många som 1 210 000. Om kvoten rollatoranvändare kvarstår kan vi alltså år 2060 räkna med 720 000 rollatoranvändare, bara i åldersgruppen 80+.Användare av rollatorer tillhör gruppen oskyddade trafikanter, men är sällan uppmärk­sammade i trafiksäkerhetskontexten. Syftet med studien är därför att analysera olycks- och skadedata, som inrapporterats till den nationella databasen STRADA, där användare av rollatorer varit involverade. Resultatet av studien ger värdefull kunskap om risker och hinder som användare av rollator utsätts för i olika typer av trafikmiljöer. Syftet är också att studien ska kunna bidra till att förbättra rörligheten för denna grupp på lång sikt.Metod: Studien baseras på olycks- och skadedata från totalt 2 020 rapporterade olyckor med rollator. Inledningsvis undersöktes om antalet rollatorolyckor ökar, minskar eller är konstant över tid. Därefter registrerades varje olycka som antingen ”singel” (N=1 668) eller ”kollision” (N=352).Resultat och slutsats: Studien visade på en nästan fördubbling av antalet rollatorrelaterade olyckor i Sverige under perioden 2007–2016. Vidare var ca tre gånger fler kvinnor än män involverade i olyckor med rollator. I en majoritet av singelolyckorna slog användaren i marken. Många av singelolyckorna orsakades av nivåskillnader/kanter, ojämnt underlag eller snubbling. Bilar, lastbilar eller bussar var inblandade i 79% av kollisionerna. Dessa inträffade ofta vid parkeringsplatser, korsningar eller vägsträcka; i många av fallen blev rollatoranvändaren påbackad.

    • Background: Injury events in homes constitute a major social problem. Falling, cutting and poisoning make up 85 per cent of all injury events in residential areas.This study is based on a unique data set that includes several million cases of falling, cutting and poisoning in Sweden during the period 1990-2013 which lead to either to death, hospitalisation or to a visit to a health care provider, and a control group randomly selected from the population.Three riskgroups are given special attention in the analysis: (1) old people, (2) children, (3) persons with disabilities and or long term illnesses.Methods: Multilevel regression analysis and geographical information systems, GIS.Results: The results show the probability for each riskgroup to be exposed to each type of injury event, and how this probability varies with place (GIS), previous exposure, type of household, socioeconomic status and type of housing.Conclusions: The project is ongoing. Our cross-sectorial group has demonstrated the importance of injury epidemiology as a guiding principle in architectural design, particularly for high-risk groups.

    • “Norm-creative crisis preparedness” is a research project aiming to explore and co-design communication strategies and methods for supporting crisis preparedness and responses before and during times of crisis. This proposed poster presents the result from a survey study where representing members of the disability movement in Sweden have shared their experiences of living and acting in a pandemic. This material can inform a novel perspective of what it means for people with various kinds of disabilities to live with and through a crisis and how they can be supported by the right crisis response. By listening to voices from the (dis)ability movement the project strives at going beyond a normative view of what constitutes crisis preparedness and explores what we can learn from people whose everyday lives have always been characterized by extraordinary circumstances. The project is strongly informed by and protects human rights of people with disabilities with all that implies of matters of equality, inclusion and justice.

Vi presenterar här en modell för hur personer med funktionsnedsättningar kan samarbeta med forskara för att generera relevant kunskap.

    • Nyhetsmedierna publicerar artiklar om funktionshinderfrågor med skiftande innehåll. Denna totalundersökning syftar till att belysa hur etik, funktionshinder och medaktörskap har rapporterats i media i Sverige under perioden 1997-2015. Syftet har varit att belysa olika perspektiv i de mediala berättelserna. Det insamlade textmaterialet som består av 26 publicerade artiklar som har analyserats med hjälp av kvalitativ innehållsanalys. Resultatet av undersökningen visar att forskarnas perspektiv dominerar i rapporteringen. Den medicinska och vårdvetenskapliga forskningen har en framträdande roll. Det opinionsbildande materialet är även det framträdande. Funktionshinderrörelsen kommer inte till uttryck. Det etiska innehållet i rapporteringen kan sorteras i fyra etiska inriktningar: det goda livet, värdigt bemötande, autonomi och forskningsetik. Sammanfattningsvis visar studien att det finns ett behov av att stärka funktionshinderrörelsens position gentemot forskarsamhället och i media. Studien visar också att det finns ett utrymme i media för en ökad rapportering av funktionshinderfrågor av etisk karaktär.

  • Ambulanskörning i praktiken : Ambulanssjuksköterskors erfarenheter av utryckningskörning, skador och förebyggande arbete - en Websurvey 2021

    • Det finns en stor kunskap och erfarenhet hos ambulanssjuksköterskor om betydelsen av förarutbildning, förebyggande arbete och förhållningssätt till ambulanskörning och utryckningskörning. Föreliggande studie baseras på röster från 67 ambulanssjuksköterskor och deras erfarenheter av skadehändelser, risker, utsatthet men även förslag till förebyggande åtgärder och utbildningsbehov. Under år 2021 genomfördes en elektronisk Websurvey riktad till enbart ambulanssjuksköterskor. Totalt 67 ambulanssjuksköterskor deltog i undersökningen. Var fjärde ambulanssjuksköterska i studien hade erfarenhet av att ha varit inblandad i en skadehändelse med ambulansfordon som de själva kört. Studien visar att en övervägande majoritet av ambulanssjuksköterskorna vill ha ett nationellt förarbevis för alla som kör ambulansfordon vid utryckningskörning inom ambulansverksamheten (89%). De saknar också en erfarenhetsåterföring och feedback från avvikelserapporteringar som de gjort vid incidenter i samband med ambulanskörning och utryckningskörning (55%). Var tredje ambulanssjuksköterska uppgav i undersökningen att de hade en kollega som hade alkohol- och drogmissbruk inom ambulansverksamheten. 40 procent av ambulanssjuksköterskorna ansåg att ambulansfordonen borde utrustas med alkolås som en skadepreventiv åtgärd. 60 procent av deltagarna i studien ansåg att Vårdförbundet inte arbetade tillräckligt aktivt med frågor om säkerhet och ambulanskörning.

Artikeln diskuterar även vilka pedagogiska utmaningar som finns för att använda ICF i den högre utbildningen i framtiden. I översikten beskrivs förekomsten av ICF inom den högre utbildningen i olika länder sedan lanseringen av ICF fram till år 2016. Det torde också finnas betydande utmaningar i ett utbyte mellan forskare och praktiker att dela erfarenheterna med varandra. Den samlade erfarenheter av ICF är idag betydande.

    • This article describes the role that media coverage plays in creating awareness of the psychosocial support available to people in Sweden who are affected by crises, accidents, and trauma. The connection between media coverage, psychosocial support and traffic accidents has not been made clear in the literature or previous research. Trauma and fatal injuries in road accidents in Sweden has decreased in recent decades. Developments in China and the European countries shows that many people are killed on the roads and the resulting trauma would be significant for relatives and families who are affected. Therefore, it is important that those affected can get the support needed and requested.  The importance of insurance and insurance companies in China has been desribed by Dellien (Dellien, 2011). In Sweden the municipal authorities operate POSOM-groups that provide psychological and social care in the event of major community crisis. However, knowledge and awareness of social support for trauma victims is also very important for insurance companies in Sweden. The economic and social costs of long-term consequences is very important to prevent in society. A Swedish study show that a system called “pay-as-you-speed” (PAYS) could save lives and create at safer road transport system (Stigson et al, 2014). This article describes how team leaders in the POSOM-groups experience accessibility, communication and interactions with the media. The article focuses on how to increase awareness of emergency crisis support for one of today's major public health problems - traffic accidents in the community.In China, there is a growing demand for knowledge and skills in providing psychosocial support in the context of crisis and trauma (Cuiling, 2010). Traffic injuries in China has been highlighted in several studies in recent years (Wu and Cheung, 2006; Hu et al., 2008; Wu et al., 2008; Zhao, 2009, Alcorn, 2011; Yuan et al., 2012). Both China and Sweden have a great deal of experience and knowledge about treatment for victims of major disasters and accidents (Kulling, 1994; Hagström, 1995; Kulling and Riddez, 2001; Broberg et al., 2005; Lundin and Jansson, 2007; Berg Johannesson et al., 2009; Arnberg et al., 2012). In China several studies have provided insights on major disasters such as earthquakes and their consequences for the inhabitants (Cuiling, 2010; Fan et al., 2011; Ma et al., 2011; Ya-Hong et al., 2012).The consequences of traffic accidents can create to significant social and economic burdens in China and Europe (Berg et al., 2005; Tierens et al., 2012). In light of the long-term social, psychological and economic consequences of crises, trauma, and accidents in the community, there are several good reasons to try to reduce the damage and costs for individuals, insurance company and society.China is one of the countries most affected by fatal and serious injuries as a result of road accidents. In Europe more than 1.3 million traffic accidents occur annually, resulting in approximately 43,000 deaths 1.7 million people receiving injuries (European Commission, 2013).  In China, 200,619,351 people were killed in traffic accidents during the period 2000–2005 while 2,972,229 people were injured. In the same period, 3183 people were killed in traffic accidents in Sweden, while 150,000 people were injured in traffic accidents. During the period 2000–2010, 1,434,194 people were killed in road traffic accidents in China and Europe. The death rates has declined in recent years but still remains at a high level. In Sweden, the death rate of is the same as it was in 1940 (Central Statistical Office, 2011).  In Table 1 a total of 17 randomly selected accidents and disasters in both countries are presented. In these accidents and disasters a total of 3761 people have been killed and 996 injured.

    • Motor vehicles are an indispensible and daily tool of police work. The risk that police officers face when driving has attracted much academic attention, although in Sweden research in this field has been limited. In this paper, it is our intention to identify risks involving police cars, to review the causes of police car accidents in Sweden, and to give a better understanding of injury prevention in the Swedish police context. We note that, during the last few years, police officers' road traffic training has become more structured, covering emergency service driving and pursuits. Nevertheless, we call for more comprehensive driver education in police officer training, along with the establishment of strict routines for the gathering and processing of traffic incident data.

    • Traumavården upplevs som välutvecklad av en majoritet av de tillfrågade trafikskadade som ingått i en nationell studie år 1996 av levnadsvillkor för trafikskadade i Finland. Hela 70 procent ansåg att de blivit positivt bemötta av läkare såväl under det akuta omhändertagandet efter olyckan som under rehabiliteringsprocessen. Studien visar att läkarens vilja att lyssna och sensitivitet i rehabiliteringsprocessen är något som värderats högt. Kvinnor som erfarit traumatiska olyckor är mer tillfredsställda med läkarkontakten än männen.
