  • European fitness landscape for children and adolescents: updated reference values, fitness maps and country rankings based on nearly 8 million test results from 34 countries gathered by the FitBack network

    • (1) To develop reference values for health-related fitness in European children and adolescents aged 6–18 years that are the foundation for the web-based, open-access and multilanguage fitness platform (FitBack); (2) to provide comparisons across European countries.MethodsThis study builds on a previous large fitness reference study in European youth by (1) widening the age demographic, (2) identifying the most recent and representative country-level data and (3) including national data from existing fitness surveillance and monitoring systems. We used the Assessing Levels of PHysical Activity and fitness at population level (ALPHA) test battery as it comprises tests with the highest test–retest reliability, criterion/construct validity and health-related predictive validity: the 20 m shuttle run (cardiorespiratory fitness); handgrip strength and standing long jump (muscular strength); and body height, body mass, body mass index and waist circumference (anthropometry). Percentile values were obtained using the generalised additive models for location, scale and shape method.ResultsA total of 7 966 693 test results from 34 countries (106 datasets) were used to develop sex-specific and age-specific percentile values. In addition, country-level rankings based on mean percentiles are provided for each fitness test, as well as an overall fitness ranking. Finally, an interactive fitness platform, including individual and group reporting and European fitness maps, is provided and freely available online (www.fitbackeurope.eu).ConclusionThis study discusses the major implications of fitness assessment in youth from health, educational and sport perspectives, and how the FitBack reference values and interactive web-based platform contribute to it. Fitness testing can be conducted in school and/or sport settings, and the interpreted results be integrated in the healthcare systems across Europe.

  • No Difference in Penetrance between Truncating and Missense/Aberrant Splicing Pathogenic Variants in MLH1 and MSH2: A Prospective Lynch Syndrome Database Study

    • Background. Lynch syndrome is the most common genetic predisposition for hereditary cancer. Carriers of pathogenic changes in mismatch repair (MMR) genes have an increased risk of developing colorectal (CRC), endometrial, ovarian, urinary tract, prostate, and other cancers, depending on which gene is malfunctioning. In Lynch syndrome, differences in cancer incidence (penetrance) according to the gene involved have led to the stratification of cancer surveillance. By contrast, any differences in penetrance determined by the type of pathogenic variant remain unknown. Objective. To determine cumulative incidences of cancer in carriers of truncating and missense or aberrant splicing pathogenic variants of the MLH1 and MSH2 genes. Methods. Carriers of pathogenic variants of MLH1 (path_MLH1) and MSH2 (path_MSH2) genes filed in the Prospective Lynch Syndrome Database (PLSD) were categorized as truncating or missense/aberrant splicing according to the InSiGHT criteria for pathogenicity. Results. Among 5199 carriers, 1045 had missense or aberrant splicing variants, and 3930 had truncating variants. Prospective observation years for the two groups were 8205 and 34,141 years, respectively, after which there were no significant differences in incidences for cancer overall or for colorectal cancer or endometrial cancers separately. Conclusion. Truncating and missense or aberrant splicing pathogenic variants were associated with similar average cumulative incidences of cancer in carriers of path MLH1 and path_MSH2.

  • Risk-Reducing Gynecological Surgery in Lynch Syndrome: Results of an International Survey from the Prospective Lynch Syndrome Database

    • Purpose: To survey risk-reducing hysterectomy and bilateral salpingo-oophorectomy (BSO) practice and advice regarding hormone replacement therapy (HRT) in women with Lynch syndrome. Methods: We conducted a survey in 31 contributing centers from the Prospective Lynch Syndrome Database (PLSD), which incorporates 18 countries worldwide. The survey covered local policies for risk-reducing hysterectomy and BSO in Lynch syndrome, the timing when these measures are offered, the involvement of stakeholders and advice regarding HRT. Results: Risk-reducing hysterectomy and BSO are offered to path_MLH1 and path_MSH2 carriers in 20/21 (95%) contributing centers, to path_MSH6 carriers in 19/21 (91%) and to path_PMS2 carriers in 14/21 (67%). Regarding the involvement of stakeholders, there is global agreement (~90%) that risk-reducing surgery should be offered to women, and that this discussion may involve gynecologists, genetic counselors and/or medical geneticists. Prescription of estrogen-only HRT is offered by 15/21 (71%) centers to women of variable age range (35–55 years). Conclusions: Most centers offer risk-reducing gynecological surgery to carriers of path_MLH1, path_MSH2 and path_MSH6 variants but less so for path_PMS2 carriers. There is wide variation in how, when and to whom this is offered. The Manchester International Consensus Group developed recommendations to harmonize clinical practice across centers, but there is a clear need for more research.

  • PSCF and CWT methods as a tool to identify potential sources of 7Be and 210Pb aerosols in Granada, Spain

    • This research is focused on studying the preferred source regions and the pathways of the air masses with high particulate concentrations impacting on the activity concentrations of 7Be and 210Pb aerosols in Granada atmosphere. For this purpose, three different source-receptor methods have been used: Cluster Analysis, Potential Source Contribution Function (PSCF), and Concentration Weighted Trajectory (CWT). Air filter samples were weekly collected and analysed in Granada university (Spain 37.177N, 3.598 W, 687m a.s.l.) during 12 years (2006–2017) for the activity concentration of 7Be, and during 5 years (2010–2014) for the one of 210Pb. The time series of the collected data indicate that the concentration of both radiotracers present a cyclical and seasonal pattern, in association with their origins and atmospheric conditions. Clustering analysis showed that the air masses arriving to Granada can be classified as: (1) tropical continental air masses coming from the Mediterranean Sea, (2) tropical and warm polar maritime air masses produced over the Atlantic Ocean, and (3) continental air masses originated over Europe and Northern Africa. The PSCF and CWT methods confirmed that the main source areas of 7Be are located in the Atlantic coast of southern Morocco, and Northern Africa. On the other hand, southern France and the Algerian desert were found to be the main region sources of 210Pb. In addition, the Mediterranean Basin has been postulated as a strong source region for 7Be and 210Pb. Furthermore, the PSCF and CWT models show that the regions with larger 7Be/210Pb ratios are located in the Atlantic Ocean, due to frequent stratospheric intrusions specially during the winter months.

  • Sources of the seasonal-trend behaviour and periodicity modulation of 7Be air concentration in the atmospheric surface layer observed in southeastern Spain

    • The components and the periodicity characterising airborne 7Be monthly concentration collected in Granada (SE Spain) between 2000 and 2018 are studied. These 19 years of measurements are particularly significant for the 7Be analysis as they cover the complete 24th solar cycle, 2008–2018. A Time Series Decomposition (TSD) technique has been applied to determine the different components present in the dataset: the trend, seasonal and irregular components. Then, the atmospheric parameters (Precipitation (Pp), Relative Humidity (RH), Temperature (Temp), Wind speed (WS), Total Cloudiness (TC), Low Cloudiness (LC)), teleconnection indices and the solar activity (Sun Spot Number, SSN) were used to justify the variability of each component. SSN and the teleconnection indices (Western Mediterranean Oscillation (WeMO), Atlantic Multidecadal Oscillation (AMO) and Quasi Biennial Oscillation (QBO)) are found to have a major impact on the trend component of 7Be. In turn, all the atmospheric parameters were found to have strong impact on the seasonal component. In order to observe the change in the impact of the parameters from the 24th solar cycle (from 2008 to 2018) to the complete time series (starting in 2000), the comparison between the two periods has been carried out. The results show that, over the 24th solar cycle, the SSN impacted predominantly on the trend component, whereas atmospheric parameters showed a slightly higher impact on the seasonal component. In addition, some atmospheric factors (Temp, RH, Pp, TC, LC) appeared to partially affect the irregular component. The analysis of the two phases of the 24th solar cycle shows that the influence of SSN is higher during the descending phase than during the ascending one. This behaviour is opposed to the one found for the 23rd solar cycle, which clarifies the special characteristics of the 24th cycle. Besides that, the Continuous Wavelet Transform (CWT) analysis was applied in order to extract the possible periodicities characterising 7Be data. The analysis revealed three sets of periodicities. Then, the Wavelet Coherence Analysis (WCA) method was particularly useful to study coherences between 7Be data and teleconnection indices. The 7Be concentrations in the large period (11–14 years) detected by CWT was found to be mainly modulated by WeMO, AMO and QBO, while the NAO modulates the smaller periods. © 2019 Elsevier Ltd

  • Chemiluminescent fingerprints from airborne particulate matter: A luminol-based assay for the characterization of oxidative potential with kinetical implications

    • In this study, a new chemiluminescent method based on the dependence of luminol light emission induced by free radicals in airborne particulate matter (PM) is proposed as a screening assay for the rapid characterization of samples from different sources based on their redox properties. This parameter is considered critical for assessing particulate matter toxicity and its impacts on human health. We propose a cell-free, luminescent assay to evaluate the redox potential of particulate matter directly on the filters employed to collect it. A joint chemometric approach based on Principal Component Analysis and Hotelling Analysis was applied to quickly sort out ambient particulate samples with a significantly different light emission profile caused by Luminol reaction. Based on Spearman correlation analysis, the association of the samples light emission intensity with their chemical composition and emission sources was attempted. The overall methodology was tested with certified reference materials and applied to two series of particulate matter samples previously subjected to thorough chemical speciation and subsequent source apportionment. The results show the effectiveness of the luminescent method, allowing the quick assessment of particulate matter oxidative potential, but providing further evidence on the complexity of the oxidative potential determination in this kind of samples. The chemometric processing of the whole dataset clearly highlights the distinct behavior among the two series of samples, the certificate standard reference materials, and the blank controls, supporting the suitability of the approach.
