  • Rationale and Study Design for an Individualized Perioperative Open Lung Ventilatory Strategy in Patients on One-Lung Ventilation (iPROVE-OLV)

    • Objective: The aim of this clinical trial is to examine whether it is possible to reduce postoperative complications using an individualized perioperative ventilatory strategy versus using a standard lung-protective ventilation strategy in patients scheduled for thoracic surgery requiring one-lung ventilation. Design: International, multicenter, prospective, randomized controlled clinical trial. Setting: A network of university hospitals. Participants: The study comprises 1,380 patients scheduled for thoracic surgery. Interventions: The individualized group will receive intraoperative recruitment maneuvers followed by individualized positive end-expiratory pressure (open lung approach) during the intraoperative period plus postoperative ventilatory support with high-flow nasal cannula, whereas the control group will be managed with conventional lung-protective ventilation. Measurements and Main Results: Individual and total number of postoperative complications, including atelectasis, pneumothorax, pleural effusion, pneumonia, acute lung injury; unplanned readmission and reintubation; length of stay and death in the critical care unit and in the hospital will be analyzed for both groups. The authors hypothesize that the intraoperative application of an open lung approach followed by an individual indication of high-flow nasal cannula in the postoperative period will reduce pulmonary complications and length of hospital stay in high-risk surgical patients.   • Global phenological insensitivity to shifting ocean temperatures among seabirds

    • Reproductive timing in many taxa plays a key role in determining breeding productivity(1), and is often sensitive to climatic conditions(2). Current climate change may alter the timing of breeding at different rates across trophic levels, potentially resulting in temporal mismatch between the resource requirements of predators and their prey(3). This is of particular concern for higher-trophic-level organisms, whose longer generation times confer a lower rate of evolutionary rescue than primary producers or consumers(4). However, the disconnection between studies of ecological change in marine systems makes it difficult to detect general changes in the timing of reproduction(5). Here, we use a comprehensive meta-analysis of 209 phenological time series from 145 breeding populations to show that, on average, seabird populations worldwide have not adjusted their breeding seasons over time (-0.020 days yr(-1)) or in response to sea surface temperature (SST) (-0.272 days degrees C-1) between 1952 and 2015. However, marked between-year variation in timing observed in resident species and some Pelecaniformes and Suliformes (cormorants, gannets and boobies) may imply that timing, in some cases, is affected by unmeasured environmental conditions. This limited temperature-mediated plasticity of reproductive timing in seabirds potentially makes these top predators highly vulnerable to future mismatch with lower-trophic-level resources(2).

    • Hydrochemical investigations of groundwater and surface water were carried out to better understand the spatial distribution of As, major ions and trace elements.The study was carried out to evaluate the sources of dissolved species and elucidate the processes that govern the evolution of natural water in the Lower Katari Basin.The study area is close to the Titicaca Lake (Cohana Bay) formed by sediments of the Quaternary system, deposited in the fluvio-glacial to fluvio-lacustrineenvironment and geologic formations of the Devonian and Neogene system of volcanic origin. The study area has several environmental problems mainly caused bycontaminants such as heavy metals, nutrients, and bacteria. These problems are linked to the urban and industrial wastes, natural geologic conditions, and miningactivities carried out upstream of the Katari Basin, where rivers discharge into the Cohana Bay.A total of 37 water samples were collected during wet season, 31 groundwater samples including drinking water wells and six surface water samples. Thehierarchical cluster analysis and principal component analysis were applied to hydrochemical data. Results show high salinity in groundwater related to theevaporation causing serious problems for the groundwater quality and rendering it unsuitable for drinking. Dissolved As concentration ranges from 0.7 to 89.7 μg/L;the principal source of As could be the alteration of volcanic rocks, more than 48% of the shallow groundwater samples exceeded the WHO guideline value for As andmore than 22% for NO3-. Groundwater has neutral to slightly alkaline pH, and moderately oxidizing character. The groundwater chemistry reveals considerablevariability, ranging from Na-SO4,Cl type through mixed Na-HCO3 type and Ca,Na-HCO3,Cl type. The distribution of trace elements shows a large range of concentrations.Speciation of As indicates that the predominant oxidation state is As (V). The geochemical modelling indicates that As could be associated with ironoxides and hydroxides which are probably the most important mineral phases for the As adsorption. The spatial distribution and the variation of dissolved Asconcentration in groundwater is governed by the variability in geological characteristics of the region that raises a significant concern about drinking water quality.

    • Single-cell technologies have become essential to driving discovery in both basic and translational investigative dermatology. Despite the multitude of available datasets, a central reference atlas of normal human skin, which can serve as a reference resource for skin cell types, cell states, and their molecular signatures, is still lacking. For any such atlas to receive broad acceptance, participation by many investigators during atlas construction is an essential prerequisite. As part of the Human Cell Atlas project, we have assembled a Skin Biological Network to build a consensus Human Skin Cell Atlas and outline a roadmap toward that goal. We define the drivers of skin diversity to be considered when selecting sequencing datasets for the atlas and list practical hurdles during skin sampling that can result in data gaps and impede comprehensive representation and technical considerations for tissue processing and computational analysis, the accounting for which should minimize biases in cell type enrichments and exclusions and decrease batch effects. By outlining our goals for Atlas 1.0, we discuss how it will uncover new aspects of skin biology.

  • Characterization of fan spray atomizers through numerical simulation

    • The present paper focuses on the mathematical modeling of industrial fan spray atomizers. The two-phase flow taking place inside the nozzle's tip and the exterior region near the outlet of three different industrial nozzle designs has been modeled and simulated. As a result, valuable information has been obtained regarding the influence of the inner geometry on the flow and also the formation and development of the liquid sheet. Characteristic magnitudes such as the discharge coefficient and the liquid sheet thickness factor have been obtained and validated through experimental measurements. The accumulation of liquid at the border of fan-shaped liquid sheets, also known as rim, has been studied in the analyzed designs, revealing the presence of a tangential velocity component in the liquid sheet and a relationship between the incoming flow rate of the rim and the angle of the liquid sheet. The dependence of the results on turbulence modeling has also been analyzed, drawing interesting conclusions regarding their influence on the liquid sheet mean flow characteristics and on the surrounding gas. Thus, the mathematical model developed has been proved to be a useful tool for nozzle manufacturers; it provides the most important characteristic parameters of the liquid sheet formed given certain nozzle geometry and, additionally, those data necessary to carry out studies of instability, breakup and atomization of the liquid sheet.

The present paper presents criteria for the determination of the dominant disintegration regime in industrial fan-spray atomizers through CFD simulations accounting for the atomizer’s geometry.

    • The main objective of this work is to determine the most important design parameters in fan spray atomizers and their effect in the atomization features in order to provide a design tool for nozzle manufacturers. A mathematical model relating the geometry and operating conditions to the characteristics of the spray has been obtained and validated. Three different sub-models have been used in series, so that the results of one model become inputs of the next one, namely multiphase flow simulation, instability and break-up model and maximum entropy model. These have been joined achieving a global model of the whole primary atomization process.

    • The present paper focuses on the linear spatial instability of a viscous two-dimensional liquid sheet bounded by two identical viscous gas streams. The Orr–Sommerfeld differential equations and the boundary conditions of the flow configuration are numerically solved using Chebyshev series expansions and the collocation method. The strong dependence of the instability parameters on the velocity profiles is proven by using both quadratic and error functions to define the base flow in the liquid sheet and the gas shear layer. The sensitivity of the spatial instability growth rate to changes in the dimensionless parameters of the problem is assessed. Regarding the liquid sheet Reynolds number, it has been observed that, when this parameter increases, both the most unstable growth rate and the corresponding wavenumber decrease, whereas the cutoff wavenumber increases. The results of this analysis are compared with temporal theory through Gaster transformation. The effects liquid and gas viscosity have on instability are studied by comparing the instability curves given by the presented model with those from an inviscid liquid sheet and a viscous liquid sheet in an inviscid gaseous medium. The model presented in this paper features a variation in the cutoff wavenumber with all the governing parameters of the problem, whereas that provided by cases that account for an inviscid surrounding gas depends only on the liquid sheet Weber number and the ratio of gas to liquid densities. Results provided by the presented model have been experimentally validated and show that quadratic profiles have a greater capacity to predict the disturbance wavelength.

    • Sustainability requires a combination of meaningful co-production of locally relevant solutions, synthesis of insights gained across regions, and increased cooperation between science, policy and practice. The Programme for Ecosystem Change and Society (PECS) has been coordinating Place-Based Social-Ecological Sustainability Research (PBSESR) across the globe and emphasizes the need for regional scientific nodes from diverse biocultural regions to inform sustainability science and action. In this paper, we assess the strengths of the PBSESR communities in Latin America and the Caribbean (LAC). We provide an overview of PBSESR literature associated with this region and highlight the achievements of two prominent regional networks: The Social-Ecological Systems and Sustainability Research Network from Mexico (SocioEcoS) and the South American Institute for Resilience and Sustainability Studies from Uruguay (SARAS Institute). Finally, we identify the potential in these nodes to constitute a regional PECS node in Latin America and discuss the capacity needed to ensure such function. The results of the literature review show that while still loosely interconnected across the region, networks play key roles in connecting otherwise cloistered teams and we illustrate how the SocioEcoS network (focusing on transdisciplinary co-production of knowledge towards sustainability) and the SARAS Institute (focusing on innovative approaches for looking at complex social-ecological problems, rooted in slow science and arts) operate as key connectors in the region. We conclude that these organizations combined can embody a Latin American node for PECS, and would thereby not only contribute to regional but also global capacities to advance the sustainability agenda.

  • Novel genes and sex differences in COVID-19 severity

    • Here, we describe the results of a genome-wide study conducted in 11 939 coronavirus disease 2019 (COVID-19) positive cases with an extensive clinical information that were recruited from 34 hospitals across Spain (SCOURGE consortium). In sex-disaggregated genome-wide association studies for COVID-19 hospitalization, genome-wide significance (P < 5 × 10−8) was crossed for variants in 3p21.31 and 21q22.11 loci only among males (P = 1.3 × 10−22 and P = 8.1 × 10−12, respectively), and for variants in 9q21.32 near TLE1 only among females (P = 4.4 × 10−8). In a second phase, results were combined with an independent Spanish cohort (1598 COVID-19 cases and 1068 population controls), revealing in the overall analysis two novel risk loci in 9p13.3 and 19q13.12, with fine-mapping prioritized variants functionally associated with AQP3 (P = 2.7 × 10−8) and ARHGAP33 (P = 1.3 × 10−8), respectively. The meta-analysis of both phases with four European studies stratified by sex from the Host Genetics Initiative (HGI) confirmed the association of the 3p21.31 and 21q22.11 loci predominantly in males and replicated a recently reported variant in 11p13 (ELF5, P = 4.1 × 10−8). Six of the COVID-19 HGI discovered loci were replicated and an HGI-based genetic risk score predicted the severity strata in SCOURGE. We also found more SNP-heritability and larger heritability differences by age (<60 or ≥60 years) among males than among females. Parallel genome-wide screening of inbreeding depression in SCOURGE also showed an effect of homozygosity in COVID-19 hospitalization and severity and this effect was stronger among older males. In summary, new candidate genes for COVID-19 severity and evidence supporting genetic disparities among sexes are provided.

    • Objective: To define “best possible” outcomes for bariatric surgery (BS)(Roux-en-Y gastric bypass [RYGB] and sleeve gastrectomy [SG]).Background: Reference values for optimal surgical outcomes in well-defined low-risk bariatric patients have not been established so far. Consequently, outcome comparison across centers and over time is impeded by heterogeneity in case-mix.Methods: Out of 39,424 elective BS performed in 19 high-volume academic centers from 3 continents between June 2012 and May 2017, we identified 4120 RYGB and 1457 SG low-risk cases defined by absence of previous abdominal surgery, concomitant procedures, diabetes mellitus, sleep apnea, cardiopathy, renal insufficiency, inflammatory bowel disease, immunosuppression, anticoagulation, BMI>50 kg/m2 and age>65 years. We chose clinically relevant endpoints covering the intra- and postoperative course. Complications were graded by severity using the comprehensive complication index. Benchmark values were defined as the 75th percentile of the participating centers’ median values for respective quality indicators.Results: Patients were mainly females (78%), aged 38±11 years, with a baseline BMI 40.8 ± 5.8 kg/m2. Over 90 days, 7.2% of RYGB and 6.2% of SG patients presented at least 1 complication and no patients died (mortality in nonbenchmark cases: 0.06%). The most frequent reasons for readmission after 90-days following both procedures were symptomatic cholelithiasis and abdominal pain of unknown origin. Benchmark values for both RYGB and SG at 90-days postoperatively were 5.5% Clavien-Dindo grade ≥IIIa complication rate, 5.5% readmission rate, and comprehensive complication index ≤33.73 in the subgroup of patients presenting at least 1 grade ≥II complication.Conclusion: Benchmark cutoffs targeting perioperative outcomes in BS offer a new tool in surgical quality-metrics and may be implemented in quality-improvement cycle.ClinicalTrials.gov Identifier NCT03440138

    • A parametric study based on design of experiments (DoE) techniques was carried out by computational simulation in order to evaluate the effect that design parameters have on heat transfer and pressure loss of an impinging jet in a cross-flow configuration. The main effects of each parameter and the interactions between parameters were analyzed in detail through the Response Surface Methodology (RSM). Additionally, the potential of the impinging jet in a cross-flow configuration was assessed by calculating the optimal values of the parameters and comparing the cooling efficiency of the resulting configuration with the efficiency of the conventional cross-flow configuration. It was found that the degree to which the average heat transfer coefficient is enhanced as the result of adding an impinging jet depends on the height of the cooled component. Specifically, it was found that the higher the component, the more significant the enhancement.

Results of the heat transfer and the sprayed refrigerant film thickness measurements are presented. The film thickness measurements have been made with a high speed camera equipped with a long distance microscope. It has been found that there is a relation between the variation in the average Nusselt number and the film thickness along the spray cooling boiling curve. The heat transfer regimes along that curve are related not only with a variation in the average Nusselt number but also with changes in the film thickness. The qualitative analysis of those variations has served to understand better the heat transfer mechanisms occurring during the spray cooling.

    • Experimental measurements in a spray cooling test rig were carried out for two different heater surface roughnesses and for two different types of nozzles with the dielectric refrigerant R134a. In this paper, results of the heat transfer measurements are presented. The analysis of the results explains the influence of the volumetric flow rate, the surface roughness and the type of nozzle (through the spray parameters) on the spray cooling boiling curve, on the Nusselt number and on the efficiency. It has been found that the effect of a smooth roughness is to delay the onset of the nucleate boiling, but once this regime has started, boiling is so fast that the CHF (Critical Heat Flux) is reached at lower heater temperatures and lower heat fluxes. In a companion paper the sprayed refrigerant film thickness measurements and its relation with the heat transfer measurements are presented.

    • COVID-19-related inflammation, endothelial dysfunction, and coagulopathy may increase the bleeding risk and lower the efficacy of revascularization treatments in patients with acute ischemic stroke (AIS). We aimed to evaluate the safety and outcomes of revascularization treatments in patients with AIS and COVID-19.This was a retrospective multicenter cohort study of consecutive patients with AIS receiving intravenous thrombolysis (IVT) and/or endovascular treatment (EVT) between March 2020 and June 2021 tested for severe acute respiratory syndrome coronavirus 2 infection. With a doubly robust model combining propensity score weighting and multivariate regression, we studied the association of COVID-19 with intracranial bleeding complications and clinical outcomes. Subgroup analyses were performed according to treatment groups (IVT-only and EVT).Of a total of 15,128 included patients from 105 centers, 853 (5.6%) were diagnosed with COVID-19; of those, 5,848 (38.7%) patients received IVT-only and 9,280 (61.3%) EVT (with or without IVT). Patients with COVID-19 had a higher rate of symptomatic intracerebral hemorrhage (SICH) (adjusted OR 1.53; 95% CI 1.16-2.01), symptomatic subarachnoid hemorrhage (SSAH) (OR 1.80; 95% CI 1.20-2.69), SICH and/or SSAH combined (OR 1.56; 95% CI 1.23-1.99), 24-hour mortality (OR 2.47; 95% CI 1.58-3.86), and 3-month mortality (OR 1.88; 95% CI 1.52-2.33). Patients with COVID-19 also had an unfavorable shift in the distribution of the modified Rankin score at 3 months (OR 1.42; 95% CI 1.26-1.60).Patients with AIS and COVID-19 showed higher rates of intracranial bleeding complications and worse clinical outcomes after revascularization treatments than contemporaneous non-COVID-19 patients receiving treatment. Current available data do not allow direct conclusions to be drawn on the effectiveness of revascularization treatments in patients with COVID-19 or to establish different treatment recommendations in this subgroup of patients with ischemic stroke. Our findings can be taken into consideration for treatment decisions, patient monitoring, and establishing prognosis.The study was registered under ClinicalTrials.gov identifier NCT04895462.

    • The air flow around a cubic obstacle mounted on one wall of a rectangular channel was studied experimentally. The obstacle represents an electronic component and the channel the space between two parallel printed circuit boards (PCBs). The flow was produced by the combination of a channel stream and a jet which issued from a circular nozzle placed at the wall opposite from where the component is mounted. With this aim, a test rig was designed and built to carry out experiments with both the above mentioned configurations and other cooling arrangements. Planar Particle Image Velocimetry (PIV) was employed to measure the instantaneous flow velocity on several planes covering the space around the component. The mean velocity and the Reynolds stresses were obtained from averaging the instantaneous velocity, and the mean flow showed a complex pattern with different features such as recirculation bubbles, vortices, detachment and reattachment zones. The influence of two parameters, namely the channel Reynolds number and the jet-to-channel Reynolds number ratio, on these flow features was studied considering nine cases that combined three values of the channel Reynolds number (3410, 5752 and 8880) and three values of the ratio (0.5, 1.0 and 1.5). The results show that the Reynolds number ratio determines the drag produced on the jet and the deflection from its geometric axis due to the channel stream. In all the cases corresponding to the lowest value of the ratio, the jet was dragged and did not impact the component. This fact accounts for the non-existence of the Upper Horseshoe Vortex and changes in the flow characteristics at the region over the component.

    • In quantitative magnetic resonance T 1 mapping, the variable flip angle (VFA) steady state spoiled gradient recalled echo (SPGR) imaging technique is popular as it provides a series of high resolution T 1 weighted images in a clinically feasible time. Fast, linear methods that estimate T 1 maps from these weighted images have been proposed, such as DESPOT1 and iterative re-weighted linear least squares. More accurate, non-linear least squares (NLLS) estimators are in play, but these are generally much slower and require careful initialization. In this paper, we present NOVIFAST, a novel NLLS-based algorithm specifically tailored to VFA SPGR T 1 mapping. By exploiting the particular structure of the SPGR model, a computationally efficient, yet accurate and precise T 1 map estimator is derived. Simulation and in vivo human brain experiments demonstrate a twenty-fold speed gain of NOVIFAST compared with conventional gradient-based NLLS estimators while maintaining a high precision and accuracy. Moreover, NOVIFAST is eight times faster than the efficient implementations of the variable projection (VARPRO) method. Furthermore, NOVIFAST is shown to be robust against initialization.

    • Historical geological events and climatic changes are believed to have played important roles in shaping the current distribution of species. However, sympatric species may have responded in different ways to such climatic fluctuations. Here we compared genetic structures of two corvid species, the Azure-winged Magpie Cyanopica cyanus and the Eurasian Magpie Pica pica, both widespread but with different habitat dependence and some aspects of breeding behavior. Three mitochondrial genes and two nuclear introns were used to examine their co-distributed populations in East China and the Iberian Peninsula. Both species showed deep divergences between these two regions that were dated to the late Pliocene/early Pleistocene. In the East Chinese clade of C. cyanus, populations were subdivided between Northeast China and Central China, probably since the early to mid-Pleistocene, and the Central subclade showed a significant pattern of isolation by distance. In contrast, no genetic structure was found in the East China populations of P. pica. We suggest that the different patterns in the two species are at least partly explained by ecological differences between them, especially in habitat preference and perhaps also breeding behavior. These dissimilarities in life history traits might have affected the dispersal and survival abilities of these two species differently during environmental fluctuations.
