  • Design and Baseline Characteristics of the Finerenone in Reducing Cardiovascular Mortality and Morbidity in Diabetic Kidney Disease Trial

    • <b><i>Background:</i></b> Among people with diabetes, those with kidney disease have exceptionally high rates of cardiovascular (CV) morbidity and mortality and progression of their underlying kidney disease. Finerenone is a novel, nonsteroidal, selective mineralocorticoid receptor antagonist that has shown to reduce albuminuria in type 2 diabetes (T2D) patients with chronic kidney disease (CKD) while revealing only a low risk of hyperkalemia. However, the effect of finerenone on CV and renal outcomes has not yet been investigated in long-term trials. <b><i>Patients and</i></b> <b><i>Methods:</i></b> The Finerenone in Reducing CV Mortality and Morbidity in Diabetic Kidney Disease (FIGARO-DKD) trial aims to assess the efficacy and safety of finerenone compared to placebo at reducing clinically important CV and renal outcomes in T2D patients with CKD. FIGARO-DKD is a randomized, double-blind, placebo-controlled, parallel-group, event-driven trial running in 47 countries with an expected duration of approximately 6 years. FIGARO-DKD randomized 7,437 patients with an estimated glomerular filtration rate ≥25 mL/min/1.73 m<sup>2</sup> and albuminuria (urinary albumin-to-creatinine ratio ≥30 to ≤5,000 mg/g). The study has at least 90% power to detect a 20% reduction in the risk of the primary outcome (overall two-sided significance level α = 0.05), the composite of time to first occurrence of CV death, nonfatal myocardial infarction, nonfatal stroke, or hospitalization for heart failure. <b><i>Conclusions:</i></b> FIGARO-DKD will determine whether an optimally treated cohort of T2D patients with CKD at high risk of CV and renal events will experience cardiorenal benefits with the addition of finerenone to their treatment regimen.     • Dijksterhuis and van Knippenberg (1998) reported that participants primed with a category associated with intelligence (professor) subsequently performed 13% better on a trivia test than participants primed with a category associated with a lack of intelligence (soccer hooligans). In two unpublished replications of this study designed to verify the appropriate testing procedures, Dijksterhuis, van Knippenberg, and Holland observed a smaller difference between conditions (2%-3%) as well as a gender difference: Men showed the effect (9.3% and 7.6%), but women did not (0.3% and -0.3%). The procedure used in those replications served as the basis for this multilab Registered Replication Report. A total of 40 laboratories collected data for this project, and 23 of these laboratories met all inclusion criteria. Here we report the meta-analytic results for those 23 direct replications (total N = 4,493), which tested whether performance on a 30-item general-knowledge trivia task differed between these two priming conditions (results of supplementary analyses of the data from all 40 labs, N = 6,454, are also reported). We observed no overall difference in trivia performance between participants primed with the professor category and those primed with the hooligan category (0.14%) and no moderation by gender.

  • IFNγ and iNOS-Mediated Alterations in the Bone Marrow and Thymus and Its Impact on Mycobacterium avium-Induced Thymic Atrophy

    • Disseminated infection with the high virulence strain ofMycobacterium avium25291 leads to progressive thymic atrophy. We previously showed thatM. avium-induced thymic atrophy results from increased glucocorticoid levels that synergize with nitric oxide (NO) produced by interferon gamma (IFNγ) activated macrophages. Where and how these mediators act is not understood. We hypothesized that IFNγ and NO promote thymic atrophy through their effects on bone marrow (BM) T cell precursors and T cell differentiation in the thymus. We show thatM. aviuminfection cause a reduction in the percentage and number of common lymphoid progenitors (CLP). Additionally, BM precursors from infected mice show an overall impaired ability to reconstitute thymi of RAGKO mice, in part due to IFNγ. Thymi from infected mice present an IFNγ and NO-driven inflammation. When transplanted under the kidney capsule of uninfected mice, thymi from infected mice are unable to sustain T cell differentiation. Finally, we observed increased thymocyte deathviaapoptosis after infection, independent of both IFNγ and iNOS; and a decrease on active caspase-3 positive thymocytes, which is not observed in the absence of iNOS expression. Together our data suggests thatM. avium-induced thymic atrophy results from a combination of defects mediated by IFNγ and NO, including alterations in the BM T cell precursors, the thymic structure and the thymocyte differentiation.

  • Oceanographic processes and morphosedimentary products along the Iberian margins: A new multidisciplinary approach

    • Our understanding of bottom-currents and associated oceanographic processes (e.g., overflows, barotropic tidal currents) including intermittent processes (e.g., vertical eddies, deep sea storms, horizontal vortices, internal waves and tsunamis) is rapidly evolving. Many deep-water processes remain poorly understood due to limited direct observations, but can generate significant depositional and erosional features on both short and long term time scales. This paper represents a review work, which describes for the first time these oceanographic processes and examines their potential role in the sedimentary features along the Iberian continental margins. This review explores the implications of the studied processes, given their secondary role relative to other factors such as mass-transport and turbiditic processes, and highlights three major results: a) contourite depositional and erosional features are ubiquitous along the margins, indicating that bottom currents and associated oceanographic processes control the physiography and sedimentation; b) the position of interfaces between major water masses and their vertical and spatial variation in time specifically appears to exert primary control in determining major morphologic changes along the slope gradient, including the contourite terraces development; and c) contourites deposits exhibit greater variation than the established fades model suggests. Therefore, a consistent facies model however faces substantial challenges in terms of the wide range of oceanographic processes that can influence in their development. An integrated interpretation of these oceanographic processes requires an understanding of contourites, seafloor features, their spatial and temporal evolution, and the near-bottom flows that form them. This approach will synthesize oceanographic data, seafloor morphology, sediments and seismic images to improve our knowledge of permanent and intermittent processes around Iberia, and evaluate their conceptual and regional role in the margin's sedimentary evolution. Given their complexes, three-dimensional and temporally-variable nature, integration of these processes into sedimentary, oceanographic and climatological frameworks will require a multidisciplinary approach that includes Geology, Physical oceanography, Paleoceanography and Benthic Biology.

    • Our understanding of the role of bottom-current and associated oceanographic processes (as overflows, barotropic currents, tides or intermittent processes like giant vertical eddies, deep sea storms, horizontal vortices, internal waves and tsunamis) is rapidly improving. But, most of these oceanographic deep-water processes remains poorly understood and have been rarely observed but believed to be capable of generating depositional and erosional features in many contexts at both short and long term. In present compilation we described these oceanographic processes including examples or their potential role in the sedimentation processes around the Iberia margin. Importance of these processes is highlighted and its implication discussed to demonstrate that these oceanographic processes are very important is affecting the shape and evolution of the Iberian continental margin and adjacent oceanic basins although is not still considered as important as other factors as mass-transport deposits and turbiditic processes. To have a better understanding on the implication of all these oceanographic processes connections have to be made between (contourite) sea-floor features, their spatial and temporal evolution, and the near-bottom flows that form them. The only way to achieve that in next decades is by a more intensive collaboration between specialist from Geology, Oceanography and Benthic Biology in new multi-disciplinary studies. Consequently, further multi-disciplinary approach uses oceanographic data, sea-floor morphology, sediments and seismic characterization will be essential for improve our knowledge of these permanent and intermittent processes around Iberia and evaluate their conceptual and regional role in the margins evolution over time and space.

  • Meta-analysis assessing the sensitivity and specificity of 18F-FDG PET/CT for the diagnosis of prosthetic valve endocarditis (PVE) using individual patient data (IPD)

    • Importance The use of 18F-FDG PET/CT in diagnostic algorithms for PVE has increased since publication of studies and guidelines advocating its use. The assessment of test accuracy has been limited by small study sizes. We undertook a systematic review using individual patient data (IPD) meta-analysis techniques. Objective To estimate the summary sensitivity and specificity of 18F-FDG PET/CT in diagnosing PVE. We also assessed the effect of patient factors on test accuracy as defined by changes in the odds ratios associated with each factor. The effect of the PET/CT study on the final diagnosis was also assessed when compared to the preliminary Duke classification to determine in which patient group 18F-FDG PET/CT had the greatest utility. Study Selection Studies were included if PET/CT was performed for suspicion of PVE and IPD of both the PET/CT result and final diagnosis defined by a gold-standard assessment was available. There were 3 possible final diagnoses ("definite PVE," "possible PVE," and "rejected PVE"). Results Seventeen studies were included with IPD available for 537 patients (from 538 scans). The summary sensitivity and specificity were 85% (95% CI 74.2%-91.8%) and 86.5% (95% CI 75.8%-92.9%) respectively when patients with final diagnosis of "possible PVE" were classified as positive for PVE. When this group was classified as negative for PVE, sensitivity was 87.4% (95% CI 80.4%-92.1%) and specificity was 84.9% (95% CI 71.5%-92.6%). Patients with a known pathogen (especially coagulase negative staphylococcal species), elevated CRP, a biological or aortic valve infection appeared more likely to have an accurate PET/CT diagnosis. Those with a mechanical valve, prior antibiotic treatment or a transcatheter aortic valve replacement valve were less likely to have an accurate test. Time since valve implantation and the presence of surgical adhesive did not appear to affect test accuracy. Of the patients with a preliminary Duke classification of "possible PVE," 84% received a more conclusive final diagnosis of "definite" or "rejected" PVE after the PET/CT study. Conclusions and Relevance 18F-FDG PET/CT has high sensitivity and specificity in diagnosing PVE and the diagnostic utility is greatest in patients with a preliminary Duke classification of "possible PVE." Some patient factors appear to affect test accuracy, though these results should be interpreted with caution given low patient numbers for subgroup analyses.

    • A serial millisecond crystallography (SMX) facility has recently been implemented at the macromolecular crystallography beamline, MX2 at the Australian Synchrotron. The setup utilizes a combination of an EIGER X 16M detector system and an in-house developed high-viscosity injector, "Lipidico." Lipidico uses a syringe needle to extrude the microcrystal-containing viscous media and it is compatible with commercially available syringes. The combination of sample delivery via protein crystals suspended in a viscous mixture and a millisecond frame rate detector enables high-throughput serial crystallography at the Australian Synchrotron. A hit-finding algorithm, based on the principles of "robust-statistics," is employed to rapidly process the data. Here we present the first SMX experimental results with a detector frame rate of 100 Hz (10 ms exposures) and the Lipidico injector using a mixture of lysozyme microcrystals embedded in high vacuum silicon grease. Details of the experimental setup, sample injector, and data analysis pipeline are designed and developed as part of the Australian Synchrotron SMX instrument and are reviewed here.     • The presence of pore water in mixtures can cause premature failure of hot-mix asphalt pavements. The processes typically associated with moisture damage are complex and occur over a long period of time in the field. Short of being able to simulate each of the possible mechanisms of moisture damage directly, the ideal laboratory-based conditioning system should accelerate the penetration of moisture through the asphalt film and at the same time minimize complicating effects. This paper presents the results of an experiment conducted to determine whether it was possible to use cyclic pore pressures to induce enough damage to distinguish between mixtures known to be highly resistant from mixtures known to be susceptible to moisture damage. Experimental constraints included requirements that conditioning be accomplished within a reasonable length of time and that typical laboratory equipment be used. Evaluation of the resulting effects of moisture damage included the use of the Superpave (R) indirect tension test and the energy ratio parameter. Findings show that cyclic pore pressures can be used to accelerate moisture damage enough to distinguish between mixtures known to be strippers and those known to be highly resistant to moisture damage. The use of cyclic pore pressures to accelerate moisture damage in mixtures may minimize the introduction of other confounding damage effects on the mixtures.

    • This paper describes the use of the displacement discontinuity boundary element method to model the microstructure of asphalt mixtures. The problem of indirect tension test (IDT) was used to demonstrate the application of the method in the modeling of the cracking behavior of asphalt mixtures. An IDT sample was modeled by three sets of displacement discontinuity boundary elements based on location and function: (1) on the periphery of the specimen, (2) inside the specimen to represent the aggregate structure, and (3) internal fracture path elements inside the aggregate structure. The mastic was modeled by a nonlinear failure law. The simulation of the cracking behavior of the IDT test with this method showed a good agreement with laboratory observations. The predicted stress-strain curves matched the experimental results for both vertical compressive stress and horizontal tensile stress. Crack patterns from these simulations were also similar to the patterns observed visually in the laboratory. In summary, explicit fracture modeling has shown promise as a tool for studying the crack growth and localization of asphalt mixtures.

    • An understanding of the cracking behavior of hot mix asphalt mixtures provides an important basis for the development of mixtures with improved cracking resistance and pavement cracking perfonnance models. This paper describes the use of random assemblies of displacement discontinuity boundary elements to model the behavior of asphalt mixtures. These random assemblies of boundary elements form particles of varying sizes, which are arranged to simulate the discrete nature of mixtures. To account for the aggregate and mastic properties, each particle is connected to other particles by specifying the cohesion, friction, and tensile properties of the material between particles. The crack initiation and crack growth are simulated using two distinct crack growth laws, based on sequential and parallel crack growth rules. Cracks are allowed to grow along particle boundaries. A series of simulations were performed of the load-deformation and cracking behavior of a typical 19.0-mm nominal maximum aggregate size mixture commonly used by the Florida Department of Transportation. The effects of crack growth rules, particle size, and localization to geometric effects were studied. In summary, the method presented appears to provide a valuable tool for studying the mechanistic behavior of asphalt mixtures.

  • Identification of a Physical Model to Evaluate Rutting Performance of Asphalt Mixtures

    • The objective of this study is to identify a physical model that can provide reliable predictions about a mixture's ability to resist permanent deformation under realistic stress states. Key differences were identified between stress states under the existing Asphalt Pavement Analyzer (APA) loading device (hose) and stress states under radial truck tires, which may indicate potentially different rutting mechanisms. It was shown that the APA hose was not capturing the critical lateral stresses found to be detrimental to rutting and cracking of HMA pavements. A new loading device (rib) was designed and constructed for use in the APA that more closely represents stress states found under radial tires. Contact-stress measurements under the two loading devices - hose and rib - showed that the rib was able to reproduce the lateral stresses found under individual ribs on a radial-tire tread. Subsequent finite element modeling also showed that the rib appeared to generate similar shear stress patterns to those found under the modeled radial-tire load. A new method was developed to measure deformations on the surface of APA specimens, where a contour gauge was used to record and store the entire surface profile of the sample throughout the progress of the test. An area-change parameter, which reflects volume change, was introduced to calculate the volumetric changes in the specimen. The area-change parameter can be used to determine whether specimen rutting is primarily due to shear instability or consolidation. Two mixtures of known field performance - poor and good - were tested to evaluate the test's ability to predict performance with the new loading device and the new measurement and interpretation system. Results showed that the new system (loading strip and profile measurement method) appears to have greater potential of evaluating a mixture's potential for instability rutting than the original (hose and single rut-depth measurement) configuration.

  • Larvicidal Activities against Aedes aegypti of Supernatant and Pellet Fractions from Cultured Bacillus spp. Isolated from Amazonian Microenvironments

    • The Aedes aegypti mosquito is the primary vector of Dengue, Chikungunya and Zika causing major problems for public health, which requires new strategies for its control, like the use of entomopathogenic microorganisms. In this study, bacteria from various Amazonian environments were isolated and tested for their pathogenicity to A. aegypti larvae. Following thermal shock to select sporulated Bacillus spp., 77 bacterial strains were isolated. Molecular identification per 16S RNA sequences revealed that the assembled strains contained several species of the genus Bacillus and one species each of Brevibacillus, Klebsiella, Serratia, Achromobacter and Brevundimonas. Among the isolated Bacillus sp. strains, 19 showed larvicidal activity against A. aegypti. Two strains of Brevibacillus halotolerans also displayed larvicidal activity. For the first time, larvicidal activity against A. aegypti was identified for a strain of Brevibacillus halotolerans. Supernatant and pellet fractions of bacterial cultures were tested separately for larvicidal activities. Eight strains contained isolated fractions resulting in at least 50% mortality when tested at a concentration of 5 mg/mL. Further studies are needed to characterize the active larvicidal metabolites produced by these microorganisms and define their mechanisms of action.

    • It is now well-recognized that top-down cracking is a major form of distress in hot mix asphalt pavements A detailed analysis and evaluation of 22 field test sections throughout the state of Florida resulted in the development and verification of energy-based criteria for top-down cracking of hot mix asphalt. The work clearly indicated that there is no single mixture property or characteristic that can reliably predict top-down cracking performance of hot mix asphalt. A parameter termed the Energy Ratio, which was derived using the HMA Fracture Mechanics Model developed at the University of Florida, was determined to accurately distinguish between pavements that exhibited top-down cracking and those that did not, except for mixtures with excessively low or unusually high dissipated creep strain energy thresholds. The Energy Ratio, which is defined as the dissipated creep strain energy threshold of the mixture divided by the minimum dissipated creep strain energy required, is determined on the basis of tensile properties that can be obtained from a modulus, creep, and strength test performed with the Superpave IDT at a temperature of 10degreesC. The Energy Ratio accounts for the effects of pavement structural characteristics on top-down cracking performance. Therefore, it can be used to suitably integrate asphalt mixture properties in the pavement design process. In addition, a rational approach was developed to adjust the minimum Energy Ratio criterion for different traffic level pavements. In conclusion, two energy-based criteria were recommended to control top-down cracking of hot mix asphalt: 1) a minimum dissipated creep strain energy threshold; and 2) a minimum Energy Ratio for mixtures with a dissipated creep strain energy threshold greater than the minimum.

    • In this paper we analyze the equilibrium points of a collaborative transportation task, composed of two unmanned aerial vehicles and a payload - in this case, a bar. Moreover, centralized and decentralized linear model predictive controllers are designed, where the nonlinear dynamics are linearized around the equilibrium points previously analyzed. A comparison between the centralized and decentralized formulations is provided, based on experimental results for both setups, and considering the time to solution and performance of each controller. Our findings provide new operational equilibrium points that can be paired with predictive model-based controllers for efficient operation.

    • Background Rhabdoid brain tumours, also called atypical teratoid rhabdoid tumours, are lethal childhood cancers with characteristic genetic alterations of SMARCB1/hSNF5. Lack of biological understanding of the substantial clinical heterogeneity of these tumours restricts therapeutic advances. We integrated genomic and clinicopathological analyses of a cohort of patients with atypical teratoid rhabdoid tumours to find out the molecular basis for clinical heterogeneity in these tumours. Methods We obtained 259 rhabdoid tumours from 37 international institutions and assessed transcriptional profiles in 43 primary tumours and copy number profiles in 38 primary tumours to discover molecular subgroups of atypical teratoid rhabdoid tumours. We used gene and pathway enrichment analyses to discover group-specific molecular markers and did immunohistochemical analyses on 125 primary tumours to evaluate clinicopathological significance of molecular subgroup and ASCL1-NOTCH signalling. Findings Transcriptional analyses identified two atypical teratoid rhabdoid tumour subgroups with differential enrichment of genetic pathways, and distinct clinicopathological and survival features. Expression of ASCL1, a regulator of NOTCH signalling, correlated with supratentorial location (p=0.004) and superior 5-year overall survival (35%, 95% CI 13-57, and 20%, 6-34, for ASCL1-positive and ASCL1-negative tumours, respectively; p=0.033) in 70 patients who received multimodal treatment. ASCL1 expression also correlated with superior 5-year overall survival (34%, 7-61, and 9%, 0-21, for ASCL1-positive and ASCL1-negative tumours, respectively; p=0.001) in 39 patients who received only chemotherapy without radiation. Cox hazard ratios for overall survival in patients with differential ASCL1 enrichment treated with chemotherapy with or without radiation were 2.02 (95% CI 1.04-3.85; p=0.038) and 3.98 (1.71-9.26; p=0.001). Integrated analyses of molecular subgroupings with clinical prognostic factors showed three distinct clinical risk groups of tumours with different therapeutic outcomes. Interpretation An integration of clinical risk factors and tumour molecular groups can be used to identify patients who are likely to have improved long-term radiation-free survival and might help therapeutic stratification of patients with atypical teratoid rhabdoid tumours.
