  • Outcomes after perioperative SARS-CoV-2 infection in patients with proximal femoral fractures: an international cohort study

    • Studies have demonstrated high rates of mortality in people with proximal femoral fracture and SARS-CoV-2, but there is limited published data on the factors that influence mortality for clinicians to make informed treatment decisions. This study aims to report the 30-day mortality associated with perioperative infection of patients undergoing surgery for proximal femoral fractures and to examine the factors that influence mortality in a multivariate analysis.SettingProspective, international, multicentre, observational cohort study.ParticipantsPatients undergoing any operation for a proximal femoral fracture from 1 February to 30 April 2020 and with perioperative SARS-CoV-2 infection (either 7 days prior or 30-day postoperative).Primary outcome30-day mortality. Multivariate modelling was performed to identify factors associated with 30-day mortality.ResultsThis study reports included 1063 patients from 174 hospitals in 19 countries. Overall 30-day mortality was 29.4% (313/1063). In an adjusted model, 30-day mortality was associated with male gender (OR 2.29, 95% CI 1.68 to 3.13, p<0.001), age >80 years (OR 1.60, 95% CI 1.1 to 2.31, p=0.013), preoperative diagnosis of dementia (OR 1.57, 95% CI 1.15 to 2.16, p=0.005), kidney disease (OR 1.73, 95% CI 1.18 to 2.55, p=0.005) and congestive heart failure (OR 1.62, 95% CI 1.06 to 2.48, p=0.025). Mortality at 30 days was lower in patients with a preoperative diagnosis of SARS-CoV-2 (OR 0.6, 95% CI 0.6 (0.42 to 0.85), p=0.004). There was no difference in mortality in patients with an increase to delay in surgery (p=0.220) or type of anaesthetic given (p=0.787).ConclusionsPatients undergoing surgery for a proximal femoral fracture with a perioperative infection of SARS-CoV-2 have a high rate of mortality. This study would support the need for providing these patients with individualised medical and anaesthetic care, including medical optimisation before theatre. Careful preoperative counselling is needed for those with a proximal femoral fracture and SARS-CoV-2, especially those in the highest risk groups.Trial registration numberNCT04323644

  • The young HD 73583 (TOI-560) planetary system: two 10-M-circle plus mini-Neptunes transiting a 500-Myr-old, bright, and active K dwarf

    • We present the discovery and characterization of two transiting planets observed by TESS in the light curves of the young and bright (V = 9.67) star HD73583 (TOI-560). We perform an intensive spectroscopic and photometric space- and ground-based follow-up in order to confirm and characterize the system. We found that HD73583 is a young (similar to 500 Myr) active star with a rotational period of 12.08 +/- 0.11 d, and a mass and radius of 0.73 +/- 0.02 M-circle dot and 0.65 +/- 0.02 R-circle dot, respectively. HD 73583 b (P-b = 6.3980420(-0.0000062)(+0.0000067 )d) has a mass and radius of 10.2(-3.1)(+3.4) M-circle plus and 2.79 +/- 0.10 R-circle plus, respectively, which gives a density of 2.58(-0.81)(+0.95) g cm(-3). HD 73583 c (P-c = 18.87974(-0.00074)(+0.00086) d) has a mass and radius of 9.7(-1.7)(+1.8) M-circle plus and 2.39(-0.09)(+0.10) R-circle plus, respectively, which translates to a density of 3.88(-0.80)(+0.91) g cm(-3). Both planets are consistent with worlds made of a solid core surrounded by a volatile envelope. Because of their youth and host star brightness, they both are excellent candidates to perform transmission spectroscopy studies. We expect ongoing atmospheric mass-loss for both planets caused by stellar irradiation. We estimate that the detection of evaporating signatures on H and He would be challenging, but doable with present and future instruments.

  • TOI-1468: A system of two transiting planets, a super-Earth and a mini-Neptune, on opposite sides of the radius valley

    • We report the discovery and characterization of two small transiting planets orbiting the bright M3.0V star TOI-1468 (LSPM J0106+1913), whose transit signals were detected in the photometric time series in three sectors of the TESS mission. We confirm the planetary nature of both of them using precise radial velocity measurements from the CARMENES and MAROON-X spectrographs, and supplement them with ground-based transit photometry. A joint analysis of all these data reveals that the shorter-period planet, TOI-1468 b (P-b = 1.88 d), has a planetary mass of M-b = 3.21 +/- 0.24M(circle plus) and a radius of R-b = 1.280(-0.039)(+0.038) R-circle plus, resulting in a density of rho(b) = 8.39(-0.92)(+1.05) g cm(-3), which is consistent with a mostly rocky composition. For the outer planet, TOI-1468 c (P-c = 15.53 d), we derive a mass of M-c = 6.64(-0.68)(+0.67) M-circle plus,aradius of R-c = 2.06 +/- 0.04 R-circle plus, and a bulk density of rho(c) = 2.00(-0.19)(+0.21) g cm(-3), which corresponds to a rocky core composition with a H/He gas envelope. These planets are located on opposite sides of the radius valley, making our system an interesting discovery as there are only a handful of other systems with the same properties. This discovery can further help determine a more precise location of the radius valley for small planets around M dwarfs and, therefore, shed more light on planet formation and evolution scenarios.

    • In this paper, we report the discovery of TOI-220b, a new sub-Neptune detected by the Transiting Exoplanet Survey Satellite (TESS) and confirmed by radial velocity follow-up observations with the HARPS spectrograph. Based on the combined analysis of TESS transit photometry and high precision radial velocity measurements, we estimate a planetary mass of 13.8 +/- 1.0M(circle plus) and radius of 3.03 +/- 0.15R(circle plus), implying a bulk density of 2.73 +/- 0.47. TOI-220b orbits a relative bright (V=10.4) and old (10.1 +/- 1.4Gyr) K dwarf star with a period of similar to 10.69d. Thus, TOI-220b is a new warm sub-Neptune with very precise mass and radius determinations. A Bayesian analysis of the TOI-220b internal structure indicates that due to the strong irradiation it receives, the low density of this planet could be explained with a steam atmosphere in radiative-convective equilibrium and a supercritical water layer on top of a differentiated interior made of a silicate mantle and a small iron core.

  • TOI-1130: A photodynamical analysis of a hot Jupiter in resonance with an inner low-mass planet

    • The TOI-1130 is a known planetary system around a K-dwarf consisting of a gas giant planet, TOI-1130 c on an 8.4-day orbit that is accompanied by an inner Neptune-sized planet, TOI-1130 b, with an orbital period of 4.1 days. We collected precise radial velocity (RV) measurements of TOI-1130 with the HARPS and PFS spectrographs as part of our ongoing RV follow-up program. We performed a photodynamical modeling of the HARPS and PFS RVs, along with transit photometry from the Transiting Exoplanet Survey Satellite (TESS) and the TESS Follow-up Observing Program (TFOP). We determined the planet masses and radii of TOI-1130 b and TOI-1130 c to be Mb = 19.28 ± 0.97M⊕ and Rb = 3.56 ± 0.13 R⊕, and Mc = 325.59 ± 5.59M⊕ and Rc = 13.32−1.41+1.55 R⊕, respectively. We have spectroscopically confirmed the existence of TOI-1130 b, which had previously only been validated. We find that the two planets have orbits with small eccentricities in a 2:1 resonant configuration. This is the first known system with a hot Jupiter and an inner lower mass planet locked in a mean-motion resonance. TOI-1130 belongs to the small, yet growing population of hot Jupiters with an inner low-mass planet that poses a challenge to the pathway scenario for hot Jupiter formation. We also detected a linear RV trend that is possibly due to the presence of an outer massive companion.

  • A review of farm-scale nutrient budgets for organic farms as a tool for management of soil fertility

    • On organic farms, where the importation of materials to build/maintain soil fertility is restricted, it is important that a balance between inputs and outputs of nutrients is achieved to ensure both short-term productivity and long-term sustainability. This paper considers different approaches to nutrient budgeting on organic farms and evaluates the sources of bias in the measurements and/or estimates of the nutrient inputs and outputs. The paper collates 88 nutrient budgets compiled at the farm scale in nine temperate countries. All the nitrogen (N) budgets showed an N surplus (average 83.2 kg N ha-1 yr-1). The efficiency of N use, defined as outputs/inputs, was highest (0.9) and lowest (0.2) in arable and beef systems respectively. The phosphorus (P) and potassium (K) budgets showed both surpluses and deficits (average 3.6 kg P ha-1 yr-1, 14.2 kg K ha-1 yr-1) with horticultural systems showing large surpluses resulting from purchased manure. The estimation of N fixation and quantities of nutrients in purchased manures may introduce significant errors in nutrient budgets. Overall, the data illustrate the diversity of management systems in place on organic farms, and suggest that used together with soil analysis, nutrient budgets are a useful tool for improving the long-term sustainability of organic systems.

    • We report on 8.4-GHz very-long-baseline interferometry (VLBI) observations of SN 2001gd in the spiral galaxy NGC 5033 made on 2002 June 26 (2002.48) and 2003 April 8 (2003.27). We used the interferometric visibility data to estimate angular diameter sizes for the supernova by model fitting. Our data nominally suggest a relatively strong deceleration for the expansion of SN 2001gd, but we cannot dismiss the possibility of a free supernova expansion. From our VLBI observations on 2003 April 8, we inferred a minimum total energy in relativistic particles and magnetic fields in the supernova shell of Emin= (0.3-14) × 1047 erg, and a corresponding equipartition average magnetic field of Bmin= 50-350 mG. We also present multiwavelength Very Large Array (VLA) measurements of SN 2001gd made at our second VLBI epoch at frequencies of 1.4, 4.9, 8.4, 15.0, 22.5 and 43.3 GHz. The VLA data are well fitted by an optically thin, synchrotron spectrum (α=-1.0 +/- 0.1; Sν~να), partially absorbed by thermal plasma. We obtain a supernova flux density of 1.02 +/- 0.05 mJy at the observing frequency of 8.4 GHz for the second epoch, which results in an isotropic radio luminosity of (6.0 +/- 0.3) × 1036 erg s-1 between 1.4 and 43.3 GHz, at an adopted distance of 13.1 Mpc. Finally, we report on an XMM-Newton X-ray detection of SN 2001gd on 2002 December 18. The supernova X-ray spectrum is consistent with optically thin emission from a soft component (associated with emission from the reverse shock) at a temperature of around 1 keV. The observed flux corresponds to an isotropic X-ray luminosity of LX= (1.4 +/- 0.4) × 1039 erg s-1 in the 0.3-5 keV band. We suggest that both radio and X-ray observations of SN 2001gd indicate that a circumstellar interaction similar to that displayed by SN 1993J in M 81 is taking place.

    • Artificial RNA reagents such as small interfering RNAs (siRNAs) and aptamers often must be chemically modified for optimal effectiveness in environments that include ribonucleases. Mycoplasmas are common bacterial contaminants of mammalian cell cultures that are known to produce ribonucleases. Here we describe the rapid degradation of nuclease-stabilized RNA oligonucleotides in a human embryonic kidney 293 (HEK) cell culture contaminated with Mycoplasma fermentans, a common species of mycoplasma. RNA with 2′-fluoro-or 2′-O-methyl-modified pyrimidines was readily degraded in conditioned media from this culture, but was stable in conditioned media from uncontaminated HEK cells. RNA completely modified with 2′-O-methyls was not degraded in the mycoplasma-contaminated media. RNA zymogram analysis of conditioned culture media and material centrifuged from the media revealed several distinct protein bands (ranging from 30 to 68kDa) capable of degrading RNA with 2′-fluoro-or 2′-O-methyl-modified pyrimidines. Finally, the mycoplasma-associated nuclease was detected in material centrifuged from the contaminated culture supernatants in as little as 15 minutes with an RNA oligo-containing 2′-O-methyl-modified pyrimidines and labeled with a 5′-fluorescein amidite (FAM) and 3′-quencher. These results suggest that mycoplasma contamination may be a critical confounding variable for cell culture experiments involving RNA-based reagents, with particular relevance for applications involving naked RNA (e.g., aptamer-siRNA chimeras). © 2012 Mary Ann Liebert, Inc.

  • TOI-199 b : A Well-characterized 100 day Transiting Warm Giant Planet with TTVs Seen from Antarctica

    • We present the spectroscopic confirmation and precise mass measurement of the warm giant planet TOI-199 b. This planet was first identified in TESS photometry and confirmed using ground-based photometry from ASTEP in Antarctica including a full 6.5 hr long transit, PEST, Hazelwood, and LCO; space photometry from NEOSSat; and radial velocities (RVs) from FEROS, HARPS, CORALIE, and CHIRON. Orbiting a late G-type star, TOI-199 b has a 104.854 − 0.002 + 0.001 day period, a mass of 0.17 ± 0.02 M J, and a radius of 0.810 ± 0.005 R J. It is the first warm exo-Saturn with a precisely determined mass and radius. The TESS and ASTEP transits show strong transit timing variations (TTVs), pointing to the existence of a second planet in the system. The joint analysis of the RVs and TTVs provides a unique solution for the nontransiting companion TOI-199 c, which has a period of 273.69 − 0.22 + 0.26 days and an estimated mass of 0.28 − 0.01 + 0.02 M J . This period places it within the conservative habitable zone.

    • Many cell surface signaling receptors, such as the neurotrophin receptor, TrkB, have emerged as potential therapeutic targets for diverse diseases. Reduced activation of TrkB in particular is thought to contribute to neurodegenerative diseases. Unfortunately, development of therapeutic reagents that selectively activate particular cell surface receptors such as TrkB has proven challenging. Like many cell surface signaling receptors, TrkB is internalized upon activation; in this proof-of-concept study, we exploited this fact to isolate a pool of nuclease-stabilized RNA aptamers enriched for TrkB agonists. One of the selected aptamers, C4-3, was characterized with recombinant protein-binding assays, cell-based signaling and functional assays, and, in vivo in a seizure model in mice. C4-3 binds the extracellular domain of TrkB with high affinity (KD ∼2 nM) and exhibits potent TrkB partial agonistic activity and neuroprotective effects in cultured cortical neurons. In mice, C4-3 activates TrkB upon infusion into the hippocampus; systemic administration of C4-3 potentiates kainic acid-induced seizure development. We conclude that C4-3 is a potentially useful therapeutic agent for neurodegenerative diseases in which reduced TrkB activation has been implicated. We anticipate that the cell-based aptamer selection approach used here will be broadly applicable to the identification of aptamer-based agonists for a variety of cell-surface signaling receptors.     • Background: The evidence base to assess the efficacy and effectiveness of alcohol brief interventions (ABI) is weakened by variation in the outcomes measured and by inconsistent reporting. The 'Outcome Reporting in Brief Intervention Trials: Alcohol' (ORBITAL) project aims to develop a core outcome set (COS) and reporting guidance for its use in future trials of ABI in a range of settings.Methods/design: An international Special Interest Group was convened through INEBRIA (International Network on Brief Interventions for Alcohol and Other Drugs) to inform the development of a COS for trials of ABI. ORBITAL will incorporate a systematic review to map outcomes used in efficacy and effectiveness trials of ABI and their measurement properties, using the COnsensus-based Standards for the selection of health Measurement INstruments (COSMIN) criteria. This will support a multi-round Delphi study to prioritise outcomes. Delphi panellists will be drawn from a range of settings and stakeholder groups, and the Delphi study will also be used to determine if a single COS is relevant for all settings. A consensus meeting with key stakeholder representation will determine the final COS and associated guidance for its use in trials of ABI.Discussion: ORBITAL will develop a COS for alcohol screening and brief intervention trials, with outcomes stratified into domains and guidance on outcome measurement instruments. The standardisation of ABI outcomes and their measurement will support the ongoing development of ABI studies and a systematic synthesis of emerging research findings. We will track the extent to which the COS delivers on this promise through an exploration of the use of the guidance in the decade following COS publication.
