    • BACKGROUND: Recurrent stroke is a frequent, disabling event after ischemic stroke. This study compared the efficacy and safety of two antiplatelet regimens--aspirin plus extended-release dipyridamole (ASA-ERDP) versus clopidogrel. METHODS: In this double-blind, 2-by-2 factorial trial, we randomly assigned patients to receive 25 mg of aspirin plus 200 mg of extended-release dipyridamole twice daily or to receive 75 mg of clopidogrel daily. The primary outcome was first recurrence of stroke. The secondary outcome was a composite of stroke, myocardial infarction, or death from vascular causes. Sequential statistical testing of noninferiority (margin of 1.075), followed by superiority testing, was planned. RESULTS: A total of 20,332 patients were followed for a mean of 2.5 years. Recurrent stroke occurred in 916 patients (9.0%) receiving ASA-ERDP and in 898 patients (8.8%) receiving clopidogrel (hazard ratio, 1.01; 95% confidence interval [CI], 0.92 to 1.11). The secondary outcome occurred in 1333 patients (13.1%) in each group (hazard ratio for ASA-ERDP, 0.99; 95% CI, 0.92 to 1.07). There were more major hemorrhagic events among ASA-ERDP recipients (419 [4.1%]) than among clopidogrel recipients (365 [3.6%]) (hazard ratio, 1.15; 95% CI, 1.00 to 1.32), including intracranial hemorrhage (hazard ratio, 1.42; 95% CI, 1.11 to 1.83). The net risk of recurrent stroke or major hemorrhagic event was similar in the two groups (1194 ASA-ERDP recipients [11.7%], vs. 1156 clopidogrel recipients [11.4%]; hazard ratio, 1.03; 95% CI, 0.95 to 1.11). CONCLUSIONS: The trial did not meet the predefined criteria for noninferiority but showed similar rates of recurrent stroke with ASA-ERDP and with clopidogrel. There is no evidence that either of the two treatments was superior to the other in the prevention of recurrent stroke. (ClinicalTrials.gov number, NCT00153062.)

  • Telmisartan to prevent recurrent stroke and cardiovascular events

    • BACKGROUND: Prolonged lowering of blood pressure after a stroke reduces the risk of recurrent stroke. In addition, inhibition of the renin-angiotensin system in high-risk patients reduces the rate of subsequent cardiovascular events, including stroke. However, the effect of lowering of blood pressure with a renin-angiotensin system inhibitor soon after a stroke has not been clearly established. We evaluated the effects of therapy with an angiotensin-receptor blocker, telmisartan, initiated early after a stroke. METHODS: In a multicenter trial involving 20,332 patients who recently had an ischemic stroke, we randomly assigned 10,146 to receive telmisartan (80 mg daily) and 10,186 to receive placebo. The primary outcome was recurrent stroke. Secondary outcomes were major cardiovascular events (death from cardiovascular causes, recurrent stroke, myocardial infarction, or new or worsening heart failure) and new-onset diabetes. RESULTS: The median interval from stroke to randomization was 15 days. During a mean follow-up of 2.5 years, the mean blood pressure was 3.8/2.0 mm Hg lower in the telmisartan group than in the placebo group. A total of 880 patients (8.7%) in the telmisartan group and 934 patients (9.2%) in the placebo group had a subsequent stroke (hazard ratio in the telmisartan group, 0.95; 95% confidence interval [CI], 0.86 to 1.04; P=0.23). Major cardiovascular events occurred in 1367 patients (13.5%) in the telmisartan group and 1463 patients (14.4%) in the placebo group (hazard ratio, 0.94; 95% CI, 0.87 to 1.01; P=0.11). New-onset diabetes occurred in 1.7% of the telmisartan group and 2.1% of the placebo group (hazard ratio, 0.82; 95% CI, 0.65 to 1.04; P=0.10). CONCLUSIONS: Therapy with telmisartan initiated soon after an ischemic stroke and continued for 2.5 years did not significantly lower the rate of recurrent stroke, major cardiovascular events, or diabetes.   • Strongly Deformed Nuclear Shapes at Ultra-High Spin and Shape Coexistence in N\sim 90 Nuclei

    • The N similar to 90 region of the nuclear chart has featured prominently as the spectroscopy of nuclei at extreme spin has progressed. This talk will present recent discoveries from investigations of high spin behavior in the N similar to 90 Er, Tm and Yb nuclei utilizing the Gammasphere gamma-ray spectrometer. In particular it will include discussion of the beautiful shape evolution and coexistence observed in these nuclei along with the identification of a remarkable new family of band structures. The latter are very weakly populated rotational sequences with high moment of inertia that bypass the classic terminating configurations near spin 40-50 (h) over bar, marking a return to collectivity that extends discrete gamma-ray spectroscopy to well over 60 (h) over bar. Establishing the nature of the yrast states in these nuclei beyond the oblate band-termination states has been a major goal for the past two decades. Cranking calculations suggest that these new structures most likely represent stable triaxial strongly deformed bands that lie in a valley of favored shell energy in deformation and particle-number space.

  • Evolution of structure and shapes in Er 158 to ultrahigh spin

    • The level structure of Er158 has been studied using the Gammasphere spectrometer via the Cd114(Ca48,4n) reaction at 215 MeV with both thin (self-supporting) and thick (backed) targets. The level scheme has been considerably extended with more than 200 new transitions and six new rotational structures, including two strongly coupled high-K bands. Configuration assignments for the new structures are based on their observed alignments, B(M1)/B(E2) ratios of reduced transition probabilities, excitation energies, and comparisons with neighboring nuclei and theoretical calculations. With increasing angular momentum, this nucleus exhibits Coriolis-induced alignments of both neutrons and protons before it then undergoes a rotation-induced transition from near-prolate collective rotation to a noncollective oblate configuration. This transition occurs via the mechanism of band termination around spin 45ħ in three rotational structures. Two distinct lifetime branches, consistent with the crossing of a collective "fast"rotational structure by an energetically favored "slow"terminating sequence, are confirmed for the positive-parity states, and similar behavior is established in the negative-parity states. Weak-intensity, high-energy transitions are observed to feed into the terminating states. At the highest spins, three collective bands with high dynamic moments of inertia and large quadrupole moments were identified. These bands are interpreted as triaxial strongly deformed structures and mark a return to collectivity at ultrahigh spin.

    • A high-spin rotational band was observed in the N=90 nucleus Yb-160 with moment of inertia and decay characteristics very similar to recently discovered sequences in Er-157,Er-158. These latter structures were discussed in terms of strongly deformed triaxial bands. Detailed cranked Nilsson-Strutinsky calculations were performed that predict that well-deformed triaxial structures are also expected at high spin in Yb-160. Within this interpretation the observed discontinuity in the dynamic moment of inertia around h omega=0.40-0.45 MeV can be explained as a crossing between i(13/2) neutron levels.

  • The Importance of Eurekan Mountains on Cenozoic Sediment Routing on the Western Barents Shelf

    • The importance of topography generated by Eocene Eurekan deformation as a sediment source for sandstones deposited on the western Barents Shelf margin is evaluated through a sediment provenance study conducted on wellbore materials retrieved from Spitsbergen and from the Vestbakken Volcanic Province and the Sørvestsnaget Basin in the southwest Barents Sea. A variety of complementary techniques record a provenance change across the Paleocene-Eocene boundary in wellbore BH 10-2008, which samples Paleogene strata of the Central Tertiary Basin in Spitsbergen. Sandstones containing K-feldspar with radiogenic Pb isotopic compositions, chrome spinel in the heavy mineral assemblage, and detrital zircons and rutiles with prominent Palaeoproterozoic and Late Palaeozoic—Early Mesozoic U-Pb age populations are up-section replaced by sandstone containing albitic plagioclase feldspar, metasedimentary schist rock fragments, a heavy mineral assemblage with abundant chloritoid, metamorphic apatite with low REE contents, metapelitic rutile with Silurian U-Pb ages and zircons with predominantly Archaean and Palaeoproterozoic U-Pb age populations. Our results clearly demonstrate the well-known regional change in source area from an exposed Barents Shelf terrain east of the Central Tertiary Basin during the Paleocene to the emerging Eurekan mountains west and north of the Central Tertiary Basin during the Eocene. Eocene sandstones deposited in the marginal basins of the southwestern Barents Shelf, which were sampled in wellbores 7316/5-1 and 7216/11-1S, contain elements of both the Eurekan and the eastern Barents Shelf provenance signatures. The mixing of the two sand types and delivery to the southwest margin of the Barents Shelf is consistent with a fill and spill model for the Central Teritary Basin, with transport of Eurekan-derived sediment east then south hundreds of kilometres across the Shelf.

    • Background: The fractional exhaled nitric oxide (FENO) test is a point-of-care test that is used in the assessment of asthma. Objective: To provide evidence-based clinical guidance on whether FENO testing is indicated to optimize asthma treatment in patients with asthma in whom treatment is being considered. Methods: An international, multidisciplinary panel of experts was convened to form a consensus document regarding a single question relevant to the use of FENO. The question was selected from three potential questions based on the greatest perceived impact on clinical practice and the unmet need for evidencebased answers related to this question. The panel performed systematic reviews of published randomized controlled trials between 2004 and 2019 and followed the Grading of Recommendations, Assessment, Development, and Evaluation (GRADE) evidence-to-decision framework to develop recommendations. All panel members evaluated and approved the recommendations. Main Results: After considering the overall low quality of the evidence, the panel made a conditional recommendation for FENO-based care. In patients with asthma in whom treatment is being considered, we suggest that FENO is beneficial and should be used in addition to usual care. This judgment is based on a balance of effects that probably favors the intervention; the moderate costs and availability of resources, which probably favors the intervention; and the perceived acceptability and feasibility of the intervention in daily practice. Conclusions: Clinicians should consider this recommendation to measure FENO in patients with asthma in whom treatment is being considered based on current best available evidence.

  • Jurassic high heat production granites associated with the Weddell Sea rift system, Antarctica

    • The distribution of heat flow in Antarctic continental crust is critical to understanding continental tectonics, ice sheet growth and subglacial hydrology. We identify a group of High Heat Production granites, intruded into upper crustal Palaeozoic metasedimentary sequences, which may contribute to locally high heat flow beneath the West Antarctic Ice Sheet. Four of the granite plutons are exposed above ice sheet level at Pagano Nunatak, Pirrit Hills, Nash Hills and Whitmore Mountains. A new UPb zircon age from Pirrit Hills of 178.0±3.5Ma confirms earlier RbSr and UPb dating and that the granites were emplaced approximately coincident with the first stage of Gondwana break-up and the developing Weddell rift, and ~5m.y. after eruption of the Karoo-Ferrar large igneous province. Aerogeophysical data indicate that the plutons are distributed unevenly over 40,000km2 with one intruded into the transtensional Pagano Shear Zone, while the others were emplaced within the more stable Ellsworth-Whitmore mountains continental block. The granites are weakly peraluminous A-types and have Th and U abundances up to 60.7 and 28.6ppm respectively. Measured heat production of the granite samples is 2.96–9.06μW/m3 (mean 5.35W/m3), significantly higher than average upper continental crust and contemporaneous silicic rocks in the Antarctic Peninsula. Heat flow associated with the granite intrusions is predicted to be in the range 70–95mW/m2 depending on the thickness of the high heat production granite layer and the regional heat flow value. Analysis of detrital zircon compositions and ages indicates that the high Th and U abundances are related to enrichment of the lower-mid crust that dates back to 200–299Ma at the time of the formation of the Gondwanide fold belt and its post-orogenic collapse and extension.

The nucleus of Er-159 was populated by the reaction Cd-116(Ca-48, 5n gamma) at a beam energy of 215 MeV, and the resulting gamma decays were studied using the Gammasphere spectrometer. New rotational bands and extensions to existing sequences were observed, which are discussed in terms of the cranked shell model, revealing a diverse range of quasiparticle configurations. At spins around 50 (h) over bar, there is evidence for a change from dominant prolate collective motion at the yrast line to oblate non-collective structures via the mechanism of band termination. A possible strongly deformed triaxial band occurs at these high spins, which indicates collectivity beyond 50 (h) over bar. The high-spin data are interpreted within the framework of cranked Nilsson-Strutinsky calculations.

The gamma decays were detected using the highly efficient Gammasphere spectrometer. The discovery of these bands, which extend discrete-line spectroscopy in these nuclei to ultrahigh spin of similar to 60h, is consistent with recent observations of high-spin collective structures in isotopes of Er, Yb, and Tm around N=90. Cranked Nilsson-Strutinsky calculations suggest that these bands may arise from well-deformed triaxial configurations with either positive or negative gamma deformation.

    • The Cretaceous marks an episode of enhanced magmatism, sedimentation and tectonic processes along the entire proto-Pacific convergent margin of West Gondwana. Cretaceous magmatism across the Antarctic Peninsula is dominated by the intrusive Lassiter Coast intrusive suite which developed as episodic ‘flare-up’ events during the mid-Cretaceous, at a time of increased convergence. Volcanic rocks of this age are poorly defined, as a consequence of limited field observations and an absence of accurate geochronology. Recent field mapping, combined with unpublished field observations has identified a region >10,000 km2 of dominantly subaerial rhyolitic pyroclastic and epiclastic successions from northern Palmer Land of the Antarctic Peninsula. Volcanic successions up to 1500 m in thickness consist of dominantly silicic ignimbrites, lavas, heterolithic breccias and lahar deposits, fed by caldera-forming eruptions. The volcanic rocks of this region were widely considered to be Early Cretaceous in age based on field relationships and early geochronology. New U-Pb zircon ages identify three distinct volcanic episodes during the Late Cretaceous/Early Cenozoic at ~108 Ma, ~93 Ma and ~64 Ma. Lu-Hf data indicate significant proportions of ancient crust in the petrogenesis of the volcanic rocks and the slightly negative εHf values are consistent with a mid-position (~120 km) within the magmatic arc, relative to more juvenile compositions close to the trench.

    • Long-lived continental margin arcs are characterized by episodes of large-volume magmatism (or flare-ups) that can persist for ∼30 m.y. before steady-state arc conditions resume. Flare-up events are characterized by the emplacement of large-volume granodiorite-tonalite batholiths and sometimes associated rhyodacitic ignimbrites. One of the major flare-up events of the West Gondwana margin occurred during the mid-Cretaceous and was temporally and spatially associated with widespread deformation and Pacific plate reorganization. New U-Pb geochronology from the Lassiter Coast intrusive suite in the southern Antarctic Peninsula identifies a major magmatic event in the interval 130–102 Ma that was characterized by three distinct peaks in granitoid emplacement at 130–126 Ma, 118–113 Ma, and 108–102 Ma, with clear lulls in between. Mid-Cretaceous magmatism from elsewhere in West Antarctica, Patagonia, and New Zealand also featured marked episodicity during the mid-Cretaceous and recorded remarkable continuity along the West Gondwana margin. The three distinct magmatic events represent second-order episodicity relative to the primary episodicity that occurred on a cordillera scale and is a feature of the North and South American Pacific margin. Flare-up events require the development of a highly fusible, lower-crustal layer resulting from the continued underplating of hydrous mineralogies in the melt-fertile lower crust as a result of long-lived subduction. However, the actual trigger for melting is likely to result from external, potentially tectonic factors, e.g., rifting, plate reorganization, continental breakup, or mantle plumes.

  • Triassic magmatism and metamorphism in the Antarctic Peninsula : Identifying the extent and timing of the Peninsula Orogeny

    • The mid to Late Triassic marks an episode of magmatism, deformation and metamorphism along the proto-Pacific margin of the Antarctic Peninsula and Patagonia. Calc-alkaline magmatism at ~227 Ma developed in a convergent margin setting across the central Antarctic Peninsula and North Patagonian Massif. Two distinct deformation and metamorphic events have been identified at ~221 Ma and ~207 Ma based on new U–Pb zircon ages from a metamorphic orthogneiss complex, investigated for the first time, from the elevated plateau region of central Palmer Land. These two events are interpreted to date the timing of the multi-phase Peninsula Orogeny, which correlates with the Chonide Event of central Patagonia. The onset of deformation in the Antarctic Peninsula is linked to tectonic events in central Patagonia and is associated with non-collisional slab dynamic processes during flat slab subduction in the Late Triassic. No consistent tectonic regime is evident during the mid-to Late Triassic episodes of deformation and metamorphism, but an initial period of extension/transtension (mid-Triassic) and a compressional regime in the Late Triassic is favoured in the Antarctic Peninsula and Patagonia. The newly acquired metamorphic ages provide no clear evidence for a segmented terrane model for the Antarctic Peninsula, as Late Triassic metamorphic ages are identified from across the central and eastern domains. New U–Pb zircon ages are also presented from a suite of strongly foliated granitoids that cross-cut the Triassic orthogneisses. They are dated in the interval 140–136 Ma and represent an episode of Early Cretaceous magmatism, transtensional deformation and widespread metamorphism across northwest and central Palmer Land.

    • High-spin states in Tm-160,Tm-161 were populated using the Te-128(Cl-37, 5n and 4n) reactions at a beam energy of 170 MeV. Emitted gamma rays were detected in the Gammasphere spectrometer. Two rotational bands with high moments of inertia were discovered, one assigned to Tm-160, while the other tentatively assigned to Tm-161. These sequences display features similar to bands observed in neighboring Er, Tm, Yb, and Lu nuclei which have been discussed in terms of triaxial strongly deformed structures. Cranked Nilsson Strutinsky calculations have been performed that predict well-deformed triaxial shapes at high spin in Tm-160,Tm-161.
