  • The Miami International Evidence-based Guidelines on Minimally Invasive Pancreas Resection

    • Objective: The aim of this study was to develop and externally validate the first evidence-based guidelines on minimally invasive pancreas resection (MIPR) before and during the International Evidence-based Guidelines on Minimally Invasive Pancreas Resection (IG-MIPR) meeting in Miami (March 2019).Summary Background Data: MIPR has seen rapid development in the past decade. Promising outcomes have been reported by early adopters from high-volume centers. Subsequently, multicenter series as well as randomized controlled trials were reported; however, guidelines for clinical practice were lacking. Methods: The Scottisch Intercollegiate Guidelines Network (SIGN) methodology was used, incorporating these 4 items: systematic reviews using PubMed, Embase, and Cochrane databases to answer clinical questions, whenever possible in PICO style, the GRADE approach for assessment of the quality of evidence, the Delphi method for establishing consensus on the developed recommendations, and the AGREE-II instrument for the assessment of guideline quality and external validation. The current guidelines are cosponsored by the International Hepato-Pancreato-Biliary Association, the Americas Hepato-Pancreato-Biliary Association, the Asian-Pacific Hepato-Pancreato-Biliary Association, the European-African Hepato-Pancreato-Biliary Association, the European Association for Endoscopic Surgery, Pancreas Club, the Society of American Gastrointestinal and Endoscopic Surgery, the Society for Surgery of the Alimentary Tract, and the Society of Surgical Oncology. Results: After screening 16,069 titles, 694 studies were reviewed, and 291 were included. The final 28 recommendations covered 6 topics; laparoscopic and robotic distal pancreatectomy, central pancreatectomy, pancreatoduodenectomy, as well as patient selection, training, learning curve, and minimal annual center volume required to obtain optimal outcomes and patient safety.Conclusion: The IG-MIPR using SIGN methodology give guidance to surgeons, hospital administrators, patients, and medical societies on the use and outcome of MIPR as well as the approach to be taken regarding this challenging type of surgery. © 2019 Wolters Kluwer Health, Inc.   • Hybridization and speciation

    • Hybridization has many and varied impacts on the process of speciation. Hybridization may slow or reverse differentiation by allowing gene flow and recombination. It may accelerate speciation via adaptive introgression or cause near-instantaneous speciation by allopolyploidization. It may have multiple effects at different stages and in different spatial contexts within a single speciation event. We offer a perspective on the context and evolutionary significance of hybridization during speciation, highlighting issues of current interest and debate. In secondary contact zones, it is uncertain if barriers to gene flow will be strengthened or broken down due to recombination and gene flow. Theory and empirical evidence suggest the latter is more likely, except within and around strongly selected genomic regions. Hybridization may contribute to speciation through the formation of new hybrid taxa, whereas introgression of a few loci may promote adaptive divergence and so facilitate speciation. Gene regulatory networks, epigenetic effects and the evolution of selfish genetic material in the genome suggest that the Dobzhansky-Muller model of hybrid incompatibilities requires a broader interpretation. Finally, although the incidence of reinforcement remains uncertain, this and other interactions in areas of sympatry may have knock-on effects on speciation both within and outside regions of hybridization.

  • Identification of constrained sequence elements across 239 primate genomes

    • Noncoding DNA is central to our understanding of human gene regulation and complex diseases1,2, and measuring the evolutionary sequence constraint can establish the functional relevance of putative regulatory elements in the human genome3,4,5,6,7,8,9. Identifying the genomic elements that have become constrained specifically in primates has been hampered by the faster evolution of noncoding DNA compared to protein-coding DNA10, the relatively short timescales separating primate species11, and the previously limited availability of whole-genome sequences12. Here we construct a whole-genome alignment of 239 species, representing nearly half of all extant species in the primate order. Using this resource, we identified human regulatory elements that are under selective constraint across primates and other mammals at a 5% false discovery rate. We detected 111,318 DNase I hypersensitivity sites and 267,410 transcription factor binding sites that are constrained specifically in primates but not across other placental mammals and validate their cis-regulatory effects on gene expression. These regulatory elements are enriched for human genetic variants that affect gene expression and complex traits and diseases. Our results highlight the important role of recent evolution in regulatory sequence elements differentiating primates, including humans, from other placental mammals.

  • The landscape of tolerated genetic variation in humans and primates

    • Personalized genome sequencing has revealed millions of genetic differences between individuals, but our understanding of their clinical relevance remains largely incomplete. To systematically decipher the effects of human genetic variants, we obtained whole-genome sequencing data for 809 individuals from 233 primate species and identified 4.3 million common protein-altering variants with orthologs in humans. We show that these variants can be inferred to have nondeleterious effects in humans based on their presence at high allele frequencies in other primate populations. We use this resource to classify 6% of all possible human protein-altering variants as likely benign and impute the pathogenicity of the remaining 94% of variants with deep learning, achieving state-of-the-art accuracy for diagnosing pathogenic variants in patients with genetic diseases.

  • A global catalog of whole-genome diversity from 233 primate species

    • The rich diversity of morphology and behavior displayed across primate species provides an informative context in which to study the impact of genomic diversity on fundamental biological processes. Analysis of that diversity provides insight into long-standing questions in evolutionary and conservation biology and is urgent given severe threats these species are facing. Here, we present high-coverage wholegenome data from 233 primate species representing 86% of genera and all 16 families. This dataset was used, together with fossil calibration, to create a nuclear DNA phylogeny and to reassess evolutionary divergence times among primate clades. We found within-species genetic diversity across families and geographic regions to be associated with climate and sociality, but not with extinction risk. Furthermore, mutation rates differ across species, potentially influenced by effective population sizes. Lastly, we identified extensive recurrence of missense mutations previously thought to be human specific. This study will open a wide range of research avenues for future primate genomic research.

  • Ergogenic effect of hyperoxic recovery in elite swimmers performing high-intensity intervals

    • This investigation tested the hypothesis that breathing oxygen-enriched air (FiO2=1.00) during recovery enhances peak (Ppeak) and mean power (Pmean) output during repeated high-intensity exercise. Twelve elite male swimmers (21 ± 3 years, 192.1 ± 5.9cm, 79.1 ± 8.2kg) inhaled either hyperoxic (HOX) or normoxic (NOX) air during 6-min recovery periods between five repetitions of high-intensity bench swimming, each involving 40 maximal armstrokes. Oxygen partial pressure (pO2) and saturation (SO2), [H+], pH, base excess and blood lactate concentration were measured before and after all intervals. The production of the reactive oxygen species (ROS) hydrogen peroxide was measured before, directly after and 15min after the test. Ppeak and Pmean with HOX recovery were significantly higher than with NOX throughout the third, fourth and fifth intervals (P<0.001-0.04). With HOX, electromyography activity was lower during the third, fourth and fifth intervals than during the first (P=0.05-0.001), with no such changes in NOX (P=0.99). There were no differences in blood lactate, pH, [H+] or base excess and ROS production at any time point with either HOX or NOX recovery. These findings demonstrate that the Ppeak and Pmean of elite swimmers performing high-intensity intervals can be improved by exposure to oxygen-enriched air during recovery. © 2011 John Wiley & Sons A/S.

  • THE ROLE OF FISSION IN NEUTRON STAR MERGERS AND ITS IMPACT ON THE r-PROCESS PEAKS

    • Comparing observational abundance features with nucleosynthesis predictions of stellar evolution or explosion simulations, we can scrutinize two aspects: (a) the conditions in the astrophysical production site and (b) the quality of the nuclear physics input utilized. We test the abundance features of r-process nucleosynthesis calculations for the dynamical ejecta of neutron star merger simulations based on three different nuclear mass models: The Finite Range Droplet Model, the (quenched version of the) Extended Thomas Fermi Model with Strutinsky Integral, and the Hartree-Fock-Bogoliubov mass model. We make use of corresponding fission barrier heights and compare the impact of four different fission fragment distribution models on the final r-process abundance distribution. In particular, we explore the abundance distribution in the second r-process peak and the rare-earth sub-peak as a function of mass models and fission fragment distributions, as well as the origin of a shift in the third r-process peak position. The latter has been noticed in a number of merger nucleosynthesis predictions. We show that the shift occurs during the r-process freeze-out when neutron captures and beta-decays compete and an (n,gamma)-(gamma,n) equilibrium is no longer maintained. During this phase neutrons originate mainly from fission of material above A = 240. We also investigate the role of beta-decay half-lives from recent theoretical advances, which lead either to a smaller amount of fissioning nuclei during freeze-out or a faster (and thus earlier) release of fission neutrons, which can (partially) prevent this shift and has an impact on the second and rare-earth peak as well.

    • AIMS: The body responds to exercise training by profound adaptations throughout the cardiorespiratory and muscular systems, which may result in improvements in maximal oxygen consumption (VO2 peak) and mitochondrial capacity. By convenience, mitochondrial respiration is often measured at supra-physiological oxygen levels, an approach that ignores any potential regulatory role of mitochondrial affinity for oxygen (p50mito ) at physiological oxygen levels.METHODS: In this study, we examined the p50mito of mitochondria isolated from the Vastus lateralis and Triceps brachii in 12 healthy volunteers before and after a training intervention with 7 sessions of sprint interval training using both leg cycling and arm cranking. The changes in p50mito were compared to changes in whole-body VO2 peak.RESULTS: We here show that p50mito is similar in isolated mitochondria from the Vastus (40 ± 3.8 Pa) compared to Triceps (39 ± 3.3) but decreases (mitochondrial oxygen affinity increases) after 7 sessions of sprint interval training (to 26 ± 2.2 Pa in Vastus and 22 ± 2.7 Pa in Triceps, both p<0.01). The change in VO2 peak modeled from changes in p50mito was correlated to actual measured changes in VO2 peak (R2 =0.41, p=0.002).CONCLUSION: Together with mitochondrial respiratory capacity, p50mito is a critical factor when measuring mitochondrial function, it can decrease with sprint interval training and should be considered in the integrative analysis of the oxygen cascade from lung to mitochondria.

    • Quantum mechanics dictates the band structure of materials that is essential for functional electronic components. With increased miniaturization of devices, it becomes possible to exploit the full potential of quantum mechanics through the principles of superposition and entanglement. We propose a class of quantum rectifiers that can leverage entanglement to dramatically increase performance by coupling two small spin chains through an effective double-slit interface. Simulations show that rectification is enhanced by several orders of magnitude even in small systems, and that the effect survives in a noisy environment. Realizable using several of the quantum technology platforms currently available, our findings reveal the importance of quantum entanglement in seemingly contradictory applications such as heat and noise control.

  • Quantum Wheatstone Bridge

    • We propose a quantum Wheatstone bridge as a fully quantum analog to the classical version. The bridge is a few-body boundary-driven spin chain exploiting quantum effects to gain an enhanced sensitivity to an unknown coupling. The sensitivity is explained by a drop in population of an entangled Bell state due to destructive interference as the controllable coupling approaches the unknown coupling. A simple criterion for the destructive interference is found, and an approximate expression for the width of the drop is derived. The sensitivity to the unknown coupling is quantified using the quantum Fisher information, and we show that the state of the bridge can be measured indirectly through the spin current. Our results are robust toward calibration errors and generic in the sense that several of the current state-of-the-art quantum platforms could be used as a means of realization. The quantum Wheatstone bridge may thus find use in fields such as sensing and metrology using near-term quantum devices.

  • Does upper-body compression improve 3 × 3-min double-poling sprint performance?

    • Purpose: To evaluate whether upper-body compression affects power output and selected metabolic, cardiorespiratory, hemodynamic, and perceptual responses during three 3-min sessions of double-poling (DP) sprint. Method: Ten well-trained male athletes (25 ± 4 y, 180 ± 4 cm, 74.6 ± 3.2 kg) performed such sprints on a DP ski ergometer with and without a long-sleeved compression garment. Result: Mean power output was not affected by such compression (216 ± 25 W in both cases; P = 1.00, effect size [ES] = 0.00), although blood lactate concentration was lowered (P < .05, ES = 0.50-1.02). Blood gases (ES = 0.07-0.50), oxygen uptake (ES = 0.04-0.28), production of carbon dioxide (ES = 0.01-0.46), heart rate (ES = 0.00-0.21), stroke volume (ES = 0.33-0.81), and cardiac output (ES = 0.20-0.91) were also all unaffected by upper-body compression (best P = 1.00). This was also the case for changes in the tissue saturation index (ES = 0.45-1.17) and total blood content of hemoglobin (ES = 0.09-0.85), as well as ratings of perceived exertion (ES = 0.15-0.88; best P = .96). Conclusion: The authors conclude that the performance of well-trained athletes during 3 × 3-min DP sprints will not be enhanced by upper-body compression. © 2014 Human Kinetics, Inc.

    • Purpose: This study aimed to quantify the intensity profile of elite downhill mountain bike races during competitions. Methods: Seventeen maledownhill racers (22 ± 5 y; 185.1 ± 5.3 cm; 68.0 ± 3.9 kg; VO 2peak: 59.4 ± 4.1 mL min kg -1) participated in the International German DownhillChampionships in 2010. The racers' peak oxygen uptake and heart rate (HR) at 2 and 4 mmol L -1 blood lactate (HR2 and HR 4), were assessed during an incremental laboratory step test (100 W, increase 40 W every 5 min). During the races, the HR was recorded and pre- and postrace bloodlactate concentrations as well as salivary cortisol levels were obtained. Results: During the race, the absolute time spent in the "easy" intensity zone was 23.3 ± 6.8 s, 24.2 ± 12.8 s (HR 2-HR 4) in the "moderate" zone, and 151.6 ± 18.3 s (>HR 4) in the "hard" zone. Eighty percent of the entire race was accomplished at intensities >90% HRpeak. Blood lactate concentrations postrace were higher than those obtained after the qualification heat (8.0 ± 2.5 mmol L -1 vs 6.7 ± 1.8 mmol L -1, P < .01). Salivary levels of cortisol before the competition and the qualification heat were twice as high as at resting state (P < .01). Conclusions: This study shows that mountain bike downhill races are conducted at high heart rates and levels of blood lactate as well as increased concentration of salivary cortisol as marker for psycho-physiological stress. © 2011 Human Kinetics, Inc.

    • This study was designed to assess systemic cardio-respiratory, metabolic and perceived responses to incremental arm cycling with concurrent electrical myostimulation (EMS). Eleven participants (24 ± 3 yrs; 182 ± 10 cm; 86 ± 16.8 kg) performed two incremental tests involving arm cycling until volitional exhaustion was reached with and without EMS of upper-body muscles. The peak power output was 10.1% lower during arm cycling with (128 ± 30 W) than without EMS (141 ± 25 W, p = 0.01; d = 0.47). In addition, the heart rate (2-9%), oxygen uptake (7-15%), blood lactate concentration (8-46%) and ratings of perceived exertion (4-14%) while performing submaximal arm cycling with EMS were all higher with than without EMS (all p < 0.05). Upon exhaustion, the heart rate, oxygen uptake, lactate concentration, and ratings of perceived exertion did not differ between the two conditions (all p > 0.05). In conclusion, arm cycling with EMS induced more pronounced cardio-respiratory, metabolic and perceived responses, especially during submaximal arm cycling. This form of exercise with stimulation might be beneficial for a variety of athletes competing in sports involving considerable generation of work by the upper body (e.g., kayaking, cross-country skiing, swimming, rowing and various parasports).

    • In this study, we tested the hypothesis that breathing hyperoxic air (FinO2 = 0.40) while exercising in a hot environment exerts negative effects on the total tissue level of haemoglobin concentration (tHb); core (T-core) and skin (T-skin) temperatures; muscle activity; heart rate; blood concentration of lactate; pH; partial pressure of oxygen (PaO2) and carbon dioxide; arterial oxygen saturation (SaO2); and perceptual responses. Ten well-trained male athletes cycled at submaximal intensity at 21 degrees C or 33 degrees C in randomized order: first for 20 min while breathing normal air (FinO2 = 0.21) and then 10 min with FinO2 = 0.40 (HOX). At both temperatures, SaO2 and PaO2, but not tHb, were increased by HOX. Tskin and perception of exertion and thermal discomfort were higher at 33 degrees C than 21 degrees C (p < 0.01), but independent of FinO2. Tcore and muscle activity were the same under all conditions (p > 0.07). Blood lactate and heart rate were higher at 33 degrees C than 21 degrees C. In conclusion, during 30 min of submaximal cycling at 21 degrees C or 33 degrees C, T-core, T-skin and T-body, tHb, muscle activity and ratings of perceived exertion and thermal discomfort were the same under normoxic and hyperoxic conditions. Accordingly, breathing hyperoxic air (FinO2 = 0.40) did not affect thermoregulation under these conditions.
