    • This book advances critical discussions about what coloniality, decoloniality and decolonization mean and imply in the Nordic region. It brings together analysis of complex realities from the perspectives of the Nordic peoples, a region that are often overlooked in current research, and explores the processes of decolonization that are taking place in this region. The book offers a variety of perspectives that engage with issues such as Islamic feminism and the progressive left; racialization and agency among Muslim youths; indigenizing distance language education for Sami; extractivism and resistance among the Sami; the Nordic international development endeavour through education; Swedish TV-reporting on Venezuela; creolizing subjectivities across Roma and non-Roma worlds and hierarchies; and the whitewashing and sanitization of decoloniality in the Nordic region. As such, this book extends much of the productive dialogue that has recently occurred internationally in decolonial thinking but also in the areas of critical race theory, whiteness studies, and postcolonial studies to concrete and critical problems in the Nordic region. This should make the book of considerable interest to scholars of history of ideas, anthropology, sociology, cultural studies, postcolonial studies, international development studies, legal sociology and (intercultural) philosophy with an interest in coloniality and decolonial social change.

    • Medical or health-oriented screening programs are amongst the most debated aspects of health care and public health practices in health care and public health ethics, as well as health policy discussions. In spite of this, most treatments of screening in the research literature restrict themselves to isolated scientific aspects, sometimes complemented by economic analyses or loose speculations regarding policy aspects. At the same time, recent advances in medical genetics and technology, as well as a rapidly growing societal focus on public health concerns, inspires an increase in suggested or recently started screening programs. This book involves an in-depth analysis of the ethical, political and philosophical issues related to health-oriented screening programs. It explores the considerations that arise when heath care interacts with other societal institutions on a large scale, as is the case with screening: What values may be promoted or compromised by screening programs? What conflicts of values do typically arise – both internally and in relation to the goals of health care, on the one hand, and the goals of public health and the general society, on the other? What aspects of screening are relevant for determining whether it should be undertaken or not and how it should be organised in order to remain defensible? What implications does the ethics of screening have for health care ethics as a whole? These questions are addressed by applying philosophical methods of conceptual analysis, as well as models and theories from moral and political philosophy, medical ethics, and public health ethics, to a large number of ongoing and proposed screening programs which makes this book the first comprehensive work on the ethics of screening. Analyses and suggestions are made that are of potential interest to health care staff, medical researchers, policy makers and the general public.

    • In this article, we draw on Michael Lipsky's work on street-level bureaucrats and discretion to analyse a real case setting comprising an interview study of 30 Swedish doctors regarding their experiences of changes in clinical work following patients being given access to medical records information online. We introduce the notion of invisibility work to capture how doctors exercise discretion to preserve the invisibility of their work, in contrast to the well-established notion of invisible work, which denotes work made invisible by parties other than those performing it. We discuss three main forms of invisibility work in relation to records: omitting information, cryptic writing and parallel note writing. We argue that invisibility work is a way for doctors to resolve professional tensions arising from the political decision to provide patients with online access to record information. Although invisibility work is understood by doctors as a solution to government-initiated visibility, we highlight how it can create difficulties for doctors concerning accountability towards patients, peers and authorities.

    • This paper examines the accomplishment of making technology work, using the discourse around telemedicine in Swedish healthcare during 1994-2003. The paper will compare four projects launched in the mid-1990s and policymakers’ visions of healthcare through telemedicine. I will employ a sociotechnical approach developed within Actor-Network Theory that understands functioning technology not as something intrinsic but as an outcome of an ongoing process of negotiations. In the paper, I will extend the sociotechnical approach of what constitutes working technology to include spatiotemporal matters. I will also approach the closely related issue of space that has become a concern of Actor-Network Theory scholars interested in the accomplishment and continued workings of technology as it travels. In this discussion, an emphasis on fixed relations (network space) has been challenged by investigations into changing relations (fluid space). This paper suggests that in order to travel well, technology must be both fixed and fluid.⁰

  • Communicative challenges among physicians, patients, and family caregivers in cancer care: An exploratory qualitative study in Ethiopia

    • Background: Cancer is a growing concern in Ethiopia. Though communication is essential for the treatment process, few studies have looked at communication in Ethiopian cancer care. Due to the large number of patients and scarcity of resources, it is vital to understand how to manage consultations in order to effectively help as many patients as possible in this challenging work environment. Thus, research is needed to analyze and understand the communicative challenges experienced by physicians, patients, and family caregivers, in order to successfully handle patient care in practice. Objective We explore communication in Ethiopian cancer care and present the main challenges faced by physicians, patients, and family caregivers. Methods This explorative qualitative study was conducted at the Oncology Department of the Tikur Anbessa (Black Lion) Specialized Teaching Hospital (TASH) in Addis Ababa, Ethiopia. A triangulation of data collection methods was used: 91 audio-recorded, semi-structured interviews and 21 video-recordings of authentic interactions during hospital rounds. The aim was to obtain as complete a picture as possible of communication from the perspectives of physicians, patients, and family caregivers. The interviews were analyzed using thematic content analysis and the identified themes were supported by excerpts from the transcribed recordings. Results Eight themes emerged from the data. Workload and time pressure, in combination with restricted space for privacy, limited the possibilities for physicians to deliver detailed information and provide emotional support. Furthermore, patient literacy levels, in combination with no or little cancer awareness, financial problems, reliance on traditional and religious treatments, the stigma of cancer, and a fatalistic attitude, resulted in delays in patients seeking care and participating in positive health behaviors, and, subsequently, often resulted in an unwillingness to openly discuss problems with physicians and adhere to treatment. The study also illustrates the paramount role of family in physician-patient communication in Ethiopia. Though family caregivers provide a valuable interpreting support when patients have limited language skills, they can also prevent patients from sharing information with physicians. Another important finding is that family caregivers were often responsible for making decisions about treatment and avoided telling patients about a poor prognosis, believing that conveying bad news may upset them. All of these themes have important implications for the role of ethically acceptable communication in patient-centered care. Conclusions This study has identified a number of serious challenges for successful and ethically acceptable health communication in Ethiopian cancer care. The study contributes to our understanding of the complexity around the role of family, combined with patients’ dependency on family members for communication, support, and access to care, which creates particular ethical dilemmas for the medical staff. The questions raised by this study concern how to organize consultations to achieve patient-centered health communication, while maintaining a constructive alliance with the family and not jeopardizing the patient’s continued access to care. The integration of communication training for medical students in Ethiopia, with a focus on ethical guidelines for family-centered patient consultation suitable for these circumstances, would be an essential step.

    • The cat (Felis catus, Linneaus 1758) has lived around or with humans for at least 10,000 years, and is now one of the most popular pets of the world with more than 600 millionindividuals. Domestic cats have developed a more extensive, variable and complex vocal repertoire than most other members of the Carnivora, which may be explained by their social organisation, their nocturnal activity and the long period of association between mother and young. Still, we know surprisingly little about the phonetic characteristics of these sounds, and about the interaction between cats and humans.Members of the research project Melody in human–cat communication (Meowsic) investigate the prosodic characteristics of cat vocalisations as well as the communication between human and cat. The first step includes a categorisation of cat vocalisations. In the next step it will be investigated how humans perceive the vocal signals of domestic cats. This paper presents an outline of the project which has only recently started.

    • In 2020, for the first time in history, there were more people in the world aged 60 years and over than there were children below the age of 5 years. The population aged over 65 years is projected to increase from one billion in 2019 to more than two billion in 2050, and those aged over 80 years are projected to increase from 143 to 426 million, with the largest increase occurring in the developing world (UN World Population Prospects, 2019). This demographic trend constitutes the largest global health challenge, according to the World Health Organisation (WHO). The European Union has set it as one of the major challenges in Horizon 2020 and it has important societal implications (European Commission, 2020). The proportion of retired individuals will increase, leading to an increased ratio between those who have exited the workforce and those still active in the labour market. Thus, ageing represents a global societal and scientific challenge requiring integrated efforts, multidisciplinary translational research approaches and social innovations that build on ideas of potentials and capabilities, emphasising the value of old age.

    • Countries worldwide have very different national regulations on human embryonic stem (ES) cell research, informed by a range of ethical values. Some countries find reason to extend the applicability of their regulations on such research to its citizens when they visit other countries. Extraterritorial jurisdiction has recently been identified as a potential challenge towards global regulation of ES cell research. This chapter explores the implications and impact of extraterritorial jurisdiction and global regulation of ES cell research on researchers, clinicians and national health systems, and how this may affect patients. The authors argue that it would make ethical sense for ES cell restrictive countries to extend its regulations on ES cell research beyond its borders, because, if these countries really consider embryo destruction to be objectionable on the basis on the status of the embryo, then they ought to count it morally on par with murder (and thus have a moral imperative to protect embryos from the actions of its own citizens). However, doing so could lead to a legal situation that would result in substantial harm to central values in areas besides research, such as health care, the job market, basic freedom of movement, and strategic international finance and politics. Thus, it seems that restrictive extraterritorial jurisdiction in respect to ES cell research would be deeply problematic, given that the ethical permissibility of ES cell research is characterised by deep and wide disagreement.

    • During the past decade, tangible sensor technologies have matured and become less expensive and easier to use, leading to an explosion of innovative musical designs within video games, smartphone applications, and interactive art installations. Interactive audio has become an important design quality in commercially successful games like Guitar Hero , and a range of mobile phone applications motivating people to interact, play, dance, and collaborate with music. Parallel to the game, phone, and art scenes, an area of music and health research has grown, showing the positive results of using music to promote health and wellbeing in everyday situations and for a broad range of people, from children and elderly to people with psychological and physiological disabilities. Both quantitative medical and ecological humanistic research show that interaction with music can improve health, through music’s ability to evoke feelings, motivate people to interact, master, and cope with difficult situations, create social relations and experience shared meaning. Only recently, however, the music and health field has started to take interest in interactive audio, based on computer-mediated technologies’ potential for health improvement. Here, we show the potential of using interactive audio in what we call interactive musicking in the computer-based interactive environment Wave. Interactive musicking is based on musicologist Christopher Small’s concept “musicking”, meaning any form of relation-building that occurs between people, and people and things, related to activities that include music. For instance, musicking includes dancing, listening, and playing with music (in professional contexts and in amateur, everyday contexts). We have adapted the concept of "musicking" on the design of computer-based musical devices. The context for this chapter is the research project RHYME. RHYME is a multidisciplinary collaboration between the Centre for Music and Health at the Norwegian Academy of Music, the Oslo School of Architecture and Design (AHO), and Informatics at the University of Oslo. Our target group is families with children with severe disabilities. Our goal is to improve health and wellbeing in the families through everyday musicking activities in interactive environments. Our research approach is to use knowledge from music and health research, musical composition and improvisation, musical action research, musicology, music sociology, and soundscape studies, when designing the tangible interactive environments. Our focus here is interaction design and composition strategies, following research-by-design methodology, creating interactive musicking environments. We describe the research and design of the interactive musicking environment Wave, based on video documentation, during a sequence of actions. Our findings suggest some interactive audio design strategies to improve health. We base the design strategies on musical actions performed while playing an instrument, such as impulsive or iterative hitting, or sustainable stroking of an instrument. Musical actions like these can also be used for musicking in everyday contexts, creating direct sound responses to evoke feelings that create expectations and confirm interactions. In opposition to a more control-oriented, instrument and interface perspective, we argue that musical variation and narrative models can be used to design interactive audio, where the audio is seen as an actor taking many different roles, as instrument, co-musician, toy, etc. In this way, the audio and the interactive musicking environments will change over time, answering with direct response, as well as nose-thumbing and changing response, motivating creation, play, and social interaction. Musical variation can also be used to design musical backgrounds and soundscapes that can be used for creating layers of ambience. These models create a safe environment and contribute to shared meaning.

    • While violence against health care workers is being progressively recognised as a serious problem in the healthcare industry, it remains an under-studied area of enquiry in global public health. Anthropologists have long observed that violence toward patients is tied to institutional care practises in multiple ways, including repression, misrecognition and silencing. But research on health care staff’s experience of violence is still lacking. This article aims to address this literature gap by providing research on the daily experience of vulnerability to violence that health care providers face during their work. To do so, the paper ethnographically explores the effects and perception of violence against health care workers in an emergency department (ED) in northern Italy, a place with a dramatic escalation of violent incidents. The article illustrates how the ED staff attended to the experience of suffering of potentially violent patients. In so doing, ED professionals shifted the responsibility of violence against them from violent individuals to violent structures shaping health inequities. The paper thus argues that ED professionals display a structural competence perspective when dealing with violence.

    • A new landscape of prenatal testing (PNT) is presently developing, including new techniques for risk-reducing, non-invasive sampling of foetal DNA and drastically enhanced possibilities of what may be rapidly and precisely analysed, surrounded by a growing commercial genetic testing industry and a general trend of individualization in healthcare policies. This article applies a set of established ethical notions from past debates on PNT for analysing PNT screening-programmes in this new situation. While some basic challenges of PNT stay untouched, the new development supports a radical individualization of how PNT screening is organized. This reformation is, at the same time, difficult to reconcile with responsible spending of resources in a publicly funded healthcare context. Thus, while the ethical imperative of individualization holds and applies to PNT, the new landscape of PNT provides reasons to start rolling back the type of mass-screening programmes currently established in many countries. Instead, more limited offers are suggested, based on considerations of severity of conditions and optimized to simultaneously serve reproductive autonomy and public health within an acceptable frame of priorities. The new landscape of PNT furthermore underscores the ethical importance of supporting and including people with disabilities. For the very same reason, no ban on what may be analysed using PNT in the new landscape should be applied, although private offers must, of course, conform to strict requirements of respecting reproductive autonomy and what that means in terms of counselling.

    • Objective: The aim of this study was to explore expecting parents’ perceptions of the Childbirth Journey as an intervention that includes medical information for parental support, constructed as a serious game.Methods: In this qualitative study, semi-structured interviews were held with expecting parents in Sweden who were able to talk about specific parts of the Childbirth Journey they appreciated or found difficult to understand. A phenomenographic methodology was employed for data analysis.Results: Participants perceived the Childbirth Journey to be easily accessible and customized with reliable information. The design and features of the intervention were perceived by the expecting parents to enhance the intervention’s usability, appeal, and trustworthiness. When parental couples used the Childbirth Journey together, it gave them an opportunity to discuss and better understand each other’s situation. The participants proposed several changes to the existing version of the game, mostly related to extending practical information and illustrated scenarios but also to the further development of the game’s design and animations. The participants found the Knowledge portal to be the most appealing part of the Childbirth Journey.Conclusions: The Childbirth Journey intervention was concluded to be a valuable digital complement to in-person profes- sional support, especially given the current COVID-19 pandemic restrictions in place in Sweden, which do not allow antenatal visits by partners. However, in its current form, the Childbirth Journey has some deficiencies and would therefore benefit from further development and exploration.

    • The vision of legendary criminologist Cesare Lombroso to use scientific theories of individual causes of crime as a basis for screening and prevention programmes targeting individuals at risk for future criminal behaviour has resurfaced, following advances in genetics, neuroscience and psychiatric epidemiology. This article analyses this idea and maps its ethical implications from a public health ethical standpoint. Twenty-seven variants of the new Lombrosian vision of forensic screening and prevention are distinguished, and some scientific and technical limitations are noted. Some lures, biases and structural factors, making the application of the Lombrosian idea likely in spite of weak evidence are pointed out and noted as a specific type of ethical aspect. Many classic and complex ethical challenges for health screening programmes are shown to apply to the identified variants and the choice between them, albeit with peculiar and often provoking variations. These variations are shown to actualize an underlying theoretical conundrum in need of further study, pertaining to the relationship between public health ethics and the ethics and values of criminal law policy.

    • OBJECTIVE: To quantify the association between long working hours and alcohol use.DESIGN: Systematic review and meta-analysis of published studies and unpublished individual participant data.DATA SOURCES: A systematic search of PubMed and Embase databases in April 2014 for published studies, supplemented with manual searches. Unpublished individual participant data were obtained from 27 additional studies.REVIEW METHODS: The search strategy was designed to retrieve cross sectional and prospective studies of the association between long working hours and alcohol use. Summary estimates were obtained with random effects meta-analysis. Sources of heterogeneity were examined with meta-regression.RESULTS: Cross sectional analysis was based on 61 studies representing 333 693 participants from 14 countries. Prospective analysis was based on 20 studies representing 100 602 participants from nine countries. The pooled maximum adjusted odds ratio for the association between long working hours and alcohol use was 1.11 (95% confidence interval 1.05 to 1.18) in the cross sectional analysis of published and unpublished data. Odds ratio of new onset risky alcohol use was 1.12 (1.04 to 1.20) in the analysis of prospective published and unpublished data. In the 18 studies with individual participant data it was possible to assess the European Union Working Time Directive, which recommends an upper limit of 48 hours a week. Odds ratios of new onset risky alcohol use for those working 49-54 hours and ≥55 hours a week were 1.13 (1.02 to 1.26; adjusted difference in incidence 0.8 percentage points) and 1.12 (1.01 to 1.25; adjusted difference in incidence 0.7 percentage points), respectively, compared with working standard 35-40 hours (incidence of new onset risky alcohol use 6.2%). There was no difference in these associations between men and women or by age or socioeconomic groups, geographical regions, sample type (population based v occupational cohort), prevalence of risky alcohol use in the cohort, or sample attrition rate.CONCLUSIONS: Individuals whose working hours exceed standard recommendations are more likely to increase their alcohol use to levels that pose a health risk.

    • Immersive virtual reality (VR) videos can replicate complex real-life situations in a systematic, repeatable and versatile manner. New product development trajectories should consider the complexities of daily life eating situations. The creation of immersive contexts of a product with varying levels of appropriateness could be a useful tool for product developers in evaluating the extent to which context may influence food acceptance and eating behavior. This study explored virtual reality (VR) as an efficient context-enhancing technology through evaluations of protein-enriched rye breads and compared the effects of a VR-simulated congruent (VR restaurant) and incongruent (VR cinema) contexts on the acceptance in older consumers. A total of 70 participants were immersed in the two VR contexts and a neutral control context in a randomized order. The responses indicating the desire and liking for rye breads were measured, and the extent of immersion during context exposure was assessed by levels of the sense of presence and engagement. Immersive VR induced positive sensations of presence and a heightened level of engagement. The VR restaurant and neutral contexts were perceived as more appropriate for consuming rye breads and induced higher desire and liking for rye breads, which supported the notion of the alignment of congruent contexts with food desire and liking. The study provides new perspectives, practical methodologies, and discoveries in regard to the creation and application of VR-immersed contexts in food product evaluation. Moreover, it focused on a consumer segment (older consumers) that has seldom been investigated in previous relevant studies. The findings suggest that immersive VR technology, as a tool for evaluating contextual factors, is important for new product development. The good user experience among older consumers further indicated the potential value of VR as a context-enhancing tool for product development.

    • The current study assessed the responses from a survey titled ”Life and Health – Young People 2005”, completed by 2.878 15-16 year-old adolescents in mainstream schools in the county of Örebro, Sweden. Thirty-nine percent of students with hearing loss (slight, mild, or moderate) and 6% of students with normal hearing reported tinnitus often or always during the past three months. Almost no gender difference was observed among students with normal hearing reporting tinnitus (boys 6.3%, girls 5.6%); however, a gender difference was noticed among hard-of-hearing (HH) students (boys 50%, girls 28%). Adolescents with both hearing loss and tinnitus reported considerably higher scores for mental symptoms, substance use, and school problems than other students. Anxiety in the past three months, male gender, and alcohol consumption in the past year were associated with tinnitus in HH students; irritation and anxiety in the past three months, disability, use of illicit drugs, and truancy predicted tinnitus in the normal hearing group. Consequently, students with a hearing loss and tinnitus are at high risk and should be monitored for subsequent problems.

    • The development of forensic psychiatric risk assessments is discussed from a clinical point of view using the example of Sweden. A central task in forensic psychiatry has traditionally been to identify dangerous, mentally disordered subjects considered to be prone to commit violent acts. Over time, “dangerousness” has been reworded into “risk”. Nevertheless, such assessments have generally been based on the psychiatric factors characterising the individual patient, while group interaction, situational factors, or social and cultural circumstances, such as the availability of alcohol and drugs, have been largely overlooked. That risk assessments have a focused on people with a diagnosis of “mental disorder” and been used as grounds for coercive measures and integrity violations has somehow been accepted as a matter of course in the public and political debate. Even the basic question whether offenders with a mental disorder are really more prone to criminal recidivism than other offenders seems to have been treated light-handedly and dealt with merely by epidemiological comparisons between groups of persons with broad ranges of psychosocial vulnerability and the general population. Legal texts, instructions and guidelines from the authorities in charge are often vague and general, while actors in the judicial system seem to put their trust in psychiatric opinions. The exchange of professional opinions, general public expectations, and judicial decision processes poses a huge risk for misunderstandings based on divergent expectations and uses of terminology.

  • Geographies of eHealth: Studies of Healthcare at a Distance

    • This thesis examines the proliferation of healthcare services using information and communication technology to overcome spatial and temporal obstacles. These services are given such names as telemedicine and telecare, which are sometimes grouped together as telehealthcare under the umbrella term eHealth. My main argument is that a prevalent and overoptimistic rhetoric of how the possibilities of digitalization are expected to produce a homogenous and ubiquitous healthcare space conceals many of the spatiotemporal complexities involved in introducing telehealthcare and in the overall organizing of healthcare. To counteract such simplifications, I contend that we need a relational understanding of the technical and the geographical as always nested in the social and vice versa. With such an approach, it is arguably possible to begin to tease apart the many spatiotemporal entanglements of these innovations and to trace their political ramifications. This position is developed by integrating perspectives from science and technology studies with insights from human geography. The four constituent papers of this thesis pursue this argument in qualitatively grounded case studies of telehealthcare and its geographies. Paper I looks at various initiatives for fetal tele-ultrasonography, demonstrating that this practice cannot be reduced to a mere transparent relay for the speedy transmission of digital information across space and time. The paper investigates how its introduction could affect medical knowledge production, power hierarchies, and subject positions, for example, the status attributed to the fetal figure. Paper II traces Swedish transformations of telehealthcare. The use of telemedicine to reach those outside medicine’s range has arguably been accompanied by efforts to achieve intra-organizational streamlining via telemedicine. This process has continued with the emergence of telecare for personal use directed toward the overlapping groups of the elderly people and patients with chronic conditions. I contend that this shift can be understood through a geographical lens as attempts to save space and time by keeping as many patients as possible out of costly hospitalization and preventing them from engaging scarce specialist resources. Paper III compares four telemedicine projects in Sweden. In detailing how the purpose of practicing telemedicine differed between these projects in relation to, for example, the specifics of distance, care availability, and treated medical conditions, the paper demonstrates the existence of many versions of telemedicine. Whereas this fluidity could further the spread of telemedicine, it could also cause problems. To various actors wanting to use telemedicine in a homogenous and fixed way for national streamlining purposes, this diversity has generated confusion when they wished to align telemedicine in a preferred direction. The paper concludes that technology travels best when it can contain both fluid and fixed relationships. Paper IV argues that, whatever is claimed about creating a space- and time-independent healthcare by means of telehealthcare, the use of telecare to connect the standardized spaces of healthcare with the fluid everyday lives of elderly people and patients with chronic conditions actually works by unfolding new spaces of visibilities and establishing new temporalities as well. By investigating these spatiotemporalities, I demonstrate how these applications draw together discourses on individual freedom with medically derived algorithms and concerns about how to make best use of scarce healthcare resources.

    • The Balaton Ecomuseum, which is being continuously developed since 2017, will have a holistic approach, where the objectives of the ecomuseum embrace the whole cultural landscape of Lake Balaton as one unit with several thematic routes in one system and shall not be restricted to one particular subject area or a part of local heritage. One of these thematic routes is the recently developingHerbs and Spices Network, led by Zánka Herb Valley Visitor and Training Centre based on the collection, cultivation and processing of medicinal plants, herbs and spices. The place of herbs and spices in the diet needs to be considered in reviewing health benefits, including definitions of the food category and the way in which benefits might be viewed, and therefore researched.Here we describe the already established system of the Zánka Herb Valley Visitor and Training Centre, the potential of the Balaton Region in the development of herbal medicine illustrated by the scientific presentation of the 30 most collected herbs in the region and examples of other herbal centres, which are intended to become a part of the network. Herbalmedicine, as an important part of the intangible cultural heritage, with hundreds of years old recipes for herbal concoctionshas been known since ancient times before science related to modern medicine developed and continues to be used for generationsuntil now. Furthermore, the medical effects of many agricultural crops should be better understood, such as the grapevine, which is being investigated for its medical compounds or the medicinal properties of other fruits and vegetables not sufficiently known to the general public. In this study we present a new system of the culture and interactive education of the collection, cultivation and use of medicinal plants, herbs and spices applying a learning by doing approach and a network embracing the whole area of the Balaton Ecomuseum.

    • ORU2015 – Executive SummaryDuring 2015, all research performed from 2008 to 2014 at Örebro University, as well as research at Örebro University Hospital, has been evaluated. This report – ORU2015 – presents the background, planning and implementation of the research assessment and its results. Chapter I includes the panel evaluations, and chapter II presents the bibliometric data. Of the 38 subunits of evaluation, 8 are within the Faculty of Business, Science and Engineering, 17 are within the Faculty of Humanities and Social Sciences, 7 within the Faculty of Medicine and Health, and 6 at Region Örebro County’s University Hospital. The evaluation had a meta-analytical approach (see Annex A), and the external multidisciplinary panel assessed the research in each subunit of evaluation (see Annex B). The panel’s evaluation material consisted of a research overview, documentation on academic staff and competence, as well as on funding, self-evaluations and bibliometric data. The self-evaluations by each subunit addressed (i) scientific quality and scientific impact, (ii) impact and outreach, (iii) internationalisation, and (iv) research – education interaction. Each overarching evaluation unit was also assessed, including a SWOT analysis, by the respective heads of schools and deans. Apart from the self-evaluations, the material was retrieved from the university databases, Web of Science and Academic Archive Online (DiVA). The subunits had the opportunity to update their research information for the research overview prior to making the material available to the panel. The fourteen panellists, representing economics, natural sciences and technology, humanities, social sciences, medicine and health sciences, met for two days in October at Örebro University for the evaluation discussions. The agreed evaluation statements were delivered shortly thereafter. The great variability in the subunits’ scientific practices, scale, and establishment had to be accounted for in the panel evaluations. The evaluation subunits range from very large (up to 60 researchers), to medium sized (about 20 researchers), and to quite small subunits (fewer than nine researchers). The points of reference for the panel’s statements were the (i) quality of research, (ii) research environment and infrastructure, (iii) scientific and social interaction and (iv) future potential. Gradings ranged between Excellent (5) and Insufficient (1). The key data in the bibliometric assessment was scientific impact, vitality, productivity and international visibility, as indicated by the publications of each subunit. It can be seen from the panel statement of a subunit and the matching bibliometric data that these two assessments correspond to a large extent, but not completely.It is concluded from the panel evaluation that there are Excellent (5), Very Good (4), Good (3), Sufficient (2), as well as Insufficient (1) subunits at the university. A majority of the fourteen subunits that performed well (grade 3 – 5) are medium-sized, whilst the majority of the sixteen weakly performing subunits (grade 1 – 2) are small in size. Of course, for the humanities and social sciences, the Web of Science data only contains output to a limited degree. Therefore data from DiVA has been used and compared as well. For some subunits this makes a difference, but of the 16 subunits that show a weak performance according to Web of Science data, ten also perform weakly as shown in DiVA. Only three of these subunits score Good and one Very Good in DiVA.It can be seen from ORU2015 that the research volume, especially expressed in scientific publications per year and citations, has roughly doubled since ÖRE2010. In 2014, the total number of publications in Web of Science by researchers at the university and the university hospital reached some 600 and the number of citations were 14,000 the same year. The ‘findings’ of ORU2015 provide an important basis for decisions by leaders at all levels of the university in terms of strategic planning, support, and development of the research for the future.

    • Question Emissions of high concentrations of antibiotics from manufacturing sites select for resistant bacteria and may contribute to the emergence of new forms of resistance in pathogens. Many scientists, industry, policy makers and other stakeholders recognize such pollution as an unnecessary and unacceptable risk to global public health. An attempt to assess and reduce such discharges, however, quickly meets with complex realities that need to be understood to identify effective ways to move forward. This paper charts relevant key actor-types, their stakes and interests, incentives that can motivate them to act to improve the situation, as well as counterincentives that may undermine such motivation. Method The actor types and their respective interests have been identified using research literature, publicly available documents, websites, and the knowledge of the authors. Result Thirty-three different types of actor-types were identified, representing e.g. commercial actors, public agencies, states and international institutions. These are in complex ways connected by differing and partly similar interests that sometimes may conflict, sometimes pull in the same direction. Some actor types can act to create incentives and counterincentives for others in this area. Conclusions The analysis demonstrates and clarifies the challenges in addressing industrial emissions of antibiotics, notably the complexity of the relations between different types of actors, their international dependency and the need for transparency. The analysis however also suggest possible ways of initiating incentive-chains to eventually improve the prospects of motivating industry to reduce emissions. High resource consumer states, especially in multinational cooperation, hold a key position to initiate such chains.

    • This chapter charts and critically analyses the ethical challenge of assessing how much (and what kind of) evidence is required for the justification of interventions in response antibiotic resistance (ABR), as well as other major public health threats. Our ambition here is to identify and briefly discuss main issues, and point to ways in which these need to be further advanced in future research. This will result in a tentative map of complications, underlying problems and possible challenges. This map illustrates that the ethical challenges in this area are much more complex and profound than is usually acknowledged, leaving no tentatively plausible intervention package free of downsides. This creates potentially overwhelming theoretical conundrums when trying to justify what to do. We therefore end by pointing out two general features of the complexity we find to be of particular importance, and a tentative suggestion for how to create a theoretical basis for further analysis.

    • Since a couple of decades, the notion of a precautionary principle plays a central and increasingly influential role in international as well as national policy and regulation regarding the environment and the use of technology. Urging society to take action in the face of potential risks of human activities in these areas, the recent focus on climate change has further sharpened the importance of this idea. However, the idea of a precautionary principle has also been problematised and criticised by scientists, scholars and policy activists, and been accused of almost every intellectual sin imaginable: unclarity, impracticality, arbitrariness and moral as well as political unsoundness. In that light, the very idea of precaution as an ideal for policy making rather comes out as a dead end. On the basis of these contrasting starting points, Christian Munthe undertakes an innovative, in-depth philosophical analysis of what the idea of a precautionary principle is and should be about. A novel theory of the ethics of imposing risks is developed and used as a foundation for defending the idea of precaution in environmental and technological policy making against its critics, while at the same time avoiding a number of identified flaws. The theory is shown to have far-reaching consequences for areas such as bio-, information- and nuclear technology, and global environmental policy in areas such as climate change. The author argues that, while the price we pay for precaution must not be too high, we have to be prepared to pay it in order to act ethically defensible. A number of practical suggestions for precautionary regulation and policy making are made on the basis of this, and some challenges to basic ethical theory as well as consumerist societies, the global political order and liberal democracy are identified

    • Denna rapport har till syfte att ge en kunskaps-översikt över tillgänglig forskning på temat ”Fysisk arbetsmiljö för hälsa, välmående och prestation”. Kunskapsöversiktens innehåll fokuserar på hur en frisk och välmående fysisk arbetsplats utformas, för att ge förutsättningar för god arbetsprestation parallellt med hög grad av välmående. Rapporten är avsiktligen vinklad bort från problemfokuserad litteratur om skaderisker, likaså om insatser för hälsopromotion som syftar till att individen tar ett större eget ansvar för att öka sin fysiska aktivitet eller ändring av livsstil. Istället riktas fokus mot friskfaktorer som härrör från en väl genomtänkt och avsiktlig formgivning (design) av arbetsplatsen, både i form av layout och dess ingående tekniska lösningar (exempelvis utrustningar, möbler med mera). Översikten inkluderar till stor del litteratur från ergonomi- och design-fältet, i syfte att betona kunskap om den avsiktligt formgivna arbetsmiljön och dess ingående designparametrar. En systematisk litteratursökning har genomförts med sökning i två databaser, Scopus och Web of Science. Till en början identifierades 4299 sökträffar som uppfyllde sökkriterierna. Efter sållning, granskning av abstracts och kvalitetsutvärdering av 446 fulltexter inkluderades och kategoriserades till slut 317 artiklar i två huvudsakliga kategorier: 196 primär- och litteraturstudier om fysisk belastning på arbetsplatser, samt 121 designprocessorienterade artiklar vars innehåll ger råd om, vägleder och organiserar formgivningen av en hälsosam och välfungerande fysisk arbetsplats. Kvalitetsgranskning genomfördes på samtliga inkluderade artiklar; empiriska primärstudier utvärderades med McGill Mixed Methods Appraisal Tool (MMAT), litteraturstudier med Critical Appraisal Skills programmes (CASP) mall för kunskapsöversikter, och den designprocessorienterade litteraturen genomgick en förenklad kvalitetsgranskning med specialanpassad mall. Den inkluderade kunskapen spänner över många olika studietyper, metodansatser, interventionsavsikter och yrkesgrupper. Inom kategorin primär- och litteraturstudier identifierades fältstudier (134), laboratoriska studier (45), enkätbaserade tvärsnittsstudier (53) och litteraturstudier (14). Ett första resultat var att friskfaktorer av olika slag identifierades; dessa uttrycks oftast i form av önskade utfall som är spridda över ett spektrum mellan välbefinnande å ena sidan och prestation å andra sidan, till exempel välmående, nöjdhet, återhämtning, komfort, engagemang, prestation, produktivitet, effektivitet, kreativitet, problemlösning och samarbete. Vissa ”negativt” formulerade utfall är också indikatorer på fysisk belastningsfriskhet, till exempel minskad stress, fysisk belastning, fysisk skaderisk eller felhandling. Som synes håller sig inte alla dessa utfall strikt till den fysiska arbetsmiljön, utan påverkar även kognitiva och organisatoriska friskfaktorer. Resultaten redovisades även indelat i de yrkeskategorier som kunde betraktas som utmärkande i materialet. Den största proportionen av yrkesrelaterad litteratur om friskfaktorer handlar om kunskapsarbete (som domineras av kontorsstudier), följt av studier från vårdsektorn, därefter från industriella tillämpningar (inklusive byggbranschen) och därefter övriga yrken och studier som inkluderar flera yrkeskategorier samtidigt. Den design-processorienterade litteraturen innehåller mycket vägledning i hur arbetsplatser bör utformas generellt för att förbättra friskhet. De flesta studier om designprocesser som ger processrekommendationer om hur utformningsarbetet bör fortlöpa och organiseras rekommenderar en deltagande (participativ) ansats, för att skapa bättre förståelse för medarbetarnas behov och skapa engagemang under designprocessen både för design av arbetsredskap och av arbetsplatser. Det finns dock en del studier som visar att deltagande processer inte alltid leder till önskade effekter.  Andra värdefulla bidrag från denna litteraturkategori berör användandet av olika verktyg för simulering och utvärdering samt metoder för att bedöma lämpligheten av arbetsplatsutformningen för specifika grupper av användare. Både arbetsplatsdesign och tekniska produktlösningar för att stödja friskhet på arbetsplatsen har utvärderats av litteraturen. Ett fåtal studier fokuserade på att utvärdera arbetsredskap för olika yrkeskategorier, ofta med belastningsergonomiska och riskkartläggande perspektiv, men även med hänsyn till aspekter som nöjdhet, preferens och användarvänlighet. Andra specifika spår i litteraturen är artiklar om sittande och stående, särskilda arbetskrav och förutsättningar för äldre arbetskraft, återhämtning. Användning av nya teknikutvecklingar i design- och utvärderingsprocesser är en annan trend som syns i litteraturen. Rapporten lyfter fram tre områden som kan bli allt vanligare: robotar som arbetsredskap inklusive hur arbetsfördelning mellan robotar och människor bör ske, användning av bärbar teknologi (wearables) eller rörelseregistrerande teknik (till exempel sensorer och kameror) för aktivitets- och/eller biometrisk mätning och slutligen ökad användning av simuleringar, exempelvis via så kallade digitala tvillingar, för design och underhåll av arbetsplatser. Enligt en kategori studier framhålls att designmässiga arbetsplatsförändringar bör kombineras med utbildning, träning och kunskapskomplettering för att uppnå beteendeförändringar hos användare av arbetsplatsen eller arbetsredskapen, detta för att erhålla de största och mest varaktiga effekterna på hälsa och välmående. Den designprocessorienterade litteraturen gav en rik och varierad vägledning i olika processer, ansatser, verktyg och metoder för hur en friskhetsfrämjande arbetsplatsdesign kan stödjas. Bidragen kan vara vägledande både för individ- och makroperspektiv, det vill säga systemsynen i artiklarna varierar mellan olika systemnivåer och kan vägleda såväl olika processteg som val och utvärdering av specifika utrustningar och layouter samt ge råd för hur en deltagande process som involverar medarbetarna bör gå till. Bland verktygen som beskrivs för dessa ändamål finns metoder, analytiska modeller, mätutrustningar och simuleringstekniker. Denna översikt hade en bred ansats, vilket har visat sig vara både en styrka och en svaghet. Det är svårt att finna klara, allmänna rekommendationer med hänsyn till så många olika specialsammanhang och skilda arbetssystems syften. Även om renodlade studier om enskilda ”friskhetsfrämjande” faktorer finns, är multi-utfallsinriktade ansatser en mycket mer intressant och relevant väg att gå för framtida forskning och utveckling. För att komma längre och djupare med sådana kombinationer av syften, tror författarna att en framgångsfaktor är att begränsa sig till fördjupade studier inom avgränsade yrkesområden, till exempel industri, vård, kunskapsarbete och andra, mer specifika områden som i denna översikt har sammanförts med andra kategorier. En ytterligare rekommenderad fördjupning skulle vara att fokusera på utvärderingar av belastningsreducerande arbetsredskap, särskilt i syfte att vägleda organisationer, arbetsgivare och formgivare i processen att upphandla, välja, införskaffa och implementera utrustning och arbetsredskap. Något som bör beforskas mer är parallella utvärderingar av fysisk och kognitiv (eller mental) arbetsbelastning från samma arbete. Detta är svårt men mycket viktigt att undersöka. Denna studie har även funnit få bidrag som berör tidsrelaterade faktorer som till exempel schemaläggning, rotation med mera, vilka presenterats som strategier för en hälsosammare grad av fysisk belastning i arbetet. En närbesläktad aspekt som vore intressant att undersöka närmare är hur interaktionen med digitala gränssnitt påverkar den fysiska belastningen, då användningen av fler digitala tjänster leder till mer fysisk interaktion med ”smarta” gränssnitt och handhållna/bärbara tekniker både i våra yrken och privat. Vidare identifierade översikten få studier som beskriver misslyckade designprocesser.

    • The paper explores the room, both conceptually and ethically, for questioning, contradicting and adapting positions to patients in three specific areas: care of patients with vulnerable cpapcities for taking responsibility (adolescents and psychiatry), contexts of constrained autonomy (forensic care), and public health (antobiotic resistance stewardship and vaccination). These areas in various ethically relevant ways exhibit non-standard settings, compared to usual assumptions about the nature of patients and institutional contexts made when person centredness and shared decision-making are held out as primarily emancipating concepts. The paper probes to what extent that ethical idea may be maintained also in such non-standard settings.
