    • Up to 15 percent of all diagnosed pregnancies end in miscarriage. The loss can cause deep sorrow with complex emotional reactions as a result. Research shows that women often feel alone with their experiences of miscarriage and experience a wide range of emotions. The aim of this study was to describe the mourning woman’s experiences of early miscarriage. Six women who repeatedly had undergone miscarriage at a desired pregnancy were interviewed. The interviews were semi structured and analyzed with qualitative content analysis. The findings show that the miscarriages significantly affected the women’s life. Several psychological needs were identified, of which the most important were the need to feel unconditionally understood and having people at their side who attentively listen, provide security and respect their loss. These needs were rarely met in a satisfactory manner in health care. Regarding the women’s strong emotional reactions, it is desirable to have a routinely scheduled follow-up in which the women’s entire experience of the miscarriage is taken into consideration.

    • Aim: Reviewing the scientific literature with regards to urinary catheter washouts in order to prevent occlusion. Background: Patients with long-term indwelling urinary catheters often suffers from catheter blockage. Encrustations and the forming of biofilm on the catheter surface are among the most common causes of occlusion. It is unknown whether urinary catheter washout is the treatment of choice to prevent occlusion. Methods: A systematic review of the scientific literature regarding urinary catheter washouts. The study was conducted by two independent reviewers. The search used MeSH and freetext terms, in The Cochrane Database of Systematic Reviews, Academic Search Elite, CINAHL, MedLine and SweMed+, and an additional mapping of clinical guidelines in order to update praxis. Findings: The study shows that there is insufficient scientific evidence as to whether urinary catheter washouts prevent catheter blockage. Conclusion: There is insufficient evidence that urinary catheter washouts are beneficial. Individual care planning and evaluation of long-term indwelling catheter care is important in minimizing the risk of harmful side effects.

    • The nursingprofession is demanding and variable; nurses must have knowledge and proficiency practically, technically and be able to teamwork in the relation with all kinds of patients. At the same time they should perform nursing according to science and tried practice. One way to develop the engagement in patients and bring new knowledge into nursing is to participate in clinical supervision. The aim of the present study was to find out how nurses in somatic care describes what they learn and how knowledge integrates and develops in clinical supervision. Eight nurses were interviewed and their responses were taped, written down word for word and analysed according to a qualitative approach. The results shows that practically all knowledge were integrated by reflection. With the help of reflection the nurses could be taught by each others experiences, develop nursing and become skilful nurses.

    • Purpose: The purpose was to investigate the validity and reliability of short STAI (State-Trait Anxiety Inventory) for measuring children's anxiety in connection with procedures in hospital. Background: The children´s level of anxiety during hospitalization can cause problems and it should be evaluated with a valid and reliable instrument. STAIC-S (STAI for children) has previously been validated in children for this purpose but may be too complex to use. Short STAI has only been evaluated in adults and should be tested in children before it can be used. Methods: Children aged five to 16 filled in both STAIC-S and short STAI before and after an examination or treatment at the hospital. Results: Twenty children were included. Satisfactory internal reliability was found for short STAI with Cronbachs’s alpha 0.82. Correlation coefficients between the instruments were 0.88 before and 0.75 after the procedure. Significantly lower values were found after compared to before demonstrating constructive validity. Short STAI was easy to fill in but seven of 16 participants received help from their parents. Conclusion: Short STAI was shown to be a reliable and valid instrument for measuring anxiety in children, but a larger study is needed to confirm the validity and reliability further

  • Effects of process-oriented group supervision as reported by nursing students : a pilot study

    • One method to ensure that nursing students are better prepared for their future professional role can be to offer them process-oriented group supervision. The aim of this prospective, longitudinal pilot study was to evaluate the effects of a process-oriented group supervision programme (PGSP) comprising educational, supportive and developmental areas based on reports by nursing students undergoing a 3-year nursing education. The students (N=61) evaluated their experiences of the PGSP by means of a questionnaire, which they filled in after each study year. An independent t-test was conducted to compare the educational, supportive, and developmental areas after the first (2002), second (2003) and third year (2004) as well as over the whole 3-year study period. The result showed no significant difference in scores in the educational area. However, there was a significant increase in the supportive area (p=.03) over the 3-year period, which was especially noticeable during the first year (p=.013). There was also an increase in the developmental area over the 3-year period (p=.021) as well as during the first year (p=.024). Thus, PGSP seems to develop nursing students in their professional identity and personal growth. However, as this was a pilot study, a research implication is to perform a large-scale study over a longer period of time.

    • The purpose of this study was to describe degree projects on a bachelor level in nursing where peer review was used as quality assurance. Two student groups, 45 nursing students and seven registered nurses, participated. This case study describes quality assurance through collaboration between examiners. The results are focused on students´, supervisors´ and examiners` experiences during the degree process from the examiners perspective. Time disposal and criteria for a passing grade constituted factors associated with obstacles for students as well for supervisors and examiners. Peer review between two universities created opportunities for increasing awareness and validation of the quality of degree project and increased reliability for the examiner’s assessment. The study highlighted the need for collaboration between supervisors and examiners throughout the work process with student’s degree project. A model of group supervision e.g. with eight students, two supervisors and one examiner with regularly utilisation of peer-review for quality assurance is a further development to reach quality assurance in a degree project.

  • Pain among persons living at a nursing home

    • Pain is common among elderly. Very sick elderly persons living in nursing homes often have communication problems because of dementia or stroke, which causes problem to judge pain and evaluate effects of pain treatment. The aim of this study was to describe the prevalence, localisation and expressions of pain and to evaluate the usefulness of some pain assessment methods for nursing home residents. The study included 66 elderly living in a nursing home of which 49% had dementia as main medical diagnosis. A specially trained registered nurse (RN) performed pain assessment by * presenting a set of pain assessment tools * interpreting the nursing home residents' reactions during activity and * interviewing the resident about perceived pain. Two physiotherapists (PTs) performed physical examination including assessment of joint motion and palpation of joints and muscles. Contractures and ambulating status were noted. Cognitive Performance Scale (CPS) assessed cognitive capacity. When assisted by the RN 61% of the residents used independent or by assistance a verbal rating scale. The RN made an estimation of pain in all the nursing home residents and the PTs examined 92% of the residents. Pain was identified in 88% of the residents, 42% of the residents were treated with analgesics and pain it-as documented in the nurses' daily notes in 27% of the patient records. All residents with contractures had pain in the engaged joint. This study indicates that it is possible to assess pain by combining interview and interpretation of the residents' reactions during activity. Detection of pain by physical examination is a valuable complement especially for persons with decreased ability to communicate.

    • Background: Heart failure is a clinical syndrome where the heart is no longer able to maintain adequate blood circulation to the tissue. Many suffer from symptoms difficult to handle such as dyspnoea, fatigue, and physical weakness.Aim: The purpose of the study was to investigate peoples’ experiences of living with congestive heart failure.Method: A systematic literature review was used. Relevant articles were found in the Medline and Cinahl databases. The articles were examined, classified and critically appraised. Twenty articles were included. A qualitative content analysis was used to organize the results.Results: The results are described in three main categories: losses, protecting independences and readjustment. The category ‘losses’ details physical, emotional and social changes that people experience as losses caused by the debilitating symptoms of heart failure. The category ‘protecting independences’ shows the different ways in which people try to keep control over their lives. The category ‘readjustment’ presents how people try to seek a new ‘wholeness’ in life after the changes heart failure have caused by using acceptance, adaptation and finding new meanings.Conclusion: Living with congestive heart failure can be experienced as a synopsis of the «suffering human being» on a continuum, a time axle; from falling ill to seeking a new meaning in life.

    • The mean scores of the Creative Climate Questionnaire for 152 staff members working in 58 sheltered homes for individuals with autism wererated closer to creative than to stagnant organizations. There was a significant difference between two types of settings. In sheltered homes, thepersons with autism had their daily activity outside the home. Here staff members had higher scores in all creative dimensions compared tohomes with daily activities. Staff in sheltered homes, where the residents leave the premises for their daily work or activities, perceived the workclimate as significantly more creative and satisfying.

    • A central part of the nursing profession is the supervision in clinical practice of nursing students and a consequence of this is that the nurse needs to be prepared and have sufficient knowledge to support student's learning process. The aim of this study was to gain a deeper understanding of nurses' experiences of the supervision of nursing students in clinical practice. Fifteen nurses with experience of being supervisor were interviewed and the data material was analyzed with a Grounded Theory approach. The results comprise a core category “supervision was perceived as developing and stimulation” and four categories; to have sufficient with time, to have a working co-operation, to have sufficient knowledge and to get confirmation. These categories formed a conceptual model explaining the nurse’s experiences of the process of supervision and their needs of time, co-operation, knowledge and confirmation if the supervision should be perceived as developing and stimulating. More research needs to be conducted to expand the perspective on how supervision education and opportunities for reflection during the clinical education contribute to the student's learning process.

    • The multicultural society has impact on the nursing profession and multicultural aspects of education have impacts on international cooperation. Materials from 10 years in a Nordic network within higher education institutions within nursing was analysed in order to develop cooperation, mobility, curriculum and research. Methods of benchmarking were used in analysing political documents, memorandum, descriptions of the institutions, course programs and reports from teachers and student. The result indicated that the process from planning to realization was unclear, ambitions were higher than outcomes; experiences of mobility were mostly described in terms of personal experiences, enriching and useful, but relations to political goals instituional strategies were invisible. Effects on academic subjects or competence were not reported,nor were they asked for. Weeklong courses with more participants were succesful. However the research method implied new insights in the results: experiences became visible effects beyond an individual level exists, but they had to be named and asked for.

    • The aim with this literature review was to describe content, objectives, design, and outcomes of support groups for parentally bereaved children three to twelve years of age. In a support group with children in similar situation the children's thoughts, experiences and feelings will be normalised through different activities. Pedagogical methods include creative activities of different kinds, e.g. painting, writing letters and poems, reading stories, play and games. The purposes of the methods are to accept and understand the reality of the loss, to understand and work through the grieving, to enhance family communication, and to keep the memory of the death and go on living. Organisation and structure are described in relation to development levels and ages of children and youths. Outcomes of groups are characterized by clinical observations and a dearth of evaluative studies. All bereaved children should have the opportunity to participate in a support group when a parent, sibling or other relative dies. Well developed services for bereaved children should be a matter of health care policy and quality.

    • Ehlers-Danlos syndrome (EDS) is a hereditary connective tissue disorder with generalized joint hypermobility, complications of instability, chronic musculoskeletal pain, a bleeding tendency, oral health problems and food hypersensitivity. A connection between hypermobility of joints and gastrointestinal problems is described. This study shows that indigestion is common in this group of 250 respondents from the Swedish EDS group. In total, 30% of the respondents reported no sensitivity to food at all, but 47% reported hypersensitivity for more than four food items. Most frequent reported food items were carbohydrates, histamine- and birch-related foods. Indigestion, itching and diarrhoea were most common problems related to food hypersensitivity.

    • Patient classification in anaesthetic care is an undeveloped field and it is therefore urgent to construct an instrument for this purpose. The aim of the study was to develop an instrument to measure anaesthetic nursing care with focus on the unique care needed by every single patient. The Delphi technique was used. The method involved five series of inquiries to a panel of experts, consisting of ten nurses. The instrument developed, included 13 nursing care areas, with predefined alternatives graded from1 to 4, where 4 indicated high workload, and 1 indicated low workload. The instrument was tested in a pilot study and showed agreement with the clinical picture of the patients needs. An initial inter-rater reliability test showed very good agreement for all the nursing care areas. The instrument, however, needs to be tested for validity and reliability in a larger sample.

    • The aim of this study was to investigate validity and reliability of an instrument for patient classification in anaesthetic nursing care. The study included two parts. The first part involved two panels of experts, one internal consisting of nurse anaesthetists from the local hospital, and one external represented by nurse anaesthetists from nine different hospitals in Sweden and one in Iceland. The internal experts gave suggestions for improvements of the existing instrument. Thereafter the external experts gave their opinion about both the internal panels elucidation as well as the existing instrument. The instrument was thereafter adjusted in concordance to their suggestions. The second part involved an investigation of the inter-rater reliability, with one hundred double ratings before the changes in the instrument and one hundred ratings after the changes in the instrument. Statistical analyses for inter-rater reliability were performed with Spearmans correlation coefficient. The first assessment showed good inter-rater reliability in four of thirteen items, while the inter-rater reliability on the second version showed good agreement in eleven of thirteen items. Results indicated that the changes in the instrument were positive in terms of reliability. The instrument thus improved in terms of validity and reliability and may be of use in anaesthetic wards with similar context.

    • Background: A more complex and knowledge-based society has resulted in increasing specialization and greater demands on employees in the fields of practice. At Gjøvik University College, a new supervision model was tested for nursing students in practice. The main elements were: daily supervisors were given increased responsibility for assessing the students, while at the same time participating in three group counseling meetings. A portfolio contained the student’s work requirements. A weekly reflection hour was formalized. Purpose: The purpose was to elucidate how the supervision model influenced the student’s learning situation. Method: The SVIP model was evaluated by the use of focus group interviews. The data were analyzed using qualitative content analysis. Findings: Three categories: structure, inclusion and self-confidence describe the students’ need for clear guidelines and objectives, as well as their need to be included and to be seen. Conclusion: Various aspects of clinical practice, including daily supervision are important for student learning. Counseling meetings where the supervisor role is recognized and developed through the tutor, and daily supervisor sharing of experiences and knowledge, should permeate the cooperation.

    • The aim of this study is to describe nurses´ strategies to bring out empathy when encountering patients who are felt difficult to empathize with. The study is empirical and based on qualitative semi-structured interviews with registered nurses at a surgical ward. The data material was analyzed using content analysis. The result of the study shows that nurses employ different types of deliberate strategies in order to secure an empathetic treatment of these patients. In the study, the strategies are summarized in four head categories: strategies focusing on the patient, to decide upon one's set of mind, strategies focusing on oneself and indirect factors.

    • Assessing pain in children is one of nurses’ most difficult tasks and provides the foundation for efficient pain management. Young children lack the verbal skills to describe their experience of pain and they also have problems in differentiating pain from other unpleasant experiences. The aim of this study was to review current research regarding pain assessment in children. A literature search was carried out in the CINAHL and Medline databases, using the search words «pain assessment», «pain measurement», «pain rating scales», «children» and «nursing» and was limited to the years 1997–2002. Twelve articles satisfied the inclusion criteria and were analysed. The results are highlighted in five areas: Assessment of pain in children; Assessment of pain in infants; Tools and scales for assessing pain; Factors influencing the nurse’s assessment of pain; pain assessment as a basis of efficient pain therapy. The results of the study point to problems in pain assessment and the importance of children’s pain being adequately assessed. Accordingly, it is of utmost importance when possible to be able to take into account the individual child’s experience of pain.

    • The shortage of specialist trained nurses in the operating theatre and psychiatric care is a problem in Swedish health care. There is a great needfor recruitment since in both areas nurses have a high average age and few students register in these specialties at university. The reason for thelow interest for these specialties is not clear. The purpose of the study was to investigate nurses’ and nurse students’ conceptions of the professionalrole of operating theatre and psychiatric care nurses. A qualitative approach with phenomenographic method was used. Twelve nurses andfour nurse students were interviewed. Three categories of conceptions within each specialty emerged. Operating theatre nurses’professional rolewas perceived as: Dependent assistant, Responsible monitor and Fragmented nurse. Psychiatric nurses’ professional role was perceived as:Empathetic agent, Conscious diplomat and Fragmented nurse. The informants had difficulties in understanding the professional role in bothfields. One conclusion is that the theoretical and clinical training in basic nurse education play an important role for choosing specialist trainingas operating theatre nurse or psychiatric nurse.

    • Background: A large part of the work a nurse conducts at a Primary Care Centre consists of counselling patients over the phone. The patients are sorted and prioritised on the basis of these conversations. Aim: The aim of this study is to describe how the patients experience the caring relationship with the nurse during phone counselling at a Primary Care Centre. Method: In-dept interviews were conducted with ten adult Swedish speaking persons who contacted a nurse at two Medical Care Centres, during a two week period. Results: The patients experienced  that they received a good caring relationship with the nurses. This meant to be treated friendly, be taken seriously and to be able to have a dialog about their problems with the nurse. They wanted to feel unique, empowered and treated as individuals. They could sometimes feel disappointed when an unbalance occurred between the expectations of the patients and the advice they received. Nurses that were stressed influenced the caring relationship in a negative way. Conclusions: Overall the patients experienced a good caring relationship with the nurses, but the nurses were not always aware of how they where perceived.

    • Syfte. Syftet med studien var att undersöka om sjuksköterskor byter barns perifera venkateter vid klinisk indikation. Bakgrund. Att sätta en perifer venkateter är en vanligt förekommande procedur på sjukhus idag men detta kan vara smärtsamt och traumatiskt för barn. Det finns gudielines för när dessa ska bytas när det gäller vuxna men inga sådana riktlinjer för barn kunde identifieras. En Cochrane review från 2010 konkluderade att den perifera venkatetern ska bytas när det finns en klinisk indikation. Detta innebär vid tromboflebit men även vid smärta, rodnad, infiltration, svullnad läckage och ocklusion. Metod. Designen var prospektiv och observerande. Dagliga observationer gjordes på sjukhuset av en av forskarna och noteringar gjordes i ett protokoll. Resultat. Trettiotre barn med sammalagt 47 perifera venkatetrar deltog och 104 observationer gjordes. Sammanlagt utvecklade 42% (14/33) av barnen komplikationer. Bland de barn som fick grad 2 och 3 bytade sjuksköterskan inte den perifera venkatetern när det fanns klinisk indikation. Konklusion. Denna studie visar att tromboflebit kan utvecklas bland barn med en perifer venkateter och att den inte bytades när det fanns klinisk indikation. Det är de professionellas ansvar att reducera smärtsamma upplevelser för barn i samband med sjukhusvistelse och ytterligare forskning kring sjuksköterskors kliniska beslut behöver göras.

    • To be safe is a basic human need. Although feelings of being unsafe has been identified as a common problem among persons who live with heart failure there are few qualitative studies focusing on the problem. The aim of this study was to describe what it means to be safe or unsafe for men with heart failure. Seven men with heart failure, functional class NYHA II-III, were interviewed in focus groups. The interviews were analysed with content analysis. Threat of sudden death was described as both a source of safety and unsafety, and living with a fear of drowning in their own body fluid was seen as a source of unsafety for the men. In meetings with caregivers and relatives it was of importance to be received as a unique and valuable person. The confidence to the heart specialist was almost supernatural and the specialist nurse was spoken about as valuable for the feeling of being safe. In conclusion, to be seen with respect by physicians and nurses who have good ability to listen to the men's experience of living with heart failure can probably increase the men's feelings of safety. Further, it is important that physicians and nurses have good knowledge about heart failure.

  • The workload of caring in anaesthesia care : test of validity and reliability

    • Purpose: The purpose of this study was to test the reliability and validity of an existing patient classification instrument in anaesthetic nursing in a new context.Background: A patient classification system has to be used by many nurses, so consistency in how it is used is important.Methods: The study was divided into three parts. The first part was intended to measure interreliability using double assessments of 150 anaesthesia cases. In the second part, the anaesthetic nurses carried out the workload and, in part three, content validity was examined using a questionnaire and meetings.Results: The result showed good to very good consistency across the board in the nurses' assessments, which also included an overall assessment of the workload (r 0,85). The workload showed the highest mean care level for the indicator preparations/surgical position and the lowest for respiration. The questionnaire replies showed that the nurses were positive about workload, but 43.3 per cent thought that the instrument only described part of their work.Conclusion: The instrument was shown to be of use in different contexts, but the reliability and validation process should continue so as to increase reliability. Clarifying the progression between the different care levels for each indicator in the instrument can be a way of increasing its usefulness in different services. In terms of clinical work, the measurement of workload has started a valuable process of reflection on anaesthetic nursing that helps to enable the nurses to show what they do and why.

    • Previous research found that spirituality seems difficult to give meaning and to work with in caring. The aim of this interview study was to explore how the nursing staff experienced the spiritual dimension within caring. Data was obtained from ten nursing staff at a Swedish infection department and analyzed with content analyze. The result shows that conditions to discover and to meet spirituality depend on the staff’s ripeness, that self-knowledge affect consciousness about spirituality and in the relationship of care spirituality is visible. The meaning of spirituality is to relate to God and other people, seeking the meaning of life and attitude at fateful occasions. This study shows there is a lot of knowledge released about patients’ spiritual needs only by asking: what do you believe in? The knowledge from this study is usable booth in nursing and in educational settings.

    • The aim of the study was to find areas that were important to highlight when collecting data for the nursing history and the nursing status of the gay male patient, with an emphasis on psychosocial issues. Literature was searched in the data-bases Psyc-Lit and CINAHL, and the results were presented using the VIPS model (Ehrenberg, Ehnfors, Thorell-Ekstrand 1996). Areas that need to be out in focus were: the coming out process, sexual behaviour and safer sex, lifestyle and significant other(s). To discuss issues related to sexuality is difficult both to patients and nurses, but could be relieved using the PLISSIT model (Annon 1977). When informing on safer sex, one must bear in mind the lifestyles like »the chosen family« (Henriksson 1995) and open relations. But also the »symbolic meaning« given to the unprotected anal intercourse by some, and where the cessation of the use of condoms symbolises trust and love, and that the relationship is for real-»risk factor love« (Henriksson 1995, Linder 1996).

    • Increasingly people use the Internet to find health-related information. However, to obtain information of good quality there may be obstacles. The aim of this literature review was to gain knowledge about use of the Internet in developing health related knowledge. Two questions were posted: What factors can be obstacles in understanding health-related information on the Internet? What effects can health related information obtained from the Internet have for the user? In a systematic search in CINAHL and Pubmed, and a systematic review of two specific journals from the years 2000 to 2004, 14 scientific articles were found. Use of the Internet is related to the individual’s age, socio-economical factors, ethnicity, and health. Information obtained from the Internet may influence the patient-caregiver relation, as well as the individual’s sense of security and independence. There are learning needs for the individual in how to use the Internet and needs for improvement of the search-engines and websites

    • Aim: To evaluate quality of websites containing health information about heart attack in the Swedish language, accessible via one common used search engine available to the general public. Background: Increasingly, information about different health problems is sought on the Internet. From a layman perspective, such information can be hard to find, to understand and to trust. Health care professionals should be able to inform and teach the individual safe ways of finding health information of good quality on the Internet, which involves assessments of the websites. Methods: In one common used search engine, the Google, one search was performed during February 2006. Seventy websites were evaluated using one or all the European Union (EU) quality criteria. Findings: No website met all the criteria, but websites found on the search engines first search pages, met the criteria in the best way. Those websites were primarily aimed at mediating health information to the general public provided by government authorities, county councils, universities and companies. Conclusions: Ethnic minority groups, and individuals with low literacy skills/or visual impairments, may have difficulties finding suitable information. The EU quality criteria need to be refined in order to better assess the quality on different kind of websites.

    • Four key concepts for good nursing care and a list of key words for nursing documentation in patient records were established and to some extent tried in clinical practice in Sweden. The method consisted of the following steps: extensive literature review, review of nursing records, development of a list of key words on two levels, a first level corresponding to the nursing process, and a second level consisting of subdivisions for possible use in practice, use and assessment in clinical practice by nurses and students, expert panel judgement and refinement of the key words including an examination of semantic accuracy of the Swedish key words by an expert in Nordic languages. The proposed key words are presented both in English and Swedish and explanations, comments and references are given. The version of key words presented here is subject to further testing for possible modifications.

Traditionally, the outlook on illness and health is pathogenic, i.e. preventing illness and disease in order to achieve health, by focusing lifestyle behaviors such as physical activity and smoking. Lifestyle based on a salutary perspective cannot be measured or observed from the outside but must be described by the individual since lifestyle is an aspect of one’s life situation. Little is known about how lifestyle is perceived and what it means in ordinary people’s lives. Data were collected through interviews with 25 women selected by strategic sampling and analyzed using a phenomenographic approach. Three structural aspects emerged: enjoying life, living life, and balancing everyday life. Women described how they used lifestyle as a tool for achieving a sense of satisfaction, relaxation and happiness in everyday life. Lifestyle was also perceived as a never-ending balancing act between different personal needs and the expectations of others and society. It involved how the women used their time or lack of time as effectively as possible in order to organize their day. Lifestyle seems to be a tool in which intrinsic behaviors may be central to enhance quality of life and health. Intrinsic behaviors should be encouraged in salutary health promotion work.

    • Background: Pain is a symptom that affects quality of life negatively for many people. As a person gets older, the symptoms will increase and more than 50 % of the elderly in the municipal health care is expected to have some sort of pain. This makes pain management a key question.Aim: The aim of this study was to describe what nurses perceive affect pain management for older people.Method: The study is based on interviews with 18 nurses from two municipalities in Sweden. The method used was qualitative content analysis.Results: Four categories affecting pain management were revealed: Proficiency, communication, organization and treatment strategies. Within each category three to four sub-categories were formed, such as medicine and older, documentation, work environment and pharmacological treatment. Several variations were revealed within each sub- categories. The results showed possibilities for good pain relief but the lack of tailored pain assessment tools, communication and treatment strategies tailored for older patients in pain made it difficult. The emerging theme was an unstructured organization makes it difficult to provide good pain relief.Conclusion: If evidence-based pain management strategies including pain assessment tools adapted in municipal health were developed, more patients should receive adequate pain treatment.

    • The study aimed a) to describe existing models in clinical nursing supervision in Sweden and b) to obtain a deeper understanding of the paradigms governing different supervision models. All University Colleges of Nursing and Health Sciences in Sweden were mailed a questionnaire concerning different aspects of clinical nursing supervision. Of the one third (n=13) of the Colleges who had some kind of educational program in nursing supervision, five different models of supervision were identified. The Colleges educational programs in nursing supervision were used as a base to select at random four clinical nursing supervisors, who then were interviewed. The interviews were analysed using a paradigm interpreting method resulting in a chart of the paradigm components held by the clinical nursing supervisors. The results showed that although the supervision programmes of the 13 colleges were of different direction, length and content, there were some pervading components in all programmes. The Colleges showed poor knowledge about each others programs. Analysis of the interviews with the clinical nursing supervisors showed they had difficulties in articulating the theoretical frame of their supervision, whereas the practical aspects were easier to express. The rituals around the supervision sessions seemed more important than the theoretical frame. Another interpretation were that the supervisors personal paradigm also influenced the way the supervision was carried out.

    • The purpose of this article is to show the application of Giorgi's descriptive phenomenological human scientific method. A brief description is used in order to provide an example of the human scientific process, from data collection to data analysis. First, the data collection is presented, usingthephenomenon of frustration as an example. Some of the sufficient conditions for data gathering is also discussed in this section. Second, the data analysis is explicated. More specifically, the brief example of the phenomenon of frustration was taken through the four steps of Giorgi's human scientific method. The data analysis section accounts for the descriptive nature ofthe method as well as the critical attitude ofthe researcher. In addition, this section also points to the contrast between descriptive phenomenological human science and interpretative based qualitative methods. A brief description of the relation between constituents and structure closes this section. Overall, the implicit purpose of providing this article of the application of Giorgis's method, in the Swedish language, is to make his method more accessible to the graduate as well as to the undergraduate student in the Nordic countries.

    • The purpose of the article is to provide a Swedish account of Amedeo Giorgi's descriptive phenomenological human scientific method as a necessary clarification for the Scandinavian researcher and student interested in using a descriptive qualitative method in their health science related research. First, key terms in phenomenological philosophy are presented. Terms such as intentionality, the phenomenological reduction, description, and essence are described in order to establish an introduction to the philosophical, foundation. Second, Giorgi's human scientific method is outlined, i.e., from data gathering to data analysis. The data gathering section includes some aspects of the process of interviewing. In the section on data analysis, the methodical steps are presented successively. The researcher, using the phenomenological reduction, I) reads the data to get a sense of the whole 2) divides the interview into meaning units 3) describes the phenomenological human scientific meaning 4) discovers the structure of the phenomenon. The overview is meant as an introduction to a forthcoming article articulating the application of the phenomenological method.

    • The purpose of this article is to outline a phenomenological pedagogical method for teaching nurses and caregivers as well as university and college students in the field of nursing science how to increase their own sensitivity to their reflective processes and how it relates to their empathic abilities. The pedagogical method has its theoretical foundation in Husserlian phenomenology. The phenomenological tradition provides the pedagogical method with a systematic and rigorous approach toward issues such as subjectivity, reflection, intersubjectivity, empathy, and meaning. Practical training procedures where based on a classical Rogerian design, although altered in order to be more general and thus to apply to most occupations in which presence to another person's world is valued as part of the human service profession. The method consists of three steps that are on a pedagogical level interrelated. By introducing the relation between empathy and intersubjectivity as well as reflection and subjectivity in a practical, training situation, professionals and students in nursing science are able to enhance their own understanding of human meaning. Plausible benefits for caretakers include feelings of being understood and experiences of a sense of self-worth and well being.
