  • Influence of affective personality on cognition-mediated emotional processing : Need for empowerment

    • Background and Objectives: The notion that both positive and negative affect contribute not only to affective personality profiles but also to self-report estimations of both psychological and physical well-being or discomfort. Method: Two studies are described, based upon applications of psychometric and clinical instruments, including the Positive Affective and Negative Affect Scale (PANAS), Stress and Energy (SE), Dispositional optimism (LOT), Locus of Control, Beck Depression Inventory (BDI), Perfectionism, Hospital Anxirty and Depression (HAD) and the Uppsala Sleep Inventory (USI). In Study 1, one hundred and twenty-two high school students (Gothenburg, Sweden), aged 17-19 years participated, whereas in Study 2, two hundred and eight professional office employees from a number of government agencies and corporations (Borås, Sweden), aged 27-61 years participated. Measures for HAD-anxiety and HAD-depression from five other studies of affective personality profiles were included also. Results: The results indicated marked differences in both psychological and physical well-being or discomfort due to affective personality profiles, whereby "Self-fulfilling" individuals, expressing high positive affect and low negative affect, presented markedly better health profiles than the other three types of affective personality, i.e. "Low affective", "High affective" and "Self-destructive". Conclusions: In this regard the notion of "empowerment" as a vehicle for putative reinforcements of cognitive abilities that modulate emotional systems is discussed.

  • Working draft : Classifications of interventions in mental health care. An expert review

    • Background and objectives: Specific classifications of mental health interventions have encountered many issues in their integration into a general classification of interventions. Nonetheless, there has not been any previous review on the content and structure of current classifications in relation to mental health care. This expert review aimed to compare the mental health interventions provided in a series of reference classification systems for the incorporation of mental health care into the International Classification of Health Interventions (ICHI).Methods: Twelve classifications are described with regards to the structure of the classification (unit of analysis, sections, multiaxiality, granularity) and context of utilization (purpose, descriptors, neutrality, interoperability and implementation).Results: Major problems identified include a granularity unbalance (i.e. differences in the number of codes and its specificity with other areas such as rehabilitation), unclear units of analysis (i.e. differences between procedures, interventions, packages of care and care programs), lack of clearly stated evidence-based interventions in a mental health context; and lack of a well-defined taxonomical tree. An ontology approach to the definition of the different entities involved in the throughput of mental care, including their hierarchical relationships and conceptual map, may have contributed to the failure of previous systems together with the development of systems to classify mental health interventions separate from generic health interventions.Conclusions: The present review provides additional ground for the development of the ICHI knowledge-base and highlights the importance of taxonomical disambiguation and international comparability in the development and implementation of classifications of mental care interventions.

  • Digitally excluded in a highly digitalized country: An investigation of Swedish outpatients with psychotic disorders and functional impairments

    • Access to digital devices and digital services increases accessibility to mental health services. We investigated access to smartphones and digital identification methods (digital-IDs) in an outpatient unit focusing on patients with psychotic disorders and functional impairments. Patients’ case managers completed an online anonymous survey. Most patients (85%) did not have digitalIDs, which is required in Sweden to access digital health care. High age and living in assisted living facilities influenced patients' access to smartphones and digitalID negatively. Even in a highly digitalized society, outpatients with psychotic disorders and functional impairments have much less access to digital technology than the population on average.

  • The association between pain and psychiatric conditions in adolescents: The impact of gender and ADHD diagnosis

    • Background and objectives: The relationship between pain and psychiatric conditions in children and adolescents has been understudied. This study aimed to investigate the influence of gender on the association between pain and psychiatric diagnoses, as well as the specific relationship between pain and attention deficit/hyperactivity disorder (ADHD), in a sample of adolescents from the general population. Additionally, the study explored whether pain frequency or intensity in individuals with ADHD was influenced by coexisting psychiatric disorders and ADHD medications.Methods: This cross-sectional study included 1608 conveniently sampled Swedish upper secondary school students aged 15-19 years. Data were collected at the end of 2020 using the electronic "Mental and Somatic Health without borders" survey.Results: A significant positive association (p < 0.001) was observed between pain frequency, intensity, and the presence of any psychiatric diagnosis. Female adolescents reported more frequent and intensive pain in the groups with or without any psychiatric diagnosis and in those with ADHD, however the presence of a psychiatric diagnosis had a comparatively lesser impact on pain frequency in females when compared to males. In adolescents with ADHD, pain frequency, but not intensity, showed a significant further increase. Moreover, the presence of coexisting depression and/or anxiety further heightened the association between pain frequency and ADHD. Notably, common ADHD medications did not have a significant impact on pain experiences. Headache emerged as the most prevalent type of pain across all groups of adolescents. Back pain specifically appeared as the next most common type of pain among adolescents with ADHD.Conclusion: The findings suggest a positive association between pain and the presence of psychiatric diagnoses, including ADHD, in adolescents. Gender modified this association. Back pain arised specifically coupled to ADHD. Common ADHD medications did not show a significant impact on pain experiences in this study. These results highlight the importance of a holistic approach to child and adolescent care.(c) 2023 The Author(s).     • The present study examined 61 men at forensic psychiatric investigation. We compared subjects with or without an assessed previous conduct disorder (CD versus non CD). The CD group showed more DSM-HI-R personality disorders, exhibited more repeated violent criminality and more mixed drug abuse. The CD group differed from the non CD group according to specific personality traits assessed by the Karolinska Scales of Personality (KSP) showing lower scores on the Social desirability and Socialization scale, and higher scores on the Impulsiveness, Monotony avoidance, Verbal aggression, Iritability and Suspicion scales. The CD group had higher psychopathy scores assessed by the Psychopathy Checklist-Revised (PCL-R). The results are in accordance with previous findings in juvenile conduct disordered men. They indicate that personality traits are detectable early in life and stable over time influencing behaviours such as criminality and drug abuse.

    • he present study evaluates the relatioships between the thyroid hormones, triiodothyronine (T3) and thyroxine (FT4), in adult life and a history of conduct disorder in 61 men from a forensic psychiatric population. The study population was divided into four groups based on the presence or not of an assessed conduct disorder, as well as a previous child psychiatric contact. The highest T3 values were found in patients with both a previous conduct disorder and a previous child psychiatric contact. There was a positive relationship between conduct disorder and serum levels of FT4. These results may indicate that T3 is a biological marker of aggressive behaviour in children.

    • Background and Objectives: Mental health problems are common in Primary Health care and clinicians require valid and reliable instruments to make good treatment plans for these patients. The Outcome Questionnaire-45 (OQ-45) was developed by Lambert and colleagues in order to help improve outcomes of treatment. The aims of this study were to examine the psychometric properties of the Swedish version of OQ-45 when administered by an automated technique and to evaluate if the OQ-45 uniquely could contribute to the description of the patients' difficulties and needs beyond demographic characteristics and other instruments. Methods: The study comprised 816 patients with mental ill-health taking part in a large randomized controlled trial, Regassa. The OQ-45 data were collected by Interactive Voice Response IVR, a computerized, automated telephone technique. The OQ-45 consists of 45 items summarized in a total score and in three subscales; Symptoms of distress, SD, Interpersonal relations, IR, and Social role functioning, SR. Depression was measured by MADRS and health-related quality of life by EQ-5D. Results: The OQ-45 total score showed good psychometric properties, but there was little support for its three factor structure. The OQ-45 significantly predicted level of depression and health-related quality of life in patients. Conclusions: The OQ-45 contributed uniquely to the description of the patients problems. The results indicate that the total score of the Swedish version of OQ-45 can be valuable to use for clinicians in the field.

    • Acceptance, attrition and outcome of the same outpatient treatment programme for alcoholics was studied first in a non-randomized and later in a randomized study. There was a lower (15%) rate of acceptance and a higher (19%) rate of attrition in the randomized study. Among treatment completers drinking outcome during the third year after commencement of treatment did not differ between the non-randomized and the randomized study. It is concluded that the characteristics of subjects accepting a randomly assigned study should always be clearly presented. The absence of a patient-therapist matching in a randomized design could negatively effect attrition and probably also acceptance of treatment, while it seems to be less influential on outcome in those completing therapy.
