    • The depolymerisation of softwood kraft lignin was investigated, under hydrothermal conditions at 290 °C and 250 bar, with guaiacol in the reactor feed to evaluate its impact on the formation of char and on the molecular weights of the products. The effect of residence time was investigated in the time span 1-12 min. Lignin is depolymerised during the process and guaiacol is both formed and consumed during the reaction, with clearly noticeable changes as early as in the first minute of reaction. Although the addition of guaiacol in the reactor feed causes a reduction in the weight average molecular weight of the products, the yield of char increases. Longer residence times result in repolymerisation of the reaction products as well as a further increase in the yield of monoaromatic components and char.

  • Self‐reported documentation of goals and outcomes of nutrition care : A cross‐sectional survey study of Scandinavian dietitians

    • BackgroundThe documentation of goals and outcomes of nutrition care in Electronic Health Records is insufficient making further exploration of this of particular interest. Identifying common features in documentation practice among Scandinavian dietitians might provide information that can support improvement in this area.AimsTo explore the associations between clinical dietitians' self-reported documentation of patients' goals and outcomes and demographic factors, self-reported implementation of the systematic framework the Nutrition Care Process 4th step (NCP) and its associated terminology, and factors associated with the workplace.MethodsData from a cross-sectional study based on a previously tested web-based survey (INIS) disseminated in 2017 to dietitians in Scandinavia (n = 494) was used. Respondents were recruited through e-mail lists, e-newsletters and social media groups for dietitians. Associations between countries regarding the reported documentation of goals and outcomes, implementation levels of the NCP 4th step, demographic information and factors associated with the workplace were measured through Chi-square test. Associations between dependent- and independent variables were measured through logistic regression analysis.ResultsClinically practicing dietitians (n = 347) working in Scandinavia, Sweden (n = 249), Norway (n = 60), Denmark (n = 38), who had completed dietetic education participated. The reported documentation of goals and outcomes from nutrition intervention was highly associated with the reported implementation of NCP 4th step terminology (OR = 5.26; p = 0.009, OR = 3.56; p = 0.003), support from the workplace (OR = 4.0, p < 0.001, OR = 8.89, p < 0.001) and area of practice (OR = 2.02, p = 0.017). Years since completed dietetic training and educational level did not have any significant associations with documentation practice regarding goals and outcomes.ConclusionFindings highlight strong associations between the implementation of the NCP 4th step terminology and the documentation of goals and outcomes. Strategies to support dietitians in using standardized terminology and the development of tools for comprehensive documentation of evaluation of goals and outcome are required.

  • Fatigue after heart transplantation - a possible barrier to self-efficacy

    • Rationale Recovery after heart transplantation is challenging and many heart recipients struggle with various transplant-related symptoms, side-effects of immunosuppressive medications and mental challenges. Fatigue has been reported to be one of the most common and distressing symptoms after heart transplantation and might therefore constitute a barrier to self-efficacy, which acts as a moderator of self-management. Aim To explore the prevalence of fatigue and its relationship to self-efficacy among heart recipients 1-5 years after transplantation. Research method An explorative cross-sectional design, including 79 heart recipients due for follow-up 1-5 years after transplantation. Three different self-assessment instruments were employed; The Multidimensional Fatigue Inventory-19, Self-efficacy for managing chronic disease 6-Item Scale and The Postoperative Recovery Profile. Ethical approval The study was approved by the Regional Ethics Board of Lund (Dnr. 2014/670-14/10) with supplementary approval from the Swedish Ethical Review Authority (Dnr. 2019-02769). Results The reported levels of fatigue for the whole group were moderate in all dimensions of the Multidimensional Fatigue Inventory-19, with highest ratings in the General Fatigue sub-scale. Those most fatigued were the groups younger than 50 years; pretransplant treatment with Mechanical Circulatory Support; not recovered or had not returned to work. Self-efficacy was associated with the sub-dimensions Mental Fatigue (rho = -0 center dot.649) and Reduced Motivation (rho = -0 center dot 617), which explained 40 center dot 1% of the variance when controlled for age and gender. Study limitations The small sample size constitutes a limitation. Conclusions The moderate levels of fatigue reported indicate that it is not a widespread problem. However, for those suffering from severe fatigue it is a troublesome symptom that affects the recovery process and their ability to return to work. Efforts should be made to identify those troubled by fatigue to enable sufficient self-management support.

    • IntroductionFirst-line managers in Swedish communities have responsibility for that care to older people is provided, staffing is sufficient and the budget is balanced in their unit. It is a struggle with limited resources due to a growing population in need of care. This can lead to missed nursing care. The aim was to describe first-line managers perceptions of missed nursing care in community health care for older people.MethodsA qualitative design with a phenomenographic approach, interviewing 24 first-line managers. Ethics approval for the study was received from the Research Ethics Committee at Karlstad University (Dnr HNT 2020/566).ResultsThe results are shown in six descriptive categories containing 15 perceptions. The descriptive categories are 'occurrence of missed nursing care', 'becoming aware of missed nursing care', 'reasons for missed nursing care', 'missed nursing care has consequences for the older persons', 'missed nursing care has consequences for the staff' and 'taking action to decrease missed nursing care'.ConclusionsIt is important for first-line managers to become aware of the existence and reasons for missed nursing, as it has consequences for older people and staff. Managers need to take missed nursing care seriously in order to work with improvements for maintaining good quality of care and patient safety.

    • Background All professions in surgical care have a responsibility to include patients in their health care. By Swedish law, all care should be done in dialogue with the patient. The essential part of health care is the meeting between patient and healthcare professional. In the interaction, a decision can be made, and needs can be identified to a safer care. Previous studies on patient participation have focussed on patients perspectives in surgical care, but there is a paucity of studies about the personnels perspective of estimated patient involvement in surgical care. Aim The aim of this study was to identify and describe healthcare personnels view and behaviour regarding patient involvement in surgical care. Method A quantitative study with various professions was conducted. A validated questionnaire was used, remaining questions grouped under following areas: patient involvement, acute phase, hospital time, discharge phase and questions on employment and workplace. Results A total of 140 questionnaires were sent out to a surgical clinic in Sweden, and 102 questionnaires were answered. All professionals stated that clear information is an important part of patient involvement in surgical care. Statistically significant differences existed between the professions in the subscale information. Physicians rated their information higher than the Registered Nurses (p = 0.005) and the practical nurses did (p = 0.001). Hindrances to involving patients were lack of time and other priority tasks. Conclusions Professionals in surgical care graded information to be the most important thing for patient involvement. Participation in important decisions, including the possibility to express personal views and ask questions, is important factors for patient involvement. Barriers against patient involvement are lack of time and prioritisation of other work activities.

    • ObjectiveTo report data from a point pressure ulcer (PU) prevalence survey on prevalence, PU categories, locations and preventive interventions at one Norwegian nursing home.MethodsA cross-sectional research design was used. One nursing home in Norway participated in the prevalence survey in 2020. The data were collected on one selected day. A total of 74 out of 88 residents (84.1%) participated. Descriptive statistical analyses were run.ResultsThe overall prevalence of PUs was 27% amongst all participants in the nursing home, who together had a total of 57 PUs categorised as category I–III. One major finding was that the most common site of the PUs was on the residents' toes. Interestingly, the prevalence of PUs in the residents' sacrum was considerably low. The most frequently used PU preventive interventions were foam chair cushions, nutritional supplements and pressure-reducing heel protection.ConclusionThis study identified a high prevalence of PUs, predominantly on residents' toes. Although preventive strategies were implemented, their application appeared limited. Implementing obligatory care packages and annual nationwide PU surveys might be worth considering in municipalities.

    • Background: The design of the physical environment is a critical factor in patient care and is known to influence health, well-being, clinical efficiency, and health-related outcomes. To date, there has been no general review of the physical environment of modern Swedish stroke units.Aim: To explore the physical environment of inpatient stroke units in Sweden and describe the design and structure of these units. Methods: This was a cross-sectional study. Data were collected in Sweden from April to July 2021 via a survey questionnaire.Results: The layout of the stroke units varied broadly, such as the number of single-bed and multi-bed rooms. More than half the stroke units comprised spaces for rehabilitation and had an enriched environment in the form of communal areas with access to computers, games, books, newspapers, and meeting places. However, they offered sparse access to plants and/or scenery.Conclusions: Healthcare environments are an essential component of a sustainable community. From a sustainability perspective, healthcare facilities must be built with high architectural quality and from a long-term perspective. Research on the physical environment in healthcare should contribute to improved quality of care, which can be achieved through building healthcare facilities that support the performance of care and recovery. Therefore, mapping of areas of interest for further investigation is crucial.

  • The Professional Nurse Self-Assessment Scale II - Translation and cultural adaptation for Nordic countries

    • BACKGROUND: It is important to map the clinical competence of newly graduated nurses in Nordic countries. The use of a common Nordic instrument could provide insights into nurses' levels of self-assessed clinical competence and perceptions of their need for professional development.AIM: To translate and culturally adapt the original Norwegian version of the Professional Nurse Self-Assessment Scale II (PROFFNurse SAS II) into (1) Danish, (2) Finnish and (3) Icelandic versions.METHOD: The PROFFNurse SAS II was translated and cross-culturally adapted. This translation was inspired by the process used in the Guidelines for Cross-Cultural Adaptation.RESULT: The translation and cultural adaptation processes employed the required steps and provided specific details. In addition, practical issues encountered during the translation process while translating and adapting instruments that may influence future translations were revealed. This study found that having a professional bilingual/bicultural agency translator was partly problematic in the process of translation and found that it is important to adjust the translations to each country's specific words used in nursing.CONCLUSION: Translating the PROFFNurse SAS II instrument into all Nordic languages enables us to use the instrument from a Nordic perspective and across various countries. This is important when comparing self-awareness and reflecting on nurses' clinical competencies. Professional development is central to valuing and developing clinical competence and allowing for the discovery of gaps in clinical competence.

    • Background In Sweden, antenatal and child health care are offered free of charge to all expectant and new parents. Professionals in antenatal and child health care play an important role in supporting parents. Previous research shows that same-sex mothers face heteronormative assumptions and insufficient support during their transition to parenthood. Objective To explore professionals' experiences of supporting two-mother families in antenatal and child health care. Method A qualitative method with focus group discussions was used. An interview guide was followed, and the discussions were held online. The data was analysed according to inductive content analysis. Settings and participants The participants were midwives (n = 8) and nurses (n = 5) in antenatal and child health care from different parts of Sweden. Participants were recruited through the coordinating midwives and child health care nurses in the different regions. Findings One main category was identified: Striving to be open-minded in supporting same-sex mothers. Health care professionals described meeting well-prepared mothers, with an equal commitment between each other, and mothers on guard against heteronormative views. Professionals provided support through empowerment by creating a safe environment and aiming at providing equal support to all parents or tailored support to same-sex mothers. Mothers described handling challenges, as a balancing act to acknowledge both mothers. Struggling with documents and communication and a lack of information were other challenges to be handled. Professionals reflected on their own professional competence and expressed that knowledge acquired through education, experience and personal interest all contributed to their competence. Conclusions Forms and documentation need to be updated to be gender neutral to be including to a variety of family constelleations. Health care professionals need time to reflect on norms and challenges to better support both mothers in a two-mother family.

    • Cellulose nanofibrils (CNF) are mixed with plasticizers; sorbitol and glycerol, through high-pressure homogenization to prepare multifunctional biohybrid films. The resulting plasticized films obtained after solvent evaporation are strong, flexible and demonstrate superior toughness and optical transparency. The oxygen barrier properties of the biohybrid films outperform commercial packaging materials. The sorbitol-plasticized CNF films possess excellent oxygen barrier properties, 0.34 cm3·μm/m2·day·kPa at 50 % relative humidity, while significantly enhancing the toughness and fracture strength of the films. CNF films plasticized by 20 wt.% of sorbitol and glycerol could before rupture, be strained to about 9 % and 12 %, respectively. The toughness of the plasticized films increased by ca. 300 % compared to the pristine CNF film. Furthermore, the water vapor barrier properties of the biohybrid films were also preserved by the addition of sorbitol. CNF films plasticized with sorbitol was demonstrated to simultaneously enhance fracture toughness, work of fracture, softening behavior while preserving gas barrier properties. Highly favorable thermomechanical characteristics were found with CNF/sorbitol combinations and motivate further work on this material system, for instance as a thermoformable matrix in biocomposite materials. The unique combination of excellent oxygen barrier behavior, formability and optical transparency suggest the potential of these CNF-based films as an alternative in flexible packaging of oxygen sensitive devices like thin-film transistors or organic light-emitting diode displays, gas storage applications and as barrier coatings/laminations in packaging applications, including free-standing films as aluminium-replacement in liquid board and primary packaging, as replacement for polyethylene (PE) in wrapping paper, e. g. sweats and confectionary.

  • Macroprudential policy in the EU : A political economy perspective

    • In response to the lessons of the global financial crisis, macroprudential policy is now firmly established as a financial policy area to prevent excessive risk taking in the financial sector and mitigate its effects on the real economy. However, macroprudential policy is facing several challenges relating to its political sensitivity and institutional context. These include political and interest group resistance, weaknesses in the governance framework, and limited institutional memory among policy makers. This article seeks to contribute to the contextual understanding of macroprudential policy by exploring how factors relating to these challenges influence policy in the EU. More specifically, it develops and empirically tests a number of hypotheses on how wider institutional and structural factors influence the actual use of macroprudential measures across Europe. The findings yield considerable support for theoretical predictions that institutions and contexts matter - Political pressure and interest group resistance tend to influence the intensity of macroprudential policy stances. Weaker policy stances characterise countries where banking systems depend on domestic banks, whereas the opposite holds for financial systems with significant market shares of other financial intermediaries. Results on institutional arrangements show that governance arrangements on relating to transparency influence policy stances. The results also indicate that policy makers' inertia is best counteracted by appointing a single macroprudential authority. The results differ somewhat depending on whether countries are based in the Euro zone or not. This suggests that policy frameworks that are multi-layered and complex pose tricky conundrums on how to ensure sufficient institutional autonomy, policy capacity and discretion among macroprudential authorities. This also has implications on other policy areas macroprudential policy, such as microprudential policy, crisis management and fiscal policy. © 2019 Elsevier Inc.

  • Modeling of tensile index using uncertain data sets

    • The objective of this investigation is to analyze and model tensile index. Two approaches are used, one based on training and validation data, while the other novel approach tests models using all possible combinations of data points. This approach is focused on small data sets which have here been obtained from nineteen pulp samples at different refining conditions in a full-scale TMP production line with a CD-76 refiner as a primary stage. From each pulp sample twenty handsheet strips for tensile index measurements were performed. Initially, specific energy and the external variables (dilution water feed rates and plate gaps) are used as predictors in a modeling approach based on an adjusted R 2 {R^{2}} approach. Thereafter, the resulting models are compared with a combination of specific energy and internal variables (primarily consistencies) obtained from temperature measurements inside the refining zones using a soft sensor concept. It is found that specific energy and internal variables as predictors outperform the external variables when estimating tensile index.

    • The goal of this paper is to model the pulp properties fiber length, shives width and freeness. This will be done utilizing specific energy, flat zone inlet consistency and the internal variables, consistencies and fiber residence times estimated from refining zone soft sensors. The models are designed using more than 3600 hours of data from a RGP82CD refiner. The pulp properties are sampled using a measurement device positioned after the latency chest. Such measurements are noisy and irregularly sampled which opens for a number of challenges to overcome in modeling procedures. In this paper it is shown that the models for shives width and fiber length are capable of predicting most of the major dynamics. However, for freeness no reliable linear models can be derived. When estimating fiber length, the specific energy together with flat zone inlet consistency, fiber residence times and the consistency in the conical zone were the dominant inputs. For shives width it was found that a similar set of inputs resulted in the best models, except that the consistencies during normal process conditions did not significantly influence shives width. Furthermore, fiber residence times were shown to have considerably more pronounced impact on fiber length compared with shives width estimates.

    • Chemithermomechanical pulp (CTMP) is often used in central layers of multiply paperboards due to its high bulk and strength. Such a CTMP should consist of well-separated undamaged fibres with sufficient bonding capacity. The basic objective of this work is to optimize process conditions in low-consistency (LC) refining, i. e. to select or ultimately develop new optimal LC refiner filling patterns, in order to produce fibrillar fines and improve the separation of fibres from each other while preserving the natural fibre morphology as much as possible. Furthermore, the aim is to evaluate if this type of work can be done at laboratory-scale or if it is necessary to run trials in pilot- or mill-scale in order to get relevant answers. First stage CTMP made from Norway spruce (Picea abies) was LC refined in mill-, pilot- and laboratory-scale trials and with different filling patterns. The results show that an LR1 laboratory refiner can favourably be used instead of larger refiners in order to characterize CTMP with regard to tensile index and z-strength versus bulk. A fine filling pattern resulted in CTMP with higher tensile index, z-strength and energy efficiency at maintained bulk compared to a standard filling pattern.

Such a CTMP should consist of well-separated undamaged fibres with sufficient bonding capacity. The basic objective of this work is to examine the effect of refining on bulk, taking into account conditions such as temperature, sulphonation, refining gap and refiner size. First stage CTMP made from Norway spruce (Picea abies) were produced in pilot and mill scale trials. Two new parameters, Equivalent temperature related to softness and Refining gentleness are introduced that take into account refining conditions as actual temperature, softening temperature, bound sulphonate content, refining gap and refiner diameter. The results show that bulk increases linearly with refining gentleness.

    • In this article, Katie Eriksson's theory of caritative caring ethics and the theory of evidence, are described. Both theories are anchored in caritas, that is love, mercy and compassion. The theory of caritative caring ethics was first described by Eriksson in 1995, where seven assumptions or basic categories were elaborated. These were: the human being's dignity, the care relationship, invitation, responsibility, virtue, obligation or duty, and good and evil. Eriksson's theoretical contribution is that she makes a distinction between caring and nursing ethics, between inner and external ethics, and between natural and clinical ethics. Concerning the theory of evidence, Eriksson claims that a multidimensional scientific view of evidence in caring that focuses on the patient's world is necessary and vital. To see, realise, know, attest and revise constitute the ontological definitions of the concepts of evidence and evident. The theories are united by the core concepts of testimony and witnessing the human being's suffering. Eriksson points out that it is in the ethical acts that deeds are formed, based on ethos. The anchorage in an ethos means to have firm value-loaded judgements of an inner motive. Moreover, the anchorage in ethos presupposes a personal and natural ethic. The good deeds are realised in the relationship between the patient and the carer, but the caring ethics is not a professional or external ethics. Caring ethics is an ontological inner ethics meaning fellowship and the right to exist, but it is the patient's world and reality that decides the foundation and starting point for caritative caring ethics in clinical practice. The ultimate purpose and goal of caring are to guarantee the patient's dignity and absolute value as a human being.

    • This methodological article aims to describe three methodological strategies for using drawings as a part of qualitative data collection methods in caring research based on hermeneutics. In some research interview situations, participants may have difficulties to express their experiences and feelings in words. The consequences may be that the descriptions in research reports will become superficial and not authentic, meaning, "telling it as it is". Drawn pictures may facilitate and support reflection related to the deepening of experiences and thoughts, and communicate and express more than words can do. It may also reveal thoughts and feelings the person drawing the picture was not aware of. Three methodological strategies are described: (1) Drawing a picture as an introduction or starting point for an interview, (2) During an ongoing interview, encouraging the participant to draw a picture when further explanation or description is needed for deepening the communication and (3) Drawing something in a pre-existing picture. The theoretical foundation of Gadamer's hermeneutic philosophy is discussed in relation to what a drawing is representing and presents. The interpretation of the drawn picture depends primarily on the creator of the picture, but at the same time the interpretation and understanding is a movement between the interviewer's and the participant's horizons, and thus is open for preunderstanding and new understanding. In contrast to an ordinary interview between two parties, an interview involving a drawing adds something specific to the conversation as it becomes a "trialogue" and not only a dialog. The drawn picture stands on its own. Using the participant's drawing can, therefore, be understood as an ongoing process with three parties involved: (1) the participant, (2) the researcher and (3) the drawing.

Transitions are complicated and multi-dimensional, and to understand this process, it is crucial to identify the factors that facilitate or stand in the way of a healthy transition.Aim: The phenomenological study aimed to describe newly graduated nurses’ expectations of transitioning into the nursing profession at the start of a clinical nursing introduction programme, including education, supervision, and critical reflection with peers.Method: The study was based on seven group interviews with newly graduated nurses. The interviews utilised open-ended and follow-up questions and were carried out as a dialogue to enable reflection on the phenomenon of interest. This was explored and illuminated using the reflective lifeworld research approach, based on phenomenological epistemology.Findings: “Expectations of transition into the nursing profession viathe Clinical Nursing Introduction Programme” is signified by an oscillating movement between uncertainty, security, challenge, and growth on the threshold of a new identity. The phenomenon is constituted by the courage to grow, responsibility and fear, belonging and vulnerability, and support and challenge.Conclusion: Transitioning into the nursing profession viathe Clinical Nursing Introduction Programme means having the best of both worlds. Newly graduated nurses have the opportunity to receive education and structured support at the same time as they work independently in clinical practice.

    • The objective for this work was to investigate the possibility to use a pressure screen equipped with a micro-perforated screen basket to produce a fine fraction from bleached chemical pulp. Trials were performed with unrefined bleached chemical hardwood pulp, and with unrefined and refined bleached chemical softwood pulp. The effect of feed concentration, feed flow, and volumetric fine fraction flow was evaluated. The difference between the fine fraction (i. e. the particles passing the screen) and the feed was analysed by studying the fibre morphology. The results showed that high feed concentration was positive for both the fine fraction concentration and the separation efficiency. A higher fine fraction concentration was also obtained when using hardwood pulp, which was explained by the shorter fibre length. Refining of the pulp prior to the fractionation proved beneficial, as a larger share of the refined pulp passed the screen, resulting in a twice as high concentration of the fine fraction when compared to unrefined pulp.

    • The objective in this work was to obtain a fine fraction of kraft pulp, with as high concentration as possible, in a pilot-scale fractionation with micro-perforated screen baskets. The influence of screen basket surface, hole size, feed concentration, pulp type and refining segment design was investigated. The results showed that a smooth screen basket surface improved the fractionation efficiency of the unrefined pulp compared to a profiled screen basket, despite a larger hole size. A significantly higher fine fraction concentration was obtained when using refined hardwood pulp compared to when using softwood pulp, which was explained with its lower average fibre length and narrower and thus more flexible fibre fragments. The pilot trials also showed that the screening process could be operated at feed concentrations similar to those directly after a refiner, 30-40 g/l. This was demonstrated in a process layout with partial recirculation where a refiner and a micro-perforated screen basket were operated in series in pilot scale.

    • In food packaging, low-density polyethylene (PE) coating is applied to paperboards to act as a functional barrier and to provide the smoothness required to enhance printability. These characteristics are related to the material’s surface roughness, the parameter monitored during the manufacturing process. Measurement of surface roughness using optical profilometry has gained importance in the paper industry. The optical instruments used to measure surface roughness are limited spatially by the relationship with the light wavelength at which they operate. A scanning electron microscope (SEM) is an alternative for overcoming the spatial resolution limitation, and the use of stereo-photogrammetry on SEM images can be seen as an alternative profilometry technique to measure surface roughness. In this investigation, the surface topography of industrially manufactured high-quality PE-coated paperboard was studied, comparing the SEM stereo-photogrammetry technique with a reference profilometry method, i. e., chromatic confocal microscopy (CCM). We found close agreement between the calculated surface roughness and the results of the techniques used and compared them according to the new ISO 25178 Geometric Product Specifications. We concluded that SEM stereo-photogrammetry provides comparable accurate alternative profilometry method for characterizing the surface roughness of PE-coated paperboard in the micrometer scale.

    • Cellulose can be directly dissolved in cold alkali without derivatization. However, this requires low cellulose molecular weight, i. e. low pulp viscosity, preferably below 300 mL g−1. This can be achieved by for example acid or enzymatic hydrolysis of the dissolving pulp. However, it would be beneficial to manufacture pulp with sufficiently low viscosity without an additional treatment stage prior to dissolution. Unit processes in pulping can be operated in such a way as to reduce the molecular weight of cellulose. The approach of the study was to modify the conditions in unit pulping processes in order to obtain a low pulp viscosity of fully bleached prehydrolysis kraft pulp. A high charge of alkali in the oxygen delignification reduced the cellulose molecular weight significantly. Increased temperature, 120 °C compared to 98 °C, had also a significant effect on viscosity. By performing peroxide bleaching at acidic pH, the viscosity could be sufficiently reduced even when oxygen delignification was performed at lower temperature. However, for high brightness, a chlorine dioxide stage is needed.

Detta är mycket beklagansvärt ur vetenskaplig synvinkel, men även ur samhällets och allmänhetens perspektiv. Fornminnen och fornsaker, däribland naturligtvis vikingatida skattfynd, är vårt gemensamma kulturarv. Depåfyndet består av en samling om cirka 900 mynt och silverföremål, som enligt ägaren hittats i en gammal chiffonjé på en gård utanför Lund i Skåne. Sammansättningen visar tydligt att föremålen kommer från en vikingatida skatt som deponerades efter 1018 e.Kr., men det är okänt när, var och hur den ursprungligen upphittades.

    • BackgroundGrowing care queues, reduced access to care and cancelled surgery are realities for some patients being treated with total hip or knee replacement surgery in Sweden.Most of the patients on the waiting lists have experienced pain and limited motion for a varying period of time, with a negative effect on their everyday lives. Overbooked surgical schedules are already contributing to the lengthy waiting times, but, with the addition of cancellations, longer waiting times will increase still further and may affect patients’ well‐being.MethodsIn the present study, we aimed to illuminate the experience of having planned surgery cancelled, based on narratives from 10 participants. The interview transcriptions were analysed using a phenomenological hermeneutic approach.ResultsThe comprehensive analyses revealed that the participants described the agony of being deselected and the additional impression of being excluded. Metaphors of being damaged and feeling physical pain were used and the interpretations referred to the cancellations as unpleasant. Additionally, the important relationship and the trust between the health workers and the patient were negatively affected by the cancellation.ConclusionAfter the cancellation, the participants expressed being vulnerable and from their perspective the cancelled surgery affected them deeply; in fact, much more than the healthcare workers appeared to understand. Therefore, information around the cancellation must be given respectfully and with dignity, in a dialogue between the patient and the healthcare workers. Taken together, to enable an opportunity to be involved in the continued care. The cancellations should be seen as an interruption, in which the patients’ chance of living a pain‐free, active life is postponed.

    • BackgroundTransgender persons face barriers to health care including discrimination and lack of awareness by professionals. This study aimed to describe transgender persons’ experiences of encounters with healthcare staff.MethodsParticipants were recruited through postings in social media and on websites targeting people identifying as transgender. Through an online form, 21 people identifying as transgender provided written self‐reports. These were analysed using a method for inductive qualitative content analysis. The study was conducted in Sweden in 2018.ResultsThe results describe transgender persons’ experiences and perceptions of encounters with healthcare staff in one theme: treat me with respect, and three main categories: accept me for who I am; treat me according to my needs; and meet me with competence .ConclusionsHealthcare professionals can contribute in restoring and upholding transgender people’s trust in health care by accepting their identity and focusing on their healthcare needs while also being informed about transgender people’s needs and realities. A key point in this is recognising transgender person’s vulnerability to violations of dignity in relation to health care.

    • Several Cretaceous amber-bearing localities were recently discovered, including the Oriente Basin of Ecuador (Hollin Formation), and the Eastern Cordillera of Colombia (Une Formation at La Popa mine). A palynological study of 48 samples was carried out to date them and assess their broad floristic content. We date the Hollin Formation as spanning the late Aptian to the early Albian based on the co-occurrence of Callialasporites trilobatus, Retitricolpites operculatus and Callialasporites dampieri, among others. The sandstone lens containing amber from Une Formation at La Popa mine were dated as latest Aptian to early Cenomanian based upon the co-occurrence of Callialasporites dampieri, Afropollis jardinus, Ischyosporites variegatus and Gnetaceaepollenites retangularis. The palynoflora is dominated by high spore diversity suggesting humid climate conditions in the northwestern margin of Gondwana.

Methods: In this observational follow-up study, a combination of hospital care and HCS was compared to estimated alternative care solely at the hospital. Data on one year of healthcare utilisation for 32 children, supplied by the hospital and HCS, were collected from administrative systems. Corresponding healthcare unit prices were collected from healthcare pricelists. The human-capital approach was applied to estimate productivity losses and the value of productivity losses for 25 parents. Family characteristics, including parental work absenteeism and income, were collected by a questionnaire distributed to parents at five time points during a year. Descriptive and comparative statistics were used for analysis and carried out with ethical approval. Results: Healthcare costs for children receiving a combination of hospital care and HCS varied among children with estimated average healthcare cost savings of SEK 50101 per child compared to the alternative of care provided only in the hospital. The reduced costs were related to children receiving nonpalliative HCS care tasks. Average annual productivity losses due to parental work absenteeism were estimated at 348hours with an associated monetary value estimated at SEK 137524 per parent. Conclusion: County-based HCS, provided as complement to and substitute for hospital care for ill children, does not increase healthcare cost and should be a prioritized area when organising paediatric health care. Productivity losses vary greatly among parents and are pronounced also when children receive HCS with signs of gender-related differences.

    • This study examines the connectedness and risk spillovers between Chinese FinTech and traditional financial institutions by using quantile-based vector autoregression (QVAR) networks. Specifically, by using daily data from January 2014 to June 2022, we focus on system-, sector-, and institution-level quantile connectedness characteristics, with the following findings. At the system level, the QVAR networks linking FinTech and traditional financial institutions are more connected at the extreme quantiles than at the median quantile. At the sector level, banks, real estate firms, and FinTech sectors act as net risk receivers, whereas securities and insurers act as net risk emitters. At the institutional level, risk transmission and reception of institutions significantly increase when market conditions rapidly change. We also investigate the determinants of quantile connectedness by using an exponential random graph model and find that (i) across different quantiles, the book-to-market and return on equity of institutions have a positive impact on their risk spillovers; (ii) at the extreme quantiles, the book-to-market is more pronounced than the return on equity; and (iii) at the median quantile, banks and FinTech institutions are more connected than insurers, real estate firms, securities, and other financials.

While the recyclability rates of fiber-based packagingare high, packaging boards with high wet-strength can poseproblems due to problems with repulping. This manuscriptinvestigated the possibilities to reduce the use of polyamideepichlorohydrin resins (PAE) by supporting the processsystem with carboxy-methylated cellulose (CMC) throughmachine trials in mill scale production. The focus of theinvestigation was on the wet-strength of the board productand the repulpability value. The results given from thefull-scale trial were positive, indicating a potential in usingCMC while reducing the PAE addition. The results showedthat wet tear strength and wet tensile strength of the boardwere maintained, while higher repulpability rates weregiven, encouraging better recyclability of the board material. This will be beneficial to the environment both withlower use of non-renewable chemicals and possibilities forhigher degrees of recycling of the board products.

    • This paper compares policy outcomes under the runoff voting system and the plurality rule by exploiting the assignment of municipalities in Italy to different voting systems based on a population threshold. Mayors, who are the head of the local political power, are elected using a plurality rule in municipalities having fewer than 15,000 residents, while they are elected using a runoff system in municipalities above the threshold. Regression-discontinuity estimates show that municipalities under the runoff system spend 12 percent more than those under the plurality rule and that additional expenditures are not, financed via higher taxation.

However, there is no consensus regarding the competence and education needed by nurses working in post-anaesthesia care units.AIM: The aim of this study is to describe and compare levels of education and technical skills in registered nurses working in post-anaesthesia care units in Sweden, as well as the education that post-anaesthesia care unit nurse managers' desire for registered nurses working in post-anaesthesia care units.METHODS: This descriptive cross-sectional study was conducted in Sweden between September and December of 2019. A web-based survey was developed that included questions about the levels of education and technical skills possessed by registered nurses working in Swedish post-anaesthesia care units and desired by nurse managers for these nurses. The survey was evaluated for content validity by four experts. The survey was distributed to the nurse manager of each studied post-anaesthesia care unit. All nurse managers received written information and were informed that submitting the survey was considered as consenting to participate in the study.RESULTS: Most surveyed nurses held a postgraduate diploma in specialist nursing. Registered nurses performed many tasks autonomously; however, there was a significant difference between specialist nurses and registered nurses, with specialist nurses being more autonomous than registered nurses. Most of the nurse managers (n = 31/45) wanted the registered nurses in their units to have education in postoperative care. The relatively low overall response rate of 58% is a study limitation.CONCLUSIONS: Registered nurses working in post-anaesthesia care units in Sweden must have various technical skills. Some of these skills are only performed by specialist nurses, indicating that postoperative care is an advanced level of nursing care.

Through the process of encircling discourses, valuable insight is given into fields that otherwise would be lost or would not be available.ConclusionOur summary stance is that discourse analysis as it is presented in this article is strongly advisable for use in nursing and caring sciences.

    • Objective Continuity of care as provided by midwives promoting the health and well-being of new mothers during the postnatal period is critical; thus, access to midwifery services needs to be facilitated. The aim of this study was to describe new mothers' and midwives' experiences and perceptions of a new coordinated postnatal care intervention in a midwifery clinic. Design New mothers responded to open-ended questions in a survey, and midwives were interviewed individually about the intervention. A deductive content analysis research related to continuity of care concepts was used. Setting and participants The study was carried out at a midwifery clinic in a larger city in Sweden, for an eight-month period in 2019 and 2020. Two hundred and sixteen answers from new mothers and nine interviews with midwives were analysed. Intervention All registered pregnant women at the midwifery clinic received enhanced postnatal support based on a new coordinated postnatal care model. The focus was on continuity of care, from pregnancy to the postnatal period and included planning for the first weeks after childbirth at the end of pregnancy, early postnatal contact and several visits to the midwifery clinic. Findings New mothers describing the coordinated postnatal care model highlighted continuity and accessibility as empowering factors that made them feel assured and confident. Midwives emphasised the pregnancy to postnatal continuity as crucial to providing care based on individual needs. Key conclusions/Implications for practice Using a structured and coordinated care model as a midwife that includes planning for the postnatal period together with the pregnant woman at the end of pregnancy may be a good and relatively easy way to create continuity and thus ensure satisfaction and confidence in expectant and new mothers.

    • Background: Living with chronic obstructive pulmonary disease stage III or IV has a major impact on a person's everyday life. This qualitative study focuses on women with chronic obstructive pulmonary disease stage III or IV. Aim: The aim of this study was to describe women's experiences of living with chronic obstructive pulmonary disease stage III or IV. Method: A purposive sample of fifteen women with chronic obstructive pulmonary disease stage III or IV participated in the study. Data were collected through individual semi-structured interviews that were subjected to qualitative content analysis. Findings: One theme was identified, stabilizing an ever-present breathlessness by restoring strength, and three categories are as follows: managing a restricted everyday life as an expert of their illness, being afraid of contracting infections leading to suffocation and suffering and importance of continuous help and support from significant others and digital media. Breathlessness restricted women with chronic obstructive pulmonary disease, living with a body they have to wait for. Managing everyday life was adapted to their limited abilities and energy. This required detailed planning, good knowledge of their breathing and body. Women were afraid of contracting life-threatening infections that caused suffering, especially COVID-19. The fear leads to isolation and digital media was described as an important means of communication. Significant others gave support and help that was practical and emotional. Women with chronic obstructive pulmonary disease experienced lack of continuous help and support from healthcare professionals. Conclusion: Stabilizing an ever-present breathlessness by restoring strength required women with chronic obstructive pulmonary disease stage III or IV to conduct detailed planning to manage everyday life. Being afraid of contracting infections and the consequences of suffocation had increased since the pandemic COVID-19 outbreak, which led to self-isolation and an inactive everyday life. To get help, support and socialize, women used digital media.

    • Background Being a significant other (SO) to a person with borderline personality disorder (BPD) affect their health. High incidence of substance use disorder, post-traumatic stress disorder, stress, fear, anxiety, depression, family burden and grief are common. Some specific therapies for BPD, have included support to SOs, however resources are scarce and to participate in the support it assumes that the person with BPD is included in these therapies. Although the SO support has been shown to be helpful, they all have a similar structure, and only a small exclusive group of SOs have access to the support.Aim The aim was to describe experiences and need of support for significant others to persons with borderline personality disorder from the perspective of themselves and of health care workers.Methods Data was collected via two focus groups. One with five SOs to persons with BPD, one with five health care workers. Two interview sessions in each group were conducted and data were analysed with qualitative content analysis. The study was approved by the research ethics committee of Lund (2016-1026).Results The results revealed four themes; not being seen by health care professionals creates hopelessness, being seen by healthcare professionals creates trust, experience of support - helpful or shameful and the step from loosely structured support to a structured support group. Both groups expressed a need for further support as a complement to already existing support.Conclusions The need of support is extensive. The results suggest a professional coordinator intended for SOs and peer support groups not linked to a particular psychiatric treatment yet offering support in a structured way. Further studies examining these complements to existing support, is therefore recommended.

    • I Sverige är cannabis, särskilt ungdomars bruk av substansen, kontroversiella ämnen som uppmärksammas såväl i media och politik som bland myndigheter och allmänhet. I denna artikel presenterar vi ett forskningsprojekt som belyser det svenska cannabisbruket, med fokus på vilka som gör det, deras åsikter och motiv, samt hur det kan gå till när de möter preventionsinsatser. Våra resultat pekar mot att cannabisbruk kan ha många olika betydelser och funktioner i människors liv, samt att den svenska förbudspolitiken sätter ramarna för hur användare förstår sina handlingar och hur de bemöts av omgivningen. Yttre inflytande från en drogliberal omvärld har dock inneburit att det knappast längre går att identifiera en allenarådande ”berättelse” om vad cannabis är, vilka effekter substansen har och vad som kännetecknar användarna. Vårt projekt understryker vikten av en mer nyanserad politik och praktik i förhållande till cannabisbruk i Sverige.

    • This work reports the evaluation of a tactile sensor for the potential of using it to measure the compliance of folding cartons. A tactile sensor would make it possible to measure the mechanical behavior locally around the contact point, in contrast to existing methods that measure the global mechanical behavior of the carton. Research on the haptic sense has shown that the local mechanical behavior is more important than the global behavior when humans assess compliance of objects. It is shown that the response of the tactile sensor correlates strongly with the bending stiffness of the board, but also with geometric features. A method for reducing the 22-dimensional output of the sensor to single meaningful feature using linear discriminant analysis is proposed and tested. The results show that the sensor is a good candidate for a method that incorporates both cutaneous and kinaesthetic information in the measure of carton compliance.

    • The importance of sensory information in product purchasing decisions has gained increasing attention in recent years. Tactile properties of packaging are usually measured with the help of trained evaluators. An objective, fast and repeatable method that describes the mechanical interaction and does not rely on a panel would have many benefits. We propose and evaluate such a method for measuring the mechanical interaction between a deformable finger-like shaped sensor and a package. Evaluation of the method shows good repeatability, the variability in the measurement result is within a few percent in most cases. The method captures indentation differences at contact between sensor and package due to measurement position and package design.

    • There is limited research on nurses' experiences of nursing care in the operating room. The operating room nurses' responsibility is to ensure good nursing care before, during and after surgery. In an increasingly technological health care environment, there is always a risk of turning the focus away from nursing care towards technology and medicine. Integration of past experiences into the role as an operating room nurse becomes a challenge for those who recently worked as general nurses. The present study aimed to explore newly trained operating room nurses' experiences of nursing care in an operating room. Semi-structured interviews were performed with ten operating room nurses with a maximum three years' work experience from an operating room. The interviews were subjected to qualitative content analysis. The findings revealed three themes describing operating room nurses' searching for their new role. They experienced a gap between theory and practice, felt alone and insignificant and had to find their own place. The operating room nurses' experienced threats to safe nursing when they lacked time for the patients as well as for their own recovery, and they lacked feedback in order to improve care. They ensured security for patients by establishing one-to-one contact, protecting patients' well-being and working in teams for the patients' best interest, participants also focused on the task at hand instead of the patient as a person. New ways of organising work in operating units, and well-functioning teams can be a key to a successful integration of experiences from ward nurse to an operating room nurse, and provide support so that they feel more visible, at ease and safe in their new profession.

    • Kunskapen om sambanden mellan sociala livsvillkor och hälsa var tidigt en central utgångspunkt för hälso- och sjukvårdskuratorernas arbete, men har över tid tenderat att underordnas ett fokus på mer individinriktade insatser. Samtidigt beskrivs ofta hälso- och sjukvårdskuratorerna ha en otydlig yrkesroll och en svag professionell ställning. Den nya legitimationsgrundande examen utgör en unik möjlighet att förtydliga hälso- och sjukvårdskuratorernas yrkesroll, och kan idealt leda till utvecklandet av ett samlat begreppsligt ramverk för arbetet. I denna essä lyfter jag ramverket om de sociala bestämningsfaktorerna för hälsan som ett lämpligt begreppsligt ramverk för hälso- och sjukvårdskuratorernas arbete. Ramverket passar väl in i kuratorernas generalistexpertis och biopsykosociala perspektiv, och skulle kunna bidra till att tydliggöra yrkesgruppens unika kompetens. Ramverket om de sociala bestämningsfaktorerna för hälsan kan också användas för att återaktualisera och tydliggöra behovet av kurativa insatser inte bara på individ- utan även på meso- och makronivå.
