  • Detecting Prolonged Sitting Bouts with the ActiGraph GT3X.

    • The ActiGraph has a high ability to measure physical activity, however, it lacks an accurate posture classification to measure sedentary behaviour. The aim of the present study was to develop an ActiGraph (waist-worn, 30Hz) posture classification to detect prolonged sitting bouts, and to compare the classification to proprietary ActiGraph data. The activPAL, a highly valid posture classification device, served as reference criterion.1 Both sensors were worn by 38 office workers over a median duration of 9 days. An automated feature selection extracted the relevant signal information for a minute based posture classification. The machine-learning algorithm with optimal feature number to predict the time in prolonged sitting bouts (≥5 and ≥10 minutes) was searched and compared to the activPAL using Bland-Altman statistics. The comparison included optimised and frequently used cut-points (100 and 150 counts-per-minute (cpm), with and without low-frequency-extension (LFE) filtering). The new algorithm predicted the time in prolonged sitting bouts most accurate (bias ≤7 minutes/day). Of all proprietary ActiGraph methods, only 150 cpm without LFE predicted the time in prolonged sitting bouts non-significantly different from the activPAL (bias ≤18 minutes/day). However, the frequently used 100 cpm with LFE accurately predicted total sitting time (bias ≤7 minutes/day). To study the health effects of ActiGraph measured prolonged sitting, we recommend using the new algorithm. In case a cut-point is used, we recommend 150 cpm without LFE to measure prolonged sitting, and 100 cpm with LFE to measure total sitting time. However, both cpm cut-points are not recommended for a detailed bout analysis.

    • Competitive breath-hold divers try to achieve maximum times, depths, or distances underwater, thereby risking hypoxic syncope. In the present study, the cardiorespiratory responses to dynamic apnea (simultaneously initiated apneas and dynamic leg exercise) were investigated in 10 breath-hold divers. The divers performed 60 s dynamic apneas with the face in air (A) or face immersed in cold water (AFI). During apneas, the arterial oxygen saturation was reduced (A: -10%), but to a lesser extent during AFI (-6%, P<0.01), reaching a nadir 10-15 s post-apnea. Also, changes in end-tidal O(2) and CO(2) pressures (P(et)O(2)/P(et)CO(2)) were smaller during AFI than A (DeltaP(et)O(2): 8.2 vs 8.7 kPa, P<0.01; DeltaP(et)CO(2): 3.1 vs 3.2 kPa, P<0.05). The heart rate was lower during AFI than A (66 vs 78 bpm, P<0.01), reflecting an augmented diving response during AFI. The maximum safe breath-hold time under the conditions of the present study was calculated to be 101 and 106 s for A and AFI, respectively, consistent with the dynamic apnea times achieved by world-class apnea divers. It is concluded that the augmented diving response during face immersion apneas is associated with a slower reduction of the pulmonary (and arterial) oxygen store, probably delaying the occurrence of a hypoxic syncope.

    • To determine the incidence of anterior cruciate ligament (ACL) injuries in the general population; the pathology associated with a knee sprain verified by magnetic resonance imaging (MRI); and the agreement between clinical findings and MRI.Materials and methods: Inclusion criterion was an acute rotational trauma to the knee associated with effusion. One hundred and fifty-nine consecutive patients, mean age 27 years and 36% women, were included after clinical assessment at the orthopedic emergency unit. Patients were referred to an MRI examination (1.0 or 1.5 T) performed within a median of 8 days of the initial visit.Results: The annual incidence of MRI verified ACL injuries was 0.81/1000 inhabitants aged 10-64 years. Fifty-six percent (n=89) of those included had sustained an ACL injury of whom 38% had an associated medial meniscus tear. There was a poor agreement between initial clinical antero-posterior laxity and MRI verified presence of an ACL tear (kappa 0.281). Every second patellar dislocation was diagnosed as a ligament injury. Conclusions: Our findings indicate that the incidence of ACL injuries is higher than previously described. We also show that the first clinical examination after an acute knee trauma has a low diagnostic value. Further assessment with MRI improves the chances of a correct diagnosis of intraarticular pathology and is recommended in the early phase after a rotational knee trauma.

    • The aim of this study was to assess whether a school-based program with expanded physical education lessons was effective in increasing children's physical capacity and in preventing excessive weight gain in children. The study performed in 2000-2003 comprised 132 children, 73 boys and 59 girls at baseline 6-9 years and in follow-up 9-12 years, attending two different schools with a similar size, appearance and structure in a rural area. The norm school (N-school) followed the stipulated curricular time, one to two physical education lessons a week, while the intervention school (I-school) increased it to four lessons. More positive changes in physical index (the sum of the age-standardized results in 11 physical tests) were found among children in the I-school than in the N-school. The number of children who increased body mass index (BMI) increased in both schools, but a lower increase in BMI could be seen in the I-school. Expanded physical education lessons could increase physical status among both overweight and normal-weight children, in particular aerobic fitness. The weekly dose of physical activity must be higher than 40 min a day and must start earlier in children's life to be more effective in combating BMI increase.

    • AIMS: Reduced functional performance and muscular dysfunction after knee injury and in knee osteoarthritis (OA) is suggested to be a factor in OA development. Validated functional performance tests applicable in the clinic and large-scale studies are lacking. The aim was to study the reliability and validity of 10 functional performance tests.METHODS: Two hundred and eighty-five subjects, 15-22 years post-meniscectomy, performed 10 functional performance tests. The mean age was 54 years (SD+/-11.2) and 79% were men; 52% had radiographic OA, and 48% were categorized as symptomatic. The tests were evaluated for test-retest reliability, discriminative ability (younger vs older age, men vs women, symptom-free vs symptomatic) and floor and ceiling effects.RESULTS: Two of the 10 tests, maximum number of knee bendings in 30 s and one-leg hop for distance, had good test-retest reliability (ICC 0.92, 95% CI 0.86-0.96 and 0.93, 95% CI 0.87-0.97) and were able to discriminate with regard to age, gender and symptoms, and had acceptable floor effects (9% and 3%, respectively).CONCLUSION: This study suggests the use of two functional performance tests: knee bendings/30 s and one-leg hop for distance, easy to use for evaluation of interventions due to knee injury and knee OA and when attaining long-term data of natural disease history.

    • This study evaluates associations between objectively measured daily physical activity vs aerobic fitness and body fat in children aged 8-11 years. A cross-sectional study of 225 children aged 7.9-11.1 years was performed. Abdominal fat mass (AFM) and total body fat (TBF) were quantified by dual-energy x-ray absorptiometry. TBF was calculated as percentage of total body mass (BF%). Body fat distribution was calculated as AFM/TBF. Aerobic fitness was measured by indirect calorimetry during a maximal cycle ergometer exercise test. Daily physical activity was assessed by accelerometers for 4 days and daily accumulation of moderate-to-vigorous and vigorous activity was calculated. Significant relationships (P<0.05) existed for vigorous activity vs ln BF% (r=-0.40), ln AFM (r=-0.35), TBF/AFM (r=-0.22) and aerobic fitness (r=0.38), whereas moderate-to-vigorous activity displayed weaker relationships (-0.22, -0.18, -0.12 NS, and 0.25). Multiple regression analyses with inclusion of possible confounders concluded that vigorous activity was independently related to aerobic fitness and ln BF% or ln AFM. Moderate-to-vigorous activity was only independently related to aerobic fitness. In this population, low daily accumulation of vigorous activity was, already in children aged 8-11 years, associated with more body fat and lower aerobic fitness. A similar relation was not found for daily accumulation of moderate-to-vigorous activity.

    • Training studies in children have suggested that endurance training can give enlargement of cardiac dimensions. This relationship has not been studied on a population-based level in young children with objective methods. A cross-sectional study was made of 248 children (140 boys and 108 girls), aged 8-11 years, from a population-based cohort. Left ventricular end-diastolic diameter (LVDD) and left atrial end-systolic diameter (LA) were measured with echocardiography and indexed for body surface area (BSA). Physical activity was assessed by accelerometry, and the duration of vigorous physical activity per day (VPA) was calculated. Acceptable accelerometer and echocardiography measurements were obtained in 228 children (boys=127, girls=101). Univariate correlations between VPA and LVDD were indexed for BSA in boys (r=0.27, P<0.05) and in girls (r=0.10, NS). Multiple regression analysis showed that independent factors for LVDD, indexed for BSA for boys, were age and VPA. LA indexed for BSA was not related to physical activity variables in either gender. No clear relationship exists between cardiac size and daily physical activity in children aged 8-11 years. This suggests that significant cardiac remodelling due to volume exposure secondary to a high amount of physical activity begins later in life.

  • Motor skills and school performance in children with daily physical education in school : a 9-year intervention study

    • The aim was to study long-term effects on motor skills and school performance of increased Physical Education. All pupils born 1990-92 from one school were included in a longitudinal study during nine years. An intervention group (n=129) achieved daily PE (5x45 minutes/week) and if needed one extra lesson of adapted motor training. The control group (n=91) had PE two lessons/week. Motor skills were evaluated by the MUGI observation checklist and school achievements by marks in Swedish, English, Mathematics, PE and proportion of pupils who qualified for upper secondary school. Findings: In school year 9 there were no motor skills deficits in 93% of pupils in the intervention group compared to 53% in the control group (p<0.001), 96% of the pupils in the intervention group compared to 89% in the control group (p<0.05) qualified for upper secondary school. The sum of evaluated marks was higher among boys in the intervention group than in the control group (p<0.05). Interpretation: Daily PEH and adapted motor skills training during the compulsory school years is a feasible way to improve not only motor skills but also school performance and the proportion of pupils who qualify for upper secondary school.   • Effects of functional exercise training on performance and muscle strength after meniscectomy: a randomized trial.

    • Muscular deficits and functional limitations have been found years after meniscectomy of the knee. The purpose of this randomized controlled trial was to examine the effect of functional exercise training on functional performance and isokinetic thigh muscle strength in middle-aged patients subsequent to meniscectomy for a degenerative tear. Four years after meniscectomy, 45 patients (29 men, 16 women) were randomized to functional exercise training, supervised by a physical therapist, three times weekly for 4 months or to no intervention. The exercise program comprised of postural stability training and functional strength and endurance exercises for leg and trunk muscles. Outcomes were three functional performance tests and isokinetic muscle strength. Thirty patients (16 exercisers/14 controls) completed the study. Compared with control patients, the exercise group showed significant improvement in one-leg hop (change 8 vs 2 cm; P=0.040), hamstrings strength 60 degrees /s (P=0.033), and quadriceps endurance 180 degrees /s (P=0.001). Functional exercise training was well tolerated and improved functional performance and thigh muscle strength in this group of middle-aged subjects with a previous degenerative meniscal injury and partial meniscectomy.

    • Our purpose was to examine self-efficacy of knee function, physical activity (PA) and health-related quality of life (HRQoL) in post-meniscectomy patients and controls as well as to explore the impact of gender and the association between outcomes. Ninety-nine post-meniscectomy patients (27% women), mean age 44.5 years, mean (range) of follow-up time 3 (1-5) years, and 94 controls (34% women), mean age 45 years, completed the following questionnaires: the Knee Self-Efficacy Scale (K-SES(ABC) ), the Physical Activity Scale (PAS) and the Short Form-36 (SF-36). Patients scored lower than controls in K-SES(ABC) and in the SF-36 subscales Physical Functioning and Bodily Pain (P≤0.002). Forty-six percent of the patients had resumed pre-injury PA, but current PA did not differ between the groups. In the patients, K-SES(ABC) correlated strongly with four physical SF-36 subscales and one mental scale (Vitality) (r(s) =0.56-0.85, P<0.001) and moderately with three subscales: (r(s) =0.35-0.46, P<0.001) and with PAS (r(s) =0.42, P<0.001). Females scored lower than males in K-SES(ABC) (P=0.006) and in four SF-36 subscales (P<0.04), but reported similar PA as men. We conclude that meniscectomy in middle-aged individuals may lead to lower self-efficacy of knee function, a sedentary lifestyle and poorer HRQoL.

    • Mechanical force is generated within skeletal muscle cells by contraction of specialized myofibrillar proteins. This paper explores how the contractile force generated at the sarcomeres within an individual muscle fiber is transferred through the connective tissue to move the bones. The initial key point for transfer of the contractile force is the muscle cell membrane (sarcolemma) where force is transferred laterally to the basement membrane (specialized extracellular matrix rich in laminins) to be integrated within the connective tissue (rich in collagens) before transmission to the tendons. Connections between (1) key molecules outside the myofiber in the basement membrane to (2) molecules within the sarcolemma of the myofiber and (3) the internal cytoplasmic structures of the cytoskeleton and sarcomeres are evaluated. Disturbances to many components of this complex interactive system adversely affect skeletal muscle strength and integrity, and can result in severe muscle diseases. The mechanical aspects of these crucial linkages are discussed, with particular reference to defects in laminin-alpha 2 and integrin-alpha 7. Novel interventions to potentially increase muscle strength and reduce myofiber damage are mentioned, and these are also highly relevant to muscle diseases and aging muscle.

  • Sports and leisure-time physical activity in pregnancy and birth weight: a population-based study.

    • We examined the association between sports and other leisure-time physical activities during pregnancy and birth weight of babies born after 37 completed weeks of gestation. All Danish-speaking pregnant women attending routine antenatal care at the Department of Obstetrics, Aarhus University Hospital, Denmark, from August 1989 to September 1991 were invited to participate in the study. A total of 4458 healthy women who delivered after 37 completed gestational weeks participated in this study. The associations between sports (0, 1-2, 3+ h/week) or leisure-time physical activity (sedentary, light, and moderate to heavy) and birth weight were examined by linear and logistic regression and adjusted for potential confounding factors such as smoking, parity, schooling, pre-pregnancy body mass index and gestational age. The results showed that pregnant women who practiced sports or were moderate to heavy leisure-time physical active during the early second or the early third trimester gave birth to infants with a similar birth weight as inactive women. The proportion of newborns with a low (<2500 g) or a high birth weight (>/=4500 g) was also unchanged. In conclusion, in this large population-based study, we found no association between sports and leisure-time physical activity and low-birth weight, high-birth weight, or average-birth weight.

    • The present intervention was designed to investigate whether a 14-week period of regular recreational association football (F) or endurance running (R) has an effect on the risk of falls and bone fractures due to gains in muscle function and volumetric bone mineral density (vBMD). Fifty healthy untrained Danish premenopausal women were randomized into two training groups (F and R) that trained 1.8 +/- 0.3 (+/- SD) and 1.9 +/- 0.3 h/week, respectively, and these groups were compared with an inactive control group (C). Jumping and dynamic muscle strength were tested and tibial vBMD was measured using peripheral quantitative computed tomography. Total vBMD in left and right tibia increased by 2.6 +/- 2.3% and 2.1 +/- 1.8% (P < 0.005), respectively, in F and by 0.7 +/- 1.3% (P=0.05) and 1.1 +/- 1.5% (P < 0.01), respectively, in R without any significant changes in C. Similar results were found for trabecular vBMD. In F, peak jump power increased by 3 +/- 6% (P < 0.05), and hamstring strength during fast (240 degrees/s) and slow (30 degrees/s) contractions increased by 11 +/- 25% and 9 +/- 21%, respectively, (P < 0.05) without any significant changes in R or C. In conclusion, 14 weeks of regular recreational football improved peak jump power, maximal hamstring strength and vBMD in the distal tibia, suggesting a decreased fracture risk due to stronger bones and a reduced risk of falling.

    • Eccentric exercises are commonly used as a treatment for various muscle and tendon injuries. During complex motions such as the forward lunge, however, it is not always clear which muscles may be contracting eccentrically and at what time. Because this exercise is used during rehabilitation, the purpose of this investigation was to determine what type of contractions take place during two different types of forward lunge and assess the implications for rehabilitation. Five experienced athletes performed five cycles for each of the walking and jumping forward lunges. Motion analysis was used to calculate the shortening or elongation of each muscle based on the change of position of their origin and insertion points during the lunge. Electromyography of the lateral hamstrings, rectus femoris and lateral gastrocnemius was combined with the muscle length change data to determine when isometric, concentric and eccentric activations occur during the lunge. Eccentric contractions in both the quadriceps and gastrocnemius were observed during the lunge. No hamstring eccentric contractions were found; however, the hamstrings showed isometric contractions during the first part of the stance phase.

  • Effect of intensive training on lower leg structural strength: an in vivo study in ovariectomized rats

    • The aim of this study was to investigate the effect of training on the in vivo tibial structural strength during the development of post-ovariectomy osteoporosis. Seventeen mature Wistar rats (215 g) were ovariectomized and randomized into two groups. The sedentary control group was kept cage confined, while 3 days postoperatively the trained group started treadmill running with high intensity for 1 h 5 days a week. All were given a low calcium diet (Ca 0.01%). After 8 weeks the animals were anaesthetized and the right lower legs fractured during muscle contraction in three-point ventral bending. The left legs were fractured at the same level after removal of all soft tissues. Histomorphometry of the meta- and diaphysis of the distal tibiae was performed. Weight-gain was higher in sedentary (108 g) than in trained (61 g) rats (P<0.0001). There were no significant differences in mechanical results between the groups at in vivo or in vitro fracture. Correcting for weight-gain differences did not change these results. Histomorphometry showed no differences between the groups. Corticosterone was higher in trained than in sedentary rats (P<0.02), and corticosterone may have had a negative influence both on muscle and bone. The study could not show an effect of high intensity training in the early phase after ovariectomy on in vivo or in vitro fracture strength.

    • Mechanical load through physical activity has been shown to be one of the best stimuli to increase the bone strength. This effect of mechanical load accounts for both the accrual of bone mineral and structural skeletal adaptations. Exercise prescription also includes a "window of opportunity" in the late pre- and early peri-pubertal period, where exercise is supposed to insert the most obvious beneficial effects, even if physical activity provides recordable skeletal benefits during all growth. There is also evidence that benefits in bone mass and bone structure obtained by mechanical load during growth may be maintained at advanced age. The notion that former male athletes have lower fracture risk than expected by age, support this view. Physical activity could therefore to be recommended at growth and adolescence as one possible strategy to reduce the future burden of fragility fractures.

    • We present a detailed comparison of ectopic ossification (EO) found in tendons of biglycan (Bgn), fibromodulin (Fmod) single and double Bgn/Fmod-deficient (DKO) mice with aging. At 3 months, Fmod KO, Bgn KO and DKO displayed torn cruciate ligaments and EO in their quadriceps tendon, menisci and cruciate and patellar ligaments. The phenotype was the least severe in the Fmod KO, intermediate in the Bgn KO and the most severe in the DKO. This condition progressed with age in all three mouse strains and resulted in the development of large supernumerary sesmoid bones. To determine the role of exercise in the extent of EO, we subjected normal and DKO mice to a treadmill exercise 3 days a week for 4 weeks. In contrast to previous findings using more rigorous exercise regimes, the EO in moderately exercised DKO was decreased compared with unexercised DKO mice. Finally, DKO and Bgn KO mice tested using a rotarod showed a reduced ability to maintain their grip on a rotating cylinder compared with wild-type controls. In summary, we show (1) a detailed description of EO formed by Bgn, Fmod or combined depletion, (2) the role of exercise in modulating EO and (3) that Bgn and Fmod are critical in controlling motor function.

  • Heavy-resistance training in older Scandinavian men and women: short- and long-term effects on arm and leg muscles

    • The short- and long-term effects of heavy-resistance training (85% of one-repetition maximum (RM)) on elbow flexion and knee extension dynamic and isokinetic strength and on morphology in the biceps brachii and vastus lateralis muscles were evaluated during 1 year in 35 Scandinavian men and women, aged 70-77 years, 12 of whom formed a control group. After the first 11 weeks of training (n = 23; 3 times/week) elbow flexion and knee extension dynamic strength (1 RM) had increased [mean +/- SD] 49% +/- 16 and 163% +/- 75, respectively, with no significant difference between men and women. For the following 27 weeks, strength was maintained with one training session per week (n = 12) but dropped without training (n = 11). After the final 11 weeks of training (n = 11; 3 times/week), strength had further increased 32% +/- 16 in both the arm and the leg. Isokinetic strength measurements (Cybex II; 30 degrees/s) revealed similar but smaller gains than for dynamic strength. Muscle biopsies (n = 20) taken at the start and after the first 11 weeks of training showed a significant increase in the area of both type 1 and type 2 fibers in the biceps brachii muscle and a positive significant correlation between the percentage increase in the proportional area of type 2 fibers in the vastus lateralis muscle and the percentage increase in knee extension dynamic muscle strength. In conclusion, older Scandinavian men and women have a high capacity both to improve and to maintain muscle strength, some of which is mediated through an adaptation in the muscle fiber type population.

    • This non-randomized prospective controlled study evaluates a daily school-based exercise intervention program of 40 min/school day for I year in a population-based cohort of 81 boys aged 7-9 years. Controls were 57 age-matched boys assigned to the general school curriculum of 60min/ week. Bone mineral content BMC; g) and areal bone mineral density (aBMD; g/cm(2)) were measured with dual X-ray absorptiometry (DXA) of the total body, the third lumbar vertebra (L3) and the femoral neck (FN). Bone width for L3 and FN was calculated from the lumbar spine and hip scan. No differences between the groups were found at baseline in age, anthropometrics or bone parameters. The mean annual gain in L3 BMC was 5.9 percentage points higher (P < 0.001), L3 aBMD a mean 2.1 percentage points higher (P = 0.01) and L3 width a mean 2.3 percentage points higher (P = 0.001) in the cases than in the controls. When all individuals were included in one cohort, the total duration of exercise including both school-based and spare-time training correlated with L3 BMC (r = 0.26, P = 0.003), L3 aBMD (r = 0.18, P = 0.04) and L3 width (r = 0.24, P = 0.006). The study suggests that exercise in pre-pubertal boys influences the accrual of bone mineral and bone width and that a 1-year school-based exercise program confers skeletal benefits, at least in the lumbar spine.

    • Quadriceps muscle weakness is common after knee injuries. This weakness is caused, in part, by reduced voluntary activation (VA) because of central activation failure (CAF). Superimposed electrical stimulation techniques are used to assess VA and to detect CAF. The aim of this study was to assess VA during knee extension in young healthy women and men, and to evaluate subjective discomfort from the electrical stimulation. The quadriceps muscle in six young healthy women (mean age 22 years) and six young healthy men (mean age 29 years) was stimulated during maximal voluntary contractions using a 100 Hz pulse train. Data were collected from two test sessions separated by 6-8 days and each session comprised of two trials. A visual analog scale for pain (VAS-pain) was used to evaluate subjective discomfort. Overall, young healthy, moderately active men and women did have the ability to fully activate their knee extensors isometrically, but they did not achieve full activation on every trial. In those trials where a CAF was detected, the degree was small (mean less than 2%), and did not vary between the two test sessions. Subjective discomfort was generally moderate and tolerable (mean VAS-pain score 35 mm). These results will assist the clinical assessment of muscle weakness following a knee injury and facilitate the design and evaluation of appropriate rehabilitation interventions.

    • There is limited knowledge of knee laxity in the long term after a complete anterior cruciate ligament (ACL) tear treated without ACL reconstruction. The aim of this study was (1) to describe the clinical course of knee laxity after a complete ACL tear over 15 years, and (2) to study the association between knee laxity and meniscal injuries and the development of knee osteoarthritis (OA). We studied 100 consecutive subjects [mean (SD) age 26 (8) years] presenting with acute ACL injury prospectively. The initial treatment in all subjects was knee rehabilitation without reconstructive surgery. The subjects were examined with Lachman's and pivot-shift tests at baseline, 6 weeks, 3 months, 1 year, 3 years and 15 years after the injury. Sagittal knee laxity was also evaluated with the KT-1000 arthrometer at the 15-year follow-up. During follow-up, 22 subjects were ACL reconstructed due to unacceptable knee instability. There was only a mild remaining knee laxity [median Lachman grade and pivot-shift test value of 1 on a 4-grade scale (0-3)] after 15 years in subjects treated without primary ACL reconstruction. Knees with higher anterior sagittal knee laxity 3 months after the injury had a worse long-term outcome with respect to meniscal injuries and knee OA development.

    • With increasing age, human skeletal muscles gradually decrease in volume, mainly due to a reduced number of motor units and muscle fibers, and a reduced size of type 2 fibers. As a result, progressive weakening and impaired mobility occur. High-resistance strength training is beneficial, even in the very old, and could possibly reverse some of the detrimental effects of age-related weakness. The importance of exercise for older people affords an excellent opportunity for the medicine community as a major source of information and promotion of physical activity for this rapidly growing segment of the population. In this review, we summarize the current knowledge of the effects of aging on the human neuromuscular system, describe some of the major underlying mechanisms of the aging atrophy and focus on the importance of strength training to improve muscle function in older people.

    • An increased risk of developing asthma has been reported among swimmers exposed to chloramine in pool arenas. The aim of the present study was to compare the prevalence of asthma and respiratory symptoms among elite aspiring swimmers compared with age-matched controls with different degrees of physical activity. We also aimed to relate these findings to mental and psychosocial factors. One hundred and one elite swimmers and 1628 age-matched controls answered a questionnaire containing questions about respiratory symptoms, lifestyle factors, mental and physical well-being. The controls were divided into three different groups according to the degree of physical activity, no physical activity, recreational training and elite training. Swimmers reported significantly more asthma symptoms, with 36.6% having physician-diagnosed asthma, compared with 16.2% among the controls. Use of regular medication was more common (14.9% vs 8.0%) and more swimmers reported an exacerbation of their asthma during the previous 12 months (16.8%) vs (5.8%) for the controls. Despite an increased prevalence of asthma symptoms, the swimmers reported best physical performance and best mental and physical well-being. They also had a healthier lifestyle without smoking and low alcohol consumption.

    • Achilles tendinopathy is common and treatment with eccentric exercises seems promising. We designed a prospective randomized clinical trial to test the hypothesis that eccentric calf muscle exercises reduce pain and improve function in patients with Achilles tendinopathy. Forty-four patients were recruited from primary care (mean age: 45 years; 23 women; 65% active in sports) and randomized to three treatment groups for 12 weeks: eccentric exercises, a night splint or a combination of both treatments. Pain and function were evaluated at 6, 12, 26 and 52 weeks by the Foot and Ankle Outcome Score. At 6 weeks, the eccentric group reported a significant pain reduction (27% compared with baseline, P=0.007) which lasted for 1 year (42%, P=0.001). The two groups treated with a night splint also reported significant but less pain reduction than the eccentric group. Differences between all the three groups were not significant. At 12 weeks, the eccentric group reported significantly less pain than the splint-only group (P=0.04). More patients in the eccentric group than in the splint group returned to sport after 12 weeks. We conclude that eccentric exercises seem to reduce pain and improve function in patients with Achilles tendinopathy. Our results are in line with previous studies and strengthen the recommendation that patients should undergo an eccentric exercise program prior to considering other treatments such as surgery.

    • Intervention studies have shown that high-intensity training programs with duration of 1-3 years can influence bone mass in post-menopausal women. We wanted to investigate whether moderate physical activity could be associated with reduced post-menopausal bone loss also in the long-term perspective. We evaluated changes in bone mass and bone structure by repeated single-photon absorptiometry measurements of the distal forearm in 91 moderately physically active and 21 inactive women, categorized according to information from questionnaires, from menopause and on average 25 years onwards. Data were calculated with analysis of variance and analysis of covariance tests and presented as means with 95% confidence interval. There were no group differences in bone mass or bone structure at menopause. The mean annual loss in bone mineral content was 1.2% (1.1, 1.3) in the physically active and 1.6% (1.3, 1.8) in the inactive women (after adjustment for menopausal age P = 0.02) and the mean decline in a strength index based on bone mass and bone structure was 0.7% (0.6, 0.8) in the physically active and 1.2% (0.8, 1.5) in the inactive women (P = 0.004). There were no group differences in the changes in bone structure. Physical activity is also in a long-term perspective associated with reduced post-menopausal bone loss.

    • Osteoarthritis (OA) of the knee can be defined as primary (non-traumatic) or secondary (post-traumatic). Both non-traumatic OA and post-traumatic OA have been described predominantly in the medial compartment of the knee. The objective of this study was to compare the location of structural radiographic changes in non-traumatic OA and post-traumatic OA. A non-traumatic cohort, consisting of 155 patients suffering from chronic knee pain without known major injuries, was compared with a post-traumatic cohort, consisting of 176 patients, all of whom had sustained an anterior cruciate ligament injury while playing soccer. A standardized weight-bearing knee radiography of the tibiofemoral joint was performed. Joint space narrowing (JSN) and osteophytes were graded according to the radiographic atlas of the Osteoarthritis Research Society International. JSN and osteophytes were located predominantly in the medial compartment in the non-traumatic cohort. In the post-traumatic cohort, structural changes were evenly distributed between the lateral and the medial compartments. We were thus able to demonstrate radiological differences between post-traumatic and non-traumatic OA.

    • Exercise-induced rib stress fractures have been reported frequently in elite rowers during the past decade. The etiology of rib stress fractures is unclear, but low bone mineral density (BMD) has been suggested to be a potential risk factor for stress fractures in weight-bearing bones. The present study investigated BMD in seven Danish national team rowers with previous rib stress fracture (RSF) and 7 controls (C) matched for gender, age, height, weight and training experience. Total body scan and specific scans of the lumbar spine (L2-L4), femoral neck and distal radius were performed using a DEXA scanner. The RSF subjects showed significantly lower L2-L4 BMD: RSF: 1.22 +/- 0.05 g cm(-2) (mean +/- SEM) (median 1.19 g cm(-2), range 1.02-1.37 g cm(-2)) compared to C: 140 +/- 0.04 g cm(-2) (median 1.41 g cm(-2), range 1.27-1.57 g cm(-2)) (P=0.028).The present results suggest that low bone mineral density may be a potential risk factor for the development of exercise-induced rib stress fractures in elite rowers.

    • The etiology of exercise-induced rib stress fractures (RSFs) in elite rowers is unclear. The purpose of the study was to investigate thoracic muscle activity, movement patterns and muscle strength in elite rowers. Electromyographic (EMG) and 2-D video analysis were performed during ergometer rowing, and isokinetic muscle strength was measured in seven national team rowers with a history of RSF and seven matched controls (C). RSF displayed a higher velocity of the seat in the initial drive phase (RSF: 0.25 +/- 0.03, 0.25 (0.15-0.33) m/s vs C: 0.15 +/- 0.06, 0.18 (-0.11-0.29) m/s P = 0.028) (Mean +/- SEM, median and range). Further, RSF had greater co-contraction of m. serratus anterior and m. trapezius in the mid-drive phase (RSF: 47.5 +/- 3.4, 48.5 (35.8-60.2)% EMG signal overlap vs C: 30.8 +/- 6.5, 27.0 (11.2-61.6)% P = 0.043). In addition, the RSF subjects showed a lower knee-extension to elbow-flexion strength ratio (RSF: 4.2 +/- 0.22, 4.3 (3.5-5.1) vs C: 4.8 +/- 0.16, 5.0 (4.2-5.3) P = 0.043), indicating stronger arms relative to legs compared with controls. In conclusion, increased thoracic muscle co-contraction, altered movement patterns and reduced leg/arm strength ratio were observed in the RSF subjects, which may all predispose toward an increased risk of RSF.

    • Force production profile and neuromuscular activity during slide-based and stationary ergometer rowing at standardized submaximal power output were compared in 14 male and 8 female National Team rowers. Surface electromyography (EMG) was obtained in selected thoracic and leg muscles along with synchronous measurement of handle force and rate of force development (RFD). Compared to stationary conditions, slide-based peak force decreased by 76 (57-95) N (mean 95% CI) in males (P < 0.001) and 20 (8-31) N (P < 0.05) in females. Stroke rate increased (+10.7%) and late-phase RFD decreased (-20.7%) in males (P < 0.05). Neuromuscular activity in m. vastus lateralis decreased in the initial drive phase from 59% to 51% of EMGmax in males and from 57% to 52% in females (P < 0.01-0.05), while also decreasing in the late recovery phase from 20% to 7% in males and 17% to 7% in females (P < 0.01). Peak force and maximal neuromuscular activity in the shoulder retractors always occurred in the second quartile of the drive phase. In conclusion, peak force and late-phase RFD (males) decreased and stroke rate increased (males) during slide-based compared to stationary ergometer rowing, potentially reducing the risk of overuse injury. Neuromuscular activity was more affected in leg muscles than thoracic muscles by slide-based ergometer rowing.

    • Minute-by-minute heart rate monitoring and an activity diary were used simultaneously during three days in 30 randomly selected adolescents (16 boys, 14 girls; mean age 15.0+/-1.0). Total daily energy expenditure (TDEE) and its components (energy expenditure during sleep, during rest and in physical activity) and times spent at different intensity levels (sedentary, light, moderate physical activity and vigorous physical activity) were compared. TDEE from heart rate monitoring averaged 10.9+/-2.7 MJ x d(-1) compared to 11.3+/-2.3 MJ x d(-1) from the activity diary (NS). The limits of agreement (mean+/-2 SD) were -3.54 MJ x d(-1) and 2.74 MJ x d(-1). There was no significant difference for any of the TDEE components between the methods (MANOVA). A significant method effect (P<0.001) was observed for time spent in sedentary and light physical activity (MANOVA). No significant difference was observed for time spent in moderate and vigorous physical activity. According to this, heart rate monitoring and activity diary are comparable for group assessment of TDEE and its components, and for estimating time spent in moderate and vigorous physical activity. The activity diary underestimated time spent in moderate and vigorous physical activity for inactive subjects and consequently overestimated highly active subjects.

    • Injury occurrence in all 12 female elite senior football teams in premiere league was registered during 1 year. Of 269 players, 129 (48%) sustained 237 injuries. The total injury incidence was 4.6/1000 h of football. The injury incidence during practice was 2.7 and during the game time was 13.9/1000 h. The highest injury incidence during practice was to the knee (0.8/1000 h) and thigh (0.5/1000 h), and during game time was to the knee (4.4/1000 h) and head (2.2/1000 h). In total, the location for the highest injury incidence was the knee with 1.5 injuries/1000 h of football. The majority of injuries (82%) were localized to the lower extremities. Sixty-six injuries (28%) were re-occurring injuries (re-injuries). The incidence of traumatic injuries was 3.3/1000 h of football and for overuse injuries 1.3/1000 h. Overuse injuries occurred mainly during the preseason and at the beginning of the spring season. Thirty-nine percent of the injuries were slight or minor causing absence from practice or game time of less than 1 week, 39% were moderate (absence 7-28 days) and 22% were major (absence more than 28 days). The major injuries occurred often owing to trauma and were mainly to the knee.

    • The objective was to examine the differences in anxiety ratings of elite and sub‐elite athletes when the relationship between intensity and direction scores of anxiety ratings is considered in analyses. Participants were 31 junior elite (Mean age: 17.7, SD=1.1) and 53 sub‐elite (Mean age: 17.5, SD=1.1) cross country skiers and swimmers who completed the direction modified CSAI‐2R before important competitions. Results showed that elite athletes rated a higher percent of items as facilitative to their performance whereas sub‐elite athletes rated a higher percent of items as debilitative. No significant differences between the elite and sub‐elite samples were displayed regarding rated direction scores of cognitive or somatic anxiety at moderate to high‐intensity levels. A significant difference in facilitative anxiety ratings was displayed at a low anxiety intensity level (Z=−2.20, P<0.05). Outcome performance data showed no consistent congruence with athletes' anxiety direction ratings. The findings suggest that facilitative direction scores are a consequence of low anxiety intensity, possibly combined with high self‐confidence levels. Directional anxiety researchers analyzing separate total scores of intensity and direction respectively, which is the traditional approach, may draw incorrect conclusions about the importance of facilitative ratings of anxiety symptoms.

    • To examine differences in levels of physical activity (PA), time spent at moderate-to-vigorous intensity PA (MVPA) and time spent sedentary between and within days in children from four European countries, 1954 9 - and 15-year-olds were included. PA was measured during 2 weekdays and 2 weekend days using the manufacturing technology-incorporated (MTI) accelerometer. Average count per minute, time spent sedentary, time spent at MVPA and the proportion of children accumulating > or =60 min of MVPA were calculated. Data were compared between weekdays and weekend days and between school time and leisure-time. Although not entirely consistent across countries, overall PA, time spent sedentary and the proportion of children accumulating > or =60 min of MVPA were higher during weekdays compared with weekend days. Differences in overall PA between school time and leisure-time were highly inconsistent between countries. Few children (4-31%) accumulated > or =60 min of MVPA either during school time or during leisure-time. Differences in activity patterns between weekdays and weekend days are explained by less accumulated time in MVPA during weekend days. Weekend days and leisure-time during weekdays seem appropriate targets when promoting PA in order to increase the proportion of children achieving current recommendations on health-enhancing PA.

    • The aim of this study was to examine the stability of objectively measured physical activity in Swedish children and to study variables that predicted physical activity and body mass index standard deviation score (BMI SDS) at follow-up. A total of 97 children provided valid repeated measurements of physical activity between 2002 and 2005. The children were on average 7.5 years at baseline (SD+/-0.92) and 9 years at follow-up (SD+/-0.92). The mean follow-up time was approximately 1.5 years (mean 558 days, SD+/-224). An accelerometer (Actiwatch, Cambridge Neurotechnology Ltd., Cambridge, UK) was used to measure physical activity during 7 consecutive days. Yearly weight and height were examined and BMI SDS was calculated. Baseline physical activity was significantly correlated with physical activity at follow-up (r=0.59) with a stronger correlation for boys (r=0.72) than for girls (r=0.51). High physical activity levels were more stable (r=0.74) than low physical activity levels (r=0.55). Physical activity at follow-up was explained by physical activity at baseline and season (R(2)=0.46) whereas BMI SDS at follow-up was explained by BMI SDS at baseline and age (R(2)=0.90). The results of this study suggest that physical activity levels are fairly stable in 6-10-year-old children.

    • The aim of this study was to investigate physical activity levels and factors related to physical activity among 16-year old girls and boys in secondary school practical and theoretical programs. An additional aim was to identify factors contributing to differences in physical activity patterns between the educational programs. Sixteen-year old students in the first year of secondary school in Sweden (n = 585) were systematically selected and 93% agreed to complete a questionnaire. Compared with students in theoretical programs, those in practical programs spent less time and energy per day in physical activity. Girls in practical programs were the least physically active, and boys in theoretical programs the most physically active students. Statistical analysis showed that, among students in practical programs, the influence of educational program on physical activit y level was explained by lower socio-econornic level, less parental support, lower perceived health level, and negative attitudes towards outdoor physical activities. These data suggest that promotion of physical activity should focus on students, especially girls, in practical programs. Because the difference in physical activity levels between educational programs was partly an effect of less positive parental influence, school-based programs may be especially influential in increasing physical activity levels among secondary school students.

    • The Swedish male senior national football team was followed prospectively between 1991 and 1997. During these 6 years, the team played 73 official matches and had three training camps. The senior author (J. E.) attended 57 of these matches and the three training camps and these matches and training camps, are included in the present study. Exposure to football was recorded individually for each player. The team physician examined all injuries. Total exposure was 7245 h (6235 training and 1010 match hours) and there were 71 injuries (40 training and 31 match injuries). Five (16%) of the match injuries were major, with more than 4 weeks of absence from football. The injury incidence during training was 6.5/1000 h and the injury risk during matchplay was 30.3/1000 h. A significantly higher injury incidence was found for matches lost compared to matches won or drawn (52.5 vs. 22.7/1000 h, P=0.026). No statistically significant difference for injury was found between competitive matches and friendly matches. No difference was found between home and away matches or matches on neutral ground. The risk for injury when playing in a national team compares with previously reported figures for professional football at a high level.

    • Physical activity referral (PAR) schemes or concepts occur in varying forms. Because few physical activity intervention studies have been carried out in routine health care settings, it is difficult to translate research findings into daily practice. The aim of this study was to analyze the effectiveness of a PAR scheme implemented in routine primary health care. The study did not include a control group and was based on the ordinary staff's work efforts and follow-up measures. During a 2-year period, 6300 PARs were issued. Effectiveness was measured by an increase in self-reported physical activity. Half of the patients reached reported increased physical activity both at 3 months (49%) and at 12 months (52%). The proportion of inactive patients decreased from 33% at baseline to 17% at 3 months and 20% at 12 months. The proportion of patients who were physically active on a regular basis increased from 22% at baseline to 33% at 3 months and 32% at 12 months. Neither the patient's age nor the profession of the prescriber was associated with differences in effectiveness. The patient's activity level at baseline, the type of physical activity as well as the reason for the prescription were associated with increased physical activity.

    • The aim of this retrospective study was to compare the results after arthroscopic anterior cruciate ligament (ACL) reconstruction using the four-strand semitendinosus-gracilis (ST/G) autograft in male (n=141) vs female (n=103) patients. The patients were operated on between 1996 and 2005, using interference screw fixation and drilling the femoral tunnel through the anteromedial portal. The pre-operative assessments and demographics, apart from age (males 29 years, females 26 years; P=0.02), were comparable at the time of surgery. At 25 (23-36) months post-operatively, no significant differences were found between the study groups in terms of anterior side-to-side knee laxity, manual Lachman test, Tegner activity level, Lysholm knee score, range of motion or donor-site morbidity. Both study groups improved significantly in most clinical assessments and functional scores compared with their pre-operative values. Two years after ACL reconstruction using ST/G autografts, there were no significant differences between male and female patients in terms of clinical outcome or functional scores.

    • The aim of the present investigation was to test the reliability of two sports related functional tests, a speed test (slalom-test) and an agility test (hurdle-test). Eleven athletes aged 11 years (8 boys, 3 girls) participated voluntarily in the study. All subjects completed four different test sessions for both the slalom-test and the hurdle-test using six standard track hurdles placed at 2-m intervals along a 12-m length of track. There were no significant differences between testing sessions for either the slalom-test (P=0.99) or the hurdle-test (P=0.96), showing no systematic variation between test times. The intraclass correlation coefficients were 0.96 and 0.90 respectively, indicating a good reliability. We conclude that the slalom-test and the hurdle-test are reliable sports related functional tests for measuring speed and agility in groups of young athletic individuals.

    • We investigated changes in a large battery of pro- and anti-inflammatory cytokines in elite female soccer players following two 90-min games separated by a 72-h active or passive recovery. Blood samples were taken from 10 players before, within 15-20 min, 21, 45 and 69 h after the first game and within 15-20 min after the second game. The leukocyte count was analyzed, together with several plasma pro- and anti-inflammatory cytokines, using a multiplex bead array system. After the first and second game, the total leukocytes and neutrophils increased significantly. Likewise, increases (P<0.05) in pro-inflammatory cytokines [interleukin (IL)-12, tumor necrosis factor-alpha (TNF-alpha), interferon-gamma (INF-gamma), IL-17], chemokines [monocyte chemotactic protein-1 (MCP-1), IL-8 and monokine induced by gamma interferon (MIG)], anti-inflammatory cytokines (IL-2R, IL-4, IL-5, IL-7, IL-10, IL-13, INF-alpha) and the mixed cytokine IL-6 were observed. Leukocyte and cytokine levels were normalized within 21 h. Active recovery (low-intensity exercises) did not affect the cytokine responses. A dampened cytokine response was observed after the second game as only IL-12, IL-6, MCP-1, IL-8 and MIG increased (P<0.05). In conclusion, a robust pro- and anti-inflammatory cytokine response occurs after the first but not the second soccer game. The implications of the dampened cytokine response in female players after the second game are unknown.

    • We aimed to investigate markers of oxidative stress and levels of endogenous and dietary antioxidants in 16 elite female soccer players in response to a 90-min game (average intensity 82+/-3% HRpeak). Blood samples were taken before, immediately and 21 h after the game. Plasma-oxidized glutathione, the ratio of reduced to oxidized glutathione (GSH:GSSG) and lipid peroxidation measured by d-ROMs were used as markers of oxidative stress. Plasma endogenous [uric acid, total glutathione (TGSH)] and dietary antioxidants (alpha-tocopherol, ascorbic acid, total carotenoids and polyphenols) were analyzed using liquid chromatography and the Folin-Ciocalteu method. Exercise induced an acute increase (P<0.05) in GSSG, uric acid, TGSH, alpha-tocopherol, and ascorbic acid. In parallel, the GSH:GSSG ratio and polyphenols decreased (P<0.05). GSSG, GSH:GSSG ratio, uric acid, TGSH, and ascorbic acid returned to baseline at 21 h, while polyphenols and alpha-tocopherol remained altered. Total carotenoids increased above baseline only at 21 h (P<0.05). Lipid peroxidation, measured by d-ROMs, remained unchanged throughout the study. Thus, intermittent exercise in well-trained female athletes induces a transient increase in GSSG and a decrease in the GSH:GSSG ratio, which is effectively balanced by the recruitment of both endogenous and dietary antioxidants, resulting in the absence of lipid peroxidation measured by d-ROMs.

    • The primary purpose of this study was to evaluate whether a preseason strength training programme for the hamstring muscle group - emphasising eccentric overloading - could affect the occurrence and severity of hamstring injuries during the subsequent competition season in elite male soccer players. Thirty players from two of the best premier-league division teams in Sweden were divided into two groups; one group received additional specific hamstring training, whereas the other did not. The extra training was performed 1-2 times a week for 10 weeks by using a special device aiming at specific eccentric overloading of the hamstrings. Isokinetic hamstring strength and maximal running speed were measured in both groups before and after the training period and all hamstring injuries were registered during the total observational period of 10 months. The results showed that the occurrence of hamstring strain injuries was clearly lower in the training group (3/15) than in the control group (10/15). In addition, there were significant increases in strength and speed in the training group. However, there were no obvious coupling between performance parameters and injury occurrence. These results indicate that addition of specific preseason strength training for the hamstrings - including eccentric overloading - would be beneficial for elite soccer players, both from an injury prevention and from performance enhancement point of view.
