    • In this prospective study, 30 patients with motor conversion disorder were assessed for key psychiatric and demographic variables. At reassessment 2 to 5 years later, 19 patients had completely recovered and 8 patients had improved, whereas only 3 were unchanged or worse. Contrary to other follow-up studies, none of the patients received a rediagnosis of neurological disease. The presence of a personality disorder and overall personality pathology, particularly within cluster C, the presence of a concomitant somatic disease, low DSM-IV Axis V score, and high score on the Becks Hopelessness Scale proved to be associated with poor outcome.

  • Alcohol Consumption After Severe Burn : A Prospective Study

    • BackgroundThe number of patients with alcohol-related burns admitted to burn units has increased. It has been reported previously that alcohol-related burns are an indicator of alcohol dependence, but there are few studies addressing alcohol use several years after burn injury.ObjectiveTo investigate alcohol consumption 2–7 years after burn injury and to examine possible contributing factors.MethodsConsecutive adult patients with burns (n = 67) were included during hospitalization, and an interview was performed at 2–7 (mean = 4.6) years after burn. Data assessed at baseline were injury characteristics, sociodemographic variables, coping, and psychiatric disorders. At follow-up, the Alcohol Use Disorders Identification Test was used to identify at-risk drinking.ResultsOverall, 22% of the burns were alcohol-related; however, this was not associated with at-risk drinking at follow-up. Of the former patients with burns, 17 (25%) were identified as having an at-risk drinking pattern at follow-up. One item in the Coping With Burns Questionnaire used in acute care, “I use alcohol, tobacco or other drugs to be able to handle my problems”, was the only factor found to predict an at-risk drinking pattern several years after injury.ConclusionThere were more at-risk drinkers in this burn population as compared with in the general population. The results indicate that an avoidant coping pattern, including the use of alcohol to handle problems, can be considered a potentially modifiable factor.

    • Pseudoneurological symptoms (i.e., conversion disorder), historically subsumed within the "hysteria" concept alongside phenomena such as psychogenic amnesia and multiple personality disorder have been classified as somatoform disorders since DSM-III. Since then, there have been repeated calls to reclassify conversion disorder with the dissociative disorders, as in ICD-10. The authors review issues such as the high correlations between pseudoneurological and dissociative symptoms, the high rates of trauma reported for both groups, and the position that these phenomena share underlying processes. Although reintegrating pseudoneurological symptoms with the dissociative disorders is not without complications, there is a strong case for such a reclassification.

  • Managing for the common good : The case of Soplicex

    • The aim of this paper is to point to the potential value of an approach to management based on the idea of the common good, as opposed to classical capitalism based on private ownership. Such an approach makes it possible to resist a pursuit of short term oriented gains and a maximization of narrowly defined profits, and, instead, to focus on humanistic values, as to adopt a long term perspective. The much cited notion of the “tragedy of the commons” was based on deficient material and argumentation, but, most of all, it completely disregards of the issue of management. Using a case study developed through a longitudinal action research project in a big service enterprise we call “Soplicex”, we present the strategic process grounded in learning, as well as the building of a strong structure centred on teams. The engagement of the employees was, originally, strongly oriented towards the idea of the common good. The consultants and researchers adopted this principle as the guiding rule in their work with the organization. Even though the process was interrupted by the takeover by a foreign investor, we show how the findings of the study remain relevant for alternative organizing and managing today and in the future. The conclusions of this paper reach further than just being reflections on a historical case study: a model of management is presented, concerned with the care and protection of the common good.
