    • A combination of poor oral hygiene and dry mouth may be hazardous to the oral health status. However, systematic assessments in order to detect oral health problems are seldom performed in the nursing care of the elderly. The aims of this study were to investigate the occurrence of oral health problems measured using the Revised Oral Assessment Guide (ROAG) and to analyse associations between oral health problems and age, gender, living conditions, cohabitation, reason for admission, number of drugs, and functional and nutritional status. One registered nurse performed oral health assessments using ROAG in 161 newly admitted elderly patients in rehabilitation care. Oral health problems were found in 71% of the patients. Thirty per cent of these patients had between four and eight problems. Low saliva flow and problems related to lips were the most frequent oral health problems. Problems in oral health status were significantly associated with presence of respiratory diseases (problems with gums, lips, alterations on the tongue and mucous membranes), living in special accommodation (low saliva flow, problems with teeth/dentures and alterations on the tongue), being undernourished (alterations on the tongue and low saliva flow) and being a woman (low saliva flow). The highest Odds ratio (OR) was found in problems with gums in relation with prevalence of respiratory diseases (OR 8.9; confidence interval (CI) 2.8–27.8; P < 0.0005). This study indicates the importance of standardised oral health assessments in order to detect oral health problems which can otherwise be hidden when the patients are admitted to the hospital ward.

    • Interpersonal relationships are important for communication, oral health education and patients' satisfaction with dental care. To assess patients' attitudes towards dental caregivers, a Swedish version of the revised Dental Belief Survey (DBS-R) and a comparable and partly new instrument the Dental Hygienist Belief Survey (DHBS) have been evaluated. The aim of the present study was to investigate if patients' attitudes towards dental hygienists (DH) and dentists (D) differ with regard to the separate items in DBS-R and DHBS. The study was a comparative cross-sectional study with 364 patients (students, general patients and patients with periodontal disease). All patients completed the DBS-R and DHBS surveys. The overall pattern in the results showed that participants in general had a less negative attitude towards DH when compared with that towards D. This was most pronounced among students and least pronounced among patients with periodontal disease. No statistically significant difference could be found in items with regard to feelings of shame and guilt in dental care situations, indicating that these items were rated on a more negative level also for DH. The conclusion is that participants had a less negative attitude towards DH with the exception of situations which may give rise to feelings of shame and guilt, an important finding for future dental hygiene care.

Abstract: Objectives: The objective of the study was to analyse the relationship between dental anxiety and symptoms of general anxiety and depression among 15-year-old individuals. Methods: The sample analysed included 221 randomly selected 15-year-old individuals living in the city of Jönköping, Sweden. One questionnaire captured sociodemography and dental history, while dental anxiety was assessed by the Dental Fear Survey (DFS) and symptoms of general anxiety and depression by the Hospital Anxiety and Depression Scale (HADS). Results: About 6% of the adolescents were classified as dentally anxious. Symptoms of general anxiety and depression were significantly correlated with dental anxiety in both the bivariate and multivariate analyses. The latter analyses were adjusted for gender and previous painful experiences of dental care. Individuals with high dental anxiety showed general anxiety scores on a clinical level (mean=9.8, SD=4.3). Conclusions: Symptoms of general anxiety and depression were shown to be significantly correlated with dental anxiety among 15-year-old individuals.

Abstract: Objective: To evaluate the Dental Hygienist Beliefs Survey (DHBS) and the test-retest reliability of DHBS in a group of general dental patients. Material and methods: The DHBS, which is a questionnaire constructed to assess patients' specific attitudes towards dental hygienists (DHs), was distributed together with the Dental Anxiety Scale adapted to specifically assess fear of DH treatment (DHAS). It was hypothesized that DHBS would correlate with DHAS and gender. The questionnaires were consecutively distributed to 80 patients at their first visit and after a clinical examination performed by a DH student. Retest assessments of DHBS were conducted approximately two weeks later in conjunction with the next visit at the DH student and before treatment (scaling session). The final study sample included 77 adult general dental patients in treatment at an education clinic for DH students. Results: The results verified a statistically significant correlation between DHBS and DHAS. The DHBS sum of scores showed high internal consistency with Cronbach's a coefficient of 0.88 and 0.91 at the first and second assessments, respectively, and the test-retest reliability of the DHBS was acceptable with intraclass correlation coefficient of 0.76. No statistically significant association was found between DHBS and gender. Conclusion: The results suggest that the DHBS is a reliable and stable scale to use to assess patients' specific attitudes towards DHs. Moreover, DH beliefs are associated with fear of DH treatment.

    • The aim was to evaluate and test the psychometric properties of the Dental Hygienist Beliefs Survey (DHBS) in a Swedish sample of different patient groups and students. It was hypothesized that negative dental hygienist beliefs would discriminate between fearful and non-fearful study groups. The DHBS was distributed together with the revised Dental Beliefs Survey (DBS-R) and the Dental Anxiety Scale (DAS). The study sample included 394 subjects (130 students, 144 general dental patients, 90 periodontal patients and 30 patients on a waiting list for dental fear treatment). The results verified that the DHBS discriminates well between dentally fearful and non-fearful study groups. The DHBS had high internal consistency (Cronbach's alpha = 0.96-0.98) in all the groups. The correlation between the DHBS and the DBS-R was high (rho = 0.82, P < 0.001). Furthermore, the DHBS correlated significantly with the DAS, as well as with a low but significant correlation to age (more negative attitudes in younger age groups) and gender (more negative attitudes amongst women). Regression analysis showed that gender and the DHBS items: 23, 16 and 28, i.e. items related to feeling helpless, worries/fears not being taken seriously and fear about 'bad news' possibly preventing treatment, were the most important predictors of dental fear. The results suggest that the DHBS may be a valid and reliable scale to use in order to assess patient's specific attitudes to dental hygienists. However, the psychometric properties including test-retest analysis and the underlying factor structure of the DHBS need to be further explored.

Methods:  Data were obtained and categorized by interviewing 12 dental hygienists, who worked actively with tobacco cessation interventions. Qualitative content analysis was used for analysis. Results:  The latent content was formulated into the core category ‘the invisible oral health promotion work’. The informants thought that they had a responsibility to work with tobacco cessation. They perceived the financial system in which they perform the activity as frustrating, because tobacco cessation has no treatment code in the dental care insurance. This was one of several reasons why they had to integrate it in other treatment procedures. The results identified three categories: ‘balance in the meeting’, ‘possibilities and hindrance’ and ‘procedures’. In the narratives, both positive and negative aspects were displayed. Conclusions:  The financial conditions for tobacco cessation interventions need to be reformed and the activity has to be given a higher priority in the organization of dental care. Practical training in performing tobacco cessation interventions is important during the dental hygiene education; otherwise, tobacco cessation interventions will remain invisible in oral health promotion in the future.

    • Objectives: The aim of the present study was to investigate current practice and perceived barriers regarding tobacco cessation activities among Swedish dental hygienists (DHs). A secondary aim was to study knowledge about the importance of tobacco cessation in relation to different oral health conditions. Methods: A questionnaire including twenty-five questions was mailed to 400 randomly selected DHs in Sweden asking them about their tobacco cessation routines and perceived barriers. Questions were also included regarding knowledge about the importance of tobacco activities in relation to different conditions: caries, gingivitis, periodontitis, and dental implants. The study was approved by the Ethics Committee at Kristianstad University, Sweden. Results: The response rate was 57% n = 229). Forty-five percent n = 103) of these had received courses in tobacco cessation during or after their dental hygienist graduation. Tobacco cessation services were given by 64% n = 146) of the DHs. To improve the work with tobacco cessation 53% n = 121) of the DHs expressed a need for written guidelines and 37% n = 84) wished for courses. Of those DHs (n = 83) who did not provide tobacco cessation advice, 69% (n = 57) reported insufficient competence, while the remaining 31% (n = 26) reported other obstacles to offering the service. Conclusions: The present study indicates that to provide tobacco cessation advice in clinical practice, DHs require theoretical knowledge and clinical experience in this topic.

    • The aim of the present study was to describe dental hygienists' experiences of their physical and psychosocial work environment. The study was descriptive in design and used a qualitative approach. Eleven dental hygienists participated in the study and data were collected during spring 2008 using semi-structured interviews. The material was analysed using qualitative content analysis. The results showed that the dental hygienists experienced their work environment as motivating and facilitating, but at the same time as trying. The three categories revealed a theme: Being controlled in a modern environment characterized by good relationships. Motivating factors were the good relationship with co-workers, managers and patients, seeing the results of your work, having your own responsibility and making your own decisions. The new, pleasant and modern clinics, good cooperation between co-workers and varying duties were described as facilitating factors. The trying factors, as described by the dental hygienists, were above all being controlled by time limits or by some elements of the work, such as teamwork. The dental hygienists also felt stress because appointments were too-short. To conclude, the participants described their work environment as trying in several ways, despite the modern clinics and good relationships.

    • Objective: The aim of this study was to assess the effect of oral health on aspects of daily life measured by the Dental Impact Profile (DIP) in 35- to 47-year-old individuals in Norway, and to study associations between reported effects and demographic variables, subjectively assessed oral health, general health, oral health behaviour and clinical oral health.Material and methods: A stratified randomized sample of 249 individuals received a questionnaire regarding demographic questions, dental visits, oral hygiene behaviour, self-rated oral health and general health and satisfaction with oral health. The DIP measured the effects of oral health on daily life. Teeth present and caries experience were registered by clinical examination. Bi- and multivariate analyses and factor analysis were used.Results: Items most frequently reported to be positively or negatively influenced by oral health were chewing and biting, eating, smiling and laughing, feeling comfortable and appearance. Only 1% reported no effects of oral health. Individuals with fewer than two decayed teeth, individuals who rated their oral health as good or practised good oral health habits reported more positive effects than others on oral quality of life (P = 0.05). When the variables were included in multivariate analysis, none was statistically significant. The subscales of the DIP were somewhat different from the originally suggested subscales.Conclusions: This study showed that most adults reported oral health to be important for masticatory functions and confirmed that oral health also had impacts on other aspects of life.

    • Dental health has mostly been measured by dental staff disregarding patient’s experiences. However, clinical conditions alone do not fully indicate how people feel affected by their oral status. The aim of this study was to investigate how clinical recorded dental health, self-rated dental health, satisfaction with dental health were related to oral health-related quality of life (OHRQoL) assessed by Oral Health Impact Profile (OHIP-14) in 68–77 years old. A total of 151 individuals completed a questionnaire on self-rated dental health, satisfaction with dental health and the short form of OHIP-14. Clinical examination was performed registering number of teeth and dental caries. In total 63% of the individuals rated their dental health as good, and 59% were satisfied with their dental health. Using the OHIP-14 42% reported no problems or oral discomfort at all. The proportion of individuals reporting problems or discomfort varied between 13% and 43% according to the dimensions of OHIP-14. The most frequently reported problems were physical pain (43%), psychological discomfort (28%) and psychological disability (28%). Individuals who rated their dental health as poor and those who were dissatisfied with their dental health had significantly lower OHRQoL than other individuals. The study showed relationship between self-evaluations of dental health and OHRQoL in 68–77 years old. Individuals with few teeth reported lower OHRQoL than others, but no association between clinical caries status and OHRQoL could be found.

    • OBJECTIVES: The aim of this study was to describe the use of tobacco and changes in its use over time among individuals living in Jönköping, Sweden, and to analyse tobacco habits in relation to socioeconomic conditions, personality aspects and dental care habits. METHODS: This study comprised three epidemiological cross-sectional studies, involving a random selection of individuals aged between 15 and 70 years, and was conducted in 1983, 1993 and 2003. The participants were asked to complete a questionnaire. RESULTS: The results revealed a statistically significant reduction from 34% tobacco users in 1983 to 27% in 1993 and 28% in 2003. The main decrease was seen among smokers. At the same time, the number of users of snuff increased in all the age groups between 20 and 60 years of age. The use of tobacco was therefore largely unchanged in 1993 and 2003. In 2003, there was a statistically significant difference between users and non-tobacco users when it came to the frequency of dental visits; more tobacco users than non-tobacco users did not visit a dentist at all or did not visit a dentist regularly. In 1993, non-tobacco users brushed their teeth more frequently than tobacco users and this difference was statistically significant. CONCLUSIONS: During the 20-year study, there was a reduction in the number of smokers and an increase in the number of snuff users. There was a difference between tobacco users and non-tobacco users when it came to the frequency of dental visits and oral hygiene habits.

    • Objectives: To describe and evaluate an individually tailored treatment programme based on a behavioural medicine approach to oral hygiene self-care for patients with chronic periodontitis.Methods: Two experimental single-case studies with multiple-baseline design across different self-administered oral hygiene behaviours were conducted. Cognitive Behavioural techniques were used to organize the strategies for the intervention and the approach to counselling was inspired by and structured in accordance with Motivational Interviewing. The central features in the programme were the individual analysis of knowledge and oral hygiene habits, individually set goals for oral hygiene behaviour, practice of manual dexterity for oral hygiene aids, continuous self-monitoring of the behaviour and prevention of relapse.Results: Both participants reached the predecided criteria for clinical significance in reducing plaque and bleeding on probing. Reductions of periodontal probing depth were achieved as well. The positive results remained stable throughout the 2-year study period.Conclusion: The successful application of this educational model suggests that it could be used as a method for tailoring interventions targeted to oral hygiene for patients with periodontal conditions. The programme will now be tested in a larger randomized controlled trial.

    • Periodontitis is an infectious disease that impacts people's oral health and can lead to the loss of teeth. There are several factors that explain why some people develop this disease. PURPOSE: The aim of this study was to describe patients' perceptions of living with periodontitis. METHOD: A qualitative method inspired by phenomenography was chosen, because the focal point was the patients' perceptions of this phenomenon. A total of 10 patients, all of whom were being treated at a specialist clinic for periodontology in the south of Sweden, were interviewed. RESULTS: Two main categories were identified: perceptions of disease and perceptions of having the disease under control, together with subcategories. The first category highlights the patients' perception of what causes periodontitis, being given the diagnosis, after the diagnosis and the consequences of periodontitis. The other main category highlights the patients' perception of their own responsibility and the professionals' responsibility. The patients' oral health-related quality of life had been influenced by periodontitis. There is no guarantee that the patient has understood what is actually required of him/her during treatment, despite information about the diagnosis. It can often take some time for patients to understand the nature of their disease. CONCLUSION: Acceptance and control of the disease result in less inconvenience in the patients' daily lives. It creates an optimistic future perspective, despite feelings of anxiety about facing consequences such as tooth loss.

    • Aim: To evaluate the abrasivity of different toothpastes both quantitatively and qualitatively with a profilometer technique and to correlate these findings to the radioactive dentin abrasivity (RDA) value. Materials and methods: Acrylic plates were exposed to brushing in a brushing machine with 11 different toothpastes. The results were evaluated using a profilometer. A surface roughness value (Ra-value), that is, a qualitative measurement, and also a volume value, that is, a quantitative measurement, were calculated from the profilometer results. These values were then correlated to each other and to the RDA-value. A comparison between RDA, Ra and volume measurements was performed using linear fitting procedure. Results: The results showed that the correlation between RDA and Ra measurements was low (R-2 = 0.04) and also that the correlation between RDA and volume measurements was low (R-2 = 0.00002). Correlation between Ra and abraded volume was high (R-2 = 0.87). Conclusions: It is important to consider not only the RDA-value when evaluating toothpaste abrasivity. From the profilometer results both a quantitative (volume) and qualitative (roughness) measurement of the abrasivity of a toothpaste can be obtained.

    • Objectives: To explore and describe attitudes towards tooth-brushing among Iranian adolescents.Methods: A series of focus-group sessions were held with 37 Iranian adolescents in schools. The groups comprised five to eight adolescents. All focus-group discussions were tape-recorded and then transcribed verbatim. All transcripts, codes and categories were read several times to extract a theme. Data were analysed using a qualitative content analysis approach.Results: Four major categories emerged from the analysis: brushing teeth is a necessary evil, parental influence on not brushing teeth, brushing teeth is insignificant, and brushing teeth is a health hazard. The theme identified in the latent content described that tooth-brushing is not part of the adolescents’ activities of daily living.Conclusions: Health educators should stress on the engagement of parents, awareness of the adolescents on brushing techniques and causes of toothache, and address any misconceptions regarding tooth-brushing.

    • Aim: To evaluate the relative abrasivity of different toothbrushes both qualitatively and quantitatively. Materials and Methods: Acrylic plates were exposed to brushing in a brushing machine with ten different toothbrushes with water alone and with a toothpaste. The results were evaluated using a profilometer after one and 6 h of brushing (corresponding to 2000 and 12 000 double strokes, respectively). A surface roughness value (Ra-value) and also a volume loss value were calculated from the profilometer measurements. These values were then compared to each other. Results: The results showed that brushing with water alone caused less abrasion than when a toothpaste was added. Six-hour brushing with water caused less abrasion than 1 h with a toothpaste. The number of filaments or filament diameter influenced the results in various ways. When brushing with water, the harder toothbrush (Jordan Medium) caused more abrasion (higher Ra-value), but when adding the toothpaste, the softer toothbrush (Jordan soft) caused more abrasion after 12 000 double strokes. Conclusion: Besides supporting the fact that a toothpaste is needed to create a significant abrasion, this study also showed that a softer toothbrush can cause as much and in some cases more abrasion than harder ones. When conducting abrasivity studies, it is important to look at both the quantitative and qualitative aspect of abrasivity.

    • The oral health of dependent residents at special facilities has often been reported as being poor, but it is difficult to relate oral health to general health and define the need for oral health care. Microbiological analyses of the oral flora have been suggested as a suitable method for evaluating oral health in this group of patients. A study was performed at a nursing facility where 33 individuals participated. The aim was to describe their oral flora in relation to other health variables and to classify the residents on different risk levels. An oral examination of the residents was made at the facility, together with a 3-day food record and an oral microbiological analysis. The analysis classified the residents in different categories according to both acid-producing bacteria and the flora correlated with a reduction in general health. The categories were based on previous studies conducted at the Department of Microbiology (Faculty of Odontology in Göteborg). The present study revealed that the level for acid-producing bacteria was high in 12 individuals and the micriobial level according to decreased general health was high in seven individuals. A high level of acid-producing bacteria was related to functional impairment, which was in turn related to nutritional problems and help with oral hygiene. The microbial level according to the reduction in general health did not significantly correlate with other variables.

    • Aim: The aim of this study was to compare the usefulness of two different questionnaires assessing oral health-related quality of life (OHRQoL) at the basic examination and after initial dental hygiene treatment (DHtx).Methods: A total of 42 patients referred for periodontal treatment completed the Oral Health Impact Profile (OHIP-14) and the General Oral Health Assessment Index (GOHAI) at the basic periodontal examination. They underwent DHtx and completed the questionnaires once again after the treatment.Results: No statistically significant differences could be found between the two assessments, neither for the total scores nor for any of the separate items of the OHIP-14 or the GOHAI. However, the GOHAI questionnaire seems to result in a greater variety in the responses indicating that the floor effect is not as pronounced as for the OHIP-14. Those who had rated their oral health as good reported significantly better OHRQoL on both questionnaires. The same pattern was found for patients who reported that they were satisfied with their teeth. After DHtx and necessary extractions, there was a statistically significant correlation between the number of teeth and the total scores on both questionnaires. No other statistically significant correlations with periodontal variables could be found.Conclusion: No statistically significant difference could be found after DHtx compared to before in regard to OHRQoL assessed with OHIP-14 and GOHAI. However, there was a greater variety in the responses with the GOHAI questionnaire; it may hereby be more useful for patients with periodontal disease.

    • The aim was to analyse frequency and proportion of Lactobacillus species in supragingival plaque in subjects with hyposalivation and the Lactobacillus species ability to ferment sucrose, mannitol, sorbitol and xylitol.Material and methods: Ten subjects treated with radiation therapy (RT), 10 subjects with primary Sjogren's syndrome (pSS), and matched controls were included. Supragingival plaque was collected interproximally 12-13 and 15-16 and analysed using cultivation technique. Lactobacillus colonies were randomly selected and stored at -70 degrees C. The Lactobacillus strains fermentation pattern was analysed using basal medium with 1% of respective carbohydrate/sugar alcohol added.Results: Lactobacilli were more frequent at site 15-16 than at site 12-13. Lactobacilli were about twice as frequent in the pSS and RT groups than in their respective control group. For those harbouring lactobacilli, the proportion of the total count at site 15-16 was 13 +/- 27% for the RT group and 0.003 +/- 0.003% for their controls, 3.2 +/- 5.8% for the pSS group and 0.06 +/- 0.2% for their controls. We have further analysed 28 of 114 collected Lactobacillus strains. All strains gave a pH <6.0 and at 95% a pH <5.5 at sucrose fermentation. Mannitol could be fermented by 82% of the strains, sorbitol by 75% and xylitol by 64%. A pH <5.5 was obtained for 82% of the strains with mannitol, 75% with sorbitol and 32% with xylitol.Conclusion: Subjects with hyposalivation have a marked increase in lactobacilli. Of the strains analysed this far 95% gave a pH <5.5 at sucrose-fermentation. A pH <5.5 was obtained for 82% of the strains with mannitol, 75% with sorbitol and 32% with xylitol. As those sugar substitutes are included in tooth pastes, chewing gums and saliva-stimulating tablets and sprays, it is likely that the lactobacilli are further promoted in subjects with hyposalivation.

    • The aim of this study was to analyse quality of life (QoL) pretreatment and up to 24months post radiation therapy (RT) in patients with head and neck (H&N) cancer.Twenty-nine patients (19 men and 10 women) with a mean age of 59±8years were included. The stimulated salivary secretion was measured and the patients completed the European Organisation for Research and Treatment of Cancer Quality of Life questionnaires (EORTC QLQ-C30 and H&N35) and the Hospital Anxiety and Depression scale (HADS) pretreatment and at 6, 12 and 24months post RT.At all time-points after RT (6, 12, and 24months), patients with hyposalivation (stimulated secretion rate ≤0.7mL/min) reported clinically significant differences (> 10 points) regarding insomnia, swallowing, social eating, dry mouth, and sticky saliva. Statistically significant differences were found for emotional functioning and insomnia at 12months (P<0.05 for both) and for sticky saliva at both 12 and 24months (P<0.05 and P<0.01). The number of clinically significant differences increased from 10 at both 6 and 12months post-RT to 14 functioning/symptom scales and single items at the 24months follow-up. At 24months post RT, 21% of patients with hyposalivation had HADS scores suggesting anxiety problems compared to 7% for those with stimulated salivary secretion rates >0.7mL/min.Patients with hyposalivation showed deterioration in health related quality of life (QoL) at 24months compared with 12months post RT. Most pronounced were problems with insomnia, swallowing, social eating, dry mouth, and sticky saliva.

DMFT was 3.8, 3.6 (mean  SD; range0–15). A majority of the individuals had periodontal pockets ≥4mm (mean 4.3, 2.6) (94.9%) and gingival bleeding on probing >20%(88.5%). Two or more decayed teeth were shown to be significantly associated (OR 4.6, CI 1.2–17.1) with one or more oral impacts on daily performances in a multivariate logistic regression analysis.Conclusions: This study shown that there is a significant association between decayed teeth and oral impacts on daily performances. More research is needed, however, for deeper understanding of oral health problems and their impacts on daily life in Zambia.

    • Aim This 3-month double-blind randomized placebo-controlled study evaluated the clinical and microbial effects of an essential oil mouth rinse used as an adjunct to mechanical plaque control by patients in supportive periodontal care. Material and methods Fifty patients were randomly allocated to an essential oil group (Listerine® Coolmint; Johnson & Johnson, New Brunswick, NJ, USA) or placebo group to rinse twice per day as an adjunct to mechanical plaque control. At baseline and after 3 months, plaque index (PI), gingivitis index (GI), probing pocket depth, bleeding on probing (BoP) and clinical attachment level were registered. Subgingival plaque samples were collected for the detection and quantification of Aggregatibacter actinomycetemcomitans, Porphyromonas gingivalis, Tannerella forsythia, Treponema denticola, Micromonas micros, Prevotella intermedia, Fusobacterium genus and Streptococcus mutans by means of real-time PCR (qPCR). Patient's compliance, satisfaction and side effects were registered. Results Twenty-three patients in the essential oil group (mean age: 57) and 21 in the placebo group (mean age: 55) with acceptable oral hygiene at intake (mean PI <1.5 on a scale of 5) adhered to the study protocol. Gingivitis index, PI and BoP significantly reduced over time (P ≤ 0.029); however, between group analyses revealed no significant differences. There was no significant change over time neither in detection frequency nor load for any of the microbiota. Daily rinsing with an essential oil rinse was found safe and perceived beneficial by the patients. Conclusion Patients in supportive periodontal care who are fairly compliant with oral hygiene may not benefit from additional mouth rinsing using an essential oil solution.

    • Objectives Adequate oral hygiene, that is self-performed infection control, is crucial to prevent periodontal disease. Epidemiological studies reveal poor oral hygiene conditions among Swedish adolescents and indicate a need for more effective prevention programs. The aim of the current study was to analyse adolescents' experiences of a person-centred, theory-based, oral health education program for improved oral hygiene. Methods Data were obtained by interviewing 19 adolescents treated by dental hygienists in accord with the person-centred education program in a preceding clinical field study ( NCT02906098). Study participants were selected to reflect a variation of male and female adolescents, treated at clinics in areas with various socio-demographic profiles within Region Vastra Gotaland, Sweden. Interviews were audio-taped, transcribed verbatim and analysed with qualitative content analysis. Results A main theme was identified: 'Adolescents on a guided and challenging journey towards beneficial oral hygiene behavior'. The results elucidate the importance of a person-centred approach in therapy. The adolescents described insight on a personal level about the importance of improved oral hygiene as fundamental for behavioural change. Planning and monitoring of the behaviour, with guidance and support by the dental hygienist, was considered to facilitate change and encouraged further behavioural efforts. However, the adolescents expressed a need of reminders and support to keep up oral hygiene routines over time. Conclusions The study brings knowledge on factors of importance in educational interventions to increase beneficial health behaviours among adolescents and emphasize areas for further improvements of such interventions.

Methods: Semi-structured interviews were conducted with 13 DHs who had applied the person-centred, theory-based, behavioural intervention directed at adolescents with poor oral hygiene in a field study within the Public Dental Service, Region Västra Götaland, Sweden. The interviews were audio-taped, transcribed verbatim and analysed using qualitative content analysis. Results: The main theme ‘From individual experts to partners – DHs changing direction from a disease-centred towards a person-centred approach’ illustrated a changed professional approach among DHs, from exerting their roles as experts to encouraging partnership in treatment by supporting the adolescents in taking health behavioural decisions and responsibility for their oral health. The DHs considered the changed approach as challenging, but also more enjoyable, compared to conventional information/instruction. Adequate knowledge and skills, personal interest and willingness for a change as well as support from colleagues and clinic management were identified as prerequisites for implementing the person-centred, theory-based, behavioural intervention in daily practice, while the expenditure of time needed in relation to economic demands in care were seen as barriers. Conclusions: The findings elucidate that DHs considered the application of a person-centred, theory-based, behavioural intervention to be challenging but also enjoyable. For such an intervention to be implemented in daily practice, prerequisites and barriers need to be considered on both personal/professional and organizational levels.

    • Objective: To investigate attitudes to dental care, and to assess possible associations with socio‐economic and clinical variables over a period of ten years, and to investigate the association between OHRQoL assessed by oral impact on daily performance (OIDP), and socio‐economic, dental care habits, smoking and oral status.Materials and methods: Cross‐sectional studies performed in the county of Dalarna, Sweden, in 2003, 2008 and 2013. Random samples of 1,107‐1,115 dentate individuals, aged 30‐85 years, who answered a questionnaire and who were radiographically and clinically examined were included.Results: The importance of preventive treatment, regular recalls and meeting the same caregiver as on previous visits became less important. In individuals with alveolar bone loss, meeting the same caregiver as on previous visits was important (P<.05). In individuals with manifest caries, information on treatment cost was important, while prevention became less important (P<.05). OIDP was reported by 31% of the individuals in the study, and frequent impact was reported by 10%. Individuals with manifest caries lesions, less than 20 remaining teeth, and temporomandibular disorders (TMD) reported OIDP to a significantly higher degree, compared to orally healthy individuals.Conclusion: Attitudes important in maintaining and improving good oral health, such as preventive care and regular recalls to dentistry, became less important during this period of 10 years. Oral impact was found to be associated with irregular dental visits and limited economy for dental care, individuals with less than 20 remaining teeth, TMD and manifest caries.
