    • This is a case report of a 51-year-old woman with bilateral stress fractures of the pelvic rami and a history of anorexia nervosa (AN). AN is a psychiatric condition of low weight caused by restricted food intake, impaired body image and an exaggerated fear of gaining weight in addition to compensating behaviour such as excessive physical activity. Among patients with AN, reduced bone density is common, and a higher risk of fractures is present. Stress fractures should be suspected in patients, who have AN and experience pain from the musculoskeletal system without a history of trauma.

  • [Sexually transmitted infection as a cause of anal cancer]

    • Interviews were carried out with 423 women and 93 men with invasive or in situ anal cancer in Denmark and Sweden in a search for clues to the aetiology of this neoplasm. Patients with rectal adenocarcinoma (n = 534) and persons drawn from the background population (n = 554) served as controls. Multivariate logistic regression analyses confirmed previous observations of a strong association between either male homosexual experience or a history of anogenital warts and the risk for anal cancer. Moreover, hitherto unknown, but strong and consistent associations were observed between measures of high heterosexual activity and the risk for anal cancer among both sexes. Polymerase chain reaction analysis revealed human papilloma-virus DNA in the majority (88%) of anal cancer specimens but in none of 20 examined rectal adenocarcinomas. It is concluded that most anal cancers appear to be caused by sexually transmitted types of human papillomaviruses and, consequently, that anal cancer is a potentially preventable neoplasm.

    • Patients with diabetes have an unfavourable prognosis after an acute myocardial infarction. The DIGAMI 2 study investigated the effect of various metabolic treatment strategies in type 2 diabetic patients with acute myocardial infarction: acutely introduced, long-term insulin treatment did not improve survival when compared with conventional management at similar levels of glucose control. However, good glucose control seems important since the glucose level was found to be a strong predictor of long-term mortality in this patient category.

    • The refeeding syndrome (RFS) is a potentially fatal condition involving fluid and electrolyte imbalances after refeeding in patients with anorexia nervosa. Low-calorie diet added thiamine and minerals is the standard approach to prevent RFS. In a recent systematic review starting with a higher calorie amount than earlier has been recommended, and in another review, it is proposed that a restriction in the amount of carbohydrates may allow for a higher calorie intake early on to enable a safe and faster weight gain. There are still many unanswered questions, but these studies may point to a future change in the guidelines.

    • INTRODUCTION: Functional Magnetic Resonance Imaging (fMRI) allows important functions in the brain cortex to be mapped noninvasively. The purpose with this work was to investigate a possible correlation between the distance from the tumour margin to fMRI activity and postoperative neurological deficits by means of a standardised method for measurement of distance. A second purpose was to investigate the influence of preoperative fMRI on the neurosurgical decision-making process. MATERIAL AND METHODS: Retrospective study of 25 patients. The inclusion criteria were surgery or biopsy after fMRI plus a three-month postoperative assessment. A total of 14 patients complied with these requirements (six men and eight women, the mean age was 39 years). fMRI raw data was collected using a three tesla magnetic resonance scanner (Signa HDx R14M5, GE Healthcare). The distance from the tumour margin to fMRI activation was measured using GE-reformat version 4.2 after raw data had been postprocessed using GE BrainwavePA version 1.3.08130. The neurosurgeons valuation of fMRI in the preoperative decision-making process was obtained using a standard questionnaire. RESULTS: There was a trend of association between distance from tumour to eloquent functional areas and the patients' postoperative neurological outcome (Fisher's exact test: distance < 15 mm, p = 0.43; distance < 10 mm, p = 0.14). fMRI proved very useful when deciding whether to operate or not (42%), when deciding the surgical approach (50%) and when deciding the extent of the surgical approach (83%). CONCLUSION: The standardised method for measurement of distance between tumour margin and fMRI activity can contribute to the preoperative risk assessment in patients with brain tumours.

    • Patients, health-care professionals and the public expect safe health care. The system, however, is not safe and patients are being harmed. Workplace and organizational conditions and human factors contribute to these harms and a system approach is needed to avoid them. In clinical microsystems (CMS), the frontline units of health care, staff and patients can make care safer by learning about their system, its processes, members and purposes. Approaches from patient safety should be integrated in the daily work of every member of the CMS to reduce risk, implement safe practices and learn from errors. We summarize methods for use in CMS and offer ideas for fostering a proactive culture of patient safety.

    • Eighty patients with monoradicular sciatica were examined by myelography, computed tomography (CT) and magnetic resonance imaging (MRI) and all had subsequent surgery. The images were evaluated by a decision-analytic regret function. The largest amount of diagnostic information was gained from CT followed by MRI and myelography. Myelography was not significantly informative. The results suggest that CT or MRI should be the first choice examination in patients with suspected lumbar disc herniation.

Lægediagnosticeret astma i Danmark er øget fra 4 til 7-10%, og allergisk rinitis eller kronisk ikkeallergisk rinitis er øget fra 10% til 20-25% over en 20-årig periode [2]. Astma er den hyppigste kroniske sygdom hos unge og unge voksne, og en høj forekomst af astma i befolkningen må naturligt følges af en høj forekomst af astma hos idrætsudøvere, men astma er ikke noget, som idrætsudøvere generelt synes at have forud for deres debut som eliteidrætsudøvere.

    • Remicade/infliximab is effective in rheumatoid arthritis (RA), but response failure is frequent. Sera from 106 RA patients were monitored using an RIA for functional infliximab and an RIA for anti-infliximab antibody (Ab). S-infliximab varied considerably, e.g. 0-22 microg/ml before the 3rd infusion, and 44% were Ab-positive after 6 months. Low s-infliximab was associated with Ab development and later therapeutic failure, and high Ab levels could be related to dose increases, side-effects and cessation of therapy. Pharmacological monitoring should help optimize anti-TNF therapies.

    • Injuries and diseases of the hand naturally have an enormous impact on hand function and on quality of life, both occupational and social. The majority of hand-injury patients are under 30 years of age. Hand surgery, an established specialty in Sweden since 1969, is of great importance in terms of clinical developments, education and research. In the coming decade, scientific and clinical advances are to be expected in several fields such as nerve injuries including brachial plexus lesion, microsurgery, flexor tendon injuries and tendon transfer. Bioimplant research and new advances at the biotechnological interface will yield new options in nerve reconstruction, microchip implants in the nervous system, and the restoration of muscle-tendon function following injury. Artificial limbs with advanced motor and sensory functions will be important future aids in the rehabilitation of amputees. Transplantation of human hands is another promising reconstructive procedure which may open iup new perspectives in the coming millennium.

    • Lifetime risk of developing breast cancer for Danish women is approx. 9%. Diagnostics are done by mammography, biopsy, and examination. Breast-conserving surgery followed by radiotherapy is standard for treatment of early-stage breast cancer. Studies report unsatisfying outcomes in up to one third of the patients. This review summarises the current surgical techniques. Oncoplastic breast surgery makes it possible to combine wide-margin oncological surgery with immediate partial breast reconstruction, thus improving cosmetic outcome. Techniques involve volume reduction, volume displacement and volume replacement, depending on breast size and tumour characteristics.

    • Malignant melanoma (MM) is a frequent form of cancer with increasing incidence. 6-10% of patients with MM report a family history of MM, and in most populations 2% of unselected cases of MM carry a CDKN2A mutation. tvWe present a family with 24 cases of MM in nine persons from several generations, caused by a previously undescribed germ-line intronic mutation in CDKN2A. Through genetic counselling and genetic testing high-risk persons in the family are located and offered regular screening for MM.

    • Acute acalculous cholecystitis (AC) is an inflammatory process of the gallbladder, often caused by other severe diseases. The etiology is multifactorial, but the pathogenesis is in all cases a reduction in the gallbladder's emptying capacity. The mortality of AC reaches 85% without treatment. This fact makes it important to consider AC as a possible complication to major surgery, multi-trauma, and artificial ventilation. Symptoms resemble "normal" cholecystitis, but may be masked by the underlying severe disease. Fever without known focus may be the only symptom. Ultrasound, CT-scanning or cholangiography are used to verify the diagnosis, supplemented by laboratory investigations. Treatment is cholecystectomy. Percutaneous ultrasonic guided cholecystotomy is a good alternative if the diagnosis made in an early stage. The mortality is considerably reduced by early intervention.

    • A Cochrane metaanalysis and a study performed on children have recently confirmed that therapeutic hypothermia does not improve outcome after severe traumatic brain injury (TBI). TBI is not comparable to a short episode of global ischemia, where therapeutic hypothermia has been shown to improve outcome. The difference may be explained by the fact that hypothermia-induced stress after a traumatic brain injury reduces cerebral perfusion in the penumbra zone, where local circulation is already reduced. Thus, to date there is no indication for therapeutic hypothermia in TBI patients.

    • INTRODUCTION: A questionnaire study was conducted among Danish general practitioners (GPs) as part of an international evaluation of the Institute for Rational Pharmacotherapy (IRF). The aim was to investigate GPs' use and opinion of the IRF's activities. MATERIAL AND METHODS: A questionnaire was sent to a random sample of 500 GPs, who were asked about their level of knowledge and frequency of use of IRF's activities, as well as their opinion on the relevance, credibility and independence of the IRF's activities. RESULTS: The response rate was 59%. IRF is generally not the first source of information about new drugs, but is frequently used in the search for information about comparability between drugs and in relation to sudden drug warnings. IRF's Rational Pharmacotherapy bulletin and its GP courses, which nine out of ten GPs knew about, were considered to be highly relevant by 80% and 71%, respectively. All of IRF's activities were considered highly credible. Most GPs felt that the IRF's information increased their confidence in prescribing decisions (84%) and supported them in their role as a GP (88%). CONCLUSION: IRF's activities support the GP's prescribing role through the production of credible, neutral and evidence-based pharmacotherapeutic information that is not available elsewhere. IRF could further refine its dissemination methods, however, in order to reach more GPs and to increase the use of its information.

    • Thrombotic microangiopathy can be caused by several conditions which are difficult to diagnose from the clinical presentation alone. Deficient enzyme activity of a newly-discovered enzyme, ADAMTS-13, can lead to thrombotic thrombocytopenic purpura (TTP). Lack of ADAMTS-13 activity causes increased concentrations of high molecular weight von Willebrand factor forms and increased platelet aggregation. Measurement of ADAMTS-13 activity is useful for the diagnosis of TTP and may also be relevant as a prognostic test for recurrent TTP.

This article is based on a systematic review of the literature and describes epidemiology, etiology, pathogenesis, diagnostic positional tests and treatment using canalith repositioning, liberatory and 360 degrees rotation manoeuvres. Diagnostic pitfalls, mastoid vibration, postmanoeuvre activity restriction and the ideal number of repositioning manoeuvres per treatment session are discussed.

    • The crucial aspect of open as well as laparoscopic rectal cancer surgery is to find the correct dissection plane to avoid damage of the nerves and to create a perfect specimen. By doing so, a "specimen oriented" resection will be achieved and the risk of a positive circumferential resection margin minimized. Currently, and for the foreseeable future, open surgery remains optimal for complex cases and in cases where a low difficult restorative resection is needed. Optimal surgery remains a crucial part in the curative treatment of this technically challenging cancer.

    • INTRODUCTION: General practitioners are often interested in doing research but are hampered by lack of time and research training. Interpreting the results of others can also be difficult. For this reason a course in basic research methods for GPs was started in Sweden in 1989. It was originally aimed at GPs but was later extended to hospital physicians as well. The course is given regionally and at present is held in six different locations in southern Sweden as well as in Hillerod, Denmark. The aim of this study is to evaluate the course as part of a research project to recommend changes to the courses in accordance with the course evaluations. MATERIALS AND METHODS: The course consists of theory (lectures/seminars six hours a month) and practice (project work) over a period of 18 months. Questionnaires were mailed to the 112 physicians who, starting in 1997 and 1999, completed the course. Eighty-five percent responded to the questionnaire, which asked if the course goals were relevant and if they thought they had achieved them. RESULTS: The most frequent reasons given for attending the course were a desire to learn how to read scientific articles critically and how to carry out one's own research projects. Two thirds of the participants thought that the theoretical lectures and project work had supplemented each other well. Most of the participants thought that the goals were very relevant but fewer--between 57% and 77%--felt those goals had been achieved. DISCUSSION: The course provides important empirical information with regard to future specialist training in Denmark. The plan is for all physicians to have research training--amounting to a total of 60 days--with lectures in research methodology and tutored research projects. The Danish College of General Practitioners has planned research training in accordance with the experience from these courses.

    • Dupuytren's disease is a hereditary fibroproliferative disease commonly affecting the palmar fascia of the hand, which results in progressive and irreversibly fixed flexion contractures of the hand. It may occur in the sole of the foot or in the penis. Generally, treatment has been up against a high frequency of complications and recurrence, but promising results are reported from less invasive treatment options, e.g. needle fasciotomy and enzymatic fasciotomy with collagenase. This is a detailed, comparative review of available non-operative and surgical treatments with focus on the enzymatic fasciotomy.

    • INTRODUCTION:This study investigated whether heads of state commonly regarded as dictators have a higher prevalence of partially treated supralabial hirsutism (PTSLH), commonly called a moustache, than non-dictatorial states. Design: retrospective observational study. Setting: the world political arena from 1901 to 2000. Participants: cohort of 139 dictators, 122 preceding political leaders in the respective countries, 122 succeeding political leaders also in the respective countries, as well as 76 Nobel peace prize laureates as controls. Interventions: none. Main outcome measures: the prevalence of PTSLH was 122 preceding political leaders.RESULTS: Of 139 dictators 49 (35%) demonstrated photographic evidence of PTSLH, while 85 (61%) did not. Of 48 preceding leaders 22 (46%) had PTSLH (p = 0.18 compared to dictators); of 33 following leaders ten (30%) had PTSLH (p = 0.59 compared to dictators). Finally of 78 Nobel peace prize laureates 31 (40%) had PTSLH (p = 0.47 compared to dictators).CONCLUSIONS: Most dictators did not have PTSLH. Moreover, the prevalence of PTSLH among dictators did not differ from controls. These data do not lend support to the commonly held notion that PTSLH is a predictor of a future dictatorial political career. Electorates the world over can now support political candidates with PTSLH without increased fear of becoming victims of a dictatorial system or having an increased risk of mortality after their ascent to power.

    • The present paper reviews studies carried out by the Bandim Health Project during the armed conflict in Guinea-Bissau, 1998-1999. Common health interventions like vaccination and breastfeeding, as well as focused interventions such as nutrition interventions aimed at malnourished children are effective measures. Population-based studies of health status gives important information about health conditions for groups at special risk, and provides information about which interventions are the most effective in an emergency.
