  • Strategies in treatment of tendon overuse injury. The chronic painful tendon.

    • The etiology and pathogenesis to chronic tendon pain is unknown, and treatment is known to be difficult. Treatment is often based on opinions and not findings in scientific studies. Recent research, using the intra-tendinous microdialysis technique, has shown that in chronic painful Achilles-, patellar-, and extensor carpi radialis brevis (ECRB) tendons, there were no signs (normal Prostaglandin-2 levels) of a so-called chemical inflammation. Furthermore, in biopsies from chronic painful Achilles tendons, pro-inflammatory cytokines were not up-regulated, again showing the absence of an intra-tendinous inflammation. Consequently, if the purpose is to treat a chemical inflammation, there is no science backing up for treatment of theses conditions with anti-inflammatory agents (NSAIDs, corticosteroidal injections). Interestingly, Substance-P (SP) and Calcitonin Gene Regulated Peptide (CGRP) nerves have been demonstrated in close relation to vessels in biopsies from these chronic painful tendons, indicating the existence of a possible so-called neurogenic inflammation. Using ultrasonography (US) + color Doppler (CD), and immunhistochemical analyses of biopsies, a vasculo/neural (SP- and CGRP-nerves) ingrowth in the chronic painful tendinosis tendon, but not in the pain-free normal tendon, has recently been found. A specially designed treatment, using US- and CD-guided injections of the sclerosing agent Polidocanol, targeting the neovessels outside the tendon, has in pilot studies on chronic painful Achilles-, and patellar tendons been shown to cure the tendon pain in the majority of patients. A recent randomized double-blind study, verified the importance of injecting the sclerosing substance Polidocanol.

  • Blood lactate recovery and respiratory responses during diagonal skiing of variable intensity

    • The aims of the study were to investigate blood lactate recovery and respiratory variables during diagonal skiing of variable intensity in skiers at different performance levels. Twelve male cross-country skiers classified as elite (n=6; VO2max=73±3 ml. kg-1. min-1) or moderately trained (n=6; VO2max=61±5 ml. kg-1. min-1) performed a 48-min variable intensity protocol on a treadmill using the diagonal stride technique on roller skis, alternating between 3 min at 90% and 6 min at 70% of VO2max. None of the moderately trained skiers were able to complete the variable intensity protocol and there was a difference in time to exhaustion between the two groups (elite: 45.0±7.3 min; moderately trained: 31.4±10.4 min) (P<0.05). The elite skiers had lower blood lactate concentrations and higher blood base excess concentrations at all 70% workloads than the moderately trained skiers (all P<0.05). In contrast, VE/VO2 and VE/VCO2 at the 70% VO2max workloads decreased independently of group (P<0.05). Partial correlations showed that VO2max was related to blood lactate at the first and second intervals at 70% of VO2max (r=-0.81 and r=-0.82; both P<0.01) but not to VE/VO2, VE/VCO2 or the respiratory exchange ratio. Our results demonstrate that during diagonal skiing of variable intensity, (1) elite skiers have superior blood lactate recovery compared with moderately trained skiers, who did not show any lactate recovery at 70% of VO2max, suggesting it is an important characteristic for performance; and (2) the decreases in respiratory exchange ratio, VE/VO2, and VE/VCO2 do not differ between elite and moderately trained skiers.

  • Modern unexplored martial arts : what can mixed martial arts and Brazilian Jiu-Jiutsu do for youth development?

    • Contemporarily, two martial arts have emerged as highly popular among youth; Mixed Martial Arts (MMA) and Brazilian Jiu-Jiutsu (BJJ). Despite their popularity, we know little of how they affect individuals sociopsychologically. The current study sought to explore how the currently underexplored martial art disciplines may contribute to sociopsychological development among young people. In addition, it was investigated whether individuals who are predisposed to different traits may favour one sport over the other. This study was conducted with a longitudinal design; over the course of 5 months, 113 participants completed training in either condition. The results show that both groups displayed increased self-control and pro-social behaviour; however, MMA practitioners also reported increased aggressiveness, whereas BJJ practitioners experienced a decline in aggression. Accordingly, individuals who trained in MMA displayed substantially higher pre-existing aggression levels than the BJJ practitioners. The current results further corroborate research suggesting that modern martial arts and MMA may not be suitable for at-risk youth to practice, whereas traditional martial arts and sports with a healthy philosophical foundation may be effective in reducing antisocial behaviour while enhancing socially desirable behaviour among young people.

  • Vitamin D in relation to bone health and muscle function in young female soccer players

    • The present work investigated serum vitamin D (25(OH)D) status in relation to bone and muscle qualities and functions in 19 female soccer players (13–16 years) resident at northern latitude with very low sun exposure (∼32–36 h/month) during winter season (late January to early March). Serum 25(OH)D, parathyroid hormone and bone turnover markers osteocalcin (OC) and beta carboxy-terminal collagen cross-links (β-Ctx), as well as body composition and muscle performance were examined. Hormones were tested using routine laboratory methods. Fat mass, lean mass, and bone mineral density in whole body, as well as femur and lumbar spine were evaluated with dual-energy X-ray absorptiometry. Muscle performance was assessed through isokinetic knee extension and flexion, countermovement jump, and sprint running. 25(OH)D was low (50.5 ± 12.8 nmol l−1), whereas the values of bone turnover markers were markedly high (OC: 59.4 ± 18.6 μg l−1; β-Ctx: 1075 ± 408 ng l−1). All bone and muscle measurements were normal or above normal. 25(OH) D was not significantly correlated with most of the parameters of bone and muscle quality or function, except the knee extension time to peak torque (r = −0.50, p = .03). In conclusion, the level of vitamin D is markedly low in adolescent female soccer players during the winter in Sweden. However, vitamin D levels did not significantly correlate with measures of bone and muscle except a moderate correlation in time to peak torque in the knee extensors. The practical implication of low vitamin D levels in young growing female athletes remains unclear.

  • The impact of social influence on physical activity among adolescents: a longitudinal study

    • Abstract in Undetermined Objectives: The purpose of the current study was to examine the impact of social support and negative social influence from various contexts on adolescents’ current and intended physical activity (C&IPA) two years later. In addition, social influence related to C&IPA at baseline (16–17 years old) is compared to follow-up (18–19 years old). Methods: Questionnaire data was obtained from 404 students at the age of 16–17 and again at the age of 18–19. Relationships among variables were examined with structural equation modelling (SEM). Results: Social influence experienced by high-school students at 16–17 years of age had a direct suppressing effect and an indirect positive effect on their C&IPA two years later. Social support for physical activity was more complex and related to positive as well as negative relations at the age of 16–17 compared to two years later. Conclusions: The results imply that social support to adolescents should be offered over a longer period of time, with increased focus on its significance as a link from an initially external regulation to a more internal regulation of physical activity as students get older. Closer examination of maintaining, pro-activating and intervening effects of social support could enhance our understanding of direct and indirect effects of social support on physical activity.

    • The capacity to predict the heart rate (HR) and speed at the first (VT1) and second (VT2) ventilatory thresholds was evaluated during an incremental ski-mountaineering test using heart rate variability (HRV). Nine skiers performed a field test to exhaustion on an alpine skiing track. VT1 and VT2 were individually determined by visual analysis from gas exchanges (VT1V and VT2V) and time-varying spectral HRV analysis (VT1fH, VT2fH and VT2H). VT1 could not be determined with the HRV methods used. On the contrary, the VT2 was determined in all skiers. No significant difference between HR and speed at VT2H and VT2V was observed (174.3 +/- 5.6 vs. 174.3 +/- 5.3 bpm, and 6.3 +/- 0.9 and 6.3 +/- 0.9 km h(-1), respectively). Strong correlations were obtained for HR (r = 0.91) and speed (r = 0.92) at VT2H and VT2V with small limits of agreement (+/- 3.6 bpm for HR). Our results indicated that HRV enables determination of HR and speed at VT2 during a specific ski-mountaineering incremental test. These findings provide practical applications for skiers in order to evaluate and control specific training loads, at least when referring to VT2.

    • This study investigates the enjoyment and cohesion of school children participating in a school-based high-intensity physical activity (PA) intervention. Both enjoyment and cohesion have been found to be important factors for adherence to regular physical and sport activity, an important outcome of PA interventions. The sample consisted of 300 pupils (mean age: 9.3 years; 52.7% female) assigned to a team sport intervention, an individual sport intervention, or a control group for 10 months. The Physical Activity Enjoyment Scale and Youth Sport Environment Questionnaire were used to measure enjoyment and cohesion. The Yo-Yo IR1C test determined fitness improvements. Results showed that enjoyment and cohesion (social) measured at the beginning of the intervention significantly predict fitness improvements achieved after 10 months. No differing developmental effects over time could be found in the intervention groups with regard to cohesion and enjoyment when comparing them to the control group. However, enjoyment and cohesion (social) significantly decreased in the groups that performed individual sports. Team sports seem to be more advantageous for the development of enjoyment and cohesion, which are both factors that positively impact the health outcomes of the intervention. © 2016 European College of Sport Science.

  • The associations between motivational climate, basic psychological needs and dropout in volleyball – A comparison across competitive levels

    • The purpose of this study was to investigate the importance of motivational climate for the satisfaction of psychological needs and dropout in recreational, intermediate and elite volleyball. Seven thousand nine hundred thirty six volleyball players from all 321 volleyball clubs across Denmark were invited to participate in the study. Three thousand three thirty answered the questionnaire and 2150 were included in the analysis. Dropout from Volleyball was measured as the proportion of players that had stopped playing volleyball over the last year. The coach-created motivational climate was measured using the Motivational Climate Scale for Youth Sports. The satisfaction of players’ psychological needs was measured using an adapted version of the basic psychological needs in exercise scale. The psychometric scales were validated and showed good model fit. For volleyball players of all levels, the degree of mastery climate predicted the satisfaction of the players’ basic psychological needs satisfaction during volleyball which was, in turn, associated with lower dropout rates. Performance climate had a weak negative association with the satisfaction of psychological needs on the intermediate level only. When adjusting the models for the negative association between performance climate and mastery climate this negative association became nonsignificant and a weak positive association to needs satisfaction emerged for players at the elite level. Findings confirm that the coach-created mastery climate in volleyball teams is important for the satisfaction of players’ basic psychological needs and continuation within the sport across the recreational, intermediate and elite levels. Highlights Coach-created mastery climate in volleyball teams was positively associated with the satisfaction of the players’ basic psychological and negatively associated with dropout. These associations between coach created climate, need satisfaction and dropout were similar across different sporting levels. Performance orientation had little influence and seemed mainly problematic if it was at the expense of mastery climate. © 2022 European College of Sport Science.

    • Our knowledge of sports-related injuries in para-sport is limited and there are no data on how Paralympic athletes themselves perceive an injury. The aim of this qualitative study was to explore Paralympic athletes’ perceptions of their experiences of sports-related injuries, risk factors and preventive possibilities. Eighteen Swedish Paralympic athletes with vision impairment, intellectual impairment, spinal cord injury, cerebral palsy, myelomeningocele, dysplasia and neuromuscular disorder, representing 10 different para-sports, were interviewed. The qualitative phenomenographic method was used to interpret the data. The analysis revealed nine categories of perceptions of experiences. The athletes perceived that their impairments were involved in the cause and consequential chains associated with a sports-related injury. Other categories that denoted and described these injuries were: sport overuse, risk behaviour, functional limitations, psychological stressors, the normalised pain, health hazards, individual possibilities to prevent sports-related injuries and unequal prerequisites. This qualitative study revealed that Paralympic athletes’ perceptions of their experiences of sports-related injuries are complex and multifactorial, and in several ways differ from able-bodied athletes. This needs to be considered in the sports health and safety work within the Paralympic Movement as well as in the design of future injury surveillance systems and preventive programmes.

  • Salivary testosterone and cortisol responses to four different rugby training exercise protocols

    • This study assessed the acute response of salivary testosterone and cortisol concentrations to four exercise protocols in 27 elite male rugby players. Each athlete completed four protocols in random order on separate in-season weeks. Two protocols were resistance training based consisting of four exercises (high pull, bench press, squat and chin-ups/prone row): Protocol 1 consisted of 5 sets of 15 repetitions at 55% of 1 repetition maximum (1 RM) with 1-minute rest (5 x 15-55%). Protocol 2 consisted of three sets of five repetitions at 85% 1 RM with 2-minute rest (3 x 5-85%). Protocol 3 was a strongman (STRNG) session consisting of three stations within a circuit of exercises that included exercises such as battling ropes, prowler push, farmer's walk and tyre flips. Protocol 4 was based on boxing and wrestling inspired exercises (combative - COMB). Salivary testosterone and cortisol concentrations were measured before (PRE) and immediately after exercise (POST). Testosterone did not significantly change as a result of any intervention, whereas cortisol declined and the testosterone to cortisol (T/C) ratio increased significantly in both the 5 x 15-55% and 3 x 5-85% protocol. When results were retrospectively grouped and analysed according to the protocol that demonstrated the greatest absolute testosterone response, significant (P < 0.01) increases for the 5 x 15-55%, STRNG and COMB protocols were observed. The individualised hormone response to exercise observed in this study highlights the importance of recognising a protocol-dependent approach to training athletes. Furthermore this study also highlights a potential usefulness of employing STRNG and COMB training protocols as an alternative stimulus to resistance training.

    • This study investigated whether commercially available compression garments (COMP) exerting a moderate level of pressureand/or neuromuscular electrical stimulation (NMES) accelerate recovery following a cross-country sprint skiing competitioncompared with a control group (CON) consisting of active recovery only. Twenty-one senior (12 males, 9 females) and 11junior (6 males, 5 females) Swedish national team skiers performed an outdoor sprint skiing competition involving foursprints lasting ∼3–4 min. Before the competition, skiers were matched by sex and skiing level (senior versus junior) andrandomly assigned to COMP (n = 11), NMES (n = 11) or CON (n = 10). Creatine kinase (CK), urea, countermovementjump (CMJ) height, and perceived muscle pain were measured before and 8, 20, 44 and 68 h after competition. NeitherCOMP nor NMES promoted the recovery of blood biomarkers, CMJ or perceived pain post-competition compared withCON (all P > .05). When grouping all 32 participants, urea and perceived muscle pain increased from baseline, peaking at8 h (standardised mean difference (SMD), [95% confidence intervals (CIs)]): 2.8 [2.3, 3.2]) and 44 h (odds ratio [95%CI]: 3.3 [2.1, 5.1]) post-competition, respectively. Additionally, CMJ was lower than baseline 44 and 68 h postcompetitionin both males and females (P < .05). CK increased from baseline in males, peaking at 44 h (SMD: 1.4 [−0.4,0.9]), but was decreased in females at 20 h post-competition (SMD: −0.8 [−1.4, −0.2]). In conclusion, cross-countrysprint skiing induced symptoms of exercise-induced muscle damage peaking 8–44 h post-competition. However, neitherCOMP nor NMES promoted physiological or perceptual recovery compared with CON.

  • An elite endurance athlete's recovery from underperformance aided by a multidisciplinary sport science support team

    • Overload training resulting in an overreached state is common in elite sports, and if undetected can develop into an overtraining syndrome. This risk is accentuated by the lack of reliable measures of overreaching. Coaches and scientists therefore have to use a combination of tests in the monitoring process. This article presents a case study of the recovery from underperformance of a young elite endurance athlete and the work of a multidisciplinary sport science support team. When it was determined that the athlete's performance had deteriorated, and that this was due solely to the stress of training, training load was radically reduced for a period of 14 days. A combination of physiological, biochemical, and psychological measurements were then used to monitor the recovery process. The purpose of this article is to describe how coaches and sport science teams can help in monitoring training and recovery in practical settings, allowing detection of the early signs of overreaching before a more serious overtraining syndrome develops.

  • Are athletes burning out with passion?

    • Passion is a strong motivational force towards an activity considered very important, possibly to the extent that the activity forms a part of an individual’s identity. Two forms of passion, harmonious and obsessive, are thought to lead to different cognitive and affective responses. Although being passionate about sport appears to be important from a motivational perspective, it possibly also increases the risk for burnout, which is a negative consequence blamed partly on too much training and inadequate recovery. The question voiced in this study is whether harmonious passion and obsessive passion pose equal risks for burnout. Participants were 94 female and 164 male competitive athletes from 21 sports. The results,  analysed using partial correlation and multivariate analyses of variance, showed that athletes with an obsessive passion scored higher on a burnout inventory than did harmoniously passionate athletes. Obsessively passionate athletes also scored higher on perceived stress and negative affect, and lower on positive affect. These findings support the assumption that even though the two forms of passion may be an integral part of elite sports, athletes scoring high on obsessive passion may be at greater risk of developing burnout than more harmoniously passionate athletes. Burning bright and burning out can thus be seen as two potential consequences for athletes driven by passion. Athletes and coaches who are aware of this may be better equipped to avoid the potential negative consequences associated with too much stress and too little recovery.

  • Associations of physical activity, sedentary time, and diet quality with biomarkers of inflammation in children

    • We investigated the associations of physical activity (PA), sedentary time (ST), and diet quality with biomarkers of inflammation in 390 children (192 girls, 198 boys) aged 6–8 years. PA energy expenditure (PAEE), light PA, moderate PA (MPA), vigorous PA (VPA), moderate-to-vigorous PA (MVPA), and ST were assessed by combined movement and heart rate sensor. Finnish Children Healthy Eating Index was calculated using data from 4 d food records. Body fat percentage (BF%) was measured by dual-energy X-ray absorptiometry. High-sensitivity C-reactive protein (Hs-CRP), leptin, interleukin-6 (IL-6), adiponectin, tumour necrosis factor-α, and glycoprotein acetyls were measured from fasting blood samples. PAEE, MPA, VPA, and MVPA were inversely associated with hs-CRP (β=−191 to −139, 95% CI=−0.294 to −0.024), leptin (β=−0.409 to −0.301, 95% CI=−0.499 to −0.107), IL-6 (β=−0.136 to −0.104, 95% CI=−0.240 to −0.001) and PAEE, MPA, and MVPA were inversely associated with glycoprotein acetyls (β=−0.117 to −0.103, 95% CI=−0.213 to −0.001). ST was directly associated with hs-CRP (β=0.170, 95% CI=0.070–0.269), leptin (β=0.355, 95% CI=0.265–0.445), and IL-6 (β=0.105, 95% CI=0.005–0.205). VPA was inversely associated with hs-CRP, leptin, and IL-6 in children with higher BF% (β=−0.344 to −0.181, 95% CI=−0.477 to −0.033) but not among children with lower BF% (β=−0.007–0.033, 95% CI=−0.183–0.184). In conclusion, PA was inversely and ST directly associated with circulating levels of biomarkers of inflammation among children. Furthermore, we observed that PA was inversely associated with these biomarkers for inflammation in children with a higher BF%. Highlights Systemic inflammation, as indicated by increased circulating concentrations of biomarkers for inflammation, may be important in causal pathways leading to insulin resistance, sub-clinical atherosclerosis, and eventually clinical manifestations of cardiovascular diseases. Higher levels of physical activity and lower levels of sedentary time were associated with more favourable inflammatory profile. Body fat percentage modified these associations and especially vigorous intensity physical activity was inversely associated with biomarkers of inflammation on children with higher body fat percentage but not in children with lower body fat percentage.

    • The present study identified profiles of perfectionism and inauthenticity at baseline and tested whether they differed in the maladaptive outcomes of controlled motivation, performance anxiety, and exhaustion after a nine-month period. We purposefully selected elite junior performers (NT1 = 219; NT2 = 156), 16–19 years of age, from Norwegian talent development schools in sports and performing arts. The participants completed questionnaires to report their perceptions of the study variables. The results of the latent profile analysis indicated a multidimensionality of perfectionism, thereby identifying four profiles. Although our identified profiles are in line with the 2 × 2 model of perfectionism; however, the results of the mean differences between the identified profiles did not align with the 2 × 2 model’s hypotheses. The elite junior performers who displayed non-perfectionism demonstrated to be the most adaptive profile. They reported the lowest level of inauthenticity and the maladaptive outcomes of controlled motivation, performance anxiety, and exhaustion. The mixed perfectionism profile, displaying high levels of perfectionistic concerns (PC) and perfectionistic strivings (PS), demonstrated to be the least adaptive profile. This profile reported higher levels of inauthenticity and was even more maladaptive than the PC dominated profile contrary to the proposed hypotheses. Findings showed that a heightened vulnerability of perfectionism seems evident in PC, independent of the reported PS levels. Because only one out of five elite junior performers were distributed in the non-perfectionism profile, the vulnerability of perfectionism might be an important risk factor to note in talent development settings. © 2020 The Author(s).     • PURPOSE: To study the effect of inhaling a beta-agonist (salbutamol) compared to placebo on skiing and cycling performance in well-trained elite athletes.METHODS: Three different exercise protocols were used, all with a cross-over double blind placebo-controlled design. Participants inhaled 800 µg salbutamol or a placebo prior to the test, which was repeated on a following day with the participants inhaling the other substance. Fifteen junior elite skiers performed four free-style high intensity sprints (1100 m/work time 3.5-4.5 min). Twelve elite cyclists carried out a short cycling protocol, starting with two 5 min submaximal workloads followed by a maximal intermittent performance test to exhaustion. Another 12 elite cyclists performed the maximal intermittent performance test to exhaustion after a 150 min long submaximal cycling protocol.RESULTS: Group mean time for the ski sprints increased, with no difference between treatment groups. In the short cycling protocol time to exhaustion was 9.1% (95% CI 52-161) lower after inhaling salbutamol compared to placebo and in the long cycling protocol time to exhaustion was 9.1% (95% CI - 121-267) lower after inhaling salbutamol compared to placebo. Blood lactate, heart rate and ventilation increased during submaximal exercise with salbutamol compared to placebo in the short cycling protocol (p < .05).CONCLUSION: This study could not confirm any positive performance effects from inhaling 800 µg salbutamol compared to placebo in skiing and high-intensity intermittent cycling performance. Instead, time to exhaustion in the maximal intermittent performance test was lower in both cycling protocols. HighlightsThere was no difference in performance time between salbutamol and placebo treatment in real-life applicable repeated ski sprints.Time to exhaustion in the maximal intermittent performance test was 9.1% lower after inhaling salbutamol compared to placebo, both when performed after 10 and 150 min of submaximal cycling.

    • The aim of the study was to examine the temporal relationship between facets of sport-specific dispositional mindfulness (i e., Awareness, Refocusing, Nonjudgemental attitude) and shooting performance in actual competitions, in a population of 25 elite biathlon athletes. Findings indicated that mindfulness facets were positively associated with shooting performance, with higher levels of Awareness, Refocusing, and Awareness being related to better shooting performance in competitions. In spite of the fact that the explained variance was quite small, even a minor shooting performance enhancement may increase an athlete's chance of winning a biathlon competition.

    • Eleven elite kayakers performed an identical weekly training schedule each week during a 3-week training-camp. The Profile of Mood States (POMS) inventory (‘‘right now’’ instructions) was completed before and after selected workouts each week to assess both training-induced mood disturbances and the extent of recovery following short and long rest. The ratio of POMS vigour to POMS fatigue scores was used as an ‘‘energy index’’. Energy index scores were compared with ratings of perceived exertion (RPE) to address the size of the training load. Performance measures were obtained during the first and last week. The results showed that the energy index decreased (pB/0.01) throughout the camp and did not return to initial values following either the short (one night) or long rest (two nights and one day). Performance measures and RPE remained unaffected throughout training, as did the POMS depression scores, indicating that the athletes experienced an overreached state but did not develop staleness. Our results suggest that repeated evaluation using POMS fatigue, vigour, and depression scores during periods of intensified training may help prevent athletes from becoming severely overreached and reduce the likelihood of staleness.

    • The aim of the study was twofold, namely (i) to determine the prevalence of symptoms of common mental disorders (CMDs) among current and retired professional football and handball players and (ii) to explore the relationship of psychosocial stressors with the outcome measures under study. A total of 1155 players were enrolled in an observational study based on a cross-sectional design. Questionnaires based on validated scales were set up and distributed among current and retired professional football and handball players by the Danish football and handball players’ union. In professional football, the highest prevalence (4 weeks) of symptoms of CMDs was 18% and 19% for anxiety/depression among current and retired players, respectively. In professional handball, the highest prevalence (4 weeks) of symptoms of CMDs was 26% and 16% for anxiety/depression among current and retired players, respectively. For both the current and retired professional football and handball players, a higher number of severe injuries and recent adverse life events (LE) were related to the presence of symptoms of CMD. Players exposed to severe injuries and/or recent adverse LE were 20–50% times more likely to report symptoms of CMD. The results suggest that it is possible to recognize the population of professional athletes that are more likely to develop symptoms of CMD. This could create the opportunity to intervene preventively on athletes that suffered from severe injury and/or recent adverse LE that could lead to a faster and safer recovery and psychological readiness to return to play. © 2017 The Author(s).     • Introduction: The syndrome of Relative Energy Deficiency in Sport (RED-S) consensus statements recognise that male athletes might have impaired fertility in terms of the Exercise Hypogonodal Male Condition (ExHMC). Thus, the aims of this study were to (1) identify risk of ExHMC in active males in various sports and (2) determine if associations between risk of ExHMC and health problems interfering with training and competition exist. Methods: A questionnaire was distributed online (November 2018–January 2019) using questions derived from the “Androgen Deficiency in the Aging Male Questionnaire” (ADAM-Q) to assess risk of ExHMC. Additional questions were included to collect information on participant demographics, injury and illness history and dietary habits. Logistic regression analyses explored differences between groups. Results: Risk of ExHMC was identified in 23.3% (n = 185) of 794 questionnaire participants. Following multivariate analyses, risk of ExHMC was independently associated with a lower than normal sex drive rating within the last month (OR 7.62, 95%CI 4.99–11.63) and less than three morning erections per week within the last month (OR: 4.67, 95%CI 3.23–6.76). Risk of ExHMC was associated with 15–21 days absence from training or competition during the previous 6 months due to overload injuries in the univariate analysis (OR = 2.69, 95% CI = 1.24–5.84). Conclusion: Risk of ExHMC and associated symptoms in this heterogeneous sample may be indicative of RED-S. Confounding factors such as over-training, medication use, fatigue and psychological stress need to be considered. Identification of male athletes exhibiting physiological symptoms associated with RED-S requires more research.

    • The present study aimed to investigate motivations for the dual career of European student-athletes living in countries providing different educational services for elite athletes: State-centric regulation–State as sponsor/facilitator (State), National Sporting Federations/Institutes as intermediary (Federation) and Laisser Faire, no formal structures (No Structure). Therefore, the European Student-athletes’ Motivation towards Sports and Academics Questionnaire (SAMSAQ-EU) was administered to 524 European student-athletes. Exploratory Factor Analysis, and Confirmatory Factor Analysis were applied to test the factor structure, and the reliability and validity of the SAMSAQ-EU, respectively. A multivariate approach was applied to verify subgroup effects (P ≤ 0.05) according to gender (i.e., female and male), age (i.e., ≤24 years, >24 years), type of sport (i.e., individual sport and team sport) and competition level (i.e., national and international). Insufficient confirmatory indexes were reported for the whole European student-athlete group, whereas distinct three factor models [i.e., Student Athletic Motivation (SAM); Academic Motivation (AM); Career Athletic Motivation (CAM)] emerged, with acceptable reliability estimates, for State (SAM = 0.82; AM = 0.75; and CAM = 0.75), Federation (SAM = 0.82; AM = 0.66; and CAM = 0.87) and No Structure (SAM = 0.78; AM = 0.74; and CAM = 0.79) subgroups. Differences between subgroups were found only for competition level (P < 0.001) in relation to SAM (P = 0.001) and CAM (P < 0.001). For SAM, the highest and lowest values emerged for Federation (national, 5.1 ± 0.5; international, 5.4 ± 0.5) and State (national, 4.5 ± 0.9; international, 4.8 ± 0.7). The opposite picture emerged for CAM (Federation: national, 3.3 ± 0.7; international, 3.5 ± 0.9; State: national, 5.0 ± 0.8; international, 5.0 ± 0.9). Therefore, despite SAMSAQ-EU demonstrated to be a useful tool, results showed that European student-athletes’ motivation for dual career has to be specifically investigated according to social contexts.

    • Fairness in sport is a widely shared meritocratic norm. Its application is usually restricted to equality of opportunity to compete for victory. Paralympic sports lay down a further challenge in that equality of opportunity must be shaped by considerations of fairness, evidenced by the development of discrete competition categories to construct fair and meaningful contests. In this article we extend these philosophical ideas to consider how Fair Equality of Opportunity might operate in the context of Paralympic sports classification. We articulate three conceptions of fairness relevant to these sports: (i) background fairness; (ii) procedural fairness; and (iii) stakes fairness.     • The purpose of this study was to investigate whether Danish children from immigrant backgrounds are less physically active than children from the ethnic majority, and to investigate the possible reasons for any differences found. Accelerometer measures of physical activity as well as questionnaire data about organised sports, family demography, resources and values were collected from 594 children of whom 67 had other ethnic background than Danish. Data were collected when the children were 6-7 years old and again later when the children were 9-10 years old. It was found that children from immigrant backgrounds were not less physically active than other children when their amounts of daily physical activity were measured by direct objective measures, despite their participation rate in organised sports being much lower. Using multiple logistic regression modelling, this study showed that lack of parental experience with organised sports and lack of economic/material resources explained much of the difference in sports participation. Children of immigrant background had significant lower participation in club sports but this did not affect their overall physical activity level.

    • The purpose of this double-blinded, crossover randomized and counterbalanced study was to compare the effects of ingesting a tepid commercially available carbohydrate-menthol-containing sports drink (menthol) and an isocaloric carbohydrate-containing sports drink (placebo) on thermal perception and cycling endurance capacity "in a simulated home virtual cycling environment". It was hypothesized that the addition of menthol would improve indicators of thermal perception and improve endurance exercise capacity. Twelve healthy, endurance-trained males (age 29 +/- 5 years, height 181 +/- 6 cm, body mass 79 +/- 2 kg and V?O(2)max 57.3 +/- 6.4 mL kg(-1) min(-1)) completed two experimental trials on a stationary bicycle without external air flow. Each trial consisted of (1) cycling for 60 min at 90% of the first ventilatory threshold while receiving a fixed amount of menthol or placebo every 10 min followed immediately by (2) cycling until volitional exhaustion (TTE) at 105% of the intensity corresponding to the respiratory compensation point. TTE did not differ between both conditions (541 +/- 177 and 566 +/- 150 s for menthol and placebo; p > 0.05) and neither did ratings of perceived thermal comfort or thermal sensation (p > 0.05). Also, the rectal temperature at the end of TTE was comparable between menthol and placebo trials (38.7 +/- 0.2 degrees C and 38.7 +/- 0.3 degrees C, respectively; p > 0.05). The present results demonstrate that the addition of menthol to commercially available sports drink does not improve thermal comfort or endurance exercise capacity during similar to 65 min of intense virtual cycling.

    • Musculoskeletal injuries are the most common reason military service members cannot perform their military duties. Not only are they costly and associated with long-term disability, often long after completion of military service, but injuries also adversely affect the military readiness of a nation. This can be seen as a threat to national security and part of the impetus behind many efforts to better understand, predict, and mitigate injury risk in the military. A systematic review of literature published between 1995 and October 31, 2020 was conducted to identify significant risk factors of musculoskeletal injury in military populations across the world. 74 out of 170 eligible studies addressed comprehensive injuries, providing 994 unique risk factors. 46 of these studies provided data that could be included in a meta-analysis, which was possible for 15 predictor variables. Seven predictors were significant in meta-analysis: female sex(RR=1.46;95CI 1.30,1.64), high body mass index(RR=1.36;95CI 1.21,1.53), functional movement screen pain (RR=1.70;95CI 1.55,1.87) or scores ≤ 14(RR=1.42 95CI 1.29,1.56), prior injury(RR=1.54;95CI 1.32,1.80), slower running performance(RR=1.33;95CI 1.18,1.51), and poorer push-up performance(RR=1.15;95CI 1.04,1.27). Low BMI, height, weight, smoking, physical activity scores, and sit-up and jump performance were not significant risk factors in the meta-analysis. Most studies had a high risk of bias. Lack of raw data and large heterogeneity in definitions of predictors and injury outcomes limited comparison across many studies.

    • In competitive apnea divers, the nutritional demands may be essentially different from those of, for example, endurance athletes, where energy resources need to be maximised for successful performance. In competitive apnea, the goal is instead to limit metabolism, as the length of the sustainable apneic period will depend to a great extent on minimising oxygen consumption. Many but not all elite divers fast before performing static apnea in competition. This may increase oxygen consumption as mainly lipid stores are metabolised but could also have beneficial effects on apneic duration. Our aim was to determine the effect of over-night fasting on apnea performance. Six female and seven male divers performed a series of three apneas after eating and fasting, respectively. The series consisted of two 2-min apneas spaced by 3 min rest and, after 5 min rest, one maximal effort apnea. Apneas were performed at supine rest and preceded by normal respiration and maximal inspiration. Mean (±SD) time since eating was 13 h (±2 h 43 min) for the fasting and 1 h 34 min (±33 min) for the eating condition (P < 0.001). Mean blood glucose was 5.1 (±0.4) mmol/L after fasting and 5.9 (±0.7) mmol/L after eating (P<0.01). Lung volumes were similar in both conditions (NS). For the 2-min apneas, nadir SaO2 during fasting was 95 (±1)% and 92 (±2)% (P < 0.001) on eating and ETCO2 was lower in the fasting condition (P < 0.01) while heart rate (HR) during apnea was 74 (±10) bpm for fasting and 80 (±10) bpm for eating conditions (P < 0.01). Maximal apnea durations were 4 min 41 s (±43 s) during fasting and 3 min 51 s (±37 s) after eating (P < 0.001), and time without respiratory contractions was 31 s (25%) longer after fasting (P < 0.01). At maximal apnea termination, SaO2 and ETCO2 were similar in both conditions (NS) and apneic HR was 63 (±9) bpm for fasting and 70 (±10) bpm for eating (P < 0.01). The 22% longer apnea duration after fasting with analogous end apnea SaO2 levels suggests that fasting is beneficial for static apnea performance in elite divers, likely via metabolism-limiting mechanisms. The oxygen-conserving effect of the more pronounced diving response and possibly other metabolism-limiting mechanisms related to fasting apparently outweigh the enhanced oxygen consumption caused by lipid metabolism.

High physical index indicated high fitness and low physical index indicated low fitness. ANOVA test, chi-square test and logistic regression analysis were used to compare active and inactive children. The prevalence of one pain location (head, abdomen or back) was 26%, two 11% and three 4% (n=206). Female gender, living in single-parent families, low PA and low subjective health were associated with reported recurrent pain. Children reporting high levels of PA had high physical index and reported low prevalence of pain symptoms. The physical index and level of self-reported PA decreased gradually the more pain locations. Physically active children had higher fitness levels and reported less pain symptoms than inactive peers. Coping with pain is an integral part of PA, and active children learn to cope with unpleasant body sensations which together with high fitness may reduce the perception of pain. ANOVA test, chi-square test and logistic regression analysis were used to compare active and inactive children. The prevalence of one pain location (head, abdomen or back) was 26%, two 11% and three 4% (n=206). Female gender, living in single-parent families, low PA and low subjective health were associated with reported recurrent pain. Children reporting high levels of PA had high physical index and reported low prevalence of pain symptoms. The physical index and level of self-reported PA decreased gradually the more pain locations. Physically active children had higher fitness levels and reported less pain symptoms than inactive peers. Coping with pain is an integral part of PA, and active children learn to cope with unpleasant body sensations which together with high fitness may reduce the perception of pain.

    • People’s implicit beliefs of ability have been suggested as an antecedent of achievement goal adoption, which has in turn been associated with behavioural, cognitive and affective outcomes. This study examined a conditional process model with team sport athletes’ approach-avoidance achievement goals as mediators between their implicit beliefs of sport ability and sport-related cognitive anxiety. We expected gender to moderate the paths from implicit beliefs of ability to approach-avoidance goals and from approach-avoidance goals to cognitive anxiety. Team sport athletes with a mean age of 20 years (163 females and 152 males) responded to questionnaires about their implicit beliefs of sport ability, approach-avoidance goals and sport-related cognitive anxiety. Incremental beliefs, gender and the interaction between them predicted mastery approach goals. Gender also predicted mastery-avoidance goals, with females reporting higher levels than males. Mastery- avoidance goals, gender and the interaction between them predicted cognitive anxiety, with females reporting higher levels of anxiety than males. Entity beliefs positively predicted performance-avoidance goals and the interaction between performance-approach and gender predicted anxiety. The indirect effects also showed gender differences in relation to performance-approach goals. Taken together, our results suggest that coaches trying to create a facilitating climate for their male and female athletes may be wise to consider their athletes’ anxiety and achievement goal patterns as these may affect both the athletes’ well-being and performance.

  • Sport specialization in Swedish football players : investigating a model of antecedents and outcomes

    • Drawing on data from the present and former football players (N = 1026) selected to a national football talent programme at the age of 15, this study explores a model of sport specialisation. We examined three specific aspects of sport specialisation including early football specialisation, participation in youth elite football training environments (i.e. academies) and enrolment in upper secondary football specialisation schools. Antecedents of these sport specialisation factors included gender (i.e. sociocultural), grit (i.e. personality) and perceptions of family finances (i.e. social). Outcomes focused on adult football participation at the age of 21 including elite skill acquisition (i.e. playing elite football) and personal development (i.e. participation in non-elite football). Findings revealed that females were less likely to gain access to elite football training or school specialisation environments. There was also a positive association between grit and participation in elite training environments. In terms of outcomes, players, who got trained in elite training environments during adolescence, were twice as likely to play elite football at the age of 21, while those who attended football specialisation schools were more likely to participate in non-elite football at the age of 21. Early specialisation was not associated with either adult participation outcome. This is one of the few studies to date addressing diverse antecedents and outcomes of sport specialisation factors. Understanding how sport specialisation practices relate to future skill acquisition and personal development can provide guidance for maximising the benefits of youth sport programming.

    • The present study was designed to analyse and compare the kinetics and kinematics associated with three different starting strategies during classic cross-country ski racing. Inside a ski tunnel, 12 elite male skiers performed three sets of three 38m starts. Each set included one start using: double poling only (DP), diagonal stride only (DIA) and freely chosen (FREE) (i.e. where subjects used the strategy or combination of strategies they felt was fastest) in random order. The first 18m was performed on a series of force plates that measured horizontal and vertical forces followed by 20m of a standard snow track. Additionally, cycle characteristics and joint angles were measured. DIA and FREE were faster over 38m than DP (P<.01). Net horizontal impulse (taking into account both positive and negative impulses) 5-10m after the start was lower during DP than during DIA and FREE (both P<.05). All subjects skied faster when using only DIA for the entire 38m. Furthermore, the sum duration and frequency of propulsive contacts over the first 18m was less in DP than DIA and FREE (P<.01). In conclusion, differences between the starting strategies examined was especially pronounced during the initial cycles. Transition from DIA to DP during the start also slowed the skiers, but optimal timing for such a transition was not elucidated.

  • Cabozantinib Resolves Bone Scans in Tumor-Naive Mice Harboring Skeletal Injuries

    • The receptor tyrosine kinase inhibitor cabozantinib (XL184, BMS-907351 Cometriq) has displayed impressive clinical activity against several indications, culminating in its recent approval for medullary thyroid cancer. Among malignancies with tropism for the bone (prostate, breast), one striking feature of early clinical reports about this drug has been the rapid and complete resolution of bone scans, a phenomenon almost never observed even among therapies already shown to confer survival benefit. In castration-resistant prostate cancer, not all conventional response indicators change as dramatically posttreatment, raising the possibility that cabozantinib may impair the ability of bone-seeking radionuclides to integrate within the remodeling bone. To test this hypothesis, we surgically induced bone remodeling via physical insult in non-tumor-bearing mice and performed F-18-sodium fluoride (F-18-NaF) positron emission tomographic (PET) and technetium 99m-methylene diphosphonate (Tc-99m-MDP) single-photon emission computed tomographic (SPECT) scans pre- and posttreatment with cabozantinib and related inhibitors. A consistent reduction in the accumulation of either radiotracer at the site of bone remodeling was observed in animals treated with cabozantinib. Given that cabozantinib is known to inhibit several receptor tyrosine kinases, we drugged animals with various permutations of more selective inhibitors to attempt to refine the molecular basis of bone scan resolution. Neither the vascular endothelial growth factor receptor (VEGFR) inhibitor axitinib, the MET inhibitor crizotinib, nor the combination was capable of inhibiting F-18-NaF accumulation at known bioactive doses. In summary, although the mechanism by which cabozantinib suppresses radionuclide incorporation into foci undergoing bone remodeling remains unknown, that this phenomenon occurs in tumor-naive models indicates that caution should be exercised in interpreting the clinical significance of this event.

    • The positron emission tomography (PET) ligand [C-11] carfentanil is a selective agonist for mu-opioid receptors and has been used for studying mu-opioid receptors in the human brain. However, it is unknown if [C-11] carfentanil binding differentiates between subtype receptors mu(1) and mu(2). In this study, we investigated whether mu(1) and mu(2) can be studied separately through receptor subtype-selective inhibition of [C-11] carfentanil by pharmacologic intervention. [C-11] Carfentanil binding characteristics on rat brain sections were assessed either alone or in the presence of the mu-receptor inhibitor cyprodime or the mu(1)-specific inhibitor naloxonazine. [C-11] Carfentanil binding in the living rat brain was similarly studied by small animal PET/computed tomography during baseline conditions or following displacement by cyprodime or naloxonazine. Autoradiography binding studies on rat brain sections demonstrated that [C-11] carfentanil has higher affinity and binding potential for mu(1) than for mu(2). [C-11] Carfentanil binding to mu(2) in vivo could not be detected following specific blocking of mu(1), as predicted from the low binding potential for mu(2) as measured in vitro. [C-11] Carfentanil binding is preferential for mu(1) compared to mu(2) in vitro and in vivo. Clinical studies employing [C-11] carfentanil are therefore likely biased to measure mu(1) rather than mu(2).

    • Affibody molecules, small (7 kDa) scaffold proteins, are a promising class of probes for radionuclide molecular imaging. Radiolabeling of Affibody molecules with the positron-emitting nuclide 68Ga would permit the use of positron emission tomography (PET), providing better resolution, sensitivity, and quantification accuracy than single-photon emission computed tomography (SPECT). The synthetic anti-HER2 ZHER2:S1 Affibody molecule was conjugated with DOTA at the N-terminus, in the middle of helix 3, or at the Cterminus. The biodistribution of 68Ga-and 111In-labeled Affibody molecules was directly compared in NMRI nu/nu mice bearing SKOV3 xenografts. The position of the chelator strongly influenced the biodistribution of the tracers, and the influence was more pronounced for 68Ga-labeled Affibody molecules than for the 111In-labeled counterparts. The best 68Ga-labeled variant was 68Ga-[DOTA-A1]-ZHER2:S1, which provided a tumor uptake of 13 ± 1 %ID/g and a tumor to blood ratio of 39 ± 12 at 2 hours after injection. 111In-[DOTA-A1]-ZHER2:S1 and 111In-[DOTA-K58]-ZHER2:S1 were equally good at this time point, providing a tumor uptake of 15 to 16 %ID/g and a tumor to blood ratio in the range of 60 to 80. In conclusion, the selection of the best position for a chelator in Affibody molecules can be used for optimization of their imaging properties. This may be important for the development of Affibody-based and other protein-based imaging probes.
