  • Systematic review with meta-analysis: Effects of probiotic supplementation on symptoms in functional dyspepsia

    • The pathophysiology of functional dyspepsia (FD) remains poorly understood, but alterations of the small intestinal microbiome have been observed. The place of probiotics in treatment is uncertain. We performed a systematic review and meta-analysis of the currently available randomized, controlled trials (RCTs) to evaluate the potential beneficial effects and risks of probiotics in FD. Pubmed, EMBASE, Scopus, Web of Science and the Cochrane Controlled Trials Register were searched (up to May 2019) for RCTs evaluating the effects of probiotic supplementation compared to placebo in adults with FD. Two reviewers independently assessed eligibility, trial quality and extracted information from identified articles. To compare the effects of probiotics with placebo, risk ratios (RRs) with 95% confidence intervals (CIs) were pooled using random effects models. Six trials, including 422 participants were included but only three RCTs could be included in the meta-analysis. Lactobacillus strains showed potential positive effects in terms of improving upper gastrointestinal (GI) symptoms in patients with FD. Probiotic supplementation tended to improve global dyspepsia score (n = 3 RCTs, risk ratio [RR]: 1.35, 95% CI 0.99 to 1.84; P = 0.061) and bacterial composition in the GI tract. Probiotics were well tolerated without any serious adverse events. While the available data suggest that supplementation with probiotics may improve GI symptoms in patients with FD, the evidence is insufficient to draw clear conclusions regarding efficacy. Thus, high-quality RCTs are needed to establish the beneficial effects of probiotic supplementation on FD outcomes.

    • The influence of oat and barley β-glucans on some features of diverse probiotic strains was investigated. Neither barley nor oat β-glucans supported the growth of any of the investigated strains when used as the sole carbon source in fermentation media. No protective effect by β-glucans was observed on bacterial survival to in vitro simulation of the human digestive tract. Moreover, the presence of β-glucans did not enhance the ability of probiotic bacteria to adhere on human intestinal cells. The immunomodulatory activities of probiotics, β-glucans, and their combinations were studied and compared in vitro by transcriptional analysis of immune-related genes on LPS-stimulated THP-1 macrophages. Immune modulating properties were evidenced. In particular, mixtures of probiotic microorganisms and barley β-glucans exhibited synergistic effects in modulating the transcriptional level of several immune-related genes, leading to an overall enhanced anti-inflammatory effect. Together, these findings suggest a promising application of probiotic bacteria and β-glucans in the preparation of dietary mixtures presenting health-promoting features such as immunomodulatory properties.

  • Development of an instant-mix probiotic beverage based on fermented quinoa with reduced phytate content

    • Quinoa (Chenopodium Quinoa Willd.), a South American grain, has great potential for innovation and development of improved food products because of its protein content and nutritional benefits. The study aimed to develop an instant-mix beverage based on fermented quinoa with reduced phytate content and viable lactic bacteria. Two different lactic fermentation processes were designed known as Fermentation type 1 (F1) and Fermentation type 2 (F2) using Lactobacillus plantarum 299v® to obtain fermented quinoa flour. In F1, roasted quinoa grains were fermented for 9 h whereas raw quinoa grains were fermented for 6 h in F2 followed by drying, roasting and fermenting again for 3 h. Phytate reduction, Lactobacillus growth, acidity, protein content, and estimated mineral absorption were evaluated along with organoleptic acceptability of the product. The fermented quinoa samples (frozen at −20 °C and −80 °C) were freeze-dried for the development of instant powder mix beverage with 3 different fruit flavours. Phytate was significantly reduced (p < 0.05) in 61.8% and 64.4% in F1 and F2 respectively, which increased the estimated mineral absorption (Phy:Fe, 5.5 and 5.3); (Phy:Zn, 9.3 and 8.8) in both fermentations. High microbial viability was obtained in the fermented freeze-dried samples. The developed instant-mix of pineapple-orange flavour with 5% of fermented quinoa flour showed highest acceptability among all flavours during the sensory evaluation. Thus, fermentation is an effective way to improve nutritional and sensorial properties of quinoa. The developed instant-mix beverage with fermented quinoa had reduced phytate content, high probiotic count and better sensorial characteristics than a commercial product.

    • Horticultural plant materials not usually used from onion, carrot, beetroot, sea buckthorn, black and red currants as well as a wastewater powder from olive oil production were analyzed for total phenols content (FC), ferric reducing ability of plasma (FRAP), radical scavenging capacity (ABTS), and for major phenolic compounds by HPLC-MS. Antioxidant capacity and phenol content varied significantly between species and cultivars, with extracts of sea buckthorn leaves being superior. In different species, different phenolic compounds were closely associated with FRAP, ABTS and FC. For instance, hydrolysable tannins were major antioxidants in sea buckthorn whereas quercetin was the major antioxidant in onion peel and skin. This study shows that horticultural plant materials usually left in the field or waste materials from processing may have high antioxidant properties, and that extracts of these materials therefore could be of potential interest for development of antioxidant food additives.

  • Autochthonous microorganisms of white quinoa grains with special attention to novel functional properties of lactobacilli strains

    • Quinoa sourdough can safely be produced if the hygienic threshold of miss-fermentation during the start-up period is overcome. The aims of the present study are to map the microbiota of spontaneously formed quinoa sourdough without backslopping and select Lactiplantibacillus-strains. Viable count from grains and sourdough were performed and picked isolates were identified by gene sequencing. Lactiplantibacillus interspecies differentiation was evaluated by specific primers targeting the recA region, fermentation capacity and enzymatic abilities applying colorimetric assays, respectively. High initial levels of Enterobacteriaceae and other opportunistic pathogens decreased gradually, and the pH dropped below 4. The results indicate that spontaneous fermentation involves a variety of quinoa autochthonous microorganisms, with low presence of lactobacilli due to the delay lactic acid fermentation. Lactobacilli strains able to ferment xylose and glycerol and metabolize phenolic compounds were identified holding great prospects as suitable starter cultures for production of fermented quinoa products.

    • Spices are rich in distinct polyphenols which might act on the gut by inhibiting glucose uptake and modulating appetite responses. To investigate this hypothesis, healthy adults were randomly assigned to receive isovolumetric (220 ml) spice-based (contained total polyphenol concentration to 185 mg gallic-acid equivalents) or control beverages followed by a standard bread breakfast containing 50 g available carbohydrates in a cross-over trial. Postprandial glucose, insulin, PYY and appetite responses were evaluated. Ultra-high-performance liquid chromatography coupled to electrospray quadrupole time-of-flight mass spectrometry (UHPLC-DAD-ESI-QTOF-MS) was used for polyphenols profiling. Cinnamon and turmeric lowered early blood glucose increment up to 45 min compared to control. Turmeric increased p-PYY and lowered ‘desire to eat’ and ‘prospective consumption (quantity of food wanted to it)’ compared to control. By offering appetite modulation and glucose lowering effects, certain spices (e.g. turmeric and cinnamon) may be important in lowering cardiometabolic risk.

  • Oat β-glucan containing bread increases the glycaemic profile

    • A net postprandial glucose increment beyond 2 h has been shown to improve glucose and appetite regulation at a subsequent meal. Such an improved glycaemic profile (GP) has been reported for bread containing guar gum. In the present study three commercially available β-glucans from barley and oat were baked into yeast leavened bread products. Only oat beta-glucan containing bread met the criteria of β-glucan molecular weight and was included in a meal study. The three levels of oat β-glucans reduced the GI and glucose iPeak by 32–37% compared to a white wheat reference bread. Furthermore, the highest oat β-glucan level increased GP by 66% compared to the reference bread. It is concluded that the oat β-glucans were suitable for use in baking, since the MW remained relatively high. Thus, the oat ingredient showed an interesting potential to be used when tailoring the glycaemic profile of bread products.

    • Different dietary sources of bioactives may reduce cardiometabolic risk. This work investigated the feasibility of using a high-fat/high-caloric meal challenge as a tool for assessing the cardiometabolic protective effects of three bioactive-rich foods. Thirteen healthy, but overweight volunteers (65.1 ± 5.3 years old, fasting glycemia, ≤6.1 mmol L−1) received a high-fat reference breakfast meal (RM; 910 kcal, 50 E% fat) or three isocaloric test meals incorporating cinnamon (CM, 3 g), bilberry (BM, 100 g) or lingonberry (LM, 100 g) to the high-fat breakfast. Circulating biomarkers associated with cardiometabolic risk were measured postprandially for 4 h. LM and BM attenuated the elevation of cholesterolemia caused by RM. BM also modulated the triacylglyceride response. CM reduced glycemic response, postprandial endotoxemia and C-reactive protein, but increased cholesterolemic response. These postprandial response-modulating actions of bilberries, lingonberries and cinnamon suggest the high-fat/high caloric meal model as a tool for screening protective effects of bioactive-rich foods.

    • In recent years, the increase of the global incidence of obesity and obesity-related disorders has been associated with metabolic imbalance and low grade inflammation. Fat-rich meals and diets contribute importantly to those alterations. The aim of this work was to evaluate the impact of exchanging butter by Hass avocado-oil on postprandial metabolic parameters in healthy overweight volunteers consuming a hypercaloric-hyperlipidic breakfast. Thirteen healthy volunteers consumed a control meal (CM) consisting of: butter, eggs, bacon, wheat bread, potatoes and iced sugar, or a test meal (TM), where butter was totally replaced by Hass avocado-oil. Blood biomarkers were measured postprandially during 240 min. Participants were 65.1 ± 5.3 years old, body mass index 28.1 ± 1.8 kg/m2 and fasting glycemia ≤6.1 mmol/L. The consumption of CM or TM resulted in similar high-density lipoprotein and glucagon-like peptide-1 (GLP-1) responses, but TM improved postprandial profiles of insulin, glycemia, total cholesterol, low-density lipoprotein, triacylglycerols, C-reactive protein and interleukin-6 (P < 0.05). The study shows potential of Hass avocado-oil for counteracting the negative impact of a high fat and hypercaloric breakfast meal on important biomarkers related to cardiometabolic health.

    • It is postulated that exocrine pancreatic insufficiency (EPI) can evoke neurological disorders. In the present study pancreatic-like enzymes of microbial origin (PLEM) were examined as a functional food component, with the goal of improving cognitive function and brain structure in a pig model of EPI. EPI in the present study induced alterations in the behaviour of the pigs as well as degenerative changes within the morphological structure of the hippocampus. EPI leads to a reduced number of pyramidal neurons and decreased levels of neural cell adhesion molecules (NCAM) in the CA1 area of the hippocampus. Here, we provide evidence that the use of PLEM as a functional food, in the form of dietary supplementation with PLEM for 10 days, restored pig behaviour and the histo-morphology of the hippocampus in EPI pigs. Thus, we suggest that the use of PLEM as a functional food ingredient should be considered in the prevention and/or postponement of the development of EPI-related encephalopathy.     • An exocrine pancreatic insufficient (EPI) pig model was used in the study. The effects of milk formula pre-digestion with immobilized microbial lipase, on the absorption and tissue accretion of long chain polyunsaturated fatty acids (LCPUFA) were assessed. Thirteen male EPI pigs, 10 weeks of age, were used in the study. Six healthy pigs, 6 weeks of age, were used as controls. The pigs were fed either a regular or a pre-digested milk formula. The formula pre-digestion resulted in the decreased faecal total fat and LCPUFA excretion (by 43% and 38% respectively), increased plasma and tissue LCPUFA content (up to 38%). In conclusion, we postulate that feeding a pre-digested milk formula, is an efficient method to develop a functional milk formula, the consumption of which will ensure an increase in total fat absorption (in particular LCPUFA) in human infants as was indicated in the present study in the EPI pig model.

  • INFOGEST inter-laboratory recommendations for assaying gastric and pancreatic lipases activities prior to in vitro digestion studies

    • In vitro digestion studies often use animal digestive enzyme extracts as substitutes of human gastric and pancreatic secretions. Pancreatin from porcine origin is thus commonly used to provide relevant pancreatic enzymes such as proteases, amylase and lipase. Rabbit gastric extracts (RGE) have been recently introduced to provide gastric lipase in addition to pepsin. Before preparing simulated gastric and pancreatic extracts with targeted enzyme activities as described in in vitro digestion protocols, it is important to determine the activities of enzyme preparations using validated methods. The purpose of this inter-laboratory study within the INFOGEST network was to test the repeatability and reproducibility of lipase assays using the pH-stat technique for measuring the activities of gastric and pancreatic lipases from various sources. Twenty-one laboratories having different pH-stat devices received the same protocol with identical batches of RGE and two pancreatin sources. Lipase assays were performed using tributyrin as a substrate and three different amounts (50, 100 and 200 µg) of each enzyme preparation. The repeatability results within individual laboratories were satisfactory with coefficients of variation (CVs) ranging from 4 to 8% regardless of the enzyme amount tested. However, the inter-laboratory variability was high (CV > 15%) compared to existing standards for bioanalytical assays. We identified and weighted the contributions to inter-laboratory variability of several parameters associated with the various pH-stat equipment used in this study (e.g. reaction vessel volume and shape, stirring mode and rate, burette volume for the automated delivery of sodium hydroxide). Based on this, we established recommendations for improving the reproducibility of lipase assays using the pH-stat technique. Defining accurate and complete recommendations on how to correctly quantify activity levels of enzyme preparations is a gateway to promising comparison of in vitro data obtained from different laboratories following the same in vitro digestion protocol.

    • Antisecretory factor (AF) is a protein which regulates fluid transport in the intestine and other organs. Extensive malting of wheat releases certain chemicals which induce AF and inhibit enterotoxic diarrhoea. The aim of this study was to identify AF-inducing substances in wheat malt. The active fractions of the malt leachate contained guaiacol, ferulic acid, and vanillic acid. These three 2-methyl-catechols gave >50% inhibition of cholera toxin-induced secretion in the gut. In contrast, fully methylated catechol had the opposite action; that is, they were secretory rather than antisecretory. Guaiacol and ferulic acid were further shown to induce AF in blood. Since the 2-methoxyphenol structure is present in substances binding to the vanilloid receptor, TRPV1, the specific blocker of this receptor—capsazepine—was tested. This substance exerted >50% inhibition of cholera secretion. Thus 2-methyl-catechols in wheat malt induce AF, and probably exert their effect via the TRPV1 receptor. © 2019 Elsevier Ltd

    • Intact adipose tissue function is essential to maintain glucose and lipid homeostasis. To study the impact of altered adipose tissue function on whole-body metabolism, diet-induced obesity in mice is frequently used as a model organism. In the current study, we have examined health-promoting effects of a lingonberry supplemented diet. We found C57BL/6J mice fed a high-fat diet supplemented with lingonberry for 4 days to have significantly lowered body-weight gain, adipose tissue expansion, and reduced insulin levels, compared to mice fed an isocaloric high-fat diet. RNA-Seq analysis of epididymal adipose tissue revealed differential expression of genes related to mitochondria fission (Mief1, Dnm1, Vps35, and Opa1). Further, we detected increased gene expression and phosphorylation of perilipin-1 (pS522), and increased lipolysis in primary adipocytes from lingonberry-fed mice. Together, these data pinpoint that beneficial effects of a lingonberry enriched diet are rapidly detectable and that the adipose tissue constitutes a target for these effects.

    • Alkylresorcinols (AR) with alkyl chains in the range of C15 to C25 are phenolic lipids particularly abundant in the outer parts of wheat and rye kernels and in food products containing these parts. The content in whole grain and bran products is high (200-4000 mu g/g), whereas only trace-levels are detected in refined products. Alkylresorcinols are absorbed in humans in proportion to intake and have therefore been suggested and evaluated as biomarkers for whole grain wheat and rye intake. In humans, plasma AR concentrations reach micromolar concentrations immediately after whole grain wheat and rye product consumption and nano-molar levels at fasting conditions. Results from different model studies have indicated that AR may have some bioactivities including enzyme inhibition, suppression of adipocyte lipolysis and inhibition of colon cancer tumor growth but it is currently unknown whether AR bioactive in vivo or not. This chapter details the recent research findings on alkylresorcinols with emphasis on their occurrence, bioavailability, bioactivity and utility as biomarkers.

  • Does dairy product consumption impact the prevalence of inflammatory bowel disease? : An ecological cross-sectional analysis

    • Modulation of the microbiota of patients suffering from inflammatory bowel disease (IBD) with probiotics was suggested to avoid active disease and/or to lower the intensity of the active phase. Considering that the ecological niche of Bifidobacterium spp and Lactobacillus spp includes fermented dairy products, and that countries with highest dairy product consumption were those with lowest prevalence of IBD, we aimed at determining if consumption of dairy products could modulate the prevalence of IBD. Linear regression was performed on data from the Global Burden of Diseases, Injuries, and Risk Factors Study reporting the prevalence of IBD and from the European Association of Dairy Trade reporting national consumptions of dairy products. Consumption of milk, cheese or yoghourt per capita did not correlate with the prevalence of IBD at the national level in Western countries. Considering the limitations of this type of analysis, we recommend to continue investigating the effect of dairy product consumption on IBD by cross-sectional, longitudinal or interventional studies.

    • Cocoa is a well acknowledged polyphenol-rich functional product. However, the mechanisms and effects of cocoa products on glucose metabolism and appetite regulation are not clear. A crossover randomised study was carried out in twenty (4male; 16female) healthy middle-age (62.6 ± 6.5 yr) subjects with normal BMI (23.0 ± 2.0 kg/m2). Effects on postprandial glucose-, insulin- and subjective appetite responses to a preload (30 min) of 70% dark-chocolate (DC), with and without a preceding 4-week DC intervention, were evaluated. SCFA (acetate, propionate and butyrate), BDNF, GLP-1, and PYY were determined in plasma at fasting. An isocaloric white-chocolate-like product (CP) was included as a control. DC increased (main effects; p < 0.05) glucose- and insulin responses in comparison with CP. Postprandial hunger sensations were reduced (p < 0.05), whereas acetate and propionate were higher (p < 0.05) after 4-week DC intervention. The results suggest that cocoa may play a role in appetite regulation, however the unexpected increase in postprandial glucose- and insulin concentrations requires further investigation.

Five groups of Balb-c mice were treated for 5 weeks (Group 1 and 2 referred to as normal and diabetic control receiving normal feed). Group 3 and 4 were diabetic mice receiving 50 and 100 mg/kg/BW NLD respectively. Group 5, diabetic mice, received the standard drug, Metformin, 250 mg/kg/BW per day orally. Results indicated that NLD significantly (p < 0.05) improved the final body weight, triglyceride (TG), total cholesterol (TC) concentrations, low-density lipoprotein cholesterol and malondialdehyde (MDA) levels and enhanced the high-density lipoprotein. Furthermore, NLD enhanced the expression of CAT and suppressed iNOS gene in the pancreas. The mRNA expression of GLUT2 and GLUT4 also showed increase in the diabetic mice. Findings demonstrated the potential of NLD as a nutraceutical for the prevention and/or treatment of diabetes without toxic side effects.

    • Two Rubus rosifolius raspberry varieties described as ‘Red’ and ‘Wine Red’ were analyzed for their phytochemical constituents, impact on cytochromes P450 (CYP) enzyme activity, and antioxidant properties. Ellagic acid was the most predominant of the phenolics analyzed. Three compounds were isolated from the methanol extract of the ‘Red’ variety. β-sitosterol D-glucoside-6′-acetate and trachelosperogenin A were among the six compounds isolated from the ‘Wine Red’ EtOAc extract, and, to the best of our knowledge, are being reported in the genus for the first time. Euscaphic acid demonstrated the highest antioxidant activity (175.3 AE/100 mg fresh weight, FW). Pomolic acid demonstrated moderate inhibitory activity (IC50 = 9.4 µM) against the carcinogen-activating CYP1B1 enzyme, while cyanidin 3-O-glucoside was identified as a moderate, uncompetitive inhibitor of the same enzyme (Ki = 2.3 µM). Ellagic acid, quercetin and kaempferol were important contributors to the CYP1B1 inhibitory activity of the methanol extracts, indicating potential for chemopreventive effects.

    • The efficacy of three synbiotic approaches to improve persistence and metabolic activity of Lactobacillus reuteri DSM 17938 in the human gut was determined in a single-blind, randomized, crossover, placebo-controlled trial. Synbiotics consisting of 5 x 10(8) bacterial cells and 2 g of either GOS, rhamnose or the combination of both were given to 15 healthy adults for 7 days, followed by 10 days during which only the carbohydrates were administered. Faecal samples were collected and quantitative real-time PCR was used to determine cell numbers and rRNA copy number. Although L. reuteri DSM 17938 was detectable in faecal samples after consumption, the addition of GOS, rhamnose, and the mixture of the both did not increase faecal populations, nor was persistence enhanced after consumption of the probiotic had ended. However, based on rRNA per cell ratios, the combination of GOS and rhamnose significantly stimulated metabolic activity of the L. reuteri strain. (C) 2014 Elsevier Ltd. All rights reserved.

    • Citrus limonoids are tetranortriterpenoids compounds mainly found in oranges, lemons, grapefruits, and other fruits of Citrus. They are proved to be the leading cause of bitterness in Citrus fruits and are mainly consumed for therapeutic purposes and as food. Numerous studies have focused on Citrus limonoids and intend to develop new chemotherapeutic or complementary medicine in recent years. Citrus limonoids showed various bioactivities such as anti-tumor, antioxidative, anti-inflammatory, anti-neurological diseases, immunomodulatory, anti -insect, anti-bacteria, antiviral activities, etc. This review summarized limonoids from Citrus to date, along with their chemical structures and biological activities with a particular focus on their anti-tumor potential.

    • Two probiotic products containing either live Lactobacillus plantarum strain DSM 15313 or bilberries fermented by the same bacterial strain were tested for their antihypertensive potentials and their impact on the oral and the faecal microbiota in hypertensive adults. The results showed that consumption of the tested products for three months did not reduce the blood pressure in adults with hypertension. Neither the diversity nor the composition of the oral and faecal microbiota was significantly affected by the tested probiotic products.     • Ulva polysaccharide (ULP) as main active ingredient of Ulva lactuca, has good pharmacological activity. This study further explored its anti-tumor mechanisms in hepatocellular carcinoma (HCC) with wet experiments and bioinformatic analyses. Results indicate that the differential proteins after ULP treatment are mainly enriched in immunomodulatory and complement pathways, among which the immune molecule CD5L and the key proteins of the membrane attack complex (MAC) are significantly upregulated, and the expression of MAC proteins are positively correlated with the expression of CD5L. Moreover, results from TIMER2.0 indicated that CD5L expression had positive correlation with CD8 + T cell infiltration in HCC, and single cell RNA-Seq analysis showed CD5L positive macrophage cells have higher expression of CFP, C1QA, C1QB, and C1QC, which are key molecular of complement cascades. Take together, the results suggest that ULP may exerts its anti-tumor activity by induces the expression of CD5L in HCC.
