  • An asymmetric dialysis tube as an integrated part of a pressure-monitoring sensor

    • A sensor has been designed consisting of a tube holder with a force transducer and a tube with a modified cross-section. The holder has a lid that encloses the tube. By having a stiff holder and a compliant tube, the idea is that the intraluminal pressure in the tube can be obtained from the measured force. The method is intended for non-invasive pressure measurements in blood or dialysate tubes.We have used a tube cross-sectional geometry where the outer surface is elliptic and the inner surface is circular with a relation of 2:1 between the thinnest and thickest tube sides. The pressure transducer system shows a linear relationship between the applied pressure and the sensor output (r = 0.999). Within the temperature range, 32°–36°C, which corresponds to the blood and dialysate temperatures, the sensor accuracy is within ±0.8 kPa (±6 mm Hg). This indicates that the sensor should be clinically useful during dialysis and similar applications.

  • Cross-sectional changes in an asymmetric tube with bearing on non-invasive pressure measurements

    • This study aims at investigating the radial dimensional changes, as a result of an applied intraluminal pressure for an elastic tube with non-uniform cross section. The study is related to a method for non-invasive pressure measurement using the extracorporeal tube as part of the sensor. The intended application is for monitoring of pressure in the blood and dialysate tubes during haemodialysis. The intention is to find a tube cross-section geometry that results in an expansion of the tube so that it is suitable to use as a component in a pressure sensor. The tube should have high radial compliance and expand in a well-defined manner to be able to transfer the intraluminal pressure to a transducer element sensing the radial force. Radial expansion was studied experimentally for tubes with different cross-section geometries. For small tube expansions the resolution in the experimental measurements was not sufficient to study the radial expansion. In this case, numerical simulation was performed. We conclude that a tube with essentially elliptic outer surface and circular inner surface, with a relation of 1:2 between the size of the thin and thick wall, results in a radial expansion upon application of pressure indicating that this tube is suitable for use as part of a sensor.

  • A new force measurement device for evaluating finger extension function in the healthy and rheumatoid arthritis hand

    • Although often neglected, finger extension force is of great importance for developing grip strength. This paper describes the design and evaluation of a new finger extension force measurement device (EX-it) based on the biomechanics of the hand. Measurement accuracy and test-retest reliability were analysed. The device allows measurements on single fingers as well as all the fingers (excluding the thumb) of both healthy and deformed hands. The coefficient of variation in the device was 1.8% of the applied load, and the test-retest reliability showed a coefficient of variation no more than 7.1% for healthy subjects. This study also provides reference values for finger extension force in healthy subjects and patients with rheumatoid arthritis (RA). Significant differences were found in extension strength between healthy subject and RA patients (men, p < 0.05 and women, p < 0.001). EX-it provides objective and reliable data on the extension force capacity of normal and dysfunctional hands and can be used to evaluate the outcome of therapeutic interventions after hand trauma or disease

  • Network solutions for home health care applications

    • The growing number of the elderly in industrialised countries is increasing the pressure on respective health care systems. This is one reason for recent trends in the development and expansion of home health care organisations. With Internet access available to everyone and the advent of wireless technologies, advanced telehomecare is a possibility for a large proportion of the population.In the near future, one of the authors plans to implement a home health care infrastructure for patients with congestive heart failure and patients with chronic obstructive pulmonary disease. The system is meant to support regular and ad-hoc measurements of medical parameters in patient homes and transmission of measurement data to the home health care provider.In this paper we look at network technologies that connect sensors and input devices in the patient home to a home health care provider. We consider wireless and Internet technologies from functional and security-related perspectives and arrive at a recommendation for our system.Security and usability aspects of the proposed network infrastructures are explored with special focus on their impact on the patient home.

  • An improved bioacoustic method for monitoring of respiration

    • Reliable monitoring of respiration plays an important role in a broad spectrum of applications. Today, there are several methods for monitoring respiration, but none of them has proved to be satisfactory in all respects. We have recently developed a bioacoustic method that can accurately time respiration from tracheal sounds. The aim of this study is to tailor this bioacoustic method for monitoring purposes by introducing dedicated signal processing. The method was developed on a material of ten patients and then tested in another ten patients treated in an intensive care unit. By studying the differences in the variation of the spectral content between the different phases of respiration, the described method can distinguish between inspiration and expiration and can extract respiration frequency, and respiration pause periods. The system detected 98% of the inspirations and 99% of the expirations. This method for respiration monitoring has the advantage of being simple, robust and the sensor does not need to be placed closed to the face. A commercial heart microphone was used and we anticipate that further improvement in performance can be achieved trough optimization of sensor design.

  • Views of residents of rural areas on accessibility to specialist care through videoconference

    • BACKGROUND: Accessibility to specialized health care is important for residents in rural areas. Videoconsultation can provide increased accessibility to health care. Before implementation of new technologies, it is important to study residents' views.OBJECTIVE: This study describes views of residents of rural areas on accessibility to specialist care and the use of videoconsultation as tool to increase accessibility.METHOD: A questionnaire was distributed via the primary health care centres in rural areas of northern Sweden. Data was analysed with both quantitative and qualitative methods.RESULTS: The quantitative analysis revealed three main areas: savings in time, environmental damage and cost of not having to travel were important; security of information must be coupled with increased availability of specialist care; responses depended on an individual's specific health care needs. The open-ended answers resulted into two categories: 1) experiences related to availability of specialist care and 2) views on participating in VC.CONCLUSION: Despite the fact that rural residents considered the distance as important factor in access to care, there was uncertainty regarding videoconsultation as solution. Although respondents were familiar with different types of communication technologies, they probably hesitated because they did not have information on what videoconsultation would mean to them.

  • A novel method for automatic classification of Parkinson gait severity using front-view video analysis

    • BACKGROUND: Gait impairment is an essential symptom of Parkinson’s disease (PD). OBJECTIVE: This paper introduces a novel computer-vision framework for automatic classification of the severity of gait impairment using front-view motion analysis. METHODS: Four hundred and fifty-six videos were recorded from 19 PD patients using an RGB camera during clinical gait assessment. Gait performance in each video was rated by a neurologist using the unified Parkinson’s disease rating scale for gait examination (UPDRS-gait). The proposed algorithm detects and tracks the silhouette of the test subject in the video to generate a height signal. Gait features were extracted from the height signal. Feature analysis was performed using the Kruskal-Wallis rank test. A support vector machine was trained using the features to classify the severity levels according to UPDRS-gait in 10-fold cross-validation. RESULTS: Features significantly (p< 0.05) differentiated between median-ranks of UPDRS-gait levels. The SVM classified the levels with a promising area under the ROC of 80.88%. CONCLUSION: Findings support the feasibility of this model for Parkinson’s gait assessment in the home environment. © 2021 - IOS Press. All rights reserved.

  • A system for recording of auditory evoked responses

    • A system for recording of evoked potentials from auditory stimulation was developed. The system consists of a PC equipped with an audio bandwidth board with analog input and output channels. The sound stimulus signal is generated in the computer, D/A converted, and via audio amplifier fed to earphones on the test subject. Auditory evoked potentials in response to sound stimuli are recorded via electrodes, amplified and filtered in an EEG recording system and fed to an A/D converter. The signal is analysed in the PC. The modular design of the program makes it a flexible system where stimulus and recording parameters can easily be modified and new applications can be added to standard clinical measurements.Three applications that are not possible with commercially available systems were developed and evaluated. a) A diagnostic procedure to verify hydrops in patients with Meniere's disease. b) Intraoperative recordings of auditory evoked potentials during neurootological surgery. c) Recording of mismatch negativity (MMN) potentials in evaluation of central auditory functions.

  • The lighthouse alarm and locator trial : a pilot study

    • An important factor for health is the possibility to be active and mobile. To make this possible different kind of support are needed. Integrating geographical information systems technology and user experiences is important in the development of more user-friendly positioning devices. The Lighthouse Alarm and Locator trial aimed to test a new mobile alarm system with additional functionality such as positioning and monitoring of vital signs which can be used regardless of location (in hospital, at home). The system was tested by elderly persons from a pensioner organisation and home care personnel answered up on the alarms. After the tests qualitative interviews were performed with the two groups. The results showed that their experiences of the new mobile alarm system could be described in three main categories: to be supervised, to feel safe and to be mobile. These categories formed a theme: Positioning - an ethical dilemma. The clients' mobility was perceived to increase. The personnel did not think that positioning was ethical but the clients (elderly) did.

    • INTRODUCTION:This study aimed to evaluate if use of an activity promoting computer game, used in the home (Nintendo Wii Fit; Nintendo Co Ltd, Japan), could influence balance related outcome measures in children with cerebral palsy.METHOD:Eighteen children with hemiplegic or diplegic cerebral palsy were recruited for the study. A randomised cross-over design was used with children tested at baseline, after five weeks of playing Wii Fit games and after five weeks without any intervention. Outcome measures of interest included: performance on the modified sensory organisation test, reactive balance test and rhythmic weight shift test.RESULTS:No significant difference was observed between testing occasions for any of the balance measures investigated (p > 0.05).CONCLUSION:Our results suggest that use of a Nintendo Wii balance board and Wii Fit software for a minimum of thirty minutes per day in the patient's own home, over a five week period, is not effective as a balance training tool in children with cerebral palsy.

  • Adoption of medical devices : Perspectives of professionals in Swedish neonatal intensive care

    • Advances in biomedical engineering enable us to treat increasingly severe conditions. This implies an increased need for regulation and priority setting in healthcare, to ensure appropriate safety cautions and to avoid accelerating expenditures. This interview study investigates the mechanisms behind the adoption and use of medical devices through the subjective experiences of hospital staff working with devices for neonatal intensive care. The adoption was found to be primarily initiated by vendor activities, but professionals preferably sought information about functionality from close colleagues. Full integration of devices was sometimes not achieved, and even though the adopting units had good introduction routines, there was no systematic follow-up of how adopted devices had been integrated in the work practices. Diffusion variations were, however, mainly found for temporarily tested devices and not for permanently available technologies. Three factors were found to be the major explanatory variables of the adoption of medical devices: (1) the subjective expected value of the device, (2) information and learning, and (3) the innovativeness of the adopting unit.

    • Due to an ageing population and improved treatment possibilities, a shortage in hospital beds is a fact in many countries. Home healthcare schemes using information technology (IT) are under development as a response to this and with the intention to produce a more cost-effective care. So far it has been shown that home healthcare is beneficial to certain patient groups. The trend is a widening of the criteria for admission to home healthcare, which means treatment in the home of more severe conditions that otherwise would require in-hospital care. Home informatics has the potential to become a means of providing good care at home. In this process, it is important to consider what new risks will be encountered when placing electronic equipment in the home care environment. Continuous assessment and guidance is important in order to achieve a safe and effective care. Based on a review of current knowledge this paper presents an inventory of risks and adverse events specific to this area. It was found that risks and adverse events could stem from technology in itself, from human-technology interaction conditions or from the environment in which the technology is placed. As a result from the risk inventory, this paper proposes guidelines for the planning and assessment of IT-based hospital-at-home schemes . These assessment guidelines are specifically aimed at performance improvement and thus to be considered a complement to the more general guidelines on telehomecare adopted by the American Telemedicine Association (ATA) in October 2002.

    • BACKGROUND: The non-image-forming effects of luminous radiation on people with intellectual disabilities or dementia received attention from researchers. Such studies, however, have generally been conducted using disparate methodologies which precludes generalization and reproducibility.OBJECTIVE: The aim of this study was to determine the practical applicability of measurement devices for studies investigating non-image-forming effects of luminous radiation, specifically for people with intellectual disabilities or dementia.METHODS: In three experiments, ten cognitive impaired people and thirty-nine unaffected subjects participated by wearing one or more portable devices. Six devices were assessed in total. Measurement data was accompanied with user experiences obtained from questionnaires, interviews and observations in order to assess the devices on practical and comfort issues.RESULTS: On average, the devices worn by the cognitive impaired subjects were not experienced as annoying or irritating. No significant differences are found between genders and for one of the portable devices significantly less annoyance was reported by the cognitive impaired participants compared to the unaffected group of participants.INNOVATIVE SOLUTION: The three phases of the research process in towards measuring personal luminous exposures are: selection of the most suitable portable device, application of the assessment method, and the application of the device in the (pilot) study.CONCLUSIONS: However, the findings of this study suggest that inaccuracies potentially caused by practical and comfort issues associated with the portable devices need to be considered.

    • Light therapy is increasingly administered and studied as a non-pharmacologic treatment for a variety of health-related problems, including treatment of people with dementia. Light therapy comes in a variety of ways, ranging from being exposed to daylight, to being exposed to light emitted by light boxes and ambient bright light. Light therapy is an area in medicine where medical sciences meet the realms of physics, engineering and technology. Therefore, it is paramount that attention is paid in the methodology of studies to the technical aspects in their full breadth. This paper provides an extensive introduction for non-technical researchers on how to describe and adjust their methodology when involved in lighting therapy research. A specific focus in this manuscript is on ambient bright light, as it is an emerging field within the domain of light therapy. The paper deals with how to (i) describe the lighting equipment, (ii) describe the light measurements, (iii) describe the building and interaction with daylight. Moreover, attention is paid to the uncertainty in standards and guidelines regarding light and lighting for older adults.

  • Agent-based intelligent medical diagnosis system for patients

    • BACKGROUND: According to the analysis of the challenges faced by the current public health circumstances such as the sharp increase in elderly patients, limited medical personnel, resources and technology, the agent-based intelligent medical diagnosis system for patients (AIMDS) is proposed in this research. OBJECTIVE: Based on advanced sensing technology and professional medical knowledge, the AIMDS can output the appropriate medical prescriptions and food prohibition when the physical signs and symptoms of the patient are inputted. METHODS: Three core modules are designed include sensing module, intuition-based fuzzy set theory/medical diagnosis module, and medical knowledge module. RESULTS: The result shows that the optimized prescription can reach the desired level, with great curative effect for patient disease, through a case study simulation. CONCLUSION: The presented AIMDS can integrate sensor technique and intelligent medical diagnosis methods to make an accurate diagnosis, resulting in three-type of optimized descriptions for patient selection.
