Everyday Life Condition in Stroke Survivors and Their Family Caregivers in Iranian Context

- BackgroundStroke is an unexpected major life incident that effect on physical functioning and can lead tochanges in lifestyle and inability to manage self care, work or leisure. To explore the everydaylife situation as experienced by stroke survivors, and their family caregivers, this study wasconducted 3-6 month after stroke.MethodsTwelve stroke survivors and thirteen family caregivers were interviewed and a qualitativecontent analysis was conducted to analyze data.ResultsThe results were categorized into two central themes that were identified as; (1) Being challengedby changes in everyday life situation including; the struggle with physical dysfunctions,inappropriate daily plan for leisure activities, changes in mood, being faced with economicalproblems and being worried about changing roles (2) striving to cope with new conditionsincluding; trying to preserve self-worth, seeking to support from spirituality, learning andre-learning and regaining independency.ConclusionPhysical dysfunction was the major concern for both the survivors and their family caregiversin their everyday life situation. This problem led to, and forced them, to be dependent on others.Training is needed to teach stroke survivors and their families strategies of how to cope withand overcome ongoing challenges of everyday life after stroke and to re-organize their livesand improve skills of the survivors to deal with the new situation at home.A daily plan for spending their time and leisure activities would ease this work. Moreinvestigation will be needed to clarify the process of life after stroke to cover this gap.

Adherence to treatment in patients with chronic diseases: From alertness to persistence

- Background: Adherence to treatment is a process that begins with alertness about the disease and has to end with staying in treatment. It is a concern to all health care disciplines. The aim of this study was to explore the experience of adherence to treatment patients with chronic diseases. Methods: This qualitative study was conducted during a period of 6 months in Golestan province in Iran in 2017. In-depth semi-structured interviews were done with a purposive sample of 15 patients with chronic disease (i.e. Chronic renal failure (CRF), Hypertension (HTN), Multiple sclerosis (MS) and Diabetes). Data were analyzed using qualitative content analysis in MAXQDA Software. Results: The analysis of the data in this study led to the development of ten subthemes and four themes emerged as follows: motivation, cohesion, commitment and empowerment which are necessary components for adherence to treatment in patients. “Moving from alertness to persistence” emerged as the main theme. Conclusion: The findings of the study offer helpful insights into the concept of adherence to treatment and its components for health care providers, which can be used to develop the best possible care plan and may help health care providers to support the patients to have an active role for following treatment rather than just doing it. © 2019, Shriaz University of Medical Sciences. An Initial Study on the Coordination of Rod and Line Hauling Movements in Distance Fly Casting

- Background. The double haul is a unique feature of single-handed fly casting and is used in both fly fishing and fly casting competition. The movement behaviour during the double haul has not been investigated in previous research.Objectives. Describe the coordination of the rod and line hauling movements during distance fly casting.Methods. Elite fly casters performed distance casting with four different fly rod and fly line set-ups used in fly fishing and fly casting competition. Rod and hauling movements were measured with a 3D motion analysis system.Results. The rod and line hauling movements were coordinated in an order whereby peak translational speed of the rod occurs prior to the peak speed of the angular rotation of the rod, and the peak speed of the angular rotation of the rod occurs prior to the peak speed of the line haul. This was consistent for all cast sequences, i.e., the back and forward false casts and the delivery cast, and for all four equipment set-ups, i.e., a shooting-head line cast with a relatively stiff fly rod and a long-belly line cast with three different fly rods with different stiffness and action curves. Results also showed differences in movement coordination between cast sequences and rod and line set-ups.Conclusion. Among elite casters, single-handed fly casting with double haul is coordinated in an order of events whereby the peak speed occurs first for the translation of the rod, then for the rotation of the rod and finally for the line haul.
