    • OBJECTIVE This study explored how professionals in inter-disciplinary teams perceived the implementation of the World Health Organization's International Classification of Functioning, Disability and Health, version for Children and Youth (ICF-CY) in Swedish habilitation services. DESIGN Descriptive longitudinal mixed-methods design. METHODS Following participation in a 2-day in-service training on the ICF-CY, 113 professionals from 14 interdisciplinary teams described their perceptions of the implementation of the ICF-CY at 3 consecutive time-points: during in-service training, after 1 year, and after 2.5 years. RESULTS Implementation of the ICF-CY in daily work focused on assessment and habilitation planning and required adaptations of routines and materials. The ICF-CY was perceived as useful in supporting analyses and in communication about children's needs. Professionals also perceived it as contributing to new perspectives on problems and a sharpened focus on participation. CONCLUSION Professionals indicated that the ICF-CY enhanced their awareness of families' views of child participation, which corresponded to organizational goals for habilitation services. An implementation finding was a lack of tools fitting the comprehensive ICF-CY perspective. The study points to the need for ICF-CY-based assessment and intervention methods focusing on child participation

  • A Spatiotemporal Organ-Wide Gene Expression and Cell Atlas of the Developing Human Heart

    • The process of cardiac morphogenesis in humans is incompletely understood. Its full characterization requires a deep exploration of the organ-wide orchestration of gene expression with a single-cell spatial resolution. Here, we present a molecular approach that reveals the comprehensive transcriptional landscape of cell types populating the embryonic heart at three developmental stages and that maps cell-type-specific gene expression to specific anatomical domains. Spatial transcriptomics identified unique gene profiles that correspond to distinct anatomical regions in each developmental stage. Human embryonic cardiac cell types identified by single-cell RNA sequencing confirmed and enriched the spatial annotation of embryonic cardiac gene expression. In situ sequencing was then used to refine these results and create a spatial subcellular map for the three developmental phases. Finally, we generated a publicly available web resource of the human developing heart to facilitate future studies on human cardiogenesis.

Formation of acetate can be avoided by a proper substrate feeding strategy, but most strategies require considerable process knowledge to work well. A feedback strategy is presented that requires minimal a priori information. The key idea is to exploit a characteristic change in the cell metabolism at the onset of acetate formation. By superimposing short pulses in the substrate feed rate, on-line detection of acetate formation can be made using a standard dissolved oxygen sensor. A simple feedback algorithm is then used to adjust the feed rate to avoid acetate formation while maintaining a high growth rate. The strategy was used in fed-batch cultivations of recombinant E. coli. Acetate accumulation was avoided and a good production of recombinant protein could be achieved at the first attempt for each genetic construction.

    • *Summary: The bacterium Escherichia coli is a common host organism for recombinant protein production. A problem encountered is the accumulation of the by-product acetate, which tends to reduce growth and protein production (1). Formation of acetate can be avoided by a proper substrate feeding strategy, but most strategies require considerable process knowledge to work well. The main problem is that many important process variables cannot be measured on-line. This paper presents a feedback strategy for substrate feeding that is based on standard measurements and that only requires a minimum of process knowledge. The validity of the approach is demonstrated by both simulations and experiments. *Materials and Methods: Fed-batch cultivations of E. coli BL21(DE3) with different plasmids was made in a 3 L bioreactor using a defined medium. The temperature was 37 C and pH was kept at 7.0 by titration of ammonia. DO was kept around 30% saturation by manipulation of the stirrer speed. Expression of recombinant protein was initiated by addition of IPTG. OD was measured at 620 nm. Glucose and acetate concentration was determined using enzymatic kits. *Overview and discussion of results: The key idea of the novel feedback strategy is to exploit a characteristic change in the cell metabolism at the onset of acetate formation. By superimposing short pulses in the substrate feed rate, on-line detection of acetate formation can be made using a standard dissolved oxygen sensor (2). A simple feedback algorithm is then used to adjust the feed rate to avoid acetate formation while maintaining a high cell growth rate. The feasibility of the feeding strategy is demonstrated by simulations and tuning rules that require a minimum of process specific information are derived. The strategy was used in fed-batch cultivations of recombinant E. coli. Acetate accumulation was avoided and a good production of the recombinant protein was achieved. *Conclusions: A feedback strategy for substrate feeding in E. coli cultivations has been developed.     • A strategy for control of substrate feed rate in fed-batch fermentations is presented. The main idea is to make pulses in the feed rate and to evaluate the response in the dissolved oxygen signal. This gives an indirect measurement of the substrate demand that is used for feedback control of the feed rate. Experimental data is used to develop a model of a fermentation process. The model is used to derive simple tuning rules that require little process specific information. Several simulation experiments show the feasibility of the presented strategy.

    • The circular distribution of the willow warbler Phylloscopus trochilus around the Baltic Sea shares many features with the classic examples of ring species; however, the system is much younger. It has previously been shown that a secondary contact zone is located in central Scandinavia, where there are narrow clines for several morphological traits coincident with a migratory divide. Here we analyse multiple traits and genes from > 1700 males captured on breeding territories at 77 sites spread around the Baltic Sea to test the following hypothesis. If the secondary contact zone in Scandinavia is a result of divergence in two allopatric refuge populations during the last glaciation, we expect to find a similar secondary contact zone somewhere else around the circular distribution. Our results show that the trait clines were wider and displaced from each other along the eastern side of the Baltic Sea. Analyses of 12 microsatellite loci confirmed that the genome is very similar between the terminal forms (F(ST) = 0). Two AFLP-derived markers filtered out from a genomic scan instead appear to be maintained by selection. These markers exhibited steep clines at the secondary contact zone in Scandinavia, but as for the phenotypic traits, had vastly different cline centres east of the Baltic Sea. The trait clines along the ring distribution outside the Scandinavian secondary contact zone thus seem to have been shaped by independent action of selection or drift during the process of postglacial colonization.

    • The workshop covers ICF-CY as a framework in collaborative problem solving together with families and in preschool. A short introduction to ICF-CY will be presented. The workshop will be based on case studies where the participants will use ICF-CY as a tool in assessment and intervention. The model for collaborative problem solving developed by the CHILD group includes problem formulations, problem explanations, goal setting, design of methods, implementation and follow-up. The tool uses checklists based on ICF-CY for formulation of difficulties and problems in terms of activity and participation, problem explanations in terms of body structures and functions and environmental factors. An individual intervention plan for a child with disability based on functioning in everyday life is developed based on the assessment. The utility and feasibility of ICF-CY as a tool and a common language in early childhood intervention will be discussed.

    • In line with the primary purpose of ISEI, to provide a framework and forum for professionals from around the world to communicate about advances in the field of early intervention, this symposium aims to discuss and raise issues related to the added value of international collaboration in early childhood intervention research and education today and in the future. It is informed by experiences of the EU-US Transatlantic Consortium on Early Intervention (EU-US Atlantis Programme, Excellence Mobility Project 2001-2013) and from collaboration in ISEI. Building on multilateral collaboration and agreements between universities and funding agencies, based on equal standards for quality we can share and learn from each other. By developing joint priorities building on the strengths in individual countries a common theoretical framework for research and education related to early intervention has been created guiding policy and practice.

    • Early childhood intervention and habilitation services for children with disabilities operate on an interdisciplinary basis. It requires a common language between professionals, and a shared framework for intervention goals and intervention implementation. The International Classification of Functioning, Disability and Health (ICF) and the version for children and youth (ICF-CY) may serve as this common framework and language. This overview of studies implemented by our research group is based on three research questions: Do the ICF-CY conceptual model have a valid content and is it logically coherent when investigated empirically? Is the ICF-CY classification useful for documenting child characteristics in services? What difficulties and benefits are related to using ICF-CY model as a basis for intervention when it is implemented in services? A series of studies, undertaken by the CHILD researchers are analysed. The analysis is based on data sets from published studies or master theses. Results and conclusion show that the ICF-CY has a useful content and is logically coherent on model level. Professionals find it useful for documenting children's body functions and activities. Guidelines for separating activity and participation are needed. ICF-CY is a complex classification, implementing it in services is a long-term project.

    • BACKGROUND: We still have limited knowledge about the underlying genetic mechanisms that enable migrating species of birds to navigate the globe. Here we make an attempt to get insight into the genetic architecture controlling this complex innate behaviour. We contrast the gene expression profiles of two closely related songbird subspecies with divergent migratory phenotypes. In addition to comparing differences in migratory strategy we include a temporal component and contrast patterns between breeding adults and autumn migrating juvenile birds of both subspecies. The two willow warbler subspecies, Phylloscopus trochilus trochilus and P. t. acredula, are remarkably similar both in phenotype and genotype and have a narrow contact zone in central Scandinavia. Here we used a microarray gene chip representing 23,136 expressed sequence tags (ESTs) from the zebra finch Taeniopygia guttata to identify mRNA level differences in willow warbler brain tissue in relation to subspecies and season.RESULTS: Out of the 22,109 EST probe sets that remained after filtering poorly binding probes, we found 11,898 (51.8 %) probe sets that could be reliably and uniquely matched to a total of 6,758 orthologous zebra finch genes. The two subspecies showed very similar levels of gene expression with less than 0.1 % of the probe sets being significantly differentially expressed. In contrast, 3,045 (13.8 %) probe sets were found to be differently regulated between samples collected from breeding adults and autumn migrating juvenile birds. The genes found to be differentially expressed between seasons appeared to be enriched for functional roles in neuronal firing and neuronal synapse formation.CONCLUSIONS: Our results show that only few genes are differentially expressed between the subspecies. This suggests that the different migration strategies of the subspecies might be governed by few genes, or that the expression patterns of those genes are time-structured or tissue-specific in ways, which our approach fails to uncover. Our findings will be useful in the planning of new experiments designed to unravel the genes involved in the migratory program of birds.

  • Who should record surgical complications? : Results from a third-party assessment of complications after radical cystectomy

    • Objective: In Sweden complications after radical cystectomy have been reported to the nationwide population-based Swedish Cystectomy Registry since 2011. Here, validation of the reporting was assessed in two healthcare regions.Materials and methods: Complications were ascertained from patient records by a third party not involved in the care delivered to 429 randomly selected patients from 949 who had undergone radical cystectomy since 2011 in four hospitals. Without knowledge of the outcome in the primary registration, post-operative complications within 90 days post-operatively were assessed by an independent review of patient charts, and the results were compared with the primary reports in the Swedish Cystectomy Registry.Results: The third-party assessment identified post-operative complications in 310 patients (72%). Low-grade complications (Clavien-Dindo I-II) were noted in 110 (26%) of the patients in the primary registration, but increased to 182 (42%) in the validation (p < 0.00001). High-grade complications (Clavien-Dindo III-V) were reported in 113 (26%) patients in the primary registration, but in 128 (30%) of the patients in the validation (p = 0.02). According to the third-party assessment, 18 patients (4%) had Clavien-Dindo grade IV complications and 12 (3%) died within 90 days of surgery (Clavien-Dindo grade V); corresponding values in the primary registration were 15 (3%) and 9 (2%), respectively. The readmission rate within 90 days increased from 27 to 32% in the validation (p < 0.00001).Conclusions: Compared with registry data, third-party assessment revealed more complications and readmissions after radical cystectomy. Hence such evaluation may improve the validity of reported complication data.

    • Across countries, there are important differences related to the goals, organization, and educational philosophies of care provided to young children prior to formal schooling. Those differences are likely reflected in the classroom practices and teacher-child interactions within a country's early childhood education and care (ECEC) classrooms. This study aims to evaluate the within-country relevance of two classroom observation measures primarily based on a behavioral count approach focused on teacher and child behaviors; and to examine preschool practices in Sweden, Portugal, and the U.S., as they reflect each country's ECEC goals, organization, and educational philosophies. Participants are 78 preschool settings in Sweden, 42 in Portugal, and 168 in the U.S. Results show that the measures targeted culturally-relevant behaviors and provided inter-rater reliability for the behavior count variables in the three countries. Future collaborations may address additional culturally-specific variables. The behavioral descriptions yielded by combining behavioral counts of the measures are analyzed by researchers from the relevant country for insights to the country's values related to early childhood as well as current debates regarding care for children. Measures that provide comprehensive descriptions of classroom settings and apply minimal external or comparative value judgments on the behaviors observed are of practical utility for collaborative international work.

  • Feedback control of flow rate from a peristaltic pump using balance measurements

    • In many biotechnological processes it is important to perform exactdosing of the substrate into the bioreactor. The idea in this paper is to use the measurements from a balance to control the flow rate, since there is no access to the flow itself. The goals of the controller are several: it should be capable of handling load disturbances acting on the pump, measurement noise and even shocks acting on the balance. For this purpose an observer disregarding outliers is designed for low influence of the measurement noise. The observer-based feedback term compensates the load disturbances and the model errors. A dynamic feed forward structure is introduced to speed up the setpoint response. Simulation experiments show that the suggested controller structure works well.

    • Syftet med denna utredning har varit att se över hela datahanteringen i NILS för att kunna identifiera särskilt kritiska processer och för att få en bild av helheten. I rapporten presenteras en kravspecifikation för de nya system (datahantering, statistik/redovisning och datavärdskap) som behöver byggas upp, de resurser som behövs för detta samt ges ett förslag på hur och i vilken tidsordning som systemen bör byggas upp.I rapporten presenteras den rekommenderade framtida datahanteringen inom NILS, från datafångst till datavärdskap. Data som idag ingår i datahanteringen för NILS är fältdata, tolkningsdata samt medföljande metadata, stöddata och annan nödvändig dokumentation. I denna rapport föreslås att datahantering i NILS skall delas in i fyra olika huvudprocesser.Den första processen ärDatafångstprocessen som samlar in rådata, samt kvalitetssäkrar och levererar detta. I NILS datafångstprocess ingår i dagsläget två delar, dels fältinventering, dels flygbildstolkning av 1 km x 1 km rutan. Det är nödvändigt att uppbyggnaden av fältdatabas och tolkningsdatabas samordnas vad gäller såväl systemstruktur, variabelnamn mm för att underlätta samkörningar av data. Den största kraften och resurserna skall läggas på att effektivisera datahanteringen inom datafångsten för då uppnås den största nyttan med insatserna.Den andra processen ärDatabasprocessen som tar emot rådata, metadata och stöddata från fält- och flygbildsprocessen i datafångsten och lagrar detta i ett databassystem. I systemet lagras även beräknade data, alternativt beräkningsfunktioner (script) som utför beräkningar direkt beroende på olika användares behov. För både flygbildstolkning och fältinventering skall normaliserade databaser upprättas. Detta är en absolut nödvändighet och ett krav för att Databas och Datavärdskap skall vara genomförbart.Den tredje processen ärStatistikprocessen som producerar skattningar, bearbetningar, sammanställningar och resultatrapporter från NILS grunddata i databasen.Den fjärde processen utgörs av självaDatavärdskapet där det ingår att ta emot och kontrolleradata, tillgängliggöra data för externa användare (via webb-portal bl.a.) samt att permanent arkivera kvalitetssäkrade data. I datavärdskapet ingår grunddata från NILS rådatabas samt bearbetade data, resultatrapporter, sammanställningar av data, tabeller, figurer mm som producerats i Statistikprocessen.Denna rapport ger följande övergripande förslag till hur huvudprocesserna i NILS datahantering skall genomföras och byggas upp:Beställarens uppdrag skall vara klart definierat för utförarenTydligt uppsatta mål för arbetetUtförlig planering - baserat på tillräckligt kunskapsunderlagTydligt definierade roller för personalenAtt personalen har de personella, ekonomiska och materiella resurser som behövs för genomförandet (kompetens, tid, verktyg, utrustning mm)Planera helheten, men bygg från litet till stort, undvik onödig komplexitetDelleveranser till avnämare ska ske tidigt och fortlöpandeKvalitetssäkring ska finnas i alla processerI samtliga huvudprocesser skall det ingå tillräcklig säkerhetskopiering av data, anpassad till varje process särskilda kravTillräcklig dokumentation av alla ingående delprocesserTydlig strategi för att minimera riskerFortlöpande dialog och adaptiv utveckling – för att säkerställa att målen nåsKostnadseffektivitet i alla led – beakta möjligheter till intern och extern samordningSamverkan – såväl inom som utom SLU.Under arbetets gång har särskilt kritiska moment i utvecklingsarbetet identifierats bl.a. lednings- och organisationsstrukturen, val av programvara för databaserna samt kvalitetssäkring av rådata. Dessa bör lyftas fram i det fortsatta arbetet.Rapporten ger ett förslag till en 5-årsplan för uppbyggnad av huvudprocesserna. De mest kritiska processerna är uppbyggnaden av kvalitetsäkrade rådatabaser för fältinventeringen och flygbildstolkning. Fokus de första åren ligger därför på NILSbasen och statistikdelen. Uppbyggnaden av själva datavärdskapet senareläggs.Utifrån 5-årsplanen har en kostnadsberäkning gjorts att utveckla och ta i drift samtliga huvudprocesser och komponenter i datahanteringen. Den beräknade kostnaden för 2008 är 4415 kkr, 2009 5230 kkr, 2010 7147 kkr, 2011 7760 kkr och 2012 8406 kkr.

    • Interventions that focus on implementing augmentative and alternative communication (AAC) strategies and methods have been available to children in need of AAC and their families for at least 30 years. To date, most of the research that has considered AAC in family settings has been focused on gathering evidence of the effects of AAC interventions, rather than on implementing evidence-based strategies in everyday practice to improve outcomes. The purpose of this article is to discuss the research that has focused on parents as AAC interventionists, the family as a context for AAC intervention, and the effects of AAC interventions on children and other family members. The discussion is framed within the four steps associated with the process of knowledge translation: (a) deciding on desired outcomes of interventions, (b) evaluating evidence of the effectiveness of different AAC methods to obtain the desired outcomes, (c) translating the research evidence into everyday practice, and (d) implementing knowledge in practice.

    • This paper presents a state-of-the-art discussion of communication intervention for persons with profound disabilities in Sweden. The purpose of the paper is to relate communication intervention to levels of prevention in the perspective of three decades of normalization and integration. Research papers and clinical papers by Swedish authors on communication intervention with persons with profound disabilities are reviewed. The discussion focuses on the assessment, goals/objectives, and intervention methods for secondary and tertiary prevention. Areas of future research are discussed.

    • OBJECTIVE:This article discusses the use of a third qualifier, subjective experience of involvement, as a supplement to the qualifiers of capacity and performance, to anchor activity and participation as separate endpoints on a continuum of actions.DESIGN:Empirical data from correlational studies were used for secondary analyses. The analyses were focused on the conceptual roots of the participation construct as indicated by the focus of policy documents, the support for a third qualifier as indicated by correlational data, differences between self-ratings and ratings by others in measuring subjective experience of involvement, and the empirical support for a split between activity and participation in different domains of the activity and participation component.RESULTS:Participation seems to have two conceptual roots, one sociologic and one psychologic. The correlational pattern between the qualifiers of capacity, performance, and subjective experience of involvement indicates a possible split between activity and participation. Self-ratings of participation provide information not obtained through ratings by others, and later domains in the activities and participation component fit better with measures of experienced involvement than earlier domains did.CONCLUSIONS:The results from secondary analyses provide preliminary support for the use of a third qualifier measuring subjective experience of involvement to facilitate the split between activity and participation in the International Classification of Functioning, Disability and Health, Children and Youth version, activity and participation domain.
