    • Objectives To identify perceived deficiencies in the quality of abortion care among healthy women and those with mental stress. Methods This multi-centre cohort study included six obstetrics and gynaecology departments in Sweden. Posttraumatic stress (PTSD/PTSS) was assessed using the Screen Questionnaire-Posttraumatic Stress Disorder; anxiety and depressive symptoms, using the Hospital Anxiety Depression Scale; and abortion quality perceptions, using a modified version of the Quality from the Patient's Perspective questionnaire. Pain during medical abortion was assessed in a subsample using a visual analogue scale. Results Overall, 16% of the participants assessed the abortion care as being deficient, and 22% experienced intense pain during medical abortion. Women with PTSD/PTSS more often perceived the abortion care as deficient overall and differed from healthy women in reports of deficiencies in support, respectful treatment, opportunities for privacy and rest, and availability of support from a significant person during the procedure. There was a marginally significant difference between PTSD/PTSS and the comparison group for insufficient pain alleviation. Conclusions Women with PTSD/PTSS perceived abortion care to be deficient more often than did healthy women. These women do require extra support, relatively simple efforts to provide adequate pain alleviation, support and privacy during abortion may improve abortion care.

  • Induced Abortions and Posttraumatic Stress - Is there any relation? : A Swedish multi-centre study

    • Introduction: Induced abortion is a common medical intervention. Whether psychological sequelae might follow induced abortion has long been a subject of concern among researchers, and there is lack of knowledge about the relationship between posttraumatic disorder (PTSD) and induced abortion. Aims: To study and compare PTSD, posttraumatic stress symptoms (PTSS) and anxiety- and depressive symptoms among women seeking abortion, allowing for demographic variables. Further aims were to assess risk factors and to assess PTSD and PTSS following induced abortion in relation to experienced care at the clinic. Methods: This was a multi-centre cohort study targeting women who requested an induced abortion at the outpatient clinics of the gynaecology and obstetrics departments of six public hospitals in Sweden. All women who requested an induced abortion before the end of gestational week 12 were approached for participation. PTSD, PTSS, anxiety- and depressive symptoms, personality traits and women’s perceptions of abortion care were measured by means of questionnaires. Measurements were made at the first visit before the abortion as well as three- and six-months thereafter. Data collection was performed from September 2009 to January 2011. Results: 1,514 women filled out the questionnaire before the abortion. Abortion-seeking women did not suffer from PTSD to a greater extent than the general Swedish female population. Few women (51/720) developed PTSD or PTSS after the abortion, 11 did so due to trauma experience related to the abortion. Women at risk of posttraumatic stress were more likely to be young, having anxiety- or depressive symptoms and personality traits related to neuroticism. Furthermore, women with PTSD or PTSS were more likely to perceive certain aspects of the abortion care as deficient. Conclusions: The vast majority of women coped well with the induced abortion. Few developed posttraumatic stress post abortion. The majority did so because of trauma experiences unrelated to the induced abortion. Young women and women with mental distress are vulnerable groups that need to be paid attention to in abortion care. These women are at risk for negative experiences of the abortion care, and may be at risk of PTSD or PTSS post abortion

  • Posttraumatic stress among women after induced abortion : a Swedish multi-centre cohort study

Whether psychological sequelae might follow induced abortion has long been a subject of concern among researchers and little is known about the relationship between posttraumatic stress disorder (PTSD) and induced abortion. Thus, the aim of the study was to assess the prevalence of PTSD and posttraumatic stress symptoms (PTSS) before and at three and six months after induced abortion, and to describe the characteristics of the women who developed PTSD or PTSS after the abortion. Methods: This multi-centre cohort study included six departments of Obstetrics and Gynaecology in Sweden. The study included 1457 women who requested an induced abortion, among whom 742 women responded at the three-month follow-up and 641 women at the six-month follow-up. The Screen Questionnaire-Posttraumatic Stress Disorder (SQ-PTSD) was used for research diagnoses of PTSD and PTSS, and anxiety and depressive symptoms were evaluated by the Hospital Anxiety and Depression Scale (HADS). Measurements were made at the first visit and at three and six months after the abortion. The 95% confidence intervals for the prevalence of lifetime or ongoing PTSD and PTSS were calculated using the normal approximation. The chi-square test and the Student's t-test were used to compare data between groups. Results: The prevalence of ongoing PTSD and PTSS before the abortion was 4.3% and 23.5%, respectively, concomitant with high levels of anxiety and depression. At three months the corresponding rates were 2.0% and 4.6%, at six months 1.9% and 6.1%, respectively. Dropouts had higher rates of PTSD and PTSS. Fifty-one women developed PTSD or PTSS during the observation period. They were young, less well educated, needed counselling, and had high levels of anxiety and depressive symptoms. During the observation period 57 women had trauma experiences, among whom 11 developed PTSD or PTSS and reported a traumatic experience in relation to the abortion. Conclusion: Few women developed PTSD or PTSS after the abortion. The majority did so because of trauma experiences unrelated to the induced abortion. Concomitant symptoms of depression and anxiety call for clinical alertness and support.

Women who reported symptoms of anxiety or depression when requesting abortion were more likely to have ongoing PTSD or PTSS. Also single-living women and smokers displayed higher rates of ongoing PTSD. less thanbrgreater than less thanbrgreater thanConclusions Although PTSD is rare among women who request an induced abortion, a relatively high proportion suffers from PTSS. Abortion seeking women with trauma experiences and existing or preexisting mental disorders need more consideration and alertness when counselled for termination.

    • The aim of the study is to investigate what critical and temporal thinking can mean for younger students (aged 9-10) that requires the adoption of various perspectives in the context of sustainable urban planning, and how such knowing can be orchestrated in joint theoretical exploration work. A particular area of interest is the concept of contradictions (as used in activity theory) as a potential didactic tool for the subject, i.e. in what way it can be a driver for students and teachers to jointly identify problems, explore knowledge content with different tools (learning models) and discuss possible creative solutions to environmental and sustainability issues.This study used the learning study research approach. In collaboration with teachers, lessons were designed, analysed and evaluated to extract knowledge through the iterative process that characterises this approach. The design of the research lessons was structured in accordance with Davydov’s theory of learning activity. The students’ experiences were analysed phenomenographically and the analysis of the lessons was guided by Engeström and Sannino’s conceptual tools for how contradictions can manifest themselves (such as in dilemmas, conflicts and double-binds). The results of the study include phenomenographical descriptions of what it means to know how to use a natural site for sustainable urban planning, and what critical aspects students need to be able to discern in order to be competent to participate in such a practice. The results also show that the way contradictions are manifested in joint work (involving teachers and students) affects the form of practice that develops and the opportunities for students to jointly make the problem their own and explore the complexity of sustainable urban planning using mediating tools (learning models).The discussion highlights how the results of the study can be used as tools for designing, implementing and evaluating teaching, and, more broadly, the results are reflected in ways in which the task used in the research lessons can be developed.

  • Pandemrix-induced narcolepsy is associated with genes related to immunity and neuronal survival

    • BACKGROUND: The incidence of narcolepsy rose sharply after the swine influenza A (H1N1) vaccination campaign with Pandemrix. Narcolepsy is an immune-related disorder with excessive daytime sleepiness. The most frequent form is strongly associated with HLA-DQB1*06:02, but only a minority of carriers develop narcolepsy. We aimed to identify genetic markers that predispose to Pandemrix-induced narcolepsy.METHODS: We tested for genome-wide and candidate gene associations in 42 narcolepsy cases and 4981 controls. Genotyping was performed on Illumina arrays, HLA alleles were imputed using SNP2HLA, and single nucleotide polymorphisms were imputed using the haplotype reference consortium panel. The genome-wide significance threshold was p < 5 × 10-8, and the nominal threshold was p < 0.05. Results were replicated in 32 cases and 7125 controls. Chromatin data was used for functional annotation.FINDINGS: Carrying HLA-DQB1*06:02 was significantly associated with narcolepsy, odds ratio (OR) 39.4 [95% confidence interval (CI) 11.3, 137], p = 7.9 × 10-9. After adjustment for HLA, GDNF-AS1 (rs62360233) was significantly associated, OR = 8.7 [95% CI 4.2, 17.5], p = 2.6 × 10-9, and this was replicated, OR = 3.4 [95% CI 1.2-9.6], p = 0.022. Functional analysis revealed variants in high LD with rs62360233 that might explain the detected association. The candidate immune-gene locus TRAJ (rs1154155) was nominally associated in both the discovery and replication cohorts, meta-analysis OR = 2.0 [95% CI 1.4, 2.8], p = 0.0002.INTERPRETATION: We found a novel association between Pandemrix-induced narcolepsy and the non-coding RNA gene GDNF-AS1, which has been shown to regulate expression of the essential neurotrophic factor GDNF. Changes in regulation of GDNF have been associated with neurodegenerative diseases. This finding may increase the understanding of disease mechanisms underlying narcolepsy. Associations between Pandemrix-induced narcolepsy and immune-related genes were replicated.

    • Electronic health records contain a wealth of epidemiological information about diseases at the population level. Using a database of medical diagnoses and drug prescriptions in electronic health records, we investigate the correlation between outdoor temperature and the incidence of epistaxis over time for two groups of patients. One group consists of patients that had been diagnosed with epistaxis and also been prescribed at least one of the three anti-thrombotic agents: Warfarin, Apixaban, or Rivaroxaban. The other group consists of patients that had been diagnosed with epistaxis and not been prescribed any of the three anti-thrombotic drugs. We find a strong negative correlation between the incidence of epistaxis and outdoor temperature for the group that had not been prescribed any of the three anti-thrombotic drugs, while there is a weaker correlation between incidence of epistaxis and outdoor temperature for the other group. It is, however, clear that both groups are affected in a similar way, such that the incidence of epistaxis increases with colder temperatures.

    • Background: Negative aspects, staff dissatisfaction and problems related to internal organisational factors of working in elderly care are well-known and documented. Much less is known about positive aspects of working in elderly care, and therefore, this study focuses on such positive factors in Swedish elderly care. We combined two theoretical models, the effort–reward imbalance model and the Transformational Leadership Style model. The aim was to estimate the potential associations between employee-perceived transformational leadership style of their managers, and employees' ratings of effort and reward within elderly care work.Methods: The article is based on questionnaires distributed at on-site visits to registered nurses, occupational therapists, physiotherapists (high-level education) and assistant nurses (low-level education) in nine Swedish elderly care facilities. In order to grasp the positive factors of work in elderly care, we focused on balance at work, rather than imbalance.Results: We found a significant association between employees' effort–reward balance at work and a transformational leadership style among managers. An association was also found between employees' level of education and their assessments of the first-level managers.Conclusions: We conclude that the first-level manager is an important actor for achieving a good workplace within elderly care, since she/he influences employees' psychosocial working environment. We also conclude that there are differences and inequalities, in terms of well-being, effort and reward at the work place, between those with academic training and those without, in that the former group to a higher degree evaluated their first-level manager to perform a transformational leadership style, which in turn is beneficial for their psychosocial work environment. Consequently, this (re)-produce inequalities in terms of well-being, effort and reward among the employees at the work place.

    • Neoliberal policies such as new public management (NPM) have been pivotal to the Swedish elderly care system for two decades. This article explores the discourses of NPM and work by focusing on how a good workplace is represented by professionals and managers in Swedish elderly care. Using qualitative interviews with 31 managers, nurses, physiotherapists, and occupational therapists at nine workplaces, we identified four competing meanings (“storylines”) of how a good workplace is constructed among the interviewees within an ongoing struggle between two discourses. Three storylines, i.e., striving to achieve the mission, a desire to work in elderly care, and striving for good working relationships, are linked to the neoliberal discourse of organizational effectiveness. In contrast, the fourth storyline, support and better working conditions, is related to a welfare-state discourse of traditional labor relations with strong historical roots. Four subject positions available to the managers and professionals were identified: the bureaucrat, the passionate, the professional, and the critic. We conclude that NPM is translated on top of existing discourses, such as those of traditional labor relations, care ideals, and practices, that are already established in elderly care workplaces and that counteract the new policy.

    • Objective: Policy documents govern how the prevention and care of obstetric anal sphincter injuries (OASIS) are implemented. Thus, in the absence of Swedish national guidelines on OASIS, differing views may be visible in the regional and local policy documents. Therefore, we aimed to analyse regional and local policies, guidelines, and care programs on the prevention of OASIS and care for OASIS-affected women in a Swedish context by applying a critical frame analysis inspired by Verloo.Design and setting: A cross-sectional study of existing policy documents from Swedish healthcare regions was performed. The documents were analysed using Verloo's critical frame analysis.Findings: We found that OASIS was framed as a preventable problem addressed by skilled protective manoeuvres of the healthcare staff. Education, communication, and teamwork were three frames of crucial solutions to minimise the prevalence of OASIS. However, complicating power dimensions between professional groups and between professionals and birthing women were identified. Furthermore, several discursive struggles were found, predominantly regarding the scientific evidence for the suggested prevention and care.Conclusion: The policy documents emphasised that OASIS is preventable, and improved education, communication, and teamwork could diminish the OASIS prevalence. Nevertheless, power dimensions and discursive struggles may challenge the preventive efforts. Furthermore, each Swedish region has the sovereignty to develop its policies, which was reflected in our findings and may imply inequities in care provision. Thus, there is an urgent need to develop comprehensive national high-quality guidelines of high quality for OASIS prevention and care so that all women giving birth have access to equal care and treatment in Sweden.

Under det senaste decenniet har flera myndigheter gjort riktade satsningar för att stärka praktiknära forskning till nytta för utveckling av förskola, skola och lärarutbildning. Det så kallade ULF-avtalet är exempel på en av de större satsningarna. Boken Utbildning, lärande och praktiknära forskning har ambitionen att lyfta, diskutera och problematisera frågor om samverkan, symmetri, relationer, roller, etik samt ansvar och handlingsutrymme i samverkan kring praktiknära forskning. Men framför allt vill denna bok bidra till, och fördjupa diskussionen om dilemman, utmaningar och möjligheter som kan uppstå när skolverksamhet och lärosäte ska samverka kring utveckling, lärande och praktiknära forskning I boken har vi samlat bidrag från representanter från lärosäte, lärarutbildning, forskare, skolhuvudmän, skolledare, studenter och lärare som på olika sätt har erfarenhet av lärarutbildningens och skol- och förskoleverksamhetens vetenskapliga grund och samverkan kring praktiknära forskning. Boken utgör ett led i satsningen på Framtidens lärarutbildning och ULF-avtalet vid Örebro universitet.

    • This paper outlines a method for exploring learning in educational practice. The suggested method combines an explicit learning theory with robust methodological steps in order to explore aspects of learning in school physical education. The design of the study is based on sociocultural learning theory, and the approach adds to previous research within the field, both in terms of the combination of methods used and the claims made in our studies. The paper describes a way of collecting and analysing the retrieved data and discusses and illustrates the results of a study using this combination of explicit learning theory and robust methodological steps.

  • Oral health in patients scheduled for hematopoietic stem cell transplantation in the Orastem study

    • Despite advances in transplant medicine, prevalence of complications after hematopoietic stem cell transplantation (HSCT) remains high. The impact of pre-HSCT oral health factors on the incidence and severity of complications post-HSCT is poorly understood. The aim of this prospective, observational study was to analyze oral health in patients planned for HSCT. Patients >= 18 years requiring HSCT were included from five sites between 2011-2018. General health, oral findings and patient-reported symptoms were registered in 272 patients. Oral symptoms around disease onset were reported by 43 patients (15.9%) and 153 patients (58.8%) reported oral complications during previous chemotherapy. One third of patients experienced oral symptoms at the oral examination before conditioning regimen and HSCT. In total, 124 (46.1%) patients had dental caries, 63 (29.0%) had >= one tooth with deep periodontal pockets, 147 (75.0%) had >= one tooth with bleeding on probing. Apical periodontitis was observed in almost 1/4 and partially impacted teeth in 17 (6.3%) patients. Oral mucosal lesions were observed in 84 patients (30.9%). A total of 45 (17.4%) of 259 patients had at least one acute issue to be managed prior to HSCT. In conclusion, oral symptoms and manifestations of oral disease were prevalent in patients planned for HSCT. The extent of oral and acute dental diseases calls for general oral screening of patients pre-HSCT.

  • Couples' experiences of an extended information visit about prenatal screening : decision making and satisfaction

    • The overall aim of this thesis was to describe pregnant women's and partners' views and experiences on early prenatal screening with the combined test, with special focus on the two-step information model.Interviews were performed with 15 couples who had taken part in the extended information visit about prenatal screening, describing their perceptions of the information model (I) and ten couples or women of those, for a follow-up interview exploring their decision-making process (II). Seven couples, who had not taken part in the extended information visit, were interviewed describing their views and experiences about prenatal screening (III). A questionnaire was answered by 295 women and by 223 partners about their satisfaction about the decision whether or not to participate in the combined test, and their assessment of whether or not this choice had been difficult (IV).The results showed that different opinions were expressed about the offer of the extended information visit. The separate visit was welcomed by most couples (I). The decision-making process regarding whether to take part in the test or not was described by most couples as a fairly straightforward decision, while for others it was a more complex process that required a great deal of consideration (II). An apprehension of the test, by some of those who had refrained the extended information visit, was that it was an expression of society's involvement in decisions that belong to the expectant parents (III). Ninety-three percent of both women and partners considered the decision about participating in the combined tests as uncomplicated, and well over 90%, of both women and partners were satisfied with their decision (IV).The conclusions in this thesis, are that the decision whether or not to participate in the combined test is multidimensional and influenced by different views. The two-step information model helped the pregnant woman and the partner to make a decision in a fairly straightforward process or a more complex process with mixed feelings.

    • "Samernas långa historia i norra Skandinavien har aldrig ifrågasatts. Att den ifrågasätts i mellersta Skandinavien beror delvis på tolkningssvårigheter, mycket på grund av den starka påverkan från nordisk kultur som präglat området, i ökande grad från tiden efter Kr.f. Samisk kultur här kallas ofta ”fångstkultur”, vilket innebär ett slags osynliggörande. Mot nordisk-bofast ställs utifrån detta perspektiv samisk-nomadisk. Genom arkeologiska undersökningar på 1980-talet vid ett samiskt gravfält från 1000–1100-tal på Vivallen i Härjedalen kunde en boplats med typiskt samiska härdar, daterade till ca 800- och 1200-tal, jämte en avfallshög från1000-talet, lokaliseras. Utifrån detta lade det svensknorska ”Sörsamiska projektet” fram en tvåkultursyn samiskt–nordiskt för Mellanskandinavien, till Hedmark och Dalarna i söder. Den har av vissa kritiserats som alltför ”svart-vit”. Den har dock under senare år framhållits av en rad arkeologer för södra Norge, om än medett något modernare synsätt. Jag skall här bara ta upp några arbeten, som specifikt berör sydsamisk järnålder och tidig medeltid, ur den rika floran av arbeten om samisk arkeologi från norsk sida."  Inger Zachrisson, fil. dr och docent i nordisk arkeologi, f.d. förste antikvarie vid Statens historiska museum, Stockholm. Hon har sedan 1970 arbetat med samisk arkeologi och historia från järnåldern och framåt och relationerna mellan samisk och nordisk kultur i äldre tid. Hon initierade och genomförde det Sörsamiska projektet, ett svensknorskt arkeologisk-osteologiskt forskningsprojekt, med bland annat utgrävningar av samiskafornlämningar i Härjedalen. Det resulterade bl.a.i den tvärvetenskapliga publikationen Möten i gränsland (Zachrisson et al., 1997).

    • Background: This article focuses on knowledge development and health professionals’ opportunitiesto use evidence-based practice (EBP). We studied registered physiotherapists (PT), occupationaltherapists (OT) and nurses (RN) in Swedish elderly-care institutions, a sector known for highturnover and shortages of competent staff.Objective: To examine the perspectives of healthcare providers on professional knowledge developmentand EBP in their organization.Methods: We conducted on-site qualitative interviews with a purposive sample of PTs, OTs andRNs, in six elderly care institutions. Situational analysis was used to analyze the material.Results: Three discursive professional positions were found: 1) Professional ambition in confusingwork organization; 2) Professional ambition in a knowledge-promoting work organization; and 3) Professional indifference with few aspirations for knowledge development. Professional aspirationstoward knowledge development were high in two of these positions, whereas the third representsa slightly different approach with fewer aspirations for knowledge development. Linked to theseprofessional approaches to knowledge development is a continuum of aggravating or facilitatingfactors within the work organization, including varying degrees of support from leadership of theorganization, as well as few opportunities for rewards.Discussion and conclusions: It is concluded that elderly care needs to develop strategies forevidence-based practice in order for the sector to become a sustainable arena for health professionals’career development, and in order to improve the quality of care for the elderly.

  • Team social cohesion, professionalism, and patient-centeredness : gendered care work, with special reference to elderly care – a mixed methods study

    • Background: Healthcare organisations are facing large demands in recruiting employees with adequate competency to care for the increasing numbers of elderly. High degrees of turnover and dissatisfaction with working conditions are common. The gendered notion of care work as 'women's work', in combination with low salaries and status, may contribute to negative work experiences. There is abundant information about the negative aspects of elderly care health services, but little is known about positive aspects of this work. The study aim was to investigate work satisfaction from a gender perspective among Swedish registered nurses, physiotherapists, and occupational therapists, focusing specifically on healthcare services for the elderly.Methods: A mixed methods approach was adopted in which we combined statistics and open-ended responses from a national survey with qualitative research interviews with healthcare professionals in elderly care organisations. The survey was administered to a random sample of 1578 registered nurses, physiotherapists, and occupational therapists. Qualitative interviews with 17 professionals were conducted in six elderly care facilities. Qualitative and quantitative content analyses, chi2 and constructivist grounded theory were used to analyse the data.Results: There was a statistically significant difference in overall work satisfaction between those who worked in elderly care and those who did not (64 and 74,4% respectively, p < 0.001). Nine themes were derived from open-ended responses in the questionnaire. The qualitative interviews revealed four prominent storylines: 'Team social cohesion', 'Career development and autonomy', 'Client-centeredness', and 'Invisible and ignored power structures'.Conclusions: The results show the complexity of elderly care work and describe several aspects that are important for work satisfaction among health professionals. The results reveal that work satisfaction is dependent on social interrelations and cohesion in the work team, in possibilities to use humour and to have fun together, and in the ability to work as professionals to provide client-centered elderly care. Power relations such as gendered hierarchies were less visible or even ignored aspects of work satisfaction. The storylines are clearly linked to the two central discourses of professionalism and gender equality.

  • Newer antiepileptic drugs in women of child-bearing age : pharmacokinetic studies during pregnancy, breastfeeding, and contraception

    • Epilepsy is a common neurological disorder, affecting men and women almost equally. It has been estimated that 0.3 to 0.4% of pregnant women have epilepsy. Treating women with antiepileptic drugs (AEDs) during pregnancy is particularly challenging because the potential teratogenic effects of AEDs need to be balanced against the risks associated with uncontrolled seizures, which may be a threat to both the mother and fetus. Good knowledge on the drugs' pharmacokinetics in pregnancy and during lactation is required for an optimal use of AEDs. Pregnancy is known to affect the pharmacokinetics of the older generation AEDs, but the information on pharmacokinetics of the newer AEDs in pregnant and lactating women has been very limited. The overall aim of the present research project was to investigate the pharmacokinetics during pregnancy, lactation, and contraception of some of the most frequently used of the newer generation AEDs, lamotrigine (LTG), topiramate (TPM), and gabapentin (GBP). A marked decline in plasma levels of LTG during pregnancy was found. For further investigation of the underlying mechanisms behind this pharmacokinetic alteration a new method (liquid chromatography-mass spectrometry) was developed for quantification of LTG and three of its metabolites. The observed increase in the ratio between LTG and the metabolite 2-N-GLUC in plasma during pregnancy suggests that the decreased plasma LTG levels may be related to an increased metabolism of LTG by glucuronidation. LTG and TPM were shown to transfer freely over placenta, giving an umbilical cord/maternal plasma level ratio close to one. In contrast, the observations may indicate an active transplacental transport of GBP with accumulation in the fetus, giving an umbilical/maternal plasma ratio close to 2. The elimination of LTG in newborns seemed to be slow, probably due to an undeveloped hepatic glucuronidation capacity. Preliminary observations indicate that newborns have a slightly lower capacity to eliminate GBP than adults, probably due to age-related lower renal capacity. The data suggest that newborns have a good capacity to eliminate TPM. There is an extensive passage of LTG into breast milk and a comparatively slow elimination of LTG in the newborn, which may result in infant plasma levels reaching concentrations comparable to what is seen during active therapy. However, no adverse effects were noted in the infants. The transfer of GBP and TPM into breast milk is considerable, but plasma levels in the nursed infants were low and no adverse effects observed. LTG plasma levels were reduced by more than 50% during co-medication with oral contraceptives (OC), probably due to an increase in LTG glucuronidation rate. The underlying mechanisms are not known but may be similar to those involved during pregnancy. In conclusion, our findings provide new information on the pharmacokinetics of three of the newer AEDs in relation to pregnancy and lactation. They also highlight the usefulness of TDM as a tool for identifying pharmacokinetic alterations of clinical relevance, for instance due to physiological changes such as pregnancy, and for identification of unexpected drug-drug interactions such as those between OC and LTG. Such knowledge may contribute to an individualized and safer drug therapy for women treated with those newer AEDs.

  • Topiramate kinetics during delivery, lactation, and in the neonate : Preliminary observations

    • Purpose: To study the pharmacokinetics of topiramate (TPM) during delivery, lactation, and in the neonate. Methods: TPM concentrations in plasma and breast milk were measured with fluorescence polarization immunoassay (FPIA) in five women with epilepsy treated with TPM during pregnancy and lactation. Blood samples were obtained at delivery from mothers, from the umbilical cord, and from the newborns on three occasions (24, 48, and 72 h) after delivery. Blood and breast milk also were collected from mothers 2 weeks, and 1 and 3 months postpartum. Blood samples also were drawn from the infants during breast-feeding. Three of the mother-infant pairs were studied both at delivery and during lactation, two contributed with data from delivery only. Results: The umbilical cord plasma/maternal plasma ratios were close to unity, suggesting extensive transplacental transfer of TPM. The mean milk/maternal plasma concentration ratio was 0.86 (range, 0.67-1.1) at 2-3 weeks after delivery. The milk/maternal plasma concentration ratios at sampling 1 and 3 months after delivery were similar (0.86 and 0.69, respectively). Two to 3 weeks after delivery, two of the breast-fed infants had detectable (>0.9 ╡M) concentrations of TPM, although below the limit of quantification (2.8 ╡M), and one had an undetectable concentration. Conclusions: Our limited data suggest free passage of TPM over the placenta and an extensive transfer into breast milk. Breast-fed infants had very low TPM concentrations, and no adverse effects were observed in the infants.

  • "Här kan jag äntligen tala" : tematik och litterär gestaltning i Åsa Nelvins författarskap

    • The subject of this thesis is Swedish author Åsa Nelvin’s writings. Five books are at the center of attention: The children’s books De vita björnarna (“The White Bears”) (1969) and Det lilla landet (“The Little Country”) (1971), the two novels Tillflyktens hus (“The House of Refuge”) (1975) and Kvinnan som lekte med dockor (“The Woman who played with dolls”) (1977), and the cycle of poems Gattet (“The Inlet”) (1981). The overriding aim of the the thesis is to characterize Nelvin’s literary works and her over-all literary enterprise.A major part of the thesis is devoted to textual analyses, the purpose of which is to identify fundamental themes and literary strategies in Nelvin’s works. Also treated is the author’s development from being a prose writer to becoming a lyric poet. In addition, the thesis examines how Nelvin’s writings relate to the cultural climate of the 1960s and 1970s; it deals with the reception of her works, and the question whether literary ideals and conventions have had any impact on her writings.Nelvin is generally associated with the emancipatory tradition of female autobiographers in the 1970s. Her works are considered to engage in psychological and/or gender issues and to be openly autobiographical. This thesis partly argues against that view by asserting that that the fundamental concern of Nelvin’s works is the experience of being and the creation of meaning. The textual analyses show that Nelvin’s writings in the main deal with existential matters such as time and death, art and eternity. They also show that the autobiographical elements in her works are not primarily directed towards the construction of womanhood or therapeutic confession. They rather serve as an aesthetic and meta-literary function, drawing attention to questions concerning the construction of the text (and life), and differences or correspondences between phenomena like fiction and reality.

    • This thesis aims to develop meanings of sustainable programme development. Policies are often introduced to solve societal problems identified by a government agency or some other organization. In order to realize the intentions behind the policy it is common that one or several policy driven projects are funded with large sums of money. By funding policy driven projects, the funding organizations expect that the projects can support a sustainable change of practice that can solve or at least decrease the societal problem. Research shows that policy driven projects and programmes often fail to realize the intentions behind poli-cies. Hence, it is uncertain to what extent policy driven projects contribute to a sustainable change of practice.
