  • Talk through IT : Using Common Ground to build Bridges with Weak Ties in the Design of Online Psychosocial Support

    • There is growing interest in applying information and communication technology (ICT) in healthcare. The attention to ICT stems from several factors: costs of healthcare, high prevalence of somatic diseases, longer life expectancy and technological advancements. The quest to meet the growing healthcare needs has led authorities to allocate resources to develop technological solutions. Although this research investment is helpful for the information technology sector, these projects face challenges in working with multidisciplinary teams, recruiting participants and moving research to practice. This dissertation investigates the potential of ICT use in psychosocial support communication designed for people with emotional distress associated with a somatic disease (PEDASDs). The research aims to deliver design principles (DPs) for these ICTs, for three stakeholder groups: nurses, therapists and PEDASDs. The DPs focus on communication between these stakeholder groups and not on the overall ICT design. An interpretative approach was adopted to investigate the stakeholders’ unique ICT requirements. This method allows insights into the stakeholder’s world. Three studies, using semi-structured interviews, at oncology facilities in two healthcare system models (Trinidad & Tobago and Sweden), provide the empirical data. The stakeholders revealed not only their communication support needs but also other issues including the practical distresses of living with a chronic disease, limited support, lack of information, unwillingness to associate with other PEDASDs, professionalism between healthcare providers and disconnected healthcare services. Common ground, bridges and weak ties provide the theoretical framework to explore the findings. Eight DPs emerged from the studies: the healthcare system model, need to educate the stakeholders, ICT customisation, support of stakeholders’ information needs, allow access to external resources, support multidisciplinary team communication, support for self-care and support for significant others. The findings could potentially serve as a guide for ICT projects that develop artefacts for psychosocial support and provide a theoretically grounded understanding of stakeholder communication and relationships. The potential for future work includes investigating the issues of legislation and ethics, studying the needs of significant others, exploring the potential for ICTs in other healthcare system models (particularly the out-of-pocket health system) and examining the impact of social media on psychosocial support.

    • This article contributes to our understanding of practices in innovating organizations. Previous studies have demonstrated how breakthroughs in knowledge may fail to be translated into practices if they are not aligned with existing practices, or if they cut across established boundaries and power structures. By drawing upon an ethnographic study of a medical R&D department that has been highly successful in developing new medical practices, this article investigates how such challenges can be overcome. To date, much of the literature has focused on coordination across single, well-defined boundaries. We here extend this focus and introduce the notion of 'boundary organizing' to analyse highly political and contingent processes of innovation and change within and across different practices. We add to existing literature by highlighting how the handling of multiple boundaries, the indirect effects of boundary work, the negotiation of mutual benefits and interests, and mutual adaptation are key aspects of boundary organizing.

    • Recent advancements on artificial intelligence (AI) technologies have resurfaced utopian and dystopian narratives on the future of work. The uniqueness of the AI technologies is their potential to supplement, substitute and amplify practically all tasks currently performed by humans. Hence, they hold the potential to rapidly reshape the nature of work and our economies (Frank et al., 2019). “How?” is the question. Optimists focus on its technological possibilities and postulate that AI will free humans from boring, repetitive tasks - allowing the creation of higher quality jobs - and emphasize new opportunities for organizations to create a competitive advantage. Skeptics, on the other side, highlight the possibility of increased unemployment or work degradation as human deskilling occurs due to an increased degree of decision and knowledge work delegation.

    • DIGITAL INCLUSION is a book about life in digital societies. Taking a Scandinavian outlook, it zooms in on everyday experiences of public sector digitalization in some of the world’s most digitalized countries. Grounded in studies of how exclusionary effects of digitalization are dealt with in local circumstances, the book offers insight into aspects of the digital transformation that are mostly invisible. Over the course of seven chapters, the book gives examples of how relatives help each other get access to online services. It emphasizes the broad range of situations in which digital support takes place, including by volunteers and professionals in the front line of the public sector. An overview of political digitalization strategies in Scandinavia frames the discussion of how to move towards consideration of citizen values and interests. It presents concepts and methods to move away from the narrow idea of the citizen as a ‘user’ thereby paving the way for better support of human-technology interaction and increasing public engagement in digital society decision making. Digital Inclusion is a contribution to current attempts at making digitalization serve the public better. It is written for policy makers and managers in the public sector but equally suited for citizens with a desire to understand society’s digital transformation and its everyday consequences and possibilities.

  • Interoperability Capability to interoperate in a shared work practice using information infrastructures : studies in ePrescribing

    • The ability to interoperate between systems, people, and organizations is considered an important issue within eHealth in order to deliver patient centered care. The achieving and improving of interoperability is a complex undertaking involving the evolution of an information infrastructure, sharing of knowledge and resources, governance of the interoperation between organizations, people and work practices, and handling of economic and legal matters.This thesis contributes with practical knowledge on improving interoperability, based on active participation in and empirical studies of improving interoperability in ePrescribing. A case study describes and analyzes the evolution of ePrescribing in Sweden since the early pioneering years in 1980s, its growth and consolidation before the reregulation of the pharmacy market in 2009. Apractical theory on ePrescribing is presented. A unique field experimental study measuring improvement of interoperability in ePrescribing, before and after a major intervention to improve the quality of ePrescriptions between 2004 and 2009 is presented.Furthermore, a practical theory on interoperation and interoperability is presented. Interoperability is seen as the exercised capability of organizations through their agents to interoperate in a shared work practice in an effective, efficient, and satisfactory manner based on a common ground in a mediated, prescriptive, and non-personal communicative setting using an information infrastructure for mediating interoperation.
