    • For the last two and a half decades, a network of human health experts under the Arctic Monitoring and Assessment Program (AMAP) has produced several human health assessment reports. These reports have provided a base of scientific knowledge regarding environmental contaminants and their impact on human health in the Arctic. These reports provide scientific information and policy-relevant recommendations to Arctic governments. They also support international agreements such as the Stockholm Convention on Persistent Organic Pollutants (POPs) and the Minamata Convention on Mercury. Key topics discussed in this paper regarding future human health research in the circumpolar Arctic are continued contaminant biomonitoring, health effects research and risk communication. The objective of this paper is to describe knowledge gaps and future priorities for these fields.

    • Background: Improved understanding of the asymptomatic malaria parasite reservoir is a prerequisite to pursue malaria elimination efforts. We therefore characterised temporal trends and transporter polymorphisms in asymptomatic Plasmodium infections during the transition from high to low transmission in Zanzibar. Methods: Healthy individuals participating in cross-sectional surveys conducted 2005-2013 were screened for asymptomatic malaria by PCR. Complexity/diversity of infection and transporter polymorphisms were assessed in Plasmodium falciparum positive samples. Symptomatic samples were included for comparison of polymorphisms in 2013. Results: PCR-determined parasite prevalence declined from 21.1% (CI95% 17.4-24.9) to 2.3% (CI95% 1.7-2.9) from 2005 to 2013. P. falciparum remained the predominant species; prevalence was highest in children and young adults aged 5-25 years. Parasite densities and complexity of infection, but not population genetic diversity of P. falciparum, decreased from 2005-2009. pfcrt 761 (99.2-64.7%, p < 0.001) and pfmdr1 86Y frequencies (89.4-66.7%, p = 0.03) decreased over time. Pfmdr1 (a.a.86,184,1246) YYY and YYD haplotypes were more frequent in asymptomatic than symptomatic infections in 2013 (p < 0.001). Conclusions: There is a declining, albeit persistent, reservoir of parasites present at low-densities in asymptomatic individuals in Zanzibar. This study revealed important characteristics of the remaining parasite population, including intriguing temporal trends in molecular markers associated with antimalarial resistance, which need to be further investigated.     • Open channels are very important to deliver water from main sources to laterals especially for developing countries. Production is subjective by the way that the water is scheduled, and this scheduling is subject to several irrigation constraints. In open channel projects, for instance, maximum discharge of the laterals and main channels, depending on the size of their dimensions and the water requirements for fields. The current paper shows how efficient water scheduling, regarding the delivering water from the main channel to laterals in consequent time slots, can be done by utilizing a genetic algorithm optimisation technique. This research is intended to be applied for scheduling the Shattulhilla River in Babylon City and has broad applications for open channel projects in Iraq. The obtained results clarify how the genetic algorithm optimisation modelling is a sophisticated tool which operators of irrigation projects could now utilize to timetable open channels of irrigation systems.

  • Risk Stratification of Patients With Apparently Idiopathic Premature Ventricular Contractions A Multicenter International CMR Registry

    • OBJECTIVES This study investigated the prevalence and prognostic significance of concealed myocardial abnormalities identified by cardiac magnetic resonance (CMR) imaging in patients with apparently idiopathic premature ventricular contractions (PVCs). BACKGROUND The rote of CMR imaging in patients with frequent PVCs and otherwise negative diagnostic workup is uncertain. METHODS This was a multicenter, international study that included 518 patients (age 44 +/- 15 years; 57% men) with frequent (>1,000/24 h) PVCs and negative routine diagnostic workup. Patients underwent a comprehensive CMR protocol including late gadolinium enhancement imaging for detection of necrosis and/or fibrosis. The study endpoint was a composite of sudden cardiac death, resuscitated cardiac arrest, and nonfatal episodes of ventricular fibrillation or sustained ventricular tachycardia that required appropriate implantable cardioverter-defibrillator therapy. RESULTS Myocardial abnormalities were found in 85 (16%) patients. Mate gender (odds ratio [OR]: 4.28; 95% confidence interval [CI]: 2.06 to 8.93; p = 0.01), family history of sudden cardiac death and/or cardiomyopathy (OR: 3.61; 95% CI: 1.33 to 9.82; p = 0.01), multifocat PVCs (OR: 11.12; 95% CI: 4.35 to 28.46; p < 0.01), and non-left bundle branch block inferior axis morphology (OR: 14.11; 95% CI: 7.35 to 27.07; p < 0.01) were alt significantly related to the presence of myocardial abnormalities. After a median follow-up of 67 months, the composite endpoint occurred in 26 (5%) patients. Subjects with myocardial abnormalities on CMR had a higher incidence of the composite outcome (n = 25; 29%) compared with those without abnormalities (n = 1; 0.2%; p < 0.01). CONCLUSIONS CMR can identify concealed myocardial abnormalities in 16% of patients with apparently idiopathic frequent PVCs. Presence of myocardial abnormalities on CMR predict worse clinical outcomes. (C) 2019 by the American College of Cardiology Foundation.

  • Illegal harvesting threatens fruit production and seedling recruitment of Balanites aegyptiaca in Dinder Biosphere Reserve, Sudan

    • Illegal harvesting negatively affects the forest tree populations, particularly in sub-Saharan Africa, but little is known how fruit production and seedling recruitment are impacted. We assessed recruitment parameters of Balanites aegyptiaca trees in the Dinder Biosphere Reserve (DBR) across 100 sample plots of 25 m x 40 m in both human-impacted (disturbed) and undisturbed sites. We found that the average number of fruiting branches of B. aegyptiaca in the undisturbed sites were three times as high as those in the disturbed sites (F1,98 = 139, P < 0.001). Further, fruiting branches were positively correlated with crown width (R2 = 0.71, beta = 7.1, P = 0.01) across both sites. The height and crown width of B. aegyptiaca in the undisturbed sites were double that of the disturbed sites (F1,196 = 80, P < 0.001; F1,196 = 94.8, P < 0.001). Saplings and seedlings at the undisturbed sites were three times and twice that of the disturbed sites, respectively (F1,196 = 94.5, P < 0.001; F1,196 = 100.8, P < 0.001), with a positive correlation to the average number of fruiting branches (R2 = 0.74, beta = 0.45, P < 0.001). The soil nitrogen and phosphorus contents beneath trees in the undisturbed sites were almost double that of those in the disturbed sites (F1, 196 = 68.1, P < 0.001; F1, 196 = 97.9, P < 0.001) while sodium and electrical conductivity were by about 50% lower (F1, 196 = 535.8, P < 0.001; F1, 196 = 16.1, P < 0.001). We conclude that illegal harvesting in DBR severely reduced tree structure and recruitment parameters of B. aegyptiaca, which might also have impacted soil fertility. We urge for intensive monitoring and awarenessraising programs to conserve this vulnerable tree species.
