  • Effect of 12 mg vs 6 mg of Dexamethasone on the Number of Days Alive Without Life Support in Adults With COVID-19 and Severe Hypoxemia The COVID STEROID 2 Randomized Trial

    • Question What is the effect of 12 mg vs 6 mg of dexamethasone on the number of days alive without life support at 28 days in patients with COVID-19 and severe hypoxemia? Findings In this randomized trial that included 1000 patients with COVID-19 and severe hypoxemia, treatment with 12 mg/d of dexamethasone resulted in 22.0 days alive without life support at 28 days compared with 20.5 days in those receiving 6 mg/d of dexamethasone. This difference was not statistically significant. Meaning Compared with 6 mg of dexamethasone, 12 mg of dexamethasone did not statistically significantly reduce the number of days alive without life support at 28 days. This multicenter randomized clinical trial compares the effects of 12 mg/d vs 6 mg/d of dexamethasone in patients with COVID-19 and severe hypoxemia. IMPORTANCE A daily dose with 6 mg of dexamethasone is recommended for up to 10 days in patients with severe and critical COVID-19, but a higher dose may benefit those with more severe disease. OBJECTIVE To assess the effects of 12 mg/d vs 6 mg/d of dexamethasone in patients with COVID-19 and severe hypoxemia. DESIGN, SETTING, AND PARTICIPANTS A multicenter, randomized clinical trial was conducted between August 2020 and May 2021 at 26 hospitals in Europe and India and included 1000 adults with confirmed COVID-19 requiring at least 10 L/min of oxygen or mechanical ventilation. End of 90-day follow-up was on August 19, 2021. INTERVENTIONS Patients were randomized 1:1 to 12 mg/d of intravenous dexamethasone (n = 503) or 6 mg/d of intravenous dexamethasone (n = 497) for up to 10 days. MAIN OUTCOMES AND MEASURES The primary outcome was the number of days alive without life support (invasive mechanical ventilation, circulatory support, or kidney replacement therapy) at 28 days and was adjusted for stratification variables. Of the 8 prespecified secondary outcomes, 5 are included in this analysis (the number of days alive without life support at 90 days, the number of days alive out of the hospital at 90 days, mortality at 28 days and at 90 days, and >= 1 serious adverse reactions at 28 days). RESULTS Of the 1000 randomized patients, 982 were included (median age, 65 [IQR, 55-73] years; 305 [31%] women) and primary outcome data were available for 971 (491 in the 12 mg of dexamethasone group and 480 in the 6 mg of dexamethasone group). The median number of days alive without life support was 22.0 days (IQR, 6.0-28.0 days) in the 12 mg of dexamethasone group and 20.5 days (IQR, 4.0-28.0 days) in the 6 mg of dexamethasone group (adjusted mean difference, 1.3 days [95% CI, 0-2.6 days]; P = .07). Mortality at 28 days was 27.1% in the 12 mg of dexamethasone group vs 32.3% in the 6 mg of dexamethasone group (adjusted relative risk, 0.86 [99% CI, 0.68-1.08]). Mortality at 90 days was 32.0% in the 12 mg of dexamethasone group vs 37.7% in the 6 mg of dexamethasone group (adjusted relative risk, 0.87 [99% CI, 0.70-1.07]). Serious adverse reactions, including septic shock and invasive fungal infections, occurred in 11.3% in the 12 mg of dexamethasone group vs 13.4% in the 6 mg of dexamethasone group (adjusted relative risk, 0.83 [99% CI, 0.54-1.29]). CONCLUSIONS AND RELEVANCE Among patients with COVID-19 and severe hypoxemia, 12 mg/d of dexamethasone compared with 6 mg/d of dexamethasone did not result in statistically significantly more days alive without life support at 28 days. However, the trial may have been underpowered to identify a significant difference.

    • As members of the Distinguished Reviewers Board of Scientometrics and/or as recipients of the Derek de Solla Price Medal, we wish to express our disagreement with the retraction of the paper “Predatory publishing in Scopus: evidence on cross-country differences” co-authored by Vít Macháček and Martin Srholec (Macháček & Srholec, 2021a). The retraction was discussed in the blog Retraction Watch (Oransky, 2021), where important information about the context of the retraction was disclosed, including the pressure exerted by the publisher Frontiers on Scientometrics.

  • Health Status at Long-Term Follow-Up in Patients Treated for Extremity Localized Ewing Sarcoma or Osteosarcoma: A Scandinavian Sarcoma Group Study

    • Background. The purpose of this study was to evaluate late effects and symptom complaints in long-term survivors (>5 years) of Extremity Bone Sarcoma (EBS Survivors). The results were compared with findings in age- and gender-matched individuals from the general Population (NORMs). Patients and Methods. Among 155 EBS Survivors approached, 133 (86%) were included, and 110 of them (83%) attended an outpatient examination. Health status was evaluated by a mailed questionnaire concerning demographic and current health issues, and physical examinations at the outpatient clinic. Age- and gender-adjusted normative controls were drawn from participants of the Health Study of Nord-Trondelag County (HUNT 2). Results. Median age at follow-up was 29 (15-57) years. Median follow-up Was 12 (6-22) years. Of EBS Survivors 42% had >= 1 somatic disease, 33% had ototoxicity and 13% had reduced renal Function. EBS Survivors were more likely to have heart disease (odds ratio [OR], 7.9; 95% confidence interval [95% CI], 2.5-25.3; P=0.001), hypertension (OR, 3.4; 95% Cl, 1.1-10.1; P=0.03) and thyroid disease (OR, 3.0; 95%, Cl, 1.1-8.3; P=0.04) compared to NORMs. EBS Survivors reported more diarrhoea (29% vs. 19%, P=0.02), palpitations (23% vs. 13%, P=0.01) and shortness of breath (11% vs. 5%, P=0.01) than NORMs. Conclusions. EBS Survivors have poorer health status compared to age- and gender-matched controls. Long-term follow-up of these patients is therefore mandatory. Pediatr Blood Cancer 2009;53:84-89.   • Comparison of Cerebrospinal Fluid Amyloidogenic Nanoplaques With Core Biomarkers of Alzheimer's Disease

    • Accurate biomarkers of Alzheimer’s disease (AD) are essential for early diagnosis and intervention. Available biomarkers are not sufficient to permit the monitoring of AD progression over time, and additional biomarkers are required. Measures of aggregated amyloid-β (Aβ) could be useful biomarkers for AD. Here, we investigate whether levels of Thioflavin-T (ThT) positive amyloid aggregates, i.e., nanoplaques, in cerebrospinal fluid (CSF) could serve as useful biomarkers for AD. One-hundred and eighteen memory clinic patients were AT(N) classified, and CSF nanoplaque concentrations were compared between patients on the “Alzheimer’s continuum” (A+ patients) and patients with “Normal AD biomarkers” or “Non-AD pathologic change” (A− patients). CSF nanoplaque concentrations and sizes were quantified using the novel ThT-Fluorescence Correlation Spectroscopy (ThT-FCS) assay, and core biomarkers (Aβ42, total tau and phosphorylated tau) were determined by enzyme-linked immunosorbent assays. We investigated the association between nanoplaque concentrations and core biomarkers, and the diagnostic value of nanoplaque levels. Nanoplaque levels were increased in A+ patients compared to A− patients. Nanoplaque concentrations were negatively associated with Aβ42, but not related to total tau or phosphorylated tau measures. Quantification of nanoplaques did not improve the classification of patients on the Alzheimer’s continuum compared to the core biomarkers alone. Dynamic changes in nanoplaques concentration and size throughout AD stages should be explored in longitudinal studies.

  • Matrix metalloproteinases are associated with brain atrophy in cognitively unimpaired individuals

    • Matrix metalloproteinases (MMPs) and their tissue inhibitors (TIMPs) have been linked to age-related neurodegeneration and Alzheimer's disease (AD), but their role in normal aging is poorly understood. We used linear mixed models to determine if baseline or rate of yearly change in cerebrospinal fluid (CSF) levels of MMP-2; MMP-3; MMP-10; TIMP-123 (composite of TIMP-1, TIMP-2, and TIMP-3); or TIMP-4 predicted changes in bilateral entorhinal cortex thickness, hippocampal volume, or lateral ventricle volume in cognitively unimpaired individuals. We also assessed effects on the CSF AD biomarkers amyloid-& beta;42 and phosphorylated tau181. Low baseline levels of MMP-3 predicted larger ventricle volumes and more entorhinal cortex thinning. Increased CSF MMP-2 levels over time predicted more entorhinal thinning, hippocampal atrophy, and ventricular expansion, while increased TIMP-123 over time predicted ventricular expansion. No MMP/TIMPs predicted changes in CSF AD biomarkers. Notably, we show for the first time that longitudinal increases in MMP-2 and TIMP-123 levels may predict age-associated brain atrophy. In conclusion, MMPs and TIMPs may play a role in brain atrophy in cognitively unimpaired aging. & COPY; 2023 The Author(s). Published by Elsevier Inc.   • Environmental constraints of the invasive Mnemiopsis leidyi in Scandinavian waters

    • We studied the seasonal dynamics of the invasive ctenophore Mnemiopsis leidyi over its distribution range from Skagerrak into the Baltic Proper during 1 yr and related this to ambient physical and biological variables. The appearance of M. leidyi was sporadic in the Baltic Proper, with 60-fold lower abundance than in the Skagerrak and Kattegat (mean 0.02 individual m−3 and 1.16 individual m−3, respectively). M. leidyi typically resided above the halocline (10–20 m depth), except in the Baltic Proper where they were found deeper. Smaller size classes were underrepresented in the Baltic Proper, indicating failed reproduction or high mortality of the younger life stages. Ninety percent of the M. leidyi were observed at salinities of 22–29 and 75% of the individuals in water masses warmer than 11°C, i.e., typical late summer to autumn temperatures. Results of a Generalized Additive Model (GAM) indicate that the spatio-temporal distribution of M. leidyi might be explained by advection of M. leidyi from the Skagerrak and Kattegat area to the Baltic Proper. We conclude that the low salinity (< 9) of the Baltic Proper is likely to restrict successful reproduction and establishment of M. leidyi and that the advection of individuals from higher saline source areas sustained the observed Baltic Proper occurrence.

    • There is a concern that blooms of cnidarians and ctenophores, often referred to as jellyfish, are increasing in frequency and intensity worldwide and that there is a shift from fish- to jellyfish-dominated systems. We present an idealized analysis of the competitive relationship between zooplanktivorous jellyfish that is based on a generic model, termed ‘Killing the Winner’ (KtW), for the coexistence of 2 groups utilizing the same resource. Tactile predation by jellyfish makes them less dependent on water optics than fish using vision, and we modified the KtW model to account for this particular trait difference. Expectations of the model are illustrated by use of observations from the Baltic Sea. The model predicts a general succession on how mass of the system distributes when going from an oligotrophic to a eutrophic system. Initially the mass of the system accumulates at the level of the common resource (zooplankton) and planktivorous fish (sprat/herring). At one point, with increased eutrophication, mass starts to accumulate at the level of the top predator (cod) and at a later point, at the level of the jellyfish. For those organisms utilizing vision (fishes) an optimal degree of eutrophication and water clarity is predicted due to a 2-sided effect of eutrophication.

    • There is mounting evidence that vitamin D and its metabolites play important roles in regulating plasma calcium concentrations in teleost fish as in other vertebrates. The aims of the present study were to elucidate the possible cellular target mechanisms for the rapid actions of 24R,25(OH)(2)D(3), 25(OH)D(3) and 1,25(OH)(2)D(3) in Atlantic cod enterocytes at physiological doses, and to establish the concentration and thus the physiological range of circulating 24R,25(OH)(2)D(3), 25(OH)D(3) and 1,25(OH)(2)D(3) in the Atlantic cod. The plasma concentrations of 25(OH)D(3), 1,25(OH)(2)D(3) and 24R,25(OH)(2)D(3) were 15.3 +/- 2.7nM, 125.1 +/- 12.3pM and 10.1 +/- 23.5nM respectively. Exposure of enterocytes to 10mM calcium (Ca(2+)) evoked an increase in intracellular Ca(2+) concentrations ([Ca(2+)](i)). This increase was suppressed by 24R,25(OH)(2)D(3) dose-dependently, with an EC(50) of 4.9nM and a maximal inhibition of 60%. 24R,25(OH)(2)D(3) (20nM) abolished an increase in [Ca(2+)](i) (approximately 252%) in the control enterocytes exposed to 10microM S(-)-BAYK-8644, suggesting that the hormone acts by inhibiting Ca(2+) entry through L-type voltage-gated Ca(2+) channels. Administration of 20nM 24R,25(OH)(2)D(3) to enterocytes in the absence of extracellular Ca(2+) increased [Ca(2+)](i) by approximately 20%, indicating a release of Ca(2+) from intracellular stores. Administration of 25(OH)D(3) (20nM) resulted in a biphasic change in the enterocyte [Ca(2+)](i): within 1--5s, it decreased to 87 +/- 12nM below its mean basal [Ca(2+)](i) (334 +/- 13nM), followed by a rapid recovery of [Ca(2+)](i) to a new level, 10% lower than the initial [Ca(2+)](i). The rapid decrease, the recovery rate and the final [Ca(2+)](i) were all affected dose-dependently by 25(OH)D(3), with EC(50) values of 8.5, 17.0 and 18.9nM respectively. Furthermore, the effects of 25(OH)D(3) were sensitive to sodium (Na(+)), bepridil (10microM) and nifedipine (5 microM), suggesting that 25(OH)D(3) regulates the activity of both basolateral membrane-associated Na(+)/Ca(2+) exchangers and brush border membrane-associated L-type Ca(2+) channels. Administration of 25(OH)D(3) (10nM) to enterocytes in the absence of extracellular Ca(2+) increased [Ca(2+)](i) by approximately 18%, indicating a release of Ca(2+) from intracellular stores. 1,25(OH)(2)D(3) also affected enterocyte [Ca(2+)](i) in a biphasic manner: the rapid decrease, the recovery rate, and the mean final [Ca(2+)](i) were all affected dose-dependently, with EC(50) values of 8.3, 24.5 and 7.7nM respectively. The high EC(50) values for 1,25(OH)(2)D(3) compared with circulating concentrations of 1,25(OH)(2)D(3) (130pM) suggest that this effect is pharmacological, rather than of physiological relevance in enterocyte Ca(2+) homeostasis of the Atlantic cod. It is concluded that 24R,25(OH)(2)D(3) has a physiological role in decreasing intestinal Ca(2+) uptake via inactivation of L-type Ca(2+) channels, whereas the physiological role of 25(OH)D(3) is to increase enterocyte Ca(2+) transport via activation of Na(+)/Ca(2+) exchangers, concurrent with activation of L-type Ca(2+) channels.

    • The mesopelagic coronate medusa Periphylla periphylla has been very abundant (20-320 individuals m(-2)), with as many as 2.5 individuals m(-3), for more than a decade in certain Norwegian fjords. These abundances are two to three orders of magnitude higher than reported from open ocean environments. Comparisons of the size, density and behavior of this jellyfish in three fjords (Lurefjorden, Sognefjorden and Halsafjorden) support the hypothesis that retention of P. periphylla is related to basin topography, light attenuation and photosensitivity. Furthermore, we hypothesize that the abundances of P. periphylla in Lurefjorden and Halsafjorden, but not in Sognefjorden, have been stimulated by increased light absorbance of water masses formed in the North Sea.

    • Some western Norwegian fjords host extraordinarily abundant and persistent populations of the mesopelagic, coronate scyphomedusa, Periphylla periphylla. In these environments, from late autumn to spring, the medusae undertake regular diel vertical migrations into surface waters. From unique observations obtained with a remotely operated vehicle (ROV), including observations made without artificial light, we observed that 90% of the medusae swam with their tentacles in aboral position. Stomach content analyses of surface-collected specimens revealed that the medusae ate mainly calanoid copepods, but ostracods and large euphausiids were also prominent components of their diets. The clearance rate potential of P. periphylla, assessed from in situ observations and stomach contents, was comparable to that of similar-sized, epipelagic gelatinous species. Our findings suggest that P. periphylla behave as active predators in surface waters.
