    • The Sub-millimetre and Millimetre Radiometer (SMR) is the main instrument on the Swedish, Canadian, Finnish and French spacecraft Odin. It consists of a 1.1 metre diameter telescope with four tuneable heterodyne receivers covering the ranges 486-504 GHz and 541-581 GHz, and one fixed at 118.75 GHz together with backends that provide spectral resolution from 150 kHz to 1 MHz. This Letter describes the Odin radiometer, its operation and performance with the data processing and calibration described in Paper II.

  • Eemian interglacial reconstructed from a Greenland folded ice core

    • Efforts to extract a Greenland ice core with a complete record of the Eemian interglacial (130,000 to 115,000 years ago) have until now been unsuccessful. The response of the Greenland ice sheet to the warmer-than-present climate of the Eemian has thus remained unclear. Here we present the new North Greenland Eemian Ice Drilling ('NEEM') ice core and show only a modest ice-sheet response to the strong warming in the early Eemian. We reconstructed the Eemian record from folded ice using globally homogeneous parameters known from dated Greenland and Antarctic ice-core records. On the basis of water stable isotopes, NEEM surface temperatures after the onset of the Eemian (126,000 years ago) peaked at 8 +/- 4 degrees Celsius above the mean of the past millennium, followed by a gradual cooling that was probably driven by the decreasing summer insolation. Between 128,000 and 122,000 years ago, the thickness of the northwest Greenland ice sheet decreased by 400 +/- 250 metres, reaching surface elevations 122,000 years ago of 130 +/- 300 metres lower than the present. Extensive surface melt occurred at the NEEM site during the Eemian, a phenomenon witnessed when melt layers formed again at NEEM during the exceptional heat of July 2012. With additional warming, surface melt might become more common in the future.

  • Cardiometabolic effects of genetic upregulation of the interleukin 1 receptor antagonist: a Mendelian randomisation analysis

    • Background To investigate potential cardiovascular and other effects of long-term pharmacological interleukin 1 (IL-1) inhibition, we studied genetic variants that produce inhibition of IL-1, a master regulator of inflammation. Methods We created a genetic score combining the effects of alleles of two common variants (rs6743376 and rs1542176) that are located upstream of IL1RN, the gene encoding the IL-1 receptor antagonist (IL-1Ra; an endogenous inhibitor of both IL-1 alpha and IL-1 beta); both alleles increase soluble IL-1Ra protein concentration. We compared effects on inflammation biomarkers of this genetic score with those of anakinra, the recombinant form of IL-1Ra, which has previously been studied in randomised trials of rheumatoid arthritis and other inflammatory disorders. In primary analyses, we investigated the score in relation to rheumatoid arthritis and four cardiometabolic diseases (type 2 diabetes, coronary heart disease, ischaemic stroke, and abdominal aortic aneurysm; 453 411 total participants). In exploratory analyses, we studied the relation of the score to many disease traits and to 24 other disorders of proposed relevance to IL-1 signalling (746 171 total participants). Findings For each IL1RN minor allele inherited, serum concentrations of IL-1Ra increased by 0.22 SD (95% CI 0.18-0.25; 12.5%; p=9.3 x 10(-33)), concentrations of interleukin 6 decreased by 0.02 SD (-0.04 to -0.01; -1,7%; p=3.5 x 10(-3)), and concentrations of C-reactive protein decreased by 0.03 SD (-0.04 to -0.02; -3.4%; p=7.7 x 10(-14)). We noted the effects of the genetic score on these inflammation biomarkers to be directionally concordant with those of anakinra. The allele count of the genetic score had roughly log-linear, dose-dependent associations with both IL-1Ra concentration and risk of coronary heart disease. For people who carried four IL-1Ra-raising alleles, the odds ratio for coronary heart disease was 1.15 (1.08-1.22; p=1.8 x 10(-6)) compared with people who carried no IL-1Ra-raising alleles; the per-allele odds ratio for coronary heart disease was 1.03 (1.02-1.04; p=3.9 x 10(-10)). Perallele odds ratios were 0.97 (0.95-0.99; p=9.9 x 10(-4)) for rheumatoid arthritis, 0.99 (0.97-1.01; p=0.47) for type 2 diabetes, 1.00 (0.98-1.02; p=0.92) for ischaemic stroke, and 1.08 (1.04-1.12; p=1.8 x 10(-5)) for abdominal aortic aneurysm. In exploratory analyses, we observed per-allele increases in concentrations of proatherogenic lipids, including LDL-cholesterol, but no clear evidence of association for blood pressure, glycaemic traits, or any of the 24 other disorders studied. Modelling suggested that the observed increase in LDL-cholesterol could account for about a third of the association observed between the genetic score and increased coronary risk. Interpretation Human genetic data suggest that long-term dual IL-1 alpha/beta inhibition could increase cardiovascular risk and, conversely, reduce the risk of development of rheumatoid arthritis.   • A Genome-Wide Screen for Interactions Reveals a New Locus on 4p15 Modifying the Effect of Waist-to-Hip Ratio on Total Cholesterol

    • Recent genome-wide association (GWA) studies described 95 loci controlling serum lipid levels. These common variants explain similar to 25% of the heritability of the phenotypes. To date, no unbiased screen for gene-environment interactions for circulating lipids has been reported. We screened for variants that modify the relationship between known epidemiological risk factors and circulating lipid levels in a meta-analysis of genome-wide association (GWA) data from 18 population-based cohorts with European ancestry (maximum N = 32,225). We collected 8 further cohorts (N = 17,102) for replication, and rs6448771 on 4p15 demonstrated genome-wide significant interaction with waist-to-hip-ratio (WHR) on total cholesterol (TC) with a combined P-value of 4.79 x 10(-9). There were two potential candidate genes in the region, PCDH7 and CCKAR, with differential expression levels for rs6448771 genotypes in adipose tissue. The effect of WHR on TC was strongest for individuals carrying two copies of G allele, for whom a one standard deviation (sd) difference in WHR corresponds to 0.19 sd difference in TC concentration, while for A allele homozygous the difference was 0.12 sd. Our findings may open up possibilities for targeted intervention strategies for people characterized by specific genomic profiles. However, more refined measures of both body-fat distribution and metabolic measures are needed to understand how their joint dynamics are modified by the newly found locus.

    • In this work, the role of the electric conductivity on variations of the PD phenomenon in XLPE insulation under DC stress is investigated and demonstrated. To enable this investigation, a new experimental setup simulating the insulating layer of a cable with an embedded air void defect and subjected to a DC stress and temperature has been proposed. The setup consists of two flat specimens connected in series. One of them is heated while the other one, that contains the defect, is kept at ambient temperature. Consequently, the electric fields induced by the externally applied DC stress differ between the two specimens. In this way, a conductivity variation between two dielectric layers is obtained. To facilitate the usage of this method, a new simulation model related to the proposed experimental setup has also been developed. The model is an extension of the three-capacitor model, in which variable resistors are introduced. Both simulation and experimental results indicate that the introduced discrete thermal gradient results in higher conductivity values and enhanced PD activity.

    • The long-time series of Be-7 activity in surface air have been studied with the wavelet analysis technique in order to find coherence between Be-7 activity, theoretical production in the troposphere and climatic indices. The Be-7 activity were obtained from five different locations, Angra in the tropics in Brazil, Skane in mid-latitudes in Southern Sweden, Kiruna in Polar region in Northern Sweden, Loviisa in Southern Finland and Rovaniemi in polar region in Northern Finland. The Be-7 data from the Northern hemisphere sites where tested for coherence with theoretical production of the isotope in troposphere and with the North Atlantic Oscillation index. In the Southern hemisphere separate theoretical production was calculated in order to describe local production and Southern Annular Mode was used as the climatic index. Consistent and significant coherence were found with theoretical production at Skane, Kiruna and Loviisa at time-scales of four years or longer. At Angra and Rovaniemi sites, no coherence was detected between Be-7 theoretical tropospheric production and measured activity at ground level. The coherence between Be-7 data from Skane and Angra and climatic indices is insignificant while data from Northern and Eastern Scandinavia show clear coherence with climatic indices at time-scales of four years or longer. Additionally, significant coherence was found between the cosmic ray induced production and NAO at the time band of 8-12 years whereas the coherence between cosmic ray induced production and SAM was insignificant. This feature implies that the ground level Be-7 activity contain mixed information on both production and transport. This conclusion means that further evaluation through models which enable accurate realistic models that will be investigated in future studies.

  • Genetic continuity, isolation, and gene flow in Stone Age Central and Eastern Europe

    • The genomic landscape of Stone Age Europe was shaped by multiple migratory waves and population replacements, but different regions do not all show similar patterns. To refine our understanding of the population dynamics before and after the dawn of the Neolithic, we generated and analyzed genomic sequence data from human remains of 56 individuals from the Mesolithic, Neolithic, and Eneolithic across Central and Eastern Europe. We found that Mesolithic European populations formed a geographically widespread isolation-by-distance zone ranging from Central Europe to Siberia, which was already established 10,000 years ago. We found contrasting patterns of population continuity during the Neolithic transition: people around the lower Dnipro Valley region, Ukraine, showed continuity over 4000 years, from the Mesolithic to the end of the Neolithic, in contrast to almost all other parts of Europe where population turnover drove this cultural change, including vast areas of Central Europe and around the Danube River. Genome-wide sequencing of 56 ancient hunter-gatherer and early farmer individuals from Stone Age Central and Eastern Europe reveals striking population continuity in the east in contrast to central Europe that displays extensive admixture.

    • The occurrence of Partial Discharge (PD) inside a cavity subjected to HVDC voltage can be considered as one of the most likely sources of insulation ageing and failure. In practical applications, the recognition of PD under HVDC is more difficult than under AC stress because of the lower repetition rates and the impossibility to obtain a PD pattern synchronized with the waveform of the applied voltage. In this paper, the relationship between conduction phenomena in dielectrics and the occurrence of PD is investigated by means of a novel experimental setup. A couple of dielectric cells connected in series is subjected to various HVDC stress levels. The applied voltage is divided between the two cells by subjecting them to a temperature difference. The PD activity inside an air void defect existing within one of the two dielectric cell is measured by means of a PRY-CAM. This setup reproduces, to the extreme, the effect of the distribution of the electric field in an HVDC cable on PD activity when it is exposed to a thermal gradient across the insulation. In addition, an equivalent circuit model has been developed to evaluate the PD Inception Voltage (PDIV) across the cavity. The comparison between the experimental and model results demonstrates the capability of the macroscopic conduction model to reproduce the inception of PD phenomena in solid dielectrics under HVDC stress and temperature gradient.

    • The behavior of silicone gel under electrical stress plays a significant role in the reliability and durability of high voltage electronic power devices due to its widespread use for the insulation of IGBT modules and other components. The charges accumulation at the bubble boundaries leads to significant displacements due to the establishment of Coulombic forces and the high deformability of colloidal system. The main purpose of this work is to validate a numerical approach useful to investigate, for a given silicone gel, the non-linear relation between the applied HVAC stress and the electric field over an air bubble within the insulation bulk. The analysis has been carried out by means of numerical and experimental approach in order to compare the results of a case study and validate the adopted model.

    • With the increasing penetration of renewable and decentralized energy sources into the power grid, an extended use of DC voltages is expected on both distribution and transmission levels. Generation of DC voltages by means of voltage source converters is associated with a wide spectrum of harmonic distortions at converter terminals, both on the ac and on the dc sides. This can lead to partial discharges in power cables, which deteriorate insulation material thus weakening its performance and reducing cable life-time. In the previously published paper, the effect of harmonic distortion on appearance of partial discharges in cable insulation was evaluated. Here, the study related to the PD behavior has been made focusing on the influence of multilevel converter parameters such as the number of submodules and the switching frequency chosen to generate different voltage stresses. In addition, effects of using a function generator for producing desired waveform instead of the dSPACE board as in the previous work, are highlighted. In particular, it is shown implementation of the former made it possible utilizing the Pry-Cam synchronization and thus the realization of the PD pattern.

  • Permafrost and lakes control river isotope composition across a boreal Arctic transect in the Western Siberian lowlands

    • The Western Siberian Lowlands (WSL) store large quantities of organic carbon that will be exposed and mobilized by the thawing of permafrost. The fate of mobilized carbon, however, is not well understood, partly because of inadequate knowledge of hydrological controls in the region which has a vast low-relief surface area, extensive lake and wetland coverage and gradually increasing permafrost influence. We used stable water isotopes to improve our understanding of dominant landscape controls on the hydrology of the WSL. We sampled rivers along a 1700 km South-North transect from permafrost-free to continuous permafrost repeatedly over three years, and derived isotope proxies for catchment hydrological responsiveness and connectivity. We found correlations between the isotope proxies and catchment characteristics, suggesting that lakes and wetlands are intimately connected to rivers, and that permafrost increases the responsiveness of the catchment to rainfall and snowmelt events, reducing catchment mean transit times. Our work provides rare isotope-based field evidence that permafrost and lakes/wetlands influence hydrological pathways across a wide range of spatial scales (10-105 km2) and permafrost coverage (0%-70%). This has important implications, because both permafrost extent and lake/wetland coverage are affected by permafrost thaw in the changing climate. Changes in these hydrological landscape controls are likely to alter carbon export and emission via inland waters, which may be of global significance.

    • This paper describes the main results of the EU FP7 project TUMESA - MEMS tuneable metamaterials for smart wireless applications. In this project, we studied several reconfigurable antenna approaches that combine the new technology of MEMS with the new concept of artificial electromagnetic materials and surfaces (metamaterials and metasurfaces) for realisation of millimetre wave phase shifters and beam-steering devices. MEMS technology allows to miniaturise electronic components, reduce their cost in batch production, and effectively compete with semiconductor and ferroelectric based technologies in terms of losses at millimetre wavelengths. Novel tuneable materials and components proposed in this project perform as smart beam steering devices. Fabricated with MEMS technology in batch and on a single chip, proposed tuneable devices allow substituting of larger and more complex sub-system of, e.g., a radar sensor. This substitution provides a dramatic cost reduction on a system level.

  • Immune Microenvironment in Sporadic Early-Onset versus Average-Onset Colorectal Cancer

    • Simple Summary The incidence of non-hereditary cancer in the left colon and rectum is increasing in young patients worldwide for unknown reasons. To understand this phenomenon, the biology of early-onset colorectal cancer needs to be established. Here, we investigated the immune response to tumors by selecting a highly representative group of patients younger than 45 years matched to those aged 70-75 years, excluding hereditary cases. Both T-cell distribution in tumors and expression of 770 immune-related genes were overall similar between the groups. The findings suggest that the immune response in cancer of the left colon and rectum is not dependent on age and that early-onset colorectal cancer is likely not related to immune response deficiencies. The incidence of left-sided colon and rectal cancer in young people are increasing worldwide, but its causes are poorly understood. It is not clear if the tumor microenvironment is dependent on age of onset, and little is known about the composition of tumor-infiltrating T cells in early-onset colorectal cancer (EOCRC). To address this, we investigated T-cell subsets and performed gene expression immune profiling in sporadic EOCRC tumors and matched average-onset colorectal cancer (AOCRC) tumors. Left-sided colon and rectal tumors from 40 cases were analyzed; 20 EOCRC (<45 years) patients were matched 1:1 to AOCRC (70-75 years) patients by gender, tumor location, and stage. Cases with germline pathogenic variants, inflammatory bowel disease or neoadjuvant-treated tumors were excluded. For T cells in tumors and stroma, a multiplex immunofluorescence assay combined with digital image analysis and machine learning algorithms was used. Immunological mediators in the tumor microenvironment were assessed by NanoString gene expression profiling of mRNA. Immunofluorescence revealed no significant difference between EOCRC and AOCRC with regard to infiltration of total T cells, conventional CD4(+) and CD8(+) T cells, regulatory T cells, or gamma delta T cells. Most T cells were located in the stroma in both EOCRC and AOCRC. Immune profiling by gene expression revealed higher expression in AOCRC of the immunoregulatory cytokine IL-10, the inhibitory NK cell receptors KIR3DL3 and KLRB1 (CD161), and IFN-a7 (IFNA7). In contrast, the interferon-induced gene IFIT2 was more highly expressed in EOCRC. However, in a global analysis of 770 tumor immunity genes, no significant differences could be detected. T-cell infiltration and expression of inflammatory mediators are similar in EOCRC and AOCRC. This may indicate that the immune response to cancer in left colon and rectum is not related to age of onset and that EOCRC is likely not driven by immune response deficiency.

  • DNA methylation changes measured in pre-diagnostic peripheral blood samples are associated with smoking and lung cancer risk

    • DNA methylation changes are associated with cigarette smoking. We used the Illumina Infinium HumanMethylation450 array to determine whether methylation in DNA from pre-diagnostic, peripheral blood samples is associated with lung cancer risk. We used a case-control study nested within the EPIC-Italy cohort and a study within the MCCS cohort as discovery sets (a total of 552 case-control pairs). We validated the top signals in 429 case-control pairs from another 3 studies. We identified six CpGs for which hypomethylation was associated with lung cancer risk: cg05575921 in the AHRR gene (p-valuepooled  = 4 × 10(-17) ), cg03636183 in the F2RL3 gene (p-valuepooled  = 2 × 10 (- 13) ), cg21566642 and cg05951221 in 2q37.1 (p-valuepooled  = 7 × 10(-16) and 1 × 10(-11) respectively), cg06126421 in 6p21.33 (p-valuepooled  = 2 × 10(-15) ) and cg23387569 in 12q14.1 (p-valuepooled  = 5 × 10(-7) ). For cg05951221 and cg23387569 the strength of association was virtually identical in never and current smokers. For all these CpGs except for cg23387569, the methylation levels were different across smoking categories in controls (p-valuesheterogeneity  ≤ 1.8 x10 (- 7) ), were lowest for current smokers and increased with time since quitting for former smokers. We observed a gain in discrimination between cases and controls measured by the area under the ROC curve of at least 8% (p-values ≥ 0.003) in former smokers by adding methylation at the 6 CpGs into risk prediction models including smoking status and number of pack-years. Our findings provide convincing evidence that smoking and possibly other factors lead to DNA methylation changes measurable in peripheral blood that may improve prediction of lung cancer risk.

    • To examine temporal variability of 10Be in glacial ice, we sampled snow to a depth of 160 cm at the NEEM (North Greenland Eemian Ice Drilling) drilling site in Greenland. The samples span three years between the summers of 2006 and 2009. At the same time, spatial variability of 10Be in glacial ice was explored through collection of the upper ∼5 cm of surface snow in Antarctica during part of the Swedish-Japanese traverse from Svea to Syowa station during the austral summer in 2007-2008. The results of the Greenlandic 10Be snow suggested variable concentrations that apparently do not clearly reflect the seasonal change as indicated by the δ18O data. The 10Be concentration variability most likely reflects also effects of aerosol loading and deposition pathways, possibly in combination with post-depositional processes. The Antarctic traverse data expose a negative correlation between 10Be and δ18O, while there are weaker but still significant correlations to altitude and distance to the coast (approximated by the distance to the 70th latitude). These relationships indicate that geographical factors, mainly the proximity to the coast, may strongly affect 10Be concentrations in snow in Queen Maud Land, Antarctica.

    • Groundwater-surface water (GW-SW) interactions cover a broad range of hydrogeological and biological processes and are controlled by natural and anthropogenic factors at various spatio-temporal scales, from watershed to hyporheic/hypolentic zone. Understanding these processes is vital in the protection of groundwater-dependent ecosystems (GDEs) increasingly required in water resources legislation across the world. The use of environmental tracers and indicators that are relevant simultaneously for groundwater, surface water and biocenoses-biotope interactions constitutes a powerful tool to succeed in the management task. However, tracer type must be chosen according to the scale of interest and tracer use thus requires a good conceptual understanding of the processes to be evaluated. This paper reviews various GW-SW interaction processes and their drivers and, based on available knowledge, systemises application of conservative tracers and semi-conservative and reactive environmental indicators at different spatial scales. Biocenoses-biotopes relationships are viewed as a possible transition tool between scales. Relation between principal application of the environmental tracers and indicators, examples and guidelines are further proposed for examining GW-SW interactions from a hydrogeological and biological point of view by demonstrating the usability of the tracers/indicators and providing recommendations for the scientific community and decision makers.

  • Hypomethylation of smoking-related genes is associated with future lung cancer in four prospective cohorts

    • DNA hypomethylation in certain genes is associated with tobacco exposure but it is unknown whether these methylation changes translate into increased lung cancer risk. In an epigenome-wide study of DNA from pre-diagnostic blood samples from 132 case–control pairs in the NOWAC cohort, we observe that the most significant associations with lung cancer risk are for cg05575921 in AHRR (OR for 1 s.d.=0.37, 95% CI: 0.31–0.54, P-value=3.3 × 10−11) and cg03636183 in F2RL3 (OR for 1 s.d.=0.40, 95% CI: 0.31–0.56, P-value=3.9 × 10−10), previously shown to be strongly hypomethylated in smokers. These associations remain significant after adjustment for smoking and are confirmed in additional 664 case–control pairs tightly matched for smoking from the MCCS, NSHDS and EPIC HD cohorts. The replication and mediation analyses suggest that residual confounding is unlikely to explain the observed associations and that hypomethylation of these CpG sites may mediate the effect of tobacco on lung cancer risk.

  • Quantifying excess deaths related to heatwaves under climate change scenarios : A multicountry time series modelling study

    • BACKGROUND: Heatwaves are a critical public health problem. There will be an increase in the frequency and severity of heatwaves under changing climate. However, evidence about the impacts of climate change on heatwave-related mortality at a global scale is limited.METHODS AND FINDINGS: We collected historical daily time series of mean temperature and mortality for all causes or nonexternal causes, in periods ranging from January 1, 1984, to December 31, 2015, in 412 communities within 20 countries/regions. We estimated heatwave-mortality associations through a two-stage time series design. Current and future daily mean temperature series were projected under four scenarios of greenhouse gas emissions from 1971-2099, with five general circulation models. We projected excess mortality in relation to heatwaves in the future under each scenario of greenhouse gas emissions, with two assumptions for adaptation (no adaptation and hypothetical adaptation) and three scenarios of population change (high variant, median variant, and low variant). Results show that, if there is no adaptation, heatwave-related excess mortality is expected to increase the most in tropical and subtropical countries/regions (close to the equator), while European countries and the United States will have smaller percent increases in heatwave-related excess mortality. The higher the population variant and the greenhouse gas emissions, the higher the increase of heatwave-related excess mortality in the future. The changes in 2031-2080 compared with 1971-2020 range from approximately 2,000% in Colombia to 150% in Moldova under the highest emission scenario and high-variant population scenario, without any adaptation. If we considered hypothetical adaptation to future climate, under high-variant population scenario and all scenarios of greenhouse gas emissions, the heatwave-related excess mortality is expected to still increase across all the countries/regions except Moldova and Japan. However, the increase would be much smaller than the no adaptation scenario. The simple assumptions with respect to adaptation as follows: no adaptation and hypothetical adaptation results in some uncertainties of projections.CONCLUSIONS: This study provides a comprehensive characterisation of future heatwave-related excess mortality across various regions and under alternative scenarios of greenhouse gas emissions, different assumptions of adaptation, and different scenarios of population change. The projections can help decision makers in planning adaptation and mitigation strategies for climate change.

    • An increase in the global health burden of temperature was projected for 459 locations in 28 countries worldwide under four representative concentration pathway scenarios until 2099. We determined that the amount of temperature increase for each 100 ppm increase in global CO2 concentrations is nearly constant, regardless of climate scenarios. The overall average temperature increase during 2010-2099 is largest in Canada (1.16 °C/100 ppm) and Finland (1.14 °C/100 ppm), while it is smallest in Ireland (0.62 °C/100 ppm) and Argentina (0.63 °C/100 ppm). In addition, for each 1 °C temperature increase, the amount of excess mortality is increased largely in tropical countries such as Vietnam (10.34%p/°C) and the Philippines (8.18%p/°C), while it is decreased in Ireland (-0.92%p/°C) and Australia (-0.32%p/°C). To understand the regional variability in temperature increase and mortality, we performed a regression-based modeling. We observed that the projected temperature increase is highly correlated with daily temperature range at the location and vulnerability to temperature increase is affected by health expenditure, and proportions of obese and elderly population.

    • Objectives: To determine the population distribution of cardiovascular risk in eight low- and middle-income countries and compare the cost of drug treatment based on cardiovascular risk (cardiovascular risk thresholds >= 30%/>= 40%) with single risk factor cutoff levels. Study Design and Setting: Using World Health Organization (WHO) and the International Society of Hypertension risk prediction charts, cardiovascular risk was categorized in a cross-sectional study of 8,625 randomly selected people aged 40-80 years (mean age, 54.6 years) from defined geographic regions of Nigeria, Iran, China, Pakistan, Georgia, Nepal, Cuba, and Sri Lanka. Cost estimates for drug therapy were calculated for three countries. Results: A large fraction (90.0-98.9%) of the study population has a 10-year cardiovascular risk <20%. Only 0.2-4.8% are in the high-risk categories (>= 30%). Adopting a total risk approach and WHO guidelines recommendations would restrict unnecessary drug treatment and reduce the drug costs significantly. Conclusion: Adopting a total cardiovascular risk approach instead of a single risk factor approach reduces health care expenditure by reducing drug costs. Therefore, limited resources can be more efficiently used to target high-risk people who will benefit the most. This strategy needs to be complemented with population-wide measures to shift the cardiovascular risk distribution of the whole population. (C) 2011 Elsevier Inc. All rights reserved.

    • (1) To evaluate dental arch relationships, with the Great Ormond Street, London and Oslo (GOSLON) Yardstick, of participants with Unilateral cleft lip and palate (UCLP) and treated with 1-stage palatal closure with 3 different surgical protocols (2) to compare the mean GOSLON ratings with other CLP centers. Design: Retrospective study of medical charts and dental models. Setting: Karolinska University Hospital, Stockholm, Sweden. Participants: Eighty-seven patients with UCLP operated with 1-stage palatal repair. Thirty-five were operated with Veau-Wardill-Kilner (VWK) technique 1975 to 1986, 31 with minimal incision technique (MIT) from 1987 to 1997, and 21 according to MIT with muscle reconstruction (MITmr) 1998 to 2004. Interventions: Dental casts at ages 5 (n = 87), 7 to 8 (n = 27), 10 (n = 81), 16 (n = 61), and 19 (n = 35) years were rated by 10 assessors with the GOSLON Yardstick. Information of other interventions was retrieved from patients’ charts. Main outcome measures: Mean GOSLON ratings. Results: A total of 82% of the participants were rated as having excellent to satisfactory outcome. Weighted κ statistics for the 10 assessors was good for inter-rater agreement and good/very good for intra-rater agreement. Conclusions: The mean GOSLON score in the Stockholm overall material at age 10 was 2.67. The VWK technique resulted in a greater need of orthognathic surgery than the MIT ( P < .01). The MITmr did not produce better dental arch relationships than MIT at age 5 ( P < .05). The best dental arch relationships were found in the MIT group at 10 years, mean 2.58, which is not significantly different from other centers with excellent outcome except Gothenburg and Vienna.

    • BackgroundChronic obstructive pulmonary disease (COPD) is a common lung disorder characterized by persistent and progressive airflow limitation as well as systemic changes. Metabolic changes in blood may help detect COPD in an earlier stage and predict prognosis.MethodsWe conducted a comprehensive study of circulating metabolites, measured by proton Nuclear Magnetic Resonance Spectroscopy, in relation with COPD and lung function. The discovery sample consisted of 5557 individuals from two large population-based studies in the Netherlands, the Rotterdam Study and the Erasmus Rucphen Family study. Significant findings were replicated in 12,205 individuals from the Lifelines-DEEP study, FINRISK and the Prospective Investigation of the Vasculature in Uppsala Seniors (PIVUS) studies. For replicated metabolites further investigation of causality was performed, utilizing genetics in the Mendelian randomization approach.ResultsThere were 602 cases of COPD and 4955 controls used in the discovery meta-analysis. Our logistic regression results showed that higher levels of plasma Glycoprotein acetyls (GlycA) are significantly associated with COPD (OR = 1.16, P = 5.6 × 10− 4 in the discovery and OR = 1.30, P = 1.8 × 10− 6 in the replication sample). A bi-directional two-sample Mendelian randomization analysis suggested that circulating blood GlycA is not causally related to COPD, but that COPD causally increases GlycA levels. Using the prospective data of the same sample of Rotterdam Study in Cox-regression, we show that the circulating GlycA level is a predictive biomarker of COPD incidence (HR = 1.99, 95%CI 1.52–2.60, comparing those in the highest and lowest quartile of GlycA) but is not significantly associated with mortality in COPD patients (HR = 1.07, 95%CI 0.94–1.20).ConclusionsOur study shows that circulating blood GlycA is a biomarker of early COPD pathology.

    • The fate of the vast stocks of organic carbon stored in permafrost of the Western Siberian Lowland, the world's largest peat-land, is uncertain. Specifically, the amount of greenhouse gas emissions from rivers in the region is unknown. Here we present estimates of annual CO2 emissions from 58 rivers across all permafrost zones of the Western Siberian Lowland, between 56 and 67 degrees N. We find that emissions peak at the permafrost boundary, and decrease where permafrost is more prevalent and in colder climatic conditions. River CO2 emissions were high, and on average two times greater than downstream carbon export. We suggest that high emissions and emission/export ratios are a result of warm temperatures and the long transit times of river water. We show that rivers in the Western Siberian Lowland play an important role in the carbon cycle by degassing terrestrial carbon before its transport to the Arctic Ocean, and suggest that changes in both temperature and precipitation are important for understanding and predicting high-latitude river CO2 emissions in a changing climate.

    • Variations of the cosmogenic radionuclide Be-7 in the global atmosphere are driven by cooperation of processes of its production, air transports, and removal. We use a combination of the Goddard Institute for Space Studies ModelE and the OuluCRAC: Be-7 production model to simulate the variations in the Be-7 concentration in the atmosphere for   the period from 1 January to 28 February 2005. This period features significant synoptic variability at multiple monitoring stations around the globe and spans an extreme solar energetic particle (SEP) event that occurred on 20 January. Using nudging from observed horizontal winds, the model correctly reproduces the overall level of the measured   Be-7 concentration near ground and a great deal of the synoptic variability at timescales of 4 days and longer. This verifies the combined model of production and transport of the Be-7 radionuclide in   the atmosphere. The impact of an extreme SEP event of January 2005 is   seen dramatically in polar stratospheric Be-7 concentration but is small near the surface (about 2%) and indistinguishable given the amount of intrinsic variability and the uncertainties of the surface observations.

    • Approximately 1.5 billion people worldwide are overweight or affected by obesity, and are at risk of developing type (2) diabetes, cardiovascular disease and related metabolic and inflammatory disturbances(1,2). Although the mechanisms linking adiposity to associated clinical conditions are poorly understood, recent studies suggest that adiposity may influence DNA methylation(3-6), a key regulator of gene expression and molecular phenotype(7). Here we use epigenome-wide association to show that body mass index (BMI; a key measure of adiposity) is associated with widespread changes in DNA methylation (187 genetic loci with P < 1 x 10(-7), range P = 9.2 x 10(-8) to 6.0 x 10(-46); n = 10,261 samples). Genetic association analyses demonstrate that the alterations in DNA methylation are predominantly the consequence of adiposity, rather than the cause. We find that methylation loci are enriched for functional genomic features in multiple tissues (P < 0.05), and show that sentinel methylation markers identify gene expression signatures at 38 loci (P < 9.0 x 10(-6), range P = 5.5 x 10(-6) to 6.1 x 10(-35), n = 1,785 samples). The methylation loci identify genes involved in lipid and lipoprotein metabolism, substrate transport and inflammatory pathways. Finally, we show that the disturbances in DNA methylation predict future development of type 2 diabetes (relative risk per 1 standard deviation increase in methylation risk score: 2.3 (2.07-2.56); P = 1.1 x 10(-54)). Our results provide new insights into the biologic pathways influenced by adiposity, and may enable development of new strategies for prediction and prevention of type 2 diabetes and other adverse clinical consequences of obesity.

    • Tracer technology has been used to understand water circulation in marine systems where the tracer dose is commonly injected into the marine waters through controlled experiments, accidental releases or waste discharges. Anthropogenic discharges of I-129 have been used to trace water circulation in the Arctic and North Atlantic Ocean. Here, I-129, together with I-127, is utilized as a tracer of water pathways and circulation in the Baltic Sea through collection of seawater depth profiles. The results indicate the presence of I-129 signatures which are distinct for each water mass and provide evidence for: (1) inflow water masses through the Drogden Sill that may reach as far as the SW of the Arkona Sea, (2) a portion of North Atlantic water in the bottom of Arkona basin, (3) cyclonic upwelling which breaks through the halocline in a pattern similar to the Baltic haline conveyor belt and (4) more influx of fresher water from the Gulf of Finland and Bothnian Sea in August relative to April. These findings provide advances in labeling and understanding water pathways in the Baltic Sea.

    • The semi-enclosed Baltic Sea represents a vital economic and recreational resource for more than 90 million people inhabiting its coasts. Extensive contamination of this sea by a variety of anthropogenic pollutants has raised the concern of the people in the region. Quantifying seawater inflow is crucial for estimating potential environmental risks as well as to find the best remedial strategy. We present here a model to estimate water inflow from the North Sea to the Baltic Sea by utilizing 1291 as a tracer. The results predicted inflow range of 230-450 km(3)/y with best fit value around 330 km(3)/y from the North Sea to the Baltic Sea during 1980-1999. Despite limited time series data on I-129, the model presented here demonstrates a new management tool for the Baltic Sea to calculate inflow water compared to conventional methods (such as salinity, temperature and hydrographic models).
