    • Reduced lung function is associated with cardiovascular mortality, but the relationships with atherosclerosis are unclear. The population-based Swedish CArdioPulmonary BioImage study measured lung function, emphysema, coronary CT angiography, coronary calcium, carotid plaques and ankle-brachial index in 29,593 men and women aged 50–64 years. The results were confirmed using 2-sample Mendelian randomization. Lower lung function and emphysema were associated with more atherosclerosis, but these relationships were attenuated after adjustment for cardiovascular risk factors. Lung function was not associated with coronary atherosclerosis in 14,524 never-smokers. No potentially causal effect of lung function on atherosclerosis, or vice versa, was found in the 2-sample Mendelian randomization analysis. Here we show that reduced lung function and atherosclerosis are correlated in the population, but probably not causally related. Assessing lung function in addition to conventional cardiovascular risk factors to gauge risk of subclinical atherosclerosis is probably not meaningful, but low lung function found by chance should alert for atherosclerosis.

    • Detta kapitel undersöker Hanne Bramness barnlyriska bok Vintersong (2014) utifrån ett främst anknytningsteoretiskt perspektiv. Frågor som diskuteras är poesins relation till det lilla barnets utveckling, såväl kognitivt, känslomässigt och socialt. Genom att de språkliga drag som vi kallar poetiska är bland de första som vi utvecklar som barn är de knutna till kroppsminnen av våra allra tidigaste känslor och relationer. Detta gör att poesin även senare i livet kan påminna oss om dessa minnen, och därmed också om de anknytningsmönster som formades där och då. Detta kommer särskilt tydligt till ytan i Vintersong, dels på grund av att relationer mellan diktjagets då och nu lyfts fram, dels genom att dikterna så tydligt använder sig av språkets klang- och rytmmässiga drag.

    • Bengt Emil Johnson was one of the most prominent poets of the short-lived yet highly vivid Swedish concretist movement of the early 1960’s. As for most poets involved in this movement, his carreer soon turned to other manners of writing, in his case more successfully than for many of his fellow concretists. During the second half of the 1960’s he was involved in creating the interartial genre of text-sound-composition, and ever since then he has been an internationally acclaimed composer of electro-acoustic music. Since the 1970’s he has also been recognised as an important innovator in the field of nature-poetry, displaying an attitude as keen and attentive towards the abundance of language as it is towards that of nature. Through this sensitivity he has been able to examine and unfold intriguing correspondences between language and nature. Apart from being a poet during the first half of the 1960’s, Johnson was also a writer of manifestos and essays concerning aesthetic matters. He quite frequently appeared in the press as well as in several of the important Swedish periodicals of the time. Quite a few of the aesthetical texts he wrote during this period were concerned with the artistic potential that was to be found in new technology, especially sound-technology. Particularly interesting are a number of texts which he wrote in collaboration with Lars-Gunnar Bodin. Johnson and Bodin were not, however, the only Swedish artists writing manifestos and essays from these perspectives during those years. Someone else who did was Öyvind Fahlström, the patriarch of the Swedish concretist movement. In my paper I trace a rhetoric in a number of aesthetic essays and manifestos by all these writers published between the years 1954-1965, a rhetoric which in several ways has its aim fixed on connecting technological and scientific breakthrough to artistic breakthrough. In this way I try to point out the fact that a number of the ideals behind the radical avant-garde writing strategies used by Johnson and his compatriots during his concretist years had a technological origin. I also go on to show to what extent these strategies linger in Johnson’s later writing, what part they play, and in what ways they appear. The examples I use from his later writing consist of three, among themselves quite different, poems from his 1989 poetry collection Bland orrar och kor (Amongst Black Grouse and Cows). I aim to show that Johnson, in these poems, still uses techniques appropriated from, and inspired by, mid-century sound-technological landmarks such as the contact-microphone, multiple layer recording, cut-up- and pitching techniques. And all of these technologies and techniques were discussed in the aesthetic writings of Johnson, Bodin, and Fahlström, respectively, during the 1950’s and 1960’s. And all of them were mentioned as holding great artistic potential.

    • När en ung Fredrik Böök 1912, i uppsatsen ”Det romantiska. Försök till en psykologisk definition”, definierar ”det romantiska” hävdar han att det består i ”betonandet af en företeelses för det diskursiva tänkandet oåtkomliga sammanhang”. Att detta uttalande är något av en anakronism blir tydligt i de olika specificeringar av denna formel som han anför; det som beskrivs är snarare vad Bööks tid lärt av det romantiska, vad som föranlett det moderna genombrottet. Trots detta är det relevant att stanna upp vid hans beskrivning av det romantiska. Böök är nämligen inte den enda som beskrivit en romantisk linje på detta vis. Det finns en mytbildning kring romantiken och det romantiska som i högsta grad är levande än idag, och påverkar och präglar både vetenskap och litteratur. I en sådan mytbildning blir det som Böök benämner ”det romantiska” i det närmaste att betrakta som en beskrivning av ”det estetiska”, eller någon form av litterär essens. I detta paper utreder jag hur myten om ”det romantiska” tar sig uttryck i Bengt Emil Johnsons diktning. Denna myt som genom hela Johnsons extremt heterogena författarskap innebär en central tematik – vad åstadkommer den i hans texter? ”Det romantiska”, som jag använder det och som Böök beskriver det, talar starkt emot tydliga avgränsningar och all form av definition och överskådlighet: I samma mån som vi önska full klarhet och öfverskådlighet, en fast och handterlig form, en afslutad och rationell helhet, tränga vi undan de tillströmmande associationerna. Hvarje definition är en afgränsning. I samma mån som vi däremot tillfredsställa behofvet att följa föreställningsassociationerna i deras oändliga progress och regress, i samma mån föras vi in i ett tillstånd som är romantiskt (Böök). Av denna anledning är det särskilt intressant att notera hur olika typer av gränser – såväl i motivtecknandet, som vad gäller genremässiga, mediala och semiotiska aspekter – problematiseras och suddas ut i Johnsons dikter – särskilt som detta utgör en viktig del av deras särart. Tematiken smittar av sig på de olika diskurser som är aktiva i Johnsons poesi, vilket i sig innebär att gränserna mellan dessa diskurser luckras upp och blir problematiska.

    • This article discusses how picturebooks for children can make use of poetry in order to empower child readers to bridge the asymmetric aetonormativity between child and adult inherent in the picturebook format. The article launches the concept of the poetic zone, springing from both epistemologically based poetry theory and norm critical theory. Two picturebooks are used as examples: Gro Dahle’s and Svein Nyhus’s Snill [What a Girl!], from 2002, and Lina Ekdahl and Emma Hanquist’s Vi smular vi härmar en gök [We Crumble We Imitate a Cuckoo], from 2016. Analyses of the poetic qualities of the two books show how they both pinpoint, and performatively empower, the child reader’s perspective. This takes place in the relationships between child and adult within the books’ plots, as well as in relation to the picturebook format. The poetic qualities that contribute in this sense concern both imagery, and rhythmical as well as auditory aspects of language.

    • This article treats poetry books of two prominent Scandinavian poets from the new millennium from a gender-political perspective. Ursula Andkjær Olsen's debut book "Lulus sange og taler" (2000) and Ida Börjel's debut "Sond" (2004) are studied from a theoretical perspective based on Jean-Jacques Lecercle's ideas, in particular his concept of ”the remainder of language”. The articel shows how both poets take as their starting point a sort of aporetic stance on speech, by clearly showing a desire to speak, while simultaneously being painfully aware of the impossibility of doing just that, within the presently gendered discourse of speech. By employing Lecercle's theories, the article reveals how the choice of this aporetic standpoint, in itself, does enable elements of (in this case - gendered) language which have been shut out, to stand out, and become visible. Additionally, within this process the ideology of the discourse becomes obvious.

    • This paper is an attempt to take into joint account Jacques Rancière’s take on “politics” as a heterogeneous force, and Gilles Deleuze’s and Felix Guattari’s concept of “becoming”, when approaching contemporary avant-garde poetry. It could be argued that such an operation runs the risk of being self-confirming, or circular, due to the fact that the ideas of an ideal heterogeneity that permeate both these theoretical concepts also does play a vital part in the political avant-garde of the day. This risk might also be why such ventures are so rare. However, that particular line of argument could of course be reversed, just as well. If contemporary avant-garde, on the one hand, and critical theory, on the other, share certain basic assumptions, why should the relation between the two not be examined more closely? In this paper I will examine how Swedish contemporary poet Lars-Mikael Raattamaa, in his book Mallamerik, mallammer, malameri, mallame, amerik, mallameka, merrika…, attempts a move away from conventional poetic form. This attempt is in many ways analogical to how “the political” according to Rancière moves away from “polis”, and how the “becoming” (and related concepts in the writings of Deleuze/Guattari, like “nomadic”, “de-territorialisation”, “flesh”, “the molecular”, “the rhizom”) moves away from form and stability. This particular Raattamaa-book employs a poetics which could be described in terms of “maximalism”. More specifically, a modus of the DJ is used in order to create a kind of heterogeneous flow. Within this flow, certain conventional literary cue points, such as punctuation, syntax, author, are questioned, at least symbolically. Through text-strategies that could be labelled “maximalist”, or “political”, or “becoming”, Raattamaa performs a severe critique of the supposedly stable hierarchies of conventional poetic language, i.e. the “polis” (Rancière), or the “body” (Deleuze/Guattari).

    • The paper studies the political implications of linguistic and literary conventions, through theorists like Jean-Jacques Lecercle, Jacques Rancière, and Marjorie Perloff. My touchstone will be the poetry of Swedish contemporary poet Ida Börjel. What is focused in Börjel’s writing is her tendencies to use “found objects”, and to question the representational function of language, as its primary function. These tendencies correspond well to e.g. Rancière’s thoughts on literature and its relation to life, as well as to Lecercle’s discussions on the barriers between what is inside language’s conventions, and what is left outside (“the remainder”). The paper dwells on certain political dimensions of theories like theirs, from a perspective which might, at least to a certain extent, be described as culture-semiotic: What ideologies come with a certain semiotic understanding, with certain linguistic conventions? And what ideologies surface when these conventions, this understanding, is questioned in one way or the other? Concepts like parataxis and language materialism will be crucial to my discussion, since these concepts (or at least certain facets of them) stress functions of language and literature which are not representational. Lecercle’s theories on “the remainder” of language prove very effective when applied not specifically as linguistic tools primarily, but as tools for literary analysis. Especially when one approaches a literature which tries to scrutinize, and even transgress, its own boundaries and conventions – as does Börjel’s. This concept also proves a useful complement in relation to Rancière’s more qualitative concept of the “political”, and its relationship to literature specifically. Rancière speaks mainly of the novel, and Lecercle mainly of language (in more general terms). Within the habitat of Marjorie Perloff’s theories of experimental poetry, these two theoretical perspectives can find an environment where literary form and politics have a lot to say to each other. And the outcome of such a discussion would have relevance when it comes to the relationship between such politically radiant pairs as nature and culture, or ecology and economy.

    • I detta paper utgår jag ifrån vilka högskolepedagogiska överväganden som går att göra utifrån det faktum att varje lärarstudent är student i lärsituationen under utbildningen, och pedagog i lärsituationen efter utbildningen, dit lärarutbildningen syftar. Jag talar om denna grundläggande förutsättning som en kluvenhet mellan de två delarna ”lärar-” och ”-student” i ordet ”lärarstudent”, en kluvenhet som många lärarstudenter har, och bör ha. I min egen undervisningspraktik från olika lärarutbildningar upplever jag att denna kluvenhet alltid är en betydelsefull högskolepedagogisk faktor i en förhållandevis svår balansakt mellan de två lärsituationerna. Fel använt tenderar det momentum som kluvenheten därmed innebär att fort ett problem i undervisningen, med resultatet att studenterna inte känner sig bekväma med vare sig den litteratur eller den undervisning som används på deras lärarutbildningskurser. I mitt paper utreder jag några metoder för att genom att verbalisera detta momentum göra det till ett stöd i lärandet, något som kan hjälpa studenterna att se sin egen lärarprofessionalitet, sina styrkor och svagheter, och att i första hand vara en närvarande auktoritet (i ordets bästa bemärkelse) och låta relationen och kommunikationen mellan elever och lärare vara i fokus.

    • Over the last decade, the Swedish school system has been discussing the intersection between aesthetics and learning with much enthusiasm. The aim of this article is to show how this discussion can be invigorated by contemporary poetic practice, and how focus thus can be shifted from what Jan Thavenius has labelled »market aesthetics« and »modest aesthetics«, to what he labels »radical aesthetics«, i.e. an overriding aesthetics which is critical by nature, and therefore able to permeate the school system. Much contemporary poetry looks upon itself as a critical art form, at least as much as a creative one. The example used in the article – a controversial radio program, and poetic performance from 2014, by Athena Farrokhzad – uses poetry to highlight the ideological and religious discourses of con- temporary – »mediatized« – society. Appropriating the poetic methods she uses, and employing them meta-critically within the classroom, could bring the school system closer to Thavenius’s ideal »radical aesthetics«.

    • Detta kapitel närmar sig frågan om författar- och diktarpositionen genom att problematisera kroppens betydelse. Mad hjälp av analyser av böcker skrivna av Maja Lee Langvad och Athena Farrokhzad synliggörs hur diskussionen om det lyriska subjektet flyttats över i ett politiskt register, genom att kroppen görs till brännpunkt, exempelvis i en pronominal konflikt mellan ett "du" och ett "vi". Hos de två poeterna är skrivandets praxis framhävd, konkretiserad och tydliggjord - hos Lee Langvad genom en rad "objets trouvés", ofta hämtade från juridiska dokument, hos Farrokhzad genom bland annat genom att bokens text är tryckt i vitt på svarta band. På ett liknande vis synliggörs också "det lyriska subjektet" som ett fenomen som skriver sig fram i, och skriver över, andras text.

    • Tove Jansson’s picturebook Hur gick det sen? Boken om Mymlan, Mumintrollet och lilla My (The Book about Moomin, Mymble and Little My) from 1952 is a book which makes use of its material aspects in many regards, employing the interactivity of the picturebook format to its maximum. Given the fact that it is written in verse, and is considered a classic by all means, it is surprising to find that the function that poetry plays within the book has been scarcely explored in prior research. By examining how the format of this particular picturebook guides the manner in which the child reader experiences it, and by studying the function of poetry within that framework, the article shows that poetry potentially plays a vital role in how child readers make sense of it. In particular, the role of poetry manifests itself as an opportunity for child readers to – at their own pace, and in their own manner – deal with the delicate balance between the attachmentseeking struggles of the anxious character of Moomin, on the one hand, and the exploratory individuation of the adventurous character of Little My, on the other. Through these examinations, the article demonstrates how the use of poetry can facilitate an emancipatory opportunity for child readers within picturebooks – and how this potential is ever-present within the format and the reading situation of picturebooks for children in general.

    • This is an article on how Scandinavian poetry has, over the last couple of decades, dealt with its inherent associations with autonomy, exclusion and marginalization. Due to the rise of new media, and the effects of what the social sciences call the "mediatization" of society, contemporary poetry has in large chosen to walk away from its Romantic-Modernist tradition of exclusion and marginalization (and the ideological implications of that), without compromising on autonomy. One of the ways in which this has been accomplished is by the use of what the article labels "impure large forms", inspired by the impure ideals of e.g. 1960s Scandinavian concrete poetry and the 1970-1980s North American L=A=N=G=U=A=G=E-movement.     • The article shows how the digital experience of the relationship between application and code manifests itself in a couple of transmedial poetic works created in the new millennium, and what this experience does to the relationship between the understanding of the concepts of culture and nature. The works being examined are, firstly, digital animations of Bengt Emil Johnson’s concrete poetry from the 1960’s, made by Christian Yde Frostholm in 2005, secondly Frostholm’s own poetry/photo-work “Afrevne ord” from 2004, and finally Johannes Heldén’s work “Evolution” from 2014. In all these cases the relationship between application and code is being put into play, partly due to the digital form, partly due to a foregrounded meta-perspective upon this form. This meta-perspective also sheds light on the idea of a divide between culture and nature – and questions the premises of that idea.

    • This chapter examines the epistemological potential of a poetry that moves between languages and media, and how this potential can be put into use in relation to the Anthropocene, regarded as a planetary crisis. The literary example that works as a touchstone is the transmedial and multilingual work of Swedish/Finnish/German poet and artist Cia Rinne (b. 1973). As the primary backdrop for these ideas lies the philosophy of Gilles Deleuze and Félix Guattari. In combination with poetic practice, their ideas of e.g. “schizoanalysis,” “planes of immanence,” “the rhizomatic,” “lines of flight,” and “the minoritarian,” can assist in conceptualizing classrooms that do not keep on repeating the discourses that have taken the relationship between humankind and Earth to the threshold state of the Anthropocene.

    • Poetry has gained new momentum over the last couple of decades, due to the digital revolution. It could be argued, somewhat paradoxically, that this is to a large extent indebted to the fact that poetry for a long period before this moved further and further into a hermetic margin: due to this marginalized position poetry, ironically, seemed to have nothing to lose when new media came along. In comparison to e.g. prose, poetry has embraced the digital revolution to a much larger extent. An outcome of this development is that poetry has (over the last 10-15 years) been busy creating all sorts of alliances – not only with new media, but also with other art forms – in an unprecedented way (the phenomenon is by no means new, but the level of extent is). These alliances have resulted in a wide range of new forms of poetry, all based on ideas of heterogeneity, and even impurity. Stefan Kjerkegaard (2010) has made a distinction between ”lyric” and ”poetry” within contemporary poetic practice, in order to tell the dense, esoteric, and principally monologic, tradition (lyric) apart from the more recent tradition which is instead based on impurity, heterogeneity and mediatization (poetry). My paper suggests that the study of this second, more recent, poetic tradition offers vast opportunities, not least within our educational systems. Contemporary poetry has, like few other art forms, taken on the challenges of mediatization and of digital information technologies. Within it, you can therefore see media and marketing logic, being both employed and scrutinized to an unusually high degree (i.e. contemporary poetry does not only relate to media logic, it also involves in a discussion on its political (Cf. Rancière) consequences). The didactic interface of new media-studies when studying contemporary poetry would therefore be very rewarding – and vice versa. In my paper I will show this by using a couple of poetic examples from the last decade.

    • Research shows that teachers and teacher students believe that poetry is important within the school system, but still shun it. The main reason is the widespread notion that poetry is difficult. And indeed, poetry can be just that, but it can also be fun and easy – this is the basic duality of poetry. As a university teacher of literature, I come across this shunning on a regular basis. However, it resembles another conflict that is just as frequent amongst teacher trainees: the one between the university classroom, and the classroom where the students will eventually be teaching. Through sets of thinking that embrace patience and risk, as opposed to reliability and guarantees (exemplified by Gert Biesta’s G.C. Spivak’s theories), these two conflicts could be dealt with simultaneously. The same goes for pedagogy: when choosing pedagogical material that address this duality of poetry, two things tend to happen in the classroom: a) poetry becomes fun (and a bit less difficult); b) poetry helps the students see the difference – and the relation – between the two classrooms. In this chapter I argue that this happens due to the analogy between the duality of poetry and that of teacher training.

    • Poetic form varies, poetic forms vary – not least in our time. This variety is one of the paradoxical traits of what we might refer to as “poetic”. This aspect could be, and has been, described from a number of theoretical positions. Joseph M. Conte has referred to as a binary between “protean” and “proteinic”, Paul Hernadi has spoken of it in terms of “concurrency”, Mutlu Konuk Blasing calls it the “non-rational order” of poetic language, whereas Lucy Alford has recently talked in terms of varieties of “poetic attention”. In contrast to other discourses, the poetic does invariably involve (the potential to) its own negation, and a certain, unavoidable and inherent, degree of randomness and arbitrariness, no matter how much a poet attempts to be in control of the process. This has to do with poetry’s actual working on language, not only with it, or through it. Or in Alford’s terms: “how poems compose attention and how attention is in turn poetry’s most essential ‘raw material’” (Alford 4). In his critique of the legacy of Ferdinand de Saussure’s semiotic theory, Jean-Jacques Lecercle states that this legacy tends to overlook the premise that saying ”I speak language” always needs to be complemented by also claiming that “language speaks”. Furthermore, Lecercle claims that it is within poetry and nonsensical texts that this premise is most obvious. What he refers to as “the remainder of language” is precisely that which cannot be controlled. The more we try to control it, the more it tends to become performative – and bite back! It could be claimed that Lecercle’s complementary statement might be stretched even further, into “language speaks me” (Alfredsson 45). In my paper, I study these facets of the poetic within a few various poetic forms. The general idea is that through the poetic aspect of “concurrency”, poetic texts always come with a potential to overturn not only themselves and their claims, but also other control and power structures. The remainder of language might – in its biting back – bring with it other remainders, groups and discourses that are being kept out of power. My readings will exemplify a number of such power structures, such as class, gender, race, and age.

    • What happens when a writer crosses the boundaries of literature? This is a question which it is quite pointless to ask, unless you state your theoretical point of departure. When that has been done, however, it is one of the most important issues which a literary scholar can raise. Through certain parts of the theories of Jean-Jacques Lecercle (mainly his concept of the “remainder” in language), Marjorie Perloff (mainly her discussions of how free verse is convention-based, and of what comes out of that insight: what she labels “after free verse”), and Jacques Rancière (mainly his concepts “polis” and “the political”), I aim to discuss what happens when Swedish contemporary poets Anna Hallberg and Lars-Mikael Raattamaa during recent years have sought to actively cross the boundaries of literature. These theoretical perspectives enable me to dwell on the political implications of this kind of boundary-crossing. This political dimension consists of a critique not only of the conventions of literature, but also of the modernist view of e.g. free verse. The crossing of literature’s boundaries not only moves them. More importantly, in these cases, it identifies them; it even highlights them, rendering them open for scrutiny.

    • This doctoral thesis departs from the idea that the poetry of Bengt Emil Johnson (b. 1936) could very well be described as part of a long lyrical tradition, rather than as a reaction against this very tradition, which is the more common view. Throughout the study, this basic notion is employed as a sort of interface, or method, upon Johnson’s development as a poet. This development departs from 1950’s neo-romanticist poetic ideals, moves along through the 1960’s “open art” and its heavy critique of con¬ventions, and into a 1970’s writing which is in many ways a synthesis of the first two. By elaborating on a notion which Johnson himself labels “untranslatability” – a term with many facets, one of which could be described as a kind of manifestation of Johnson’s own perspective upon lyrical tradition – this study points out vital parallels between e.g. ecology and semiotics, place and language, and between romanti¬cism, modernism and post-modernism. It also adds new perspectives to the fields of genre-study and versification. The study primarily focuses on the part of Johnson’s oeuvre which could be described as the synthesis (1973-) between a tradition based and a disruption based period. This conduct of things enables the pointing out of mutual factors between the seemingly contradictory ideas which pervade the two earlier periods. Generally, the analyses in the thesis focus on connections and similarities between a traditionally lyrical, and a dis¬ruptively avantgardist manner of writing poetry. Where others have put much thought into examining what separates these manners, this study searches for their common ground.

    • The purpose of this article is to study what the French linguist Jean-Jacques Lecercle identified as ”the remainder of language” in his book The Violence of Language (1990), and how this is being put into play in two poetry books from 2008, by the Swedish poets Anna Hallberg and Lars Mikael Raattamaa. By explicitly challenging a large number of conventions of language, typography, and poetry, these books raise critical issues on how the language system works. These issues specifically concern what parts and aspects of language are allowed in within the linguistic system, and what parts and aspects are being kept out. These regulational mechanisms are demonstrated, and scrutinised, in these books – giving them touches of meta-critique, as well as of ideology critique. This scrutiny not only shows the relevance of identifying and understanding the mechanisms of exclusion that Lecercle identified within academic linguistics 25 years ago. It also brings them into contemporary society, where they – due to globalisation, and technological development – have disseminated, and become part of almost everyone’s daily experience. The results of the article stress how this dissemination increases the societal relevance of the kind of scrutiny undertaken by Hallberg and Raattamaa, and how it can assist in re-ideologising language.

It is important to ensure that the treatments which are provided are both clinically effective and economically justifiable. Based on recent new evidence, routine oxygen therapy is no longer recommended in MI patients without hypoxemia. By using data from a nationwide randomized clinical trial, we estimated oxygen therapy related cost savings in this important clinical setting. Methods: The DETermination of the role of Oxygen in suspected Acute Myocardial Infarction (DETO2X-AMI) trial randomized 6,629 patients from 35 hospitals across Sweden to oxygen at 6 L/min for 6–12 h or ambient air. Costs for drug and medical supplies, and labor were calculated per patient, for the whole study population, and for the total annual care episodes for MI in Sweden (N = 16,100) with 10 million inhabitants. Results: Per patient, costs were estimated to 36 USD, summing up to a total cost of 119,832 USD for the whole study population allocated to oxygen treatment. Applied to the annual care episodes for MI in Sweden, costs sum up to between 514,060 and 604,777 USD. In the trial, 62 (2%) patients assigned to oxygen and 254 (8%) patients assigned to ambient air developed hypoxemia. A threshold analysis suggested that up to a cut-off of 624 USD spent for hypoxemia treatment related costs per patient, avoiding routine oxygen therapy remains cost saving. Conclusions: Avoiding routine oxygen therapy in patients with suspected or confirmed MI without hypoxemia at baseline saves significant expenditure for the health care system both with regards to medical and human resources. However, no evidence regarding treatment effects on health-related quality of life (HRQoL) exists. In this study, we investigated the impact of routine oxygen supplementation on HRQoL 6–8 weeks after hospitalization with acute MI. Secondary objectives included analyses of MI subtypes, further adjustment for infarct size, and oxygen saturation at baseline and 1-year follow-up.Methods: In the DETermination of the role of Oxygen in suspected Acute Myocardial Infarction (DETO2X-AMI) trial, 6,629 normoxemic patients with suspected MI were randomized to oxygen at 6 L/min for 6–12 h or ambient air. In this prespecified analysis, patients younger than 75 years of age with confirmed MI who had available HRQoL data by European Quality of Life Five Dimensions questionnaire (EQ-5D) in the national registry were included. Primary endpoint was the EQ-5D index assessed by multivariate linear regression at 6–10 weeks after MI occurrence.Results: A total of 3,086 patients (median age 64, 22% female) were eligible, 1,518 allocated to oxygen and 1,568 to ambient air. We found no statistically significant effect of oxygen therapy on EQ-5D index (−0.01; 95% CI: −0.03–0.01; p = 0.23) or EQ-VAS score (−0.57; 95% CI: −1.88–0.75; p = 0.40) compared to ambient air after 6–10 weeks. Furthermore, no significant difference was observed between the treatment groups in EQ-5D dimensions. Results remained consistent across MI subtypes and at 1-year follow-up, including further adjustment for infarct size or oxygen saturation at baseline.Conclusions: Routine oxygen therapy provided to normoxemic patients with acute MI did not improve HRQoL up to 1 year after MI occurrence. A secondary objective was to investigate outcomes in patients developing hypoxemia.BACKGROUND: In the DETO2X-AMI (Determination of the Role of Oxygen in Suspected Acute Myocardial Infarction) trial, 6,629 normoxemic patients with suspected MI were randomized to oxygen at 6 l/min for 6 to 12 hours or ambient air.METHODS: The study population of 5,010 patients with confirmed MI was divided by baseline oxygen saturation into a low-normal (90% to 94%) and a high-normal (95% to 100%) cohort. Outcomes are reported within 1 year. To increase power, all follow-up time (between 1 and 4 years) was included post hoc, and interaction analyses were performed with oxygen saturation as a continuous covariate.RESULTS: The composite endpoint of all-cause death, rehospitalization with MI, or heart failure occurred significantly more often in patients in the low-normal cohort (17.3%) compared with those in the high-normal cohort (9.5%) (p < 0.001), and most often in patients developing hypoxemia (23.6%). Oxygen therapy compared with ambient air was not associated with improved outcomes regardless of baseline oxygen saturation (interaction p values: composite endpoint, p = 0.79; all-cause death, p = 0.33; rehospitalization with MI, p = 0.86; hospitalization for heart failure, p = 0.35).CONCLUSIONS: Irrespective of oxygen saturation at baseline, we found no clinically relevant beneficial effect of routine oxygen therapy in normoxemic patients with MI regarding cardiovascular outcomes. Low-normal baseline oxygen saturation or development of hypoxemia was identified as an independent marker of poor prognosis.
