  • A framework for regulating underwater noise during pile driving

    • Pile driving is a common technique used during the construction of bridges, offshore wind power, and underwater infrastructure or shoreline structures. It is the process by which a foundation, beam or pole is hammered or vibrated down into the bottom, which can generate extremely loud noise that propagates throughout the surrounding water and sediment. The noise can reach such high levels that marine animals are at risk of disturbance, injury or even death.Sweden currently lacks established thresholds stating the level at which underwater noise potentially disturbs or injures marine animals. Hence, there are no guidance values for allowable underwater noise levels from noiseproducing activities to avoid serious environmental impacts. Several countries in Europe have defined thresholds for when underwater noise can result in severe negative environmental impacts as well as standards for measuring, analysing and reporting underwater noise levels.The purpose of this study is to review the scientific literature on underwater noise from pile driving and its effects on marine life. The study aims to define the noise levels that can cause injury and other negative effects and, on this basis, recommend noise levels that can be used to establish guidance values for regulating underwater noise for Swedish waters and species. The study presents examples of the factors that contribute to sound propagation in Swedish waters and how this influences the noise level from a pile strike as a function of distance at four study areas along the Swedish coast. Additionally, the study contains a thorough technical description of pile driving activities, basic underwater acoustics and noise effects on marine animals. These effects (injury and behavioral, e.g., flight, but not subtle effects) are demonstrated on representative species such as the harbour porpoise (Phocoena phocoena), Atlantic cod (Gadus morhua), Atlantic herring (Clupea harengus) and on fish larvae and eggs. The study’s authors look atthe original sources of information that other countries base their guidelines and thresholds on, so the recommendations follow scientifically determined levels rather than values that have been rounded off or otherwise altered.The study presents sound levels in three different units, each with different biological relevance to the effects caused by a pile driving activity. None of the sound levels have been frequency weighted for a specific species, as this method is not yet fully established. The first unit used is the sound pressure level SPL(peak), which is the maximum overpressure or underpressure of the noise pulse generated by the pile strike. This unit has a high relevance for behavioural effects. The sound exposure level, SEL, is the calculated energy level over a period of time and expresses the energy of the entire sound pulse. SEL is the unit most related to hearing impairing effects. SEL(ss) is the value for a single strike while SEL(cum) is the cumulative value of a determined number of pulses over a period of time. The review revealed that for Atlantic cod and Atlantic herring there are currently no studies that can be used to determine a species’ specific threshold value for injury, but studies show that loud noise can affect both species negatively. Because of this, the recommended noise levels for injury are based mainly on studies on other species exposed to pile driving noise in laboratory environments, supported by studies conducting large-scale experiments in tanks and oceans. The levels at which fish are at risk of death or sustaining serious injury to internal organs is SPL 207 dB re 1 μPa, SEL(ss) 174 dB re 1 μPa2s and SEL(cum) 204 dB re 1 μPa2s. Note that for injury in fish, the cumulative sound exposure level has higher relevance than the single-strike level as the cited studies found injuries after a certain time period of exposure. The thresholds for fish larvae and eggs are based on the fact that no negative effects were observed at exposures of up to SPL(peak) 217 dB re 1 μPa, SEL(ss) 187 dB re 1 μPa2s and SEL(cum) 207 dB re 1 μPa2s. However, there are relatively few studies on early life stages of fish. There are more species-specific studies on harbour porpoises regarding noise than there are for Atlantic cod and Atlantic herring. Nonetheless, only a few can be used to determine thresholds that will lead to injury or negative behavioural effects. The levels at which there is a risk of a temporary impact on hearing, i.e. temporary threshold shift (TTS), for the harbour porpoises is SPL(peak) 194 dB re 1 μPa, SEL(ss) 164 dB re 1 μPa2s and SEL(cum) 175 dB re 1 μPa2s. When it comes to TTS, the cumulative sound exposure level, SEL(cum), is of primary importance. However, this unit is dependent on a specific time and number of pulses. For permanent threshold shift (PTS), the level is set to SPL(peak) 200 dB re 1 μPa, SEL(ss) 179 dB re 1 μPa2s and SEL(cum) 190 dB re 1 μPa2s. The recommended level should be revised as new relevant studies are conducted.

    • Vid byggnation av broar, havsbaserad vindkraft och andra havsbaserade eller strandnära konstruktioner används det oftast någon form av pålningsteknik för att få ner konstruktionen i botten. Detta innebär att ett fundament, balk eller spont hamras eller vibreras ner i botten, vilket kan generera mycket höga ljudnivåer som sprids ut i vattnet och ner i botten. Ljudnivåerna är så höga att marina organismer kan störas, skadas eller till och med dödas. Idag saknar Sverige fastställda ljudnivåer för när undervattensbuller blir så högt att de kan skada djur i havet. Det saknas därför också vedertagna begränsningsvärden som anger vilka nivåer av undervattensbuller som kan tillåtas för bullrande aktiviteter utan att riskera allvarliga miljöeffekter. Flera länder i Europa har någon form av gränsvärden för när bullernivån under vattnet kan ge upphov till allvarlig miljöpåverkan liksom standarder för hur undervattensbuller skall mätas och rapporteras. Syftet med denna studie var att ta fram ett vetenskapligt underlag rörande ljudet från pålning i havet och dess påverkan på det marina livet. Slutmålet var att utifrån den vetenskapliga information som finns idag ge förslag på ljudnivåer för skador och negativ påverkan som sedan kan användas för att ta fram begränsningsvärden för reglering av undervattensbuller anpassade för svenska vatten och arter. Studien ger ett antal exempel på vilka faktorer som påverkar ljudutbredningen i svenska vatten och hur detta påverkar ett pålningsslags ljudnivå som funktion av avstånd i fyra typområden kring den svenska kusten. Vidare presenteras ingående både tekniska beskrivningar av pålningsaktiviteter, undervattensakustik samt påverkan på marina djur. Denna påverkan (skada och flyktbeteende men ej subtila effekter) demonstreras med hjälp av ett antal typarter som tandvalen tumlare (Phocoena phocoena) och fiskarterna torsk (Gadus morhua) och sill (Clupea clupea) samt fiskägg och fisklarver. I denna studie har författarna gått tillbaka till de originalkällor av information som andra länders gränsvärden grundas på, så att rekommendationerna bygger på vetenskapliga nivåer och inte värden som har avrundats eller på annat sätt ändrats.Studien presenterar ljudnivåer i tre olika enheter då dessa har olika biologisk relevans för påverkan från en pålningsaktivitet. Inga av dessa värden har frekvensviktats för att anpassas för en specifik art då denna metod ännu inte är helt vedertagen. Den första enheten är ljudtrycksnivå SPL(topp), d.v.s. det maximala över- eller undertryck som den av pålningsslaget genererade ljudpulsen har. Denna enhet har hög relevans för beteendepåverkan. För ljudexponeringsnivå SEL, beräknas ljudnivån över en viss tid och tar då med energin i hela ljudpulsen. SEL är den enhet som visats vara bäst relaterad till hörselskador. SEL(enkel) är värdet för en enkel puls och för det kumulativa SEL(kum) har antalet pulser under en viss tid summerats.Litteraturstudien på torsk och sill visar att det i dagsläget inte finns några studier som kan användas för att fastställa en artspecifik ljudnivå för skada men litteraturen visar tydligt på att höga bullernivåer kan påverka torsk och sill negativt. Istället baseras de föreslagna nivåerna i huvudsak på studier på andra arter som har exponerats för pålningsljud i laboratoriemiljö med stöd av studier från mer storskaliga experiment i tankar och hav. De nivåer då fisk riskerar att dödas eller få allvarliga skador på inre organ är 207 dB re 1 μPa SPL(topp), 174 dB re 1 μPa2s SEL(enkel) och 204 dB re 1 μPa2s SEL(kum). Notera att för skada på fisk har det kumulativa värdet högre relevans än enkelvärdet för SEL eftersom studier visar att skador uppkommer efter en viss tids exponering. Nivåerna för påverkan på fiskägg och larver grundas i att inga negativa effekter har observerats vid exponering för ljudtryck från pålning upp till 217 dB re 1 μPa SPL(topp), 187 dB re 1 μPa2s SEL(enkel) och 207 dB re 1 μPa2s SEL(kum). Det finns emellertid mycket få studier relaterat till pålningsljud för dessa livsstadier. För tumlare finns det fler artspecifika studier gjorda relaterat till buller än för torsk och sill. Det är dock endast ett fåtal som kan användas för att bestämma ljudnivåer som leder till skada eller negativ beteendepåverkan. De ljudnivåer som riskerar ge tillfällig hörselnedsättning (TTS) hos tumlare är 194 dB re 1 μPa SPL(topp), 164 dB re 1 μPa2s SEL(enkel) och 175 dB re 1 μPa2s SEL(kum). Det är framförallt den kumulativa ljudexponeringsnivån SEL(kum) som har stor betydelse för just TTS, dock hänger detta värde ihop med en specifik tid och antalet pulser vilket kan vara svårt att uppskatta i förväg. Vidare avseende permanent hörselskada (PTS) är ljudnivån 200 dB re 1 μPa SPL(topp), 179 dB re 1 μPa2s SEL(enkel) och 190 dB re 1 μPa2s SEL(kum). Föreslagna nivåer bör uppdateras när nya relevanta forskningsstudier tillkommer.

    • Age has a major effect on brain volume. However, the normative studies available are constrained by small sample sizes, restricted age coverage and significant methodological variability. These limitations introduce inconsistencies and may obscure or distort the lifespan trajectories of brain morphometry. In response, we capitalized on the resources of the Enhancing Neuroimaging Genetics through Meta-Analysis (ENIGMA) Consortium to examine age-related trajectories inferred from cross-sectional measures of the ventricles, the basal ganglia (caudate, putamen, pallidum, and nucleus accumbens), the thalamus, hippocampus and amygdala using magnetic resonance imaging data obtained from 18,605 individuals aged 3-90 years. All subcortical structure volumes were at their maximum value early in life. The volume of the basal ganglia showed a monotonic negative association with age thereafter; there was no significant association between age and the volumes of the thalamus, amygdala and the hippocampus (with some degree of decline in thalamus) until the sixth decade of life after which they also showed a steep negative association with age. The lateral ventricles showed continuous enlargement throughout the lifespan. Age was positively associated with inter-individual variability in the hippocampus and amygdala and the lateral ventricles. These results were robust to potential confounders and could be used to examine the functional significance of deviations from typical age-related morphometric patterns.

    • Delineating the association of age and cortical thickness in healthy individuals is critical given the association of cortical thickness with cognition and behavior. Previous research has shown that robust estimates of the association between age and brain morphometry require large-scale studies. In response, we used cross-sectional data from 17,075 individuals aged 3-90 years from the Enhancing Neuroimaging Genetics through Meta-Analysis (ENIGMA) Consortium to infer age-related changes in cortical thickness. We used fractional polynomial (FP) regression to quantify the association between age and cortical thickness, and we computed normalized growth centiles using the parametric Lambda, Mu, and Sigma method. Interindividual variability was estimated using meta-analysis and one-way analysis of variance. For most regions, their highest cortical thickness value was observed in childhood. Age and cortical thickness showed a negative association; the slope was steeper up to the third decade of life and more gradual thereafter; notable exceptions to this general pattern were entorhinal, temporopolar, and anterior cingulate cortices. Interindividual variability was largest in temporal and frontal regions across the lifespan. Age and its FP combinations explained up to 59% variance in cortical thickness. These results may form the basis of further investigation on normative deviation in cortical thickness and its significance for behavioral and cognitive outcomes.

  • Integrated continuous biomanufacturing on pilot scale for acid-sensitive monoclonal antibodies

    • In this study, we demonstrated the first, to our knowledge, integrated continuous bioprocess (ICB) designed for the production of acid-sensitive monoclonal antibodies, prone to aggregate at low pH, on pilot scale. A high cell density perfusion culture, stably maintained at 100 × 106 cells/ml, was integrated with the downstream process, consisting of a capture step with the recently developed Protein A ligand, ZCa; a solvent/detergent-based virus inactivation; and two ion-exchange chromatography steps. The use of a mild pH in the downstream process makes this ICB suitable for the purification of acid-sensitive monoclonal antibodies. Integration and automation of the downstream process were achieved using the Orbit software, and the same equipment and control system were used in initial small-scale trials and the pilot-scale downstream process. High recovery yields of around 90% and a productivity close to 1 g purified antibody/L/day were achieved, with a stable glycosylation pattern and efficient removal of impurities, such as host cell proteins and DNA. Finally, negligible levels of antibody aggregates were detected owing to the mild conditions used throughout the process. The present work paves the way for future industrial-scale integrated continuous biomanufacturing of all types of antibodies, regardless of acid stability.

  • Reducing the EPEI-Time Using Discrete Event Simulation

    • One of the cornerstones in LEAN production is ‘make to order’, which requires small batch sizes and, thus, short Every Part Every Interval (EPEI) times. EPEI-time is defined as the time it takes to produce all product variants, before the first variant in the cycle returns in the schedule. However, many companies are reluctant to reduce their EPEI-times due to the increased number of set-ups. This skepticism is also supported by parts of existing theory, while other research contributions mean that companies often can reduce batch-sizes without affecting productivity. This paper presents a case study which uses discrete event simulation (DES) to evaluate the relation between EPEI-time and productivity. The results show that it is possible to reduce the EPEI-time and still maintain productivity and service levels to customers, without any investments. Increased variation in the production schedule evened out the load among the machines and, hence, the time lost in set-ups was gained in more parallel work.

  • Housing tenure and residential mobility in Stockholm 1990-2014

    • In this thesis the links between housing tenure, income and selective, segregation generating, residential mobility are explored. The development of these links is analysed against the background of housing regime changes in Stockholm between 1990 and 2014. Housing policy changes in Sweden, and Stockholm, promote ownership through, for instance, housing tenure conversions and the tax-system. What this development means for residential mobility trends and may mean for ethnic and economic segregation is explored in three articles. Paper 1 contrasts two time periods and provides an analysis of residential mobility and economic sorting. It is shown that the socioeconomic composition of movers is relatively stable over time while the increasingly owner dominated housing market in Stockholm contributes to stronger socio-spatial residential patterns through the strengthened economic sorting of movers across the whole income scale. Paper 2 has a focus on ethnic and socioeconomic differences in out-mobility from poor neighbourhoods. The findings indicate that foreign background residents are dependent upon housing wealth and income to be able to leave poor neighbourhoods when they move, while the Swedish background group has a variety of resources at their disposal when they move. Paper 3 analyses how housing tenure affects moving, and movers' destinations, in neighbourhoods with high concentrations of non-western foreign-born residents in Stockholm. By comparing two cohorts (1993-2000 and 2001-2008) it is analysed how this relationship develops over time. Housing tenure and income do not seem to be pivotal for who moves, but increasingly important for where movers end up. Results display ethnic differences and how the changing housing market in Stockholm reproduces ethnic segregation. Two main conclusions from the thesis are that (i) the changing housing regime in Stockholm produces stronger economic sorting of movers – this has been affected by the geographically, socioeconomically and ethnically uneven gains from the housing market transformations experienced over the last three decades. (ii) The residential mobility patterns and the economic stratification of residential mobility opportunities that the reconfiguration of Stockholm’s housing market gives rise to increases the economic, political and social marginalization of neighbourhoods characterized by low income levels and high shares of foreign-born residents. Combating ethnic segregation is today even more closely related to the socioeconomic differences between the foreign-born and native-born parts of the population.

  • Simulation in Virtual World to Promote Communication

    • Introduction Communication between ambulance professionals and patients is essential for understanding the patient's lifeworld (Wireklint Sundström & Dahlberg 2010). Simultaneously, communication is challenging to teach and learn within the framework of specific courses. However, simulation in virtual worlds can support the development of new skills such as communication (Combs, Sokolowski & Banks 2016). AimThe aim of this work was to design a simulation-based platform for communication training among ambulance nurse students (ANS). MethodsA qualitative action research approach was used (Coghlan & Casey 2001). Second Life® (SL) was selected since it was an existing virtual world. SL is a web-based flexible three-dimensional platform that allows customization. Interaction and communication with other virtual people can be done through avatars in real time (Hodge, Collins & Giordano 2011). Three ANS and five teachers participated, none of the participants had prior experience of SL. Observations and interviews were used as data and analysed using thematic analysis. Results The participants’ experiences generated three themes: Understanding the virtual worldIt was easy to interact and communicate with other virtual people. However, it took time to feel comfortable to navigate in SL. Technological challengesOne challenge was related to audio-visual problems e.g. not compatible headset, interfering echoes and that the image was distorted at times, which made it difficult to act and move the avatar. Another challenge was associated with the 3D modelling e.g. the capability to use of coordinates, positioning, object dimensioning and the fact that accidental deletions could not be restored. A third challenges that influenced the communication was the difficulty of visualizing clinically relevant care measures such as diagnostic examinations or drug treatment. Finally, there was a challenge to customize the avatars to look like ambulance professionals or a severely ill patient. Learning through avatars Learning through avatars requires that the participants take responsibility for delivering a convincing performance.  Immersion was limited since actions do not take place from a first-person viewpoint. There is a need that the scenario is based on realistic conditions e.g. interiors, equipment, clothing, avatar appearance and behaviour. Conclusion The present system is not suitable for training of medical assessment. Teachers who are considering using virtual worlds in the training for future ambulance professionals should note that an appropriate design is crucial for how the simulation is experienced.

  • Polymer Brushes on Silica Nanostructures Prepared by Aminopropylsilatrane Click Chemistry: Superior Antifouling and Biofunctionality

    • In nanobiotechnology, the importance of controlling interactions between biological molecules and surfaces is paramount. In recent years, many devices based on nanostructured silicon materials have been presented, such as nanopores and nanochannels. However, there is still a clear lack of simple, reliable, and efficient protocols for preventing and controlling biomolecule adsorption in such structures. In this work, we show a simple method for passivation or selective biofunctionalization of silica, without the need for polymerization reactions or vapor-phase deposition. The surface is simply exposed stepwise to three different chemicals over the course of ∼1 h. First, the use of aminopropylsilatrane is used to create a monolayer of amines, yielding more uniform layers than conventional silanization protocols. Second, a cross-linker layer and click chemistry are used to make the surface reactive toward thiols. In the third step, thick and dense poly(ethylene glycol) brushes are prepared by a grafting-to approach. The modified surfaces are shown to be superior to existing options for silica modification, exhibiting ultralow fouling (a few ng/cm2) after exposure to crude serum. In addition, by including a fraction of biotinylated polymer end groups, the surface can be functionalized further. We show that avidin can be detected label-free from a serum solution with a selectivity (compared to nonspecific binding) of more than 98% without the need for a reference channel. Furthermore, we show that our method can passivate the interior of 150 nm × 100 nm nanochannels in silica, showing complete elimination of adsorption of a sticky fluorescent protein. Additionally, our method is shown to be compatible with modifications of solid-state nanopores in 20 nm thin silicon nitride membranes and reduces the noise in the ion current. We consider these findings highly important for the broad field of nanobiotechnology, and we believe that our method will be very useful for a great variety of surface-based sensors and analytical devices.

  • Effects of different laser welding parameters on the joint quality for dissimilar material joints for battery applications

    • For battery pack assemblies, it is crucial that the laser welded cell-to-busbar joints demonstrate both high mechanical strength and minimal electrical resistance. The present study investigates the effect of different laser welding parameters, on the mechanical strength, electrical resistance, porosity formation and joint microstructure, for dissimilar material cell-to-busbar joints. Laser welding experiments are performed, on thin nickel-plated copper and steel plates. The plates are joined in an overlap configuration, using laser beam wobbling and power modulation. Both circular and sinusoidal laser beam wobbling are used as selected strategies to increase the interface width of the joints, where also a comparison is made between the two methods. The joint quality is evaluated using joint geometry analysis, shear strength tests, computed tomography scanning and electrical resistance measurements. The results show that circular laser beam wobbling gives a larger joint shear strength compared with sinusoidal laser beam wobbling. In addition, it is observed that both the total pore volume and material mixing are significantly increased with increasing laser power and wobbling frequency for circular laser beam wobbling. However, for the sinusoidal laser beam wobbling the wobbling frequency does not show a significant impact on the total pore volume.

    • Static form errors due to in-process deflections is a major concern in flank milling of thin-walled parts. To increase both productivity and part geometric accuracy, there is a need to predict and control these form errors. In this work, a modelling framework for prediction of the cutting force-induced form errors, or thickness errors, during flank milling of a thin-walled workpiece is proposed. The modelled workpiece geometry is continuously updated to account for material removal and the reduced stiffness matrix is calculated for nodes in the engagement zone. The proposed modelling framework is able to predict the resulting thickness errors for a thin-walled plate which is cut on both sides. Several cutting strategies and cut patterns using constant z-level finishing are studied. The modelling framework is used to investigate the effect of different cut patterns, machining allowance, cutting tools and cutting parameters on the resulting thickness errors. The framework is experimentally validated for various cutting sequences and cutting parameters. The predicted thickness errors closely correspond to the experimental results. It is shown from numerical evaluations that the selection of an appropriate cut pattern is crucial in order to reduce the thickness error. Furthermore, it is shown that an increased machining allowance gives a decreased thickness error for thin-walled plates.

    • This thesis consists of four papers on applications of stochastic processes. In Paper I we study an open population SIS (Susceptible - Infective - Susceptible) stochastic epidemic model from the time of introduction of the disease, through a possible outbreak and to extinction. The analysis uses coupling arguments and diffusion approximations. In Paper II we propose a model describing an economy where companies may default due to contagion. The features of the model are analyzed using diffusion approximations. We show that the model can reproduce oscillations in the default rates similar to what has been observed empirically. In Paper III we consider the problem of finding an optimal betting strategy for a house-banked casino card game that is played for several coups before reshuffling. A limit result for the return process is found and the optimal card counting strategy is derived. This continuous time strategy is shown to be a natural generalization of the discrete time strategy where the so called effects of removals are replaced by the infinitesimal generator of the card process. In Paper IV we study interest rate models where the term structure is given by an affine relation and in particular where the driving stochastic processes are so-called generalised Ornstein-Uhlenbeck processes. We show that the return and variance of a portfolio of bonds which are continuously rolled over, also called rolling horizon bonds, can be expressed using the cumulant generating functions of the background driving Lévy processes associated with the OU processes. We also show that if the short rate, in a risk-neutral setting, is given by a linear combination of generalised OU processes, the implied term structure can be expressed in terms of the cumulant generating functions.

    • In machining, the heat flow into the workpiece during the cutting process is often a major concern. The temperature rise can lead to substantial residual stresses or elastic in-process deformations which may result in the dimensional tolerance requirements being violated. In the present study a modelling strategy is developed for determination of the heat load during indexable drilling. The heat load on the workpiece is determined from 3D thermomechanical Coupled Eulerian Lagrangian analyses of orthogonal turning for various chip thicknesses and cutting speeds. The determined heat load is then transferred to a 3D transient heat transfer analysis of the indexable drilling process for the determination of the temperature field. Thereby, this modelling technique avoids the complex cutting process that is performed in real cutting simulations and thereby reducing the computational complexity of the problem considerably. The simulated temperatures are compared with experimentally measured temperatures and some conclusions are drawn regarding the modelling approach.

    • The focus of this report is on the growing importance of skills upgrading in SMEs, coupled with the search for a comprehensive policy approach to meet the new needs and opportunities in this area. Emphasis is placed on the diversity of SMEs, ranging from the vast community of relatively stable and mature firms to the more limited group of high-tech and/or potentially fast-growing categories of firms which we may know as gazelles or similar names. The report argues that it is time to apply a specific double-edged approach to strengthen these firms’ competencies and the preconditions for regional and societal growth in increasingly knowledge-based societies. SMEs are more vulnerable than large firms; they invest less in R&D and their processes for skills upgrading are often problematic. Public funding is commonly provided to educational or public research institutions, earmarked for promoting courses adapted to the needs, for instance, of traditional or “blue-collar” sectors. There is an inadequate interface between traditional educational institutions and the specific knowledge and competency needs of SMEs and entrepreneurs. This situation, which is due to fundamental incentive problems, leads to frustrating outcomes. The report highlights the presence of: partly contradictory interests between owners/managers and workers; lack of information, skills and/or time at management level; and differences in bridging the highly specialised demands for skills development at business level and the more general supply of skills by education providers such as universities. Against this backdrop, the report examines a new way forward – an approach that aims to address the upgrading of core skills at SMEs and to increase the supply of adequate external, professional services while also trying to augment potential positive synergies between these two levels. Examining the policies pursued by a set of selected countries, however, permits identification of only a few serious attempts to foster a coherent approach to encompass direct training of SMEs and measures to bolster complementary service providers. Most programmes remain piecemeal and have various limitations. Weaknesses commonly relate to the dominant influence of supply-side perspectives in training programmes. There is also generally a lack of effective identification of and adaptation to SME needs. Still, the report identifies a few cases of clear-cut synergies between measures at the two levels, notably in Belgium and Ireland.

    • In machining, the heat generated during the process deforms the components and the final shape might not meet specified tolerances. There is therefore a need for a compensation strategy which requires knowledge of the workpiece temperature field and the associated thermal distortions. In this work, a methodology is presented for the determination of the heat load for indexable insert drilling of AISI 4140. Compared to previous research, this work has introduced a varying heat load. The heat load is extracted from thermo-mechanical finite element simulations for different nominal chip thicknesses and cutting speeds using the coupled Eulerian-Lagrangian formulation of an orthogonal turning process. The heat load is then transferred to a simplified 2D axisymmetric heat transfer model where the in-process temperature field in the workpiece is predicted. To verify the methodology, the predicted temperatures are compared to the experimentally measured temperatures for various feed rates. It is found that the model is capable of predicting the workpiece temperatures reasonably well. However, the methodology needs to be further explored to validate its applicability.

    • Prediction of solar power generation is important in order to optimize energy exchanges in future micro-grids that integrate a large amount of photovoltaics. However, an accurate prediction is difficult due to the uncertainty of weather phenomena that impact produced power. In this paper, we evaluate the impact of different clustering methods on the forecast accuracy for predicting hourly ahead solar power when using machine learning based prediction approaches trained on weather and generated power features. In particular, we compare clustering methods using clearness index and K-means clustering, where we use both euclidian distance and dynamic time-warping. For evaluating prediction accuracy, we develop and compare different prediction models for each of the clusters using production data from a swedish SmartGrid. We demonstrate that proper tuning of thresholds for the clearness index improves prediction accuracy by 20.19% but results in worse performance than using K-means with all weather features as input to the clustering.

    • Evacuation preparedness includes ensuring proper infrastructure, resources and planning for moving people from a dangerous area to safety. This is especially important and challenging during mass gatherings, such as large concerts. In this paper, we present the Emergency Exit Layout Problem (EELP) which is the problem of locating a given number of emergency exits and deciding their width such that the time it takes to evacuate the crowd from an arena is minimized. The EELP takes into account the geography of the arena and its surroundings, as well as the number of pedestrians in the crowd and the distribution of these within the arena. The EELP is formulated as a two-stage stochastic mixed integer linear program to handle the uncertainty related to the location of the possible incidents and the distribution of the pedestrians. Two cases are studied, a large concert planned at the Leangen trotting track in Trondheim and a smaller indoor arena. For each case, the EELP is solved for different scenarios, and the suggested layouts are evaluated using an agent-based simulation model. In particular, the potential of incorporating detailed assessment regarding the location and probability of specific incidents and the distribution of pedestrians are investigated. The computational study shows that making a more detailed risk assessment has little effect on the large concert, but a significant impact on the location of the emergency exits for the smaller indoor case. The results also indicate that it is more important to consider the location and probability of specific incidents rather than the pedestrian distribution.

    • Background: Internet administered cognitive behaviour therapy (CBT) is a promising new way to deliver psychological treatment, but its effectiveness in regular care settings and in relation to more traditional CBT group treatment has not yet been determined. The primary aim of this study was to compare the effectiveness of Internetand group administered CBT for panic disorder (with or without agoraphobia) in a randomised trial within a regular psychiatric care setting. The second aim of the study was to establish the cost-effectiveness of these interventions. Methods: Patients referred for treatment by their physician, or self-referred, were telephone-screened by a psychiatric nurse. Patients fulfilling screening criteria underwent an in-person structured clinical interview carried out by a psychiatrist. A total of 113 consecutive patients were then randomly assigned to 10 weeks of either guided Internet delivered CBT (n = 53) or group CBT (n = 60). After treatment, and at a 6-month follow-up, patients were again assessed by the psychiatrist, blind to treatment condition. Results: Immediately after randomization 9 patients dropped out, leaving 104 patients who started treatment. Patients in both treatment conditions showed  significant improvement on the main outcome measure, the Panic Disorder Severity Scale (PDSS) after treatment. For the Internet treatment the within-group effect size (pre-post) on the PDSS was Cohen's d = 1.73, and for the group treatment it was d = 1.63. Between group effect sizes were low and treatment effects were maintained at 6-months follow-up. We found no statistically significant differences between the two treatment conditions using a mixed models approach to account for missing data. Group CBT utilised considerably more therapist time than did Internet CBT. Defining effect as proportion of PDSS responders, the cost-effectiveness analysis concerning therapist time showed that Internet treatment had superior cost-effectiveness ratios in relation to group treatment both at post-treatment and follow-up. Conclusions: This study provides support for the effectiveness of Internet CBT in a psychiatric setting for patients with panic disorder, and suggests that it is equally effective as the more widely used group administered CBT in reducing panic-and agoraphobic symptoms, as well as being more cost effective with respect to therapist time.

    • Background: Carriers of the 1q21.1 distal and 15q11.2 BP1-BP2 copy number variants exhibit regional and global brain differences compared with noncarriers. However, interpreting regional differences is challenging if a global difference drives the regional brain differences. Intraindividual variability measures can be used to test for regional differences beyond global differences in brain structure.Methods: Magnetic resonance imaging data were used to obtain regional brain values for 1q21.1 distal deletion (n = 30) and duplication (n = 27) and 15q11.2 BP1-BP2 deletion (n = 170) and duplication (n = 243) carriers and matched noncarriers (n = 2350). Regional intra-deviation scores, i.e., the standardized difference between an individual's regional difference and global difference, were used to test for regional differences that diverge from the global difference.Results: For the 1q21.1 distal deletion carriers, cortical surface area for regions in the medial visual cortex, posterior cingulate, and temporal pole differed less and regions in the prefrontal and superior temporal cortex differed more than the global difference in cortical surface area. For the 15q11.2 BP1-BP2 deletion carriers, cortical thickness in regions in the medial visual cortex, auditory cortex, and temporal pole differed less and the prefrontal and somatosensory cortex differed more than the global difference in cortical thickness.Conclusions: We find evidence for regional effects beyond differences in global brain measures in 1q21.1 distal and 15q11.2 BP1-BP2 copy number variants. The results provide new insight into brain profiling of the 1q21.1 distal and 15q11.2 BP1-BP2 copy number variants, with the potential to increase understanding of the mechanisms involved in altered neurodevelopment.

    • Compressed sensing (CS) is a universal low-complexity data compression technique for signals that have a sparse representation in some domain. While CS data compression can be done both in the analog- and digital domain, digital implementations are often used on low-power sensor nodes, where an ultra-low-power (ULP) processor carries out the algorithm on Nyquist-rate sampled data. In such systems an energy-efficient implementation of the CS compression kernel is a vital ingredient to maximize battery lifetime. In this paper, we propose an application-specific instruction-set processor (ASIP) processor that has been optimized for CS data compression and for operation in the subthreshold (sub-VT) regime. The design is equipped with specific sub-VT capable standard-cell based memories, to enable low-voltage operation with low leakage. Our results show that the proposed ASIP accomplishes 62× speed-up and 11.6× power savings with respect to a straightforward CS implementation running on the baseline low-power processor without instruction set extensions.

    • Fluorescence-based multispectral imaging of rapidly moving or dynamic samples requires both fast two-dimensional data acquisition as well as sufficient spectral sensitivity for species separation. As the number of fluorophores in the experiment increases, meeting both these requirements becomes technically challenging. Although several solutions for fast imaging of multiple fluorophores exist, they all have one main restriction; they rely solely on spectrally resolving either the excitation- or the emission characteristics of the fluorophores. This inability directly limits how many fluorophores existing methods can simultaneously distinguish. Here we present a snapshot multispectral imaging approach that not only senses the excitation and emission characteristics of the probed fluorophores but also all cross term combinations of excitation and emission. To the best of the authors’ knowledge, this is the only snapshot multispectral imaging method that has this ability, allowing us to even sense and differentiate between light of equal wavelengths emitted from the same fluorescing species but where the signal components stem from different excitation sources. The current implementation of the technique allows us to simultaneously gather 24 different spectral images on a single detector, from which we demonstrate the ability to visualize and distinguish up to nine fluorophores within the visible wavelength range.

    • Various initiatives are in place worldwide to reduce the time from the occurrence of an out-of-hospital cardiac arrest to the start of cardiopulmonary resuscitation (CPR) and defibrillation using an Automated External Defibrillator (AED). In addition to optimizing the management of emergency medical services and other professional emergency response resources, these include utilizing close-by volunteers using mobile phone dispatching, and delivering AEDs using Unmanned Aerial Vehicles (UAVs), commonly known as drones. The latter two examples have previously been studied separately, and it has been proved that the time to administer CPR and AED delivery can be reduced. In this paper, the potential of joint planning and dispatching of volunteers and AED delivering drones is investigated. Optimization models are used to determine good locations for drones. These are then evaluated, together with new, adapted dispatch strategies for the volunteers, using a simulation model. Results of a case study, with data for the county of Vastra Gotaland in Sweden, show that drones can indeed help reduce the time to defibrillation, and, by adaptive dispatching, the time to CPR can be reduced as well. Thus, it is shown that by joint planning of drones and volunteers, it may be possible to improve the survival probability from out-of-hospital cardiac arrest.

  • How a Point-of-Care Dashboard Facilitates Co-production of Health Care and Health for and with Individuals with Psychotic Disorders : A Mixed-methods Case Study

    • BackgroundIndividuals with psychotic disorders experience widespread treatment failures and risk early death. Sweden’s largest department specializing in psychotic disorders sought to improve patients’ health by developing a point-of-care dashboard to support joint planning and co-production of care. The dashboard was tested for 18 months and included more than 400 patients at two outpatient clinics.MethodsThis study evaluates the dashboard by addressing two questions:Can differences in health-related outcome measures be attributed to the use of the dashboard?How did the case managers experience the accessibility, use, and usefulness of the dashboard for co-producing care with individuals with psychotic disorders? This mixed-method case study used both Patient-Reported Outcome Measures (PROM) and data from a focus group interview with case managers. Data collection and analysis were framed by the Clinical Adoption Meta Model (CAMM) phases: i) accessibility, ii) system use, iii) behavior, and iv) clinical outcomes. The PROM used was the 12-item World Health Organization Disability Assessment Schedule (WHODAS 2.0), which assesses functional impairment and disability. Patients at clinics using the dashboard were matched with patients at clinics not using the dashboard. PROM data were compared using non-parametric statistics due to skewness in distribution. The focus group included five case managers who had experience using the dashboard with patients.ResultsCompared to patients from clinics that did not use the dashboard, patients from clinics that did use the dashboard improved significantly overall (p = 0.045) and in the domain self-care (p = 0.041). Focus group participants reported that the dashboard supported data feedback-informed care and a proactive stance related to changes in patients’ health. The dashboard helped users identify critical changes and enabled joint planning and evaluation.ConclusionDashboard use was related to better patient health (WHODAS scores) when compared with matched patients from clinics that did not use the dashboard. In addition, case managers had a positive experience using the dashboard. Dashboard use might have lowered the risk for missing critical changes in patients’ health while increasing the ability to proactively address needs. Future studies should investigate how to enhance patient co-production through use of supportive technologies.

    • Psychotic disorders like schizophrenia have a typical onset in early adulthood with symptoms of hallucinations and disturbances of thought. Despite knowledge on what constitutes effective schizophrenia care, more than 70% of treatment attempts fail in Sweden, sometimes leading to early death. An appraisal of schizophrenia care shows a lack of ways to jointly plan and evaluate care, and an absence of a trustworthy theory-of-change. The Learning Health System (LHS) is a vision that has been translated into theories and models associated with improved outcomes for patients with other chronic conditions. The aim of this thesis is to enhance the understanding of the applicability of the LHS vision in the context of schizophrenia care, from the perspectives of both individuals and the health system in enabling coproduction of better health by addressing two research questions:i) How can improvement of health for individuals with schizophrenia and improvement of system performance be supported by coproduction in an LHS model?ii) Can an LHS-based intervention, i.e. the use of a point of care dashboard, contribute to better health for individuals with schizophrenia?Studying the existing published knowledge of LHS show that the concept has not yet been applied in mental healthcare settings but has potential to increase patient coproduction, continuous improvement and better health. Different forms of coproduction are supported in the most comprehensive LHS models and applications, ranging from dashboards at point of care to platforms that can help facilitate improvement initiatives.A case study, focused on studying the use and usefulness of a point-of-care dashboard at patient visits in outpatient care at the Department of Schizophrenia Spectrum Disorders at Sahlgrenska University Hospital in western Sweden. Use of the dashboard is associated with improved communication and health for patients. Assessment of the dashboard-project’s complexity using the Non-adoption, abandonment, scale-up, spread and sustainability complexity assessment tool (NASSS-CAT) was perceived as helpful in evaluating challenges and provided insight that can guide future development. An LHS model, that builds on both the reviewing of the literature and practical testing, is proposed.Further research is proposed in two areas, exploration of how dashboard initiatives can support coproduction and better health for individuals with complex chronic conditions and further development of LHS models by studying different LHS initiatives regarding system properties, forms of coproduction at play and effects on health outcomes for individuals and populations.

    • BACKGROUND: Co-production of health is defined as 'the interdependent work of users and professionals who are creating, designing, producing, delivering, assessing, and evaluating the relationships and actions that contribute to the health of individuals and populations'. It can assume many forms and include multiple stakeholders in pursuit of continuous improvement, as in Learning Health Systems (LHSs). There is increasing interest in how the LHS concept allows integration of different knowledge domains to support and achieve better health. Even if definitions of LHSs include engaging users and their family as active participants in aspects of enabling better health for individuals and populations, LHS descriptions emphasize technological solutions, such as the use of information systems. Fewer LHS texts address how interpersonal interactions contribute to the design and improvement of healthcare services.OBJECTIVE: We examined the literature on LHS to clarify the role and contributions of co-production in LHS conceptualizations and applications.METHOD: First, we undertook a scoping review of LHS conceptualizations. Second, we compared those conceptualizations to the characteristics of LHSs first described by the US Institute of Medicine. Third, we examined the LHS conceptualizations to assess how they bring four types of value co-creation in public services into play: co-production, co-design, co-construction and co-innovation. These were used to describe core ideas, as principles, to guide development.RESULT: Among 17 identified LHS conceptualizations, 3 qualified as most comprehensive regarding fidelity to LHS characteristics and their use in multiple settings: (i) the Cincinnati Collaborative LHS Model, (ii) the Dartmouth Coproduction LHS Model and (iii) the Michigan Learning Cycle Model. These conceptualizations exhibit all four types of value co-creation, provide examples of how LHSs can harness co-production and are used to identify principles that can enhance value co-creation: (i) use a shared aim, (ii) navigate towards improved outcomes, (iii) tailor feedback with and for users, (iv) distribute leadership, (v) facilitate interactions, (vi) co-design services and (vii) support self-organization.CONCLUSIONS: The LHS conceptualizations have common features and harness co-production to generate value for individual patients as well as for health systems. They facilitate learning and improvement by integrating supportive technologies into the sociotechnical systems that make up healthcare.     • Background: Health care is becoming more complex. For an increasing number of individuals, interacting with health care means addressing more than just one illness or disorder, engaging in more than one treatment, and interacting with more than one care provider. Individuals with severe mental illnesses such as schizophrenia are disproportionately affected by this complexity. Characteristic symptoms can make it harder to establish and maintain relationships. Treatment failure is common even where there is access to effective treatments, increasing suicide risk. Knowledge of complex adaptive systems has been increasingly recognized as useful in understanding and developing health care. A complex adaptive system is a collection of interconnected agents with the freedom to act based on their own internalized rules, affecting each other. In a complex health care system, relevant feedback is crucial in enabling continuous learning and improvement on all levels. New technology has potential, but the failure rate of technology projects in health care is high, arguably due to complexity. The Nonadoption, Abandonment, and challenges to Scale-up, Spread, and Sustainability (NASSS) framework and complexity assessment tool (NASSS-CAT) have been developed specifically to help identify and manage complexity in technology-related development projects in health care. Objective: This study aimed to use a pilot version of the NASSS-CAT instrument to inform the development and deployment of a point-of-care dashboard supporting schizophrenia care in west Sweden. Specifically, we report on the complexity profile of the project, stakeholders' experiences with using NASSS-CAT, and practical implications. Methods: We used complexity assessment to structure data collection and feedback sessions with stakeholders, thereby informing an emergent approach to the development and deployment of the point-of-care dashboard. We also performed a thematic analysis, drawing on observations and documents related to stakeholders' use of the NASSS-CAT to describe their views on its usefulness. Results: Application of the NASSS framework revealed different types of complexity across multiple domains, including the condition, technology, value proposition, organizational tasks and pathways, and wider system. Stakeholders perceived the NASSS-CAT tool as useful in gaining perspective and new insights, covering areas that might otherwise have been neglected. Practical implications derived from feedback sessions with managers and developers are described. Conclusions: This case study shows how stakeholders can identify and plan to address complexities during the introduction of a technological solution. Our findings suggest that NASSS-CAT can bring participants a greater understanding of complexities in digitalization projects in general.

  • Acute heart failure after non-cardiac surgery: incidence, phenotypes, determinants and outcomes

    • Aims Primary acute heart failure (AHF) is a common cause of hospitalization. AHF may also develop postoperatively (pAHF). The aim of this study was to assess the incidence, phenotypes, determinants and outcomes of pAHF following non-cardiac surgery.Methods and results A total of 9164 consecutive high-risk patients undergoing 11 262 non-cardiac inpatient surgeries were prospectively included. The incidence, phenotypes, determinants and outcome of pAHF, centrally adjudicated by independent cardiologists, were determined. The incidence of pAHF was 2.5% (95% confidence interval [CI] 2.2-2.8%); 51% of pAHF occurred in patients without known heart failure (de novo pAHF), and 49% in patients with chronic heart failure. Among patients with chronic heart failure, 10% developed pAHF, and among patients without a history of heart failure, 1.5% developed pAHF. Chronic heart failure, diabetes, urgent/emergent surgery, atrial fibrillation, cardiac troponin elevations above the 99th percentile, chronic obstructive pulmonary disease, anaemia, peripheral artery disease, coronary artery disease, and age, were independent predictors of pAHF in the logistic regression model. Patients with pAHF had significantly higher all-cause mortality (44% vs. 11%, p < 0.001) and AHF readmission (15% vs. 2%, p < 0.001) within 1 year than patients without pAHF. After Cox regression analysis, pAHF was an independent predictor of all-cause mortality (adjusted hazard ratio [aHR] 1.7 [95% CI 1.3-2.2]; p < 0.001) and AHF readmission (aHR 2.3 [95% CI 1.5-3.7]; p < 0.001). Findings were confirmed in an external validation cohort using a prospective multicentre cohort of 1250 patients (incidence of pAHF 2.4% [95% CI 1.6-3.3%]).Conclusions Postoperative AHF frequently developed following non-cardiac surgery, being de novo in half of cases, and associated with a very high mortality.

  • Influence of scanning strategy on residual stresses in laser powder bed fusion manufactured alloy 718: Modeling and experiments

    • Residual stresses are a known phenomenon in additively manufactured materials. The residual stresses increase the risk of cracks, limit in-service performance, and distort printed parts. In this work, thermo-mechanical finite element models using the hatch-by-hatch and layer-by-layer approach, and the inherent strain method has been developed and applied to predict the effects of different scanning strategies on the deflection and the residual stresses of two PBF-LB processed geometries. To account for viscoplasticty and relaxation effects, a mechanism-based material model have been implemented and used. It is shown that the hatch-by-hatch approach and inherent strain method both successfully predicted the experimentally measured deflections of the first geometry, which was printed using different scanning directions. To predict the stress field experimentally, high-energy synchrotron measurements have been used to. The thermo-mechanical models and the inherent strain method both captures the trend of experimentally measured residual stress fields, although with an overall underprediction. The predictions of the models were evaluated, and their accuracy discussed in terms of physical aspects of the Powder Bed Fusion – Laser Beam process.

  • Novel genetic loci associated with hippocampal volume

    • The hippocampal formation is a brain structure integrally involved in episodic memory, spatial navigation, cognition and stress responsiveness. Structural abnormalities in hippocampal volume and shape are found in several common neuropsychiatric disorders. To identify the genetic underpinnings of hippocampal structure here we perform a genome-wide association study (GWAS) of 33,536 individuals and discover six independent loci significantly associated with hippocampal volume, four of them novel. Of the novel loci, three lie within genes (ASTN2, DPP4 and MAST4) and one is found 200 kb upstream of SHH. A hippocampal subfield analysis shows that a locus within the MSRB3 gene shows evidence of a localized effect along the dentate gyrus, subiculum, CA1 and fissure. Further, we show that genetic variants associated with decreased hippocampal volume are also associated with increased risk for Alzheimer's disease (r(g) = -0.155). Our findings suggest novel biological pathways through which human genetic variation influences hippocampal volume and risk for neuropsychiatric illness.

  • Tailored vs. Standardized Internet-Based Cognitive Behavior Therapy for Depression and Comorbid Symptoms: A Randomized Controlled Trial

    • Background and Aims: Major depression can be treated by means of cognitive behavior therapy, delivered via the Internet as guided self-help. Individually tailored guided self-help treatments have shown promising results in the treatment of anxiety disorders. This randomized controlled trial tested the efficacy of an Internet-based individually tailored guided self-help treatment which specifically targeted depression with comorbid symptoms. The treatment was compared both to standardized (non-tailored) Internet-based treatment and to an active control group in the form of a monitored online discussion group. Both guided self-help treatments were based on cognitive behavior therapy and lasted for 10 weeks. The discussion group consisted of weekly discussion themes related to depression and the treatment of depression. Methods: A total of 121 participants with diagnosed major depressive disorder and with a range of comorbid symptoms were randomized to three groups. The tailored treatment consisted of a prescribed set of modules targeting depression as well as comorbid problems. The standardized treatment was a previously tested guided self-help program for depression. Results: From pre-treatment to post-treatment, both treatment groups improved on measures of depression, anxiety and quality of life. The results were maintained at a 6-month follow-up. Subgroup analyses showed that the tailored treatment was more effective than the standardized treatment among participants with higher levels of depression at baseline and more comorbidity, both in terms of reduction of depressive symptoms and on recovery rates. In the subgroup with lower baseline scores of depression, few differences were seen between treatments and the discussion group. Conclusions: This study shows that tailored Internet-based treatment for depression is effective and that addressing comorbidity by tailoring may be one way of making guided self-help treatments more effective than standardized approaches in the treatment of more severe depression.

    • Molecular docking plays an important role in drug discovery as a tool for the structure-based design of small organic ligands for macromolecules. Possible applications of docking are identification of the bioactive conformation of a protein−ligand complex and the ranking of different ligands with respect to their strength of binding to a particular target. We have investigated the effect of implicit water on the postprocessing of binding poses generated by molecular docking using MM-PB/GB-SA (molecular mechanics Poisson−Boltzmann and generalized Born surface area) methodology. The investigation was divided into three parts: geometry optimization, pose selection, and estimation of the relative binding energies of docked protein−ligand complexes. Appropriate geometry optimization afforded more accurate binding poses for 20% of the complexes investigated. The time required for this step was greatly reduced by minimizing the energy of the binding site using GB solvation models rather than minimizing the entire complex using the PB model. By optimizing the geometries of docking poses using the GBHCT+SA model then calculating their free energies of binding using the PB implicit solvent model, binding poses similar to those observed in crystal structures were obtained. Rescoring of these poses according to their calculated binding energies resulted in improved correlations with experimental binding data. These correlations could be further improved by applying the postprocessing to several of the most highly ranked poses rather than focusing exclusively on the top-scored pose. The postprocessing protocol was successfully applied to the analysis of a set of Factor Xa inhibitors and a set of glycopeptide ligands for the class II major histocompatibility complex (MHC) Aq protein. These results indicate that the protocol for the postprocessing of docked protein−ligand complexes developed in this paper may be generally useful for structure-based design in drug discovery.

    • A single nucleotide substitution in intron 3 of IGF2 in pigs abrogates a binding site for a repressor and leads to a 3-fold up-regulation of IGF2 in skeletal muscle. The mutation has major effects on muscle growth, size of the heart, and fat deposition. Here, we have identified the repressor and find that the protein, named ZBED6, is previously unknown, specific for placental mammals, and derived from an exapted DNA transposon. Silencing of Zbed6 in mouse C2C12 myoblasts affected Igf2 expression, cell proliferation, wound healing, and myotube formation. Chromatin immunoprecipitation (ChIP) sequencing using C2C12 cells identified about 2,500 ZBED6 binding sites in the genome, and the deduced consensus motif gave a perfect match with the established binding site in Igf2. Genes associated with ZBED6 binding sites showed a highly significant enrichment for certain Gene Ontology classifications, including development and transcriptional regulation. The phenotypic effects in mutant pigs and ZBED6-silenced C2C12 myoblasts, the extreme sequence conservation, its nucleolar localization, the broad tissue distribution, and the many target genes with essential biological functions suggest that ZBED6 is an important transcription factor in placental mammals, affecting development, cell proliferation, and growth.

  • Fed-batch production assessment of a tetravalent bispecific antibody : A case study on piggyBac stably transfected HEK293 cells

    • The transition from preclinical biological drug development into clinical trials requires an efficient upscaling process. In this context, bispecific antibody drugs are particularly challenging due to their propensity to form aggregates and generally produce low titers. Here, the upscaling process for a tetravalent bispecific antibody expressed by a piggyBac transposon-mediated stable HEK293 cell pool has been evaluated. The project was performed as a case study at Testa Center, a non-GMP facility for scale-up testing of biologics in Sweden, and encompassed media adaptation strategies, fed-batch optimization and a novel antibody purification technology. The cell pool was adapted to different culture media for evaluation in terms of cell viability and titers compared to its original Expi293 Expression Medium. These parameters were assessed in both sequential stepwise adaption and direct media exchanges. By this, a more affordable medium was identified that did not require stepwise adaptation and with similar titers and viability as in the Expi293 Expression Medium. Fed-batch optimizations resulted in culture densities reaching up to 20 x 106 viable cells/mL with over 90 % viability 12 days postinoculum, and antibody titers three times higher than corresponding batch cultures. By implementing a novel high-speed protein A fiber technology (Fibro PrismA) with a capture residence time of only 7.5 s, 8 L of supernatant could be purified in 4.5 h without compromising the purity, structural integrity and function of the bispecific antibody. Results from this study related to medium adaptation and design of fed-batch protocols will be highly beneficial during the forthcoming scale-up of this therapeutic antibody.

  • Effects of continuous feedback on households’ electricity consumption: Potentials and barriers

    • Two field experiments were carried out to study (a) the effects on energy savings of continuous visual feedback via in-home displays, and (b) the motives for responding or not. In study 1, 40 participants living in separate or semi-detached houses in two different towns participated. All participants received a questionnaire and a list of possible energy saving measures. Households were then randomly assigned to an experimental condition (display) or a control condition (no display). In study 2, 32 households in rented apartments participated. No significant differences between the conditions were found for either of the studies. In study 2, semi-structured interviews were conducted among nine of the households. Through an analysis of interview transcripts barriers were identified explaining why the feedback intervention was not sufficient to change behaviour and reduce consumption. The barriers experienced indicate that there is a risk of overconfidence in IHDs. For the development of energy policies and more wide-scale implementation, it is important to be aware of the potential obstacles to success.

  • Mortality with Paclitaxel-Coated Devices in Peripheral Artery Disease.

    • The results of a recent meta-analysis aroused concern about an increased risk of death associated with the use of paclitaxel-coated angioplasty balloons and stents in lower-limb endovascular interventions for symptomatic peripheral artery disease.We conducted an unplanned interim analysis of data from a multicenter, randomized, open-label, registry-based clinical trial. At the time of the analysis, 2289 patients had been randomly assigned to treatment with drug-coated devices (the drug-coated-device group, 1149 patients) or treatment with uncoated devices (the uncoated-device group, 1140 patients). Randomization was stratified according to disease severity on the basis of whether patients had chronic limb-threatening ischemia (1480 patients) or intermittent claudication (809 patients). The single end point for this interim analysis was all-cause mortality.No patients were lost to follow-up. Paclitaxel was used as the coating agent for all the drug-coated devices. During a mean follow-up of 2.49 years, 574 patients died, including 293 patients (25.5%) in the drug-coated-device group and 281 patients (24.6%) in the uncoated-device group (hazard ratio, 1.06; 95% confidence interval, 0.92 to 1.22). At 1 year, all-cause mortality was 10.2% (117 patients) in the drug-coated-device group and 9.9% (113 patients) in the uncoated-device group. During the entire follow-up period, there was no significant difference in the incidence of death between the treatment groups among patients with chronic limb-threatening ischemia (33.4% [249 patients] in the drug-coated-device group and 33.1% [243 patients] in the uncoated-device group) or among those with intermittent claudication (10.9% [44 patients] and 9.4% [38 patients], respectively).In this randomized trial in which patients with peripheral artery disease received treatment with paclitaxel-coated or uncoated endovascular devices, the results of an unplanned interim analysis of all-cause mortality did not show a difference between the groups in the incidence of death during 1 to 4 years of follow-up.     • Background: There is a need to better distinguish viral infections from antibiotic-requiring bacterial infections in children presenting with clinical community-acquired pneumonia (CAP) to assist health care workers in decision making and to improve the rational use of antibiotics. Objective: The overall aim of the Trial of Respiratory infections in children for ENhanced Diagnostics (TREND) study is to improve the differential diagnosis of bacterial and viral etiologies in children aged below 5 years with clinical CAP, by evaluating myxovirus resistance protein A (MxA) as a biomarker for viral CAP and by evaluating an existing (multianalyte point-of-care antigen detection test system [mariPOC respi] ArcDia International Oy Ltd.) and a potential future point-of-care test for respiratory pathogens. Methods: Children aged 1 to 59 months with clinical CAP as well as healthy, hospital-based, asymptomatic controls will be included at a pediatric emergency hospital in Stockholm, Sweden. Blood (analyzed for MxA and C-reactive protein) and nasopharyngeal samples (analyzed with real-time polymerase chain reaction as the gold standard and antigen-based mariPOC respi test as well as saved for future analyses of a novel recombinase polymerase amplification-based point-of-care test for respiratory pathogens) will be collected. A newly developed algorithm for the classification of CAP etiology will be used as the reference standard. Results: A pilot study was performed from June to August 2017. The enrollment of study subjects started in November 2017.
