  • Participation in leisure activities and binge drinking in adults – findings from a Swedish general population sample.

    • The aim of this cross-sectional study was to estimate the associations between participation in different leisure activities and binge drinking in Swedish adults. The study was based on a questionnaire in a general population sample (n=3567) of individuals aged 19–64 years old. Men and women were defined as binge drinkers if they reported that they had consumed alcohol at least once a month, and stated that at a typical drinking occasion they consumed five or more standard glasses (12g of alcohol). Multivariate analyses found associations between binge drinking and socializing with friends among men aged 19–30 years (odds ratio, OR 2.88), in the 31–64 years old age group (OR 1.87). Corresponding results was found in younger women (OR 2.36). A higher OR was also found for younger men who regularly attended sporting events as spectators (OR 1.83), and among respondents in the older age group who regularly played computer or video games (OR 2.11 for women and 1.61 for men). A lower OR for binge drinking was found for men who regularly participated in religious services in both age-groups. Lower prevalence of binge drinking among women was only found in the younger group among those who regularly participated in sports/athletics or other training (OR 0.51). Our findings suggest that prevention strategies could benefit from an everyday life approach, but also that different interventions should be used in relation to specific leisure activities.

    • Objective: The main objective was to report mechanisms and precursors for post-endovascular aneurysm repair (EVAR) rupture. The second was to apply a structured protocol to explore whether these factors were identifiable on follow up computed tomography (CT) prior to rupture. The third objective was to study the incidence, treatment, and outcome of post-EVAR rupture. Methods: This was a multicentre, retrospective study of patients treated with standard EVAR at five Swedish hospitals from 2008 to 2018. Patients were identified from the Swedvasc registry. Medical records were reviewed up to 2020. Index EVAR and follow up data were recorded. The primary endpoint was post-EVAR rupture. CT at follow up and at post-EVAR rupture were studied, using a structured protocol, to determine rupture mechanisms and identifiable precursors. Results: In 1 805 patients treated by EVAR, 45 post-EVAR ruptures occurred in 43 patients. The cumulative incidence was 2.5% over a mean follow up of 5.2 years. The incidence rate was 4.5/1 000 person years. Median time to post-EVAR rupture was 4.1 years. A further six cases of post-EVAR rupture in five patients found outside the main cohort were included in the analysis of rupture mechanisms only. The rupture mechanism was type IA in 20 of 51 cases (39%), IB in 20 of 51 (39%) and IIIA/B in 11 of 51 (22%). One of these had type IA + IB combined. One patient had an aortoduodenal fistula without another mechanism being identified. Precursors had been noted on CT follow up prior to post-EVAR rupture in 16 of 51 (31%). Retrospectively, using the structured protocol, precursors could be identified in 43 of 51 (84%). In 17 of 27 (63%) cases missed on follow up but retrospectively identifiable, the mechanisms were type IB/III. Overall, the 30 day mortality rate after post-EVAR rupture was 47% (n = 24/51) and the post-operative mortality rate was 21% (n = 7/33). Conclusions: Most precursors of post-EVAR rupture are underdiagnosed but identifiable before rupture using a structured follow up CT protocol. Precursors of type IB and III failures caused the majority of post-EVAR ruptures.

    • Internationaliseringen av den svenska akademin har kommit olika långt vid olika lärosäten och inom olika ämnesområden. Det är dock uppenbart att Sveriges lärosäten aktivt kommer att behöva ytterligare internationalisera sin verksamhet för att kunna påverka ännu mer lokalt, regionalt, nationellt och internationellt. Internationellt samarbete finns givetvis sedan länge inom utbildning och/eller forskning, men sällan sammanhållen med samverkan med näringsliv och offentlig verksamhet. Allt fler lärosäten funderar nu på hur man strategiskt ska koppla ihop internationalisering med lärosätets ambition för samverkan. Högskolornas nästa utmaning är att bygga kompletta internationella kunskapsmiljöer där samverkan kommer att behövas i lika stor grad som på den nationella arenan.

  • Incidence of Ischemic Stroke in Individuals with and without Aortic Valve Stenosis : A Danish Retrospective Cohort Study

    • Background and Purpose - Aortic valve stenosis may lead to atrial and ventricular remodeling, predisposes to atrial fibrillation, and may also be an independent risk factor of ischemic stroke. However, information on stroke rates among persons with aortic valve stenosis are sparse. We aimed to determine the incidence rates and relative risks of ischemic stroke in individuals with diagnosed aortic valve stenosis compared with age- and sex-matched controls. Methods - All patients with incident aortic valve stenosis aged >18 years (n=79 310) and age- and sex-matched controls were identified using the Danish nationwide registries (1997-2017). Incidence rates per 1000 person-years (PY) and multivariable adjusted hazard ratios with 95% CIs were reported. Results - In total, 873 373 individuals (median age 77 years, 51.5% men, 9.1% with aortic valve stenosis) were included. Ischemic stroke occurred in 70 205 (8.0%) individuals during 4 880 862 PY of follow-up. Incidence rates of ischemic stroke were 13.3/1000 PY among the controls compared with 30.4/1000 PY in patients with aortic valve stenosis, corresponding to a hazard ratio of 1.31 (95% CI, 1.28-1.34). In all age-groups, the incidence rates and relative risks were significantly increased in patients with aortic valve stenosis compared with controls, but the relative risk was greater for younger individuals (eg, age group, 18-45 years: hazard ratio, 5.94 [95% CI, 4.10-8.36]). In patients with aortic valve stenosis above 65 years of age, the risk of ischemic stroke was markedly lower after aortic valve replacement (30.3 versus 19.6/1000 PY before and after valve replacement). Among people with atrial fibrillation the incidence rate of ischemic stroke was 1.5 times higher when aortic valve stenosis was present (33.0/1000 PY versus 49.9/1000 PY). Conclusions - People with aortic valve stenosis have a significantly increased risk of ischemic stroke compared with age- and sex-matched controls. Future studies are warranted to explore whether antithrombotic therapy may be beneficial in some individuals.

    • BACKGROUND: Particulate air pollution affects cardiovascular and pulmonary disease and mortality. A main hypothesis about the mechanisms involved is that particles induce inflammation in lower airways, systemic inflammation and oxidative stress. OBJECTIVES: To examine whether short-term exposure to wood smoke in healthy subjects affects markers of pulmonary inflammation and oxidative stress. METHODS: 13 subjects were exposed first to clean air and then to wood smoke in a chamber during 4-hour sessions, 1 week apart. The mass concentrations of fine particles at wood smoke exposure were 240-280 mug/m(3), and number concentrations were 95 000-180 000/cm(3), about half of the particles being ultrafine (<100 nm). Blood and breath samples were taken before and at various intervals after exposure to wood smoke and clean air and examined for exhaled nitric oxide and Clara cell protein in serum and urine, and malondialdehyde in exhaled breath condensate. RESULTS: Exposure to wood smoke increased alveolar nitric oxide 3 hours post-exposure while malondialdehyde levels in breath condensate were higher both immediately after and 20 hours after exposure. Serum Clara cell protein was increased 20 hours after exposure. CONCLUSIONS: Wood smoke at levels that can be found in smoky indoor environments caused an inflammatory response and signs of increased oxidative stress in the respiratory tract, especially in the lower airways.

  • Outcomes associated with dual antiplatelet therapy after myocardial infarction in patients with aortic stenosis

    • Background: Acquired loss of the largest von Willebrand factor multimers is a common hemostatic disturbance in patients with aortic valve stenosis (AS), resulting in impaired platelet adhesion and increased bleeding risk. AS is also associated with atherosclerosis and myocardial infarction (MI). Our aim was to study the clinical outcomes associated with AS in MI patients treated with dual antiplatelet therapy (DAPT) in a nationwide hospital-based register study. Methods: Based on nationwide hospital discharge registers from Sweden (2005–2010) and Denmark (2005–2015), we calculated 1-year incidence rates and hazard ratios of bleeding, recurrent MI, and all-cause mortality in MI patients with and without AS treated with DAPT. Results from both countries were also combined in a meta-analysis. Results: We included 50,460 MI patients from Sweden and 50,307 MI patients from Denmark, of which 3% had AS. The bleeding rates (per 100 person-years) in Sweden and Denmark were 3.2 and 3.3 among patients without AS vs. 9.2 and 8.3 among patients with AS. All-cause mortality rates were 7.1 vs. 28.7 in Sweden and 5.8 vs. 30.7 in Denmark among patients without and with AS, respectively. Patients with AS had an increased risk of bleeding, recurrent MI and all-cause mortality. Combined results from both countries were similar for bleeding (hazard ratio 1.59 [0.98–2.59]), recurrent MI (1.78 [1.25–2.54]), and all-cause mortality (1.76 [1.26–2.47]). Conclusion: AS was associated with an increased risk of bleeding, recurrent MI and mortality after MI when treated with DAPT. Individualized selection of antiplatelet therapy may be warranted in this high-risk population.

  • DLA Class II Alleles Are Associated with Risk for Canine Symmetrical Lupoid Onychodystropy (SLO)

    • Symmetrical lupoid onychodystrophy (SLO) is an immune-mediated disease in dogs affecting the claws with a suggested autoimmune aethiology. Sequence-based genotyping of the polymorphic exon 2 from DLA-DRB1, -DQA1, and -DQB1 class II loci were performed in a total of 98 SLO Gordon setter cases and 98 healthy controls. A risk haplotype (DRB1*01801/DQA1*00101/DQB1*00802) was present in 53% of cases and 34% of controls and conferred an elevated risk of developing SLO with an odds ratio (OR) of 2.1. When dogs homozygous for the risk haplotype were compared to all dogs not carrying the haplotype the OR was 5.4. However, a stronger protective haplotype (DRB1*02001/DQA1*00401/DQB1*01303, OR = 0.03, 1/OR = 33) was present in 16.8% of controls, but only in a single case (0.5%). The effect of the protective haplotype was clearly stronger than the risk haplotype, since 11.2% of the controls were heterozygous for the risk and protective haplotypes, whereas this combination was absent from cases. When the dogs with the protective haplotype were excluded, an OR of 2.5 was obtained when dogs homozygous for the risk haplotype were compared to those heterozygous for the risk haplotype, suggesting a co-dominant effect of the risk haplotype. In smaller sample sizes of the bearded collie and giant schnauzer breeds we found the same or similar haplotypes, sharing the same DQA1 allele, over-represented among the cases suggesting that the risk is associated primarily with DLA-DQ. We obtained conclusive results that DLA class II is significantly associated with risk of developing SLO in Gordon setters, thus supporting that SLO is an immune-mediated disease. Further studies of SLO in dogs may provide important insight into immune privilege of the nail apparatus and also knowledge about a number of inflammatory disorders of the nail apparatus like lichen planus, psoriasis, alopecia areata and onycholysis.

    • Aims Left ventricular hypertrophy, obesity, hypertension, and N-terminal B-type natriuretic peptide (Nt-proBNP) predict left ventricular diastolic dysfunction with preserved systolic function (DD-PSF). Self-rated health (SRH) is shown to be associated with chronic diseases, but the association of SRH with DD-PSF is unclear. In light of the clinical implications of DD-PSF, the following goals are of considerable importance: (1) to determine the role of SRH in patients with DD-PSF in the general population and (2) to study the association between Nt-proBNP and DD-PSF. Methods and results The current study is a cross-sectional study conducted on a random sampling of a rural population. Individuals 30-75 years of age were consecutively subjected to conventional echocardiography and tissue velocity imaging. Data were collected on 500 (48%) men and 538 (52%) women (n = 1038). DD-PSF was the main outcome, and SRH and Nt-proBNP were the primary indicators. Diabetes mellitus, hypertension, and obesity were accounted for as major confounders of the association with SRH. DD-PSF was identified in 137 individuals, namely, 79 men (15.8%) and 58 women (10.8%). In a multivariate regression model, SRH (OR 2.95; 95% CI 1.02-8.57) and Nt-proBNP (quartile 4 vs. quartile 1 OR 4.23; 95% CI 1.74-10.26) were both independently associated with DD-PSF. Conclusions SRH, evaluated based on a descriptive question on general health, should be included in the diagnostic process of DD-PSF. In agreement with previous studies, our study confirms that Nt-proBNP is a major indicator of DD-PSF.

    • Olivine is used extensively in iron-pellet production as an additive in LKAB blast furnace pellets, in order to improve the high temperature properties of the finished product during reduction. As the contribution of olivine into the process depends on the available surface area, the present study was designed to find out the effect of olivine and its fineness on the oxidation-sintering and subsequent dissociation of olivine in iron ore agglomerates. Agglomerates were exposed to different experimental conditions to study the effect of olivine on the behavior of magnetite and hematite at high temperatures. Olivine particles were found to react significantly only above 1 000°C. Porosity of the final product was found to depend largely on olivine fineness. The finer the olivine the lower the porosity of the final product. It is found also that irrespective of the starting iron oxide the ratio between hematite and spinel phase was the same after heating in air. Olivine fineness affects significantly the rate of hematite dissociation, the finer the olivine the higher the dissociation rate. Upon cooling the weight lost due to the dissociation was again regained

    • Prostate tissues from patients with prostate cancer and benign prostatic hyperplasia (BPH) frequently contain histological inflammation, and a proportion of these patients show evidence of Propionibacterium acnes infection in the prostate gland. We developed a multicolor fluorescent in situ hybridization (FISH) assay targeting P. acnes 23S rRNA along with a 14-kb region of the P. acnes genome. This assay was used to analyze prostate tissues from patients with prostate cancer and BPH. P. acnes infection of the prostate gland was demonstrated in prostatic tissue in 5 of 10 randomly selected prostate cancer patients. FISH analysis and confocal laser microscopy imaging revealed intracellular localization and stromal biofilm-like aggregates as common forms of P. acnes infection in prostate tissues from both prostate cancer and BPH patients. A sequential analysis of prostate tissue from individual patients suggested that P. acnes can persist for up to 6 years in the prostate gland. These results indicate that P. acnes can establish a persistent infection in the prostate gland. Further study is needed to clarify the link between this bacterium and prostatic inflammation which may contribute to the development of BPH and prostate cancer.

  • Epidemiology of valvular heart disease in a Swedish nationwide hospital-based register study

    • Objective: Transitions in the spectrum of valvular heart diseases (VHDs) in developed countries over the 20th century have been reported from clinical case series, but large, contemporary population-based studies are lacking.Methods: We used nationwide registers to identify all patients with a first diagnosis of VHD at Swedish hospitals between 2003 and 2010. Age-stratified and sex-stratified incidence of each VHD and adjusted comorbidity profiles were assessed.Results: In the Swedish population (n=10 164 211), the incidence of VHD was 63.9 per 100 000 person-years, with aortic stenosis (AS; 47.2%), mitral regurgitation (MR; 24.2%) and aortic regurgitation (AR; 18.0%) contributing most of the VHD diagnoses. The majority of VHDs were diagnosed in the elderly (68.9% in subjects aged ≥65 years), but pulmonary valve disease incidence peaked in newborns. Incidences of AR, AS and MR were higher in men who were also more frequently diagnosed at an earlier age. Mitral stenosis (MS) incidence was higher in women. Rheumatic fever was rare. Half of AS cases had concomitant atherosclerotic vascular disease (48.4%), whereas concomitant heart failure and atrial fibrillation were common in mitral valve disease and tricuspid regurgitation. Other common comorbidities were thoracic aortic aneurysms in AR (10.3%), autoimmune disorders in MS (24.5%) and abdominal hernias or prolapse in MR (10.7%) and TR (10.3%).Conclusions: Clinically diagnosed VHD was primarily a disease of the elderly. Rheumatic fever was rare in Sweden, but specific VHDs showed a range of different comorbidity profiles . Pronounced sex-specific patterns were observed for AR and MS, for which the mechanisms remain incompletely understood.

    • BACKGROUND: Leisure activities and drinking patterns are factors that can affect health and ability to return to work after a sick-leave. Associations between participation in leisure activities and binge drinking among sick-listed individuals have been paid little attention in the research literature. Objective: The aim of this study was to examine associations between leisure activities and binge drinking in a sample of newly sick-listed women and men. PARTICIPANTS: The study included 2,888 individuals aged 19-64 years. Methods: Cross-sectional questionnaire data from the Health Assets Project, Sweden, was used. Participation in 18 leisure activities was estimated. Binge drinking was defined as consuming alcohol at least once a month, and typically consuming five or more glasses. RESULTS: Among women aged 19-30 years who regularly went to concerts (OR 2.36) and wrote (OR 2.39) associations were found with binge drinking. Lower OR was found among women aged 31-64 who regularly went to the cinema (OR 0.43), out in the nature (OR 0.46) or participated in sports (OR 0.57). Among men, associations were found between socializing with friends and binge drinking in both age groups (OR 3.83 respectively 1.63). Among younger men who attended sporting events OR was 2.31, and among older men participating in religious communities OR was 0.28. CONCLUSIONS: This study contributes to understanding the interplay between leisure activities and health behavior. In particular, social activities in men were associated with binge drinking while the opposite was true for recreational activities in older women.

  • Characterization of novel glomerular proteins : role in physiology and disease

    • Glomerular damage accounts for the majority of chronic kidney disease cases, the prevalence of which is dramatically escalating worldwide, mainly due to increase in diabetes and associated nephropathy. The costs for treatment of CKD represent an enormous burden to the health care system. Chronic kidney disease frequently progresses to end-stage renal disease with the only curative treatment options today are lifelong dialysis or kidney transplantation. In order to better understand and treat glomerular disease before it has reached this irreversible stage, it is imperative to understand the cell biology and physiology of the glomerulus including delineation of the molecular make-up of glomerular filtration barrier. This barrier constitutes the endothelial cells of the glomerular capillaries with its bound surface layer of a carbohydrate-rich meshwork (glycocalyx), the glomerular basement membrane, and the podocytes with the sub podocytes space and the attached slit diaphragm spanning between neighboring and interdigitating foot processes. Even though the concerted interplay of all layers of the glomerular filtration barrier is important for the ultra-filtration of the plasma, the podocytes and their injuries have recently been acknowledged as the major culprit of glomerular disease. In this thesis work, I describe two novel podocyte specific proteins, TDRD5 and RHPN1, which we show in vivo to be important for the integrity of the glomerular filtration barrier. TDRD5 has previously been considered a male germ cell line specific protein important for retro transposon silencing and spermiogenesis in mice. Here I show that it is expressed in mouse and human podocytes and is enriched in the zebrafish glomerulus. By using the in vivo morpholino knock down technique in zebrafish larvae it was demonstrated that TDRD5 expression is required for the proper formation of the zebrafish pronephros. RHPN1 is a RHOA binding protein which functions as an attenuator of the polymerization of actin stress fibers by regulating the phosphorylation of nonmuscle myosin II regulatory light chain through RHOA downstream effectors. Deletion of the Rhpn1 gene leads to neonatal albuminuria and glomerular basement membrane abnormality. The phenotype was characterized. As a part of efforts to elucidate expression signatures of glomerular diseases, the global glomeruli expression pattern of Adriamycin treated nephrotic mice was analyzed by RNA sequencing. The Adriamycin induced nephropathy mouse model mimics the human disease focal segmental glomerular sclerosis which constitutes one of the main causes of chronic kidney disease. The diabetic nephropathy associated single nucleotide polymorphism 3q locus was shown in our work to be a remote cis-acting variant differentially regulating glomerular NCK1 expression, implicating an important role for glomerular NCK1 in diabetic nephropathy pathogenesis.

    • Background: Attention deficit hyperactivity disorder (ADHD) is associated with an increased risk of poor mental health. However, the understanding of ADHD-related burden and impairments in women during the postpartum period is limited. The aim with the present study was to examine the risk of depression and anxiety disorders during the postpartum period among women with and without an ADHD diagnosis.Methods: We used register-based data to identify women who gave birth to their first and/or second child between 2005 and 2013 in Sweden (n = 773,047), of which 0.5 % (n = 3515) had a diagnosis of ADHD prior to pregnancy. Diagnoses of depression and anxiety disorders up to one year after delivery were collected from the national patient register.Results: A total of 16.76 % of the women with an ADHD diagnosis were also diagnosed with depression disorders in the postpartum period, prevalence ratio (PR) 5.09 (95 % confidence interval (CI), 4.68-5.54). A total of 24.92 % of the women with an ADHD diagnosis were also diagnosed with anxiety disorders in the postpartum period, PR 5.41 (5.06-5.78). Stratified results revealed that having a diagnosis of ADHD increased the risk for both depression and anxiety disorders postpartum, beyond other well-known risk factors.Limitations: There is a potential risk of surveillance bias as women diagnosed with ADHD are more likely to have repeated visits to psychiatric care and might have an enhanced likelihood of also being diagnosed with depression and anxiety disorders postpartum, compared to women without ADHD.Conclusions: ADHD is an important risk factor for both depression and anxiety disorders postpartum. Therefore, ADHD needs to be considered in the maternal care, regardless of sociodemographic factors and the presence of other psychiatric disorders.

    • What should design students learn, not to lose hope, but be able to design in the current dire climate, energy and health situation? What can we design and still stay on track with the Sustainable Development Goals (SDG) in 2030? What can we design in the long run, not worsening critical human and non-human systems? In the paper we question "good intentions" and business-as-usual approaches, to challenges facing many of our most existential systems. We suggest changes to the Design field's understanding of the SDG's. Further, we suggest focusing on several solutions at the same time, in local communities, with collaboration with others and design educations. The paper argues that a holistic and systemic view is required that rather focuses on root-problems, than the symptoms these problems cause. Based on a multidisciplinary selection of scientific literature, the paper shows how acclaimed systemic approaches often harm the health of both human and non-human systems. In addition, the paper argues that these acclaimed systemic approaches tend to suggest "solutions" that stand in the way of more realistic solutions emerging from supportive and social environments. One of the insights from the literature, is how individuals by reconnecting to community- and practice-based activities strengthen hope of better futures. In the paper we argue how designers and design educations, can act multidisciplinary, with others, to become agents towards the kind of holistically, community-based, and radical changes required to heal all broken systems. We suggest how designers can situate themselves in the responsible workshop of 2030.

    • BackgroundHeart failure is an established risk factor for poor outcomes in patients undergoing non-cardiac surgery, yet risk stratification remains a clinical challenge. We developed an index for 30-day mortality risk prediction in this particular group. Methods and resultsAll individuals with heart failure undergoing non-cardiac surgery between October 23 2004 and October 31 2011 were included from Danish administrative registers (n=16827). In total, 1787 (10.6%) died within 30days. In a simple risk score based on the variables from the revised cardiac risk index, plus age, gender, acute surgery, and body mass index category the following variables predicted mortality (points): male gender (1), age 56-65years (2), age 66-75years (4), age 76-85years (5), or age >85years (7), being underweight (4), normal weight (3), or overweight (1), undergoing acute surgery (5), undergoing high-risk procedures (intra-thoracic, intra-abdominal, or suprainguinal aortic) (3), having renal disease (1), cerebrovascular disease (1), and use of insulin (1). The c-statistic was 0.79 and calibration was good. Mortality risk ranged from <2% for a score <5 to >50% for a score 20. Internal validation by bootstrapping (1000 re-samples) provided c-statistic of 0.79. A more complex risk score based on stepwise logistic regression including 24 variables at P<0.05 performed only slightly better, c-statistic=0.81, but was limited in use by its complexity. ConclusionsFor patients with heart failure, this simple index can accurately identify those at low risk for perioperative mortality.

    • Cystic fibrosis (CF) is caused by a mutation in the cystic fibrosis transmembrane conductance regulator (CFTR), a chloride channel expressed in epithelial cells. The effects of genistein and 4-phenylbutyrate (PBA) on CFTR were studied in three human airway epithelial cell lines expressing wild-type or DeltaF508 CFTR: Calu-3, CFSMEo-, and CFBE41o- cells. The cells were loaded with the fluorescent dye N-(ethoxycarbonylmethyl)-6-methoxyquinolinium bromide (MQAE) and chloride efflux was studied. Forskolin and 3-isobutyl-1-methylxanthine (IBMX) induced chloride efflux in Calu-3 cells but not in CF lines. Genistein (2.5-50 microM) alone was able to induce chloride efflux in all cell lines. Genistein did not enhance the effect of forskolin and IBMX. PBA had little or no effect on genistein-induced chloride efflux. The effect of genistein seen at low concentrations makes genistein interesting for possible pharmacological treatment of CF, since it is known that similar concentrations can be obtained in plasma by a soy-rich diet.

    • Effects of radiation and biodistribution of radionuclides are often studied in animal models. Circadian rhythm affects many biological functions and may influence the biokinetics of radionuclides and observed responses. The aim of this study was to investigate if the time during the day of I-131 injection affects the biodistribution and absorbed dose to tissues in mice. Biodistribution studies were conducted on male C57BL/6 N mice for three diurnal time-series: the animals were i.v. injected with 160 kBq I-131 at 8 am, 12 pm or 4 pm. The activity concentration in organs and tissues was measured at 1 h to 7 days after administration and absorbed dose at day 7 was determined. Comparison between the three time-series showed statistically significant differences in activity concentration in all investigated tissues and organs. Administration performed at 12 pm resulted in general in higher absorbed dose to the organs than injection performed at 8 am and 4 pm. Time of day of administration affects the biodistribution of I-131 in mice and consequently the absorbed dose to individual organs. These findings advocate that subsequent biodistribution studies and dosimetry calculations should consider time-point of administration as a variable that could influence the results.

    • Background: Radionuclides are routinely used to diagnose and treat many different types of cancer. I-131 is a well-established radioisotope used in e.g. treatment of thyroid cancer and neuroblastoma. Accurate knowledge of I-131 biodistribution is essential to correctly estimate the absorbed dose to normal organs and determine potential risks from I-131 exposure, which is especially important when treating children. Many biological functions in living organisms follow a circadian rhythm. Nevertheless, little is known about diurnal variations in radionuclide biodistribution. This study investigates if circadian rhythm affects I-131 biodistribution in mice and absorbed dose to organs and tissues. Materials & Methods: The radioactivity concentration in mice tissues was studied at different time-points after administration of I-131, and absorbed doses were calculated. The effect of circadian rhythm was studied by varying the time of administration. Results: Difference in activity concentration between the administration time-points was observed at many time-points after administration for most investigated tissues. For some organs differences were also observed in the absorbed dose. The highest activity concentration and absorbed dose were found in the thyroid regardless of time of administration. Conclusion: The results demonstrate that the biodistribution of I-131 in mice is influenced by the time of day of administration. These findings advocate that circadian rhythm should be considered in biodistribution studies and suggests that time-point of administration of radiopharmaceuticals containing I-131 for therapy can be further optimized. An optimized time-point could result in higher absorbed dose to the tumor and/or lower absorbed dose to normal tissues.

    • The effects of long exposures to ethanol, water and 0.1 M aqueous solutions of ammonia, sodium hydroxide, tetrapropylammonium hydroxide (TPAOH) and hydrochloric acid on thin TPA-silicalite-1 membranes were studied. Single gas permeation experiments, porosimetry and scanning electron microscopy were used to characterize the membranes. It was found that a short exposure (24 h) will only dissolve synthesis residues and will not affect membrane quality negatively. The only medium that had an effect after 24 h was sodium hydroxide, which almost dissolved the film completely. After exposing TPA-silicalite-1 membranes for 30 days in the various liquids, the membrane quality decreased in the order ethanol < 0.1 M hydrochloric acid < 0.1 M TPAOH < water < 0.1 M ammonia < 0.1 M sodium hydroxide due to dissolution of the silicalite-1 crystals. This study has shown that prolonged exposure to aqueous solutions will lead to dissolution of silicalite-1 crystals causing an increase in micro- and mesopores in the film. The amount and size of the pores will depend on the pH of the aqueous medium. Higher pH gives a higher dissolution and hence more non-zeolitic pores in the silicalite-1 film. Ethanol has no effect on the dissolution of the zeolite film even after 30 days. This finding has an effect in membrane preparation and in several membrane applications such as pervaporation and separation of hydrocarbons isomer mixtures.

    • The ribosome and tRNA levels of Escherichia coli cells, transformed with a native or mutated Vitreoscilla hemoglobin genes (vhb), were investigated using asymmetrical flow field-flow fractionation (AFFFF). Mutagenesis of vhb by error-prone PCR was carried out to alter the growth behavior of microaerobically cultivated native VHb-expressing E. coli. A VHb mutant, pVMT1, was identified, which was able to reach a remarkably high final A600 of 15, the value of which being 160% higher than that of a VHb control carrying pVHb8 (A600 5.8). AFFFF revealed that cells expressing mutant vhbs showed up to a doubling in the number of active 70S ribosomes cell-1, an almost 3-fold increase in the number of tRNAs cell-1, and up to a 26% increase in the mass fraction of active 70S ribosomes.

  • Factors affecting MFI membrane quality

    • Zeolites are crystalline aluminosilicates with molecular sieving properties and are widely used in industrial applications such as catalysis, sorption and ion-exchange. Zeolite membranes are highly interesting due to their capability to continuously separate molecules under severe conditions. The MFI zeolite structure has suitable pore diameter for many applications, and its thermal and chemical stability is high. In order to obtain high performance MFI membranes, a thin and defect free film is needed for high flux and high selectivity. In this thesis, parameters affecting quality of zeolite membranes have been investigated. Different MFI-zeolite membranes were prepared using seed crystals and hydrothermal synthesis. Thereafter, membranes were characterized using scanning electron microscopy, single gas permeation measurements, porosimetry and separation experiments. The effect of grain boundaries was investigated by preparing membranes with small crystal size. It was found that a high amount of grain boundaries reduced the quality of the membranes. In another study, the effect of exposure to aqueous solutions was found to increase the amount of non-zeolitic pores in the membrane, and thus it decreased the quality of the membrane. However, no such effect was found for ethanol that was identified as a safe rinsing media. Membranes with different Si/Al ratio were also prepared and evaluated. It was observed that the Si/Al ratio of a MFI membrane influences the performance of the membrane. It was found that an increase of aluminium makes the membrane more polar which leads to an improved adsorption of, and selectivity for polar molecules. The effect of support invasion was also evaluated and the regularity and extension of the support invasion was observed to affect membrane quality. The effect of heating rate during calcination was investigated and no correlation between heating rate and membrane quality was observed. Calcination of membranes was studied in-situ by high temperature synchrotron radiation and a model for crack formation was postulated. In summary, this work has shown that in order to obtain high quality membranes, the amount of grain boundaries, the film thickness and support invasion should be controlled. To obtain the best performance of the membranes the Si/Al ratio should be chosen with respect to the application. Finally, water exposure affects the lifetime of the MFI membranes in certain applications.

    • Expression of the gene encoding bacterial hemoglobin (VHb) from Vitreoscilla has been previously used to improve recombinant cell growth and enhance product formation under microaerobic conditions. It is very likely that the properties of VHb are not optimized for foreign hosts; therefore, we used error-prone PCR to generate a number of randomly mutated vhb genes to be expressed and studied in Escherichia coli. In addition, the mutated VHb proteins also contained an extension of eight residues (MTMITPSF) at the amino terminus. VHb mutants were screened for improved growth properties under microaerobic conditions and 15 clones expressing mutated hemoglobin protein were selected for further characterization and cultivated in a microaerobic bioreactor to analyze the physiological effects of novel VHb proteins on cell growth. The expression of four VHb mutants, carried by pVM20, pVM50, pVM104, and pVM134, were able to enhance microaerobic growth of E. coli by approximately 22%, 155%, 50%, and 90%, respectively, with a concomitant decrease of acetate excretion into the culture medium. The vhb gene in pVM20 contains two mutations substituting residues Glu19(A17) and Glu137(H23) to Gly. pVM50 expresses a VHb protein carrying two mutations: His36(C1) to Arg36 and Gln66(E20) to Arg66. pVM104 and pVM134 express VHb proteins carrying the mutations Ala56(E10) to Gly and Ile24(B5) to Thr, respectively. Our experiments also indicate that the positive effects elicited by mutant VHb-expression from pVM20 and pVM50 are linked to the peptide tail. Removal of the N-terminal sequence reduced cell growth approximately 23% and 53%, respectively, relative to wild-type controls. These results clearly demonstrate that it is possible to obtain mutated VHb proteins with improved characteristics for improving microaerobic growth of E. coli by using combined mutation techniques, addition of a peptide tail, and random error-prone PCR.

  • Noncardiac Surgery in Patients With Aortic Stenosis: A Contemporary Study on Outcomes in a Matched Sample From the Danish Health Care System

    • BackgroundPast research has identified aortic stenosis (AS) as a major risk factor for adverse outcomes in noncardiac surgery; however, more contemporary studies have questioned the grave prognosis. To further our understanding of this, the risks of a 30-day major adverse cardiovascular event (MACE) and all-cause mortality were investigated in a contemporary Danish cohort. HypothesisAS is not an independent risk factor for adverse outcomes in noncardiac surgery. MethodsAll patients with and without diagnosed AS who underwent noncardiac surgery in 2005 to 2011 were identified through nationwide administrative registers. AS patients (n=2823; mean age, 75.5years, 53% female) were matched with patients without AS (n=2823) on propensity score for AS and surgery type. ResultsIn elective surgery, MACE (ie, nonfatal myocardial infarction, ischemic stroke, or cardiovascular death) occurred in 66/1772 (3.7%) of patients with AS and 52/1772 (2.9%) of controls (P=0.19), whereas mortality occurred in 67/1772 (3.8%) AS patients and 51/1772 (2.9%) controls (P=0.13). In emergency surgery, 163/1051 (15.5%) AS patients and 120/1051 (11.4%) controls had a MACE (P=0.006), whereas 225/1051 (21.4%) vs 179/1051 (17.0%) AS patients and controls died, respectively (P=0.01). Event rates were higher for those with symptoms (defined as use of nitrates, congestive heart failure, or use of loop diuretics), compared with those without symptoms (P<0.0001). ConclusionsAS is associated with high perioperative rates of MACE and mortality, but perhaps prognosis is, in practice, not much worse for patients with AS than for matched controls. Symptomatic patients and patients undergoing emergency surgery are at considerable risks of a MACE and mortality.

  • Optimisation of radionuclide therapy by reduction of normal tissue damage

    • Background:177Lu-octreotate is used to treat patients with neuroendocrine tumours (NETs) often resulting in prolonged life and better quality of life, but is today seldom a cure for these patients. Optimisation of 177Lu-octreotate therapy can be achieved by reducing the risk of side effects on the main dose limiting organs, kidneys and bone marrow, enabling higher administered activity. One proposed option is co-administration with the antioxidant α1-microglobulin (A1M). Aims: 1) to investigate if co-administration with A1M results in a negative (protective) effect on the tumour response to 177Lu-octreotate 2) to study the normal tissue response in mice following 177Lu-octreotate administration with or without A1M and A1M alone, 3) and to propose biomarkers for 177Lu-octreotate induced kidney damage. Methods: Biodistribution of 177Lu was investigated in mice bearing human GOT2 NET after injection of 177Lu-octreotate with or without A1M. Tumour volume and regulation of apoptosis related genes were studied on human GOT1 NET in mice after injection of 177Lu-octreotate with or without A1M and A1M alone. Effects on normal tissues were studied in mice injected with 177Lu-octreotate with or without A1M and A1M alone. Early proteomic responses were investigated in kidney tissues and bone marrow. Regulation of apoptosis related genes was investigated in kidney tissues. Late effects on kidneys were studied based on expression of proposed markers for kidney damage. Results and conclusions: No negative impact of A1M were observed on the therapeutic effects of 177Lu-octreotate in NET. A tissue-dependent early proteomic response was observed in kidney tissue, including regulation of previously observed radiation responsive proteins. No clear changes in regulation of these radiation-induced proteins was observed after co-administration of A1M. Regulation of pro- and anti-apoptotic genes was observed in kidney cortex and kidney medulla following 177Lu-octreotate exposure. Indication of an A1M initiated pro-survival response was observed in kidney medulla when 177Lu-octreotate was combined with A1M. Promising results were found for KIM-1, CDKN1A and S100A6 as biomarkers for 177Lu-octreotate induced late kidney injury, and RBP4 as an early responding urinary biomarker. No clear protective effect of A1M on late radiation induced effects on kidneys were observed.

Zeolite films in membranes must be thin, to obtain a high flux and free from defects for high selectivity. Many parameters are affecting the properties of zeolite films and in this thesis some of them are investigated. MFI zeolite membranes were prepared using seed crystals and hydrothermal synthesis and characterized with scanning electron microscopy, single gas permeation measurements, porosimetry and separation experiments. Membranes grown in one or several steps with seeding in-between the synthesis steps were compared. It was observed that membranes comprised of small crystals and consequently high concentration of grain boundaries had low separation performance. Grain boundaries could also be opened by extensive rinsing, which reduced membrane quality. The influence of the calcination rate on the performance of zeolite membranes of a particular type was also studied. It was demonstrated that the calcination rate does not affect the membrane quality.

    • Treatment of neuroendocrine tumors with Lu-177-octreotate results in prolonged survival and improved quality of life for the patient. However, the treatment is today limited by side effects on kidney and bone marrow, and complete tumor remission is rarely seen. A possible way to minimize dose-limiting toxicity and to optimize this treatment method is to use radioprotectors in conjunction with radiotherapy. A recombinant form of alpha(1)-microglobulin ( rA1M) was recently shown to preserve kidney structure and function after Lu-177-octreotate injection in mice and was suggested as a radioprotector in peptide receptor radionuclide therapy. The aims of this work were to investigate the influence of rA1M on the in vivo biokinetics of Lu-177-octreotate, with a focus on tumor tissue, and to study the impact of rA1M on the therapeutic response in tumor tissue subjected to Lu-177-octreotate treatment. Methods: The biodistribution of Lu-177-octreotate was examined in BALB/c nude mice with GOT2 tumors 1-168 h after injection with either Lu-177-octreotate or coadministration of 177Lu-octreotate and rA1M. The effects of rA1M on the tumor response after Lu-177-octreotate treatment were studied in BALB/c nude mice with GOT1 tumors. Three groups of mice were administered rA1M, Lu-177-octreotate, or both. Another group served as untreated controls. Tumor volume was measured to follow the treatment effects. Results: No statistically significant difference in biodistribution of Lu-177 was observed between the groups receiving Lu-177-octreotate or coinjection of Lu-177-octreotate and rA1M. The therapy study showed a decrease in mean tumor volume during the first 2 wk for both the Lu-177-octreotate group and the coadministration group, followed by tumor regrowth. No statistically significant difference between the groups was found. Conclusion: rA1M did not negatively impact absorbed dose to tumor or therapeutic response in combination with Lu-177-octreotate and may be a promising kidney protector during Lu-177-octreotate treatment of patients with neuroendocrine tumors.

The film were characterized using SEM, single gas permeation, porosimetry and mixture separation experiment Films with three or four layers were of high quality i.e with minor defects according to the porosimetry experiments but showed poor separation of binary mixtures. This result may be attributed to the small crystal size and/or large amount of grain boundaries in the films.

    • Background: I-131 is well-established in nuclear medicine. The thyroid is a target organ when treating thyroid cancer with unbound I-131, but also a risk organ in I-131-based radionuclide therapy. Exposure to I-131 can also occur from the environment at nuclear accidents. Accurate knowledge of I-131 biodistribution is essential to correctly estimate the absorbed dose to organs and determine potential risks from both medical and hazard exposure. Many biological functions follow a circadian rhythm. Nevertheless, circadian rhythm remains an unknown factor in radionuclide biodistribution. Aim: The purpose of this study was to investigate if circadian rhythm affects I-131 biodistribution in mice and hence absorbed dose to mouse tissues. Methods: The radioactivity concentration in various tissues was studied at different time points after administration of I-131 and absorbed doses were calculated according to the MIRD formalism. The effect of circadian rhythm was studied by varying the time of administration. Male C57BL/6N mice were i.v. injected with I-131 at 8 am, 12 pm or 4 pm and killed after 1h to 7d. Results: Statistically significant difference in activity concentration and absorbed dose between the three injection series was observed for at least one time point after injection for many tissues. Highest activity concentration and absorbed dose were found in the thyroid. Conclusion: The results demonstrated that the biodistribution of I-131 in mice is influenced by the time of day of administration to a certain extent. These findings advocate that circadian rhythm should be considered in biodistribution studies and dose calculations.

    • The increased need for efficient material processing and efficient utilisation of more complex raw materials and the need for recycling or reusing byproduct and waste streams, are all increased challenges in material processing. To cope with these challenges, there is a need for new basic physical and thermodynamic data. The present paper gives four examples, as well as preliminary data, of areas where increased knowledge of fundamental parameters will increase the possibility for a sustainable extraction of metals. The examples include measurement of solubility of pure individual slag minerals, determination of distribution of leachable elements between different mineralogical phases in slag, influence of alumina on liquidus temperature of a copper slag and thermal diffusivity measurements in magnetite based iron ore pellets, all important in different ways to increase the sustainability of the respective materials involved

    • S-nitrosogluthatione is an endogenous substance, present at decreased levels in the lungs of CF patients and was recently found to induce mature CFTR in airway epithelial CF cell lines. We show that S-nitrosoglutathione in physiological concentrations increases the presence of ΔF508 CFTR in the cell membrane and induces cAMP dependent chloride transport in cystic fibrosis airway epithelial cells. The properties of S-nitrosoglutathione include other potential benefits for the CF patient and make this agent an interesting candidate for pharmacological treatment of CF that needs to be further evaluated.Genistein was found to increase the chloride efflux in both normal and ΔF508 cells without stimulation of cAMP elevating agents and without prior treatment with phenylbutyrate. Genistein, in concentrations close to those that can be detected in plasma after a high soy diet, could induce chloride efflux in cells with the ΔF508 CFTR mutation and its possible use in the treatment of CF should therefore be further investigated.Studies on nasal epithelial cells from CF patients showed cAMP dependent chloride efflux in some of the patients with severe genotypes. This may complicate in vitro evaluation of clinical treatment of these patients. The presence of cAMP dependent chloride transport did not necessarily lead to a milder phenotype. Other factors than CFTR may influence the clinical development of the disease.Cystic fibrosis (CF) is the most common monogenetic disease among Caucasians. A defective cAMP regulated chloride channel (cystic fibrosis transmembrane conductance regulator, CFTR) in epithelial cells leads to viscous mucus, bacterial infections, inflammation and tissue damage in the lungs that cause death in 95% of the cystic fibrosis patients. There is no cure for the disease although existing treatment has dramatically prolonged the life expectancy. The aim of this thesis was to study pharmacological agents for their ability to restore the cellular deficiency in CF airway epithelial cells. X-ray microanalysis, MQAE fluorescence and immunocytochemistry were used to evaluate the effects.
